RESUMO O enriquecimento com microrganismos benéficos pode aumentar os benefícios do composto orgânico. Este estudo teve o objetivo de avaliar o enriquecimento de um composto maturado com quatro bactérias promotoras de crescimento de plantas e fungo micorrízico (Rhizophagus clarus) usando braquiária (Urochloa brizantha) como planta hospedeira, compreendendo sete tratamentos: controle (composto sem braquiária, bactérias ou fungo micorrízico); composto + braquiária; composto + braquiária + fungo micorrízico; composto + braquiária + fungo micorrízico + Azorhizobium sp.; composto + braquiária + fungo micorrízico + Azoarcus sp.; composto + braquiária + fungo micorrízico + Bacillus subtilis; e composto + braquiária + fungo micorrízico + Azotobacter sp., em delineamento inteiramente casualizado com três repetições. A biomassa aérea da braquiária, teores de P e N, colonização micorrízica e características químicas do composto foram monitoradas cinco vezes por 183 dias. Bacillus subtilis e Azotobacter aumentaram o teor de N-amônio no composto em duas e três avaliações, respectivamente, enquanto Azotobacter e Azoarcus diminuíram os teores de N-nitrato em pelo menos uma avaliação. Apesar da alta disponibilidade de P (951 a 2927 mg kg-1), a colonização micorrízica chegou a 53%. Em experimento de campo com os compostos produzidos, no delineamento em blocos ao acaso com seis repetições, os compostos com braquiária dobraram a colonização micorrízica do milho (Zea mays) e do trigo (Triticum eaestivum), independente da bactéria promotora de crescimento e, dependendo da bactéria associada, aumentou a produtividade de grãos.

ABSTRACT Enrichment with beneficial microorganisms may increase the benefits of organic compost. The aim of this study was to evaluate the enrichment of a mature compost with plant growth-promoting bacteria and arbuscular mycorrhizal fungus (Rhizophagus clarus), using brachiaria (Urochloa brizantha) as host plant, totaling seven treatments: control (compost with no bacteria, brachiaria or mycorrhizal fungus); compost + brachiaria; compost + brachiaria + mycorrhizal fungus; compost + brachiaria + mycorrhizal fungus + Azorhizobium sp.; compost + brachiaria + mycorrhizal fungus + Azoarcus sp.; compost + brachiaria + mycorrhizal fungus + Bacillus subtilis; and compost + brachiaria + mycorrhizal fungus + Azotobacter sp., in a completely randomized design with three replicates. Brachiaria shoot biomass, N and P concentrations, mycorrhizal colonization, and chemical characteristics of the compost were assessed five times over 183 days. B. subtilis and Azotobacter increased ammonium-N concentration in the compost in two and three sampling dates, respectively. In contrast, Azotobacter and Azoarcus decreased the concentrations of nitrate-N in at least one sampling. Despite high P availability in the compost (951-2927 mg kg-1), mycorrhizal colonization reached up to 53%. In a field trial with the produced composts, in a randomized block design with six repetitions, the composts with brachiaria doubled the mycorrhizal colonization of corn (Zea mays) and wheat (Triticum aestivum), independent of the growth-promoting bacteria and, depending on the associated bacteria, increased grain yields.

Resumo Desde o início da pandemia de COVID-19, muitas questões surgiram referentes à segurança do manejo anestésico de pacientes acometidos pela doença. A anestesia regional, seja esta periférica ou neuroaxial, é alternativa segura no manejo do paciente COVID-19, desde que o emprego de modalidades que minimizam o comprometimento da função pulmonar seja escolhido. A adoção dessa técnica anestésica minimiza os efeitos adversos no pós-operatório e oferece segurança para o paciente e equipe, desde que sejam respeitados os cuidados com proteção individual e de contágio interpessoal. Respeito às contraindicações e emprego criterioso das técnicas e normas de segurança são fundamentais. Este manuscrito tem por objetivo revisar as evidências disponíveis sobre anestesia regional em pacientes com COVID-19 e oferecer recomendações práticas para sua realização segura e eficiente.

