Abstract Objective: To describe the external and middle ear of goats, using computed tomography and endoscopic dissection, comparing them with the anatomy of the human ear, aiming to evaluate the viability of this animal model for training in otological endoscopic surgery. Methods: Images of the external and middle ear of goats were produced by computed tomography in the coronal and axial planes and analyzed. The radiological anatomy of the external and middle goat and human ear are described. Some measures of surgical interest were described, such as the length of the external auditory canal, external and internal cross-sectional area of the external auditory canal, middle ear depth, vertical angle of the external auditory canal, length of malleus and incus. After the tomographic study, we analyzed the importance of evaluations through the endoscopic dissection of the goat ear. Results: The bony canal is extremely tortuous and shows a bony prominence with significant elevation near the lower edge of the tympanic membrane. The middle ear has a large tympanic bulla in the hypotympanum region. The malleus is shaped similarly to that of the human, but its handle is located anterior to its head. The mastoid is not pneumatized. Compared to the human mastoid, there was a statistically significant difference in the external and internal cross-sectional area, at the depth of the middle ear, in the vertical angle of the canal and in the length of the incus. Conclusion: Goat morphometric anatomy showed that this model is suitable for surgical simulation in the initial training for endoscopic otological surgery. It is an important contribution to the development of skills to accelerate the subsequent endoscopic otological surgical training in humans, despite some differences found. Resumo Introdução Técnicas de timpanoplastia com diferentes tipos de enxerto têm sido usadas para fechar perfurações timpânicas desde o século XIX. A cartilagem tragal e a fáscia temporal são os tipos de enxerto mais usados, com resultados funcionais e morfológicos semelhantes na maioria dos casos. Embora ainda com poucas evidências publicadas, a mucosa nasal tem se mostrado uma boa opção de enxerto. Objetivo Comparar os resultados cirúrgicos e audiológicos da timpanoplastia tipo I com enxerto de mucosa nasal e de fáscia temporal em seis meses de seguimento. Método Foram selecionados aleatoriamente 40 pacientes candidatos a timpanoplastia tipo I divididos nos grupos enxerto de mucosa nasal e de fáscia temporal com 20 em cada grupo. Os parâmetros avaliados incluíram sucesso cirúrgico, ou seja, a taxa de fechamento completo da perfuração timpânica, e resultados audiológicos; diferença entre o gap quadritonal médio do gap aéreo-ósseo pré e pós-operatório após seis meses da cirurgia. Resultados O fechamento completo da perfuração timpânica foi alcançado em 17 de 20 pacientes no grupo mucosa nasal, como também no grupo fáscia temporal. O ganho quadritonal médio do gap aéreo-ósseo foi de 11,9 dB no grupo mucosa nasal e 11,1 dB no grupo fáscia temporal, respectivamente. Não houve diferença estatisticamente significante entre os grupos. Conclusão O enxerto de mucosa nasal pode ser considerado semelhante ao da fáscia temporal quando se considera o sucesso cirúrgico de pega do enxerto e o ganho audiológico.

Abstract Introduction Tympanoplasty techniques with different types of graft have been used to close tympanic perforations since the 19th century. Tragal cartilage and temporalis fascia are the most frequently used types of graft. They lead to similar functional and morphological results in most cases. Although little published evidence is present, nasal mucosa has also been shown to be a good alternative graft. Objective Surgical and audiological outcomes at the six-month follow-up in type I tympanoplasty using nasal mucosa and temporalis fascia grafts were analyzed. Methods A total of 40 candidates for type I tympanoplasty were randomly selected and divided into the nasal mucosa and temporalis fascia graft groups with 20 in each group. The assessed parameters included surgical success; the rate of complete closure of tympanic perforation and hearing results; the difference between post- and pre-operative mean quadritonal airway-bone gap, six months after surgery. Results Complete closure of the tympanic perforation was achieved in 17 of 20 patients in both groups. The mean quadritonal airway-bone gap closures were11.9 and 11.1 dB for the nasal mucosa and temporalis fascia groups, respectively. There was no statistically significant difference between the groups. Conclusion The nasal mucosa graft can be considered similar to the temporal fascia when considering the surgical success rate of graft acceptance and ultimate audiological gain.

