Introdução: Metástase óssea é a neoplasia maligna mais comum do esqueleto, e a tomada de decisão cirúrgica depende de múltiplos fatores, incluindo as complicações pós-operatórias e a expectativa de vida. A identificação de novos fatores prognósticos pode auxiliar na tomada de decisão. Objetivo: Analisar em metástases de ossos longos, a incidência de complicações e sobrevida pós-operatórias até 1 ano correlacionando-as com NLR e PLR. Método: Revisão de 160 prontuários de operados por metástase óssea no esqueleto apendicular. Além de características epidemiológicas, foram determinados os valores de NLR e PLR correlacionando-os com sobrevida e complicações. Resultado: Mulheres representaram 64,5% com tumor primário na mama em 62,6%; o fêmur proximal foi o mais acometido; sobrevida média foi 13,2 meses e a de 1 ano 34,7%; ressecção tumoral com endoprótese foi mais comum. A taxa de complicação pós-cirúrgicas foi de 10% e o tempo médio para a ocorrência de complicações pós-operatórias foi de 27,9 dias (0-140). Foi encontrada associação da variável neutrófilos com a complicação pós-operatória (p=0,04). A cada 100 unidades a mais de neutrófilos houve aumento de 1% nas chances de complicações pós-cirúrgicas. Valores médios do NLR e PLR foram, respectivamente, 5,3 (0,2-30,7) e 199,7 (32,1-676,7).  Os pacientes com NLR ≥ 2 (p<0,001) apresentaram diminuição na sobrevida de 92,3% para 62,5% no 3° mês e de 61,5% para 31,3% em 1 ano. Aqueles com PLR ≥209 (p<0,001) apresentaram diminuição na sobrevida de 69% para 59,3% no 3° mês, e de 40,2% para 25,9% em 1 ano. Conclusão: Não foi verificada associação positiva entre o NLR e o PLR com complicações pós-operatórias, mas com sobrevida fortemente sim, a partir do 3° mês de pós-operatório.

Introduction: Bone metastasis is the most common malignant neoplasm of the skeleton, and surgical decision-making depends on multiple factors, including postoperative complications and life expectancy. The identification of new prognostic factors can assist in decision making. Objective: In long bones metastases, to analyze the incidence of complications and postoperative survival up to 1 year, correlating them with NLR and PLR. Method: Review of 160 medical records of patients who underwent surgery for bone metastasis in the appendicular skeleton. In addition to epidemiological characteristics, NLR and PLR values were determined, correlating them with survival and complications. Result: Women represented 64.5% with a primary breast tumor in 62.6%; the proximal femur was the most affected; median survival was 13.2 months and in 1 year 34.7%. Tumor resection with endoprosthesis was more common. The post-surgical complication rate was 10% and the average time for post-operative complications to occur was 27.9 days (0-140). An association between the neutrophil variable and postoperative complications was found (p=0.04). For every 100 more units of neutrophils there was a 1% increase in the chances of post-surgical complications. Mean NLR and PLR values were, respectively, 5.3 (0.2-30.7) and 199.7 (32.1-676.7). Patients with NLR NLR ≥ 2 (p<0,001) showed a decrease in survival from 92,3% to 62,5% at the 3rd month, and from 61,5% to 31,3% at 1 year. Those with PLR ≥209 (p<0.001) showed a decrease in survival from 69% to 59.3% at the 3rd month, and from 40.2% to 25.9% at 1 year. Conclusion: There was no positive association between NLR and PLR with postoperative complications, but strongly yes with survival from the 3rd month after surgery.

Introduction: In general, inflammation stimulates the production and release of neutrophils and, at the same time, decreases the production of lymphocytes. Lymphopenia reflects that cell-mediated immunity is impaired, while neutrophilia represents a response to systemic inflammation in these cancers. Objective: To review the incidence of complications and postoperative survival rates in patients with bone metastases in long bones, correlating them with markers NLR and PLR. Method: Narrative review carried out collecting information published on virtual platforms in Portuguese and English, initially carried out by searching for descriptors related to the topic, which were: “lower extremity, surgery, metastasis, epidemiology, postoperative complications, neutrophils, lymphocytes, platelets”. The extension incorporated AND or OR, by title and/or summary, and full reading of the texts most related to the topic. Result: 21 articles were included. Conclusion: The higher both the NLR and PLR are associated with lower survival in patients with bone metastases when undergoing surgical treatment, especially after 3 months postoperatively. However, there is still no confirmation that they signal any outcome, favorable or not, in relation to postoperative complications.