Abstract Since the beginning of the COVID-19 pandemic, many questions have come up regarding safe anesthesia management of patients with the disease. Regional anesthesia, whether peripheral nerve or neuraxial, is a safe alternative for managing patients with COVID-19, by choosing modalities that mitigate pulmonary function involvement. Adopting regional anesthesia mitigates adverse effects in the post-operative period and provides safety to patients and teams, as long as there is compliance with individual protection and interpersonal transmission care measures. Respecting contra-indications and judicial use of safety techniques and norms are essential. The present manuscript aims to review the evidence available on regional anesthesia for patients with COVID-19 and offer practical recommendations for safe and efficient performance.

Resumo Objetivou-se com este trabalho investigar as características sanitárias e de manejo das revendas de aves vivas, bem como a identificação e caracterização antigênica de Salmonella enterica e sua sensibilidade a antimicrobianos. Foram aplicados questionários estruturados, além de 627 amostras coletadas nas gaiolas, sendo 209 de excretas, 209 de ração e 209 suabes de bebedouros. Essas amostras foram processadas por bacteriologia convencional. Os isolados obtidos foram submetidos ao teste de suscetibilidade e a 12 antimicrobianos pelo método de difusão em disco. Verificou-se que 91,7% das revendas estudadas alojam Gallus gallus domesticus juntamente a outras espécies animais, comercializam aves com escassa documentação zoosanitária, a vigilância ativa é insatisfatória e utilizam e comercializam antimicrobianos de forma indiscriminada. Detectou-se a presença da Salmonella enterica em 1,4% (9/627) das amostras analisadas nas gaiolas, sendo 1,9% (4/209) em excretas, 0,95% (2/209) em ração e em 1,4% (3/209) em suabes de bebedouros, as quais foram caracterizadas, antigenicamente, como Salmonella Heidelberg, Gallinarum, Risen, Ndolo, Saint Paul, Mbandaka e subesp enterica O:6,7. Na determinação da suscetibilidade aos antimicrobianos, obteve-se resistência de 44,4% (4/9) para Trimetoprim-sulfametoxazol, 33,3% (3/9) para enrofloxacina, 22,2% (2/9) para ciprofloxacina, ceftiofur, amoxicilina e 11,1% (1/9) para tetraciclina e fosfomicina. Salmonella Heidelberg, além dos sorovares Gallinarum, Risen, Saint Paul e Mbandaka, todos mostraram resistência a pelo menos um dos antimicrobianos testados. Trimetoprim-sulfametoxazol e enrofloxacina foram os antimicrobianos que apresentaram menor eficácia. Sorovares como Heidelberg, Gallinarum e Mbandaka apresentam multirresistência aos antimicrobianos de uso comum em medicina humana e veterinária, o que implica em riscos potenciais para saúde única.

Abstract The objective of this study was to investigate the sanitary and management characteristics of live-bird resellers as well as identify and undertake an antigenic characterization of Salmonella enterica and its sensitivity to antimicrobials. Structured questionnaires were applied and 627 samples were collected from the cages, consisting 209 samples of excreta, 209 of feed and 209 drinker swabs. These were processed by conventional bacteriology. The obtained isolates were subjected to the susceptibility test and to 12 antimicrobial tests by the disk diffusion method. Of the studied resellers, 91.7% house Gallus gallus domesticus, together with other animal species; sell birds with little zoosanitary documentation; have unsatisfactory active surveillance; and use and sell antimicrobials indiscriminately. The presence of Salmonella enterica was detected in 1.4% (9/627) of the samples analyzed in the cages, with 1.9% (4/209) found in excreta, 0.95% (2/209) in feed and in 1.4% (3/209) in drinker swabs. These were characterized antigenically as Salmonella Heidelberg, Gallinarum, Risen, Ndolo, Saint Paul, Mbandaka and subsp enterica O:6,7. When susceptibility to antimicrobials was determined, 44.4% resistance (4/9) was detected for trimethoprim-sulfamethoxazole, 33.3% (3/9) for enrofloxacin, 22.2% (2/9) for ciprofloxacin, ceftiofur and amoxicillin and 11.1% (1/9) for tetracycline and fosfomycin. Salmonella Heidelberg, as well as serovars Gallinarum, Risen, Saint Paul and Mbandaka, showed resistance to at least one of the tested antimicrobials. Trimethoprim-sulfamethoxazole and enrofloxacin were the antimicrobials that showed the least efficacy. Serovars such as Heidelberg, Gallinarum and Mbandaka have multiresistance to antimicrobials commonly used in human and veterinary medicine, implying potential risks to One Health.

