RESUMO CONTEXTO: A associação entre distúrbios urinários e constipação funcional vem sendo observada em crianças e adultos, sendo a constipação funcional uma queixa comum em indivíduos com bexiga hiperativa. OBJETIVO: Avaliar a prevalência de constipação funcional, bexiga hiperativa e seus subtipos seco/úmido em mulheres e determinar quais os sintomas intestinais estão mais associados e são preditores de bexiga hiperativa. MÉTODOS: Estudo de corte transversal com mulheres abordadas aleatoriamente em locais públicos. Os critérios de exclusão foram: anormalidades neurológicas/anatômicas do intestino ou do trato urinário documentadas. A constipação foi definida como ≥2 sintomas positivos daqueles listados nos critérios de Roma. Alterações urinárias (frequência urinária aumentada, urgência, incontinência e noctúria) foram definidas por um escore ≥2 no respectivos itens do Questionário Internacional de Consulta sobre Incontinência - Bexiga Hiperativa. Foi denominada de bexiga hiperativa seca a presença de sintomas de urgência sem incontinência urinária e bexiga hiperativa úmida quando a urgência estava associada a incontinência urinária. RESULTADOS: Foram entrevistadas 516 mulheres com idade média de 35,8±6 anos. As taxas de constipação funcional, bexiga hiperativa, bexiga hiperativa seca e bexiga hiperativa úmida na amostra estudada foram de 34,1%, 15,3%, 8,9% e 6,4%, respectivamente. Foi observada associação entre constipação funcional e bexiga hiperativa / bexiga hiperativa seca, sendo a constipação funcional fator preditor para esse subtipo de bexiga hiperativa (OR=2,47). O escore de qualidade de vida foi pior nas mulheres com constipação funcional em comparação com as não constipadas e ainda pior nas mulheres com constipação funcional associada a bexiga hiperativa úmida (mediana 22,5; IC 95%: 17,25-35,25). A presença de manobras manuais estava significativamente associada aos dois subtipos de bexiga hiperativa. Os fatores preditivos independentes para bexiga hiperativa foram manobras manuais (OR=2,21) e <3 defecações/semana (OR=2,18), sendo este último o único fator preditivo para bexiga hiperativa seca (OR=3,0). CONCLUSÃO: Em mulheres, a constipação funcional está associada a bexiga hiperativa e seu subtipo seco, particularmente na população mais jovem. Além disso, essa associação é responsável por piores escores de qualidade de vida, principalmente quando a incontinência urinária está presente. A presença de manobras manuais e menos de três defecações por semana em mulheres devem nos direcionar a procurar por bexiga hiperativa.

ABSTRACT BACKGROUND: An association between urinary disorders and functional constipation has been registered in children and adults, with functional constipation being a common complaint in individuals with overactive bladder. OBJECTIVE: To evaluate the prevalence of functional constipation, overactive bladder and its dry/wet subtypes in women and to determine which bowel symptoms predict overactive bladder. METHODS: A cross-sectional study of women randomly approached in public spaces. Exclusion criteria: neurological/anatomical abnormalities of the bowel or urinary tract. Constipation was defined as ≥2 positive symptoms of those listed in the Rome criteria. Urinary abnormalities (frequent urination, urgency, incontinence, nocturia) were defined by a score ≥2 in the respective item of the International Consultation on Incontinence Questionnaire - Overactive Bladder. Dry overactive bladder was defined as urgency without incontinence, while wet overactive bladder included incontinence. RESULTS: A total of 516 women with a mean age of 35.8±6 years were interviewed. Rates of functional constipation, overactive bladder, dry overactive bladder and wet overactive bladder were 34.1%, 15.3%, 8.9% and 6.4%, respectively. Functional constipation was associated with overactive bladder and dry overactive bladder, with functional constipation predicting dry overactive bladder (OR=2.47). Quality of life was poorer in constipated women compared to non-constipated and even worse in constipated women with wet overactive bladder (median 22.5; 95%CI: 17.25-35.25). Manual maneuvers were significantly associated with both overactive bladder subtypes. Independent predictive factors for overactive bladder were manual maneuvers (OR=2.21) and <3 defecations/week (OR=2.18), with the latter being the only predictive factor for dry overactive bladder (OR=3.0). CONCLUSION: Functional constipation is associated with overactive bladder and its dry subtype, particularly in the younger population. In addition, this association is responsible for lower quality of life scores, especially when urinary incontinence is present. The presence of manual maneuvers and less than three defecations per week should direct us to look for overactive bladder.

