A técnica de denervação coxofemoral é realizada em cães displásicos, com o intuito de aliviar a dor. O objetivo deste trabalho foi avaliar alívio da dor, melhora da função articular, reabilitação muscular e progressão da instabilidade em 10 cães displásicos submetidos à técnica de denervação da articulação coxofemoral. A graduação da apresentação clínica foi realizada através de avaliações específicas da andadura, testes de estação bípede, de rotação com abdução externa e de iliopsoas. A reabilitação muscular foi avaliada através da circunferência da coxa e exame físico. A avaliação da instabilidade foi feita através de técnica radiográfica para índice de distração. Na graduação da claudicação e teste de estação bípede foi verificado que houve redução significante a partir de um mês pós-cirúrgico. Já nos testes de abdução com rotação externa, houve redução da dor a partir do sétimo dia pós-cirúrgico; no teste de iliopsoas houve redução significante da dor, em todos os tempos do pós-cirúrgico. Houve aumento significante da circunferência da coxa a partir do sétimo dia. Foi observado que 90% tiveram redução do índice de distração, refletindo melhora da instabilidade. A técnica é eficaz no alívio da dor, na reabilitação muscular após a melhora na capacidade de exercitar-se e na redução da instabilidade, sendo motivo de satisfação para os proprietários.

The hip denervation technique has been performed in dysplasic dogs, demonstrating to be efficient in the relief of pain. The objective of this work was to evaluate pain relief, improvement of joint function, muscular rehabilitation and instability progression in 10 dysplasic dogs submitted to the hip joint denervation technique. The grading of the clinical presentation was performed according to specific evaluation of walking, biped station, rotation with external abduction, subluxation and iliopsoas. The muscular rehabilitation was evaluated through the circumference of the thigh and physical examination. The instability was evaluated through the values of distraction index (DI). In the claudication graduation and the biped station test, we verified that there was a significant reduction after one month post-surgery. In the abduction with external rotation tests, there was a reduction in pain after the seventh day; in the iliopsoas test there was a significant reduction in pain in all the post-surgery times. There was a significant increase in thigh circumference after the seventh day. We observed that 90% of the articulations had a reduction in their DI, reflecting an improvement of their instability. The technique is efficient in the relief of pain, muscular rehabilitation after the improvement of the ability to exercise, and in reduction of instability, being the cause of owner satisfaction.

O presente trabalho teve como objetivo verificar a recuperação da locomoção e o tempo para cicatrização óssea de fraturas ilíacas fixadas com parafusos, fios de aço e cimento ósseo de polimetilmetacrilato. Dezesseis cães de ambos os sexos, com peso de 1,8 a 16kg e idade entre sete meses e 11 anos, foram submetidos à osteossíntese da fratura de ílio, provocadas por acidente automobilístico. Em dois animais, realizou-se osteossíntese bilateral, totalizando 18 ossos ilíacos operados. A abordagem ao ílio foi lateral e quando necessária estendida caudalmente através da osteotomia do trocanter maior. As fraturas foram reduzidas e, em cada segmento ósseo, foram implantados dois a três parafusos e banda de tensão com fio de aço entre os parafusos adjacentes à linha de fratura. Sobre esses implantes aplicou-se cimento ósseo misturado com cefazolina sódica e após o endurecimento prosseguiu-se com a síntese dos tecidos moles com suturas rotineiramente utilizadas na clinica cirúrgica. No período pós-operatório, foram realizadas avaliações até 90 dias após a osteossíntese e observou-se locomoção apropriada em 15 animais. Avaliações radiográficas demonstraram sinais de completa consolidação óssea entre 60 e 90 dias. Falha da estabilização ocorreu em dois casos, obrigando a reintervenção cirúrgica em um deles. A partir dos resultados obtidos, é possível concluir que, em cães com até 16kg de peso, a fixação de fraturas ilíacas utilizando parafusos cimentados com PMMA constitui uma eficiente técnica, que proporciona adequada estabilidade, precoce recuperação funcional e cicatrização óssea.

The present study aimed to verify the time for locomotion recovery and bone healing in canine iliac fractures fixated with screws, orthopedic wires and methylmetacrylate bone cement. Sixteen dogs from both genders accidentally hit by a car and showing iliac fractures were included. Dogs aged from 7 months to 11 years and weighted between 1.8 and 16.0kg. Two dogs had bilaterally fractures stabilized, totalizing 18 ilium osteosynthesis performed. The ilium was achieved via lateral approach. The fractures were reduced and on each bone segment was inserted two to three screws connected by a band tension. Over them a mixture of bone cement added with cefalozin was applied as a definitive fixation device. Suture of soft tissues was made as usual. Postoperative evaluations were periodically performed. After 3 months, 15 dogs showed proper use of the operated limb. Radiographic signs of complete bone healing were observed between 60 and 90 days. Stabilization failure occurred in two cases and one of them required another surgery. Both cases showed a good outcome. Based on these results it was possible to conclude that in dogs weighting up to 16.0kg, iliac fracture fixation with screws, orthopedic wires and bone cement is an efficient technique promoting early limb use and complete bone healing as a result of adequate bone stabilization.

O objetivo deste trabalho foi avaliar os resultados da técnica de interligação extracapsular fêmoro-fabelo-tibial, como forma de tratamento da ruptura do ligamento cruzado cranial (RLCC) espontânea em cães, utilizando para isto avaliações clínicas e radiográficas. Neste estudo, foram operados 13 cães, divididos em dois grupos: ruptura unilateral (RU) e ruptura bilateral (RB). O peso variou entre 2,3 e 53kg (mediana 6,7kg) e o intervalo entre a suspeita da lesão observada pelos proprietários e os procedimentos cirúrgicos variaram entre três e 365 dias (média de 123 dias). Em nenhum dos 13 joelhos operados foram observadas complicações transoperatórias. Após 30 dias de pós-operatório, os dois grupos apresentaram diferença estatisticamente significante (P=0,05) nos parâmetros funcionais avaliados, com exceção da dor e disposição para levantar o membro contralateral no grupo de cães com ruptura bilateral. Ao exame radiográfico nos dias zero e 180, a evolução das alterações degenerativas foi observada em 84,6% dos casos. A técnica de interligação extracapsular fêmoro-fabelo-tibial é eficiente na estabilização imediata da articulação com RLCC, oferecendo recuperação funcional ao reduzir o deslocamento cranial e a rotação interna da tíbia, porém não impede a progressão da osteoartrite.

This study was aimed at evaluating the results of extracapsular femoro-fabelo-tibial inter-connection technique, for the treatment of spontaneous cranial cruciate ligament rupture (CCLR) in dogs, using for this purpose clinical and radiographic evaluation. In this study 13 dogs allocated in two groups were operated: unilateral rupture (RU) and bilateral rupture (RB). The weight ranged between 2.3 and 53kg (median 6.7kg) and the interval between the suspect of lesion observed by the owners and the surgical procedures ranged between three and 365 days (mean 123 days). None of the operated knee were observed intra-operative complication. After 30 days of postoperative the two groups showed statistically significant difference (P=0.05) in the evaluated parameters, except of the pain and willingness to hold up the contralateral limb on the group of the dogs with bilateral rupture. At the radiographic exam on the 0 and 180 days, the evolution of degenerative changes was seen in 84.6% of cases. The extracapsular femur-fabelo-tibial interconnection technique is efficient in the immediate stabilization of joint with CCLR, offering functional recovery for reduce the cranial displacement and internal rotation of the tibia, however it doesn’t stop the osteoarthritis progression.
