ABSTRACT Introduction: Although uncommon in dentistry, the concept of the life cycle holds great importance for dental professionals in identifying crucial intervention opportunities and determining the optimal timing for treatments and procedures. Objective: To carry out a review of the literature on life cycle classifications and their distinct phases, evaluating their applicability in healthcare. Methods: A literature review was performed, searching for articles in PubMed, SciELO, National Health Library (BvB), and Google Scholar databases, as well as relevant books. The keywords “life cycle,” “life stages,” “human development,” “age groups,” and “biological age” were used. Relevant articles were selected by analyzing their titles and abstracts, and read in full to confirm their inclusion in the research. Results: Nine distinct life cycle classifications were found, each with unique criteria. Conclusion: Based on the comprehensive literature review, a novel classification was proposed (The 10-phase Life Cycle), which encompasses dental, growth, physiological aging, sociocultural, and behavioral characteristics, aiming to enhance communication among healthcare professionals, particularly those engaged in the growth, development, and aging processes of human beings.

RESUMO Introdução: Embora incomum na Odontologia, o conceito de ciclo vital é de grande importância para ajudar os profissionais dessa área a identificar oportunidades cruciais para intervenções e para determinar o momento ideal para tratamentos e procedimentos. Objetivo: Realizar uma revisão da literatura sobre as classificações do ciclo vital e suas distintas fases, avaliando sua aplicabilidade na área da saúde. Métodos: Foi realizada uma revisão da literatura, buscando artigos nas bases de dados PubMed, SciELO, Biblioteca Nacional de Saúde (BvB) e Google Acadêmico; além de livros relevantes. Foram utilizadas as palavras-chave “ciclo vital”, “estágios de vida”, “desenvolvimento humano”, “faixas etárias” e “idade biológica”. Os artigos relevantes foram selecionados por meio da análise de seus títulos e resumos e lidos na íntegra para confirmar sua inclusão na pesquisa. Resultados: Nove classificações distintas de ciclo vital foram encontradas, cada uma com critérios únicos. Conclusão: Com base nessa revisão abrangente da literatura, é proposta uma nova classificação (o ciclo vital de 10 fases) que engloba características dentárias, de crescimento, envelhecimento fisiológico, socioculturais e comportamentais, com o objetivo de melhorar a comunicação entre os profissionais da área de saúde, particularmente aqueles envolvidos nos processos de crescimento, desenvolvimento e envelhecimento do ser humano.

RESUMO Introdução: A apneia obstrutiva do sono (AOS) afeta uma importante parcela da população e caracteriza-se pela obstrução total ou parcial recorrente da via aérea superior (VAS) durante o sono, o que afeta negativamente a qualidade de vida dos pacientes no curto prazo e no longo prazo, e constitui importante problema de saúde pública para a sociedade. A área de atuação do ortodontista está em íntima relação com a VAS, o que o coloca em uma posição estratégica para diagnosticar falhas na passagem de ar e intervir quando necessário. É imperativo que o ortodontista, como profissional da saúde, saiba reconhecer problemas respiratórios e manejá-los de maneira apropriada, quando indicado. Resumo Considerando que a fumaça de cigarro é um problema de saúde pública que está crescendo entre os adolescentes, o objetivo deste estudo foi investigar o impacto da fumaça de cigarro na sinalização osteogênica e osteoclastogênica da sutura palatina mediana de ratos. Foram utilizados ratos Wistar machos expostos (n=30) e não expostos à fumaça de cigarro (n=30). A exposição à fumaça de cigarro foi realizada duas vezes ao dia por 3 minutos, com um intervalo de 12 horas entre as exposições. Os animais foram mortos após o período experimental de 3, 7, 14 e 21 dias. Os tecidos coletados foram analisados em microscópico de luz e pelo RT-PCR em tempo real foi realizado para investigar a expressão gênica. s dados obtidos foram comparados usando o testes de Kruskal Wallis e Dunn (⍺ = 5%). Morfoligicamente, não houve mudança significativa na sutura palatina mediana nos ratos expostos ou não à fumaça de cigarro durante os tempo de 3, 7, 14 e 21 dias (p> 0.05). Por outro lado, a sinalização osteogênica esta aumentada nos animais expostos à fumaça e foi caracterizado por um aumento da produção de RANKL aos 3 e 14 dias (p <0.05), sem mudança na síntese da produção de RANK e osteoprotegerina (p> 0.05). Curiosamente, nos animais expostos, também foi observado um aumento precoce da síntese de osteocalcina, sialoproteína óssea e de osteopontina aos 3 dias de exposição, o que não foi mantido ao longo do tempo. A fumaça de cigarro modula a sinalização osteogênica e osteoclastogênica na sutura palatina mediana de ratos jovens, apesar de não tenha sido evidenciado alterações morfológicas.