RESUMO O costão rochoso de Praia Grande (Ubatuba, SP) é formado por matacões rochosos e recifes de areia formados por poliquetas sabelarídeos. Essas estruturas oferecem abrigo e área de forrageamento para diversos organismos marinhos, e atraem muitos turistas. O caranguejo Eriphia gonagra (Fabricius, 1781) habita essas estruturas, podendo ser utilizado como bioindicador, uma vez que estas podem ser afetadas por ações antrópicas. Por meio deste estudo avaliou-se o conteúdo estomacal e caracterizou-se os hábitos alimentares de uma população de E. gonagra, levando em consideração o sexo, tamanho e microhabitat. Amostragens mensais foram realizadas em 1996 e 1997 e os caranguejos foram capturados manualmente na Praia Grande. Assim, os espécimes amostrados foram caracterizados de acordo com o local de captura (microhabitat) como recife rochoso (RO) ou de areia (SR). Os itens mais abundantes foram moluscos, algas, poliquetos e crustáceos, sendo observada a importância destes grupos como recurso alimentar no ambiente estudado. Os moluscos foram mais abundantes nos estômagos dos caranguejos amostrados no RO, enquanto os demais item aumentavam nos caranguejos dos recifes de areia. Dentre os espécimes amostrados no SR, poliquetos foi o alimento foi mais comum, indicando que esse biótopo também é utilizado como fonte rica em proteínas. A proporção de itens diferiu também entre classes de tamanho. Conhecer tais hábitos é essencial para estudos de requisitos nutricionais, monitoramento de relações entre organismos, bem como para avaliação de impactos ecossistêmicos em costões rochosos.

ABSTRACT The rocky shores of Praia Grande, in Ubatuba (São Paulo, Brazil), are formed by rock and Sabellaridae polychaete sandy reefs. These microenvironments offer shelters and foraging areas for several marine organisms and attracts many tourists. The crab Eriphia gonagra (Fabricius, 1781) inhabits these structures and can be used as a bioindicator, since they may be affected by anthropogenic actions. This study evaluated the stomach contents and characterized the feeding habits of an E. gonagra population, inhabiting the rocky shores from Praia Grande, taking into account the sex, size, and microhabitat. Monthly samplings were performed in 1996 and 1997 and the crabs were manually captured on the rocky surface or into of the sand reefs. So, the specimens sampled were characterized according to their capture site (microhabitat) as rocky (RO) or sand reef (SR). The most abundant food items were Mollusca, algae, Polychaeta, and Crustacea, being observed the importance of these groups as a nutritive resource in the studied environment. Mollusks were more abundant in the stomachs of crabs sampled on the rocks, while the other items increased in the sand reefs crabs. In the specimens sampled in the SR, polychaetes were the item food more common, indicating that this biotope is also used as a rich source of protein. The proportion of items differed also between size classes. Knowledge of such habits is essential for nutritional requirements studies, monitoring relationships among organisms, as well as assessing future environmental impacts in consolidated coastal regions.

ResumenLepidaploa rufogrisea es una candidata prometedora en la búsqueda de herbicidas naturales, con compuestos (lactano sesquiterpeno- glaucolíde B) los cuales han sido reportados por presentar un potencial fitotóxico contra las malas hierbas. Por lo tanto, este trabajo tuvo como objetivo lograr la investigación fitoquímica de L. rufogrisea, así como también la evaluación de sus efectos biológicos sobre el modelo de planta Lactuca sativa L. Para esto, diferentes extractos (acuosos, hidroalcohólicos y hexanos) de L. rufogrisea fueron evaluados. El porcentaje de semillas germinadas, el índice de velocidad de germinación y la longitud de raíz de plántulas de L. sativa var. Cristina fueron evaluadas, así como también la citotoxicidad de los extractos en las células meristemáticas. Los extractos disminuyeron el índice de velocidad de germinación y también el crecimiento de las raíces de las plántulas de L. sativa. La proliferación celular no se vio significativamente afectada por los extractos. Sin embargo, se observaron alteraciones cromosómicas y nucleares, debido a la interferencia de los aleloquímicos presentes en L. rufogrisea en el huso mitótico. Además, el modelo de planta L. sativa demostró ser eficiente para tales estudios.