Resumo Justificativa e objetivos: Os parâmetros de peso para o uso de sugamadex em pacientes com obesidade mórbida ainda precisam ser definidos. Métodos: Um ensaio clínico prospectivo foi feito com 60 participantes com índice de massa corporal ≥ 40 kg.m-2, submetidos a cirurgia bariátrica, randomizados em três grupos: peso ideal (PI), peso corrigido em 20% (PC20) e peso corrigido em 40% (PC40). Todos os pacientes receberam anestesia intravenosa total. Rocurônio foi administrado em dose de 0,6 mg.kg-1 para intubação traqueal pelo peso ideal, seguido de infusão (0,3 a 0,6 mg.kg-1.h-1). A sequência de quatro estímulos (TOF) foi usada para monitorar a profundidade do bloqueio. Após recuperação espontânea da segunda resposta do TOF no fim da cirurgia, 2 mg.kg-1 de sugamadex foi administrado. O desfecho primário foi o tempo de reversão do bloqueio neuromuscular até obter TOF ≥ 0,9. O desfecho secundário foi a ocorrência de curarização residual pós-operatória na sala de recuperação pós-anestésica, avaliaram-se a capacidade do paciente de passar do leito cirúrgico para o de transporte, a adequação da oxigenação, o padrão respiratório, a habilidade para deglutir saliva e a clareza de visão. Resultados: Os grupos foram homogêneos quanto a gênero, idade, peso corporal total, peso corporal ideal, índice de massa corporal, tipo e tempo de cirurgia. Os tempos de reversão (segundos) foram (média ± desvio-padrão) 225,2 ± 81,2, 173,9 ± 86,8 e 174,1 ± 74,9, respectivamente, nos grupos PI, PC20 e PC40 (p = 0,087). Conclusões: Não foram observadas diferenças entre os grupos quanto ao tempo de reversão do bloqueio neuromuscular e frequência de curarização residual pós-operatória. Concluímos que o peso corporal ideal pode ser usado para calcular a dose de sugamadex para reverter o bloqueio neuromuscular moderado em pacientes com obesidade mórbida.

Abstract Background and objectives: The weight parameters for use of sugammadex in morbidly obese patients still need to be defined. Methods: A prospective clinical trial was conducted with sixty participants with body mass index ≥ 40 kg.m-2 during bariatric surgery, randomized into three groups: ideal weight (IW), 20% corrected body weight (CW20) and 40% corrected body weight (CW40). All patients received total intravenous anesthesia. Rocuronium was administered at dose of 0.6 mg.kg-1 of Ideal weight for tracheal intubation, followed by infusion of 0.3-0.6 mg.kg-1.h-1. Train of four (TOF) was used to monitor depth of blockade. After spontaneous recovery TOF-count 2 at the end of surgery, 2 mg.kg-1 of sugammadex was administered. Primary outcome was neuromuscular blockade reversal time to TOF ≥ 0.9. Secondary outcome was the occurrence of postoperative residual curarization in post-anesthesia recovery room, searching the patient's ability to pass from the surgical bed to the transport, adequacy of oxygenation, respiratory pattern, ability to swallow saliva and clarity of vision. Results: Groups were homogenous in gender, age, total body weight, ideal body weight, body mass index, type and time of surgery. The reversal times (s) were (mean ± standard deviation) 225.2 ± 81.2, 173.9 ± 86.8 and 174.1 ± 74.9 respectively, in the IW, CW20 and CW40 groups (p = 0.087). Conclusions: No differences were observed between groups with neuromuscular blockade reversal time and frequency of postoperative residual curarization. We concluded that ideal body weight can be used to calculate sugammadex dose to reverse moderate neuromuscular blockade in morbidly obese patients.

