OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

Objetivos. Analisar perfil bioquímico e caracterizar síndrome metabólica (SM) em pacientes com acompanhamento cardiológico, conforme critérios de NCEP-ATPIII e IDF. MÉTODOS: Foram estudados 200 pacientes e 140 controles, considerando colesterol total (CT), fração de colesterol de lipoproteína de alta (HDLc), baixa (LDLc) e muito baixa densidade (VLDLc), triglicérides (TG), glicemia de jejum, cintura abdominal e hipertensão arterial sistêmica (HAS). Admitiu-se nível de significância P<0,05. RESULTADOS: Pacientes apresentaram níveis elevados de glicemia (103±31,4mg/dL) e reduzidos de HDLc (48±13,4mg/dL) comparado aos controles (88±29,7mg/dL, P<0,0001; 53±15,9mg/dL, P=0,0075; respectivamente). Controles do sexo masculino, entre 31 e 50 anos, mostraram níveis aumentados de CT (215±40,4mg/dL), LDLc (134±34mg/dL), VLDLc (30±11,8mg/dL) e TG (150±59,4mg/dL) comparado às mulheres (185±38,2mg/dL, P=0,0137; 111±35,8mg/dL; P=0,0324; 19±9,7mg/dL; P=0,0009; 93±49mg/dL, P=0,0010; respectivamente). Mulheres acima dos 50 anos mostraram aumento de CT (216±35,9mg/dL), HDLc (54±12,8mg/dL) e LDLc (138±30,8mg/dL) comparado aos homens (190±44,7mg/dL, P=0,0103; 47±14,5mg/dL, P=0,0229; 119±33,3mg/dL; P=0,0176; respectivamente). Os critérios NCEP-ATPIII e IDF caracterizaram SM em 35,5% e 46% dos pacientes, respectivamente, com destaque para glicemia, TG e HAS. CONCLUSÃO: Níveis elevados de glicemia e reduzidos de HDLc destacam-se nos pacientes. Perfil lipídico alterado, em homens entre 31 e 50 anos, confere maior risco para doenças cardiovasculares em adulto jovem, enquanto padrão semelhante nas mulheres, após essa faixa etária, pode refletir alterações hormonais fisiológicas. Ambos os critérios para diagnóstico de SM distinguem pacientes de controles, destacando-se IDF, com parâmetros, no entanto, nem sempre associados a alto risco. Alta prevalência de SM em pacientes, mesmo com acompanhamento cardiológico, sugere predisposição para manifestações cardiovasculares em indivíduos brasileiros.

OBJECTIVE: To analyze the biochemical profile and to characterize metabolic syndrome (MS) in patients with cardiologic medical assistance using NCEP-ATPIII and IDF definitions. METHODS: Two hundred patients and 140 controls were studied, considering total cholesterol (TC), HDL-cholesterol (HDLc), LDL-cholesterol (LDLc), VLDL-cholesterol (VLDLc), triglycerides (TG), fasting glycemia, abdominal waist and hypertension. Significance level was defined as P<0.05. RESULTS: Patients showed increased glycemia levels (103±31.4mg/dL) and reduced HDLc levels (48±13.4mg/dL) when compared to controls (88±29.7mg/dL, P<0.0001 and 53±15.9mg/dL, P=0.0075; respectively). Male controls 31-50 years old showed increased TC levels (215±40.4mg/dL), LDL-cholesterol (134±34mg/dL), VLDL-cholesterol (30±11.8mg/dL) and TG (150±59.4mg/dL) when compared to women (185±38.2mg/dL, P=0.0137; 111±35.8mg/dL; P=0.0324; 19±9.7mg/dL; P=0.0009; 93±49mg/dL, P=0.0010; respectively). Women over 50 years of age showed increased TC concentrations (216±35.9mg/dL), HDL-cholesterol (54±12.8mg/dL) and LDL-cholesterol (138±30.8mg/dL) when compared to men (190±44.7mg/dL, P=0.0103; 47±14.5mg/dL, P=0.0229; 119±33.3mg/dL; P=0.0176; respectively). NCEP-ATPIII and IDF definitions had characterized MS in 35.5% and 46% of patients, respectively, bolding glycemia, TG and hypertension. CONCLUSION: Elevated glycemia levels and reduced HDLc levels were detected in patients. Altered lipid profile observed in men 31-50 years old signals higher risk for cardiovascular diseases in young adults, while a similar profile in aged women can reflect hormonal physiological changes. Both definitions for MS diagnosis discriminate patients from controls, especially IDF, sometimes with lower capacity to determine high risk for cardiovascular complications. The high prevalence of MS in patients, even with cardiologic medical assistance, suggests predisposition for cardiovascular manifestations in Brazilian individuals.

