Resumo Trata-se de um artigo original que aborda a força de trabalho (FT) em saúde no Brasil, suas comorbidades e alterações da saúde mental na pandemia da COVID-19. O estudo conduzido pela Fundação Oswaldo Cruz coletou dados por meio de questionário on-line com um total de 36.612 participantes: profissionais de saúde (PS, formação de nível superior), e trabalhadores invisíveis (TI, nível técnico). A prevalência de comorbidades no Brasil foi de 26,1% e 23,9%, a maior foi hipertensão arterial (27,4% e 31,9%), seguida da obesidade (18,4% e 15,1%), doenças crônicas respiratórias (15,7% e 12,9%), diabetes mellitus (10,3% e 10,4%), e depressão/ansiedade (9,1% e 11,7%), nos PS e TI, respectivamente. A região com maior frequência foi a sudeste onde concentra-se o maior contingente de trabalhadores. A FT acometida com alta carga de doenças crônicas não transmissíveis e exposta ao SARS-CoV-2, torna-se vulnerável para o adoecimento e morte. Sintomas mentais e intenso sofrimento psíquico foram relatados. Os resultados deste estudo nos permitem estimar o impacto na saúde física e mental, e nas condições de vida e de trabalho da FT. A saúde e a vida dos trabalhadores, protagonistas no enfrentamento dos desafios da pandemia, são prioridade nas políticas públicas.

Abstract This is an original article that addresses the healthcare workforce (HW) in Brazil, as well as comorbidities and mental health changes during the COVID-19 pandemic. This study was conducted by the Oswaldo Cruz Foundation and collected data through an online questionnaires from a total of 36,612 participants, health professionals (HP, with higher education level), and invisible healthcare workers (IHW, with a technical mid-level education). The overall prevalence of comorbidities in Brazil was 26.1% and 23.9%; the highest was arterial hypertension (27.4% and 31.9%), followed by obesity (18.4% and 15.1%), chronic respiratory diseases (15.7% and 12.9%), diabetes mellitus (10.3% and 10.4%), and depression/anxiety (9.1% and 11.7%), in the HW and IHW, respectively. The region with the highest frequency was the southeast, where the largest contingent of workers is located. The HW, affected with a high burden of non-communicable chronic diseases and exposed to SARS-CoV-2, proved to be vulnerable to illness and death. Mental symptoms and intense psychological suffering have been reported. These results allow us to estimate the impacts upon physical and mental health, as well as upon living and working conditions of the HW. The health and life of workers, leading role in facing health challenges of the pandemic, are a high priority in public policies.

Resumen Introducción Son necesarios tratamientos basados en evidencia para mujeres víctimas de violencia de pareja (IPV), con trastornos relacionados con estrés como ansiedad, depresión y riesgo suicida y alteraciones en la reactividad del cortisol. Objetivo Comparar los cambios en la sintomatología depresiva, ansiosa, calidad de vida y reactividad del cortisol después de aplicar Terapia de Aceptación y Compromiso (ACT, basada en la flexibilidad psicológica) o Terapia Interpersonal (IPT, basada en el empoderamiento) en mujeres expuestas a IPV y compararlos en función al pensamiento suicida. Método Una muestra clínica de 50 mujeres (entre 21 y 74 años) fueron asignadas aleatoriamente a recibir ACT o IPT, completaron antes y después, cuestionarios sobre IPV, calidad de vida, depresión y ansiedad, y mediciones de cortisol (basal, 15, 30 y 45 minutos después de un reto cognitivo) en saliva en respuesta a un reto cognitivo. Utilizamos modelos de Ecuaciones de Estimación generalizada para analizar los datos. Resultados Mejoraron significativamente todas las variables, independientemente del tipo de psicoterapia y del pensamiento suicida. Antes de la intervención, las mujeres con ideación suicida presentaron depresión y ansiedad severas, peor calidad de vida e hiper-respuesta de cortisol, en contraste con las mujeres sin ideación, quienes presentaron síntomas moderados y una respuesta de cortisol plana. El cortisol disminuyó en todas las mujeres después de ambas terapias. Discusión y conclusión Ambas terapias IPT y ACT mejoraron la salud mental y calidad de vida de las mujeres con IPV y modificaron la reactividad del cortisol, incluyendo a las pacientes que reportaron ideación suicida.