Introdução : De um modo geral, a inflamação estimula a produção e liberação de neutrófilos e, ao mesmo tempo, diminui a produção de linfócitos. A linfopenia reflete que a imunidade mediada pelas células é prejudicada, enquanto a neutrofilia representa resposta à inflamação sistêmica nesses cânceres. Objetivo : Revisar nos pacientes com metástase óssea em ossos ao longo da incidência de complicações e taxas de sobrevida pós-operatória correlacionando-as com os marcadores NLR e PLR. Método : Revisão narrativa feita colhendo informações publicadas em plataformas virtuais em português e inglês inicialmente realizada por busca dos descritores relacionados ao tema que foram: “extremidade inferior, cirurgia, metástase, epidemiologia, complicações pós-operatórias, neutrófilos, linfócitos, plaquetas” e seus equivalentes em inglês “ extremidade inferior, cirurgia, metástase, epidemiologia, sobrevivência, complicações, neutrófilos, linfócitos, plaquetas sanguíneas ”. A extensão incorporou AND ou OR, pelo título e/ou resumo, e leitura na íntegra dos textos mais relacionados ao tema.  Resultado : Foram incluídos 21 artigos. Conclusão : Quanto maiores, tanto o NLR quanto o PLR estão associados à menor sobrevida em pacientes com MO quando submetidos ao tratamento cirúrgico, especialmente após 3 meses de pós-operatório. Contudo, ainda não há confirmação de que eles sinalizam algum estágio, positivo ou não, em relação às complicações pós-operatórias.

Introduction: Evaluating the scarring differences between areas previously anesthetized with different local anesthetics is interesting to verify whether the choice of anesthetic helps in surgical healing in relation to inflammation, neovascularization and collagen deposition. Objectives: Review the existing literature with a focus on analyzing the role of local anesthetics in aiding the healing of surgical wounds. Method: Narrative review of literature selected from the SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus platforms. Initially, the review searched for keywords in the focus of the research, based on MESH/DeCS with the following terms: “local anesthetics, healing, neovascularization, collagen” with AND or OR search, considering the title and/or abstract. Next, only those that were most related to the topic were included, and the texts were read for inclusion. Results: 73 articles were included, read and interpreted focusing on the role of anesthetics in healing, pain, side effects and the best that currently emerge in analgesia and scar potential. Conclusions: No differences were observed between the anesthetics studied in terms of healing, inflammation and neovascularization. However, there was a significant difference in the production of collagens with an intensity of 6 to 12 times greater than collagen types I, II and III with the use of levobupivacaine.

Introdução: Avaliar as diferenças cicatriciais entre áreas previamente anestesiadas com diferentes anestésicos locais é interessante para verificar se a escolha do anestésico auxilia na cicatrização cirúrgica em relação à inflamação, neovascularização e deposição de colágeno. Objetivos: Revisar a literatura existente com foco de análise sobre o papel dos anestésicos locais no auxílio da cicatrização de feridas cirúrgicas. Método: Revisão narrativa da literatura selecionada das plataformas SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed e Scopus.  Inicialmente a revisão realizou a busca por palavras-chave no foco da pesquisa, com base no MESH/DeCS com os seguintes termos: “anestésicos locais, cicatrização, neovascularização, colágeno” com busca AND ou OR, considerando o título e/ou resumo. A seguir, foram incluídos somente os que tinham maior relação ao tema, e realizada a leitura dos textos para inclusão. Resultados: Foram incluídos, lidos e interpretados 73 artigos enfocando o papel dos anestésicos na cicatrização, dor, efeitos colaterais e os melhores que despontam na atualidade na analgesia e potencial cicatricial. Conclusões: Não se observou diferenças entre anestésicos pesquisados quanto à cicatrização, inflamação e neovascularização. Contudo, verificou-se diferença significativa na produção de colágenos com intensidade de 6 a 12 vezes maior de colágenos tipos I, II e III com o uso da levobupivacaína.

