OBJETIVO: Avaliar a eficácia do ácido L-glutâmico na prevenção da calcificação do pericárdio bovino implantado no subcutâneo de ratos. MÉTODO: Utilizaram-se 54 ratos Wistar, distribuídos em seis grupos, de acordo com o segmento de pericárdio bovino implantado. Inicialmente, todos os pericárdios foram fixados com glutaraldeído 0,5% por 72h. No grupo I, após a fixação, o pericárdio foi preservado em glutaraldeído 0,2% até o implante. O grupo II foi estocado em solução de Paraben. No grupo III e IV, após a fixação inicial, os pericárdios foram tratados com ácido L-glutâmico 8% com pH 7,4 e 3,5, respectivamente, sendo em seguida estocados em Paraben. Os grupos V e VI foram semelhantes aos grupos III e IV, exceto pela concentração do ácido L-glutâmico que foi de 0,8%. Os explantes foram feitos com 15, 30 e 60 dias, e as amostras submetidas à análise histológica com hematoxilina-eosina e Von Kossa, além da mensuração de cálcio por espectofotometria de absorção atômica. RESULTADOS: A mensuração por espectofotometria de absorção atômica demonstrou aumento progressivo da calcificação nos grupos I, II e III, aos 15, 30 e 60 dias. Nos grupos IV, V e VI, os níveis de cálcio permaneceram sem alteração nos períodos estudados. A análise microscópica demonstrou calcificação progressiva nos grupos I, II e III. Nos grupos IV, V e VI, a calcificação, quando observada, foi focal e de grau leve. CONCLUSÃO: O uso do ácido L-glutâmico em segmentos de pericárdio bovino, fixados pelo glutaraldeído, foi efetivo na prevenção da calcificação, quando implantados no subcutâneo de ratos por até 60 dias.

OBJECTIVE: To evaluate the efficiency of L-glutamic acid to prevent calcification of glutaraldehyde bovine pericardium implanted in rats' subcutaneous tissues. METHODS: Fifty four Wistar rats were divided in six groups according to the type of the bovine pericardium implanted. At first, all pericardia were initially cross-linked with 0.5% glutaraldehyd (GDA) fixative for 72 h. In Group I, after the initial fixation, the pericardia were preserved in 0.2% GDA fixative until the implantation, whereas in Group II they were stocked in Paraben solution. In Groups III and IV, after the initial fixation in 0.5% GDA fixative, the pericardia were treated with 8% L-glutamic acid at pH 7.4 and 3.5, respectivelly, being subsequently stocked in Paraben solution. Groups V and VI were similar to III and IV, except for the concentration of L-Glutamic acid which was 0.8%. Explantation was done at 15, 30, and 60 days, and the specimens submitted to histological analysis with Hematoxylin and eosin (HE) and Von Kossa stains, besides calcium quantification with atomic spectrofotometry. RESULTS: Microscopic analysis demonstrated severe and progressive calcification in groups I, II, and III, whereas in groups IV, V, and VI calcification, when present, was mild and focal. Spectrofotomety confirmed these findings, revealing calcium contents of 1.93µg/mg of tissue at 60 days in the control group. Groups IV and VI showed the least calcium contents (0.063 e 0.066, respectively). CONCLUSIONS: The use of L-glutamic acid in segments of bovine pericardium with glutaraldehyde fixative was effective in preventing the calcification when implanted in rats' subcutaneous up to 60 days.

OBJETIVO: Avaliar a eficácia do cloreto de alumínio, isoladamente ou em associação com o etanol, na prevenção da calcificação e da resposta inflamatória de fragmentos de parede aórtica porcina fixada em glutaraldeído (GDA), implantados no tecido subcutâneo de ratos jovens. MÉTODO: Utilizaram-se 15 ratos da linhagem Sprague-Dawley, em cujas telas subcutâneas foram implantados fragmentos de parede aórtica porcina, submetidos a três diferentes métodos de tratamento [grupos: I (GDA), II (GDA+alumínio), III (GDA+etanol+alumínio)]. Os explantes foram realizados com 15, 30 e 60 dias após as operações. Foram realizadas análises histológicas pelas colorações de hematoxilina & eosina (HE) e de alizarina, nos pHs de 4,2 e 7,0, e a dosagem de cálcio feita por espectroscopia de absorção atômica. RESULTADOS: Pelo HE, constatou-se que a matriz extracelular das paredes aórticas ficaram melhor preservadas nos explantes do grupo III. A intensidade da reação inflamatória intensa foi menor nesse grupo. Pela alizarina pH 4,2, o grupo II e III tiveram menores índices de calcificação comparado ao controle. Pela alizarina pH 7,0, o grupo III teve menor índice de calcificação comparado aos grupos I e II. Pela espectroscopia de absorção atômica, os níveis de cálcio foram semelhantes para os grupos II e III, mas significativamente menores do que os do grupo I. CONCLUSÃO: O tratamento com cloreto de alumínio diminuiu a calcificação dos fragmentos de parede aórtica porcina. O uso combinado do etanol com cloreto de alumínio foi ainda mais eficiente em inibir a calcificação, e também em diminuir a reação inflamatória.

OBJECTIVE: To evaluate the efficiency of aluminum chloride in isolation or associated with ethanol to prevent calcification and inflammatory reaction with fragments of porcine aortic wall fixed in glutaraldehyde (GDA) and subdermally implanted in young rats. METHOD: Fifteen Sprague-Dawley rats were studied. Three fragments of porcine aortic wall were implanted in the subdermal tissue. The fragments were previously subjected to three different methods of treatment: I (GDA), II (GDA + aluminum), III (GDA + ethanol + aluminum). Explantation was performed after fifteen, thirty and sixty days. Histological analysis was achieved using hematoxylin & eosin (HE) and alizarin-red at pHs of 4.2 and 7.0. Calcium content was determined by atomic absorbance spectroscopy. RESULTS: HE and alizarin red staining showed that the aortic wall extracellular matrix was best preserved in the fragments of Group III. The intensity of the inflammatory reaction was lower in this group. When stained with alizarin red at pH 4.2, Groups II and III had lower degrees of calcification compared with Group I. With alizarin red staining at pH 7.0, Group III demonstrated less calcification compared with Groups I and II. Atomic absorbance spectroscopy showed similar calcium levels for both Groups II and III, but significantly less than in Group I. CONCLUSION: Treatment with aluminum chloride inhibits calcification of fragments of aortic wall after implantation and reduces inflammatory reaction. The combined use of ethanol with aluminum chloride is more efficient to inhibit calcification and also to diminish inflammatory reaction.