O presente estudo relata a experiência operatória de pacientes portadores da fístula arterial coronária (FAC) entre a artéria coronária interventricular anterior e o tronco da pulmonar. Foram operados cinco pacientes, o sexo feminino foi mais freqüente, com 60% dos casos, e a idade variou de 40 a 46 anos. O ecocardiograma de stress e a cineangiocoronariografia foram realizados em todos pacientes. No pós-operatório, não houve mortalidade e presença de sintomas. Consideramos que procedimento operatório é a primeira escolha no tratamento da FAC, uma vez que previne as complicações do "shunt" artério-venoso, com segurança e eficácia.

The current study reports on the operative experience of patients with coronary artery fistulas between the anterior intraventricular coronary artery and pulmonary trunk. Of the five patients operated, 60% were women and the ages ranged form 40 to 46 years old. Stress echocardiogram and coronary cineangiography were accomplished for all patients. No deaths occurred in the postoperative period nor were symptoms reported. We believe that the surgical procedure is the first choice treatment for coronary artery fistulas, as it safely and effectively prevents the complications of arteriovenous shunts.

OBJETIVO: Analisar evolução e a influência prognóstica dos níveis séricos da troponina I cardíaca nos pacientes com insuficiência cardíaca congestiva (ICC) submetidos a ressincronização interventricular (RV), com seguimento de até 59 meses. MÉTODOS: Foram analisados 33 pacientes com miocardiopatia dilatada idiopática em classe funcional III/IV da NYHA, submetidos a RV. A qualidade de vida (QV) foi analisada pré e pós-operatoriamente com o Minnesota Code e a função ventricular através da ecocardiografia. Os níveis séricos da troponina I foram dosados em 23 pacientes, utilizando o teste exato de Fischer para avaliar sua relação com o evento óbito, e a curva de Kaplan-Meier para análise da taxa de sobrevivência. RESULTADOS: A QV foi significantemente melhor após a RV, com mediana de 73 pontos, no pré e 36, no pós (p<0,0001). O diâmetro diastólico do VE (DDVE) apresentou redução de 65mm, no pré para 60mm, no pós (p=0,0014), com aumento da fração de ejeção (FEVE) de 37 para 47% (p=0,0004). Nos 15 pacientes com valores normais de troponina, não ocorreram óbitos e dos oito pacientes com valores elevados, seis faleceram (p=0,0003). A curva atuarial de livres de óbitos mostrou uma taxa de sobrevivência de 47,1±13,3%, ao final de 59 meses. CONCLUSÕES: A RV em pacientes com ICC melhora a QV e os parâmetros ecocardiográficos. Constitui uma boa alternativa para os pacientes em classe funcional III/IV NYHA. Os níveis séricos elevados da troponina I cardíaca foram preditores de risco para óbito.

OBJECTIVE: To analyze the evolution and prognostic influence of the cardiac troponin I serum levels in patients with congestive heart failure (CHF) submitted to interventricular resynchronization (VR) over a 59-month follow-up period. METHOD: Thirty-three patients with idiopathic dilated myocardiopathy in NYHA functional classes III and IV were submitted to VR. The pre- and post-operative quality of life (QV) was analyzed using the Minnesota Code and the left ventricle function was assessed by echocardiography. The cardiac troponin I levels were compared in 23 patients utilizing the Fisher exact test to analyze the correlation with death and the Kaplan-Meier curve was used to analyze the survival rate. RESULTS: The QV was better after VR with a median of 73 points in the pre-operative period and 36 in the postoperative period (p-value < 0.0001). The left ventricle diastolic diameter (LVDD) reduced from 65 mm in the preoperative period to 60 mm in the postoperative period (p-value = 0.0014) with an increase in the ejection fraction from 37 to 47% (p-value = 0.0004). In 15 patients with normal cardiac troponin I levels, no deaths occurred and of the 8 patients with high levels, six died (p-value = 0.0003). The actuarial survival curve showed a survival rate of 47.1 ± 13.3% at the end of 59 months. CONCLUSION: VR in patients with CHF improves the QV and echocardiographic parameters (ejection fraction and LVDD). It is a good alternative for functional class III and IV patients. The serum levels of cardiac troponin I are predictors of risk to life.