Objectives: To analyze the influence of ropivacaine and levobupivacaine on the inflammatory phase of surgical incision healing. Method: Thirty male Wistar rats were divided into 3 groups to evaluate the effects of these local anesthetics in comparison with each other and with a control group. The animals were subjected to general anesthesia and subsequently scheduled for administration of the solutions and their samples were excised after 5 days. The scar area, neovascularization and collagen deposition were studied. Results: Regarding inflammation and neovascularization, there were no significant differences between the groups, with all of them presenting pronounced levels. As for collagen deposition, this group exhibited significantly higher levels of type I, II, and III collagens compared to groups C and R. Regarding collagen deposition, this group demonstrated significantly higher levels of type I, II and III collagen when compared to groups C and R. Conclusions: No differences were observed between the groups in relation to the variation in scar measurements, inflammation and neovascularization. However, there was a significant difference in the production of collagens with an intensity of 6 to 12 times greater in collagen types I, II and III with the use of levobupivacaine.

Introdução: Avaliar as diferenças cicatriciais entre áreas previamente anestesiadas com diferentes anestésicos locais é interessante para verificar se a escolha do anestésico auxilia na cicatrização cirúrgica em relação à inflamação, neovascularização e deposição de colágeno. Objetivos: Analisar a influência da ropivacaína e levobupivacaína na fase inflamatória da cicatrização de incisões cirúrgicas. Método: Trinta ratos Wistar machos foram divididos em 3 grupos para avaliar os efeitos desses anestésicos locais em comparação entre si e com grupo controle. Os animais foram submetidos à anestesia geral e, posteriormente, marcados para a administração das soluções e terem excisadas suas amostras após 5 dias. Estudou-se a área cicatricial, neovascularização e deposição de colágenos. Resultados: Em relação à inflamação e neovascularização, não houve diferenças significativas entre os grupos., todos apresentando níveis acentuados. Quanto à deposição de colágenos, este grupo demonstrou níveis significativamente mais altos de colágenos tipo I, II e III quando comparados aos grupos C e R. Conclusões: Não se observou diferenças entre os grupos em relação à variação das medidas cicatriciais, inflamação e à neovascularização. Entretanto, verificou-se sim diferença significativa na produção de colágenos com intensidade de 6 a 12 vezes maior de colágenos tipos I, II e III com o uso da levobupivacaína.

Abstract Objectives: to evaluate the influence of perception of care and maternal protection on breastfeeding practices on the infants’ third month of life. Methods: longitudinal study with mother-infant pairs distributed in five groupsof gestational clinical conditions. The recruitment occurred in the period 2011 to 2016 at three hospitals in the public health systems in Porto Alegre, Brazil. The Parental Bonding Instrument and the Edinburgh Postpartum Depression Scale were assessed. Exclusive and prolonged breastfeeding were analyzed by questionnaires. Data were analyzed by one-way ANOVA with Tukey’s post-hoc test, Kruskal-Wallis with Dunn’s post-hoc test, or Pearson’s chi-squared test. The significance was set at 5%. Results: 209 mother-infant pairs were investigated. Among those who did not practice breastfeeding, a lower perception of care, a higher perception of maternal protection, and a higher score of postpartum depression were observed (p=0.022, p=0.038, and p<0.001, respectively), when compared to peers who practiced. The control group had a significantly higher perception of care when compared to thediabetes mellitus group (p=0.006), and the perception of maternal protection and postpartum depression had no differences between the intrauterine groups (p>0.05). Conclusions: the perception of care and maternalprotection and the postpartum depressive symptomatology influenced breastfeeding at three months. It is possible to assume a transgenerational effect on breastfeeding, suggesting the existence of a complex model related to mental health in a sample of women who had different backgrounds of gestational clinical conditions

Resumo Objetivos: avaliar a influência da percepção do cuidado e da proteção materna sobre as práticas de aleitamento materno em lactentes no terceiro mês de vida. Métodos: estudo longitudinal, com pares mães-lactentes distribuídos em cinco grupos de diferentes condições clínicas gestacionais. O recrutamento ocorreu no período de 2011 a 2016 em três hospitais da rede pública de saúde de Porto Alegre, Brasil. Foram utilizados o Parental Bonding Instrument e o Edinburgh Postpartun Depression Scale. O aleitamento materno exclusivo e continuado foi analisado por questionários. Na análise de dados foram utilizados os testes de ANOVA com post-hoc de Tukey, Kruskal-Wallis com post-hoc de Dunn e Qui-quadrado. Resultados: foram investigados 209 pares mães-lactentes. Entre aqueles que não praticaram o aleitamento materno foi observadouma menor percepção de cuidado materno, uma maior percepção de proteção materna e ummaior escore de depressão pós-parto (p=0,022, p=0,038 e p<0,001, respectivamente) quandocomparados aos pares mães-lactentes que praticaram. O grupo controle teve significativamente maior percepção do cuidado materno quando comparado ao grupo com diabetes mellitus (p=0,006) enquanto a percepção de proteção materna e a depressão pós-parto não apresentaram diferenças entre os cinco grupos intrauterinos (p>0,05). Conclusões: a percepção de cuidado e proteção materna e asintomatologia depressiva pós-parto influenciaram o aleitamento materno aos três meses. É possível assumir um efeito transgeracional no aleitamento materno, sugerindo a existência de um modelo complexo relacionado à saúde mental numa amostra de mulheres que tinham diferentes antecedentes de condições clínicas gestacionais.