RESUMO INTRODUÇÃO: Tratamentos promissores para o fechamento da perfuração da membrana timpânica vêm sendo estudados. Terapias provenientes de engenharia de tecidos provavelmente eliminarão a necessidade de uma intervenção cirúrgica convencional. A celulose bacteriana apresenta-se como uma alternativa por ser segura, de baixa toxicidade, biocompatível. OBJETIVOS: Investigar o efeito da aplicação direta do enxerto da celulose bacteriana na cicatrização de perfurações da membrana timpânica, comparado ao procedimento convencional com fáscia autóloga. MÉTODO: Incluíram-se 40 pacientes com perfuração da membrana timpânica por otite média crônica simples. Randomizados de 1 a 40, onde os ímpares (20) foram tratados com enxerto de celulose bacteriana (CB), e os pares (20), com enxerto de fáscia temporal autóloga (fáscia). Estudo clínico controlado e randomizado. O tempo cirúrgico e de hospitalização foram o tempo de epitelização e custos hospitalares. RESULTADOS: O fechamento das perfurações foi semelhante nos dois grupos. O tempo médio da cirurgia no grupo fáscia foi de 76,50 minutos e de 14,06 minutos no grupo com celulose bacteriana (p = 0,0001). O custo hospitalar pela tabela do SUS foi de R$ 600,00 para o grupo CB e R$ 7.778,00 para o grupo fáscia (p = 0,0001). CONCLUSÃO: A celulose bacteriana promoveu o fechamento da perfuração do tímpano, mostrando-se inovador, seguro, eficaz, efetivo, minimamente invasivo e de baixo custo.

ABSTRACT INTRODUCTION: Promising treatments for tympanic membrane perforation closure have been studied. Therapies derived from tissue engineering probably eliminate the need for conventional surgery. Bacterial cellulose is presented as an alternative that is safe, biocompatible, and has low toxicity. OBJECTIVES: To investigate the effect on healing of direct application of a bacterial cellulose graft on the tympanic membrane compared to the conventional approach with autologous fascia. METHODS: Randomized controlled trial. Forty patients with tympanic membrane perforations secondary to chronic otitis media were included, and were randomly assigned to an experimental group (20), treated with a bacterial cellulose graft (BC) and control group (20), treated with autologous temporal fascia (fascia). We evaluated the surgical time, hospital stay, time of epithelialization and the rate of tympanic perforation closure. Hospital costs were compared. The statistical significance level accepted was established at p < 0.05. RESULTS: The closure of perforations was similar in both groups. The average operation time in the fascia group was 76.50 min versus 14.06 min bacterial cellulose in the group (p = 0.0001). The hospital cost by the Brazilian public health system was R$ 600.00 for the bacterial cellulose group, and R$ 7778.00 for the fascia group (p = 0.0001). CONCLUSION: Bacterial cellulose grafts promoted the closure of the tympanic membrane perforations, and were demonstrated to be innovative, effective, safe, minimally invasive, efficacious and to have a very low cost.

INTRODUÇÃO: Existem diversos biomateriais que podem ser utilizados na cirurgia otológica para preencher a cavidade da orelha média ou dar suporte a enxertos. A esponja de gelatina absorvível é a mais utilizada, mas pode provocar fibrose e prejudicar a ventilação da orelha média. OBJETIVO: Investigar os efeitos da reação inflamatória provocada pela esponja do biopolímero da cana-de-açúcar (BP) comparada a esponja de gelatina absorvível (EGA) na mucosa da orelha média de ratos. MATERIAIS E MÉTODOS: Estudo experimental prospectivo. A esponja do BP foi implantada na orelha direita e a EGA na orelha esquerda de 30 ratos Wistar fêmeas. Os animais foram sacrificados com 4 e 12 semanas após o procedimento. Avaliação histológica qualitativa foi realizada para verificar a reação inflamatória na bula timpânica. RESULTADOS: A esponja do BP provocou exsudato inflamatório mais intenso e persistente que a EGA. O BP não foi absorvido durante o tempo de observação. Traves de fibrose foram observadas apenas nos ouvidos com a EGA. Houve espessamento da mucosa e neoangiogênese no grupo da EGA. CONCLUSÃO: Apesar da reação inflamatória, a esponja do BP provocou menos fibrose e neoangiogênese quando comparada a EGA. A esponja do BP comportou-se como um biomaterial não absorvível na orelha média.

