Abstract A kitagohaite grain, measuring approximately 1.0 mm, was retrieved from an alluvial gold gravity concentrate from the southeastern Quadrilátero Ferrífero (QFe), Mariana, Brazil. The grain exhibits a cubic structure, whitish-steel-gray, and has a metallic luster. It is composed of Wt. 94.42% Pt and 5.56% Cu (n=9), yielding the empirical formula Pt6.78Cu1.22. This groundbreaking discovery marks the first documented occurrence of kitagohaite in South America. This hydrothermal deposition of kitagohaite indicates that its mineralization is constrained to temperatures <500º C, as inferred by the stability of synthetic Pt7Cu.

Abstract The study addresses the historical disparities in the distribution of the medium- and high-complexity health network and the limits to budget adjustments between the municipality of Rio de Janeiro and its neighboring municipalities of the Metropolitan region 1. An ecological study was conducted with data related to the municipality of Rio de Janeiro, chosen because it has a large assistance network, while located on the borders of vulnerable and underprivileged areas, characterizing a locus that is representative of the situations faced throughout the country. A decrease in the gross values of the programmed quotas in all municipalities of Rio de Janeiro was observed from 2016 onwards. The temporal trend of the programmed quotas remained stable for all municipalities in the Metropolitan Region 1, even with significant increases in the accomplished quotas for neighboring municipalities. The resulting overload in local expenditure prevents the increase of capacity to anticipate fluctuations in demand, both known and unexpected ones, compromising the responsiveness of the health system regarding its regular operation, as well as the ability to adjust to cope with extraordinary events, essential characteristics of resilience.

Resumo O estudo explora as históricas disparidades regionais na distribuição da rede de média e alta complexidade e os limites impostos para a o remanejamento dos tetos de financiamento entre o município do Rio de Janeiro e municípios limítrofes da Região Metropolitana 1. Foi realizado um estudo ecológico com dados referentes à cidade do Rio de Janeiro, escolhido por ter uma grande rede de assistência e limites com territórios vulneráveis e carentes de serviços de saúde, caracterizando um lócus representativo das situações enfrentadas em todo o país. Foi observado um decréscimo dos valores brutos das cotas programadas em todos os municípios do Rio de Janeiro a partir de 2016. A tendência temporal das cotas programadas se manteve estacionária para todos os municípios da Região Metropolitana 1, mesmo com aumentos significativos nas cotas para municípios limítrofes. Abstract Anthropogenic interventions have had a compromising effect on environmental health, intensifying the degradation of ecosystems, and the quantity of chemical pollutants released into nature. Therefore, research areas within the scope of environmental assessments and monitoring such as ecotoxicology have contributed to the determination of the toxic potential of contaminants. A small cyprinid known as the zebrafish (Danio rerio), the use of which has exponentially grown, is an alternative vertebrate model for scientific research, mainly in the assessment of environmental risks. The species exhibits several advantages for breeding in a laboratory, in addition to presenting multi-biomarkers of environmental toxicity. Thus, this review aims to present the main characteristics and advantages of working with this species, as well as show studies related to ecotoxicology involving biomarkers of toxicity in zebrafish. The results show a progressive trend towards employing the species in environmental risk analyses, it is an increasingly recommended species in the assessment of the toxicity level of a range of chemical pollutants. The development of future technologies must contribute to scientific advancement, rendering the potential application of this model organism an even more widespread one, which will certainly help in bridging knowledge gaps in various areas of study.

Resumen: Durante el invierno de 2023, Chile enfrentó una compleja situación relacionada con al virus respiratorio sincicial (VRS). Después de experimentar una disminución en la circulación del VRS durante los años de la pandemia de SARS-CoV-2, se observó un brote tardío en la primavera de 2022 y un inicio anticipado del brote en 2023, con un aumento significativo en el número de casos graves. La poca efectividad en la planificación estratégica y comunicación de riesgo contribuyeron a la complejidad de la situación. Para evitar lo anterior el próximo invierno, se sugieren medidas como vigilancia activa, unificación de definiciones para infecciones respiratorias agudas, identificación de variantes del VRS, educación pública sobre contagios y preparación anticipada respecto a camas hospitalarias y personal de salud. Se destaca la importancia de la inmunización y colaboración intersectorial para adquirir nuevas alternativas preventivas como también la necesidad de una comunicación temprana sobre la importancia de la inmunización e identificación de grupos de alto riesgo, mejora en capacitaciones del personal médico y planificación estratégica del Ministerio de Salud buscando un enfoque proactivo y colaborativo para abordar la compleja situación del VRS en futuros inviernos. El Comité Asesor en Vacunas y Estrategias de Inmunización de Chile ya realizó un análisis y recomendación sobre una nueva alternativa de prevención. Este grupo de trabajo apoyará cualquier decisión del Ministerio de Salud en políticas públicas que intenten un cambio en el paradigma del control de esta enfermedad por la salud de los niños/as de nuestro país.