Resumo Objetivo: Considerando a importância do rastreamento de câncer de próstata, a possibilidade de dano decorrente do rastreamento indiscriminado, a dificuldade de divulgação e adesão às diretrizes sobre o assunto, objetivamos identificar as principais diretrizes vigentes, procurar pontos de abordagem comuns e estabelecer um núcleo mínimo de condutas. Método: Revisão sistemática da literatura sobre guias de prática de rastreamento para câncer de próstata nas bases Pubmed, Lilacs e Google Scholar, além de busca ativa nos sítios de diversas entidades de saúde nacionais. Resultados: Foram selecionadas 12 diretrizes, cuja análise resultou na identificação de seis pontos comuns de conduta, com o seguinte núcleo mínimo de recomendações: (1) a indicação ou não de rastreamento: deve ser individualizada e precedida de uma decisão informada; (2) os exames utilizados: PSA com ou sem exame digital retal; (3) a idade de início geral: 50-55 anos; (4) a idade de início em homens com risco aumentado: 40 anos; (5) o intervalo entre os rastreamentos: anual ou bienal; e (6) a idade de suspensão do rastreamento: 70 anos ou expectativa de vida menor que 10 anos. Conclusão: Embora existam divergências entre elas, foi possível estabelecer um núcleo mínimo de condutas que podem ser úteis na prática diária do médico.

Summary Objective: Considering the importance of screening for prostate cancer, the possibility of damage resulting from indiscriminate screening and the difficulty of disclosure and adherence to the main guidelines on the subject, we aimed to identify current guidelines, look for common approaches and establish a core of conducts. Method: Systematic review of the literature on screening practice guidelines for prostate cancer searching the databases PubMed, Lilacs and Google Scholar and active search in the sites of several national health entities. Results: Twelve (12) guidelines were selected, whose analysis resulted in the identification of six common points of conduct, with the following minimum core of recommendations: (1) screening indication or not: must be individualized, and preceded by an informed decision; (2) tests used: PSA with or without rectal digital examination; (3) age at which initiate testing in men in general risk: 50-55 years; (4) age at which to initiate testing in men at increased risk: 40-45 years; (5) the interval between screening: annual or biennial; and (6) age at which to discontinue testing: 70 years-old or life expectancy less than 10 years. Conclusion: Although there are differences between them, it was possible to establish a minimum core of conducts that may be useful in the daily practice of the physician.

Resumo O presente trabalho objetivou estudar a biometria, tratamentos pré-germinativos e substratos para germinação de diásporos de Balfourodendron riedelianum. Os diásporos foram caracterizados na germinação quanto ao comprimento, largura, espessura, peso de mil sementes (PMS) e teor de água. Foram empregados os métodos de escarificação com lixa e ácido sulfúrico, imersão em água fria e em água a 100 °C, e o controle (sem tratamento), associados a diferentes substratos (entre papel (EP), entre areia (EA) e, entre vermiculita (EV), sendo conduzidos em câmara de germinação a 25 °C. A caracterização fisiológica foi realizada por meio da primeira contagem, germinação e índice de velocidade de germinação (IVG). Os dados biométricos foram analisados em classes de frequências e os tratamentos pré-germinativos por meio da análise de variância. Os diásporos apresentam em média 18,59 mm de comprimento; 9,03 mm de largura e 9,38 mm de espessura. O tratamento de imersão em água fria por 48 horas e o substrato entre vermiculita (EV) foi efetivo para a superação da dormência e o mais adequado para o teste de germinação da espécie.

Abstract This study investigated the biometry, pre-germination treatments and substrates for the germination of diaspores of Balfourodendron riedelianum. The diaspores were characterized during germination according to their length, width, thickness, thousand kernel weight (TKW) and water content. Dormancy overcoming was tested by diaspore scarifying with sandpaper, sulfuric acid immersion, water and water at 100 °C; together with the control (untreated), they were associated to different substrates (including paper (EP), sand (EA) and vermiculite (EV). Germination tests were conducted in a germination chamber at 25 °C. The physiological quality of seeds was performed through the first count, germination and germination speed index (GSI). Biometric data were analyzed in frequency classes and the pre-germination treatment by analysis of variance. Diaspores are on average 18.59 mm long; 9.03 mm wide and 9.38 mm thick. The treatment of immersion in cold water for 48 hours and the substrate vermiculite (EV) were effective to overcome dormancy and recommended for germination tests.

