ABSTRACT Background: This study aimed to describe the kinetics of Leishmania parasite load determined using kinetoplast DNA (kDNA)-based quantitative polymerase chain reaction (qPCR) in visceral leishmaniasis (VL) patients. Methods: Parasite load in blood was assessed by qPCR at five time points, up to 12 months post-diagnosis. Sixteen patients were followed up. Results: A significant reduction in the parasite load was observed after treatment (P < 0.0001). One patient had an increased parasite load 3 months post-treatment and relapsed clinically at month six. Conclusions: We have described the use of kDNA-based qPCR in the post-treatment follow-up of VL cases.

Results: The mean incidence of VL in the state was 6.03/100,000 inhabitants, with an increasing trend in the 40-59 age group [annual percentage variation (VPA = 3.88 [95%CI 0.49;7.40]) and in the regions located in the south of the state: Tabuleiros do Alto Parnaíba (VPA =14.19 [95%CI 3.91;25.50]) and Chapada das Mangabeiras (VPA = 12.15 [95%CI 6.69;24.96]). The average lethality was 6.02%, remaining stable. The average rate of evolution to cure was 52.58%, with a decreasing trend (VPA = -5.67 [95%CI -8.05;-3.23]). Conclusion: There was a trend towards an increase in the incidence and a reduction in the cure rate of VL.

Abstract Aim: This study aimed to verify the data reliability of muscle architecture (MA) variables, and the relationship between MA and the isometric peak torque (PT) of the monoarticular and biarticular knee extensor (KE) muscle in physically inactive women. Methods: Ten physically inactive women (24.0 ± 1.64 years; 162.9 ± 5.34 cm; 63.5 ± 11.90 kg) participated in the study. An ultrasound device assessed the MA variables (muscle thickness, fascicle length, and pennation angle) of the Vastus Lateralis (VL) and Rectus Femoris (RF), and an isokinetic dynamometer assessed the PT. Pearson correlation evaluated the relationship between PT and MA variables, with a significance level of 5%. Additionally, the intraclass correlation coefficient, coefficient of variation, and standard error of measurement. Results: Excellent reliability between images was observed, and no significant relationships were observed between the PT and MA variables of the VL and RF. Conclusion: Isolated variables of the MA of a monoarticular or a biarticular muscle do not influence the production of the isometric PT of the KE.

Resumo As leishmanioses são doenças causadas por protozoários do gênero Leishmania. A doença apresenta duas manifestações clínicas: leishmaniose visceral (LV) e cutânea (LC). No Brasil, a LV está em expansão. A participação do cão como reservatório é importante para a epidemiologia da doença, pois os casos caninos geralmente precedem os humanos. O presente estudo avaliou a ocorrência de LV em cães (LVC) do município de Ji-Paraná por meio de ensaios de PCR, utilizando-se amostras de sangue. O DNA de Leishmania foi detectado em dois dos 105 cães estudados. Os produtos da PCR foram sequenciados e confirmaram que duas amostras (1,90%) eram Leishmania infantum. Os cães tinham histórico alóctone. Os resultados positivos encontrados servem de alerta aos órgãos públicos de saúde. Isso porque Ji-Paraná é o terceiro município a registrar casos de LVC no estado de Rondônia. Dessa forma, reforça-se a importância da ampliação dos estudos sobre a epidemiologia e vigilância da LV na região.

Abstract Leishmaniases are zoonotic diseases caused by protozoa of the genus Leishmania. The disease has two clinical manifestations described in humans: visceral (VL) and cutaneous (CL) leishmaniasis. In Brazil, there has been an expansion of human VL. The participation of the dog as a reservoir of Leishmania infantum, the agent of VL, is important for the epidemiology of the disease since canine cases generally precede human cases. The present study aimed to evaluate the occurrence of Leishmania spp. infection in dogs in the municipality of Ji-Paraná by PCR assays using blood samples. Leishmania DNA was detected in two of the 105 studied dogs. The PCR products were sequenced and confirmed that the two samples (1.90%) correspond to L. infantum. The dogs had allochthonous history. Therefore, the positive results found here should serve as a warning to public health agencies. This is because Ji-Paraná is the third municipality to register cases of canine leishmaniasis (CanL) in Rondônia state. Thus, reinforcing the importance of expanding studies on the epidemiology and surveillance of VL in the region.

