INTRODUCTION: Studies comparing high frequency oscillatory and conventional ventilation in acute respiratory distress syndrome have used low values of positive end-expiratory pressure and identified a need for better recruitment and pulmonary stability with high frequency. OBJECTIVE: To compare conventional and high frequency ventilation using the lower inflection point of the pressure-volume curve as the determinant of positive end-expiratory pressure to obtain similar levels of recruitment and alveolar stability. METHODS: After lung lavage of adult rabbits and lower inflection point determination, two groups were randomized: conventional (positive end-expiratory pressure = lower inflection point; tidal volume=6 ml/kg) and high frequency ventilation (mean airway pressures= lower inflection point +4 cmH2O). Blood gas and hemodynamic data were recorded over 4 h. After sacrifice, protein analysis from lung lavage and histologic evaluation were performed. RESULTS: The oxygenation parameters, protein and histological data were similar, except for the fact that significantly more normal alveoli were observed upon protective ventilation. High frequency ventilation led to lower PaCO2 levels. DISCUSSION: Determination of the lower inflection point of the pressure-volume curve is important for setting the minimum end expiratory pressure needed to keep the airways opened. This is useful when comparing different strategies to treat severe respiratory insufficiency, optimizing conventional ventilation, improving oxygenation and reducing lung injury. CONCLUSIONS: Utilization of the lower inflection point of the pressure-volume curve in the ventilation strategies considered in this study resulted in comparable efficacy with regards to oxygenation and hemodynamics, a high PaCO2 level and a lower pH. In addition, a greater number of normal alveoli were found after protective conventional ventilation in an animal model of acute respiratory distress syndrome.

OBJETIVOS: Revisar as indicações, doses e formas de administração dos sedativos, analgésicos e relaxantes musculares mais utilizados na criança, bem como os métodos de monitorização da sedação. FONTES DOS DADOS: Levantamento bibliográfico utilizando a base de dados MEDLINE e revisão da experiência em nossas unidades de cuidados intensivos pediátricos. SÍNTESE DOS DADOS: A administração contínua de drogas analgésicas e sedativas impede o aparecimento das fases de subsedação e requer menor assistência do que na administração intermitente. O midazolan é a droga mais utilizada para sedação contínua da criança gravemente enferma. Os derivados opiáceos e os antiinflamatórios não-hormonais são os analgésicos mais utilizados na criança gravemente enferma. Os opióides associados aos benzodiazepínicos em infusão contínua são os fármacos de eleição em crianças em ventilação mecânica, especialmente a morfina e o fentanil. O uso de protocolos e a monitorização com a utilização de escores clínicos e métodos objetivos como o BIS permitem ajustar mais corretamente a medicação, evitando a supersedação, a subsedação e a síndrome de abstinência. As intervenções não-farmacológicas, como a musicoterapia, o controle de ruídos, a adequada utilização da luz, a massagem e a comunicação com o paciente, são medidas complementares que auxiliam na adaptação da criança ao ambiente hospitalar adverso. CONCLUSÕES: A sedação deve ser adaptada a cada criança em cada momento. O emprego de protocolos que facilitem uma correta seleção de fármacos, uma administração adequada e uma monitorização cuidadosa melhoram a qualidade da sedoanalgesia e reduzem seus efeitos adversos.

OBJECTIVES: To review the most frequent recommendations, doses and routes of administration of sedatives, analgesics, and muscle relaxants in children, as well as the methods for monitoring the level of sedation. SOURCES: Review of the literature using the MEDLINE database and review of the experience in pediatric intensive care units. SUMMARY OF THE FINDINGS: The continuous administration of analgesics and sedatives prevents the development of undersedation and is less demanding in terms of care than intermittent administration. Midazolam is the most commonly used drug for continuous sedation of critically ill children. Opioid derivatives and nonsteroidal anti-inflammatory drugs are the most widely used analgesics in critically ill children. Opioids combined with benzodiazepines, given in continuous infusion, are the drugs of choice in mechanically ventilated children, especially morphine and fentanyl. The use of protocols and monitoring through clinical scores and objective methods (e.g. bispectral index) allow adjusting medication more appropriately, preventing oversedation, undersedation, and the withdrawal syndrome. Non-pharmacological interventions, such as music therapy, noise control, adequate use of light, massage, conversation with the patient, are ancillary measures that help children to adapt to the adverse hospital environment. CONCLUSIONS: Sedation should be tailored to each child for each specific situation. Protocols that facilitate the correct selection of drugs, their appropriate administration and careful monitoring improve the quality of sedation and analgesia and avoid their adverse effects.

OBJETIVO: Revisar o estágio atual do conhecimento sobre a utilização do surfactante exógeno nas diferentes doenças pulmonares que levam à insuficiência respiratória aguda em crianças. FONTES DOS DADOS: Este manuscrito baseia-se na experiência clínica dos autores sobre o assunto e na revisão da literatura recente através de consulta aos bancos de dados ONIA, Mdconsult, Medline e Cochrane Database Library. SÍNTESE DOS DADOS: Apesar do sucesso obtido com a utilização do surfactante exógeno na síndrome de desconforto respiratório do recém-nascido, questões permanecem indefinidas, como o momento do seu emprego, muito precoce (profilático), baseado na idade gestacional ou em testes rápidos de maturidade pulmonar, ou então mais tardiamente, após o quadro clínico instalado. Em outras patologias graves que levam à insuficiência respiratória grave com necessidade de suporte ventilatório, o seu uso ainda é controverso, e os dados da literatura são limitados e conflitantes. Porém, relatos de uso clínico em várias destas situações, com sucesso, têm sido freqüentes. A pesquisa em surfactante tem-se centrado ultimamente na sua inativação por várias substâncias que podem estar presentes na via aérea. Nas patologias em que a inativação parece ser um fator importante, novos surfactantes com adição de adjuvantes para reverter a tendência à inativação (por exemplo: polietilenoglicol) estão atualmente em fase de testes. CONCLUSÕES: A terapia com surfactante exógeno ainda não é um tema esgotado, nem mesmo na SDR. Os surfactantes podem ser ainda aperfeiçoados, sobretudo para resistir à inibição, e as formas de utilização em outras doenças que não a SDR deverão ser aperfeiçoadas.

OBJECTIVE: To review current knowledge about the use of exogenous surfactants in the treatment of different lung diseases causing acute respiratory failure in children. SOURCE OF DATA: This review is based on the authors' experience and on recent data retrieved from ONIA, Mdconsult, Medline and the Cochrane Database Library. SUMMARY OF THE FINDINGS: In spite of the success of the use of exogenous surfactants in Respiratory Distress Syndrome (RDS) of the newborn, some questions remain unanswered, such as the optimal administration timing - either very early (prophylactic), based on gestational age or on quick tests of lung maturity, or later, when the clinical picture becomes unequivocal. In other severe diseases requiring ventilatory support, the use of surfactants is still controversial, and data in the literature are limited and conflicting. However, successful use in several other diseases has been reported. Recent studies have focused on surfactant inactivation by substances that can be found in the airways. New surfactants with the addition of substances to control inhibition, such as polyethyleneglycol, are being tested for diseases in which inactivation seems to be a significant factor. CONCLUSIONS: Therapy with exogenous surfactants, even as a treatment for RDS, has not been thoroughly investigated. Further studies should be conducted to improve surfactants - mainly their resistance to inhibition - and the treatment of diseases other than RDS.
