ABSTRACT. There is currently a lack of information in the literature on the integrated production of macauba (Acrocomia aculeata) and soybean (Glycine max) crops, and the importance of expanding integrated production systems; therefore, the objective of this work was to identify the effects of macauba shading on growth, development, and productivity of different soybean cultivars. The experiment was carried out in a randomized block design in a factorial scheme (4 × 2), with four cultivars and two production systems (monoculture and crop-forest integration), and four replicates per treatment. All soybean cultivars in the crop-forest integration system showed plant stagnation due to the shade level of the palm trees, as well as a reduction in the leaf area index and chlorophyll content in the leaves. Regarding reproductive parameters, grain, and oil yield, the cultivars responded differently between the production systems, revealing an interaction between the genotype and the environment. Our methodology was not favorable to soybean production; therefore, the management of spacing between palm trees and the selection of soybean genotypes that are more adapted to shaded environments are strategies that can allow for the integrated production of these species.

Abstract Background Amaranthus palmeri is an economically important plant species worldwide. The rapid growth and competitive potential of crops make A. palmeri a major problem. Studies on the dissemination potential of this weed in Brazil and worldwide are necessary to identify the regions with high climatic potential. Similarly, we analyzed the behavior of the species in the face of predicted climate change. Studies of this type can be performed using ecological niche modeling. Objective This work aimed to determine areas with climatic suitability for A. palmeri in the present and future climates in Brazil and globally. Methods We projected the potential distribution of A. palmeri based on the environmental requirements and stress parameters that limit this species in Brazil. Results For the current climate, our model identified regions with favorable climatic suitability for A. palmeri on most continents. The results showed that the suitability of A. palmeri in the Brazilian territory will decrease owing to predicted climate change. The future model highlighted decreases in the suitable northern, northeastern, and midwestern areas. An annual study of the occurrence of A. palmeri using the weekly growth index predicted by the model showed great potential for the species throughout the year, with a decrease in the driest months (July to August), indicating the preference of the species for moist soils. Tropical and subtropical zones are currently experiencing a reduction in suitable areas because of climate change in northeastern Brazil and western Australia. Temperate zone sites have potential areas of expansion for A. palmeri (northern USA, Russia, and China) under climate change. Conclusions Based on the results of this study, management strategies should be planned to contain the global spread of A. palmeri.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
nbsp;
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

BACKGROUND Zika virus (ZIKV) was discovered in 1947 with the virus isolation from Rhesus monkey (Macaca mulatta) in Uganda forest, Africa. Old World Primates are involved in a sylvatic cycle of maintenance of this arbovirus, however a limited knowledge about the role of New World primates in ZIKV transmission cycles has been established. OBJECTIVE This work aimed to investigate the presence of enzootic circulation of ZIKV in New World Primates from three Brazilian states: São Paulo, Paraíba, and Paraná. METHODS We analyzed 100 non-human primate samples collected in 2018 and 2020 from free-ranging and captive environments from São Paulo (six municipalities belonging to Sorocaba region), Paraíba (João Pessoa municipality), and Paraná (Foz do Iguaçu municipality) using reverse transcriptase quantitative polymerase reaction (RT-qPCR) assays, indirect enzyme-linked immunosorbent assay (ELISA), and plaque reduction neutralization test (PRNT). FINDINGS All samples (n = 141) tested negative for the presence of ZIKV genome from tissue and blood samples. In addition, all sera (n = 58) from Foz do Iguaçu’ non-human primates (NHPs) were negative in serological assays. MAIN CONCLUSION No evidence of ZIKV circulation (molecular and serological) was found in neotropical primates. In addition, the absence of antibodies against ZIKV suggests the absence of previous viral exposure of NHPs from Foz do Iguaçu-PR.

Abstract Introduction: Immobility is associated with adverse outcomes such as loss of functional capacity and longer hospitalization. Objective: To assess intra-hospital mobility at admission as a predictor of loss of functional capacity during older adults´ hospitalization. Methods: A prospective cohort study was conducted, and personal and hospital related risk factors were assessed at admission and discharge. To determine whether Short Physical Performance Battery (SPPB) on admission could predict loss of functional capacity during hospitalization, a ROC curve was performed and area under the curve (AUC) was calculated. Binary logistic regression models were used to identify predictors of loss of functional capacity. Model 1 contained only SPPB. Model 2 SPPB was matched with age, sex, instrumental activity of daily living (IADL), cognition, depression and surgery. Data were entered into SPSS version 18.0. Results: 1,191 patients were included with a mean age of 70.02 (± 7.34). SPPB cutoff point of 6.5 (sensitivity 62%, specificity 54%) identified 593 (49.8%) patients at risk for functional loss. In logistic regression, SPPB alone showed prediction of functional loss (p < 0.001, OR 1.8, 95% CI = 1.5-2.5) between admission and discharge. Model 1 explained between 22 to 32% of the variation in functional capacity. In Model 2, three variables contributed to the loss. SPPB 6.5 increased 1.8 times (95% CI = 1.3-2.4), being a woman increased 1.4 times (95% CI = 1.0-1.8) and not having surgery increased 2 times (95% CI = 1.4-2.8) the chance of having functional loss during hospitalization. Conclusion: SPPB is a good instrument to predict loss of functional capacity in hospitalized older adults.

