Abstract Introduction: Coronary artery bypass grafting (CABG) is the most frequently performed heart surgery in Brazil. Recent international guidelines recommend that national societies establish a database on the practice and results of CABG. In anticipation of the recommendation, the BYPASS Registry was introduced in 2015. Objective: To analyze the profile, risk factors and outcomes of patients undergoing CABG in Brazil, as well as to examine the predominant surgical strategy, based on the data included in the BYPASS Registry. Methods: A cross-sectional study of 2292 patients undergoing CABG surgery and cataloged in the BYPASS Registry up to November 2018. Demographic data, clinical presentation, operative variables, and postoperative hospital outcomes were analyzed. Results: Patients referred to CABG in Brazil are predominantly male (71%), with prior myocardial infarction in 41.1% of cases, diabetes in 42.5%, and ejection fraction lower than 40% in 9.7%. The Heart Team indicated surgery in 32.9% of the cases. Most of the patients underwent cardiopulmonary bypass (87%), and cardioplegia was the strategy of myocardial protection chosen in 95.2% of the cases. The left internal thoracic artery was used as a graft in 91% of the cases; the right internal thoracic artery, in 5.6%; and the radial artery in 1.1%. The saphenous vein graft was used in 84.1% of the patients, being the only graft employed in 7.7% of the patients. The median number of coronary vessels treated was 3. Operative mortality was 2.8%, and the incidence of cerebrovascular accident was 1.2%. Conclusion: CABG data in Brazil provided by the BYPASS Registry analysis are representative of our national reality and practice. This database constitutes an important reference for indications and comparisons of therapeutic procedures, as well as to propose subsequent models to improve patient safety and the quality of surgical practice in the country.

RESUMO: Este trabalho investigou as principais alterações clínicas e dermatopatológicas de equinos intoxicados natural e experimentalmente por Leucaena leucocephala. Os surtos ocorreram nos estados de São Paulo e Goiás, onde seis cavalos após a ingestão de casca e/ou folhas de L. leucocephala apresentaram alopecia, principalmente na crina e cauda. Nesses animais o diagnóstico foi baseado na observação da ingestão da planta e dos sinais clínicos. Nos animais intoxicados experimentalmente, foi realizado o exame clínico, biópsias da pele das regiões de crina, dorso e cauda e foi feita dosagem sérica de tri-iodotironina (T3) e tiroxina (T4). Alopecia da crina e cauda foi o principal sinal clínico observado, seguido de anorexia, emagrecimento e apatia em todos os equinos. Os níveis de T3 e T4 total apresentaram redução significativa (p≤0,05) na terceira semana de ingestão da leucena quando comparado aos níveis basais. As alterações histológicas observadas nas biópsias de pele demonstram acentuada telogenização dos folículos pilosos ao final dos experimentos. Os achados clínico-patológicos em equinos intoxicados são semelhantes aos observados em ruminantes. A intoxicação experimental de equinos por L. leucocephala evidenciou o acentuado declínio dos níveis dos hormônios tireoideanos, sugerindo efeito bociogênico da mimosina e seus compostos derivados, com hipotireoidismo transitório e alopecia devido à acentuada redução da atividade folicular.
ABSTRACT: This paper reports the main clinical changes of natural and experimentally poisoning by Leucaena leucocephala in horses. Outbreaks of poisoning occurred in São Paulo and Goiás States, where six horses after ingestion of bark and/or leaves of the plant showed alopecia mainly in mane and tail. The diagnosis was based on the observation of plant intake and clinical signs. In horses experimentally poisoned, clinical examination was performed and skin biopsies were taken from mane, back and tail regions, and triiodothyronine (T3) and thyroxine (T4) serum levels were assayed. Alopecia in mane and tail was the main clinical sign, accompanied by anorexia, weight loss and lethargy in all horses. T3 and total T4 showed significant decrease in serum levels (p≤0.05) in the third week of leucaena intake compared with baseline levels. Marked telogenization of hair follicles at the end of the experiments were observed in skin biopsies. The clinical and pathological findings in poisoned horses are similar to those observed in ruminants. Experimental poisoning of horses by the plant highlights the marked decline in levels of thyroid hormones, suggesting goitrogenic effect of mimosine and its derived compounds, with transient hypothyroidism and alopecia due to accentuated reduction in follicular activity.