Resumo Os cães são os principais reservatórios urbanos da Leishmania infantum, agente causador da leishmaniose visceral (VL), transmitida por vetores conhecidos como flebotomíneos. No Paraná, a primeira detecção de casos positivos caninos ocorreu em 2014, ano em que o Paraná perdeu o status de estado indene. O objetivo deste estudo foi determinar a prevalência da leishmaniose visceral canina no município de Palotina, a ocorrência de vetores que possam transmitir Leishmania infantum e o número de notificação de casos de leishmaniose visceral humana, no período de 2010 a 2020. Para determinar a ocorrência da leishmaniose visceral canina, amostras de sangue de 204 cães foram analisadas, utilizando-se o teste rápido (DPP®) para detectar anticorpos anti-L. infantum. Com o objetivo de investigar a ocorrência de potenciais vetores, coletas foram realizadas mensalmente em 18 pontos na área urbana do município. O número de casos de leishmaniose visceral humana foi investigado a partir de registros da Vigilância Epidemiológica. Nenhum cão testado foi positivo no teste sorológico. Apenas dois espécimes de Lutzomyia neivai, uma de Lutzomyia sp. e quatro de Brumptomyia brumpti foram coletados. Nenhum caso de leishmaniose visceral humana foi notificado. Esses resultados sugerem que não há evidência da circulação de L. infantum em Palotina.

Abstract Dogs are the main urban reservoir of Leishmania infantum, the causative agent of visceral leishmaniasis (VL), which is transmitted by sand flies. In the state of Paraná, the first detection of a positive dog for VL was in 2014, this year Paraná lost free status for this disease (VL). The objectives of this study were to determine the prevalence of canine visceral leishmaniasis in Palotina, the occurrence of vectors that may transmit Leishmania infantum, and the number of notifications of human visceral leishmaniasis cases from period 2010 to 2020. To determine the occurrence of canine visceral leishmaniasis, blood samples from 204 dogs were analyzed using the rapid test DPP® to detect anti-L. infantum antibodies. To investigate the occurrence of potential vectors, monthly collections were made at 18 points within the urban area of the municipality. The number of human visceral leishmaniasis cases was investigated from Epidemiological Surveillance records. None of the serologically tested dogs showed positive titration. Only two specimens of Lutzomyia neivai, one of Lutzomyia sp. and four of Brumptomyia brumpti specimens were collected. No human visceral leishmaniasis cases were reported. These results suggest that there is no evidence of circulation of L. infantum in Palotina.

ABSTRACT: Primary stability is an important requirement for the survival and success of implants during osseointegration. In recent years, immediate post-extraction implants have proven to be a successful and predictable treatment option for the replacement of teeth with a poor prognosis, but the question arises as to whether these implants reach primary stability values comparable to those placed in mature bone. The objective of the study was to compare the primary stability of immediate implants placed in post-extraction alveoli with that of implants placed in mature bone. A retrospective clinical study was carried out, with data collected on 175 implants, placed in 175 patients. All implants placed belonged to the Essential Cone model (Klockner Implant System) and were divided into two groups: immediate implants (Group A, n = 31) and implants placed in mature bone (Group B, n = 144). The primary stability of all implants was measured by insertion torque and resonance frequency analysis with Osstell ISQ. No statistically significant differences were found regarding the stability measured through the insertion torque (26.29 + 10.07 Vs 25.76 + 9.72 N / cm) but significant differences were found in the measurement of primary stability by means of AFR, the values corresponding to implants placed in the post-exodontic alveoli being lower (60.74 ± 6.17 in the VL direction and 62.19 ± 7.64 in the MD direction versus 68.34 ± 6.26 in the VL direction and 69.29 ± 7.98 in the MD direction obtained in implants placed in mature bone). The insertion torque of immediate implants is similar to that of implants placed in mature bone, but their ISQ values are significantly lower, which demonstrates a higher degree of micromotion, and therefore, a greater risk of failure during the period of osseointegration.