AbstractLepidaploa rufogrisea is a promising candidate in the search of natural herbicides, with compounds (sesquiterpene lactones-glaucolide B) which have been reported to present phytotoxic potential against weeds. Thus, this work aimed to accomplish the phytochemical investigation of L. rufogrisea as well as the evaluation of its biological effects on the plant model Lactuca sativa. To this end, different extracts (aqueous, hydroalcoholic and hexane) of L. rufogrisea at different concentrations [0.25, 0.5, 1.0, 2.0 and 4.0 % (m/v)] were evaluated from macroscopic (percentage of germinated seeds, germination speed index and root length) and microscopic analysis (mitotic index and cell cycle alterations) in plantlets (150 per treatment) and meristematic cells (10 000 per treatment) of L. sativa var. Cristina. The extracts decreased the germination speed index and also the root growth of L. sativa plantlets. Cell proliferation was not significantly affected by the extracts. However, some chromosome and nuclear alterations in the mitotic spindle were observed, due to the interference of allelochemicals present in L. rufogrisea. Moreover, the plant model L. sativa demonstrated to be efficient for this kind of studies.

Abstract This study investigated the temporal abundance, periods of reproduction and recruitment, gonad sexual maturity, and population structure of the swimming crab Achelous spinimanus (Latreille, 1819) in Macaé, a region of the Brazilian coast affected by upwelling events. The samples were collected monthly during one year. The crabs were counted, sexed and measured for maximum carapace width (MCW). Four developmental stages were described in males and females. Environmental variables as temperature, salinity, organic matter and Phi were measured. A Principal Components Analysis indicated that environmental variables were strongly correlated with seasons, affecting the abundance of this species, which was high during summer and autumn. The periods of reproduction and recruitment were also seasonal, with a peak of females with developed gonads and ovigerous females during autumn and immatures during summer and autumn. Males and females did not differ in size at gonad sexual maturity, estimated at 39.9 and 42.9 mm MCW, respectively. The environmental conditions in this upwelling region may have induced this population of A. spinimanus to show different characteristics and behaviors from other populations on the Brazilian coast, since alterations linked to the periods of reproduction and recruitment, as well as the population structure and distribution, may reveal different survival strategies of the species.

Abstract Objective: In cardiac surgery, the lung, renal and neurological events are the most frequent complications. Less common, acute liver failure is a serious complication that adds high morbidity, mortality, and costs. Therefore, this communication aimed to retrospectively evaluate five patients who presented, in 2014, severe acute liver failure in the immediate postoperative period. Methods: Retrospective data analysis of patients' medical records that showed severe liver failure has been computed in the medical records of five patients undergoing cardiac surgery at the Hospital da Faculdade de Medicina de Ribeirão Preto – USP in the immediate postoperative period from February 1, 2014 to December 12, 2014. The study selected five males patients, 60 to 67 years old, cardiopulmonary bypass mean time of 101.4 minutes (varying from 80 to 140 minutes), who presented acute perioperative liver failure. Results: The five patients showed an impressive increase of blood transaminase (serum alanine aminotransferase), suggesting acute hepatitis. The evolution of all patients was catastrophic, with severe hemodynamic effects and death. Many studies suggest that systemic hypotension is an important pathogenic factor for ischemic hepatitis. However, our data and previous studies raise the possibility that other yet unknown factors other than hypotension may be part of the pathophysiology of cardiopulmonary bypass after ischemic hepatitis (anticoagulation inadequate for the quality of heparin and protamine, etc.). Conclusion: Currently, there are no conclusive studies on the prevention of perioperative liver failure. More well-designed studies are needed on the introduction and evolution of liver dysfunction after cardiac surgery.