Abstract Considering that smoking is a public health problem that has been growing among adolescents, the aim of this study was to investigate the impact of cigarette smoke on osteogenic and osteoclastogenic signaling in middle palatal suture of rats. Male Wistar rats exposed (n = 30) or not to cigarette smoke (n = 30) were used. Exposure to smoke was carried out for two daily periods of 3 minutes each, with an interval of 12 hours between exposures. After the experimental periods of 3, 7, 14 and 21 days, the animals were euthanized. The collected tissues were analyzed using light microscopy and real-time RT-PCR was performed to investigate gene expression. The data obtained were compared using the Kruskal Wallis and Dunn tests (⍺ = 5%). Morphologically, there were no significant changes in the middle palatal suture of rats exposed or not to cigarette smoke during 3, 7, 14 and 21 days (p> 0.05). On the other hand, osteoclastogenic signaling was increased in animals exposed to smoke and was characterized by a higher production of RANKL at 3 and 14 days (p <0.05), with no change in the synthesis of RANK and osteoprotegerin (p> 0.05). Interestingly, in the exposed animals, an early increase in the synthesis of osteocalcin, bone sialoprotein and osteopontin was also identified at 3 days of exposure (p <0.05), not sustained over time (p> 0.05). Cigarette smoke modulates osteogenic and osteoclastogenic signaling in the middle palatal suture of young rats, although morphological changes have not been evidenced.

Resumo Fator de crescimento endothelial (VEGF) e fator de crescimento de fibroblasto (FGF-2) tem a capacidade de aumentar a proliferação e permeabilidade vascular. O objetivo deste estudo foi quantificar a liberação dos dois fatores de crescimento (VEGF e FGF-2) após expansão rápida da maxilla (ERM). Trinta animais foram divididos aleatoriamente em dois grupos. Grupo Controle (5 ratos - sutura intacta) e grupos Experimentais (25 ratos submetidos a ERM) que foram avaliados em períodos diferentes de tratamento. Cinco animais foram eutanaziados em diferentes períodos de avaliação aos 0, 2, 3, 5 e 7 dias após ERM. RT-PCR foi usado para avaliar a expressão gênica dos fatores de crescimento liberados nos diferentes períodos de estudo. Os dados foram submetidos à análise estatística usando ANOVA seguido do pós-teste de Tukey com nível de significância de a=0.05. RT-PCR mostrou que os RNAm de VEGF e FGF-2 estavam expressos na sutura palatina mediana intacta. Os RNAm de VEGF e FGF-2 foram estimulados nos períodos iniciais (24h) após ERM (p<0.001 e p<0.01, respectivamente). Os nívies moleculares de VEGF nunca retornaram aos valores originais, e a expressão de FGF-2 reduziu até o dia 5 (p<0.001) e de repente aumentou até o dia 7, retornando aos níveis originais. ERM aumentou a secreção de VEGF, mas diminuiu a secreção de FGF-2 quando comparado ao tecido intacto. Os resultados mostraram que estes fatores de crescimento são liberados e regulados na sutura palatina mediana após ERM e podem ser de importante contribuição para o entendimento da resposta reparadora geral do tecido da sutura durante o processo de remodelação óssea.

Abstract Vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (FGF-2) have the ability to increase vascular proliferation and permeability. The aim of this study was to quantify the release of two diffusible angiogenic growth factors (VEGF and FGF-2) after rapid maxillary expansion (RME). Thirty animals were randomly assigned to two groups. Control group (5 rats - intact suture) and Experimental groups (25 rats with RME) which were evaluated in different periods of treatment. Five animals were euthanized in different periods of healing at 0, 1, 2, 3, 5 and 7 days after RME. RT-PCR was used to evaluate the gene expression of angiogenic growth factors released on different periods of study. Data were submitted to statistical analysis using ANOVA followed by Tukey test and significance was assumed at a=0.05. RT-PCR showed that mRNAs of VEGF and FGF-2 were expressed in intact palatal suture tissue. mRNAs of VEGF and FGF-2 was upregulated in early periods (24 h) after RME (p<0.001 and p<0.01, respectively). The molecular levels of VEGF never returned to its original baseline values, and FGF-2 expression decreased up to day 5 (p<0.001) and suddenly increased at day 7, returning to its original level. RME increased VEGF secretion, but decreased FGF-2 secretion when compared to intact tissue. The results showed that these angiogenic growth factors are released and regulated in the palatal suture tissue after RME and could make an important contribution to the knowledge of overall reparative response of the suture tissue during the bone remodeling process.