RESUMEN: La estabilidad primaria es un requisito importante para la supervivencia y éxito de los implantes durante la osteointegración. En los últimos años, los implantes inmediatos postextracción han demostrado ser una opción de tratamiento exitosa y predecible para la reposición de dientes con mal pronóstico, pero surge la duda de si dichos implantes alcanzan valores de estabilidad primaria comparables a aquellos colocados en hueso maduro. Comparar la estabilidad primaria de implantes inmediatos colocados en alveolos postextracción con la de implantes colocados en hueso maduro. Se llevó a cabo un estudio clínico retrospectivo, con los datos recogidos sobre 175 implantes, colocados en 175 pacientes. Todos los implantes colocados pertenecían al modelo Essential Cone (Klockner Implant System) y se dividieron en dos grupos: implantes inmediatos (Grupo A, n=31) e implantes colocados en hueso maduro (Grupo B, n=144). La estabilidad primaria de todos los implantes se midió mediante torque de inserción y análisis de frecuencia de resonancia con Osstell ISQ. No se encontraron diferencias estadísticamente significativas respecto a la estabilidad medida a través del torque de inserción (26,29+10,07 Vs 25,76+9,72 N/cm) pero sí que se encontraron diferencias significativas en la medida de la estabilidad primaria mediante AFR, siendo inferiores los valores correspondientes a los implantes colocados en los alveolos post exodoncia (60,74 ± 6,17 en sentido VL y 62,19 ± 7.64 en sentido MD frente a 68,34 ± 6.26 en sentido VL y 69,29 ± 7.98 en sentido MD obtenidos en los implantes colocados en hueso maduro). El torque de inserción de los implantes inmediatos es similar al de los implantes colocados en hueso maduro, pero sus valores ISQ son significativamente inferiores, lo que demuestra un mayor grado de micromovimiento, y por consiguiente, un mayor riesgo de fracaso durante el período de osteointegración.

RESUMO: A Leishmaniose visceral humana (LV) está em franca expansão e distribuída nas cinco regiões geográficas do Brasil, o Nordeste sendo a região em que há o maior número de casos registrados. O principal reservatório urbano do agente etiológico da LV é o cão doméstico e sabe-se que casos caninos antecedem o aparecimento de casos humanos. Uma das ações de controle da LV, preconizada pelo Ministério da Saúde, é a eutanásia dos cães soro reagentes. Em 2013, no município de Iguatama/MG, foi realizado o primeiro inquérito sorológico canino na cidade, sendo constatada uma prevalência de 8,3% de cães soro reagentes para Leishmania infantum. Dos animais com sorologia positiva para Leishmaniose visceral canina (LVC), neste inquérito, 84% foram eutanasiados e 16% foram a óbito em, no máximo, dois anos após o diagnóstico. Nenhuma outra medida de controle para LVC foi realizada no município. Os objetivos deste estudo foram determinar a prevalência atual de cães soro reagentes para LVC no município de Iguatama e observar o impacto das eutanásias de cães soro reagentes para LV como única medida de controle realizada neste município, considerado área enzoótica para a doença. Para isso, foi realizado um novo inquérito epidemiológico canino no município de Iguatama, seguindo as normas do Manual de Vigilância e Controle da Leishmaniose Visceral. Foram colhidas amostras sanguíneas de 270 cães que foram examinadas pelo teste imunocromatográfico DPP® e pelo ensaio imunoenzimático (ELISA). Os animais que apresentaram resultados inconclusivos tiveram novas amostras de sangue coletadas e analisadas. O único animal que apresentou resultado positivo no DPP® e indeterminado no ELISA, na primeira avaliação, soro converteu quatro meses depois, o que reforça a recomendação do Ministério da Saúde de reavaliar os cães que apresentam resultado inconclusivo para LVC. Do total das 270 amostras, 21 foram reagentes nos dois testes. Portanto, a prevalência de cães soro reagentes para LVC, em 2017, na área urbana do município de Iguatama foi igual a 7,8%. A infecção pela espécie Leishmania infantum foi confirmada através da técnica molecular de PCR-RFLP em 10 cães amostrados com sorologia positiva para LVC. A comparação entre a prevalência atual (depois das eutanásias) com a encontrada em 2013 (8,3%), não mostrou diferença significativa (p=0,764). Uma vez que a soro prevalência de LVC na área urbana do município de Iguatama ainda é alta, pode-se supor que a eutanásia dos cães soro reagentes para LVC, somada à ausência de monitoramento e de outras medidas de intervenção, não contribuiu para a diminuição da transmissão do parasito, e não foi suficiente para o controle de LVC no município de Iguatama, uma área considerada, até 2013, indene para LVC.