Objetivo: O presente estudo foi realizado com a finalidade de reavaliar conceitos estabelecidos em 20 anos, com ênfase nos últimos 5 anos, sobre a utilização do azul de metileno no tratamento da síndrome vasoplégica em cirurgia cardíaca. Métodos: Foram considerados dados da literatura utilizando-se três bases de dados (MEDLINE, SCOPUS e ISI Web of Science). Resultados: Os conceitos reavaliados e reafirmados foram: 1) Nas doses recomendadas o AM é seguro (a dose letal é de 40 mg/kg); 2) O AM não causa disfunção endotelial; 3) O efeito do AM só aparece em caso de supra nivelamento do NO; 4) O AM não é um vasoconstritor, pelo bloqueio da via GMPc ele libera a via do AMPc, facilitando o efeito vasoconstritor da norepinefrina; 5) A dosagem mais utilizada é de 2 mg/kg, como bolus EV, seguida de infusão contínua porque as concentrações plasmáticas decaem fortemente nos primeiros 40 minutos, e; 6) Existe uma "janela de oportunidade" precoce para efetividade do AM. Nos últimos cinco anos, os principais desafios foram: 1) Observações de efeitos colaterais; 2) A necessidade de diretrizes, e; 3) A necessidade da determinação de uma janela terapêutica para o uso do AM em humanos. Conclusão: O efeito do AM no tratamento da SV é dependente do tempo, portanto, o grande desafio atual é a necessidade do estabelecimento da janela terapêutica do AM em humanos. Esse seria o primeiro passo para a sistematização de uma diretriz a ser seguida por possíveis estudos multicêntricos.

Objective: This study was conducted to reassess the concepts established over the past 20 years, in particular in the last 5 years, about the use of methylene blue in the treatment of vasoplegic syndrome in cardiac surgery. Methods: A wide literature review was carried out using the data extracted from: MEDLINE, SCOPUS and ISI WEB OF SCIENCE. Results: The reassessed and reaffirmed concepts were 1) MB is safe in the recommended doses (the lethal dose is 40 mg/kg); 2) MB does not cause endothelial dysfunction; 3) The MB effect appears in cases of NO up-regulation; 4) MB is not a vasoconstrictor, by blocking the cGMP pathway it releases the cAMP pathway, facilitating the norepinephrine vasoconstrictor effect; 5) The most used dosage is 2 mg/kg as IV bolus, followed by the same continuous infusion because plasma concentrations sharply decrease in the first 40 minutes; and 6) There is a possible "window of opportunity" for MB's effectiveness. In the last five years, major challenges were: 1) Observations about side effects; 2) The need for prophylactic and therapeutic guidelines, and; 3) The need for the establishment of the MB therapeutic window in humans. Conclusion: MB action to treat vasoplegic syndrome is time-dependent. Therefore, the great challenge is the need, for the establishment the MB therapeutic window in humans. This would be the first step towards a systematic guideline to be followed by possible multicenter studies.

Introduction: This study aimed to inventory the phlebotomine sandfly fauna present in the urban area of Juiz de Fora, with an emphasis on the genus Lutzomyia. Methods: Capture was performed from March to September 2012, using HP light traps placed at peridomestic sites, in a municipal kennel and a forest biome. Results: A total of 133 specimens were captured, representing eight species of the genus Lutzomyia. Lutzomyia pascalei was the most prevalent species. Conclusions: This research provides an inventory and description of the spatial locations of the phlebotomine sandfly fauna of Juiz de Fora.

The spatial and seasonal distribution of different demographic groups of the swimming crab Callinectes ornatus was analyzed with respect to its population biology in three bays (Ubatumirim, Ubatuba, and Mar Virado) in southeastern Brazil. In each bay, monthly sampling was performed during two years along six transects, established at 5, 10, 15, and 20 m depth (parallel to the beach line), in a wave-sheltered area (7.5 m depth), and at an exposed site (10 m depth). Total abundance of crabs was similar among the bays. The presence of immature individuals, adult females, and breeding females was positively correlated with the bottom and surface temperatures, whereas adult males showed an opposite trend with respect to these factors. Immature and smaller individuals were most abundant along the sheltered and shallower transects. Males were more abundant along the shallower transects, and females at 15 and 20 m. This study revealed that different demographic groups occupy the habitat in different ways, according to local features. In general, southeastern Brazil offers a range of conditions that favor reproduction and consequently the maintenance and growth of the local population of C. ornatus, contributing to the relatively high abundance of the species in this coastal area.

