Abstract Background: Hirschsprung’s Disease (HD) is characterized by intestinal sub-occlusion and the absence of enteric ganglion cells. A rectal biopsy examination is performed to confirm the diagnosis. In a recent study, we demonstrated that the analysis of 60 sections of rectal mucosa and submucosa stained by H&E may ensure a 90% diagnostic accuracy. Although the need to analyze so many sections makes the process of reading the slides more time-consuming, this encouraged us to study their distribution in the healthy rectal submucosa, to simplify the diagnosis. Objectives: To develop a method that facilitates HD diagnosis by studying the distribution of ganglion cells in the submucosal plexus. Methods: Using the calretinin technique, we studied the distribution of plexuses in 60 fragments of rectal submucosa from 19 cadavers. After the study, the reading method created was used for diagnosis in 47 cases of suspected HD, using H&E staining. The accuracy was verified by comparing the results obtained with H&E to those obtained with the acetylcholinesterase technique, the golden standard in our laboratory. Results: The study of submucosal plexus distribution showed that just by examining the submucosal region every 20 µm, approximately, it is possible to locate a ganglionic plexus, and we have already been able to diagnose HD with 93% accuracy. Conclusion: The study of ganglion cell distribution enabled the creation of a simplified method for reading the slides. The method applied achieved good accuracy and it can be used as an alternative method in HD diagnosis.

OBJETIVO: identificar as nomenclaturas diagnósticas dos exames citopatológicos cervicais utilizadas pelos laboratórios que atendem o Sistema Único de Saúde (SUS) e participantes do Monitoramento Externo de Qualidade (MEQ). Avaliar as informações adquiridas de profissionais ginecologistas que atuam no SUS sobre os tipos de classificação diagnóstica que recebem nos laudos citopatológicos cervicais. MÉTODOS: foram avaliados 94 laudos citopatológicos liberados pelos laboratórios participantes do MEQ no Estado de São Paulo e 126 questionários aplicados aos ginecologistas que atenderam o SUS. RESULTADOS: dos 94 laboratórios, 81 (86,2%) utilizam uma única nomenclatura diagnóstica: 79 (97,6%) utilizam a Nomenclatura Brasileira para Laudos Citopatológicos (NBLC), 1 (1,2%) utiliza a classificação de Papanicolaou e 1 (1,2%) utiliza a de Richart. Dos 13 (13,8%) laboratórios que utilizam mais de uma nomenclatura, 5 apresentam 2 tipos, e 8, de 3 a 4, 9 dos quais incluem a classificação de Papanicolaou. O estudo demonstrou que 52 (55,3%) laboratórios apresentaram mais de um diagnóstico descritivo num mesmo laudo. Dos 126 ginecologistas que responderam ao questionário de avaliação dos laudos citopatológicos, 78 (61,9%) disseram receber laudos dos laboratórios com apenas uma classificação diagnóstica, 48 (38,1%), laudos com mais de uma classificação, e 2 receberam as 4 classificações. Entre os 93 (73,8%) ginecologistas que preferem uma classificação, 56 (60,2%) alegaram que a NBLC contribui para a conduta clínica, 13 (14,0%) optaram pela nomenclatura de Richart, 8 (8,6%), de Reagan e 16 (17,2%), a de Papanicolaou. De 33 (26,2%) ginecologistas que preferem mais de uma nomenclatura, 5 optaram pelas 4 classificações. CONCLUSÕES: Os dados obtidos mostram que ainda há resistência por parte dos patologistas no emprego da nomenclatura oficial nos laudos citológicos do SUS. Há uma discrepância entre informações que os ginecologistas gostariam que estivessem nos laudos e as que são fornecidas pelos médicos patologistas. Maiores esforços devem ser empregados no sentido de estimular o uso da nomenclatura oficial.

