Resumo Contexto A aferição do diâmetro venoso e a escolha de um filtro de veia cava compatível são fundamentais para diminuir o risco de complicações decorrentes do implante desses dispositivos. Entretanto, são escassas as informações sobre como o diâmetro da cava inferior varia de acordo com o sexo e a idade. Objetivos Determinar a influência do sexo e da idade dos pacientes sobre o diâmetro da cava inferior e a adequação dos diferentes modelos de filtro disponíveis. Métodos Estudo analítico retrospectivo, realizado a partir de imagens de tomografia computadorizada. O diâmetro no segmento infrarrenal da veia cava inferior foi aferido em três pontos (cranial, médio e caudal). Os resultados foram classificados de acordo com o sexo e as faixas etárias. Resultados Foram analisadas tomografias de 417 pacientes: 245 mulheres e 172 homens. Os diâmetros nos pontos médio e caudal foram, respectivamente, 19,1 mm e 20,6 mm em mulheres de 81 a 92 anos, sendo estatisticamente menores (p < 0,05) quando comparados aos de mulheres com idade entre 19 e 40 anos (diâmetro no ponto médio: 22,7 mm; diâmetro no ponto caudal: 23 mm). Resultados semelhantes foram observados em homens. Os diâmetros venosos nos pontos cranial e caudal foram estatisticamente maiores em homens (ponto cranial: 24,4 mm; ponto caudal: 22,3 mm) do que em mulheres (ponto cranial: 22,6 mm; ponto caudal: 20,8 mm) em pacientes com idade entre 51 e 70 anos (p < 0,05). Conclusões O diâmetro da veia cava inferior foi menor em pacientes com idade mais avançada em ambos os sexos, e a taxa de variação do diâmetro foi semelhante entre homens e mulheres.

Abstract Background Measuring the venous diameter and choosing a compatible vena cava filter are essential to reduce the risk of complications resulting from implantation of these devices. However, there is little information on how the diameter of the inferior vena cava varies with sex and age. Objectives To determine the influence of patients’ gender and age on their inferior vena cava diameter and the suitability of the different models of available filters. Methods Retrospective analytical study based on computed tomography images. The diameter of the inferior vena cava was measured at 3 points: above the confluence of the common iliac veins, below the renal veins, and midway between these two points (cranial point, caudal point, and midpoint) using Arya® and Carestream PACS® software. The results were classified by sex and age groups. Results CT scans of 417 patients were analyzed: 245 women and 172 men. The diameters at the midpoint and caudal point were, respectively, 19.1 mm and 20.6 mm in women from 81 to 92 years old and were statistically smaller (p< 0.05) when compared to women aged 19 to 40 years (midpoint: 22.7 mm; caudal point: 23 mm). Similar results were seen in men. Venous diameters at the cranial and caudal points in patients aged from 51 to 70 years were statistically larger in men (cranial point: 24.4 mm; caudal point:22.3 mm) than in women (cranial point: 22.6 mm; caudal point:20.8 mm) (p< 0.05). Conclusions A smaller diameter was found for the inferior vena cava in older patients of both sexes and the rate of diameter change was similar among men and women.