INTRODUCTION: Several biomaterials can be used in ear surgery to pack the middle ear or support the graft. The absorbable gelatin sponge is the most widely used, but it may produce fibrosis and impair ventilation of the middle ear. OBJECTIVE: This experimental study aimed to investigate the inflammatory effects of the sugarcane biopolymer sponge (BP) in the rat middle ear compared with absorbable gelatin sponge (AGS). MATERIALS AND METHODS: Prospective experimental study design. Thirty adult female Wistar rats were allocated to receive the BP sponge into the right ear and AGS into the left ear. Animals were randomly killed at 4 and 12 weeks post-procedure. Qualitative histological assessments were performed to evaluate the inflammatory reaction in the tympanic bullae. RESULTS: The BP sponge caused inflammation more intense and persistent than AGS. The BP was not absorbed during the experiment. Fibrosis was observed only in the ears with AGS. There were thickening of the mucosa and neoangiogenesis in the group of AGS. CONCLUSION: Despite inflammation, the BP sponge produced less fibrosis and neoangiogenesis compared to AGS. The sponge BP appeared to be a non-absorbable biomaterial in the middle ear.

INTRODUÇÃO: As alterações das pregas vocais que envolvem redução do seu volume e falha no fechamento glótico causam um escape exagerado de ar durante a fonação. Para essas situações, tem sido proposta a utilização de implantes ou enxertos de materiais diversos. OBJETIVO: Definir o comportamento do biopolímero de cana-de-açúcar implantado nas pregas vocais de coelhos. MÉTODOS: Trata-se de um estudo experimental. Avaliaram-se histologicamente as pregas vocais de coelhos injetadas com biopolímero de cana-de-açúcar e solução fisiológica após 21 e 90 dias. RESULTADOS: Foi observada a presença do biopolímero, reação inflamatória leve a moderada e aumento de volume em todas as pregas vocais injetadas em relação às de controle. Não houve casos de necrose ou calcificação. DISCUSSÃO: Este trabalho mostrou maior reação inflamatória nos casos que os controles além de biointegração do material na prega vocal. Essa resposta fibrogênica com ausência de repercussões epiteliais pode nos sugerir que o biopolímero em sua forma de gel pode ser bioativo e preservar as funções vibratórias normais do epitélio. CONCLUSÃO: Neste trabalho, mostramos que apesar de produzir uma reação inflamatória nos tecidos das pregas vocais o material perdurou na prega vocal durante todo o período de estudo.

INTRODUCTION: Alterations in the vocal folds that involve volume reduction and glottal closure failure result in exaggerated air escape during speech. For such situations, the use of implants or grafts of different materials has been proposed. OBJECTIVE: To define the effect of sugarcane biopolymer gel when implanted in the vocal folds of rabbits. METHODS: This was an experimental study. The vocal folds of rabbits injected with sugarcane biopolymer and saline solution were histologically evaluated after 21 and 90 days. RESULTS: Mild to moderate inflammation and increased volume were observed in all vocal folds injected with biopolymer, when compared to controls. There were no cases of necrosis or calcification. DISCUSSION: This study showed higher inflammatory reaction in cases than in controls and biopolymer biointegration to the vocal fold. This fibrogenic response with absence of epithelial repercussions suggests that the biopolymer in its gel form can be bioactive and preserve the normal vibratory function of the epithelium. CONCLUSION: We show that in spite of producing an inflammatory reaction in vocal fold tissues, the material remained in vocal fold throughout the study period.

INTRODUÇÃO: A mucocele do seio esfenoidal é uma lesão rara e benigna. Essas lesões são provavelmente diagnosticadas tardiamente por serem assintomáticas ou causarem sintomas não específicos. As características clínicas dependem de sua localização e podem incluir dor fronto-orbitária, paralisia do nervo oculomotor, diminuição da acuidade visual, exoftalmia e anosmia. Os achados da tomografia computadorizada (TC) e ressonância nuclear magnética (RNM) de nariz e seios paranasais aumentaram a precisão do diagnóstico. O tratamento consiste na marsupialização e drenagem da mucocele por via endoscópica nasossinusal. O prognóstico em relação à visão depende da duração da perda da acuidade visual pré-operatória. OBJETIVO: Relatar um caso de mucocele esfenoidal de grandes dimensões. RELATO DE CASO: Os autores relatam um caso de mucocele do seio esfenoidal em um paciente masculino de 48 anos de idade que apresentou amaurose subitamente. COMENTÁRIOS FINAIS: As características da mucocele esfenoidal são revistas com especial atenção para os seus achados clínicos e radiológicos, bem como o tratamento cirúrgico.