RESUMO Objetivo: o objetivo desse estudo in vitro foi avaliar e comparar os efeitos de desgastes, perfurações, jateamento e envelhecimento de dentes pré-fabricados (Dpf) na resistência ao cisalhamento (RC) de braquetes ortodônticos, bem como os efeitos dos tratamentos de superfície no índice de adesivo remanescente (ARI). Métodos: 192 Dpf foram divididos em quatro grupos (n = 48): Grupo 1, sem tratamento; Grupo 2, desgaste de sulcos com broca diamantada cilíndrica; Grupo 3, duas perfurações com broca diamantada esférica; e Grupo 4, jateamento com partículas de óxido de alumínio de 50µm. Antes do experimento, metade das amostras ficou imersa em água destilada a 37oC durante 90 dias. Os braquetes foram colados com Transbond XT e os testes de cisalhamento foram realizados com uma máquina universal de ensaios. A RC foi comparada, por tratamento de superfície e por envelhecimento, utilizando-se análise de variância e teste de Tukey. Os escores ARI foram comparados entre os tratamentos de superfície usando o teste Kruskal-Wallis e o teste de Dunn. Resultados: os tratamentos de superfície aumentaram a RC dos braquetes (p < 0,01) e o envelhecimento não aumentou a RC (p = 0,45). Os Grupos 2, 3 e 4 mostraram maior RC e maior ARI do que o Grupo 1 (p < 0,05). A RC foi maior nos Grupos 3 e 4 (perfuração, jateamento) do que no Grupo 2 (desgaste) (p < 0,05). RC e ARI apresentaram correlação positiva (R2 = 0,57; p < 0,05). Conclusão: o tratamento de superfície dos Dpf aumentou a RC dos braquetes; porém, o envelhecimento não foi relevante. O jateamento e a perfuração mostraram maior RC do que o desgaste. Existe uma correlação positiva entre RC e ARI.

ABSTRACT Objective: The aim of this in vitro study was to evaluate and compare the effects of grinding, drilling, sandblasting, and ageing prefabricated teeth (PfT) on the shear bond strength (SBS) of orthodontic brackets, as well as the effects of surface treatments on the adhesive remnant index (ARI). Methods: One-hundred-ninety-two PfT were divided into four groups (n = 48): Group 1, no surface treatment was done; Group 2, grinding was performed with a cylindrical diamond bur; Group 3, two drillings were done with a spherical diamond bur; Group 4, sandblasting was performed with 50-µm aluminum oxide. Before the experiment, half of the samples stayed immersed in distilled water at 37oC for 90 days. Brackets were bonded with Transbond XT and shear strength tests were carried out using a universal testing machine. SBS were compared by surface treatment and by ageing with two-way ANOVA, followed by Tukey’s test. ARI scores were compared between surface treatments with Kruskal-Wallis test followed by Dunn’s test. Results: Surface treatments on PfT enhanced SBS of brackets (p< 0.01), result not observed with ageing (p= 0.45). Groups II, III, and IV showed higher SBS and greater ARI than the Group 1 (p< 0.05). SBS was greater in the groups 3 and 4 (drilling, sandblasting) than in the Group 2 (grinding) (p< 0.05). SBS and ARI showed a positive correlation (Spearman’s R2= 0.57; p< 0.05). Conclusion: Surface treatment on PfT enhanced SBS of brackets, however ageing did not show any relevance. Sandblasting and drilling showed greater SBS than grinding. There was a positive correlation between SBS and ARI.