OBJETIVO: Avaliar os resultados tardios de 10 anos com a operacão de Ross, analisando a sobrevida, incidência de reoperações e desempenho tardio do auto-enxerto pulmonar e homoenxerto da reconstrução da via de saída do ventrículo direito. MÉTODOS: Entre maio/1995 e fevereiro/2005, 227 pacientes com média de idade de 29,1±11 anos foram submetidos à operação de Ross. A etiologia prevalente foi a moléstia reumática em 61% dos casos. O auto-enxerto foi implantado com a técnica de substituição total da raiz em 202 casos, com cilindro intra-luminal em 20 e de forma subcoronariana em 5. A reconstrução da via de saída do ventrículo direito foi feita de forma convencional, com homoenxertos criopreservados (n= 160), com extensão proximal de pericárdio no homoenxerto (n= 41) e com homoenxertos decelularizados (n= 26). O tempo de seguimento pós-operatório variou de 1 - 118 meses ( média= 45,5 meses). RESULTADOS: A mortalidade imediata foi de 3,5% e a sobrevida tardia foi de 96,9%, aos 10 anos. Não houve episódio de tromboembolismo, constatando-se apenas dois casos de endocardite. Onze pacientes foram reoperados, por problemas envolvendo o auto e/ou homoenxerto, progressão de doença reumática mitral e insuficiência coronariana iatrogênica. Após 10 anos, 96,4% e 96,2% dos pacientes estavam livres de reoperação no auto-enxerto e no homoenxerto, respectivamente. Não foi observada dilatação tardia dos auto-enxertos.A reconstrução da via de saída do ventrículo direito com homoenxertos decelularizados diminuiu de forma significativa a incidência de gradientes tardios. CONCLUSÃO: Os resultados tardios com a operação de Ross demonstraram excelente sobrevida tardia e baixa incidência de reoperações e morbidade tardia. Consideramos este procedimento a melhor opção no tratamento cirúrgico da valvopatia aórtica em crianças e adultos jovens.

OBJECTIVE: To evaluate the 10-year outcomes of the Ross Operation, analyzing survival rate, incidence of reoperations, and late performance of pulmonary autografts and homografts in the reconstruction of the right ventricular outflow tract. METHODS: Two hundred and twenty seven patients with a mean age of 29.1±11 years underwent Ross operation from May 1995 to February 2005. The most prevalent etiology was rheumatic disease in 61% of the cases. Autografts were implanted using the total root replacement technique in 202 cases, with intraluminal cylinder in 20, and in the subcoronary position in 5. The right ventricular outflow tract was conventionally reconstructed with cryopreserved homografts (n = 160), with proximal extension of the homograft with pericardium (n = 41), and with decellularized homografts (n = 26). The postoperative follow-up ranged from 1 to 118 months (mean = 45.5 months). RESULTS: Hospital mortality was 3.5%, and long-term survival was 96.9% at ten years. No episodes of thromboembolism and only two cases of endocarditis occurred. Eleven patients underwent reoperation because of problems related to the auto and/or homograft, progression of rheumatic mitral valve disease, and iatrogenic coronary insufficiency. After 10 years, 96.4% and 96.2% of the patients were free from reoperation in the autograft and homograft groups, respectively. No late autograft dilatation was observed. Reconstruction of the left ventricular outflow tract with decellularized homografts significantly reduced the incidence of gradients on late follow-up. CONCLUSION: Late outcomes with the Ross Operation were associated with an excellent long-term survival and a low incidence of reoperations and late morbidity. We consider this procedure the best option for the surgical treatment of aortic valve disease in children and young adults.

OBJETIVO: Analisar comparativamente o uso do etanol (E) com o glutaraldeído (GDA), como método de inibição da calcificação dos heteroenxertos valvares pulmonares porcinos (HVPP) implantados na via de saída do ventrículo direito (VD) de ovinos jovens. MÉTODO: Foram utilizados dez ovinos jovens, em dois grupos contendo cinco animais cada. A operação consistiu na implantação do HVPP na via de saída do VD com circulação extracorpórea, divididos em dois grupos; o grupo do GDA (controle) e o grupo E (grupo em que os HVPP foram pré-tratados com etanol a 80%). Realizou-se estudo ecocardiográfico para medida do gradiente transvalvar pulmonar, análise macroscópica da parede do HVPP e microscópica quanto à presença de calcificação; e mensuração do cálcio na cúspide valvar após 210 dias. RESULTADOS: Na mensuração do cálcio do HVPP encontrou-se no grupo GDA (7,98 ± 6,82µg cálcio/mg tecido), no grupo E (0,31± 0,33µg cálcio/mg tecido). Em relação ao gradiente transvalvar máximo em 30 dias, não houve diferença significativa entre os grupos, já aos 90, 180 e 210 dias, o grupo GDA apresentou maior gradiente transvalvar do que o grupo do etanol. Na análise macroscópica da parede do HVPP, observou-se calcificação em ambos os grupos, sendo mais intensa no grupo do GDA. Na avaliação microscópica das cúspides do HVPP em relação à presença de calcificação, observou-se que no grupo GDA esteve presente em todos os animais, enquanto no grupo E, esteve ausente em todos. CONCLUSÃO: Conclui-se que etanol a 80% inibe a calcificação das cúspides no período analisado, mas não inibe a calcificação da parede do HVPP.