INTRODUCTION: The mucocele of the sphenoide sinus its a benign rare lesion. Those lesions are probably diagnosed late because they are asymptomatic or cause non-specific symptoms. The clinical characteristics depend on its location and can include fronto-orbital pain, oculomotor nerve palsy, decrease of visual acuity, exophthalmos and olfaction disorders. The findings of the CT and the MRI of nose and paranasal sinuses have increased the diagnostic accuracy. The treatment consists of marsupialization and drainage of the mucocele via endoscopic sinus. The prognosis for vision depends on the length loss of the visual acuity preoperative. OBJECTIVE: Report a case of sphenoid mucocele of big dimensions. CASE REPORT: The authors report a case of sphenoid sinus mucocele in a male patient of 48 years old, that has suddenly presented amaurosis. FINAL COMMENTS: The caracteristics of the sphenoid mucocele are reviewed with special attention for the clinical and radiological findings, as well as the surgical treatment.

INTRODUÇÃO: No diagnóstico diferencial das perdas sensorioneurais assimétricas fazem parte os distúrbios vasculares e dentre essas alterações encontra-se o dolicomega da artéria vértebro-basilar. Habitualmente essa doença é assintomática e quando há sintomas esses podem ser causados por compressão ou isquemia. Clinicamente podem ocorrer: perda neurossensorial, zumbido, cefaleia, hipoestesia facial, neuralgia trigeminal, vertigem, diplopia e paralisia facial entre outros. O exame de imagem de escolha para seu diagnóstico é a ressonância nuclear magnética. A terapia do dolicomega da artéria basilar pode ser intervencionista ou conservadora dependendo dos achados associados. A abordagem multidisciplinar incluindo neurologista, neurocirurgião e otorrinolaringologista para adequada condução do caso é recomendada. OBJETIVO: Relatar o caso de um paciente com perda auditiva neurossensorial assimétrica cujo diagnóstico foi de dolicomega da artéria basilar. RELATO DO CASO: JBS, 57 anos, sexo masculino, branco com história de hipoacusia neurossensorial assimétrica e zumbido tipo apito bilateral há vários anos. Exame otorrinolaringológico apresentando otoscopia, rinoscopia anterior e orofaringe normais. COMENTÁRIOS FINAIS: O tratamento consistiu de acompanhamento do paciente, controle do zumbido com medicação e uso de um aparelho de amplificação sonora individual na orelha esquerda.

INTRODUCTION: At the differentiated diagnosis of asymmetrical sensorineural hearing losses, vascular disorders are present, one of which is megadolicho basilar artery. This disease is generally asymptomatic, and when symptoms are found, they can be caused by a compression or ischemia. Clinically, sensorineural hearing loss, tinnitus, headache, facial hypoesthesia, trigeminal neuralgia, vertigo, diplopia and facial palsy, among others, are likely to occur. The image examination of choice for its diagnosis is nuclear magnetic resonance. The megadolicho basilar artery therapy can be surgical or conservative, according to the associated findings. A multidisciplinary approach, including a neurologist, neurosurgeon and an otorhinolaryngologist is recommended for a proper administration of the case. OBJECTIVE: Report the case of a patient with asymmetrical sensorineural hearing loss, diagnosed of megadolicho basilar artery. CASE REPORT: JBS, 57-year-old white male with a history of asymmetrical sensorineural hearing loss and bilateral whistle-like tinnitus for several years. The otorhinolaryngologic evaluation, including otoscopy, anterior rhinoscopy and oral pharynx, was normal. FINAL COMMENTS: The treatment consisted in following up with the patient, controlling the tinnitus by drugs and using an individual sound amplification apparatus on the left ear.

A audição exerce um papel importantíssimo no desenvolvimento e adaptação social dos pacientes cegos. OBJETIVOS: Avaliar o desempenho do processamento temporal de cegos; caracterizar a habilidade de resolução temporal, segundo tempo e frequência; a ordenação temporal de cegos usando o teste de padrão de frequência e comparar o desempenho da população estudada para os testes de processamento aplicados. METODOLOGIA: Participaram do estudo 12 adultos portadores de cegueira. O estudo foi do tipo transversal, aprovado pelo Comitê de Ética da Universidade Católica de Pernambuco sob nº 003/2008. Para a coleta de dados, foi utilizado o RGDT em suas duas versões e os testes de padrão de duração (TPD) e de frequência (TPF). RESULTADOS: Foi evidenciado excelente desempenho para o processamento temporal, média de 4,98 para o limiar composto na versão original do RGDT e 50 ms de reposta para todas as frequências na versão expandida. Os resultados do TPD e TPF variaram de 95% a 100% de acertos. Não houve diferença quantitativa quando comparados os testes avaliados, mas relatos verbais apontam o RGDT original como o mais difícil. CONCLUSÃO: A amostra avaliada apresentou bom desempenho do processamento temporal, bom desempenho para as habilidades de resolução temporal e ordenação temporal