Resumo Contexto Os aneurismas de aorta abdominal (AAAs) são os mais comuns. A incidência anual de ruptura do AAA é de oito casos por 100.000 habitantes. A detecção incidental pode beneficiar o paciente, desde que o diâmetro seja monitorado e o paciente receba o tratamento adequado. Objetivos Estimar a prevalência do diagnóstico incidental de aneurisma de aorta torácica (AAT) e de AAA em tomografias computadorizadas (TCs); avaliar a prevalência de sexo e idade dos pacientes e determinar quais as artérias acometidas e as características morfológicas dos aneurismas; determinar quais as indicações de TC mais associadas ao diagnóstico incidental de aneurismas. Métodos Estudo descritivo, retrospectivo e randomizado. Critérios de inclusão: pacientes com 50 anos ou mais submetidos a TC de tórax, abdome ou pelve. Critérios de exclusão: acompanhamento ou suspeita diagnóstica de aneurismas. Foram utilizados protocolos com questões sobre dados demográficos e anatômicos. Resultados Foram analisados 1.202 laudos radiológicos. Detectados 27 aneurismas (prevalência de 2,2%). Pacientes: 60% do sexo masculino e 40% do sexo feminino (p < 0,05). Localização: 13 casos (48,2%) na aorta ascendente (AAT); 7 (25,9%) na aorta infrarrenal (AAA); 2 (7,4%) aorta na transição toracoabdominal (ATA); 2 (7,4%) na ilíaca comum; 1 (3,7%) na ilíaca interna; 1 (3,7%) na artéria esplênica; e 1 (3,7%) na artéria renal. Conclusões A maioria dos pacientes foi do sexo masculino (60%); houve maior frequência de AAT (diâmetro médio de 4,1 cm), seguido de AAA (diâmetro médio de 4,0 cm) e ATA (diâmetro médio de 3,9 cm). A principal indicação para a realização de TC associada ao diagnóstico incidental de aneurismas foi em função de sintomas respiratórios.

Abstract Background Abdominal aortic aneurysms (AAAs) are the most common. The annual incidence of AAA rupture is eight cases per 100,000 inhabitants. Incidental detection can benefit patients if diameter is monitored and the correct treatment provided. Objectives To estimate the prevalence of incidental diagnosis of thoracic aortic aneurysm (TAA) and AAA by computed tomography (CT); to determine prevalence by age and sex of patients and determine which arteries are involved and the morphological characteristics of the aneurysms; to determine the indications for CT most associated with incidental diagnosis of aneurysms. Methods This was a descriptive, retrospective and randomized study. Inclusion criteria: patients over 50 years of age examined with CT of the thorax, abdomen or pelvis. Exclusion criteria: follow-up or diagnostic suspicion of aneurysms. Data collection instruments containing questions covering demographic and anatomic data were used. Results A sample of 1,202 radiology reports was reviewed. A total of 27 aneurysms were detected (prevalence of 2.2%). Patients: 60% were male and 40% were female (p < 0.05). Sites: there were 13 cases (48.2%) in the ascending aorta (TAA); 7 (25.9%) in the infrarenal aorta (AAA); 2 (7.4%) at the thoracoabdominal transition of the aorta (TTA); 2 (7.4%) in the common iliac artery ; 1 (3.7%) in the internal iliac artery; 1 (3.7%) in the splenic artery; and 1 (3.7%) in the renal artery. Conclusions The majority of patients were male (60%); TAA was the most common type (mean diameter: 4.1 cm), followed by AAA (mean diameter: 4.0 cm) and ATA (mean diameter: 3.9 cm). The most common indication for the CT examination that led to incidental diagnosis of an aneurysm was respiratory symptoms.

Os autores descrevem o tratamento de um paciente portador de fístula arteriovenosa entre vasos poplíteos com mais de 20 anos de evolução, após ferimento por projétil de arma de fogo. O paciente foi submetido a tratamento endovascular pelo implante de stents revestidos Viabahn (Gore®), porém o mesmo não foi bem sucedido devido à diferença de diâmetro entre a artéria poplítea proximal e a distal à fístula. A artéria femoral superficial foi submetida à cerclagem com fios de sutura ao redor do stentgraft previamente implantado. Esse recurso de improvisação permitiu uma abordagem distante do sítio da fístula, diminuindo os riscos do acesso cirúrgico em uma região com distorções anatômicas e vasos sanguíneos ectasiados, o que certamente acarretaria maior risco de lesões iatrogênicas.

The authors describe treatment of a patient who presented an arteriovenous fistula between the popliteal vessels more than 20 years after a gunshot wound. The patient underwent endovascular treatment using Viabahn (Gore®) stent grafts, but, because of the large disparity in popliteal artery diameters proximal and distal of the fistula, the endovascular treatment was unsuccessful. The superficial femoral artery was then banded around the stent graft that was deployed previously. This improvised strategy allowed an open surgical approach to be performed far from the fistula site, reducing the risks of operating in a region with anatomic distortions and significant enlargement of the surrounding venous structures, which would certainly have increased the likelihood of iatrogenic injuries.