Abstract Introduction There is a growing need to implement evidence-based psychological treatments for women victims of intimate partner violence (IPV) who commonly experience stress-related disorders such as anxiety, depression, or suicide risk, as well as altered cortisol reactivity. Objective 1. To compare the changes in depressive and anxious symptomatology, quality of life, and cortisol reactivity after two different interventions, Acceptance and Commitment Therapy (ACT, based on psychological flexibility) or Interpersonal Therapy (IPT, based on empowerment) in women exposed to IPV. 2. To compare the changes in relation to the presence of suicide thoughts. Method A clinical sample of 50 women (ages 21-74) randomly assigned to attend ACT or IPT, completed pre- and post-intervention questionnaires about intimate partner violence, quality of life, depression, anxiety, and measurements of salivary cortisol reactivity (basal, 15, 30, and 45 minutes after a cognitive challenge). We used Generalized Estimating Equation Models for data analysis. Results There was a significant improvement post-intervention in all variables, regardless of the type of psychotherapy or the presence of suicide thoughts. Before interventions, women that reported suicide thoughts had severe symptoms of depression, anxiety, worse quality of life, and a cortisol hyper-response profile, in contrast to women without suicide thoughts who had moderate symptoms and no cortisol response. Cortisol response to the cognitive test decreased in all women after both therapies. Discussion and conclusion Although different psychological approach, ACT, and IPT effectively improved mental health, quality of life, and changed cortisol reactivity of women exposed to IPV, including women at suicide risk.

Abstract This study described the growth, morphometric, biomechanical, and chemical properties of the femur, tibiotarsus, and tarsometatarsus of European and Japanese quail. Analyses were performed at 13 and 15 days of incubation, at hatch, and at 4, 7, 10, 14, 21, 28, and 35 days post-hatch (n=6/subspecies/period). Bone specimens were analyzed by cone-beam computed tomography, biomechanical assays, chemical analyses, and histomorphometry. Variables were fitted by the Gompertz function and its derivative or assessed using the analysis of variance. Analysis of the derivative of Gompertz curves showed that the growth behavior of the tarsometatarsal bone was similar between quail subspecies, and the femur and tibiotarsus of European quail increased first in width and then in length, whereas the opposite occurred in Japanese quail. There was an interaction between quail subspecies and days of growth on femoral, tarsometatarsal, and tibiotarsal bone densities. Femoral and tibiotarsal cross-sectional areas were influenced by the interaction of quail subspecies and day of growth. Interaction effects were significant for breaking strength and phosphorus percentage. European and Japanese quail have different femoral and tibiotarsal growth patterns, especially in the first few days after hatching, whereas tarsometatarsal growth is similar between subspecies.

Abstract This article discusses the process of institutional removal of young people that reside in foster care institutions for reaching adulthood. It relies on a politics of the sensitive to make visible the encounters and mismatches that take place between young people and Brazilian public policies. To do so, meetings were held with young people who had already experienced the removal process and with young people who would soon turn 18 and would have to leave the host institutions. To make these existences visible, this study invested in the writing of biographems, inspired by the works of Roland Barthes. The concepts of necropolitics and precarious lives were fundamental to understand the omissions of the State at the time of removal. It was also found that the State can maximize the precariousness of some lives, especially those marked by culturally marginalized race, gender, and class characteristics. However, it is also the encounter with public policies that ensures better living conditions for some, facilitating access to the university and the labor market. This research points out that, in the face of abandonment, young people become fireflies, producing luminosity amid the darkness and claiming the right to life.