OBJECTIVE: The objective of this work was to comparatively evaluate the use of ethanol (E) with glutaraldehyde (GDA) to inhibit the calcification of the porcine pulmonary valvar heterografts (PPVH) implanted in the right ventricle outflow tract of young sheep. METHODS: Ten young sheep were used, divided into two similar groups: the GDA (control) Group and the Ethanol Group (in which the PPVH were pre-treated with ethanol-80). The surgery consisted of implanting the PPVH in the right ventricle outflow tract using a cardiopulmonary bypass. Echocardiography was performed to measure the transvalvar gradient and macroscopic analyses of the PPVH wall and microscopic analysis for calcification were performed. The amount of calcium was measured in the valve cusp at 210 days. RESULTS: The quantity of calcium in the PPVH of the GDA Group was 7.98 ± 6.82µg calcium/mg tissue and of the E Group it was 0.31± 0.33µg calcium/mg tissue. In respect to the maximum transvalvar gradient at 30 days, there was no significant difference between the groups. At 90, 180 and 210 days however, the GDA Group showed a greater transvalvar gradient than the E Group. The macroscopic analyses of the heterograft wall evidenced calcification in both groups but it was more pronounced in the GDA Group. The microscopic analysis of the cusp valve revealed the presence of calcification in all animals of the GDA Group, while in the E Group no calcification was seen. CONCLUSION: In conclusion ethanol-80 inhibits calcification on the cusps during the period of the analysis but does not inhibit the calcification of the PPVH.

OBJETIVO: O objetivo deste trabalho foi avaliar, retrospectivamente, os primeiros oito anos de funcionamento do Banco de Valvas Cardíacas Humanas do Hospital de Caridade da Irmandade da Santa Casa de Misericórdia de Curitiba (BVCHSC), analisando aspectos relacionados às atividades de captação, processamento, armazenamento e distribuição de enxertos homólogos cardiovasculares. MÉTODO: A seleção inicial dos doadores seguiu as diretrizes nacionais para captação de órgãos humanos, além de critérios específicos do BVCHSC. Os corações foram obtidos de doadores de múltiplos órgãos, doadores em parada cardíaca e receptores de transplante cardíaco, com somatória dos tempos de isquemia inferior a 48 horas. A idade dos doadores variou desde recém-nascidos até 60 anos para as valvas aórticas e 65 para as pulmonares. Os enxertos dissecados tiveram suas dimensões mensuradas e sua morfologia avaliada, sendo classificados como categoria 0 (descartados), 1 (alterações morfológicas mínimas) ou 2 (perfeitos). Foram determinados a incidência e os germes responsáveis pela contaminação nos enxertos recebidos, assim como a eficiência da solução de descontaminação. Foram também avaliados aspectos relacionados à distribuição dos enxertos. RESULTADOS: De setembro de 1996 a fevereiro de 2005, 1059 corações provenientes de 19 Estados foram recebidos no BVCHSC. Destes, 977 (92,3%) eram de doadores de morte encefálica. Foram processados 2105 enxertos, e dentre os aórticos e pulmonares analisados, 783 eram da categoria 2, 697 pertenciam à categoria 1 e 186 foram descartados por alterações morfológicas. No total, 433 enxertos recebidos estavam contaminados, sendo a solução de antibióticos eficiente na esterilização de 330 destes casos. Quinhentos e setenta e um (27,1%) enxertos foram rejeitados em alguma fase do processo, sendo a contaminação e as alterações morfológicas as causas mais freqüentes. Foram distribuídos 1338 enxertos para 74 instituições de saúde do país, sendo mais comumente empregados na substituição da valva aórtica (529), na correção de cardiopatias congênitas (478) e durante operações de Ross (272). CONCLUSÕES: As atividades do BVCHSC durante estes oito anos de funcionamento foram satisfatórias, atingindo os objetivos propostos.

OBJECTIVE: The aim of this study was to retrospectively evaluate the first eight years of activities of the Human Heart Valve Bank of Hospital de Caridade da Irmandade da Santa Casa de Curitiba (BVCHSC), analysing aspects related to procurement, processing, storage and distribution of homologous cardiovascular grafts. METHOD: Initial donor screening was achieved according the national guidelines for human organ procurement, besides specific BVCHSC criteria. Hearts were obtained from multiple organ donors, non-beating heart donors and heart transplant recipients, with total ischemic times under 48 hours. Donor ages varied from neonates up to 60 years for aortic valves and 65 years for pulmonary valves. The dissected grafts had their dimensions measured and their morphology evaluated, using a classification system with category 0 (discarded), 1 (minimal morphological alterations) and 2 (perfect). The incidence and microorganisms responsible for organ contamination were determined, as was the efficiency of the decontamination solution. Aspects related to graft distribution were also analysed. RESULTS: From September 1996 to February 2005, 1059 hearts from 19 Brazilian states were recevied at BVCHSC. From these, 977 (92.3%) were from brain-death donors. A total of 2105 grafts were processed and of the aortic and pulmonary grafts that were analysed, 783 were in category 2, 697 in category 1 and 186 were discarded due to morphological abnormalities. In total, 433 received grafts were contamined and the sterilization solution was efficient in 330 cases. 571 (27.1%) grafts were rejected during some phase of the processing, mainly due to contamination and morphological abnormalities. A total of 1338 grafts were distributed to 74 health institutions and were more commonly used for aortic valve replament (529), correction of congenital heart diseases (478) and during the Ross procedure (272). CONCLUSIONS: The activities of the BVCHSC during the initial eight years were satisfactory, fulfilling the proposed goals.