ABSTRACT The purpose of the study was to analyze the influence of the elements communication channels, social system and time on the diffusion of the Electronic Health Record in Brasil. A qualitative case study was conducted based on innovation diffusion theory, which contains 17 interviews and observations of the daily work of eight family health teams involved in the use of technology in a city of western Minas Gerais. The data were processed by thematic-categorial content analysis and systematized with the software Atlas Ti. Communication means and interpersonal communication channels were found to strengthen technology diffusion. Acceptance of the innovation was fostered by the nurse, who encouraged and mobilized the team in this direction. It was observed that some factors contributed to resistance. For example, doctors and other professionals with longer work hours considered that the technology was complex and the time available insufficient. Diffusion of technology was also affected by strategies deriving from the social system context: creation of typing scales, reliefs and agreements, among others. Some challenges have emerged: coexistence of technologies related to the Electronic Health Record and the clinical record, conflicts, structural and training deficiencies, and underused technology. It was found that the elements communication channels, time and social system influence innovation diffusion, mainly by fostering its adoption in the context studied.

Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

ABSTRACT The use of mobile phones has dramatically increased all over the world. Such revolution in the communication amongst individuals has a great impact in patient care, supporting their self-management and promoting shared responsibility with health services. Given that improved communication facilitates compliance with scheduled procedures and reduces surgical cancellations, the current work aims to develop a communication tool named Surgery Remember@ to mitigate surgical suspensions due to patient absenteeism. The present article is a study of technological production divided into four chapters: literature review; analysis of the hospital administrative profile; software development; and process mapping for software implementation. Taking into account that in the last three years the problem of absenteeism was the main cause of institutional surgical cancellations; the development of Surgery Remember@ endeavours to reduce surgery cancellations, improving efficiency and reducing costs. It is known that sending messages three days before the surgical procedure makes it possible to replace patients in the event of cancellations, optimizing the human and material resources in the operating room. The confirmation of the pre-aesthetic consultation is also positive, for it allows the verification of perioperative assistance improvement. Hence, besides being viable and easy to implement, the software developed allows the addition of other features based on user requirements, proving to be an asset to reduce surgery cancellations.

Resumo: Introdução: A educação interprofissional (EIP) desenvolve competências colaborativas, aprimora a segurança do paciente e melhora a qualidade da atenção à saúde. Utilizou-se a Readiness for Interprofessional Learning Scale (RIPLS) via formulário eletrônico.
Abstract: Introduction: Interprofessional Education (IPE) helps develop collaborative skills, enhance patient safety and improve the quality of health care. Readiness for shared learning is directly related to IPE. Objective: To analyze Readiness for IPE among students, according to the cycles and courses. Method: This cross-sectional, descriptive study adopted a quantitative approach and used the Readiness for Interprofessional Learning Scale (RIPLS) via an electronic form. Mann-Whitney and chi-square tests were used to analyze the continuous and categorical variables, respectively. Analyses to ascertain differences between scores for the factors (1) Teamwork and Collaboration (TEC), (2) Professional Identity (IP), (3) Patient-Centered Health Care (ACP), and the overall RIPLS score, used the Kruskal-Wallis test to compare courses and cycles. Result: A response rate of 32.6% was attained from a sample of 506 students from the Nursing, Pharmacy, Physiotherapy, Speech Therapy, Medicine, Nutrition, Dentistry and Occupational Therapy courses of a Brazilian public university. The scores for factors 1, 2 and 3, and the overall RIPLS score did not differ between genders. Pharmacy students reported a lower Readiness for factor 3 (ACP) compared to students in Nursing, Physiotherapy, Medicine, Dentistry and Occupational Therapy (p=0.007). Speech therapy and Nutrition reported lower scores for the same factor compared to Dentistry and Occupational Therapy (p=0.007). Pharmacy students (p=0.004) and medicine students (p=0.016) were less likely to obtain a higher score in factor 1(TEC), while Occupational Therapy students were more likely to report greater Readiness in the same factor (p=0.024). In factor 2 (IP), fifth grade students were less likely to have positive attitudes (p=0.046). It was observed that Occupational Therapy students showed a more favorable attitude towards IPE expressed both in factor 3 (p=0.034) and in the overall score (p=0.027), while students in the Pharmacy course were less likely to report better readiness in factor 3 (p=0.003) and in the overall score (p=0.003). Conclusion: Considering the relevance of IPE in the process of redirecting health care training to build comprehensive care and ensure alignment with the Unified Health System, this study aims to reflect on the differences in readiness for IPE between undergraduate courses in health area.

ABSTRACT BACKGROUND: Being active has been shown to have beneficial effects for the health of individuals with chronic diseases. However, data on the association between multimorbidity and physical activity are limited. OBJECTIVE: To investigate the association between chronic diseases, multimorbidity and insufficient physical activity among older adults in southern Brazil, according to sex. DESIGN AND SETTING: Cross-sectional population-based and household-based study derived from the second wave (2013-2014) of the EpiFloripa Aging Cohort Study. METHODS: Insufficiency of physical activity (outcome) was ascertained using the long version of the International Physical Activity Questionnaire (≤ 150 minutes/week). Eleven self-reported chronic diseases were identified. Multimorbidity was defined from the number of chronic diseases (none; 2 or 3; or 4 or more). The adjustment variables were age, schooling, marital status, income, smoking, alcohol consumption and cognition. Additionally, each chronic disease was adjusted for the others. Associations were tested using logistic regression (crude and adjusted). RESULTS: Among the 1197 participants (≥ 63 years), women (54.0%) were more likely than men (39.6%) to be insufficiently active. In the adjusted analysis, women and men with depressive symptoms, and men with diabetes, were more likely to be insufficiently active than those without symptoms. Multimorbid women were more likely to be insufficiently active, and the magnitude of the effect was strongest for 4 or more diseases. CONCLUSION: This study indicates that the associations were sex-specific. Depressive symptoms and multimorbidity were associated with insufficient physical activity among women, while diabetes was associated with insufficient physical activity among men.