RESUMEN El nitrógeno es el elemento que presenta mayor demanda por parte del cultivo de soja, y la fijación biológica del nitrógeno atmosférico el principal medio para abastecerlo. En cambio, micronutrientes y tratamientos químicos aplicados a la semilla conjuntamente con el inoculante pueden alterar dicha fijación biológica de nitrógeno. El objetivo de este trabajo fue evaluar el efecto de productos químicos, micronutrientes y de la fertilizacion nitrogenada, en la nodulacion, desarrollo y rendimiento de la soya. El experimento fue desarrollado en campo y en invernadero en el municipio de Toledo, Brasil. El diseño experimental utilizado fue el de bloques al azar, con cuatro repeticiones. Los tratamientos fueron: T1 - Control (semillas tratadas con insecticida); T2 - Semillas tratadas con insecticidas e inoculadas con Bradyrhizobium; T3 - Semillas no tratadas inoculadas con Bradyrhizobium; T4 - Semillas tratadas con insecticidas y cobalto-molibdeno (CoMo), inoculadas con Bradyrhizobium; T5 - Semillas con CoMo inoculadas con Bradyrhizobium; T6 - Semillas tratadas con insecticidas, inoculadas con Bradyrhizobium y con aplicación de CoMo vía foliar; T7 - Semillas tratadas con insecticidas, inoculadas con Bradyrhizobium y con aplicación de nitrógeno en cubierta; T8 - Semillas tratadas con nitrógeno al voleo. No se observaron diferencias significativas entre los tratamientos sobre el número de nódulos, diametro de la tallo principal, altura de planta, longitud de raiz, masa de 1000 granos y rendimiento. La aplicacion de nitrógeno en la etapa R2 y la asociación del inoculante + CoMo sin tratamiento de semilla proporcionó mayor número de nudos, vainas y granos por planta.

ABSTRACT Nitrogen is the element most demanded by the soybean crop, and the biological fixation of atmospheric nitrogen is the main means to supply it. In contrast, micronutrients and chemical treatments applied on seeds together with the inoculant can alter the phenomenon of biological fixation of atmospheric nitrogen. This work aimed to evaluate the effect of chemical products, micronutrients, and nitrogen fertilization on the nodulation, development, and yield of soybean. The experiment was developed in a field and a greenhouse in the municipality of Toledo, Brazil. A randomized block with four repetitions was used as an experimental design. This design had eight treatments, namely: T1 - Control (seeds treated with insecticide); T2 - Seeds treated with insecticides and inoculated with Bradyrhizobium; T3 - Untreated seeds inoculated with Bradyrhizobium; T4 - Seeds treated with insecticides and cobalt-molybdenum (CoMo), inoculated with Bradyrhizobium; T5 - Seeds with CoMo inoculated with Bradyrhizobium; T6 - Seeds treated with insecticides, inoculated with Bradyrhizobium and with foliar application of CoMo; T7 - Seeds treated with insecticides, inoculated with Bradyrhizobium and with the application of nitrogen in cover; T8 - Seeds treated with nitrogen by broadcast. No significant differences were observed between treatments on the nodules numbers, stem diameter, plant height, root length, the mass of 1000 grains, and yield. The application of nitrogen at the R2 stage (a plant with an open flower in one of the two uppermost nodes of the main stem with a fully developed leaf) and in association with the inoculant + CoMo without seed treatment provided a greater number of nodes, pods, and grains per plant.