Abstract Objective: To report the early results of the BYPASS project - the Brazilian registrY of adult Patient undergoing cArdiovaScular Surgery - a national, observational, prospective, and longitudinal follow-up registry, aiming to chart a profile of patients undergoing cardiovascular surgery in Brazil, assessing the data harvested from the initial 1,722 patients. Methods: Data collection involved institutions throughout the whole country, comprising 17 centers in 4 regions: Southeast (8), Northeast (5), South (3), and Center-West (1). The study population consists of patients over 18 years of age, and the types of operations recorded were: coronary artery bypass graft (CABG), mitral valve, aortic valve (either conventional or transcatheter), surgical correction of atrial fibrillation, cardiac transplantation, mechanical circulatory support and congenital heart diseases in adults. Results: 83.1% of patients came from the public health system (SUS), 9.6% from the supplemental (private insurance) healthcare systems; and 7.3% from private (out-of -pocket) clinic. Male patients comprised 66%, 30% were diabetics, 46% had dyslipidemia, 28% previously sustained a myocardial infarction, and 9.4% underwent prior cardiovascular surgery. Patients underwent coronary artery bypass surgery were 54.1% and 31.5% to valve surgery, either isolated or combined. The overall postoperative mortality up to the 7th postoperative day was 4%; for CABG was 2.6%, and for valve operations, 4.4%. Conclusion: This first report outlines the consecution of the Brazilian surgical cardiac database, intended to serve primarily as a tool for providing information for clinical improvement and patient safety and constitute a basis for production of research protocols.

RESUMO A utilização de óleos voláteis extraídos de frutos quiropterocóricos vem sendo investigada com o propósito de potencializar a chuva de sementes provocada por morcegos frugívoros em áreas degradadas. Como filostomídeos utilizam principalmente o olfato para localizar frutos maduros, teve-se, por objetivo, testar o efeito de óleos voláteis extraídos de frutos e folhas de Piper amalago L., espécie frequentemente consumida por esses animais, como atrativo para a captura. Os óleos voláteis foram extraídos por meio de aparelho Clevenger, usando-se a técnica de hidrodestilação. A eficiência do atrativo foi testada em uma área de Floresta Estacional Semidecidual no sul do Brasil. Dez redes de neblina, divididas em dois grupos, controle (sem atrativos) e teste (com atrativos), foram expostas mensalmente, de março a junho de 2012, durante três noites consecutivas. O esforço amostral correspondeu a 33.284m².h. Obteve-se 78% (39 capturas) do total de capturas para o período no grupo teste e 22% (11 capturas) no grupo controle. Os óleos voláteis de frutos e folhas de Piper amalago atraíram morcegos da espécie Sturnira lilium (E. Geoffroy, 1810), corroborando os estudos pioneiros realizados no Paraná.
ABSTRACT The use of volatile oils extracted from chiropterochoric fruits has been investigated so that the great number of seeds spread by fruit-eating bats in degraded areas may be potentialized. Since Phyllostomidae mainly use smell to locate ripe fruits, this current experiment tested the effect of volatile oils from the fruit and leaves of Piper amalago L., frequently consumed by these animals, as bait for their capture. The volatile oils were extracted by Clevenger apparatus by hydrodistillation technique. Attraction efficiency was tested in a Semidecidual Seasonal Forest in southern Brazil. Ten mist nets in two groups, control (without any bait) and test (with bait) were exposed every month between March and June 2012, during three consecutive nights. Sample effort was equal to 33,284m².h. Thirty-nine captures (78%) occurred for the test group during the period and 11 captures (22%) for the control group. The volatile oils from the fruit and leaves of Piper amalago attracted bats of the species Sturnira lilium (E. Geoffroy, 1810), confirming pioneer studies in the state of Paraná, Brazil.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