Abstract: During the winter of 2023, Chile faced a complex situation related to the respiratory syncytial virus (RSV). After experiencing a decline in RSV circulation during the years of the SARS-CoV-2 pandemic, a late outbreak was observed in the spring of 2022 and an early onset of the outbreak in 2023, with a significant increase in the number of serious cases. The ineffectiveness of strategic planning and risk communication contributed to the complexity of the situation. To avoid the above next winter, measures such as active surveillance, unification of definitions for acute respiratory infections, identification of RSV variants, public education about infections and advance preparation regarding hospital beds and health personnel are suggested. The importance of immunization and intersectoral collaboration to acquire new preventive alternatives is highlighted, as well as the need for early communication about the importance of immunization and identification of high-risk groups, improvement in training of medical personnel and strategic planning of the Ministry of Health. seeking a proactive and collaborative approach to address the complex RSV situation in future winters. The Chilean Immunization Advisory Committee has already carried out an analysis and recommendation on a new prevention alternative. This working group will support any decision of the Ministry of Health in public policies that attempt a change in the paradigm of control of this disease for the health of the children of our country.

ABSTRACT Objective: To analyze the cognitive development of preterm infants at six and 12 months of corrected age and the associations with perinatal and socioeconomic factors. Methods: Cognitive development of 40 infants (20 preterm and 20 full-term) at six and 12 months of age was evaluated using the Bayley-III scale. Correlations between cognitive outcome and associated factors were assessed using Spearman correlation. Stepwise multiple linear regression analysis with covariance was applied to identify changes on cognitive score between six and 12 months. Results: Bayley-III cognitive score in preterm group was significantly lower than in full-term group at both six and 12 months of age. Birth weight correlated with cognitive performance at six months and head circumference at birth at 12 months, in full-terms infants. The occurrence of necrotizing enterocolitis was inversely associated with cognitive score in preterms at 12 months. An increase in cognitive score was observed between six and 12 months in both groups, but the gain was more pronounced in preterms. Conclusions: These findings suggest some cognitive recovery capacity in the first year despite the restrictions imposed by premature birth and emphasize the importance of early interventions in this population.

RESUMO Objetivo: Avaliar o desenvolvimento cognitivo de crianças pré-termo aos seis e 12 meses de idade corrigida e as associações com fatores perinatais e socioeconômicos. Métodos: O desenvolvimento cognitivo de 40 crianças (20 pré-termo e 20 a termo) foi avaliado aos seis e 12 meses de idade, utilizando a escala Bayley-III. Correlações entre resultados cognitivos e fatores associados foram avaliadas pelo teste de correlação de Spearman. A análise de regressão linear múltipla stepwise com covariância foi aplicada para identificar mudanças na pontuação cognitiva entre seis e 12 meses. Resultados: O escore cognitivo no grupo pré-termo foi significativamente menor que no grupo a termo aos seis e 12 meses. O peso ao nascer foi diretamente associado com o desempenho cognitivo aos seis meses e perímetro cefálico ao nascimento aos 12 meses, nas crianças a termo. A ocorrência de enterocolite necrosante foi inversamente associada ao desempenho cognitivo em pré-termos, aos 12 meses. Verificou-se aumento na pontuação cognitiva entre seis e 12 meses nos dois grupos, porém mais pronunciado no pré-termo. Conclusões: O estudo sugere que crianças pré-termo apresentam alguma capacidade de recuperação cognitiva no primeiro ano, apesar das restrições impostas pelo nascimento prematuro, e enfatizam a importância de acompanhamento dessa população desde os primeiros meses de vida.