INTRODUÇÃO: tratamentos combinados podem oferecer vantagens em casos de edentulismo parcial. O uso de implantes convencionais como ancoragem ortodôntica permite a movimentação ortodôntica prévia, fornecendo os espaços apropriados para inserção das coroas. OBJETIVO: este relato de caso descreve o tratamento de uma paciente, com 61 anos e 10 meses de idade, que possuía um overjet negativo que tornava impossível uma reabilitação protética ideal, comprometendo a estética dentária e facial. RELATO DO CASO: após um setup diagnóstico, os implantes convencionais foram colocados na mandíbula para ancorar a intrusão e retração dos dentes anteriores. Espaços para os incisivos laterais foram abertos na maxila, usando-se aparelho ortodôntico. CONCLUSÕES: o planejamento integrado entre Ortodontia e Implantodontia propiciou um tratamento bem-sucedido, usando implantes convencionais como ancoragem. A relação oclusal obtida apresenta estabilidade 10 anos pós-tratamento.

INTRODUCTION: Combined treatment offers advantages for partially edentulous patients. Conventional implants, used as orthodontic anchorage, enable previous orthodontic movement, which provides appropriate space gain for crown insertion. OBJECTIVE: This case report describes the treatment of a 61-year and 10-month-old patient with negative overjet which made ideal prosthetic rehabilitation impossible, thereby hindering dental and facial esthetics. CASE REPORT: After a diagnostic setup, conventional implants were placed in the upper arch to anchor intrusion and retract anterior teeth. Space gain for lateral incisors was achieved in the lower arch by means of an orthodontic appliance. CONCLUSIONS: Integrated planning combining Orthodontics and Implantology provided successful treatment by means of conventional implant-supported anchorage. The resulting occlusal relationship proved stable after 10 years.

OBJETIVO: avaliar a alteração sagital ocorrida nos incisivos inferiores com o uso do arco lingual no período de transição da dentição mista para a permanente. MÉTODOS: a amostra foi composta por 44 pacientes leucodermas (26 meninas e 18 meninos), divididos em dois grupos: (GC) grupo controle, no qual foi efetuado monitoramento do espaço da arcada inferior, sem tratamento ortodôntico/ortopédico no período avaliado (n = 14); (GE) grupo experimental, presença de suave apinhamento anteroinferior e instalação do arco lingual para manutenção do espaço (n = 30). A posição dos incisivos inferiores foi analisada em traçados cefalométricos computadorizados realizados ao início (T1) e ao final do acompanhamento, já na dentição permanente (T2). Para avaliar a posição dos incisivos foram utilizadas as medidas das análises cefalométricas de Tweed e de Steiner: IMPA, 1.NB e 1-NB. As alterações ocorridas foram analisadas pelo teste t para amostras pareadas, enquanto as diferenças entre os grupos foram avaliadas pelo teste t para amostras independentes, bem como para o dimorfismo sexual. RESULTADOS: os valores em T2 foram maiores em relação a T1 para todas as medidas no GE (IMPA, p = 0,038; 1.NB, p = 0,007; e 1-NB, p < 0,0001). Na comparação das diferenças (T2-T1) entre o GC e GE pôde-se aferir diferenças significativamente superiores para as medidas 1.NB (p = 0,002) e 1-NB (p < 0,0001) no GE. Não houve diferença significativamente estatística entre os sexos. CONCLUSÃO: concluiu-se que os incisivos inferiores foram projetados após a utilização do arco lingual para o controle do espaço no período de transição da dentição mista para a permanente, porém a projeção esteve dentro dos padrões aceitáveis, não havendo diferença entre sexos.

OBJECTIVE: The objective of this study was to evaluate a sagittal variation on the lower incisors with the use of the lingual arch on the transition from mixed to permanent dentition. METHODS: The sample was constituted of 44 Caucasian patients (26 girls and 18 boys), divided in two groups: CG, control group, monitoring the lower arch space with no orthodontic/orthopedic treatment during the rated period (n = 14); EG, experimental group, presenting anterior inferior mild crowding and installation of the lingual arch for space maintenance (n = 30). The position of the lower incisors was analyzed on computerized cephalometric tracings performed at the beginning of the monitoring (T1) and at the end, on the permanent dentition (T2). In order to evaluate the position of the incisors it was used Tweed and Steiner measurements: IMPA, 1.NB and 1-NB. The alterations were analyzed through the "t" test for paired samples, while the differences between the groups were analyzed through the "t" test for independent samples, as for sexual dimorphism. RESULTS: The values in T2 were greater in relation to T1 for all measurements on EG (IMPA, p = 0.038; 1.NB, p = 0.007 and 1-NB, p < 0.0001). On comparing the differences (T2-T1) between CG and EG, it can be gauged differences significantly superior for measure 1.NB (p = 0.002) and 1-NB (p < 0.0001) on EG. There was no statisticaly significant difference between genres. CONCLUSION: It was concluded that the lower incisors were projected after using the lingual arch to control the space on the transition from mixed to permanent dentition, however, within acceptable standards, not having difference between genres.