Resumen Este artículo aborda el proceso de desconexión institucional justificado por la edad adulta de los jóvenes que residen en los servicios de acogida. Utilizamos una política sensible para hacer visibles las reuniones y los desajustes que tienen lugar entre los jóvenes y las políticas públicas brasileñas. Con este fin, se celebraron reuniones con los jóvenes que ya habían pasado por el proceso de desconexión institucional y también con los jóvenes que pronto cumplirían los 18 años y tendrían que abandonar las instituciones de acogida. Para hacer visibles estas existencias, se redactaron biografemas, inspirados en el trabajo de Roland Barthes. Los conceptos de necropolítica y vida precaria fueron fundamentales para comprender las omisiones del Estado en el momento de la desconexión. Se encontró que el Estado puede maximizar la precariedad de algunas vidas, principalmente de aquellas marcadas por características de raza, género y clase culturalmente marginadas. Sin embargo, el encuentro con las políticas también puede garantizar mejores condiciones de vida para algunos, facilitándoles el acceso a la universidad y al mercado laboral. Esta investigación señala que, ante el abandono, los jóvenes se convierten en luciérnagas, produciendo luminosidad en medio de la oscuridad y reclamando el derecho a la vida.

Resumo Este artigo versa sobre o processo de desligamento institucional por maioridade de jovens que residem em serviços de acolhimento. Aposta-se em uma política do sensível para visibilizar os encontros e desencontros que acontecem entre as e os jovens e as políticas públicas brasileiras. Para tanto, realizaram-se encontros com jovens que já haviam passado pelo processo de desligamento e com jovens que logo completariam 18 anos e teriam de sair das instituições de acolhimento. Para tornar visíveis essas existências, investiu-se na escrita de biografemas, inspirados na obra de Roland Barthes. Os conceitos de necropolítica e vidas precárias foram fundamentais para compreender as omissões do Estado no momento do desligamento. Verificou-se que o Estado pode maximizar a precariedade de algumas vidas, especialmente daquelas marcadas por características de raça, gênero e classe culturalmente marginalizados. Contudo, é também o encontro com as políticas públicas que garante melhores condições de vida para alguns, facilitando o acesso à universidade e ao mercado de trabalho. A pesquisa aponta que, diante do abandono, as e os jovens se fazem vagalumes, produzindo luminosidades em meio à escuridão e reivindicando o direito à vida.

ABSTRACT This study aimed to analyze Resolution CNE/CP 2/2019 - which defines the current National Curriculum Guidelines for the initial teacher training and sets the Common National Base (BNC-Training) - in order to identify and discuss the implications of proposed changes in valuing teaching work. This was a qualitative study, in which data were collected through literature and documentary analysis, systematized and analyzed through two categories: Relationship teacher training - professional ethos; Professional appreciation and engagement. The results demonstrate the alignment of the new resolution with the neoliberal prescription, limiting training to technical and pragmatic aspects adjusted to an economistic perspective. This resolution clearly expresses a standardization of policies for teacher training focused on skills in order to meet the assumptions of the BNCC. Also, the present logic neglects professional development as a collective project and becomes associated with meritocratic criteria of individual performance related to liberal ideas. Finally, we consider the need to build an active/collective resistance, both at the pedagogical and political levels, against the precarity and privatization of higher education.

RESUMO Este estudo apresenta os resultados de uma pesquisa que teve por objetivo analisar a Resolução CNE/CP n.º 2/2019 - que define as atuais Diretrizes Curriculares Nacionais para a formação inicial de professores e institui a Base Nacional Comum (BNC-Formação) - de modo a identificar e discutir as implicações das mudanças propostas na valorização do trabalho docente. Trata-se de uma pesquisa qualitativa na qual os dados foram coletados por meio de análise bibliográfica e documental, sistematizados e analisados por meio de duas categorias: Relação formação docente - ethos profissional; Valorização e engajamento profissional. Os resultados demonstram o alinhamento da nova resolução com o receituário neoliberal, limitando essa formação a aspectos técnicos e pragmáticos ajustados a uma perspectiva economicista. Fica explícito que essa resolução expressa uma padronização das políticas para a formação docente centrada em competências de modo a atender aos pressupostos da Base Nacional Comum Curricular (BNCC). Entendemos também que a lógica na qual está baseada negligencia a valorização profissional como um projeto coletivo e passa a ser associada a critérios meritocráticos de desempenho individual ligados ao ideário liberal. Por fim, consideramos a necessidade de construção de uma resistência ativa/coletiva, tanto no plano pedagógico como no plano político, contra a precarização e a privatização do Ensino Superior.