Background Since the end of 2020, there has been a great deal of international concern about the variants of SARS-COV-2 B.1.1.7, identified in the United Kingdom; B.1.351 discovered in South Africa and P.1, originating from the Brazilian state of Amazonas. The three variants were associated with an increase in transmissibility and worsening of the epidemiological situation in the places where they expanded. The lineage B.1.1.7 was associated with the increase in case fatality rate in the United Kingdom. There are still no studies on the case fatality rate of the other two variants. The aim of this study was to analyze the mortality profile before and after the emergence of the P.1 strain in the Amazonas state. Methods We analyzed data from the Influenza Epidemiological Surveillance Information System, SIVEP-Gripe (Sistema de Informação de Vigilância Epidemiológica da Gripe), comparing two distinct epidemiological periods: during the peak of the first wave, between April and May 2020, and in January 2021 (the second wave), the month in which the new variant came to predominate. We calculated mortality rates, overall case fatality rate and case fatality rate among hospitalized patients; all rates were calculated by age and gender and 95% confidence intervals (95% CI) were determined. Findings We observed that in the second wave there were a higher incidence and an increase in the proportion of cases of COVID-19 in the younger age groups. There was also an increase in the proportion of women among Severe Acute Respiratory Infection (SARI) cases from 40% (2,709) in the first wave to 47% (2,898) in the second wave and in the proportion of deaths due to COVID-19 between the two periods varying from 34% (1,051) to 47% (1,724), respectively. In addition, the proportion of deaths among people between 20 and 59 years old has increased in both sexes. The case fatality rate among those hospitalized in the population between 20 and 39 years old during the second wave was 2.7 times the rate observed in the first wave (female rate ratio = 2.71; 95% CI: 1.9-3.9], p &lt;0.0001; male rate ratio = 2.70, 95%CI:2.0-3.7), and in the general population the rate ratios were 1.15 (95% CI: 1.1-1.2) in females and 0.78 (95% CI: 0.7-0.8) in males]. Interpretation Based on this prompt analysis of the epidemiological scenario in the Amazonas state, the observed changes in the pattern of mortality due to COVID-19 between age groups and gender simultaneously with the emergence of the P.1 strain suggest changes in the pathogenicity and virulence profile of this new variant. Further studies are needed to better understanding of SARS-CoV-2 variants profile and their impact for the health population.

Introdução Desde o final de 2020 tem havido grande preocupação internacional com as variantes do SARS-COV-2: B.1.1.7, identificada no Reino Unido; B.1.351, descoberta na África do Sul e P.1, que emergiu inicialmente estado brasileiro do Amazonas. As três variantes foram associadas a aumento na transmissibilidade e piora da situação epidemiológica nos locais onde se expandiram. A linhagem B.1.1.7 foi associada ao aumento da taxa de letalidade no Reino Unido. Ainda não existem estudos conclusivos sobre letalidade das outras duas variantes. O objetivo deste estudo foi analisar o perfil de mortalidade antes e depois da emergência da linhagem P.1 no Amazonas. Métodos Analisamos os dados do sistema nacional de vigilância epidemiológica, comparando dois momentos epidemiológicos distintos: durante o pico da primeira onda, entre abril e maio de 2020, e em janeiro de 2021, mês em que a nova variante passou a predominar. Calculamos as taxas de mortalidade, letalidade e letalidade entre pacientes internados, todas as taxas foram calculadas por idade e por sexo e determinados os intervalos de confiança de 95%. Achados Observamos que na segunda onda houve maior incidência e aumento na proporção de casos de COVID-19 nas faixas etárias mais jovens. Observou-se, também, um aumento na proporção de mulheres entre os casos de SARI de 40% (2.709) na primeira onda para 47% (2.898) na segunda onda e entre mortes por COVID-19 de 34% (1,051) para 47% (1.724), respectivamente. Além disso, a proporção de mortes entre 20 e 59 anos aumentou em ambos os sexos. A letalidade entre os hospitalizados na população entre 20 e 39 anos durante a segunda onda foi 2.7 vezes a primeira onda [razão de taxas sexo feminino=2,71; CI(95%)=1,9-3,9], p&lt;0.0001; razão de taxas sexo masculino=2.70(2.0-3.7)), na população geral as razões de taxa foram 1,15(1,1-1,2) no sexo feminino e 0,78(0,7-0,8) no sexo masculino. Interpretação Observamos mudanças no padrão de mortalidade por COVID-19 entre as faixas etárias e sexo simultaneamente à emergência da linhagem P.1, sugerindo mudanças nos perfis de patogenicidade e virulência, novos estudos são necessários para melhor compreensão das variantes do SARS-CoV-2 e suas consequências na saúde da população.