OBJETIVO: Avaliar a importância dos níveis séricos de troponina I-cardíaca no pré-operatório de pacientes portadores de obstrução no tronco da artéria coronária esquerda (OTCE) sem evento cardíaco prévio. MÉTODO: Foram analisados os níveis séricos da troponina I-cardíaca de 115 pacientes com doença coronariana obstrutiva, com idade variando entre 32 e 81 anos (média e desvio-padrão de 59,7±10,5 anos). Os pacientes foram divididos em dois grupos: Grupo A - 41 pacientes portadores de OTCE, variando o grau de obstrução entre 20% de perda do diâmetro e subtotal (moda de 60%); Grupo B - 74 pacientes sem OTCE. Todos os pacientes foram submetidos a cineangiocoronariografia de forma eletiva e não apresentavam infarto agudo do miocárdio prévio. O método empregado na dosagem da troponina-I cardíaca foi o da Quimioluminiscência, admitindo-se como valor normal abaixo de 0,1 nanogramas por mililitro (ng/ml). RESULTADOS: Não houve correlação entre o grau de OTCE e os níveis séricos de troponina-Ic (P= 0,4617), porém a média dos níveis séricos da troponina-I nos grupos A e B foi, respectivamente, 0,3841 ng/ml e 0,1711 ng/ml (P=0,0324); teste de Mann-Whithey; OR= 4,44 (IC95% 1,60 _ 12,31). CONCLUSÕES: Os pacientes do grupo A têm 3,44 vezes mais chance de apresentar lesão miocárdica representada por elevação de troponina I-cardíaca que o grupo B, independente do grau da OTCE. A sensibilidade para suspeita clínica de mionecrose foi relativamente baixa (31,7%), porém a especificidade foi elevada (90,5%). Destaca-se, entretanto, a importância clínica da documentação de mionecrose em uma determinada porcentagem de pacientes com lesão de tronco, sem constatação eletrocardiográfica. Assim, os pacientes com OTCE devem ser submetidos com rapidez a procedimento operatório, a fim de evitar extensão da mionecrose.

OBJECTIVE: To evaluate the importance of cardiac troponin-I serum levels in the preoperative period of patients suffering from left coronary branch obstruction but without prior cardiac events. METHOD: The cardiac troponin-I serum levels of 115 patients with obstructive coronary disease were analyzed. The ages of the patients varied between 32 and 81 years old with a mean age and standard deviation of 59.7 ± 10.5 years. The patients were divided into two groups: Group A, 41 patients suffering from left coronary branch obstruction with the degree of obstruction varying between 20% to subtotal occlusion (about 60%) and Group B, 74 patients without left coronary branch obstruction. All the patients were submitted to catheterism and no evidence of previous acute myocardial infarction (AMI) was identified. Chemoluminescence was utilized to measure the cardiac troponin-I level using the Sanofi-Pasteur Access apparatus, with values of less than 0.l nanograms per milliliter (ng/ml) considered normal. RESULTS: No association was evidenced between the degree of left coronary branch obstruction and troponin-Ic serum levels (P= 0.4617), however the average serum levels of troponin-I, in Groups A and B were 0.3841 ng/mL and 0.1711 ng/mL respectively (P=0.0324 Mann-Whitney test; OR = 4.44 95% CI 1.60 - 12.31). CONCLUSIONS: The patients of Group A have 3.44 times higher chance of presenting with myocardial injury as identified by increased cardiac troponin-I levels than Group B, independent of the degree of left coronary branch obstruction. The sensitivity of clinical suspicion of myonecrosis was relatively low (31.7%), but the specificity was high (90.5%). However the clinical importance of the documentation of myonecrosis in a determined percentage of patients with branch injury without electrocardiographic evidence is stressed. Thus, patients with left coronary branch obstruction should be quickly submitted to operative procedures, in order to avoid worsening of the myonecrosis.