ABSTRACT: Human Visceral Leishmaniasis (VL) is expanding, and it is distributed in the five geographic regions of Brazil, with the Northeast being the region that presents the most significant number of registered cases. The main urban reservoir of the etiological agent of VL is the domestic dog, and it is known that canine cases precede human cases. One of the control actions of VL, recommended by the Ministry of Health, is performing the euthanasia of seropositive dogs. In 2013, in the municipality of Iguatama/MG, the first canine serological survey was carried out in the city, with a prevalence of 8.3% of seropositive dogs for Leishmania infantum. Of the seropositive animals for Canine Visceral Leishmaniasis (CVL) in this survey, 84% were euthanized, and 16% died within two years after diagnosis. No other control measures for CVL were taken in the municipality. The objectives of the present study were to determine the current prevalence of seropositive dogs for CVL in the municipality of Iguatama and to observe the impact of euthanasia of seropositive dogs to VL as the only control measure performed in this municipality, which is considered an enzootic area of the disease. For this, a new canine epidemiological survey was carried out in the municipality of Iguatama, following the guidelines of the Manual of Surveillance and Control of Visceral Leishmaniasis. Blood samples were collected from 270 dogs and tested by DPP® immunochromatographic test and by Enzyme-Linked Immunosorbent Assay (ELISA). The animals that presented inconclusive results had new blood samples collected and analyzed. The only animal that showed a positive result in DPP® and undetermined in the ELISA, in the first evaluation, became seropositive four months later, so this study reinforces the recommendation of the Ministry of Health to reassess dogs that present inconclusive results for CVL. From a total of 270 samples, 21 (7.8%) were reagents in both tests. Thus, the prevalence of seropositive dogs for CVL, in 2017, in the urban area of the municipality of Iguatama was 7.8%. The Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) molecular technique confirmed infection by L. infantum in the ten dogs sampled with CVL-positive serology. The comparison between the current prevalence (after euthanasia) with that found in 2013 (8.3%), there was no significant difference (p=0.764). The prevalence of CVL in the urban area of the municipality of Iguatama is still high, making it possible to presume that the euthanasia of seropositive dogs for CVL, and the absence of monitoring or other interventions measures, did not contribute to decrease the transmission of the parasite and was not sufficient for the control of CVL in the municipality of Iguatama, an area considered as non-endemic to CVL until 2013.