ABSTRACT Objective To assess the incidence of febrile neutropenia without primary granulocyte colony-stimulating factor prophylaxis in patients undergoing chemotherapy with adjuvant docetaxel and cyclophosphamide, and to evaluate the toxicity profile of brand-name docetaxel (Taxotere ® ) and the generic formulation. Methods This retrospective study was conducted using data obtained from electronic medical records of patients treated at a Brazilian cancer center. Patients with breast cancer who underwent adjuvant treatment between January 2016 and June 2019 were selected. Data were analyzed using chi-square and Fisher correlation of variables, and multivariate analyses were adjusted for propensity score. Results A total of 231 patients with a mean age of 55.9 years at the time of treatment were included in the study. The majority (93.9%) had luminal histology, 84.8% were at clinical stage I, and 98.2% had a good performance status. The overall incidence of febrile neutropenia in the study population was 13.4% (31 cases). The use of brand-name docetaxel (Taxotere ® ) was the only factor associated with febrile neutropenia occurrence (OR= 3.55, 95%CI= 1.58-7.94, p=0.002). Conclusion In patients with breast cancer who require treatment with adjuvant docetaxel and cyclophosphamide regimen, the toxicity profile differs between brand-name and generic docetaxel. Regardless of the formulation used, the incidence of febrile neutropenia was less than 20%, which may allow for the omission of primary prophylactic granulocyte colony-stimulating factor use in this setting.

RESUMO A doença de Alzheimer (DA) e outras demências neurodegenerativas têm um curso progressivo com comprometimento da cognição, capacidade funcional e comportamento. A maioria dos estudos enfocou a DA. A demência grave está associada ao aumento da idade, maior morbimortalidade e aumento dos custos de cuidados. É fundamental reconhecer que a demência grave é o período mais longo de progressão, com o paciente vivendo muitos anos nesta fase. É a fase mais heterogênea do processo, com diferentes habilidades e expectativa de vida. Esta diretriz de prática concentra-se na demência grave para melhorar o manejo e o cuidado nessa fase da demência. Como um longo período no continuum da demência, as abordagens não farmacológicas e farmacológicas devem ser consideradas. Intervenções multidisciplinares (fisioterapia, fonoaudiologia, nutrição, enfermagem, entre outras) são essenciais, além de educacionais e de apoio aos cuidadores.

ABSTRACT Alzheimer’s disease (AD) and other neurodegenerative dementias have a progressive course, impairing cognition, functional capacity, and behavior. Most studies have focused on AD. Severe dementia is associated with increased age, higher morbidity-mortality, and rising costs of care. It is fundamental to recognize that severe dementia is the longest period of progression, with patients living for many years in this stage. It is the most heterogeneous phase in the process, with different abilities and life expectancies. This practice guideline focuses on severe dementia to improve management and care in this stage of dementia. As it is a long period in the continuum of dementia, clinical practice should consider non-pharmacological and pharmacological approaches. Multidisciplinary interventions (physical therapy, speech therapy, nutrition, nursing, and others) are essential, besides educational and support to caregivers.