INTRODUÇÃO: As cúspides porcinas tratadas com glutaraldeído (GDA) são um dos tecidos biológicos mais utilizados em biopróteses, porém a calcificação tardia pós-implante constitui a causa predominante de sua falência. OBJETIVO: Analisar comparativamente dois métodos de prevenção da calcificação (etanol 80% e ácido L-glutâmico 0,8%) em cúspide e parede aórtica porcina implantadas no subcutâneo em ratos, tendo como grupo controle as cúspides e os segmentos de parede aórtica fixadas em glutaraldeído (GDA), num período de 15, 30 e 60 dias após implante. MATERIAL E MÉTODOS: Foram utilizados 45 ratos jovens, distribuídos em 3 grupos contendo 15 animais cada, que por sua vez foram subdivididos em 3 subgrupos de 5 animais, nos quais foram implantados, em 2 bolsas no subcutâneo, uma cúspide e um segmento de parede aórtica em cada. Em cada grupo assim nominados: GDA (grupo controle), E80% (grupo cujas estruturas foram pré-tratadas com etanol 80%) e o AG 0,8% (grupo cujas estruturas foram pré-tratadas com ácido L-glutâmico 0,8%) foram realizadas a mensuração do cálcio e análise microscópica quanto à presença de calcificação (localização e intensidade da mesma) e de Infiltrado inflamatório (localização e tipo), no período de 15, 30 e 60 dias após implante. RESULTADOS: Na mensuração do cálcio na cúspide aórtica encontrou-se no grupo E80% 15 dias (1,30±0,21 mg cálcio/mg tecido), E80% 30 dias (1,05±0,22 mg cálcio/mg tecido) e E80% 60 dias (0,53±0,42 mg cálcio/mg tecido); no grupo AG 0,8% 15 dias (12,17±0,66 mg cálcio/mg tecido), AG 0,8% 30 dias (15,31±2,82 mg cálcio/mg tecido) e AG 0,8% 60 dias (34,24±16,28 mg cálcio/mg tecido) com o grupo controle GDA 15 dias (12,44±2,26 mg cálcio/mg tecido), GDA 30 dias (13,44±3,34 mg cálcio/mg tecido) e GDA 60 dias (50,85±8,71 mg cálcio/mg tecido). Em relação à mensuração do cálcio na parede aórtica, encontrou-se no grupo de E80% 15 dias (4,62±0,68 mg cálcio/ mg tecido), E80% 30 dias (9,47 ± 2,59mg cálcio/mg tecido) e E80% 60 dias (23,56±7,75 mg cálcio/mg tecido) no grupo de AG 0,8% 15 dias (4,31±0,85 mg cálcio/mg tecido), AG 0,8% 30 dias (7,69±1,48 mg cálcio/mg tecido) e AG 0,8% 60 dias (20,50± 1,22 mg cálcio/mg tecido) com o grupo controle GDA 15 dias (7,34±1,32 mg cálcio/mg tecido), GDA 30 dias (9,28±0,76 mg cálcio/mg tecido) e GDA 60 dias (27,60±1,08 mg cálcio/mg tecido). Na avaliação microscópica da cúspide aórtica houve uma progressiva calcificação naquelas submetidas à fixação com GDA. Este processo foi parcialmente encontrado com o AG 0,8% e totalmente ausente com o E80%. Quanto à avaliação referente aos segmentos da parede aórtica, também evidenciou-se progressiva calcificação, não sendo inibida pelos tratamentos com AG 0,8% e E80%. CONCLUSÕES: O pré-tratamento com etanol a 80% inibiu a calcificação nas cúspides aórticas porcinas, entretanto, não teve a mesma eficácia na parede aórtica. Contudo, o ácido L-glutâmico a 0,8% demonstrou minimizar a calcificação na parede aórtica. Estudos devem ser feitos para evidenciar se a ação anticalcificante do etanol a 80% mantém-se nas biopróteses aórticas porcinas se estas forem implantadas no sistema circulatório.

INTRODUCTION: The glutataldehyde (GDA) treated pigs cusps are one of most employed tissues in bioprosthesis, but is late post-implant calcification is main cause of its failure. BACKGROUND: This study aims at comparing and analyzing two methods (ethanol 80% and l-glutamic acid 0.8%) to prevent calcification in pig cusps and aortic wall implanted subcutaneously in rats, the cusps and aortic wall segments of the control were in glutaraldehyde (GDA), during a 15, 30 and 60 days period after the implant. MATERIAL AND METHODS: We used 45 young rats, distributed in 3 groups of 15 rats each, which in turn were subdivided in 3 subgroups of 5 rats each, in which we implanted one cusp and one aortic wall segment in 2 subcutaneous pouches for each rat. We called each group as follows: GDA (control group), E80% (the group whose structures were previously prepared with ethanol 80%) and GA 0.8% (group previously prepared with L-glutamic acid 0.8%); in those groups we measured calcium and performed a microscopic analysis seeking for any calcification, its location and intensity; inflammatory infiltrate, location and type, during a 15, 30, and 60-day period after the implant. RESULTS: Calcium was found in the aortic cusp in the E80% group (1.30±0.21 mg calcium/mg tissue) at day 15, (1.05±0.22 mg calcium/mg tissue) at day 30, and (0.53±0.42 mg calcium/mg tissue) at day 60; in the GA 0.8% group (12.17±0.66 mg calcium/mg tissue) at day 15, (15.31±2.82 mg calcium/mg tissue) at day 30, and (34.24±16.28 mg calcium/mg tissue) at day 60; and in the control group, GDA at day 15 (12.44±2.26 mg calcium/mg tissue), at day 30 (13.44±3.34 mg calcium/mg tissue), and at day 60 (50.85±8.71 µg calcium/mg tissue). As for the calcium measured in the aortic wall, in the E80% group we found (4.62±0.68 µg calcium/mg tissue) at day 15, (9.47±2.59 µg calcium/mg tissue) at day 30, and (23.56±7.75 µg calcium/mg tissue) at day 60; in the GA 0.8% group at day 15 (4.31±0.85 µg calcium/mg tissue), at day 30 (7.69±1.48 µg calcium/mg tissue), and at day 60 (20.50±1.22 µg calcium/mg tissue); and in the control group (GDA) at day 15 (7.34±1.32 µg calcium/mg tissue), at day 30 (9.28±0.76 µg calcium/mg tissue), at day 60 (27.60±1.08 µg calcium/mg tissue). Microscopic evaluation of the aortic cusp, showed a progressive calcification in those fixed with GDA. Such process was found partially in the GA 0.8% group, and totally absent in the E80% group. As for the assessment of the aortic wall segments, we also observed progressive calcification, which was not inhibited by the treatment with either GA 0.8% or E80%. CONCLUSIONS: We concluded that a pre-treatment with ethanol at 80% inhibited calcification in pig aortic cusps, however it was not as effective on the aortic wall. However, L-glutamic acid at 0.8% did show that it minimizes calcification in the aortic wall. Further studies are required, to evidence if the anti-calcifying action of ethanol 80% is kept if the pig aortic bioprostheses are implanted in the circulatory system.

