Objetivo: estudar a associação entre tipo de aleitamento, consumo de fibra alimentar e ocorrência de constipação em lactentes. Métodos: foram estudados 275 lactentes atendidos consecutivamente em unidade básica de saúde. O tipo de aleitamento foi classificado como predominante, misto e artificial. Constipação foi caracterizada pela eliminação de fezes duras, associada a uma das seguintes características: dor ou dificuldade ao evacuar, fezes em cíbalos ou cilíndrica, com rachaduras, e intervalo entre as evacuações maior ou igual a 3 dias. Pseudoconstipação foi caracterizada quando ocorria a eliminação de fezes amolecidas, sem dor ou dificuldade, em intervalo maior ou igual a 3 dias. Resultados: constipação foi observada em 25,1% dos lactentes. Pseudoconstipação ocorreu exclusivamente nos primeiros 6 meses de vida, em 5,1% dos 159 lactentes desta faixa etária. Constipação entre os 6 e 24 meses (38,8%; 45/116) foi mais freqüente do que no primeiro semestre de vida (15,1%, p=0,0000). A análise de regressão logística evidenciou que, no primeiro semestre, os lactentes em aleitamento artificial demonstram chance 4,5 vezes maior de apresentar constipação do que os em aleitamento predominante. Entre os de 6 a 24 meses, a estimativa de consumo de fibra alimentar (gramas/dia) foi semelhante (p=0,57) nos lactentes com constipação (mediana=9,0g; percentis 25 e 75: 6,9-13,1g) e naqueles com hábito intestinal normal (mediana=8,8; percentis 25 e 75: 6,1-12,9g). Conclusões: o consumo de fibra alimentar por crianças menores de dois anos foi semelhante entre os grupos com e sem constipação intestinal. O aleitamento natural predominante é fator de proteção contra constipação no primeiro semestre de vida.

Objective: To study the relationship between breast-feeding, dietary fiber intake and constipation in infants. Methods: The study population consisted of 275 infants consecutively enrolled in two Primary Care Clinic in the city of Embu, in the Great São Paulo. The feeding pattern were classified in predominantly breast-feeding, partially breast and cow’s milk feeding and artificial feeding. Constipation was defined by the elimination of hard stool associated with one of the following: painful or difficult defecation, hard or round cracked stools and less than three defecations a week. False constipation was defined by the elimination of soft stools without pain or difficulty but with less than three defecations a week. Results: Constipation was found in 25.1% (69/275). False constipation was found only in the first semester of life in 5.1% of 159 infants. The prevalence of constipation was higher between 6 and 24 months (38.8%, 45/116) than in the first semester of life (15.1%, p=0.000). A model of logistic regression demonstrated that infants under artificial feeding were 4.53 times more liable to develop constipation than infants who were predominantly breastfed. The daily dietary fiber intake (g/day) was similar (p=0.57) among the constipated (median=9.0 g; 25th and 75th percentiles: 6.9-13.1g) and non-constipated (median = 8.8 g; 25th and 75th percentiles: 6.1-12.9 g). Conclusions: Dietary fiber intake was similar in constipated and non-constipated infants. Breast-feeding serves as a protection factor against the development of constipation in the first semester of life.

A anemia na infância tem sido considerada um dos grandes desafios nutricionais nos países em desenvolvimento, seguida da desnutrição energética. Têm-se discutido muito a eficácia dos alimentos enriquecidos no combate a esse problema. O objetivo desse trabalho foi avaliar a aceitação diária de cereal adicionado de ferro e o seu impacto nos parâmetros antropométricos e nos níveis de hemoglobina de crianças de 1 a 4 anos de idade. Foram estudadas 54 crianças de uma creche municipal, de duas salas, sendo o grupo I constituído de 24 crianças com média de idade 2 anos e 2 meses e o grupo II constituído de 30 crianças de 3 anos e 6 meses. Essas crianças foram avaliadas, no início e no final da intervenção, quanto à ingestão alimentar, peso, estatura e níveis de hemoglobina sérica. Durante o período de dois meses, o café da manhã e o lanche da tarde, que eram constituídos de leite e pão, foram substituídos pelo cereal adicionado de ferro na forma de mingau, o que proporcionou aumento de 2 a 3mg de ferro por dia. Os resultados mostraram que houve aumento significante na adequação do índice P/E das crianças com desnutrição atual. Quanto aos níveis de hemoglobina sérica, o grupo I, cujo valor médio inicial foi de 9,9 ± 2,0g/dl, apresentou valor médio significantemente mais alto ao final do estudo, 11,4 ± 1,0g/dl (p< 0,005). O grupo II, cuja média de hemoglobina sérica foi de 11,6 ± 0,9g/dl, não apresentou diferenças significantes ao final do estudo, apresentando média de 11,9 ± 0,9g/dl. A prevalência de anemia no grupo I ao primeiro exame foi de 81,0%, reduzindo-se para 31,2% no final de dois meses. Já para o grupo II a prevalência inicial foi de 20,8%, reduzindo-se para 12,5%. A avaliação da aceitação do produto mostrou que na primeira semana a ingestão média foi menor do que nas semanas seguintes até o final do estudo. Assim, pode-se concluir que o produto utilizado neste estudo teve impacto positivo nos níveis de hemoglobina sérica das crianças que apresentavam anemia, contribuindo também para a recuperação nutricional, no caso dos desnutridos.

Anemia and energetic malnutrition are considered to be the major nutritional challenges in developing countries. The efficacy of the various programs implemented to solve these problems has been assessed by studies with enriched food products. The objective of the present study was to evaluate the daily acceptance of an iron-enriched cereal, as well as its effect on the anthropometric parameters and hemoglobin levels of children aged 1 to 4 years old. Fifty-four children enrolled at a public day care unit participated in the study. They were divided into two groups: Group I with 24 children (median age: 2 years and 2 months) and Group II with 30 children (median age: 3 years and 6 months). Their food intake, weight, height and serum hemoglobin levels were measured at the beginning and at the end of the study. During a period of two months, milk and bread, which made up the children's breakfast and afternoon snack, were substituted by the enriched cereal in the form of porridge, which increased the iron intake in 2 to 3mg per day. The results showed that there was a significant increase in the weight for height rate of both groups, with improvement of nutritional condition in those children with acute malnutrition. With regard to serum hemoglobin levels, Group I showed an initial average level of 9.9 ± 2.0g/dl. The average level at the final evaluation, 11.4 ± 1.0g/dl, was significantly higher (p< 0.05) than at the start. Group II, with average serum hemoglobin level of 11.6 ± 0.9g/dl at the start, did not show a significant difference from the level at the end (11.9± 0.9g/dl).At the start of the study the incidence of anemia in Group I was of 81%. This value went down to 31.2% at the end of two months. For Group II, the incidence decreased from 20.8% to 12.5% at the end. The average daily intake of cereal during the first week was lower than in subsequent weeks. It is concluded that the product used in this study increased the serum hemoglobin levels of the children with anemia and contributed to the nutritional improvement of those who were malnourished. In addition, the cereal was well accepted by the children.