ABSTRACT Introduction: the intervention to control Aedes aegypti, with intersectoral and population involvement, is a key element in the struggle with dengue and a powerful instrument to reduce arboviruses. Objective: to design a community interventional strategy to reduce dengue in the Community Policlinic Guillermo Tejas Silva, of the municipality Las Tunas. Materials and methods: an interventional strategy was designed to reduce the infestation indexes of Aedes aegypti in the Community Policlinic Guillermo Tejas Silva, in the period from January to December 2019. The universe was formed by 357 blocks from which 47, evaluated as highly risky, were chosen as a sample. In the design of the strategy, the following educational and technical components were combined: surveillance; focal, perifocal and adulticidal treatment. Simultaneously, a community and intersectoral treatment was carried out. Results: with the application of the interventional strategy, 100 % of the health care staff and focal duos, and 97 % of the population were trained. Besides that the infestation index was reduced from 2 % to 0.07 % and the risk in 86.2 %. The 47 blocks that were in the stratus I, were reduced to 7, and dengue occurrence to 0. Conclusions: the application of the strategy led to the community empowerment in the control of dengue, with changes in the environmental management, increment in the level of knowledge, and reduction of the infestation index.

RESUMEN Introducción: la intervención para el control del Aedes aegypti con la participación comunitaria e intersectorialidad, es un elemento clave en la lucha contra el dengue y un poderoso instrumento para la reducción de las arbovirosis. Objetivo: diseñar una estrategia de intervención comunitaria para reducir el dengue, en el Policlínico Comunitario Guillermo Tejas Silva, del municipio Las Tunas. Materiales y métodos: se diseñó una estrategia de intervención para reducir los índices de infestación por Aedes aegypti, en el Policlínico Comunitario Guillermo Tejas Silva, en el período de enero a diciembre de 2019. El universo estuvo constituido por 357 manzanas, de las que se seleccionó una muestra de 47, evaluadas de alto riesgo. En el diseño de la estrategia se combinaron los siguientes componentes educativos y técnicos: vigilancia, tratamiento focal, perifocal y adulticida. Se realizó de forma simultánea un tratamiento comunitario e intersectorial de los riesgos. Resultados: con la aplicación de la estrategia de intervención se capacitaron el 100 % del personal de salud, los dúos focales y el 97 % de la población. Además, se redujo el índice de infestación del 2 al 0,07 % y los riesgos en un 86,2 %. De 47 manzanas en el estrato I, se redujeron a siete, y la ocurrencia de dengue a 0. Conclusiones: la aplicación de la estrategia logró factibilidad del empoderamiento comunitario en el control del dengue, con cambios en el ordenamiento del medio, aumento del nivel de conocimiento y reducción del índice de infestación.

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

RESUMO Objetivo: Caracterizar os usuários, os atendimentos e as motivações para busca de atendimento online em saúde mental durante a pandemia da COVID-19. Métodos: Estudo descritivo, retrospectivo, com análise documental de prontuários de usuários atendidos entre abril e julho de 2020. Resultados: Dos 258 acessos, 159 foram efetivados, e 99 foram mensagens off-line. Apesarde o mês de maio apresentar 61 (38,36%) atendimentos, em abril ocorreram 49 (30,82%) em 18 dias de acolhimento. Mais de 89% dos acessos foram de mulheres, entre 40 e 59 anos. Dasmotivações na busca por atendimento, destacam-se a ansiedade, medo, depressão, tristeza, choro, estresse e ideação suicida. Considerações finais: O chat é uma oportunidade de intervenção rápida, precoce e com curto tempo de espera para as pessoas que necessitam de cuidado em saúde mental. Portanto, ressalta-se a importância de se investir na formação de profissionais de enfermagem para a atuação na saúde mental.

Methods: A descriptive, retrospective study, with documentary analysis of medical records of users served between April and July 2020. Results: Of the 258 accesses, 159 were complete, and 99 were offline messages. Although there were 61 (38.36%) consultations in May, in April, there were 49 (30.82%) in 18 days of reception. More than 89% of accesses were women between 40 and 59 years. Among the motivations in the search for care, anxiety, fear, depression, sadness, crying, stress, and suicidal ideation stand out. Final considerations: Chat is a quick, early and short-wait intervention opportunity for people in need of mental health care. Therefore, the study emphasizes the importance of investing in the training of nursing professionals to act in mental health.

