ABSTRACT Purpose: to analyze the use of digital resources by Brazilian phonoaudiologists before and during the period of the COVID-19 pandemic. Methods: cross-sectional study carried out with a representative sample of Brazilian phonoaudiologists, with at least one year of practical experience. An online questionnaire consisting of 28 questions divided into eight sections was prepared. Data were analyzed and are reported as absolute and relative frequency regarding the variables: respondent’s age, highest degree and years of experience; weekly working hours; region of the country; type of service and area of expertise; use of digital means before and during the pandemic and willingness to use the same kind of resource after the pandemic. Results: telehealth technologies were used by 90% of phonoaudiologists in Brazil since before the COVID-19 pandemic. A change in the profile of technologies used was observed: after the pandemic, videoconferences gained more use in contact with patients compared to the pre-pandemic period even though contact via mobile phone and instant messaging applications are the most used, both before and during the pandemic. Conclusion: the study's findings demonstrate that 90% of phonoaudiologists who answered the questionnaire in Brazil used and still make use of telehealth technologies to contact patients.

The COVID-19 pandemic brought challenges and the need for adaptation on several fronts. Updating the ways of working is necessary and promoting education and care in the health sector in line with the 21st century is essential. In addition to undergraduate students training in Speech-Language Pathology (SLP) field, based on the necessary opportunities for practical experience with patients during undergraduation, a significant portion of the population is attended by school clinics linked to universities and training centers, which work in conjunction with public health services. In this text we will bring some reflections on the relevance of using telehealth technologies for speech therapists training, considering the articulation between national curricular guidelines, professional training and ethical aspects.

A pandemia da COVID-19 trouxe desafios e necessidade de adaptação em diversas frentes. A atualização das formas de trabalho é necessária e promover ensino e atendimento no setor saúde condizentes com o século XXI é fundamental. Não estamos apenas tratando da formação de graduandos na área de Fonoaudiologia, mas do atendimento clínico a uma parcela significativa da população que depende dos serviços de clínicas-escola vinculadas às universidades e centros de formação em nossa profissão.Neste texto traremos algumas reflexões sobre a relevância do uso das tecnologias de telessaúde para a formação de fonoaudiólogos, considerando a articulação entre as diretrizes curriculares nacionais, a formação profissional e aspectos éticos.

RESUMO Objetivo Analisar as respostas parentais a perguntas que investigam sinais clássicos de autismo, em dois instrumentos diferentes: Questionário de Indicadores de Risco para o Desenvolvimento Infantil (IRDI-questionário) e Modified Checklist for Autism in Toddlers (M-Chat). Métodos Quarenta e uma crianças, sendo 80% meninos, com média de idade de 2 anos e 8 meses foram avaliadas com dois instrumentos de rastreamento de autismo, recomendados pelo Ministério da Saúde. Após a aplicação integral dos instrumentos, foram selecionadas sete perguntas que compõem os instrumentos e ilustram emblematicamente sinais clássicos de transtorno do espectro do autismo (TEA), respondidas pelos pais dos sujeitos, para posterior análise. As crianças avaliadas não tinham qualquer diagnóstico fechado de TEA ou outros transtornos. Resultados Os principais preditores de importância foram questões sobre brincar de “faz de conta”, interesse da criança por outras crianças, resposta da criança ao “manhês” e troca de olhares entre mãe e criança. Conclusão Nem todas as perguntas que abordam os sinais típicos de autismo mostraram-se bons preditores de importância na análise realizada. Há necessidade de analisar o conjunto de sinais e não apenas sinais isolados, quando se está diante de uma criança com suspeita de TEA.

ABSTRACT Purpose To analyze the parental responses to questions investigating classic signs of autism using two different instruments (IRDI-questionnaire and M-Chat). Methods Forty-one children, 80% male, with a mean age of 2 years and 8 months, who were evaluated with two autism screening tools recommended by the Brazilian Ministry of Health. After administration of both tools, seven questions were selected to make up the instruments and illustrate symbolically the classic signs of the Autistic Spectrum Disorder (ASD), answered by the parents of the subjects for further analysis. The subjects did not have a formal diagnosis of ASD, or any other diagnosis. Results The main predictors of importance were questions about “make-believe” playing, interest of the child in other children, the child’s response to “motherese”, and exchange of glances between mother and child. Conclusion Not all questions referring to the typical signs of autism showed to be good predictors of importance in the conducted analysis. The data lead us to reflect on a need to analyze a set of signs and not only isolated signs when facing a child with suspected ASD.

Objetivo Avaliar a sensibilidade do IRDI-questionário para pais, para rastreamento de transtornos do espectro do autismo. Métodos Participaram do estudo 72 sujeitos, familiares de crianças na faixa etária de 2 anos e 11 meses a 7 anos e 7 meses, divididos em dois grupos: grupo pesquisa e grupo controle. O IRDI-questionário foi aplicado nos dois grupos estudados e a CARS-BR foi aplicada no grupo pesquisa. Os dados foram submetidos à análise estatística e, para a validade, foi realizado o coeficiente de correlação de Pearson, entre o IRDI-questionário e a CARS-BR, somente para o grupo pesquisa. Na definição do ponto de corte do escore gerado pelo instrumento IRDI-questionário, foi utilizada a curva ROC e calculados os valores de sensibilidade, especificidade e valor preditivo positivo. Na análise de concordância para o IRDI-questionário, foi utilizado o coeficiente de Kappa e concordância total. Resultados O IRDI-questionário mostrou-se de fácil aplicação, rapidez no preenchimento e baixo custo. A análise da consistência interna das questões do instrumento apresentou bom índice. Na comparação com a CARS-BR, apresentou correlação positiva. A análise da curva ROC identificou bom desempenho. Conclusão A avaliação da sensibilidade do instrumento, para fins de rastreamento de transtornos do espectro do autismo, mostrou bons índices, a partir do ponto de corte estabelecido no estudo.