OBJETIVO: Analisar a evolução clínica e funcional de 96 pacientes submetidos a substituição da valva aórtica por auto-enxerto pulmonar. CASUÍSTICA E MÉTODOS: De maio/95 a março/2000, 96 pacientes com média de idades de 25,4±11,4 anos foram consecutivamente submetidos a substituição da valva aórtica por auto-enxerto pulmonar. O diagnóstico pré-operatório mais comum foi de insuficiência aórtica de origem reumática e 89% encontravam-se em classe funcional II ou III. Todos realizaram ecocardiograma bidimensional com Doppler e 42 também foram submetidos a cateterismo cardíaco antes da operação. O auto-enxerto foi implantado pela técnica de substituição total da raiz aórtica em 85 casos, como cilindro intraluminal em seis e em posição subcoronariana nos demais. A reconstrução da via de saída do ventrículo direito foi feita com homoenxertos pulmonares (84) ou aórticos (12), conservados em solução nutriente com antibióticos (34) ou criopreservados (62). Antes da alta hospitalar, todos repetiram o exame ecocardiográfico e 30 foram submetidos a cateterismo cardíaco. Através deles, foi feita avaliação do desempenho hemodinâmico dos auto e homoenxertos, assim como da função e massa ventricular esquerda. No período de acompanhamento, os pacientes foram examinados e repetiram o ecocardiograma a cada seis meses. Vinte pacientes, com tempo de evolução superior a seis meses, submeteram-se a ecocardiografia de stress com dobutamina, para estudar o desempenho hemodinâmico dos auto e homoenxertos em condição de exercício. RESULTADOS: A mortalidade hospitalar foi de 6,2%. Após um tempo médio de 32,1 meses (1 - 58), 98,9% dos pacientes estão vivos. A incidência de pacientes livres de tromboembolismo, endocardite, disfunção do auto-enxerto, disfunção do homoenxerto, reoperações mitrais e de qualquer tipo de complicação após 41 meses é de 100%, 100%, 97,1%, 96,5%, 93,9% e 87,5%, respectivamente. O auto-enxerto pulmonar teve desempenho hemodinâmico fisiológico, com média de gradiente médio de 3,8±3,3 mmHg. O grau de insuficiência valvar foi negligenciável. Mesmo em condição de exercício, os gradientes não se elevaram de forma significativa, com média de gradiente médio de 6,8±3,8 mmHg. A função hemodinâmica dos homoenxertos foi excelente na fase imediata, com gradiente médio de 4,1±4,6 mmHg. Houve, entretanto, discreto aumento desses gradientes na fase tardia, com média de 13,9±10 mmHg. Durante o exercício, houve aumento significativo destes gradientes, que foram para 33,6±18,9 mmHg. Diâmetro do homoenxerto e idade do receptor tiveram correlação inversa com o gradiente pulmonar tardio. O índice de massa ventricular diminui de 180±68 g/m² no pré-operatório para 117±32 g/m² na fase tardia. A função ventricular no pós-operatório tardio foi normal tanto em repouso como em exercício na maioria dos pacientes. CONCLUSÕES: O implante do auto-enxerto pulmonar em posição aórtica pôde ser feito com baixa mortalidade imediata. A sobrevida tardia foi excelente, com baixa incidência de complicações. O desempenho hemodinâmico dos auto-enxertos pulmonares foi próximo ao fisiológico tanto em repouso como em exercício. Houve significativa regressão da massa ventricular esquerda, e, os índices de função ventricular também foram normais na fase tardia de pós-operatório. Apesar dos homoenxertos da via de saída do ventrículo direito apresentarem desempenho hemodinâmico adequado na fase imediata, o aparecimento de discretos gradientes na fase tardia foi freqüente.