Hearing has an important role in human development and social adaptation in blind people. OBJECTIVE: To evaluate the performance of temporal auditory processing in blind people; to characterize the temporal resolution ability; to characterize the temporal ordinance ability and to compare the performance of the study population in the applied tests. METHODS: Fifteen blind adults participated in this study. A cross-sectional study was undertaken; approval was obtained from the Pernambuco Catholic University Ethics Committee, no. 003/2008. RESULTS: Temporal auditory processing was excellent - the average composed threshold in the original RGDT version was 4. 98 ms; it was 50 ms for all frequencies in the expanded version. PPS and DPS results ranged from 95% to 100%. There were no quantitative differences in the comparison of tests; but oral reports suggested that the original RGDT original version was more difficult. CONCLUSIONS: The study sample performed well in temporal auditory processing; it also performed well in temporal resolution and ordinance abilities

O desenvolvimento dos biomateriais é importante na cirurgia. O comportamento de uma nova membrana derivada da cana-de-açúcar será avaliado na orelha média do rato. OBJETIVOS: Analisar a interação da membrana do biopolímero da cana-de-açúcar na mucosa da orelha do rato. MATERIAL E MÉTODO: Estudo experimental, prospectivo e pareado com 24 ratos Wistar. A membrana do biopolímero da cana-de-açúcar foi inoculada na orelha média direita e a fáscia autóloga na orelha esquerda. Os ratos foram subdivididos em 3 grupos de 8 e sacrificados com 4, 8 e 12 semanas após a cirurgia. Foi realizada uma análise histológica da mucosa da orelha média e da membrana timpânica. RESULTADOS: Houve reação inflamatória no grupo experimental com exsudato subagudo em 50% dos casos e 30% exsudato crônico; 20% estava normal. A inflamação foi intensa inicialmente, mas diminuiu no decorrer do tempo. No grupo controle houve apenas um caso de exsudato. Miringoesclerose na membrana timpânica foi observada em ambos os grupos. A biomembrana foi absorvida tardiamente em comparação com a fáscia. CONCLUSÕES: A membrana do biopolímero da cana-de-açúcar causou reação inflamatória na orelha média, com regressão no tempo tardio do experimento e fibrose leve. Futuros estudos podem direcionar seu uso na otorrinolaringologia.

New developments on biomaterials are important in surgery. The behavior of a new membrane produced from sugarcane will be evaluated in the middle ear of rats. AIM: This study analyzed the results from the interaction of the sugarcane-base biopolymer membrane in the middle ear of a rat. MATERIALS AND METHODS: We ran an experimental, prospective, paired study with 24 Wistar rats. The sugarcane-base polymer membrane was inoculated in the right ear; and an autologous fascia in the left ear. The rats were divided in 3 groups of 8, and slaughtered at 4, 8 and 12 weeks after surgery. Histological analyses were performed on the rats' middle ear mucosa and their tympanic membranes. RESULTS: There was an inflammatory reaction on the experimental group and middle ear subacute exudate in 50%of the cases; 30% chronic exudate; and 20% was normal. In the control group there was only one case of exudate. The inflammation was initially described as intense, but it decreased over time. Myringosclerosis was observed in both groups. The sugarcane biopolymer membrane was absorbed later when compared with fascia. CONCLUSION: The sugarcane biopolymer membrane induced an inflammatory reaction in the middle ear which decreased over time, and mild fibrosis. Future studies can indicate its use in otolaryngology.