Purpose To assess the sensitivity of the Clinical Risk Indicators for Early Childhood Development (IRDI) questionnaire for parents in identifying autism spectrum disorders. Methods Seventy-two subjects, which comprised parents of children aged 2 years 11 months to 7 years 7 months, participated in the study divided into two groups, namely the study and control groups. The IRDI questionnaire was administered in both groups, and the CARS-BR was applied in the study group. The data were subjected to statistical analysis, and the Pearson correlation coefficient between the IRDI questionnaire and CARS-BR was performed only for the study group to calculate validity. In defining the cutoff score generated by the IRDI questionnaire, the receiver-operating characteristic (ROC) curve was used, and sensitivity, specificity, and positive predictive value were calculated. In the reliability analysis for the IRDI questionnaire, the Kappa coefficient and overall concordance was used. Results The IRDI questionnaire was easy to use, quick to complete, and inexpensive. Analysis of the internal consistency of the questions of the instruments presented good content. The IRDI questionnaire was positively correlated with the CARS-BR. The ROC curve analysis revealed good performance. Conclusion The evaluation of the sensitivity of the instrument for purposes of identifying autism spectrum disorders showed good performance according to the established cutoff point in the study.

OBJETIVO: Adaptar o instrumento Indicadores Clínicos de Risco para o Desenvolvimento Infantil (IRDI) para questionário retrospectivo para pais de crianças de 3 a 7 anos e aplicá-lo em dois grupos de sujeitos. MÉTODOS: Participaram do estudo 72 sujeitos - pais de crianças de 2 anos e 11 meses a 7 anos e 7 meses - divididos em dois grupos: Grupo Pesquisa e Grupo Controle. A proposta de modificação do instrumento para questionário transformou os 31 indicadores (afirmações) do primeiro em perguntas de caráter retrospectivo, dirigidas aos pais em formato autoaplicável, com respostas em escala Likert. O IRDI-questionário foi aplicado nos dois grupos estudados. RESULTADOS: O IRDI-questionário mostrou-se de fácil aplicação, com rapidez no preenchimento (tempo médio de 15 minutos) e baixo custo. A análise das questões que compõem o instrumento revelou boa consistência interna. A comparação entre os grupos, segundo as respostas dos pais às 31 questões do IRDI-questionário, mostrou diferença em 16 delas. CONCLUSÃO: Foi possível verificar que os itens do questionário recobrem os principais sinais de risco para Transtorno do Espectro do Autismo apontados na literatura. Além disso, as respostas dos pais assinalaram que houve diferença entre os grupos estudados.

PURPOSE: To adapt the Child Development Clinical Risk Indicators (IRDI) instrument to retrospective report for parents of children from 3 to 7 years old and administer it in two groups. METHODS: Participated on the study 72 subjects - parents of children aged from 2 years and 11 months to 7 years and 7 months - divided into two groups: Research Group and Control Group. The proposed modification to parent report transformed the 31 indicators (affirmations) into retrospective self-administered questions to parents, with responses in Likert scale. The IRDI-questionnaire was administered in the two groups. RESULTS: The IRDI-questionnaire showed to be easy and quick to administer (average 15 minutes) and with low cost. The analysis of the questions obtained a good internal consistency value. The comparison between the groups by the parents' answers to the 31 questions showed difference in 16 questions. CONCLUSION: The items of the IRDI-questionnaire concern the main Autism Spectrum Disorders risk signs pointed out in literature. The parents' answers showed differences between the studied groups.

TEMA: co-ocorrência entre problemas alimentares e de linguagem oral. OBJETIVO: investigar as possíveis relações entre problemas de alimentação e de linguagem oral, do ponto de vista bio-psíquico. MÉTODO: clínico-qualitativo, desenvolvido por meio de estudo longitudinal de um caso clínico, de um menino de três anos, com a queixa "não fala e não come" e com diagnóstico de doença do refluxo gastroesofágico. RESULTADOS: o caso analisado configurou-se como emblemático da presença de relações entre problemas de linguagem oral e de alimentação. CONCLUSÃO: os resultados indicam que há relação entre os problemas de alimentação e de linguagem oral. Sendo assim, sugere-se que os fonoaudiólogos que se ocupam dos problemas de linguagem em crianças, investiguem as condutas alimentares. Da mesma forma, sugere-se que o fonoaudiólogo que trabalha com o sistema estomatognático, investigue a linguagem oral de seus pacientes.

BACKGROUND: co-occurrence of feeding and language disorders. AIM: to investigate possible connections between feeding and language disorders, based on a bio-psychic perspective. METHOD: a clinical-qualitative methodology, developed through a longitudinal study of a clinical case of a three year old boy with the complaint "does not speak or eat" and with a medical diagnosis of gastroesophageal reflux disease. RESULTS: the analyzed case was considered emblematic of connections between feeding and language disorders. CONCLUSIONS: results indicate that there is a relationship between oral language and feeding problems. It is suggested for speech-language therapists who work with language problems to investigate feeding habits, as well as for those who for with the stomatognathic system to investigate the oral language of their patients.