OBJETIVO: analisar, in vitro, a degradação de força, ao longo do tempo, de elastômeros das marcas comerciais American Orthodontics, Morelli, Ormco e TP Orthodontics. MÉTODOS: a amostra constituiu-se de 80 segmentos de elastômeros em cadeia fechada na cor cinza, divididos em quatro grupos, conforme o fabricante. A distensão foi padronizada em 21mm, com liberação de força inicial variando de 300 a 370g de força. As amostras foram mantidas em saliva artificial em temperatura constante de 37ºC, e a força avaliada nos seguintes intervalos: inicial, 1h, 3h, 5h, 7h, 9h, 1 dia, 7 dias, 14 dias, 21 dias, 28 dias e 35 dias. RESULTADOS: houve diferença estatisticamente significativa na degradação de força entre os grupos avaliados, sendo que no primeiro dia houve perda de 50 a 55% em relação à força inicial. Os valores médios de força em 35 dias variaram de 122 a 148g. CONCLUSÃO: todas as marcas comerciais apresentaram degradação de força ao longo do tempo, sendo que na primeira hora a perda de força esteve entre 59 e 69% da força inicial. Porém, como existe variação dessa degradação dependendo da marca comercial, estudos como esses são importantes para orientação do uso desses elastômeros.

OBJECTIVE: To analyze the in vitro force degradation of four different brands of elastomeric chains: American Orthodontics, Morelli, Ormco and TP Orthodontics. METHODS: The sample consisted of 80 gray elastomeric chains that were divided into four groups according to their respective manufacturers. Chain stretching was standardized at 21 mm with initial force release ranging from 300 g to 370 g. The samples were kept in artificial saliva at a constant temperature of 37°C and the degradation force was recorded at the following time intervals: initial, 1, 3, 5, 7 and 9 hours, and 1, 7, 14, 21, 28, and 35 days. RESULTS: There was a statistically significant difference between the groups regarding the force degradation, mainly within the first day, as a force loss of 50-55% was observed during that time in relation to the initial force. The force delivered at 35 days ranged from 122 g to 148 g. CONCLUSION: All groups showed force degradation over time, regardless of their trademarks, a force loss of 59-69% was observed in the first hour compared to baseline. However, because the variation in force degradation depends on the trademark, studies such as the present one are important for guiding the clinical use of these materials.

OBJECTIVE: The establishment of normal occlusal relationships in patients with cleft lip and palate using rapid maxillary expansion may promote good conditions for future rehabilitation. OBJECTIVE: This study describes the clinical case of monozygotic twins with unilateral cleft lip and palate at the age of mixed dentition, who were treated using the same rapid maxillary expansion protocol, but with two different screws (conventional and fan-type expansion screw). Results were evaluated using plaster models, intraoral and extraoral photographs, and Cone-Beam computed tomography (CBCT) scans obtained before the beginning of the treatment, (T1). METHODS: The patients were followed up for 6 months after maxillary expansion, when the same tests requested at T1 were obtained again for review (T2). T1 and T2 results were compared using lateral cephalometric tracings and measurements of the intercanine and intermolar distances in the plaster models using a digital caliper. RESULTS: The two types of expansion screws corrected the transverse discrepancy in patients with cleft lip and palate. The shape of the upper arches improved at 10 days after activation. CONCLUSION: CBCT scans provide detailed information about craniofacial, maxillary and mandibular changes resulting from rapid maxillary expansion. The most adequate screw for each type of malocclusion should be chosen after detailed examination of the dental arches.