ABSTRACT: Skin grafting is a simple and very useful surgical technique for wound repair, especially where there is difficulty in performing direct synthesis or other reconstructive techniques. low-intensity laser (LIL) has already been used successfully in cases where the graft was applied to wounds shortly after its creation. However, LIL still lacks standardization due to conflicting studies on its results. Thus, this study compared the effectiveness of continuous wave LIL with pulsed wave LIL in stimulating the healing of skin grafts, applied to newly created wounds in rabbits. For this purpose, 31 rabbits were distributed into: control group (CG, n = 10), continuous wave laser group (GLC; n = 10) and pulsed wave laser group (GLP; n = 11). Postoperative management was similar between all groups, with the exception of laser application, as indicated by the group. The GLC and GLP groups showed similar evolution, with a satisfactory healing process of the graft, unlike the GC group. These results were maintained in the microscopic evaluation, as the groups treated with laser showed less inflammation, better integration with the receptor area, greater re-epithelialization and collagenization. There was no difference in vascular density between the treatment groups on immunohistochemical analysis. The newly created wound was not able to provide nutrition for the skin graft in rabbits, but LIL is an efficient adjuvant therapy in stimulating healing between the recipient area and the graft, promoting full integration. However; although, statistically there was no difference in the form of light emission, pulsed or continuous, patients who received continuous wave LIL showed superior microscopic evolution.

RESUMO: A enxertia cutânea é uma técnica cirúrgica simples e bastante útil para o reparo de feridas, principalmente, quando há dificuldade em realizar a síntese direta ou outras técnicas reconstrutivas. O laser de baixa intensidade (LBI) já foi utilizado com sucesso em casos em que o enxerto foi aplicado em feridas logo após a sua criação. No entanto, a LBI ainda carece de padronizações, mediante estudos conflitantes sobre seus resultados. Desta forma, este trabalho objetivou comparar a eficácia da LBI de onda contínua com onda pulsada na estimulação da cicatrização de enxertos cutâneos, aplicados em feridas recém-criadas em coelhos. Para tanto, foram utilizados 31 coelhos distribuídos em: grupo controle (GC, n = 10), grupo laser onda contínua (GLC; n = 10) e grupo laser onda pulsada (GLP; n = 11). O manejo pós operatório foi semelhante entre todos os grupos, com exceção da aplicação do laser, conforme o grupo indicado. Os grupos GLC e GLP apresentaram evolução semelhante, com processo cicatricial satisfatório do enxerto, ao contrário do grupo GC. Tais resultados se mantiveram na avaliação microscópica, pois os grupos tratados com laser apresentaram menor inflamação, melhor integração à área receptora, maior reepitelização e colagenização. Não houve diferença na densidade vascular entre os grupos de tratamento na análise imuno-histoquímica. A ferida recém-criada não é capaz de fornecer nutrição para o enxerto cutâneo nos coelhos, mas a LBI é uma terapia adjuvante eficiente em estimular a cicatrização entre a área receptora e o enxerto, promovendo integração total. Entretanto, embora não houve diferença estatística na forma de emissão de luz, pulsada ou contínua, os pacientes que receberam LBI em onda contínua apresentam evolução microscópica superior.

ABSTRACT Objective: This study aimed to evaluate whether the therapeutic use of caffeine for premature newborns is associated with changes in sleep habits and the presence of obstructive sleep apnea in childhood. Methods: This is a cross-sectional single-center study in which the caretakers of 87 children aged 5–10 years, born full-term or preterm, treated or not with caffeine in the neonatal period, answered questionnaires to screen for obstructive sleep apnea (Pediatric Obstructive Sleep Apnea Screening Tool [PosaST]) and to characterize the sleep habits (Children's Sleep Habits Questionnaire [CSHQ]) of their children. ANOVA and linear regression tests were performed to verify possible differences between the groups. Results: Children born prematurely who were treated with caffeine woke up significantly later on weekdays than those born at term (09h±00h58 and 07h43±1h15, respectively, p=0.022) and had longer total daily sleep time also compared to those born at term (10h24±1h08 and 09h29±1h08, respectively, p<0.001). There was no significant difference between the three groups in overall PosaST and CSHQ scores. Conclusions: Caffeine use in the neonatal period did not impair sleep habits later in life and did not lead to increased obstructive sleep apnea scores in prematurely born children compared to those born at term.

