Os transtornos do sono são comuns nos pacientes com doença de Alzheimer e interferem na qualidade de vida do paciente e de seu cuidador. Apesar da alta prevalência desses transtornos, existe pouca evidência em relação ao seu tratamento. Nosso objetivo foi revisar a literatura em relação ao tratamento não farmacológico e farmacológico dos transtornos do sono nos idosos com doença de Alzheimer em comunidade. Os tratamentos incluídos consistiram na higiene do sono e/ou no uso da luz intensa, combinados ou não com o uso da melatonina, nos inibidores de acetilcolinesterases, antipsicóticos, hipnóticos ou antidepressivos. Para além das medidas não farmacológicas, há evidência de que o uso da trazodona é efetivo no tratamento dos transtornos do sono de pacientes com doença de Alzheimer. Mais estudos sobre as estratégias farmacológicas e não farmacológicas aqui revisadas ou outras são desejáveis.

Sleep disorders are common in patients with Alzheimer dementia and affect the quality of life of patients and of their caregivers. Despite the rising number of studies in the area, almost all of them are about non-pharmacological treatment. Our objective was to review the literature concerning pharmacological and non-pharmacological approaches to treat sleep disorders of elderly patients with Alzheimer dementia in the ambulatory setting. The treatments revised consisted of sleep hygiene and/or use of intense light coupled or not with use of melatonin, cholinesterase inhibitors, antipsychotics, hypnotics or antidepressants. In addition to the non-pharmacological measures, there is evidence that the use of trazodone may aid the treatment of sleep disorders of older individuals with Alzheimer dementia. More studies are necessary to examine the non-pharmacological and pharmacological treatments revised herein.

CONTEXTO E OBJETIVO: O câncer do colo uterino é uma das mais freqüentes neoplasias na mulher. O exame de Papanicolaou é o método mais comum e econômico para rastreamento. As células esfoliativas epiteliais podem ser úteis para o monitoramento de pacientes expostas a fatores de risco para o câncer. O objetivo foi analisar a prevalência de micronúcleos em células esfoliativas da mucosa cervical uterina e associar com fatores de risco para o câncer de colo uterino. TIPO DE ESTUDO E LOCAL: Estudo transversal analítico, no Instituto de Pesquisa em Oncologia (IPON). MÉTODOS: Células esfoliativas do colo uterino foram obtidas de 101 pacientes ambulatoriais entre setembro/2004 e novembro/2005. As células foram coletadas usando espátula de Ayre e transferidas para um tubo de ensaio com soro fisiológico 0,9% para o teste do micronúcleo. Informações obtidas das pacientes foram: idade, hábitos (fumo e número de parceiros sexuais), métodos contraceptivos, história de doença sexualmente transmissível e uso de terapia hormonal. Células foram analisadas com magnificação de 1000 X e os micronúcleos contados em 1.000 células epiteliais por paciente. RESULTADOS: A comparação do grupo de pacientes fumantes ativas (7,9 ± 7,8) e passivas (7,2 ± 10,6) versus não fumantes (3,7 ± 5,1); alcoolismo e não alcoolismo (7,8 ± 1,4 e 6,9 ± 10,1); citologia inflamatória e citologia normal (10,7 ± 10,5 e 1,3 ± 1,7); neoplasia intraepitelial cervical (NIC) I, II e III e a ausência de NIC, respectivamente, (4,3 ± 4,3; 10,6 ± 5,3; 22,7 ± 11,9 e 1.3 ± 1.4) mostrou maior prevalência de micronúcleos (P < 0,05). CONCLUSÕES: A prevalência de micronúcleo nas células esfoliativas do colo uterino foi maior no grupo de pacientes com pelo menos um dos fatores de risco para câncer do colo uterino do que no grupo controle (sem fatores de risco).