RESUMEN Objetivo: Caracterizar usuarios, atenciones y motivaciones para búsqueda de atención online en salud mental durante la pandemia de COVID-19. Métodos: Estudio descriptivo, retrospectivo, con análisis documental de prontuarios de usuarios atendidos entre abril y julio de 2020. Resultados: De los 258 accesos, 159 fueron efectuados, y 99 fueron mensajes desconectadas. Aunque el mes de mayo presentó 61 (38,36%) atenciones, en abril ocurrieron 49 (30,82%) en 18 días de recepción. Más de 89% de los accesos fueron de mujeres, entre 40 y 59 años. De las motivaciones en la búsqueda por atención, destacan la ansiedad, miedo, depresión, tristeza, lloro, estrés e ideación suicida. Consideraciones finales: El chat es una oportunidad de intervención rápida, precoz y con corto tiempo de espera para las personas que necesitan de cuidado en salud mental. Así, se resalta la importancia de invertir en la formación de profesionales de enfermería para la actuación en la salud mental.

ABSTRACT OBJECTIVE To identify the prevalence of and factors associated with: (1) major depressive episodes; (2) minor psychiatric disorders (MPDs); and (3) suicidal ideation among nursing professionals from a municipality in southern Brazil. METHODS Using a cross-sectional design, we recruited 890 nursing professionals linked to 50 Primary Care units, 2 walk-in clinics, 2 hospital services, 1 emergency room service, 1 mobile emergency care service, and 1 teleconsultation service, in addition to the municipal epidemiological surveillance service and the vacancy regulation center between June and July 2020. We used the Patient Health Questionnaire-9 and the Self-Reporting Questionnaire to evaluate the studied outcomes. Associations between the outcomes and variables related to sociodemographic profile, work, health conditions, and daily life were explored using Poisson regression models with robust variance estimators. RESULTS The observed prevalence of depression, MPDs, and suicidal ideation were 36.6%, 44%, and 7.4%, respectively. MPDs were associated with the assessment of support received by the service as ‘regular’ (PR: 1.48; 95% CI: 1.19–1.85) or ‘poor’ (PR: 1.54; 95% CI: 1.23–1.94), with a reported moderate (PR: 1.63; 95% CI: 1.29–2.07), or heavy (PR: 2.54; 95% CI: 2.05–3.15) workload, and with suspected COVID-19 infection (PR: 1.44; 95% CI: 1.25–1.66). Major depressive episodes were associated with a reported lack of personal protective equipment (PR: 1.20; 95% CI: 1.01–1.42), whereas suicidal ideation was inversely related to per capita income > 3 minimum monthly wages (PR: 0.28; 95% CI: 0.11–0.68), and positively related to the use of psychotropic drugs (PR: 3.14; 95% CI: 1.87–5.26). CONCLUSION Our results suggest that nursing professionals’ working conditions are associated with their mental health status. The need to improve working conditions through adequate dimensioning, support and proper biosafety measures is only heightened in the context of the COVID-19 pandemic.

Abstract In order to monitor the effects of anthropogenic pressures in ecosystems, molecular techniques can be used to characterize species composition. Among molecular markers capable of identifying species, the cytochrome c oxidase I (COI) is the most used. However, new possibilities of biodiversity profiling have become possible, in which molecular fragments of medium and short-length can now be analyzed in metabarcoding studies. Here, a survey of fishes from the Saint Peter and Saint Paul Archipelago was barcoded using the COI marker, which allowed the identification of 21 species. This paved the way to further investigate the fish biodiversity of the archipelago, transitioning from barcoding to metabarcoding analysis. As preparatory steps for future metabarcoding studies, the first extensive COI library of fishes listed for these islands was constructed and includes new data generated in this survey as well as previously available data, resulting in a final database with 9,183 sequences from 169 species and 63 families of fish. A new primer specifically designed for those fishes was tested in silico to amplify a region of 262 bp. The new approach should guarantee a reliable surveillance of the archipelago and can be used to generate policies that will enhance the archipelago’s protection.