Se analizó la distribución espacial y temporal de distintos grupos demográficos del cangrejo nadador Callinectes ornatus en cuanto a su biologia poblacional en tres bahias (Ubatumirim, Ubatuba y Mar Virado) en el sureste de Brasil. En cada bahia se realizaron muestreos mensuales durante dos años en seis transectos, establecidos a 5, 10, 15 y 20 m de profundidad (paralelo a la linea de costa), en una zona protegida del oleaje (7.5 m de profundidad) y en un sitio expuesto al oleaje (10 m de profundidad). La abundancia total de cangrejos fue similar entre bahias. Se observó una correlación positiva entre la presencia de individuos inmaduros, hembras adultas y hembras ovigeras con la temperatura del fondo y superficial, mientras que los machos adultos mostraron una tendencia opuesta con respecto a estos factores. La mayor abundancia de individuos inmaduros y pequeños fue detectada en los transectos protegidos y menos profundos. Los machos fueron más abundantes en los transectos menos profundos, y las hembras a 15 y 20 m de profundidad. Este estudio reveló que los diferentes grupos demográficos ocupan el hábitat de diferentes maneras, de acuerdo con las caracteristicas locales. En general, el sureste de Brasil ofrece diferentes condiciones que favorecen la reproducción y consecuentemente la continuidad y el crecimiento de la población local de C. ornatus, contribuyendo a la relativamente alta abundancia de la especie en esta zona costera.

O presente trabalho contribui com o estudo da gênese e da evolução histórica da indústria de máquinas e implementos agrícolas, destacando o papel de algumas empresas e como suas estratégias moldaram a estrutura produtiva e de mercado atuais, um "oligopólio diferenciado-concentrado mundial". Do ponto de vista metodológico, este é um texto qualitativo-quantitativo, exploratório e descritivo. A pesquisa bibliográfica mostrou que existiam poucos trabalhos sobre a evolução da estrutura de mercado e das empresas atuantes neste segmento, o que gerou a necessidade de um levantamento exploratório de dados e uma ampla revisão de literatura. O texto mostra a importância do progresso técnico e dos ganhos de escala no processo competitivo deste setor. O trabalho conclui que o setor tem as características de um oligopólio diferenciado-concentrado, sendo a inovação e a diferenciação de produtos através da fidelização dos clientes à marca de dois importantes instrumentos competitivos, assim como as economias de escala e escopo. Outra característica fundamental é o caráter global desta concorrência, com poucas empresas com atuação mundial e muitas com atuação regional ou local.

This paper aims to resume the agricultural machinery history and development, as well as their majors companies, in order to understand the current market structure and the prospects for their future. The first discussion is related to the importance of technical progress to compete in this environment. We study the competition and technical progress in this industry, focusing on technological trajectories and on collecting information from the literature about the development of this industry. The study concludes thatthe industry retains some differentiated oligopoly characteristics, with innovation and product differentiation as important competitive tools. Another key aspect is the global nature of this competition.

INTRODUÇÃO: Perfusato autólogo retrógrado (PAR) é uma técnica de circulação extracorpórea (CEC) com baixos custos. Estudos anteriores demonstraram que esta técnica reduz a hemodiluição e a necessidade de transfusões de sangue por meio do aumento do hematócrito intraoperatório. OBJETIVO: Avaliar técnica de PAR em relação à CEC técnica padrão (perfusato cristaloide) em pacientes adultos. MÉTODOS: Sessenta e dois pacientes foram aleatoriamente alocados em dois grupos: 1) Grupo PAR (n = 27), constituído por pacientes operados utilizando a técnica de PAR e; 2) Grupo Controle, constituído por pacientes operados utilizando técnica padrão de CEC com cristaloides (n = 35). A PAR foi realizada drenando-se o perfusato cristaloide das linhas arterial e venosa, antes da CEC, para uma bolsa coletora de recirculação. Os principais parâmetros analisados foram: 1) parâmetros hemodinâmicos da CEC; 2) valores de hematócrito e hemoglobina; e; 3) necessidade de transfusões de sangue. RESULTADOS: Observaram-se diferenças estatisticamente significativas de transfusão no intraoperatório e diminuição da hemodiluição em CEC utilizando PAR. Os valores hemodinâmicos durante a CEC foram semelhantes, observando-se tendência de utilização de fluxos menores na CEC dos pacientes do grupo PAR. CONCLUSÃO: O presente estudo foi projetado em pequena escala para avaliar os efeitos do PAR, o que foi demonstrado em relação aos já conhecidos efeitos na diminuição da hemodiluição em CEC e transfusão sanguínea, porém não mostrou vantagens hemodinâmicas em relação à técnica padrão com perfusato cristaloide.