RESUMO Objetivo: Avaliar se o uso terapêutico de cafeína para recém-nascidos prematuros se associa a alterações nos hábitos de sono e à presença de apneia obstrutiva do sono na infância. Métodos: Este é um estudo unicêntrico transversal no qual os responsáveis por 87 crianças com idades entre cinco e dez anos, nascidas a termo ou pré-termo e tratadas ou não com cafeína no período neonatal responderam a questionários para triagem de apneia obstrutiva do sono (Pediatric Obstructive Sleep Apnea Screening Tool – PosaST) e para a caracterização dos hábitos de sono (Children's Sleep Habits Questionnaire – CSHQ) de seus filhos. Foram realizados testes de variância (ANOVA) e de regressão linear para verificar possíveis diferenças entre os grupos. Resultados: As crianças nascidas prematuras que foram tratadas com cafeína acordaram significativamente mais tarde nos dias de semana do que as nascidas a termo (09h±00h58 e 07h43±1h15, respectivamente, p=0,022) e tiveram maior tempo total de sono diário também comparadas às nascidas a termo (10h24±1h08 e 09h29±1h08, respectivamente, p<0,001). Não houve diferença significativa entre os três grupos na pontuação geral dos questionários PosaST e CSHQ. Conclusões: O uso de cafeína no período neonatal não prejudicou tardiamente os hábitos de sono e não levou ao aumento dos escores de apneia obstrutiva do sono de crianças nascidas prematuras quando comparadas com crianças nascidas a termo.

ABSTRACT This research aimed to prospect the working conditions of the rescue teams of the Mobile Emergency Care Service (SAMU-192) at the fluvial areas of the Alto Solimoes region, trough the systemic analysis of the activities of the water ambulances service popularly called ‘ambulanchas’ during the peaks of COVID-19 in the state of Amazonas, when the health system functioned under stress caused by the pandemic. Data were obtained from an exploratory cross-sectional design, based on qualitative data collected through interviews and observation of the normal functioning of the system before the pandemic. From there, models of two scenarios were developed showing the functioning of the water ambulances service when coping with the COVID-19 pandemic and the impact on the working conditions of the interprofessional rescue teams. Remote interviews with workers after the pandemic indicated that the prospection of working conditions from the instances of the models corresponded to the real functioning of the system during the COVID-19 pandemic.

RESUMO Esta pesquisa teve como objetivo prospectar as condições de trabalho das equipes de socorro do Serviço de Atendimento Móvel de Urgência (Samu-192) fluvial das áreas ribeirinhas e costeiras da região do Alto-Solimões, a partir da análise sistêmica das atividades no serviço de embarcações popularmente chamadas de ‘ambulanchas’ durante os picos da Covid-19 no estado do Amazonas, quando o sistema de saúde funcionou sob o estresse provocado pela pandemia. Os dados foram obtidos a partir de um desenho transversal exploratório, baseado em dados qualitativos coletados por meio de entrevistas e observação do funcionamento normal do sistema antes da pandemia. A partir daí modelos de dois cenários foram elaborados mostrando o funcionamento do serviço de ambulanchas ao lidar com a pandemia de Covid-19 e o impacto nas condições de trabalho das equipes de socorro interprofissionais de socorro. Entrevistas remotas com trabalhadores das ambulanchas após a pandemia indicaram que a prospecção das condições de trabalho a partir das instâncias dos modelos corresponderam ao funcionamento real do sistema durante a pandemia de Covid-19.

Resumo Introdução: O Cuidado Paliativo domiciliar, aliado ao trabalho multiprofissional e à presença de um cuidador familiar, possibilita a produção de autonomia e de alternativas criativas no cuidado. Objetivo: Compreender as condições relacionadas ao tornar-se cuidador familiar principal de um membro da família que recebe cuidados paliativos por serviço de Atenção Domiciliar. Metodologia: Pesquisa qualitativa, explicativa, utilizando a perspectiva metodológica de Strauss e Corbin da Teoria Fundamentada nos Dados. Realizamos entrevistas em profundidade com 18 participantes, sendo nove cuidadores familiares e nove profissionais de enfermagem de um Serviço de Atenção Domiciliar e as analisamos pelas codificações aberta, axial e seletiva. Posteriormente, apresentamos as categorias construídas em uma roda de conversa com 15 cuidadores familiares e nove profissionais para a validação teórica. Resultados: O processo de tornar-se cuidador familiar relaciona-se a três subcategorias analíticas: ''Assumindo o cuidado domiciliar do familiar dependente''; ''(Des) conhecendo a condição de saúde do familiar e o significado de cuidados paliativos''e ''Vivenciando os cuidados paliativos e lidando com a possibilidade de morte''. Destaca-se que os familiares tornaram-se cuidadores pela imperativa necessidade do cuidado de um parente. A compreensão sobre o significado de cuidado paliativo foi se instituindo pela vivência deste familiar e pelo lidar diário com a condição e possibilidades de morte, mediada pela equipe de Atenção Domiciliar, destacando-se os profissionais de enfermagem. Os profissionais atuantes no serviço mostraram dificuldades para abordarem o tema da morte e de cuidado paliativo com paciente e cuidador. Conclusão: Tornar-se cuidador familiar mostra-se um processo complexo, permeado por diversas interações entre pacientes, cuidadores familiares e profissionais da saúde. Identificou-se que as dificuldades nesse processo podem ser amenizadas por meio de estratégias adotadas pelos profissionais para ampliar a comunicação e o apoio frente às singularidades das famílias e, também, através da implementação de políticas públicas que favoreçam o cuidador.