CONTEXT AND OBJECTIVE: Pap smears are the most common and inexpensive screening method for cervical cancer. We analyzed micronucleus prevalence in exfoliated cervical mucosa cells, to investigate associations between increased numbers of micronuclei and risk factors for cervical cancer. DESIGN AND SETTING: Analytical cross-sectional study, at Instituto de Pesquisa em Oncologia (IPON). METHODS: Exfoliated cervical cells were obtained from 101 patients between September 2004 and November 2005. Patients' ages, habits (passive or active smoking, alcoholism and numbers of sexual partners), age at first sexual intercourse, contraceptive methods used, histories of sexually transmitted diseases, use of hormone replacement therapy, numbers of pregnancies and abortions, inflammatory cytology and cervical intraepithelial neoplasia (CIN) were obtained. Cells were collected using Ayre spatulas, transferred to vials containing 0.9% saline solution for micronucleus tests and analyzed at 1000x magnification. The number of micronuclei in 1,000 epithelial cells per patient sample was counted. RESULTS: Comparisons between groups with active (7.9 ± 7.8) and passive (7.2 ± 10.6) smoking versus no smoking (3.7 ± 5.1); with/without alcoholism (7.8 ± 1.4 and 6.9 ± 10.1); with/without inflammatory cytology (10.7 ± 10.5 and 1.3 ± 1.7); and with CIN I, II and III and no CIN (respectively 4.3 ± 4.3, 10.6 ± 5.3, 22.7 ± 11.9 and 1.3 ± 1.4) found elevated micronucleus prevalence (P < 0.05). CONCLUSIONS: We concluded that the prevalence of micronuclei in exfoliated uterine cervical cells was greater in patients with one or more risk factors for uterine cervical cancer than in patients without risk factors.

The effects of H2O2, Fe2+ and Fe3+ on curcumin-induced clastogenicity were evaluated in CHO cells. Curcumin combined with H2O2 did not increase the chromosomal aberrations more than expected based on a simple additive effect. In contrast, the combination of curcumin-Fe significantly decreased the total number of chromosomal aberrations and the number of abnormal metaphases. The clastogenicity of curcumin may be related to its pro-oxidant properties and its ability to generate free radicals.

Antioxidantes de ocorrência natural têm sido exaustivamente estudados quanto a sua capacidade de proteger organimos e células contra danos oxidativos. Muitos constituintes das plantas, incluindo cúrcuma e curcumina, parecem ser potentes antimutágenos e antioxidantes. Os efeitos de cúrcuma e curcumina na freqüência de aberrações cromossômicas induzidas pelo agente radiomimético bleomicina (BLM) foram investigados em células do ovário de hamster chinês (CHO). Três concentrações de cada droga, cúrcuma (100, 250 e 500 <FONT FACE="Symbol">m</FONT>g/ml) e curcumina (2,5, 5,0 e 10 <FONT FACE="Symbol">m</FONT>g/ml), foram combinadas com BLM (10 <FONT FACE="Symbol">m</FONT>g/ml) em células CHO tratadas durante as fases G1/S, S ou G2/S do ciclo celular. Nem cúrcuma nem curcumina evitaram o dano cromossômico induzido pela BLM em fase alguma do ciclo celular. Ao contrário, a potenciação da clastogenicidade da BLM pelo curcumina foi nitidamente observada em células tratadas durante as fases S e G2/S. A curcumina também se mostrou clastogênica na dose de 10 <FONT FACE="Symbol">m</FONT>g/ml nos protocolos de tratamento de 9 e 13 h. Contudo, o mecanismo exato pelo qual a curcumina produziu efeitos potenciadores e clastogênicos permanece desconhecido.