Abstract INTRODUCTION: Canine visceral leishmaniasis (CVL) is an endemic disease in Brazil, and integrated control actions have been adopted by the Brazilian Ministry of Health to control its spread. However, the transmission profile is unknown in areas with recent CVL cases, including Itaúna, located in the Brazilian state of Minas Gerais, where the present study was carried out. METHODS: A total of 2,302 dogs from 12 neighborhoods were serologically tested for canine VL using the current diagnostic protocol adopted by the Brazilian Ministry of Health. Test positivity rate (TPR) and CVL prevalence were determined for each neighborhood. The presence of Leishmania was assessed in 60 seropositive dogs which had been recommended for euthanasia. Twenty-two of them (37%) were asymptomatic, and 38 (63%) were symptomatic for CVL. Parasitological (myeloculture and smear/imprint) and molecular (PCR) methods were employed for Leishmania detection in bone marrow, spleen, mesenteric lymph nodes, and ear skin. The infecting Leishmania species was identified by DNA sequencing. RESULTS: CVL prevalence (per 1,000 dogs) varied from 0.0-166.67, depending on the neighborhood, with a mean of 68.96 (SD 51.38). Leishmania DNA was detected in at least one tissue from all seropositive dogs, with comparable TPR among tissues. Leishmania parasites were identified in most (54/60) seropositive dogs, and the infecting parasite was identified as Leishmania infantum in all of these. CONCLUSIONS: Prevalence of CVL is a contributor to the spread of visceral leishmaniasis in Itaúna.

Resumo Este estudo investigou o efeito de 10 semanas de treinamento de força na resposta eletromiográfica (EMG) do vasto medial, vasto lateral e reto femoral. 23 voluntárias executaram 3 a 5 séries de 6 repetições, intensidade de 50% de 1 repetição máxima (RM), 3 minutos de pausa entre as séries e duração da repetição de 6 s no exercício extensor de joelhos. Um grupo (5:1) realizou a duração da ação concêntrica em 5 s e excêntrica em 1s e outro grupo (3:3) realizou a concêntrica em 3s e a excêntrica em 3s. A atividades EMG (RMS) destas três porções do quadríceps foram registradas em cada repetição das 3 séries da primeira sessão de treinamento e nas três primeiras séries da última sessão. Os resultados mostraram diferenças na metade final das séries, sendo que no grupo 5:1 houve redução na EMG em todas as porções e o VL e o RF mostraram redução em um número maior de repetições. No grupo 3:3 o VM e o RF apresentaram redução. Para as relações de ativação entre as porções, diferenças foram identificadas apenas no grupo 3:3 nas relações VM/VL e VM/RF em um número reduzido de repetições. Estes resultados apontam que a coordenação entre as porções sofreu poucas alterações quando comparadas as repetições equivalentes de cada série. Sugere-se que os reduzidos graus de liberdade do movimento utilizado e a progressão da carga de treinamento tenham sido os fatores que conduziram a este resultado

Abstract This study investigate the effect of 10-week strenght training on the amplitude of the electromyographic (EMG) signal of vastus medialis, vastus lateralis and rectus Femoris. Twenty three untrained volunteers performed 3-5 sets (3 sets - weeks 1 and 2; 4 series - weeks 3 and 4; 5 series weeks 5 to 10) with 6 repetitions, intensity of 50% of 1 repetition maximum (1RM), 3 min rest between sets and 6 s repetition duration at the knee extensor exercise. One group (5:1) performed concentric action of 5 s and eccentric of 1 s and the other (3:3) performed concentric of 3 s and eccentric of 3 s. The VM, VL and RF EMG (RMS) activities were recorded in each repetition of the three series at the first training session and the first three series at the last session. The protocol 5:1 led to EMG reduction in all portions, with a greater number of repetitions presenting differences at the VL and RF. VM and RF presented similar results at Group 3:3. It was verified that all the differences occurred in the second half of the series. It was also verified differences in EMG ratios just in group 3:3 and only in VM/VL and VM/RF. These results suggest that the coordination was not affected when equivalent repetitions of series were compared. It was also suggested that these results were influenced by the reduced degrees of freedom of the exercise and the training load progression adopted.