Ligadura da artéria etmoidal anterior (AEA) pode ser necessária em casos de epistaxe grave refratária ao tratamento tradicional. O uso da ligadura endoscópica endonasal da AEA ainda é bastante limitado. Existem poucos estudos na literatura sobre a técnica de abordagem endoscópica desta artéria. OBJETIVOS: Demonstrar a aplicabilidade técnica da ligadura periorbitária da AEA por via endoscópica transetmoidal. MATERIAL E MÉTODOS: Estudo prospectivo. 50 fossas nasais de cadáveres foram dissecadas. Após a realização de uma etmoidectomia anterior e remoção parcial da lâmina papirácea, a periórbita foi cuidadosamente dissecada até a identificação da AEA. Após sua identificação, a artéria foi exposta e ligada dentro da órbita. RESULTADOS: Todas as dificuldades inerentes ao procedimento, as complicações associadas, a curva de aprendizado e variações anatômicas foram coletados. CONCLUSÕES: A abordagem endoscópica da AEA na órbita de cadáveres mostrou-se factível. A identificação da artéria é fácil e a técnica evita incisões externas. Este acesso parece ser uma excelente alternativa para a abordagem da AEA. Estudos clínicos futuros são necessários para comprovar os benefícios desta técnica.

Anterior ethmoidal artery (AEA) ligation may be necessary in cases of severe epistaxis not controllable with traditional therapy. Endoscopic endonasal ligation of the AEA is not used frequently; there are few studies in the literature for standardization of the endoscopic technique for this vessel. AIM: To demonstrate the feasibility of periorbital AEA ligation in a transethmoidal endoscopic approach. METHODS: A prospective study where 50 nasal cavities were dissected. After anterior ethmoidectomy and partial removal of lamina papyracea, the periorbital area was carefully dissected along a subperiosteal plane to identify the AEA. The vessel was exposed within the orbit and dissected. RESULTS: Data on technical difficulties, complications, the learning curve and anatomical variations were gathered. CONCLUSION: An endonasal endoscopic approach to the AEA within the orbit was shown to be feasible. Identifying the artery is not difficult, and this technique avoids external incisions. This approach appears to be an excellent alternative for approaching the AEA. Further clinical studies are needed to demonstarte the benefits of this technique.

PURPOSE: To translate and culturally adapt the Dysfunctional Voiding Symptom Score (DVSS), questionnaire into Brazilian Portuguese. MATERIALS AND METHODS: The 10-item Dysfunctional Voiding Symptom Score (DVSS) was translated into Brazilian Portuguese according to a standard methodology: translation, synthesis, back-translation, Expert Committee, and pre-testing. After the translation process the final version was pre-tested and patient responses were analyzed to identify necessary modifications. Reliability was evaluated using the test-retest method, and internal consistency was assessed using Cronbach’s alpha. RESULTS: The Cronbach’s alpha coefficient was calculated in the test and retest phases. Internal consistency was found to be satisfactory, as confirmed by a Cronbach’s alpha coefficient of 0.76 for the test and 0.77 for the retest. A high degree of stability was found in the test/retest, with an intraclass correlation coefficient (ICC) of 0.960 (p < 0.001; 95% CI: 0.943-0.972). CONCLUSION: The cross-cultural adaptation process of the Dysfunctional Voiding Symptom Score questionnaire to be used on Brazilian children was successfully completed following internationally accepted methodologies.

OBJETIVOS: O objetivo deste estudo é determinar a especificidade, sensibilidade e a acurácia da ultra-sonografia (USG) intra-oral e transcutânea no diagnóstico de celulite e abscesso periamigdalianos. FORMA DE ESTUDO: Clínico Prospectivo. MATERIAL E MÉTODO: Trinta e nove pacientes foram atendidos no pronto-socorro de otorrinolaringologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo com diagnóstico clínico de celulite ou abscesso periamigdaliano. Em todos os pacientes, após a avaliação inicial, foram realizadas ultra-sonografias intra-oral e transcutânea. RESULTADOS: O USG intra-oral foi realizado em 35 casos e demonstrou sensibilidade de 95,2%, especificidade de 78,5% e a acurácia de 86,9%. A USG transcutânea foi factível em todos os 39 pacientes e diagnosticou abscesso periamigdaliano em 53,8% dos pacientes. A sensibilidade foi de 80%, a especificidade de 92,8% e a acurácia de 84,5%. CONCLUSÕES: O USG intra-oral foi bastante sensível no diagnóstico de abscessos periamigdalianos. O USG transcutâneo obteve especificidade superior ao intra-oral. Porém, quando o USG transcutâneo foi realizado em pacientes com trismo, este diagnosticou todos os abscessos periamigdalianos, já que se tratava de coleções grandes, comuns em pacientes com trismo. Estes exames tiveram acurácia semelhantes.