Resumen Introducción: El cuidado paliativo en el hogar, aliado al trabajo multiprofesional y a la presencia de una persona familiar cuidadora, posibilita la obtención de autonomía y de alternativas creativas en el cuidado. Objetivo: Comprender las condiciones relacionadas al convertirse en la persona cuidadora principal de un miembro de la familia que recibe cuidados paliativos en el hogar. Método: Investigación cualitativa, utilizando la perspectiva metodológica de Strauss y Corbin de la teoría fundamentada en los datos. Se realizaron entrevistas exhaustivas a 18 participantes, 9 familiares cuidadores y 9 profesionales de enfermería de un Servicio de Atención en el Hogar. Estas entrevistas fueron analizadas por codificación abierta, axial y selectiva. Posteriormente, se presentan las categorías identificadas en una rueda de conversación con 15 familiares cuidadores y 9 profesionales para su validación teórica. Resultados: El proceso de convertirse en una persona cuidadora familiar está relacionado a tres subcategorías analíticas: ''Comprometiéndose con la asistencia en el hogar de la persona familiar dependiente.''; ''(Des) conociendo la condición de salud de la persona familiar y el significado de los cuidados paliativos''y ''Vivenciando los cuidados paliativos y lidiando con la posibilidad de muerte''. Se destaca que familiares se convierten en cuidadores por la imperativa necesidad de cuidados de un pariente. La comprensión del significado de los cuidados paliativos fue instituida por la experiencia de esta persona integrante de la familia y por el tratamiento diario de la condición y posibilidades de muerte, mediada por el equipo de asistencia en el hogar, especialmente las personas profesionales de enfermería. Las personas profesionales de enfermería que actúan en el servicio mostraron dificultades para abordar el tema de la muerte y los cuidados paliativos con pacientes y cuidador. Conclusión: Convertirse en persona cuidadora familiar es un proceso complejo, permeado por diferentes interacciones entre pacientes, cuidadores familiares y profesionales de la salud. Se identificó que las dificultades en ese proceso pueden ser paliadas a través de estrategias adoptadas por las personas profesionales para ampliar la comunicación y el apoyo frente a las singularidades de las familias y, también, a través de la implementación de políticas públicas que favorezcan a quien realiza el cuidado.

Abstract Introduction: Palliative care at home, combined with multiprofessional work and the presence of a family caregiver, enables the achievement of autonomy and creative alternatives in caregiving. Objective: To understand the conditions related to becoming the main caregiver of a family member receiving palliative care at home. Method: Qualitative research, using Strauss and Corbin's methodological perspective of Grounded Data Theory. In-depth interviews were conducted, with 18 participants: nine family caregivers and nine nursing professionals from a Home Care Service, and were analyzed them by open, axial and selective coding. Subsequently, the categories identified in a conversation with fifteen family caregivers and nine professionals are presented for theoretical validation. Results: The process of becoming a family caregiver is related to three analytical subcategories: "Engaging in the home care of the dependent family member"; " (Un) knowing the health condition of the family member and the meaning of palliative care," and "Experiencing palliative care and coping with the possibility of death". It is emphasized that family members became caregivers due to the imperative need for care of a relative. The understanding of the meaning of palliative care was instituted by the experience of this family member, by the daily treatment of the condition, and the possibilities of death, mediated by the Home Care team, especially the nursing professionals. The professionals working in the service showed difficulties in addressing the theme of death and palliative care with patients and caregivers. Conclusion: Becoming a family caregiver is a complex process, permeated by different interactions between patients, family caregivers and health professionals. It was identified that the difficulties in this process can be mitigated through strategies adopted by professionals to expand the communication and the support for the specific needs of families and, also, through the implementation of public policies that favor the caregiver.