PURPOSE: to identify the nomenclature for reporting cervical cytological diagnoses used by laboratories which render services to the Brazilian Unified Health System (SUS) and which participate in External Quality Monitoring (MEQ). To evaluate the information acquired from gynecologists of the SUS regarding the various diagnostic classifications that they receive in the cervical cytology diagnostic reports. METHODS: we evaluated 94 cytology reports issued by laboratories which participate in the MEQ in the State of São Paulo, Brazil, and 126 questionnaires applied to gynecologists who work for the SUS. RESULTS: out of the 94 laboratories, 81 (86.2%) use one diagnostic classification: 79 (97.6%) use the Brazilian Nomenclature for Cytological Reports (NBLC), 1 (1.2%) uses the Papanicolaou classification and 1 (1.2%) uses the Richart diagnostic classification. Of the 13 (13.8%) laboratories that use more than one classification, 5 use 2 types and 8 use 3 to 4 types, with 9 including the Papanicolau diagnostic classification. The study showed that 52 (55.3%) laboratories presented more than one descriptive diagnosis in the same report. Out of the 126 gynecologists who filled out a questionnaire evaluating the cytopathology reports, 78 (61.9 %) stated that they received laboratory reports with only one diagnostic classification, 48 (38.1%) received reports with more than one classification and 2 received reports with all 4 classifications. Among the 93 (73.8%) gynecologists who prefer only one classification, 56 (60.2%) claimed that the NBLC contributes to clinical practice, 13 (14.0%) opted for the Richart classification, 8 (8.6%) for the Reagan classification and 16 (17.2%) for the Papanicolaou classification. Out of 33 (26.2%) gynecologists who prefer more than one classification, 5 opted for the 4 classifications. CONCLUSIONS: these data suggest that there is still resistance on the part of pathologists about using the official nomenclature in cytology reports for SUS. There is discrepancy between the information that gynecologists would like to see in the reports and the information provided by the pathologists. Greater efforts should be made to stimulate the use of the official nomenclature.

INTRODUÇÃO: A exposição da tireóide à irradiação está associada à alteração do componente cromossômico nessa glândula. OBJETIVO: Avaliar o conteúdo de DNA (ploidia) em glândula tireóide submetida à radiação. MATERIAL E MÉTODOS: Foram irradiadas tireóides de 75 ratos; 25 foram usados como grupo controle e não sofreram irradiação. A exposição à irradiação foi realizada com o uso de Cobalt-60 na região cervical anterior direita com espaço de 3-30cm, com anel traqueal de 2º e 6º e dose de 600 centigrays (cGY). O conteúdo de DNA (ploidia) foi obtido com o método de Feulgen-Thionin e quantificado com o aparelho CAS 200 de citometria estática. RESULTADO: Dos 92 casos, 88 (95,7%) foram diplóides, independente do tempo de exposição; aneuploidia foi observada em quatro casos (4,3%), somente no grupo sacrificado aos 33 dias. Oito casos foram excluídos por problemas técnicos. CONCLUSÃO: A irradiação afeta muito precocemente o conteúdo de DNA nuclear levando a aneuploidia.

INTRODUCTION: Thyroid gland exposures to radiation induce nuclear chromosomal alteration. Objective: To evaluate the DNA content of thyroid gland submitted to radiation. MATERIALS AND METHODS: We radiated 75 rats while 25 were not radiated to be used as control group. Exposure was conducted by the use of Cobalt-60 radioactive source in the right anterior cervical region in a field of 3-30cm, comprising the second and the sixth tracheal rings with 600-centigray (cGY) doses. The DNA content (ploidy) was obtained with Feulgen-thionin stain and was quantified with CAS 200 quantitative measurement equipment. RESULTS: Diploid pattern was obtained in 88 cases (95.7%), independently of time of exposure: on the other hand, aneuploidy was observed in four cases (4.3%) only in the group sacrificed to the 33 days. Eight cases were excluded due to technical reasons. CONCLUSION: The early aneuploid pattern found in our study certainly corroborated that radiation affects thyroid gland with important consequences in terms of disorders.