In early 2020, the World Health Organization (WHO) recognized the pandemic situation of the new coronavirus (severe acute respiratory syndrome coronavirus 2, SARS-CoV-2), which causes Coronavirus Disease-2019 (COVID-19). In Brazil by the end of April 2020, another 110 thousand cases and 5,000 deaths had been confirmed. The scarcity of laboratory resources and overload of the care network, added to the broad clinical spectrum of the disease, can make it difficult to capture all mortality from this disease through epidemiological surveillance based on individual notification of cases. The aim of this study was to evaluate the excess of deaths in Brazilian capitals with the highest incidence of COVID-19, as a way of validating the method, we also evaluated a capital with low incidence. We assessed weekly mortality from all causes during the year 2020, up to the epidemiological week 17, compared with the previous year. The data were obtained through the National Civil Registry Information Center (CNIRC, acronym in Portuguese). We estimate the expected mortality and the 95% confidence interval by projecting the observed mortality in 2019 for the population of 2020. In the five capitals with the highest incidences it was possible to identify excess deaths in the pandemic period, the age group most affected were those over 60 years old, 31% of the excess deaths occurred in the population between 20 and 59 years old. There was a strong correlation (r = 0.94) between the excess of deaths in each city and the number of deaths confirmed by epidemiological surveillance. There was no excess of deaths in the capital with the lowest incidence, nor among the population under 20 years old. We estimate that epidemiological surveillance managed to capture only 52% of all mortality associated with the COVID-19 pandemic in the cities studied. Considering the simplicity of the method, its low cost and reliability for assessing the real burden of the disease, we believe that the assessment of excess mortality associated with the COVID-19 pandemic should be widely used as a complementary tool to regular epidemiological surveillance and its use should be encouraged by WHO.

No início de 2020 a Organização Mundial da Saúde (OMS) reconheceu a situação de pandemia do novo coronavírus (severe acute respiratory syndrome coronavirus 2, SARS-CoV-2), causador da Coronavirus Disease-2019 (COVID-19). No Brasil até o final de abril de 2020 já tinham sido confirmados mais 110 mil casos e de 5 mil óbitos. A escassez de recursos laboratoriais e sobrecarga da rede assistencial, somados ao amplo espectro clínico da doença, pode dificultar a captação de toda a mortalidade por esta doença pela vigilância epidemiológica baseada na notificação individual dos casos. O objetivo deste estudo foi avaliar o excesso de mortes nas capitais brasileiras com maiores incidências de COVID-19, como forma de validação do método avaliamos, também, uma capital com baixa incidência. Nós avaliamos a mortalidade semanal por todas as causas durante o ano de 2020, até a semana epidemiológica 17, comparando com o ano anterior. Os dados foram obtidos através da Central Nacional de Informações do Registro Civil (CNIRC). Nós estimamos a mortalidade esperada e o intervalo de confiança de 95% projetando a mortalidade observada em 2019 para a população de 2020. Nas cinco capitais com maiores incidências foi possível identificar excesso de mortes no período da pandemia, a faixa etária mais afetada foram aqueles com mais de 60 anos, 31% do excesso de mortes ocorreu na população entre 20 e 59 anos. Houve uma forte correlação (r=0.94) entre o excesso de mortes em cada cidade e o número de mortes confirmados pela vigilância epidemiológica. Não houve excesso de mortes na capital com mais baixa incidência, nem entre a população com menos de 20 anos. Estimamos que a vigilância epidemiológica conseguiu captar apenas 52% de toda a mortalidade associada à pandemia de COVID-19 nas cidades estudadas. Considerando a simplicidade do método, seu baixo custo e confiabilidade para avaliação da carga real da doença, acreditamos que a avaliação do excesso de mortalidade associado à pandemia de COVID-19 deveria ser amplamente utilizada como ferramenta complementar à vigilância epidemiológica regular e ter seu uso incentivado pela OMS.