ABSTRACT Introduction: The study of comorbidity requires a multilateral approach with a view to improving the quality of care for these patients by the care system. Objectives: To explore the magnitude of the comorbidity of chronic diseases in adults admitted to hospitals. Methods: Prospective-observational-longitudinal-analytical study. Patients hospitalized in a medical clinic room or clinical patients in indiscriminate hospitalization rooms are included, Multicenter study in 42 centers, with 2 years of recruitment. Consecutive sampling. Descriptive, inferential and regression statistics. Results: 5925 recruited, male gender 50,3%, age 60,66 ± 0,25 years, main origin from the guard 73%, stay 12,61 ± 0,24 days, longer in surgical (15,45 ± 0,67 vs 11,76 ± 0,23, p < 0,00001), 23% received surgical treatment. Main educational level: complete secondary school 21,6%. Economic difficulties: 20%, mortality 9,26%, prevalence of dyslipidemia, diabetes and hypertension: 22,53%, 28,82% and 51,86% with 473 new diagnoses in said pathologies, BMI: 27,88 ± 0,65, Global Charlson 2,09 ± 0,02 and in deaths 3,84 ± 0,11. The average number of pathologies per patient was 2,14 ± 0,01 and increased with age (p value for linear regression < 0,00001). Conclusions: Hypertension, diabetes and dyslipidemia represented the most prevalent entities in the clinical hospitalization room, cardiovascular, respiratory, infectious, oncological, neurological, metabolic and nephrological diseases were independent predictors of mortality.

RESUMEN Introducción: El estudio de la comorbilidad requiere de un enfoque multilateral con vistas a mejorar la calidad de la atención de los enfermos por el sistema de atención. Objetivos: Explorar la magnitud de la comorbilidad de enfermedades crónicas en adultos internados en los hospitales. Métodos: Se realizó un estudio prospectivo-observacional-longitudinal-analítico. Se incluyeron pacientes internados en las Salas de Clínica Médica o pacientes clínicos en Salas de Internación Indiscriminada. Se realizó un estudio multicéntrico en 42 centros en un período de 2 años, con un muestreo consecutivo. Para el estudio se tuvo en cuenta la estadística descriptiva, inferencial y de regresión. Resultados: El total de pacientes en el estudio fue de 5925, masculinos con el 50,3 % de edad 60,66 ± 0,25 años. Principal procedencia desde la guardia el 73 %. La estadía hospitalaria de 12,61 ± 0,24 días, mayormente en pacientes quirúrgicos (15,45 ± 0,67 vs 11,76 ± 0,23; p < 0,00001). El 23 % recibió tratamiento quirúrgico. El principal nivel educativo: secundario completo 21,6 %. Dificultades económicas: 20 %, mortalidad 9,26 %; prevalencia de dislipemia, diabetes e hipertensión: 22,53 %; 28,82 % y 51,86 % con 473 nuevos diagnósticos, IMC: 27,88 ± 0,65, Charlson global 2,09 ± 0,02 y en óbitos 3,84 ± 0,11. La media de patologías por paciente fue de 2,14 ± 0,01 y aumentó con la edad (p valor regresión lineal < 0,00001). Conclusiones: La hipertensión, la diabetes y la dislipemia representaron las entidades más prevalentes en Salas de Internación Clínica, Las enfermedades cardiovasculares, respiratorias, infectológicas, oncológicas, neurológicas, metabólicas y nefrológicas fueron predictores independientes de mortalidad.

Abstract Background: According to the literature 58.7% to 86.7% of hospitalized patients have a peripheral venous catheter. However, the use of this device is not without complications. The management of these devices generates a significant demand for care by the nursing team. Objective: Construct and validate a scale to determine the time interval for the evaluation of peripheral accesses using continuous infusion in pediatrics. Methodology: Methodological, observational, prospective, quantitative and convergent assistential study. Results: The scale was submitted to two phases for construct validation. The final sum of all scores had an agreement of 87.5%. The Linear Weighted Kappa Reliability Test showed almost perfect reliability 0.91, 0.87 and 0.88 for patient’s clinical condition, vein characteristics and final score respectively. All indicators were statistically significant (p = 0.001). Conclusion: The time interval for the evaluation of peripheral accesses in continuous infusion, considering the specificity of each patient, is possible using the Maia e Castro scale, which helps the practice of nursing care in pediatric patients.