OBJECTIVE: To evaluate the clinical and functional results of 96 patients submitted to aortic valve replacement with the pulmonary autograft. MATERIAL AND METHODS: From May/95 to March/2000, 96 patients with mean age of 25.4±11.4 years were consecutively submitted to aortic valve replacement with the pulmonary autograft. The most common preoperative diagnosis was rheumatic aortic valve insufficiency with 89% in functional class II or III. Bidimensional Doppler echocardiogram was performed in all patients and 42 were also submitted to cardiac catheterization before the operation. Pulmonary autograft was implanted as a complete root replacement in 85 patients, as a miniroot in six cases and in the subcoronary remainder position. Reconstruction of the right ventricular outflow tract was performed with pulmonary (84) or aortic homografts (12), which were stored in nutrient-antibiotic solution (34) or cryopreserved (62). Before hospital discharge, echocardiographic examination was repeated in all patients and cathterization in 30 of them in order to determine the hemodynamic performance of the auto and homografts, as well as left ventricular mass and function. During the follow-up period, patients were submitted to clinical evaluation and echocardiographic control every six months. Twenty patients with more than 6 months follow-up time were submitted to dobutamine stress echocardiography in order to determine the hemodynamic performance of the auto and homografts during exercise conditions. RESULTS: Hospital mortality was 6.2%. After a mean follow-up time of 32.1 months (1-58), 98.9% are alive. The incidence of freedom from thromboembolism, endocarditis, pulmonary autograft dysfunction, homograft dysfunction, mitral reoperations and of any kind of complications after 58 months is 100%, 100% 97.1%, 96.5%, 93.9% and 87.5% respectively. The hemodynamic performance of the pulmonary autografts was physiological, with an average mean gradient of 3.8±3.3. The degree of valvular regurgitation was negligible. Even during exercise conditions, gradients did not rise significantly, with average mean gradient of 6.8±3.8 mmHg. Hemodynamic function of the homografts was excellent during the immediate postoperative period, with average mean gradient of 4.1±4.6 mmHg. During late follow-up, however, there was a slight increase in these gradients with values of 13.9±10 mmHg. During exercise, these gradients rose significantly to 33.6±18.9 mmHg. Homograft diameter and patient age inversely correlated with late pulmonary gradients. Left ventricular mass index decreased from a preoperative value of 180±68 g/m² to 117±32 g/m² during late follow-up. Left ventricular mass and function was normal in the majority of patients in the late period. CONCLUSIONS: Aortic valve replacement with the pulmonary autograft was performed with a low operative mortality. Late survival was excellent, with a low incidence of complications. The hemodynamic performance of the pulmonary autografts at rest and during exercise was almost physiological. There was a significant regression of left ventricular mass and normal left ventricular function late postoperatively. Although the immediate hemodynamic performance of the right sided homografts was normal, mild gradients were frequently observed in the late period.

Fundamentos: A utilização de homoenxertos valvares aórticos criopreservados está associada a excelente qualidade de vida, com baixa morbidade e durabilidade satisfatória. Espera-se obter resultados semelhantes em posição mitral com o emprego de homoenxertos mitrais criopreservados. Objetivo: Avaliar os resultados imediatos e a curto prazo da substituição da valva mitral por homoenxerto mitral criopreservado. Casuística e Métodos: De julho/97 a fevereiro/98, 8 pacientes com média de idades de 40,3 ± 6,2 anos foram submetidos a substituição da valva mitral por homoenxerto mitral criopreservado. A técnica de implante consistiu de fixação látero-lateral dos músculos papilares, sutura anular contínua e anuloplastia com anel de Carpentier. Antes da alta, todos os pacientes realizaram ecocardiografia bidimensional com Doppler para análise da função valvar e ventricular. Os pacientes foram solicitados a retornar no primeiro mês e, subseqüentemente, a cada três meses de pós-operatório, para controles clínico e ecocardiográfico tardio. Resultados: Houve um óbito hospitalar de causa não relacionada ao enxerto. A avaliação ecocardiográfica antes da alta hospitalar demonstrou média das áreas valvares de 3,1 ± 0,6 cm2 e média dos gradientes médios de 3,5 ± 1,6 mmHg. A insuficiência valvar foi quantificada como inexistente ou trivial em 4 casos e leve em 3. A fração de ejeção de 57 ± 7% no pré-operatório foi adequadamente preservada no pós-operatório (62 ± 6%). Os níveis de hipertensão pulmonar regrediram significativamente de 87 ± 15 mmHg no pré-operatório para 48 ± 12 mmHg no pós-operatório. Houve também redução do tamanho da cavidade atrial esquerda de 61 ± 10 mm para 53 ± 7 mm. Nenhum paciente foi perdido do acompanhamento tardio. Após um tempo médio de seguimento de 4,1 ± 2,5 meses, os pacientes encontram-se funcionalmente bem e sem complicações pós-operatórias. O estudo ecocardiográfico tardio demonstra a persistência dos bons resultados imediatos. Conclusões: Os resultados imediatos e a curto prazo da substituição da valva mitral por homoenxerto mitral criopreservado foram bastante satisfatórios. Somente com tempos mais prolongados de observação poderemos determinar a durabilidade desse enxerto e, eventualmente, expandir as suas indicações.

Background: the use of cryopreserved aortic valve homografts is associated with excellent quality of life, low morbidity and satisfactory durability. We expect to achieve similar results in the mitral position with the use of cryopreserved mitral homografts. Objectives: Evaluate the immediate and short-term results of mitral valve replacement with cryopreserved mitral homografts. Material and Methods: Between July/97 and February/98, 8 patients with a mean age of 40.3 ± 6.2 years were submitted to mitral valve replacement with cryopreserved mitral homografts. Operative technique consisted of latero-lateral papillary muscle fixation, a running continuous suture at annulus level and annuloplasty with a Carpentier ring. Before hospital discharge, all patients were submitted to Doppler echocardiographic control for assessment of valvar and ventricular function. Patients were requested to return at the first and subsequently every 3 months postoperatively for further clinical and echocardiographic control. Results: There was one early non valve-related death. Echocardiographic evaluation before hospital discharge revealed a mean mitral valve area of 3.1 ± 0.6 cm2 and a mean gradient of 3.5 ± 1.6 mmHg. Valvar insufficiency was graded as non-existent or trivial in four cases and mild in the remaining three patients. Ejection fraction which was 57 ± 7% pre-operatively was well preserved in the postoperative period (62 ± 6%). Pulmonary hipertension reduced significantly from 87 ± 15 mmHg pre-operatively to 48 ± 12 mmHg post-operatively. There was also a reduction in the left atrial cavity from 61 ± 10 mm to 53 ± 7 mm. No patient was lost to follow-up. After a mean follow-up time 4.1 ± 2.5 months, all patients are functionally well without postoperative events. Late echocardiographic control showed persistence of the good immediate results. Conclusions: The immediate and short-term results of mitral valve replacement with mitral homografts are satisfactory, demonstrating the feasibility of the technique. Longer follow-up periods are necessary to determine durability of this graft and to eventually expand its indications.