O crescimento da população idosa brasileira tem levado a uma maior tendência ao uso de medicamentos. A sua utilização inadequada pode ser induzida por vários fatores, dentre eles a propaganda, podendo trazer danos à saúde do usuário. O objetivo do estudo foi avaliar a influência da propaganda no consumo de medicamentos por um grupo de idosos atendidos em unidade básica de saúde de Aracaju, Sergipe. No período de abril a junho de 2007, um grupo de 230 idosos, com idade acima de 60 anos, de ambos os gêneros, foi entrevistado. A maioria dos entrevistados (73%) possuía pelo menos uma doença crônica e 73,9% consumiam pelo menos um medicamento regularmente. Do total de entrevistados, 17,8% da amostra relataram utilizar medicamentos por influência da propaganda; 2,2% consideraram que os medicamentos veiculados na mídia nunca fazem mal e 6,5% acreditavam que eles sempre fazem bem. No estudo, correlações foram feitas e demonstraram que quem mais consome medicamentos influenciados pela propaganda também considera que eles sempre fazem bem e vice-versa (p= 0,04). Os dados revelaram que parte dos idosos sofreu influência da propaganda no consumo de medicamentos, não levando em consideração os riscos que estes poderiam causar.
The growth of the Brazilian elderly people has led to a trend to an increase in the medication use. The inadequate use of drugs can be induced by some factors, like advertisement, with the risk of damaging the user's health. The objective of the study was to evaluate the advertisement influence in medication use in a group of elderly patients in a primary health care unit in Aracaju, Sergipe, Brazil. 230 elderly aging over 60 years with both genders had been interviewed from April to June of 2007. The majority of the interviewees (73%) has at least a chronic health condition and 73.9% consumed regularly at least one medication. 17.8% of the sample informed to use medication motivated by publicity influence; 2.2% had considered that the medication never cause damages and 6.5% believed that always it makes well. In this study, correlations have been made and demonstrated that those who presented a higher level of consumption influenced by advertising also think that drugs used are always beneficial and vice versa (p= 0.04). The data showed that part of elderly suffered influence of advertisement to medication use, and are not conscious of risks involved.

Novas terapias para hipertensão endocraniana

OBJETIVOS: São revistos os principais conceitos envolvidos na abordagem terapêutica da hipertensão endocraniana, desde a abordagem tradicional até a apresentação das novas terapias preconizadas. FONTES DOS DADOS: Foram revistas as principais bases de dados da literatura médica, particularmente o Medline e o Lilacs, nos últimos 10 anos, além de livros textos consagrados e teses dedicadas ao assunto. SÍNTESE DOS DADOS: A hipertensão endocraniana pode estar associada a diversas lesões neurológicas cerebrais, sejam de origem traumática, infecciosa ou metabólica, podendo, nos casos graves, se constituir em importante fator de morbimortalidade. O aumento da pressão intracraniana interfere com o fluxo sangüíneo cerebral, de modo que para se manter uma pressão de perfusão cerebral adequada, é necessário tanto a redução e o controle da hipertensão endocraniana, como o combate agressivo à hipotensão arterial, desde as fases iniciais do atendimento. A monitorização adequada é fundamental, e as principais medidas adotadas no tratamento convencional envolvem a elevação da cabeceira do leito a 30 graus, o posicionamento centrado da cabeça, a hipocapnia otimizada, o perfeito equilíbrio hidroeletrolítico e metabólico, a sedação, a terapia anticonvulsiva, a drenagem liquórica, o uso de barbitúricos, manitol e furosemida. As novas terapias que vêm sendo estudadas incluem, principalmente, o dexanabinol, as soluções salinas hipertônicas, a hipotermia moderada, a craniotomia descompressiva, a otimização da pressão de perfusão cerebral e a redução da pressão na microvasculatura cerebral (terapia de Lund). CONCLUSÕES: O uso de novas terapias para o controle eficaz da hipertensão endocraniana, particularmente em casos refratários ao tratamento usual, representam um cenário promissor na abordagem destes quadros.
OBJECTIVE: The main concepts involved in the therapeutic management of intracranial hypertension are revisited, including pathophysiology, monitoring, the traditional approach, and also the presentation of recently proposed therapies. SOURCE OF DATA: The main medical literature data bases (especially Medline and Lilacs) were searched for articles published in the last 10 years, and traditional text books and dissertations focusing the subject were consulted. SUMMARY OF THE FINDINGS: Intracranial hypertension may be associated with several cerebral neurologic lesions from traumatic, infectious or metabolic origin, and in severe cases may represent an important factor for morbidity and mortality. Increases in intracranial pressure interfere with cerebral blood flow; in order to maintain an adequate cerebral perfusion pressure, it is necessary to both reduce and control intracranial hypertension and combat low blood pressure from the very beginning of treatment. Adequate monitoring is essential, and the main points addressed in conventional management include elevation of the head of bed to 30° C, central positioning of the head, optimization of hypocapnia, perfect metabolic and hydro-electrolyte balance, sedation, anti-convulsive therapy, liquor drainage, and the use of barbiturics, mannitol and furosemide. New therapies currently under investigation include the use of dexanabinol, hypertonic saline solutions, moderate hypothermia, decompressive craniectomy, optimization of cerebral perfusion pressure, and reduction in cerebral microvasculature pressure (Lund therapy). CONCLUSIONS: The use of new therapies to effectively control intracranial hypertension, especially in cases that are refractory to the usual treatment, represent a promising scenario in the management of this problem.