Marco contextual: Según la bibliografía, entre el 58,7% y el 86,7% de los pacientes hospitalizados tienen un catéter venoso periférico. Sin embargo, el uso de este dispositivo no está exento de complicaciones. La gestión de estos dispositivos genera una importante necesidad de cuidados por parte del equipo de enfermería. Objetivo: Construir y validar una escala para determinar el intervalo de tiempo para evaluar los accesos periféricos mediante infusión continua en pediatría. Metodología: Estudio metodológico, observacional, prospectivo, cuantitativo y convergente asistencial. Resultados: La escala se sometió a dos fases de validación de constructo. La suma final de todas las puntuaciones mostró una concordancia del 87,5%. La prueba de fiabilidad Kappa lineal ponderada indicó una fiabilidad casi perfecta de 0,91, 0,87 y 0,88 para el estado clínico del paciente, las características de las venas y la puntuación final, respectivamente. Todos los indicadores fueron estadísticamente significativos (p = 0,001). Conclusión: El intervalo de tiempo para evaluar los accesos periféricos en infusión continua, teniendo en cuenta la especificidad de cada paciente, es posible utilizando la Escala de Maia y Castro, que ayuda a la práctica de los cuidados de enfermería en la población pediátrica.

Resumo Enquadramento: Segundo a literatura 58,7% a 86,7% dos pacientes hospitalizados têm um cateter venoso periférico. Contudo, a utilização desse dispositivo não é isenta de complicações. A gestão desses dispositivos gera uma necessidade significativa de cuidados pela equipa de enfermagem. Objetivo: Construir e validar uma escala para determinar o intervalo tempo para a avaliação dos acessos periféricos em uso de infusão contínua em pediatria. Metodologia: Estudo, metodológico, observacional, prospetivo, quantitativo e convergente assistencial. Resultados: A escala foi submetida a duas fases para validação de constructo. A somatória final de todos os scores teve concordância de 87,5%. O Teste de confiabilidade de Kappa Ponderado Linear apontou confiabilidade quase perfeita 0,91, 0,87 e 0,88 para a condição clínica do paciente, características das veias e a pontuação final, respetivamente. Todos os indicadores foram estatisticamente significativos (p = 0,001). Conclusão: O intervalo de tempo para avaliação dos acessos periféricos em infusão contínua considerando a especificidade de cada paciente é possível utilizando a Escala Maia e Castro que auxilia a prática da assistência do enfermeiro na população pediátrica.

Abstract Food allergy is a common event, especially in the pediatric population, affecting between 3-6% of children. There are various challenges in the care of patients with food allergy, but certainly the diversity of symptoms and the outcome of the disease are important aspects in the treatment of patients, in the development of care guidelines and in the knowledge of the Natural history of disease. The classification into phenotypes allows a better understanding of the evolution of food allergy. The endotype is a subtype of a phenotype defined by its pathophysiological characteristic. Genetic, epigenetic, and environmental characteristics interfere with the construction of the phenotype and its possible endotypes. Understanding the phenotypes and endotypes of food allergies brings with it two fundamental aspects: 1) the need to understand that the analysis of pheno-types and endotypes in food allergies will allow establishing prognoses and helping in intervention with specif-ic therapies, and 2) the importance of understanding The characterization of local endotypes and phenotypes, and the dietary diversity of the different countries that make up Latin America brings with it a varied menu of foods that may pose a risk of allergy that needs to be studied.

Resumen La alergia alimentaria es un evento frecuente, especialmente en la población pediátrica, que afecta entre el 3-6% de los niños. Existen diversos desafíos en el cuidado de los pacientes con alergia alimentaria, pero ciertamente la diversidad de los síntomas y el desenlace de la enfermedad son aspectos importantes en el tratamiento de los pacientes, en la elaboración de las pautas de cuidado y en el conocimiento de la historia natural de la enfermedad. La clasificación en fenotipos permite una mejor comprensión de la evolución de la alergia alimentaria. El endotipo es un subtipo de un fenotipo definido por su característica fisiopatológica. Las características genéticas, epigenéticas y ambientales interfieren en la construcción del fenotipo y sus posibles endotipos. Entender los fenotipos y endotipos de las alergias alimentarias trae consigo dos aspectos funda-mentales: 1) la necesidad de entender que el análisis de fenotipos y endotipos en alergia alimentaria permitirá establecer pronósticos y ayudar en la intervención con terapias específicas, y 2) la importancia de comprender la caracterización de los endotipos y fenotipos locales, y la diversidad alimentaria de los diferentes países que conforman América Latina trae consigo un variado menú de alimentos que puede suponer un riesgo de alergia que es necesario estudiar.