A microcirurgia laríngea é uma forma de tratamento para diversas doenças da laringe. Devido à necessidade de instrumentos longos e manobras delicadas, movimentos involuntários representam importante dificuldade, podendo ser responsáveis por resultados pós-operatórios inadequados. OBJETIVO: Esse trabalho propõe um novo sistema de estabilização, flexível e versátil, para reduzir de forma significativa os movimentos involuntários apresentados pelo cirurgião. MATERIAL E MÉTODOS: Esse estudo experimental comparou a amplitude dos movimentos involuntários apresentados por cirurgiões com e sem o sistema de estabilização. Dez cirurgiões realizaram um total de seis manobras que mimetizavam movimentos utilizados nas microcirurgias de laringe, sendo duas delas sem o sistema de estabilização. As manobras eram repetidas com o sistema de estabilização e o fio tensionado, após isso o fio era então alongado 3 mm e as manobras eram executadas. As médias da deslocação máxima do instrumento foram comparadas entre os grupos. RESULTADOS: O deslocamento máximo foi maior na manobra com microtesoura parada sem o sistema, quando comparada com o sistema de estabilização em três situações diferentes. A média também foi maior na manobra de abrir-fechar a microtesoura sem o sistema de estabilização e com esse. CONCLUSÃO: O sistema de estabilização proposto foi eficaz na redução do tremor nas situações testadas.

Laryngeal microsurgery is a kind of treatment for various laryngeal diseases. Because of the need of long instruments and delicate maneuvers, involuntary movements represent relevant difficulty and may be responsible for unintended post-operative results. AIM: This study proposes a new stabilization system, flexible and versatile, which can significantly reduce involuntary movements made by surgeons. METHODS: This experimental study compared the amplitude of surgeons' involuntary movements with and without the stabilization system. Ten surgeons performed a total of six movements mimicking movements used in laryngeal microsurgery, two of them without the stabilization system. The maneuvers were repeated with the stabilization system and the wire stretched, and after this, the wire was then expanded 3mm and the maneuvers were performed. The average values of the maximum instrument displacement were compared between the groups. RESULTS: The maximum displacement was higher during the maneuver with the still micro-scissors without the system, when compared with the stabilization system in three different situations. The average was also higher in the maneuver to open and close the micro scissors without the system and with it. CONCLUSION: The proposed stabilization system was effective in reducing surgeon shaking in the different situations tested.

A audição é uma das principais formas de ligação entre o ser humano e o ambiente no qual ele vive, porém, a deficiência auditiva ainda é diagnosticada tardiamente no Brasil interferindo diretamente no desenvolvimento global da criança. OBJETIVO: Verificar o conhecimento de pediatras e ginecologistas a respeito dos fatores de risco para a surdez, o modo de aquisição do conhecimento e ocorrência de orientação aos pais a respeito do tema. MATERIAL E MÉTODO: Participaram 119 médicos, pertencentes a três hospitais públicos da cidade do Recife. Foi aplicada uma entrevista, antes e após campanha de esclarecimento sobre o tema. O estudo foi descritivo, transversal, tipo série de casos. A análise dos dados foi descritiva. RESULTADOS: Constatou-se que apenas 3 dos 18 fatores de risco para surdez listados obtiveram respostas acima de 50% na etapa inicial e 53,84% dos profissionais faziam orientações as famílias. Todas as respostas aumentaram na segunda etapa. CONCLUSÃO: Existe uma maior necessidade de campanhas de orientação, as ações de atenção primária à surdez constituem o meio mais barato e eficaz no combate a perda auditiva. Os otorrinolaringologistas e fonoaudiólogos devem estar presentes desempenhando trabalho educativo e de diagnóstico que visa intervenção o mais cedo possível.

Hearing is one of the main forms of connection between human being and the environment; however, hearing loss is still diagnosed very late in Brazil, which directly interferes with the child's development. AIM: The aim of this study was to check the knowledge pediatricians and gynecologists have about the risk factors for the deafness, the way they acquired such knowledge and parent education about the subject. MATERIALS AND METHODS: We enrolled 119 doctors from three public hospitals of the city of Recife. An interview was applied, before and after the educational campaign on the matter. The study was descriptive, cross-sectional, case series-type. Data analysis was descriptive and inferential. RESULTS: The results showed that only 3 of the 18 hearing loss risk factors listed had gotten answers above 50% in the initial stage of the study and 53.84% of the professionals educated the families. All the answers had increased in the second stage. CONCLUSION: The results emphasize the need to pay more attention to this matter; therefore, primary care is an inexpensive and efficient way to fight hearing loss.