Abstract INTRODUCTION: In March 2020, the World Health Organization declared the coronavirus disease (COVID-19) outbreak a pandemic. In Brazil, 110 thousand cases and 5,901 deaths were confirmed by the end of April 2020. The scarcity of laboratory resources, the overload on the service network, and the broad clinical spectrum of the disease make it difficult to document all the deaths due to COVID-19. The aim of this study was to assess the mortality rate in Brazilian capitals with a high incidence of COVID-19. METHODS: We assessed the weekly mortality between epidemiological week 1 and 16 in 2020 and the corresponding period in 2019. We estimated the expected mortality at 95% confidence interval by projecting the mortality in 2019 to the population in 2020, using data from the National Association of Civil Registrars (ARPEN-Brasil). RESULTS: In the five capitals with the highest incidence of COVID-19, we identified excess deaths during the pandemic. The age group above 60 years was severely affected, while 31% of the excess deaths occurred in the age group of 20-59 years. There was a strong correlation (r = 0.94) between excess deaths and the number of deaths confirmed by epidemiological monitoring. The epidemiological surveillance captured only 52% of all mortality associated with the COVID-19 pandemic in the cities examined. CONCLUSIONS: Considering the simplicity of the method and its low cost, we believe that the assessment of excess mortality associated with the COVID-19 pandemic should be used as a complementary tool for regular epidemiological surveillance.

Abstract INTRODUCTION: In 2014, the first cases of autochthonous chikungunya (CHIK) were recorded in Brazil. Lethality associated with this disease is underestimated. Thus, this study aimed to analyze the causes of death among individuals with CHIK in Brazil. METHODS: A descriptive observational study was conducted on individuals with CHIK who died within 6 months from symptom onset. Data pairing between the Information System for Notifiable Diseases and the Mortality Information System was performed. Deaths were classified according to case confirmation criterion, mention of CHIK in the death certificates (DCs), and disease phase. The lethality rate per 1,000 cases was corrected for underreporting and was estimated according to region, sex, age, years of education, race/color, and cause groups. RESULTS: We identified 3,135 deaths (mention of CHIK in the DCs, 764 [24.4%]). In 17.6% of these cases, CHIK was the underlying cause. Most deaths occurred in the acute (38.1%) and post-acute (29.6%) phases. The corrected LR (5.7; x1,000) was 6.8 times higher than that obtained from the Information System for Notifiable Diseases (0.8). The highest corrected LRs were estimated for among individuals living in the Northeast region (6.2), men (7.4), those with low years of education and those aged <1 year (8.6), 65-79 years (20.7), and ≥80 years (75.4). CONCLUSIONS: The LR of CHIK estimates based on information system linkage help to reveal the relevance of this disease as the direct cause or as a cause associated with serious or fatal events, provide timely interventions, and increase the knowledge about this disease.

ABSTRACT Several arboviruses have emerged and/or re-emerged in North, Central and South-American countries. Viruses from some regions of Africa and Asia, such as the Zika and Chikungunya virus have been introduced in new continents causing major public health problems. The aim of this study was to investigate the presence of RNA from Zika, Dengue and Chikungunya viruses in symptomatic patients from Rondonia, where the epidemiological profile is still little known, by one-step real-time RT-PCR. The main clinical signs and symtoms were fever (51.2%), headache (78%), chills (6.1%), pruritus (12.2%), exanthema (20.1%), arthralgia (35.3%), myalgia (26.8%) and retro-orbital pain (19.5%). Serum from 164 symptomatic patients were collected and tested for RNA of Zika, Dengue types 1 to 4 and Chikungunya viruses, in addition to antibodies against Dengue NS1 antigen. Direct microscopy for Malaria was also performed. Only ZIKV RNA was detected in 4.3% of the patients, and in the remaining 95.7% of the patients RNA for Zika, Dengue and Chikungunya viruses were not detected. This finding is intriguing as the region has been endemic for Dengue for a long time and more recently for Chikungunya virus as well. The results indicated that medical and molecular parameters obtained were suitable to describe the first report of symptomatic Zika infections in this region. Furthermore, the low rate of detection, compared to clinical signs and symptoms as the solely diagnosis criteria, suggests that molecular assays for detection of viruses or other pathogens that cause similar symptoms should be used and the corresponding diseases could be included in the compulsory notification list.