Abstract The objective of this study was to identify and discuss the tools for the promotion and evaluation of adequate and healthy eating based on the Food Guide for the Brazilian Population. The scoping review was conducted according to the JBI Manual for Evidence Synthesis via the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) checklist and by searching PubMed/MEDLINE, EMBASE, SciELO and LILACS. Sixteen studies on relevant tools, based on the food guide for individuals of all age groups, were thus included: two descriptive studies, two randomized clinical trial protocols, eleven methodological analyses, and one psychometric paper, aimed at either a focal population (n=12) or health professionals (n=4). Six studies addressed tools for promoting adequate and healthy eating, and ten developed tools for evaluating dietary practices or the knowledge and activities of health professionals. This review can therefore assist health professionals in choosing instruments for the implementation and/or dissemination of food guide recommendations, contributing to the promotion of adequate and healthy eating habits.

Resumo O objetivo deste estudo foi identificar e discutir as ferramentas de promoção e avaliação da alimentação adequada e saudável baseadas no Guia Alimentar para a População Brasileira. A revisão de escopo foi conduzida de acordo com o JBI Manual for Evidence Synthesis, utilizando para relatório de revisão de escopo o guia Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews, com busca nas bases de dados PubMed/MEDLINE, EMBASE, SciELO e LILACS. Dezesseis estudos sobre ferramentas baseadas no guia alimentar para indivíduos de todas as faixas etárias, foram incluídos: dois descritivos, dois de protocolo de estudo clínico randomizado, onze metodológicos e um psicométrico, direcionados à população (n=12) ou à profissionais de saúde (n=4). Seis estudos abordaram ferramentas para promoção da alimentação adequada e saudável e dez desenvolveram ferramentas para avaliação de práticas alimentares ou de conhecimento e atividades de profissionais da saúde. Esta revisão auxiliará os profissionais de saúde na escolha dos instrumentos para o processo de implementação e, ou, divulgação das recomendações do guia alimentar, contribuindo para a promoção da alimentação adequada e saudável.

Resumo Com o objetivo de analisar as relações interprofissionais produzidas a partir da alta responsável, na perspectiva e no agir da enfermagem durante o a pandemia de COVID-19, realizou-se um estudo qualitativo, tipo estudo de caso. A técnica de pesquisa foi a observação participante de uma enfermaria de hospital de grande porte do SUS na cidade de São Paulo. A produção de narrativas e a análise micropolítica das relações fez emergir dois planos de visibilidade: para além da alta responsável - a antropofagia dos arranjos tecnológicos do cuidado e a ambivalência da enfermagem na produção das relações interprofissionais; e alta médico-centrada e alta negociada - o entrecruzamento com outros profissionais, com as famílias e com a vida “como ela é”. A pandemia de COVID interrompeu as visitas multiprofissionais e foi um analisador das relações interprofissionais. A partir de uma inteligência astuciosa, a enfermagem negocia a alta com os médicos, que detêm este poder, e aciona a equipe, a partir de uma autonomia elástica. A alta responsável por si só não foi capaz de produzir um plano comum de ação interprofissional, de modificar os papéis instituídos no hospital, situação que recrudesceu durante a pandemia mas abriu condições para o aumento da profissionalização da equipe.

Abstract A qualitative-case study was carried out aimed at analyzing the interprofessional relationships generated by the planned discharge from the nursing actions’ perspective during the COVID-19 pandemic. The study method was the participating observation by one nurse who works in a large SUS hospital in the city of São Paulo. The production of narratives and the micropolitics analysis resulted in two diverse visibility plans: beyond the planned discharge the anthropophagy of the technological arrangements for care and the ambivalence of the nursing staff in the production of interprofessional relationships; and the medical discharge and negotiated discharge: the intersecting with other professionals, with the families and with “real” life. The pandemic interrupted the multiprofessional visits and it was an analyzer of the interprofessional relationships. Wittingly, the nursing staff negotiates the discharges with physicians, who retain this power, and sets the team in motion using an elastic autonomy. The planned discharge alone was not able to guarantee a common interprofessional action plan, was not able to modify the constituted roles in the hospital, a situation that increased during the pandemic, but allowed the right setting aimed to increase the team’s professionalism.