CASUÍSTICA E METODOS: Entre janeiro de 1983 e maio de 1999, 12.405 pacientes com diagnóstico de infarto agudo do miocárdio (IAM) foram atendidos no InCor. Destes, 127 (1,02%) apresentaram rotura de parede livre do ventrículo esquerdo como complicação do IAM. A rotura miocárdica aguda ocorreu em 98 (77,1%) pacientes e a subaguda em 29 (22,9%). RESULTADOS: Foram operados 5 pacientes na forma aguda, com 80% de mortalidade e 19 na forma subaguda, com 15,8% de mortalidade. A sobrevida pós-operatória abrangendo os dois grupos foi de 70,8%. CONCLUSÃO: Concluímos que a rotura de parede livre do ventrículo esquerdo é uma grave complicação do IAM, necessitando de atuação imediata. Nas formas agudas, observa-se rápida deterioração hemodinâmica do paciente, muitas vezes sem tempo hábil para tentativa de correção cirúrgica. As formas subagudas podem ser detectadas e monitorizadas através de exames ecocardiográficos seriados após o IAM. Nestes casos a intervenção cirúrgica precoce, muitas vezes sem a necessidade de instalação de circulação extracorpórea, tem contribuído para a sobrevida da maioria dos pacientes.
MATERIAL AND METHODS: Between January 1983 and May 1999, 12.405 patients were treated by the surgical team of the Heart Institute (InCor) with the diagnosis of acute myocardial infarction (AMI). From these patients, 127 (1.02%) had left ventricular free wall rupture as an ischemic complication of the myocardial infarction. The cardiac rupture was acute in 98 patients (77.1%) and sub-acute in 29 (22.9%). RESULTS: Twenty-four patients were operated on, 5 on acute rupture with 80% of hospital mortality and 19 on sub-acute rupture with 15.8% of hospital mortality. The post-operative overall survival including both groups was 70.8%. CONCLUSION: The conclusion was drawn that left ventricular free wall rupture is a severe complication of acute myocardial infarction that needs an immediate action. In acute ruptures, most patients develop hemodynamic deterioration without enough time to try to proceed any surgical correction. The sub-acute cases can be detected and monitored through periodic ecocardiographic exams after the AMI. In these cases the early surgical intervention, many times without using extra-corporeal circulation, has been increasing the chances of survival of the majority of these patients.

Myocardial revascularization with coronary endarterectomy. Stratification of risk factors for early mortality

OBJECTIVE: To determine the risk factors for mortality related to myocardial revascularization when performed in association with coronary endarterectomy. METHODS: We assessed retrospectively 353 patients who underwent 373 coronary endarterectomies between January '89 and November '98, representing 3.73% of the myocardial revascularizations in this period of time. The arteries involved were as follows: right coronary artery in 218 patients (58.45%); left anterior descending in 102 patients (27.35%); circumflex artery in 39 patients (10.46%); and diagonal artery in 14 patients (3.74%). We used 320 (85.79%) venous grafts and 53 (14.21%) arterial grafts. RESULTS: In-hospital mortality among our patients was 9.3% as compared with 5.7% in patients with myocardial revascularizations without endarterectomy (p=0.003). Cause of death was related to acute myocardial infarction in 18 (54.55%) patients. The most significant risk factors for mortality identified were as follows: diabetes mellitus (p=0.001; odds ratio =7.168), left main disease (<0.001; 9.283), female sex (0.01; 3.111), acute myocardial infarction (0.02; 3.546), ejection fraction <35% (<0.001; 5.89), and previous myocardial revascularization (<0.001; 4.295). CONCLUSION: Coronary endarterectomy is related to higher mortality, and the risk factors involved are important elements of a poor outcome.