A hipotermia profunda com parada circulatória total (PCT) apresentou um aumento importante nos pacientes operados em nosso Serviço e é de grande utilidade para a correção de alterações cardiovasculares complexas. Estudamos 130 pacientes de janeiro de 1995 a junho de 1996. A idade dos pacientes variou de 2 dias a 16 anos. As doenças mais freqüentes foram o defeito do septo ventricular (DSV) (26%), tetralogia de Fallot (TF) (13%), transposição das grandes artérias (TGA) (10%) e defeito do septo atrioventricular (DSAVT) (10%). Em todos os pacientes foi usado o oxigenador de membranas, a temperatura retal em PCT foi mantida em média de 15°C. O tempo médio em perfusão foi de 45 minutos e o de PCT de 27 minutos. No pós-operatório imediato, a complicação mais freqüente foi a síndrome de baixo débito (49 casos - 38%). Alterações neurológicas ocorreram em 27 (20%) pacientes: 13 (10%) convulsões, 8 (6,1%) disfunções neuropsicológicas, 5 (3,8%) sinais extrapiramidais, 4 (3%) déficits neurooftálmicos, 4 (3%) comas. Cinco destes sintomas iniciaram precocemente no POI e podem ser atribuídos à PCT (3,8%). Ocorreram 20 (15%) óbitos, 3 pacientes apresentaram déficit neurológico na alta hospitalar (2 disartrias - 1 hipertonia muscular). Concluímos que a hipotermia profunda com PCT é útil na correção de anomalias cardíacas complexas e que apresenta baixo risco de complicações neurológicas permanentes. O nosso seguimento tardio é pequeno para avaliar com precisão o desenvolvimento psicomotor dos pacientes.

Deep hypothermic circulatory arrest (DHCA) increased the number of pediatric patients operated on our Service and it is an important aid for the correction of complex cardiovascular procedures. We studied 130 patients from January, 1995 to June, 1996. Patient ages ranged from 2 days to 16 years. The most frequent pathology was VSD (26%), tetralogy of Fallot (13%), TGA (10%) and AVSD (10%). All patients used membrane oxygenators, rectal temperature was maintained at a mean of 15,4°C. Mean perfusion time was 45 minutes. In the early postoperatory stage, low output was the most frequent complication (49 - 38%). Neurological changes occured in 27 patients (20%): 13 seizures (10%), 8 neuropsychologic disfunction (6.1%), 5 extrapyramidal signs (3,8%), 4 neuro-ophthalmic deficits (3%), 4 comas. 5 of these symptoms appeared early in the postoperative period and could be attributed to DHCA (3.8%). There were 20 (15%) deaths. 3 had a permanent neurologic deficit on discharge (2 dysartrias and 1 muscular hypertonya). We concluded that DHCA is useful for the correction of complex cardiac anomalies with a low risk of neurologic complications permanent. Our late follow up is too short for precise analysis of psycomotor development in these patients.

Uma série de registros do eletrograma intracavitário tem sido utilizada para monitorização não invasiva da rejeição em pacientes transplantados usando um marcapasso de dupla câmara e eletrodos endocavitários revestidos com estrutura fractal. Os sinais têm sido avaliados usando o sistema CHARM (Computerized Heart Acute Rejection Monitoring _ sistema computadorizado para monitorização da rejeição cardíaca aguda). Os relatórios obtidos com este sistema contêm curvas com parâmetros sensíveis à rejeição, que demonstram uma boa correlação com a clínica e os resultados das biópsias convencionais. A monitorização a longo prazo, usando estas análises, mostrou ser uma ferramenta valiosa no acompanhamento destes pacientes.

Serial intramyocardial electrogram recordings have been performed for noninvasive rejection monitoring in patients after heart transplantation using implanted telemetric dual-chamber pacemaker and fractally coated endocardial loads. The signals have been evaluated using the CHARM (Computerized Heart Acute Rejection Monitoring) system. The reports containing the trend curves of the rejection sensitive parameters show good correlation with the patients clinical course and the results of endomyocardial biopsy. Long-term cardiac transplant monitoring using intramyocardial electrograms is a valuable tool for clinical patient management.

Atualmente, na constante busca de minimizar o tempo de intemação hospitalar e melhorar a recuperação no pós-operatório, várias altemativas cirúrgicas têm sido aventadas. Iniciamos o uso da estemotomia parcial como acesso para diversos tipos de operações cardíacas. O presente trabalho tem como objetivo apresentar a evolução hospitalar dos pacientes operados através desta técnica. No período de novembro de 1996 a março de 1997, estudamos 51 pacientes operados através da esternotomia parcial. A idade média foi de 46,8 anos, sendo 26 pacientes do sexo feminino. Os procedimentos mais realizados foram o tratamento cirúrgico valvar e as revascularizações miocárdicas. O acesso utilizado para os pacientes com lesões valvares foi a esternotomia parcial em "T" invertido no segmento superior do esterno; para outras lesões, uma esternotomia em "T" no segmento inferior e, pela dificuldade técnica imposta nas re-operações e nos procedimentos múltiplos, utilizou-se uma terceira variação, que foi a esternotomia parcial em "H" deitado, estendendo-se no corpo esternal do primeiro ao quarto espaço intercostal. O comprimento médio da incisão de pele foi de 9,9 cm. Foram realizadas 19 incisões em "T", 17 em "T" invertido e 15 em "H" deitado. O tempo médio de ventilação mecânica foi de 2,8 horas, de permanência na UTI de 31,5 horas e de internação hospitalar de 5,9 dias. Não houve complicação diretamente relacionada com o acesso e em apenas 1 caso foi necessária a conversão para a esternotomia total. Analisando a evolução destes 51 pacientes, pudemos concluir que a esternotomia parcial é um acesso seguro para o tratamento cirúrgico de diversas cardiopatias, isoladas, associadas ou re-operações. Traz um resultado estético favorável e facilita a recuperação no pós- operatório, devendo fazer parte do repertório de todo o cirurgião cardiovascular.