Naturally occurring antioxidants have been extensively studied for their capacity to protect organisms and cells from oxidative damage. Many plant constituents including turmeric and curcumin appear to be potent antimutagens and antioxidants. The effects of turmeric and curcumin on chromosomal aberration frequencies induced by the radiomimetic agent bleomycin (BLM) were investigated in Chinese hamster ovary (CHO) cells. Three concentrations of each drug, turmeric (100, 250 and 500 <FONT FACE="Symbol">m</FONT>g/ml) and curcumin (2.5, 5 and 10 <FONT FACE="Symbol">m</FONT>g/ml), were combined with BLM (10 <FONT FACE="Symbol">m</FONT>g/ml) in CHO cells treated during the G1/S, S or G2/S phases of the cell cycle. Neither turmeric nor curcumin prevented BLM-induced chromosomal damage in any phases of the cell cycle. Conversely, a potentiation of the clastogenicity of BLM by curcumin was clearly observed in cells treated during the S and G2/S phases. Curcumin was also clastogenic by itself at 10 µg/ml in two protocols used. However, the exact mechanism by which curcumin produced clastogenic and potentiating effects remains unknown.

A associação de radiação ionizante com drogas antineoplásicas tem importante aplicação na terapia do câncer. No presente trabalho, foram avaliados os efeitos de dois inibidores de topoisomerase II, doxorubicina (DXR) e mitoxantrona (MXN), sobre as aberrações cromossômicas induzidas pelas radiações-g em células do ovário de hamster chinês (CHO). Foram usadas as concentrações 0,5 e 1,0 mg/ml de DXR e 0,02 e 0,04 mg/ml de MXN, combinadas com duas doses de radiações gama (20 e 40 cGy). Um significativo efeito potenciador das aberrações cromossômicas foi observado em células CHO tratadas com 1,0 mg/ml de DXR e expostas a 40 cGy de radiação. Nos outros testes, a combinação da radiação-g com a DXR ou MXN apresentou um efeito próximo ao aditivo. A redução dos índices mitóticos refletiu a alta citotoxicidade das drogas quando combinadas às radiações-g.

Combined radiation and antineoplastic drug treatment have important applications in cancer therapy. In the present work, an evaluation was made of two known topoisomerase II inhibitors, doxorubicin (DXR) and mitoxantrone (MXN), with g-radiation. The effects of DXR or MXN on g-radiation-induced chromosome aberrations in Chinese hamster ovary (CHO) cells were analyzed. Two concentrations of each drug, 0.5 and 1.0 µg/ml DXR, and 0.02 and 0.04 µg/ml MXN, were applied in combination with two doses of g-radiation (20 and 40 cGy). A significant potentiating effect on chromosomal aberrations was observed in CHO cells exposed to 1.0 µg/ml DXR plus 40 cGy. In the other tests, the combination of g-radiation with DXR or MXN gave approximately additive effects. Reduced mitotic indices reflected higher toxicity of the drugs when combined with radiation.

A terapia combinada com radiação e agentes químicos tem aumentado nos regimes de tratamento do câncer. Nesse estudo foi investigado o efeito das combinações de taxol (0,08 mig/ml) com a doxorubicina (DXR 0,5 ou 1,0 mig/ml) ou com as radiações gama (20 ou 40 cGy) em dois diferentes protocolos de tratamento (pré-tratamento e simultâneo) em células CHO tratadas na fase G2 do ciclo celular. Os resultados mostraram que o taxol não teve efeito sensibilizador sobre as aberrações cromossômicas induzidas pelas radiações gama, nem teve efeito potenciador sobre as aberrações cromossômicas induzidas pela DXR em células CHO tratadas na fase G2 do ciclo celular

Combined therapy with radiation and chemotherapy has being increasingly used in cancer treatment. The effect of combinations of taxol (0.08 mug/ml) with doxorubicin (DXR, 0.5 or 1.0 mug/ml) or gamma radiation (20 or 40 cGy) was examined in two different treatment schedules (pretreatment or simultaneous treatment) using Chinese hamster ovary (CHO) cells treated at the G2 phase of the cell cycle. The results showed that taxol did not have a radiosensitizing effect on the chromosomal aberrations induced by gamma radiation nor did it have a potentiating effect on the chromosomal aberrations induced by DXR in CHO cells treated in the G2 phase of the cell cycle