INTRODUCTION: Retrograde autologous priming (RAP) is a cardiopulmonary bypass (CPB) method, at low cost. Previous studies have shown that this method reduces hemodilution and blood transfusions needs through increased intra-operative hematocrit. OBJECTIVE: To evaluate RAP method, in relation to standard CPB (crystalloid priming), in adult patients. METHODS: Sixty-two patients were randomly allocated to two groups: 1) Group RAP (n = 27) of patients operated using the RAP and; 2) Control group of patients operated using CPB standard crystalloid method (n = 35). The RAP was performed by draining crystalloid prime from the arterial and venous lines, before CPB, into a collect recycling bag. The main parameters analyzed were: 1) CPB hemodynamic data; 2) Hematocrit and hemoglobin values; 3) The need for blood transfusions. RESULTS: It was observed statistically significant fewer transfusions during surgery and reduced CPB hemodilution using RAP. The CPB hemodynamic values were similar, observing a tendency to use lower CPB flows in the RAP group patients. CONCLUSION: This investigation was designed to be a small-scale pilot study to evaluate the effects of RAP, which were demonstrated concerning the CPB hemodilution and blood transfusions.

INTRODUÇÃO: A desnutrição é uma complicação frequente nos pacientes em hemodiálise, sendo o diagnóstico precoce importante para reduzir as taxas de morbidade e mortalidade do tratamento. OBJETIVO: Investigar a utilidade dos exames bioquímicos mensais realizados na hemodiálise para triagem de pacientes com desnutrição. MÉTODOS: O diagnóstico nutricional de 252 pacientes foi feito através da avaliação objetiva e subjetiva global, classificando-os em desnutridos e não desnutridos. Paralelamente, durante 4 meses consecutivos, as concentrates de creatinina, fósforo, ureia pré-e pósdiálise e o Kt/V foram registradas para cálculo das médias. Após análise dessas variáveis através da curva ROC, calculouse a sensibilidade e a especificidade na identificação dos pacientes com desnutrição. RESULTADOS: Na curva ROC, a área sob a curva para a ureia foi de 0,683, para o fósforo 0,71, para o Kt/V 0,724 e para a creatinina 0,765. Para valores de ureia < 90 mg/dL; fósforo < 4,2 mg/dL; Kt/V > 1,6 e creatinina < 6,5 mg/dL, a especificidade variou entre 80% e 88% e a sensibilidade entre 26 % e 51% . O valor preditivo negativo variou entre 90% e 92% e o valor preditivo positivo entre 23% e 32%. A associação de dois ou mais desses índices não modificou de forma significante esses valores. CONCLUSÕES: Nossos resultados sugerem que valores de ureia < 90 mg/dL, creatinina < 6,5 mg/dL, fósforo < 4,2 mg/dL e Kt/V > 1,6 podem ser utilizados para triagem de pacientes com desnutrição.

INTRODUCTION: Malnutrition is a frequent complication in patients on hemodialysis and early diagnosis is important to reduce the morbidity and mortality of treatment. OBJECTIVE: To investigate the usefulness of biochemical tests performed monthly in order to identify hemodialysis patients with malnutrition. METHODS: The nutritional status of 252 patients was evaluated by objective and subjective global assessment, and the patients classified as malnourished and not malnourished. Then, during 4 consecutive months, serum creatinine, phosphorus, urea pre-and post-dialysis and Kt/V were recorded for calculation of averages. After analysis of these variables by the RO C curve we calculated the sensitivity and specificity of these parameters to identify patients with malnutrition. RESULTS: In ROC curve, the area under the curve for urea was 0.683, 0.71 for phosphorus, 0.724 for Kt/V and 0.765 for creatinine. For values of urea < 90 mg/dL, phosphorus < 4.2 mg/dL, Kt/V > 1.6 and creatinine < 6.5 mg/dL, the specificity ranged between 80% and 88% and sensitivity between 26% and 51%. The negative predictive value ranged between 90% and 92% and positive predictive value between 23% and 32%. The association of two or more of these índices did not change significantly these values. CONCLUSIONS: Our results suggest that serum urea < 90 mg/dL, creatinine < 6.5 mg/dL, phosphorus < 4.2 mg/dL, and Kt/V > 1.6 can be used for screening patients with malnutrition. However, using these cutoffs the parameters tend to overestimate the number of patients with malnutrition.