Recently, in the constant search for shortening hospital stay and emproving postoperative recovery, many surgical alternatives have been used. We started the use of a partial sternotomy as an approach to different types of cardiac operations. This paper aims to show the hospital outcome of patients submitted to this technique. From November 1996 to March 1997, 51 patients who underwent partial sternotomy were studied. The mean age of the patients was 46.8 years with 26 females. The most common procedures were valve surgeries followed by myocardial revascularization. The approach used in the patients with valve pathologies was a partial sternotomy in an inverted "T" incision in the superior segment of the sternum, for other pathologies a sternotomy in a "T" incision in the inferior segment was the choice and due to technical difficulties imposed by reoperations and in multiple procedures a third variation, partial sternotomy in an "H" incision was used. In the latter, the sternum was cut from the first to the fourth intercostal spaces. The average size of the skin incision was 9.9 cm. There have been 19 "T" incisions, 17 inverted "T" incisions and 15 "H" incisions. The average assisted ventilation time was 2.8 hours, the average ICU stay was 31.5 hours and the average hospital stay was 5.9 days. No complications resulted directly from the approach itself and in only one case was conversion to total sternotomy necessary. After analyzing the outcome of these 51 patients, we can conclude that partial sternotomy is a safe approach for the treatment of several isolated or associated heart diseases reoperations. It achieves a favorable aesthetic result and achieves postoperative recovery. Therefore, it should be part of every heart-surgeon's repertoire.

Fundamentos: Próteses valvares aórticas com desempenho hemodinâmico adequado possibilitariam maior regressão da hipertrofia ventricular e normalização da função ventricular. Isto possivelmente tenha implicações importantes no prognóstico tardio após a substituição da valva aórtica. Objetivo: Avaliar o desempenho hemodinâmico do auto-enxerto pulmonar em posição aórtica e a regressão da hipertrofia ventricular esquerda após a operação de Ross. Casuística e Métodos: De maio/95 a março/97, 45 pacientes com média de idades de 27,1 anos foram submetidos à operação de Ross. Todos os pacientes foram submetidos, no pós-operatório imediato, a ecocardiografia com Doppler e cateterismo cardíaco para avaliação do desempenho hemodinâmico dos auto e homoenxertos, assim como da massa e função ventricular esquerda. Catorze pacientes com evolução superior a seis meses submeteram-se a ecocardiografia de stress com dobutamina, para estudar o desempenho hemodinâmico dos auto e homoenxertos em condição de exercício. Resultados: A mortalidade hospitalar foi de 6%. Após tempo médio de seguimento de 12,8 meses (1-23), ocorreu um óbito súbito tardio e nenhuma complicação relacionada à prótese. O desempenho hemodinâmico imediato e tardio dos auto-enxertos foi praticamente normal, com média de gradiente médio de 1,8 ± 0,6 mmHg e média de gradiente instantâneo máximo de 2,9 ± 0,9 mmHg. O grau de insuficiência valvar foi desprezível. Mesmo em condição de exercício, os gradientes não se elevaram de forma significativa, com média de gradiente médio de 4,3 ± 2,5 mmHg e média de gradiente instantâneo máximo de 10,4 ± 6,1 mmHg. Os homoenxertos utilizados para a reconstrução da via de saída do ventrículo direito tiveram excelente desempenho hemodinâmico imediato; entretanto, no seguimento tardio apresentaram discreto aumento das velocidades de fluxo com média de gradiente médio de 10 ± 7,1 mmHg em repouso e 26 ± 13,3 mmHg durante o exercício. O índice de massa ventricular esquerda caiu de 168 ± 46g/m² no pré-operatório para 115 ± 32g/m² no 6º mês de evolução. A função ventricular esquerda apresentou-se normal em repouso e no exercício na maioria dos pacientes. Conclusões: Dado o desempenho hemodinâmico normal dos auto-enxertos pulmonares, a redução da massa ventricular e normalização da função do ventrículo esquerdo, além da ótima evolução tardia dos pacientes, consideramos a operação de Ross como a operação ideal para pacientes aórticos jovens.

Background: Aortic valve prosthesis with adequate hemodynamic performance should allow more complete left ventricular mass regression and return left ventricular function back to normal. This possibly affects long term prognosis after aortic valve replacement. Objective: Assessment of hemodynamic performance of the pulmonary autograft in the aortic position and the regression of left ventricular mass after the Ross procedure. Material and Methods: Between May/95 and Mar/96, 45 patients with mean age of 27.1 years were submitted to a Ross procedure. Doppler echocardiography and cardiac catheterization were performed on all patients before hospital discharge to analyze the hemodynamic performance of the auto and homografts employed, as well as to evaluate left ventricular mass and function. Fourteen patients with follow-up longer than six months were sumitted to dobutamine stress echocardiography to study hemodynamic performance of the auto and homografts at during exercise. Results: Hospital mortality was 6%. After a mean follow-up of 12.8 months (1-23), there was one late sudden death. No valve related event was noted during this period. Immediate and late hemodynamic performance of the pulmonary autografts were normal with average mean gradients of 1.8 ± 0.6 mmHg and average maximum instantaneous gradients of 2.9 ± 0.9 mmHg. Valvular insufficiency was almost null. Even during exercise, gradients did not increase significantly with average mean gradients of 4.3 ± 2.5 mmHg and average maximum gradients of 10.4 ± 6.1 mmHg. Homografts used for right ventricular reconstruction exhibited excellent immediate hemodynamic performance. However, at late follow-up an increase in flow velocities was noted with an average of mean gradients of 10 ± 7.1 mmHg at rest and 26 ± 13.3 mmHg at exercise. Left ventricular mass index dropped from 168 ± 46 g/m² preoperatively to 115 ± 32 g/m² six months after the operation. Left ventricular function was normal at rest and during exercise in the marjority of patients. Conclusions: Given the normal hemodynamic performance of the autografts, the important decrease in left ventricular mass and the normal left ventricular function late postoperatively, the Ross procedure is considered the operation of choice for young patients who need aortic valve replacement.