INTRODUÇÃO: nos pacientes com fissura labiopalatina, observa-se, usualmente, uma severa atresia da maxila, especialmente na região anterior da arcada, cujo tratamento envolve expansão maxilar. OBJETIVO: o propósito desse estudo foi avaliar o padrão de expansão de três marcas de parafusos com limitador posterior na correção da deficiência transversa. MÉTODOS: foram realizadas 18 expansões em Typodont, com 6 simulações para cada grupo: G1 - parafuso Dentaurum®, G2 - parafuso Leone®, e G3 - parafuso Morelli®. Foram realizadas, para cada ensaio, 13 ativações de 2/4 de volta, totalizando 5,2mm de abertura do parafuso. Definiram-se os momentos das medições em inicial (T1), metade das ativações (T2) e final (T3). Com o auxílio de um paquímetro, foram medidas as seguintes distâncias: interprimeiros pré-molares (IP1), interssegundos pré-molares (IP2), intermolares (IM) e comprimento da arcada (CA). Os dados obtidos foram submetidos aos testes de Kolmogorov-Smirnov, de Tukey, de Friedman e ANOVA. RESULTADOS: observou-se que todos os grupos apresentaram um padrão de abertura em forma de "V" com maior expansão na região anterior da arcada, sendo mais expressivo no grupo G3 (29,58% em IP1 e 9,73% em IM). O aumento na medida CA foi semelhante para os grupos G1 e G3 (+12,65% e +12,13%, respectivamente), com menor valor para o G2 (+8,23%). CONCLUSÕES: concluiu-se que todos os parafusos com limitador posterior utilizados nesse estudo podem ser empregados no tratamento da deficiência transversa da arcada. Entretanto, o parafuso Morelli® apresentou maior abertura na região anterior em relação à posterior, característica importante no tratamento de pacientes com fissura labiopalatina. Recomenda-se a realização de estudos clínicos para confirmação desses achados.

INTRODUCTION: Severe constriction of the maxillary arch, especially in the anterior part of the maxilla, is a common finding in cleft lip and palate patients. The usual treatment generally involves the expansion of the arch. OBJECTIVE: The aim of this study was to evaluate the expansion pattern obtained with three different fan-type expander screws. METHODS: Using Typodont, 18 expansions were executed, six for each group, according to the expander tested: G1- DentaurumTM; G2- LeoneTM and G3- MorelliTM. For each essay, 13 activations of 2/4 of a turn were done in each appliance, leading to a total activation of 5.2mm. Using a caliper, measurements were taken in the beginning of the testing (T1); in half the total turnings (T2) and in the end of the testing (T3). Different measures were considered: interpremolar width (IP1), intersecond premolar width (IP2), intermolar width (IM) and also the arch length (CA). The data obtained was analyzed through Kolmogorov-Smirnov, Tukey and Freeman tests as well as by ANOVA. RESULTS: It could be observed that all tested groups showed a "V" expansion pattern, with major expansion in the anterior region of the arch, that was even more expressive in G3 (IP1=29.85% and IM=9.73%). Increases in the CA measurements were similar for groups G1 and G3 (+12.65% and +12.13%, respectively), and lower for G2 (+8.23%). CONCLUSIONS: It could be concluded that all the fan-type expander screws tested in the present study can be used for treating transverse constriction of the maxilla. However, Morelli expander showed a greater expansion in the anterior region of the arch in relation to the posterior region, which is an important characteristic when treating cleft lip and palate patients. Clinical studies are also necessary to confirm the findings of the present study.

OBJETIVO: avaliar a contaminação bacteriana presente na ponta ativa de alicates ortodônticos utilizados no meio acadêmico. MÉTODOS: selecionou-se 34 alicates, 17 do modelo 347 e 17 do modelo 139. O grupo controle foi composto por 3 alicates de cada modelo, previamente autoclavados. Os alicates do grupo experimental, após o uso, foram imersos em 10ml do meio de cultura líquido Brain-Heart Infusion (BHI) por 2 minutos, seguido de cultivo a 37°C (24 a 48h) e semeadura (duplicata) em diferentes meios de cultura sólidos à base de ágar, para detecção e identificação de agentes microbianos. RESULTADOS: as análises microbiológicas revelaram contaminação em ambos os tipos de alicates ortodônticos. Observaram-se variadas formas bacterianas, predominando estafilococos e cocos isolados gram-positivos. Os alicates removedores de anéis apresentaram maiores índices de contaminação, com médias entre 2,83 x 10(9) e 6,25 x 10(9) UFC/ml, variando de acordo com o tipo de meio de cultura. Os alicates 139 também continham bactérias da microbiota bucal, com variações entre 1,33 x 10(8) e 6,93 x 10(9) UFC/ml, sendo que a maior média obtida refere-se ao meio de cultura próprio para desenvolvimento de estafilococos, indicando, em certos casos, presença da espécie Staphylococcus aureus, microrganismos que não fazem parte da microbiota bucal normal, sendo comumente encontrados na cavidade nasal e superfícies epiteliais das mãos. CONCLUSÃO: constatou-se que os alicates ortodônticos apresentam-se contaminados, como qualquer outro instrumental odontológico, após serem empregados em atendimentos clínicos. Em razão disso, há necessidade de submetê-los previamente aos procedimentos de esterilização após cada utilização em pacientes.