Abstract Visceral leishmaniasis (VL), or kala-azar, a serious disease resulting from a systemic infection caused by a protozoan of the genus Leishmania, is potentially fatal to humans. According to data from Sistema de Informação de Agravos de Notificação (Brazil's Information System for Notifiable Diseases) from 2015 to 2016, 6,489 new cases were recorded in Brazil in 22 of the 27 federative units. In addition to typical clinical findings, VL may be associated with autoimmune phenomena, including simulating systemic lupus erythematosus (SLE). We present the first case of autochthonous VL mimicking SLE in Santa Catarina in southern Brazil.

Abstract INTRODUCTION: This study aimed to draw clinical and epidemiological comparisons between visceral leishmaniasis (VL) and VL associated with human immunodeficiency virus (HIV) infection. METHOD: Retrospective study. RESULTS: Of 473 cases of VL, 5.5% were coinfected with HIV. The highest proportion of cases of both VL and VL/HIV were found among men. A higher proportion of VL cases was seen in children aged 0-10 years, whereas coinfection was more common in those aged 18-50 years. CONCLUSIONS: VL/HIV coinfected patients presented slightly differently to and had a higher mortality rate than those with VL only.

Resumo: Este estudo examina a associação entre uso e cobertura do solo e a ocorrência de leishmaniose visceral (LV) canina. Estudo caso-controle em que casos foram residências com cães soropositivos para LV canina e controles, as residências com cães soronegativos. Utilizaram-se imagens de sensoriamento remoto (CBERS: 2/CCD e 2B/HRC) para descrever o uso e cobertura do solo em quadrículas de 625m2 na região do estudo. A chance da LV canina foi duas vezes maior em domicílios localizados em quadrículas com área ≤ 25m2 coberta por estruturas residenciais com pouca vegetação em comparação com aquelas em que essa cobertura alcançou ≥ 600m2. Domicílios localizados em quadrículas com até metade da área coberta por estruturas residenciais com muita vegetação tiveram chances 65% menores de LV canina em comparação com aquelas situadas em áreas quase totalmente cobertas por essa característica. Como a infecção canina costuma preceder a ocorrência de casos humanos, a identificação de características de uso e cobertura do solo associadas à LV canina pode contribuir para a delimitação de áreas de risco para LV humana.

Abstract: This study examines the association between land use and land cover and the occurrence of canine visceral leishmaniasis (VL). This is a case-control study in which cases were households with seropositive dogs for canine VL and controls were households with seronegative dogs. We used remote sensing images (CBERS: 2/CCD and 2B/HRC) to describe land use and cover in squares of 625m2 in the study area. Odds of canine VL were twice as high in households located in squares with an area ≤ 25m2 covered by residential structures with little vegetation in comparison to those where the cover reached ≥ 600m2. Households located in squares with up to half of the area covered by residential structures with extensive vegetation showed 65% lower odds of canine VL in comparison to those situated in areas almost totally covered by this characteristic. Since canine infection usually precedes the occurrence de human cases, identification of the characteristics of land use and cover associated with canine VL can contribute to the demarcation of risk areas for human VL.

Resumen: Este estudio examina la asociación entre uso y cobertura del suelo y la ocurrencia de leishmaniosis visceral (LV) canina. Es un estudio de caso-control, donde los casos fueron analizados en residencias con perros seropositivos para LV canina y controles, además de residencias con perros seronegativos. Se utilizaron imágenes de teledetección (CBERS: 2/CCD e 2B/HRC) para describir el uso y cobertura del suelo en cuadrículas de 625m2 en la región del estudio. La oportunidad de la LV canina fue dos veces mayor en domicilios localizados en cuadrículas con un área ≤ 25m2, cubierta por estructuras residenciales con poca vegetación, en comparación con aquellas donde esa cobertura alcanzó ≥ 600m2. Los domicilios localizados en cuadrículas con hasta la mitad del área cubierta por estructuras residenciales con mucha vegetación tuvieron oportunidades un 65% menores de LV canina, en comparación con aquellas situadas en áreas casi totalmente cubiertas por esa característica.