AIMS: The objective of the present study was to determine the specificity, sensitivity and accuracy of intraoral and transcutaneous ultrasound (US) in the diagnosis of peritonsillar cellulitis and abscess. STUDY DESIGN: Clinical-Prospective. MATERIAL AND METHODS: Thirty nine patients were seen at the otorhinolaryngology emergency department of the University Hospital, of the School of Medicine, University of São Paulo, with a clinical diagnosis of peritonsillar cellulitis or abscess. After initial evaluation, all patients were submitted to intraoral and transcutaneous US. RESULTS: Intraoral US was performed on 35 cases and its sensitivity was of 95.2%, the specificity was of 78.5% and the accuracy was of 86.9%. Transcutaneous US was feasible in all 39 patients and diagnosed peritonsillar abscess in 53.8%. There were 5 false-negatives and 1 false-positive result, sensitivity was 80%, specificity was 92.8% and accuracy was 84.5%. CONCLUSION: Intraoral US was quite sensitive in the diagnosis of peritonsillar abscesses when performed by an experienced radiologist. Specificity was higher for transcutaneous US compared to intraoral US. However, when transcutaneous US was performed in patients with trismus, it was able to diagnose all peritonsillar abscesses, since they were large collections which are common in patients with trismus. These exams showed similar accuracy.

O Streptococcus beta-hemolítico do grupo A (SGA) é o agente etiológico mais comum das faringoamigdalites (FA). O diagnóstico etiológico correto e tratamento adequado evitam complicações supurativas e não-supurativas da faringoamigdalite estreptocócica, entretanto, métodos clínicos de diagnóstico não são confiáveis. Os métodos rápidos de detecção do antígeno do SGA podem ser utilizados no diagnóstico deste agente e evitar uso indevido de antibióticos. OBJETIVOS: Os autores objetivaram avaliar a sensibilidade e especificidade dos testes rápidos para detecção do antígeno do SGA em nosso meio. FORMA DE ESTUDO: Clínico prospectivo. METODOLOGIA: Oitenta e um pacientes com faringoamigdalite aguda, atendidos no PS-ORL do Hospital das Clínicas da FMUSP, no período de maio de 2001 a abril de 2002, foram submetidos a duas coletas simultâneas de material de orofaringe com swabs. O teste rápido de detecção do SGA foi confrontado com a cultura em placa agar-sangue ("gold standard" para o diagnóstico etiológico). RESULTADOS: De 81 pacientes, 56% tiveram teste rápido positivo e 44% negativo; 40.7% apresentaram crescimento de SGA na cultura; a sensibilidade e especificidade do teste rápido foram, respectivamente, 93,9% e 68,7%. O valor preditivo negativo e positivo foram, respectivamente, 94,2% e 67,4%. CONCLUSÕES: A alta sensibilidade do exame permite utilizá-lo com intuito de identificar pacientes com SGA. Os testes de detecção rápida do antígeno estreptocócico se mostraram uma importante arma coadjuvante no diagnóstico etiológico das faringoamigdalites.

Group A beta-hemolytic streptococcus (GAS) is an important pharyngotonsillitis etiologic agent. Correct etiologic diagnosis and early treatment prevent suppurative and non-suppurative complications of streptococcal pharyngotonsillitis, however, clinical diagnostic methods are not reliable. Within this context, rapid detection methods of GAS antigen are useful to diagnose this agent. AIM: The objective of the present study was to determine the sensitivity and specificity of rapid GAS antigen detection tests used in Brazil. STUDY DESIGN: Clinical prospective. METHODS: Eighty-one patients with clinical diagnosis of acute pharyngotonsillitis seen at the otorhinolaryngology emergency department of University Hospital, FMUSP, between May 2001 and April 2002, were submitted to two simultaneous collections of oropharyngeal material using swabs. Rapid GAS antigen detection test was compared to culture on blood agar, the gold standard for the diagnosis of this etiologic agent. RESULTS: Among the 81 studied patients, the rapid test was positive in 56% and negative in 44%. GAS growth in culture was observed for 40.7% of the patients. The sensitivity and specificity of the rapid test were, respectively, 93.9% and 68.7%, and the negative and positive predictive values were 94.2 and 67.4%, respectively. CONCLUSIONS: We concluded that the high sensitivity of the test permits its use for the identification of patients with GAS. Rapid streptococcal antigen detection tests have been shown to be an important supporting tool in the etiologic diagnosis of pharyngotonsillitis.