OBJETIVOS: O presente estudo visa avaliar a evolução a longo prazo (até 12 anos de seguimento) de pacientes submetidos a substituição valvar aórtica por bioprótese de pericárdio bovino. MÉTODO: De março 1992 a janeiro 2003, 287 pacientes foram submetidos a substituição valvar aórtica como procedimento isolado, utilizando-se bioprótese de pericárdio bovino. Destes, 189 (65,9%) eram do sexo masculino. As idades variaram de 15 a 82 anos com média e desvio padrão de 53,6±15,1 anos e mediana de 56 anos. O diâmetro das biopróteses variou de 21 a 29 mm, sendo que os mais prevalentes foram o de 23 mm (105 casos; 36,6%) e o de 25 mm (outros 105 casos; 36,6%). Apenas um paciente recebeu prótese com diâmetro de 29 mm. Os eventos estudados foram sobrevivência tardia global, sobrevivência comparativa em pacientes abaixo de 70 anos e com 70 anos ou mais e porcentagem livre de reintervenções por qualquer causa de disfunção primária da bioprótese. A análise dos resultados foi feita com auxílio de curva atuarial Kaplan-Meier. RESULTADOS: A taxa atuarial de sobrevivência global ao final de 12 anos foi de 91,7±2,2%. A separação em grupos abaixo de 70 anos (Grupo A= 252 pts) e 70 anos ou mais (Grupo B= 35 pts) mostrou que, no grupo A, a sobrevivência global foi de 94,7±1,7%, enquanto que no grupo B foi de 58,1±17,2% (Logrank test p= 0,0005; Hazard Ratio 0,20 IC95% 0,01 a 0,29). A taxa livre de reintervenções por causa primária da bioprótese foi de 96,1±2,0%, ao final de 12 anos. Os quatro pacientes que apresentaram disfunção pertenciam ao grupo A, com média de idade de 49,7 anos. CONCLUSÕES: O uso da bioprótese de pericárdio bovino em portador de doença valvular aórtica proporciona excelente taxa de sobrevivência após 12 anos de seguimento. Pacientes com idade igual ou superior a 70 anos apresentaram sobrevivência significativamente menor, mas as disfunções ocorreram exclusivamente no grupo de menor idade.

OBJECTIVES: The present study is aimed at evaluating the long-term outcomes (up to 12 years of follow-up) of patients undergoing aortic valve replacement using bovine pericardial prostheses. METHOD: From March 1992 to January 2003, 287 patients underwent aortic valve replacement as a single procedure, using bovine pericardial prostheses. Of these, 189 (65.9%) were males. Ages ranged from 15 to 82 years with a mean and standard deviation of 53.6 ± 15.1 years and median of 56 years. The diameters of the bioprostheses ranged from 21 to 29 mm, of which 23 mm (105 cases, 36.6%) and 25 mm (105 cases, 36.6%) were the most prevalent sizes. Only in 1 patient was a 29-mm prosthesis implanted. The assessed variables were late overall survival, comparative survival of patients < 70 years and ³ 70 years of age and the percentage of patients free from reoperations due to primary valve failure. Statistical analysis was performed with the aid of actuarial curves (Kaplan-Meier). RESULTS: The overall actuarial survival at the end of 12 years was 91.7 ± 2.2%. Separate analysis of patients < 70 years (Group A= 252 patients) and ³ 70 years (Group B= 35 patients) showed the overall survival in Group A was 94.7 ± 1.7% and 58.1 ± 17.2% in Group B (Logrank test P= 0.0005; Hazard Ratio 0.20 95% CI 0.01 to 0.29). The rate free from reoperation due to primary valve failure was 96.1 ± 2.0% at the end of 12 years. The 4 patients with implantation failure were in Group A and had a mean age of 49.7 years. CONCLUSIONS: The use of bovine pericardial prostheses in patients with aortic valve disease provides an excellent survival rate over 12 years of follow-up. Patients with 70 years and over had a significantly lower survival, but dysfunctions were only observed in the younger group of patients.