Este trabalho objetivou avaliar o comportamento da vegetação espontânea com relação às taxas de decomposição de diferentes coberturas do solo submetidas à variação de lâminas de irrigação. Foram utilizados resíduos vegetais de Gliricídia (Gliricidia sepium), feijão-guandu (Cajanus cajan), capim Cameroon (Pennisetum purpureum) e Bambu (Bambusa vulgaris'vitata'). As lâminas irrigadas foram caracterizadas como T5, T4, T3, T2 e T1, correspondendo a aproximadamente 117, 100, 72, 59 e 38% da ETo. A decomposição dos resíduos vegetais foi analisada por meio de covered litter, em 2, 4, 7, 15, 30, 60, 90, 120 e 150 dias após instalação (DAI). Foram realizadas amostragens da vegetação espontânea aos 26, 64, 92, 124 e 152 DAI, em uma área de 0,0625 m². Foi possível concluir que o comportamento da vegetação espontânea está diretamente relacionado com a cinética de decomposição dos resíduos vegetais.

This work was carried out in order to evaluate the behavior of spontaneous vegetation regarding the decomposition rates of different soil covers submitted to variable irrigation depths. The vegetable residues utilized were from gliricidia (Gliricidia sepium), pigeon pea (Cajanus cajan), elephant grass (Pennisetum purpureum) and bamboo (Bambusa vulgaris'vitata'). The irrigated depths were characterized as T5, T4, T3, T2 and T1, corresponding to approximately 117, 100, 72, 59 and 38% of the ETo. The decomposition of the vegetable residues was analyzed through covered litter, at 2, 4, 7, 15, 30, 60, 90, 120 and 150 days after installation (DAI) and samples of the spontaneous vegetation were collected at 26, 64, 92, 124 and 152 DAI, in a 0.0625 m² area. It was possible to conclude that the behavior of the spontaneous vegetation is directly related to the kinetics of decomposition of the vegetable residues.

Neste trabalho, a universalização do acesso às vacinas no Brasil é discutida sob a ótica da Pesquisa Operacional (PO). Um modelo militar clássico - o problema de Alocação Arma-Alvo (WTA) - é transformado em um modelo com fins humanitários, cujo objetivo é maximizar a cobertura da distribuição de vacinas na população ao invés de maximizar a destruição de alvos militares. Pode-se afirmar que há um grande espaço para o uso de técnicas de PO no sistema de saúde, pois seu planejamento ainda é feito, em geral, de forma manual por profissionais de formação exclusivamente médica. Além disso, o armazenamento digital de dados do sistema público de saúde abre novas possibilidades para a mineração de dados e análise estatística, ainda que, salvo raras iniciativas acadêmicas ou públicas, haja poucos programas permanentes para fomentar a análise estatística em dados de saúde como subsídio para o planejamento público. Em nossa pesquisa, o modelo proposto foi testado com dados de um problema WTA da literatura.

In this paper, the universal access to vaccines in Brazil is discussed from an Operations Research (OR) point of view. A classic military model - the Weapon-Target Assignment (WTA) problem - is converted to a humanitarian one, where the objective is to maximize the coverage of the vaccines in the population instead of to destroy military assets. There is an enormous space for OR techniques in the whole healthcare system, as most of the planning is still hand-made, and drawn by professionals with medical background only. In addition, the healthcare data systems open new possibilities to data mining and statistical analysis. There is still a shortage of permanent programs to analyze and use this kind of statistical information for public planning. There are initiatives from the academia and public entities in this direction. In our research, the proposed model was tested with data from an original WTA problem.