OBJECTIVE: To evaluate bacterial contamination in the active tip of orthodontic pliers used in an academic setting. METHODS: Thirty-four pliers were selected: 17 debonding pliers and 17 model 139 pliers. The control group was composed of 3 previously autoclaved pliers of each model. After use, the pliers in the experimental group were immersed in 10 ml of brain-heart infusion (BHI) culture medium for 2 minutes, incubated at 37º C for 24 to 48 h and seeded in duplicates in different agar-based solid culture media to detect and identify microbial agents. RESULTS: Microbiological analyses revealed that there was contamination in both types of orthodontic pliers. Several bacteria were detected, predominantly staphylococcus and isolated Gram-positive (G+) cocci. The debonding pliers had a greater contamination rate and mean values of 2.83 x 10(9) and 6.25 x 10(9) CFU/ml, with variations according to the type of culture medium. The 139 pliers also had all types of bacteria from the oral microbiota at values that ranged from 1.33 x 10(8) to 6.93 x 10(9) CFU/ml. The highest mean value was found in the medium to grow staphylococci, which confirmed, in certain cases, the presence of Staphylococcus aureus, which are not part of the normal oral microbiota but are usually found in the nasal cavity and on the skin of the hands. CONCLUSION: Orthodontic pliers were contaminated as any other dental instrument after use in clinical situations. Therefore, they should undergo sterilization after each use in patients.

OBJETIVO: Este estudo retrospectivo avaliou pacientes tratados com exodontias de pré-molares através da análise de telerradiografias, comparando o efeito de diferentes protocolos de exodontias no perfil. METODOLOGIA: 87 pacientes foram selecionados da clínica particular de três ortodontistas diplomados pelo Board Brasileiro de Ortodontia e Ortopedia Facial. Estes pacientes foram tratados com aparelho fixo Edgewise e foram divididos em três grupos, de acordo com a sequência de pré-molares extraídos: "Grupo 40", primeiros pré-molares superiores (22 pacientes); "Grupo 44", quatro primeiros pré-molares (43 pacientes); e "Grupo 45", primeiros pré-molares superiores e segundos inferiores (22 pacientes). O Ângulo Z e a Linha de Burstone foram utilizados para quantificar e comparar os perfis antes e após o tratamento. RESULTADOS: O teste t-student mostrou melhora no perfil em todos os grupos, visto que o valor do ângulo Z apresentou-se maior ao final do tratamento e as medidas dos lábios vieram ao encontro dos valores preconizados por Burstone. A análise das mudanças proporcionadas pelo tratamento pela ANOVA não revelou diferenças significativas entre os grupos CONCLUSÃO: Os protocolos produzem aparências faciais semelhantes ao final do tratamento.

PURPOSE: This retrospective study assessed patients treated with extraction of premolars by analysis of lateral cephalograms, comparing the effect of different tooth extraction protocols on the profile. METHODS: Eighty-seven patients were selected from the private practices of three orthodontists certified by the Brazilian Board of Orthodontics and Facial Orthopedics. These patients were treated with fixed edgewise appliances and divided into three groups according to the sequence in which premolars were extracted: "Group 40", maxillary first premolars (22 patients); "Group 44", both maxillary and mandibular first premolars (43 patients); and "Group 45", maxillary first and mandibular second premolars (22 patients). The Z angle and the Burstone line were used to quantify and compare the profiles before and after treatment. RESULTS: Student's t-test showed profile improvement in all groups, as the Z angle value was higher at treatment completion and the lip measurements were in agreement with the values recommended by Burstone. Analysis of treatment changes by the ANOVA did not reveal significant differences between groups. CONCLUSION: The protocols produced similar facial appearance at treatment completion.