Analisar criticamente os primeiros resultados do Sistema de Informação Laboratorial do Programa Nacional de Combate ao Câncer de Colo Uterino (SISCOLO-Qualidade). Foram utilizados esfregaços cervicovaginais corados pelo método de Papanicolaou, provenientes da rotina diagnóstica do Laboratório de Anatomia Patológica da Fundação Oncocentro de São Paulo (FOSP) e referentes a março de 2000. Esses casos foram reavaliados em estudo cego pela equipe do Setor de Citologia Oncótica da Divisão de Patologia do Instituto Adolfo Lutz/São Paulo. O Programa SISCOLO-Qualidade seleciona, entre os casos analisados no período, todas as citologias positivas (ASCUS, AGUS, NIC e carcinomas) e, para completar 10% do total das lâminas, todos os casos insatisfatórios e negativos aleatoriamente. Dos 8.370 casos da rotina da FOSP, o SISCOLO-Qualidade selecionou 837 amostras. Entre as amostras selecionadas, 23,4% apresentaram material representativo da junção escamocolunar, 5% foram considerados material insatisfatório e 71,1%, material satisfatório, mas limitado. O resultado da análise foi 86,62% de concordância, 12,42% de discordância de um grau e 0,96% de discordância de mais de um grau. Concluímos que o Programa SISCOLO-Qualidade é aplicável na rede de saúde pública para monitoramento externo, atendendo às expectativas de qualidade exigidas pelo Ministério da Saúde.

The aim of the present work was to analyses critically the preliminary results of the computerized Quality System of gynecologycal cytology, implanted by the Public Health Ministry of Brazil - SISCOLO. Cervicovaginal smears stained by the Papanicolaou method were utilized. These smears were obtained through the diagnostic routine undertaken at the Oncocentro Foundation Pathology Laboratory of pathology, comprising cases of March 2000. Such cases were rescreened in a blind study, by the staff of the Pathology Division of the SP Adolpho Lutz Institute's Cytology Section. The SISCOLO-Quality Program selects, among all cases analyzed in the period, all such positive cases (ASCUS, AGUS, CIN and carcinomas) and to complete 10% of all cases it selects all non-satisfying and negative cases at random. From the 8,370 cases selected in March 2000, the SISCOLO-Quality program selected 837 samples. Among the selected samples, 23,4% showed squamo-columnar junction material, 5% were considered non-satisfactory for diagnosis and 71.1%, satisfactory although limited. The rescreening analysis showed concordance in 86.62% of the cases, one grade discordance was observed in 12.42% and two or more grades discordance in 0.96% of the cases. We concluded that the SISCOLO-Quality Program has applicability in the health public system as external monitoring method and meets the Health Ministry's expectations.

Entre as estratégias de controle de qualidade, a revisão de exames negativos anteriores em pacientes com exame atual positivo é considerada uma das mais eficientes para detecção de falsos negativos. No presente estudo foram revistos os esfregaços prévios negativos de pacientes com NIC 2 ou NIC 3 nos anos de 1998 e 1999. Durante este período o Instituto Adolfo Lutz realizou 113.619 exames colpocitológicos, sendo 233 casos diagnosticados como NIC 2 ou NIC 3. Entre estes casos apenas 36 tinham exames prévios no Instituto Adolfo Lutz no período de 36 meses antes do diagnóstico original. Obtivemos 22 casos de NIC (12 NIC 1, sete NIC 2 e três NIC 3) e 14 casos com diagnóstico negativo (normal ou inflamatório), os quais pertenciam a 12 pacientes. Após revisão destes esfregaços obtivemos os seguintes resultados: insatisfatório (dois casos), inflamatório (cinco casos), NIC 1 (três casos), NIC 2 (um caso). Concluindo, a revisão dos exames anteriores negativos é uma estratégia altamente eficiente para detectar os resultados falsos negativos. Sua prática pode ser útil para orientar diversas medidas de educação continuada.

