ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Abstract In Brazil, malaria is an important public health problem first reported in 1560. Historically, fluctuations in malaria cases in Brazil are attributed to waves of economic development; construction of railroads, highways, and hydroelectric dams; and population displacement and land occupation policies. Vector control measures have been widely used with an important role in reducing malaria cases. In this review article, we reviewed the vector control measures established in the Brazilian territory and aspects associated with such measures for malaria. Although some vector control measures are routinely used in Brazil, many entomological and effectiveness information still need better evidence in endemic areas where Plasmodium vivax predominates. Herein, we outlined some of the needs and priorities for future research: a) update of the cartography of malaria vectors in Brazil, adding molecular techniques for the correct identification of species and complexes of species; b) evaluation of vector competence of anophelines in Brazil; c) strengthening of local entomology teams to perform vector control measures and interpret results; d) evaluation of vector control measures, especially use of insecticide-treated nets and long-lasting insecticidal nets, estimating their effectiveness, cost-benefit, and population acceptance; e) establishment of colonies of malaria vectors in Brazil, i.e., Anopheles darlingi, to understand parasite-vector interactions better; f) study of new vector control strategies with impacts on non-endophilic vectors; g) estimation of the impact of insecticide resistance in different geographical areas; and h) identification of the relative contribution of natural and artificial breeding sites in different epidemiological contexts for transmission.

Plant architecture influences gall abundance in a tropical montane plant species

ABSTRACT Sessile and host-specific herbivores, such as gall-inducing insects, are usually patchily distributed within the populations of their host plants, and it has been suggested that both inter- and intra-plant variation affect gall abundance, distribution and survivorship. Variation in plant traits, such as size and architecture, has been previously demonstrated as a determinant of gall distribution. We examined the influence of architectural complexity of the tropical plant Galianthe brasiliensis (Rubiaceae) on the abundance of the stem-galler Lopesia sp. (Diptera, Cecidomyiidae) along an elevation gradient where both insect and plant were distributed. A total of 120 plants were sampled along the gradient and more than 580 galls were counted, while plant architectural complexity was determined by a combination of height and branch ramification. Increased elevation did not influence plant complexity nor gall abundance (both P>0.05), but plant architectural complexity explained more than 60% of the variation in gall abundance along the gradient (R2=0.62, P<0.001). We suggest that the greater availability of meristematic tissues in more architecturally complex plants explains the results found, as this is a key resource for gall occurrence and establishment.

RESUMEN El umbuzeiro (Spondias tuberosa Arr. Cam.) es un árbol frutal originario del bioma Caatinga con gran aceptación en el mercado para consumo in natura o productos industrializados. Debido a esto, su cultivo ha adquirido gran importancia para la región del Nordeste brasileño. El objetivo fue evaluar el crecimiento, temperatura del sustrato e índices de clorofila en plántulas de umbuzeiro en niveles de compuestos orgánicos en el sustrato y aplicación foliar de fertilizante foliar orgánico (EcoLife®). El delineamiento experimental utilizado fue en bloques al azar, con arreglo factorial de 5x2, con cuatro repeticiones y tres plantas por unidad experimental. Los factores corresponden a las proporciones de compuesto orgánico a los sustratos (0, 25, 50, 75 y 100%) en el sustrato con y sin aplicación foliar de fertilizante foliar orgánico a 1%. El crecimiento, área foliar e índices de clorofila de plántulas de umbuzeiro fue mayor en los tratamientos con compuesto orgánico y aplicación foliar de fertilizante orgánico.
ABSTRACT The umbuzeiro (Spondias tuberosa Arr. Cam.) is a fruit tree originating from the Caatinga biome that enjoys high acceptance in the market for in natura consumption or industrialized products; as a result, its cultivation has become of great importance for the region of Northeast Brazil. The objective was to evaluate growth, substrate temperature and chlorophyll indices in umbuzeiro seedlings at organic substrate levels and with organic foliar fertilizer (EcoLife®) applications. The experimental design was a randomized block with a factorial arrangement of 5x2, with four replicates and three plants per experiment unit. The factors corresponded to the proportions of the organic compost to the substrates (0, 25, 50, 75 and 100%), with and without foliar applications of 1% organic foliar fertilizer. The growth, leaf area and chlorophyll indices of the umbuzeiro seedlings were higher in the treatments with organic compost and foliar applications of organic foliar fertilizer.