OBJETIVO: As fissuras de lábio e palato representam as mais comuns das malformações congênitas que envolvem a face e a cavidade bucal. A conduta a ser tomada frente às fissuras de lábio e palato depende da severidade e complexidade das mesmas, através de um tratamento, por meio de uma equipe multidisciplinar, que vise à reabilitação. O propósito deste trabalho é fornecer orientação aos profissionais da área, especialmente, ao cirurgião-dentista, para realização do tratamento em pacientes com estes defeitos congênitos e em situações de complicações pós-operatórias. DESCRIÇÃO DO CASO: O caso clínico aqui relatado ilustra o tratamento de uma paciente portadora de fissura transforame incisivo bilateral completa de lábio e palato, que sofreu trauma facial no período de reabilitação, após a cirurgia ortognática. O sucesso clínico e radiográfico pôde ser observado dois anos após o trauma por queda. CONCLUSÃO: Pacientes que realizaram cirurgia ortognática estão sujeitos a complicações pós-operatórias; infecção, acidentes com veículo motor, armas de fogo, violência doméstica estão entre as causas mais comuns citadas de complicações no pós-operatório tardio. O conhecimento dessas intercorrências possibilita que medidas preventivas sejam adotadas para evitarem-se complicações graves principalmente em pacientes com fissuras que necessitam da abordagem de uma equipe interdisciplinar composta por profissionais de saúde.

PURPOSE: Cleft lip and palate are the most common congenital deformities affecting the face and oral cavity. The procedure to be adopted when facing cleft lip and palate depends on their complexity, comprising treatment by an interdisciplinary team aiming at rehabilitation. Esthetic, functional and emotional implications occur in different manners and intensities, depending on the type of cleft. The aim of this paper is to guide professionals in this field, especially oral surgeons, for the treatment of patients with these congenital deformities and cases of postoperative complications. CASE DESCRIPTION: The present case illustrates the treatment of a patient with complete bilateral cleft lip and palate, who suffered facial trauma in the postoperative period after orthognathic surgery. The clinical and radiographic success could be observed two years after the trauma by fall. CONCLUSION: Patients submitted to orthognathic surgery are subject to postoperative complications. Infection, vehicle accidents, firearm accidents and domestic violence are among the most common causes of complications in the postoperative period. Knowledge on these problems allows the adoption of preventive measures to avoid serious complications mainly in patients with clefts who require an interdisciplinary team approach comprising multiple health care professionals.

OBJETIVO: analisar, por meio de microscopia eletrônica de varredura, as alterações químicas e estruturais de braquetes metálicos submetidos a um processo de biodegradação in vitro. MÉTODOS: a amostra foi dividida em dois grupos, de acordo com a marca comercial dos acessórios - Grupo A = Dyna-Lock, 3M/Unitek (AISI 303) e Grupo B = LG Edgewise Standard, American Orthodontics (AISI 316L). Os corpos de prova, aparelhos ortodônticos simulados, permaneceram imersos em solução salina (0,05%) por um período de 60 dias, a 37ºC, sob agitação. As alterações decorrentes da exposição dos acessórios à solução salina foram investigadas através da observação com microscópio eletrônico de varredura (MEV) e análise da composição química (EDX), realizadas antes e após o período de imersão (T0 e T5, respectivamente). RESULTADOS: em T5, houve formação de produtos de corrosão sobre a superfície dos braquetes, especialmente no Grupo A, além disso, houve alterações na composição da liga metálica dos braquetes de ambos os grupos, sendo que, no Grupo A, houve redução dos íons ferro e cromo e, no Grupo B, redução de íons cromo. CONCLUSÃO: os acessórios do Grupo A apresentaram-se menos resistentes à biodegradação in vitro, o que poderia estar associado ao tipo de aço utilizado em sua fabricação (AISI 303).

OBJECTIVES: The purpose of this study was to analyze, with the aid of scanning electron microscopy (SEM), the chemical and structural changes in metal brackets subjected to an in vitro biodegradation process. METHODS: The sample was divided into three groups according to brackets commercial brand names, i.e., Group A = Dyna-Lock, 3M/Unitek (AISI 303) and Group B = LG standard edgewise, American Orthodontics (AISI 316L). The specimens were simulated orthodontic appliances, which remained immersed in saline solution (0.05%) for a period of 60 days at 37°C under agitation. The changes resulting from exposure of the brackets to the saline solution were investigated by microscopic observation (SEM) and chemical composition analysis (EDX), performed before and after the immersion period (T0 and T5, respectively). RESULTS: The results showed, at T5, the formation of products of corrosion on the surface of the brackets, especially in Group A. In addition, there were changes in the composition of the bracket alloy in both groups, whereas in group A there was a reduction in iron and chromium ions, and in Group B a reduction in chromium ions. CONCLUSIONS: The brackets in Group A were less resistant to in vitro biodegradation, which might be associated with the type of steel used by the manufacturer (AISI 303).