Within the strategies of internal quality control, the review of previous negative Papanicolaou smears from patients with diagnosis of CIN 2 or CIN 3 is considered one of the most efficient for detection of false negative results. In the present study we reviewed the previous negative smears of cases with diagnosis of CIN 2 or CIN 3 during the years of 1998 and 1999. During this period the Adolfo Lutz Institute has performed 113.619 Papanicolaou tests; 233 cases were diagnosed as CIN 2 or CIN 3. The cases diagnosed as unsatisfactory were excluded. Among the 36 cases from which there were previous smears in the period of 36 months before the original diagnosis, we found 22 cases of CIN (12 CIN 1, seven CIN 2 and three CIN 3) and 14 cases diagnosed as inflammatory or normal. When those 14 smears were reviewed, the following diagnosis were found: unsatisfactory (two cases), inflammatory (five cases), Ascus (three cases), CIN 1 (three cases), CIN 2 (one case). In conclusion, the review of previous negative Papanicolaou smears is a highly efficient strategy in order to detect false negative results. This practice can be useful as a guide for continued education measures.

OBJETIVO: Os carcinomas epidermóides do trato aerodigestivo superior são tumores de comportamento biológico heterogêneo. O objetivo deste trabalho é verificar se a expressão imuno-histoquímica dos marcadores Ki67, PCNA e P53 apresenta correlações com parâmetros prognósticos clínico-patológicos. MÉTODOS: Determinação da expressão imuno-histoquímica dos antígenos Ki67, PCNA e P53 em espécimes tumorais fixados e embebidos em parafina de 53 pacientes com carcinoma epidermóide em diferentes sítios primários do trato aerodigestivo superior. RESULTADOS: Os marcadores tiveram altos índices de expressão imuno-histoquímica, sendo 46,5% para o Ki67, 66,5% para o PCNA e 36,5% para o P53. Não houve correlação da expressão do Ki67 e do PCNA com o estadiamento TNM (AJCC), nem com o grau de malignidade. A expressão do Ki67 apresentou correlação positiva com a expressão do PCNA (p = 0,037). O mesmo aconteceu para o PCNA e o número de mitoses por campo (p = 0,001). CONCLUSÕES: De acordo com estes resultados, concluiu-se que a determinação da imunorreatividade dos marcadores Ki67 e PCNA é um método objetivo e quantificável para avaliar proliferação celular que pode subsidiar as informações prognósticas.

BACKGROUND: Squamous cell carcinomas of upper aerodigestive tract are tumors with heterogeneous biological behaviour. The aim of present study is to assess whether any correlation exists between immunohistochemical expression of Ki67, PCNA and P53 markers and clinicopathologic parameters. METHODS: Immunohistochemical determination of Ki67, PCNA and P53 antigens in fixated and paraffin-embedded tumour especimens from 53 patients with squamous cell carcinoma in different primary locations of upper aerodigestive tract. RESULTS: The markers had high evidence average rates with 46,5% for Ki67, 66,5% for PCNA and 36,5% for P53. There was neither correlation of Ki67 and PCNA evidence nor with TNM staging nor with malignancy degree. The expression of Ki67 demonstrated correlation with the expression of PCNA (p = 0,037). The same ocurred to PCNA and the number of mitoses (p = 0,001). CONCLUSIONS: According to these results, it has been concluded that the determination of immunoreactivity of Ki67 and PCNA proliferation markers is an accurate and quantitative method which may provide prognostic information.

A nocardiose do sistema nervoso central tem alcançado importante destaque no diagnóstico diferencial de patologias tumorais nos últimos anos pelo aumento da incidência de pacientes imunodeficiêntes(pacientes submetidos a quimioterapia, transplante de órgãos ou com a síndrome de imunodeficiência adquirida - SIDA). Os autores descrevem o caso de um paciente com SIDA que desenvolveu nocardiose cerebelar na sua forma tumoral. O paciente foi submetido a craniectomia de fossa posterior para diagnóstico e tratamento. Os achados de neuroimagem e patológicos bem como a conduta cirúrgica são discutidos com base em revisão da literatura.

The authors describe a case of cerebellar abscess by Nocardiain a patient with the acquired immunodeficiency syndrome (AIDS) that was submitted to a posterior fossa craniectomy for diagnosis and treatment. Pathological and neuroimage findings are discussed as well as the surgical approach taking into account literature data on the subject.