RESUMO: O presente estudo objetivou determinar a prevalência e distribuição de lesões abscedativas, identificação do agente etiológico e avaliação das lesões histológicas em caprinos e ovinos abatidos em um matadouro-frigorífico com Serviço de Inspeção Federal do estado da Bahia. Foram coletadas 153 amostras de vísceras e linfonodos com abscessos de 1.148 animais abatidos. A maior prevalência na espécie ovina foi em macho, com faixa etária de 12 meses, sendo os principais órgãos acometidos fígado (21,2%) e linfonodo pré-escapular (20,3%). Na espécie caprina, a prevalência maior foi em macho, com faixa etária de 30 meses, sendo os linfonodos retro faríngeo (25%) e pré-escapular os mais acometidos (25%). Isolou-se os seguintes micro-organismos das amostras: Corynebacterium pseudotuberculosis em 33,33%, Escherichia coli (19,61%), Proteus mirabilis (9,80%), Pseudomonas aeruginosa (7,19%), Trueperella pyogenes (5,22%), Streptococcusspp. (5,22%) e Staphylococcus aureus (4,57%). As lesões macroscópicas e histológicas dos abscessos coletados não apresentaram diferenças entre micro-organismos isolados.

ABSTRACT: The study aimed to determine the prevalence and distribution of abscessed lesions, etiologic agent identification and assessment of histological lesions in sheep and goats slaughtered in a slaughter plant refrigerator with Federal Inspection Service in the State of Bahia. The amount of 153 samples of viscera and lymph nodes with abscesses of 1.148 slaughtered animals were collected. The highest prevalence in sheep was in males, aged 12 months, as in liver (21.2%) and prescapular lymph nodes (20.3%) the main affected organs. The prevalence in goats in male, aged 30 months and in retropharyngeal (25%) and prescapular lymph nodes (25%). The following microorganisms were isolated from the samples: Corynebacterium pseudotuberculosis 33.33%, Escherichia coli 19.61%, Proteus mirabilis 9.80%, Pseudomonas aeruginosa 7.19%, Trueperella pyogenes 5.22%, Streptococcus spp. 5.22% and Staphylococcus aureus 4.57%. The macroscopic and histological lesions of abscesses collected presented no difference between isolated microorganisms.

Abstract Participatory methodologies have been appropriated by researchers in the field of environmental health around the world. Action Research presents itself as an innovative participatory method that allows the use of multiple techniques of data collection and analysis. The purpose of this article is to reflect on the use of Action Research as a method that enables the conduction of environmental education practices by nursing based on Freirean pedagogy. Initially, there is a parallel between the role of nursing in the health care of the planet and the changes in the conception of health over the years, highlighting the role of science in this process. Subsequently, it is discussed the Action Research and its applicability in educational-environmental studies relating points of convergence between this method and Freirean pedagogy. Historical transformations in the relationship between man and the environment raise the need for the training of environmentally committed and qualified nurses in developing care-educational practices that achieve environmental care at a planetary level.

Resumen Las metodologías participativas vienen siendo incorporadas por investigadores en el campo de la salud ambiental en todo el mundo. La investigación-acción se presenta como un método participativo innovador que posibilita el uso de múltiples técnicas de recolección y análisis de datos. El objetivo de este artículo es reflexionar sobre el uso de la Investigación-Acción como método que viabilice la conducción de prácticas de educación ambiental por la enfermería pautadas en la pedagogía freireana. Inicialmente, se traza un paralelo entre el papel de la enfermería en el cuidado de la salud del planeta y las modificaciones en la concepción de salud a lo largo de los años, subrayando el papel de la ciencia en ese proceso. Posteriormente, se discurre sobre la Investigación-Acción y su aplicabilidad en los estudios educativo-ambientales, relacionando puntos de convergencia entre ese método y la pedagogía freireana. Las transformaciones históricas en la relación hombre/ambiente suscita la necesidad de formación de profesionales enfermeros ambientalmente comprometidos y calificados en desarrollar prácticas de cuidado-educativas que alcancen el cuidado ambiental a nivel planetario.

Did death occur DUE TO dengue, or in a patient WITH dengue virus infection? It seems a matter of semantics, but in fact, it underscores how challenging it is to distinguish whether the disease contributed to death, or was itself the underlying cause of death. Can a death be attributed to chikungunya virus, when some deaths occur after the acute phase? Did the virus decompensate the underlying diseases, leading to death? Did prolonged hospitalisation lead to infection, resulting in the patient’s progression to death? Were there iatrogenic complications during patient care? The dengue question, for which there has not yet been a definitive response, resurfaces prominently under the chikungunya surveillance scenario. We are facing an epidemic of a disease that seems to be more lethal than previously thought. The major challenge ahead is to investigate deaths suspected of occurring due to arbovirus infections and to understand the role of each infection in the unfavourable outcome.