OBJECTIVE: To determine the immediate behavior and the prognostic value in terms of late survival of serum troponin I measurement in patients undergoing myocardial revascularization surgery with extracorporeal circulation. METHODS: We studied 88 random patients, 65 (73.8%) of the male sex, who underwent myocardial revascularization surgery with extracorporeal circulation. Troponin measurements were performed as follows: in the preoperative period, right after intensive care unit admission, and on the first and second postoperative days. Values below 0.1 nanogram per milliliter (ng/mL) were considered normal. The cut points for late prognostic assessment were 0.5 ng/mL; 1 ng/mL; 2.5 ng/mL; and 5 ng/mL. RESULTS: The serum troponin I levels were elevated on the first postoperative day, suggesting the occurrence of specific myocardial damage. Patients with a poor prognosis could be identified, because the serum levels above 2.5 ng/mL and 5 ng/mL in the postoperative period resulted, respectively, in mortality rates of 33% and 50% in a maximum 6-month follow-up. CONCLUSION: Troponin I values around 2.5 ng/mL in the postoperative period should call attention to the need for more aggressive diagnostic or therapeutical measures.

Objetivo: Apresentar a experiência com a realização de revascularização do miocárdio com técnica minimamente invasiva, via minitoratocotomia anterior esquerda sem circulação extracorpórea (MIDCAB). Casuística e Métodos: 46 pacientes foram submetidos a MIDCAB de fevereiro de 1997 a janeiro de 1998. Todos eram portadores de doença obstrutiva na porção proximal do ramo interventricular anterior (RIA). A técnica consistiu em pequena incisão, de cerca de 8,0 cm, transversal, na região inframamária esquerda, obtendo-se fácil acesso para dissecção da artéria torácica interna (ATI) e posterior anastomose com o RIA. A avaliação dos resultados foi feita com controle clínico, laboratorial e angiográfico. Resultados: O tempo médio de operação foi de 2 horas, com cerca de 18 horas de internação na UTI e permanência hospitalar máxima de 5 dias. Não ocorreram óbitos intra-operatórios. Houve 1 caso de morte súbita após alta (15º PO). Os pacientes acompanhados ambulatorialmente não referiram dor anginosa. Documentou-se 4 casos de oclusão da ATI e 2 estenoses do RIA pós anastomose, provavelmente decorrentes da bandagem desse vaso durante o procedimento. Não houve correlação entre níveis de troponina-I e oclusão do enxerto. Verificou-se, porém, que os níveis de troponina I dos pacientes submetidos à MIDCAB foram significativamente menores que no procedimento convencional. Não ocorreram complicações clínicas de significância. Conclusão: A MIDCAB demonstrou ser procedimento útil e de baixa morbidade e mortalidade, com possibilidade de menor permanência hospitalar e conseqüente menor custo. Os baixos níveis sangüíneos de troponina I indicam menor sofrimento miocárdico com o procedimento.

Objective: To present the experience with myocardial revascularization by Minimally Invasive Direct Coronary Artery Bypass Graft via left anterior minithoracotomy without extracorporeal circulation (MIDCAB). Material and Methods: 46 patients were submitted to MIDCAB from February 1997 to January 1998. Everyone has obstructive disease in the proximal portion of the interventricular descending artery (IDA). The technique consisted of a small transverse incision, of about 8.0 cm, traverse, in the left infra-mammary area, allowing good visibility and easy access for dissection of the thoracic internal artery (TIA) and anastomosis with IDA. The evaluation of the results was made by clinical, laboratory and angiographic control. Results: The mean duration of operation was 2 hours with about 18 hours of permanence in ICU and 5 days of maximum hospital stay. There were no intra-operative deaths. There was a case of sudden death after discharge (15º post-operative day). The patients followed did not refer anginal pain. Four cases of TIA occlusion and 2 AIA stenoses were documented, probably secondary to vessel banding during the procedure. There was no correlation between troponin-I blood levels and occlusion of the graft. It was verified that the levels of troponin-I in patients submitted to MIDCAB were significantly lower than in the conventional procedure. There was no significant clinical complication. Conclusions: MIDCAB has proved to be a useful procedure with low morbidity and mortality, with the possibility of shorter hospital stay and consequent lower cost. The low Troponin-I blood levels indicate reduced myocardial injury during the procedure.
