RESUMO Objetivo: O presente estudo objetivou avaliar e comparar a intensidade da dor e a Qualidade de Vida Relacionada à Saúde Bucal (QVRSB) de mulheres com disfunção temporomandibular (DTM), antes (T1) e durante (T2) a pandemia de COVID-19. Métodos: A amostra foi composta por quarenta e uma mulheres com DTM dolorosa, que se apresentaram para manejo da DTM. As participantes informaram a intensidade da dor por meio de escala numérica, e responderam ao questionário Perfil de Impacto na Saúde Oral - 14 (Oral Health Impact Profile, OHIP-14). Os dados das participantes foram coletados duas vezes: T1 (avaliação de prontuários odontológicos preenchidos previamente à pandemia de COVID-19) e T2 (por meio de formulário on-line). Dados sociodemográficos foram coletados em T1. A análise estatística foi realizada com nível de significância de 5% (testes de Wilcoxon, qui-quadrado ou exato de Fisher, e regressões lineares múltiplas). Resultados: Nenhuma diferença foi encontrada para a intensidade da dor (p = 0,26) e o escore global do OHIP-14 (p = 0,53). Os domínios relativos à dor física (p = 0,03) e incapacidade social (p = 0,05) melhoraram. Em T1, a ocupação das participantes foi associada ao escore global do OHIP-14 e aos domínios de dor e incapacidade física. Em T2, a idade foi associada aos escores globais do OHIP-14, bem como aos domínios dor física, desconforto psicológico e incapacidade psicológica. Conclusão: A pandemia de COVID-19 não piorou a sensibilidade à dor e a QVRSB das mulheres com DTM dolorosa, e as características sociodemográficas influenciaram suas habilidades de enfrentamento.

ABSTRACT Objective: This study aimed to evaluate and compare pain intensity and Oral Health-related Quality of Life (OHRQoL) in women with temporomandibular disorder (TMD) before (T1) and during (T2) COVID-19 pandemic. Materials and Methods: Sample was composed of forty-one female participants with painful TMD, who presented for TMD treatment. Subjects were asked to indicate their pain intensity and to answer the Oral Health Impact Profile-14 (OHIP-14). Participants data were collected twice: T1 (evaluation of medical records fulfilled before COVID-19 pandemic) and T2 (by means of an online form). Socio-demographic data were assessed in T1. Statistical analysis was performed with a significance level of 5% (Wilcoxon, chi-square or Fisher’s exact tests, multiple linear regressions). Results: No difference was found in pain intensity (p=0.26) and OHIP-14 global scores (p=0.53). Physical pain (p=0.03) and social disability (p=0.05) domains improved. In T1, subject’s occupation was associated with OHIP-14 global score, physical pain, and physical disability domains. In T2, age was associated with OHIP-14 global scores as well as physical pain, psychological discomfort, and psychological disability domains. Conclusion: COVID-19 pandemic did not worsen pain intensity and OHRQoL in women with painful TMD, and it is suggested that socio-demographic characteristics influenced TMD patients coping skills during pandemic.

RESUMO O objetivo deste trabalho é relatar um caso clínico de reabilitação facial com prótese nasal acrílica implanto-retida pós-rinectomia parcial decorrente da ressecção de um carcinoma basocelular em um paciente do sexo masculino, vaqueiro, 53 anos de idade, caucasiano e normossistêmico. Após exame clínico e de posse da tomografia computadorizada, foi realizada a cirurgia de instalação dos implantes dentários convencionais na região da pré-maxila (lado esquerdo). Depois de realizada a cirurgia, esperou-se a osseointegração dos implantes por três meses e, posteriormente, foi realizada a confecção da prótese nasal que, ao final do tratamento, apresentou adaptação periférica, retenção e estética satisfatórias. O paciente tem retornado a cada 6 meses para proservação quanto à análise da cor da prótese, saúde periimplantar, higienização, troca dos magnetos e reembasamentos quando necessários. Os autores concluem que essa terapia pode ser bem indicada em defeitos nasais parciais e totais, desde que haja correto planejamento e execução das fases cirúrgica, clínica e laboratorial.

ABSTRACT The objective of this study was to report a clinical case of facial rehabilitation with an acrylic implant-retained nasal prosthesis after partial rhinectomy resulting from the resection of a basal cell carcinoma in the patient, a 53-year-old Caucasian, normosystemic, professional cowboy. After clinical examination and in possession of computed tomography images, surgery was performed for the placement of conventional dental implants in the premaxillary region (left side). After the surgery, a three-month period of waiting was observed to allow osseointegration of the implants. Later, the nasal prosthesis was prepared, and at the end of treatment, this presented satisfactory peripheral adaptation, retention and esthetics. The patient has returned for follow-up every 6 months, regarding analysis of the prosthesis color, peri-implant health, hygiene, and exchange of the magnets and relays when necessary. The authors concluded that this therapy may be well indicated in patients with partial and total nasal defects, provided that there is correct planning and execution of the surgical, clinical and laboratory phases.

RESUMO Reabilitações estéticas imediatas são práticas frequentes na Odontologia restauradora atual. Por vezes, sem um diagnóstico e um planejamento clínico preciso, duas ou três sessões clínicas têm sido o bastante para iniciar e finalizar uma reabilitação oral extensa totalmente cerâmica. Contudo é fundamental que esses tratamentos voltem a ser sustentados por princípios biológicos, mecânicos e oclusais básicos, instituídos há décadas, e não apenas por aspectos estéticos. No presente relato de caso, uma paciente de 26 anos, bruxista, com necessidades funcionais e estéticas, foi reabilitada com restaurações cerâmicas nos 6 dentes anteriores superiores, por meio de uma abordagem interdisciplinar. O enceramento diagnóstico realizado foi fundamental para prever o resultado, guiar a cirurgia periodontal, servir de referência para os preparos dentais e restaurar a dinâmica dos movimentos mandibulares. Após a cimentação final das restaurações, a proteção com placa oclusal no período do sono foi instituída. Concluiu-se que um criterioso diagnóstico e planejamento foi fundamental para o desfecho do caso, e que a qualidade e a longevidade desse tratamento restaurador parece depender intimamente do respeito aos conceitos biomecânicos, oclusais e estéticos.

ABSTRACT Immediate esthetic rehabilitation is a common practice in contemporary restorative dentistry. Sometimes without a precise diagnosis and clinical planning, two or three clinical sessions have been sufficient to start and end an extensive all-ceramic oral rehabilitation. It is essential, however, for these treatments to be based on the biological, mechanical, and occlusal principles that have been established for decades, and not only on aesthetic considerations. In this case report, a 26 year-old bruxist patient with functional and aesthetic needs, was rehabilitated with ceramic restorations in the six maxillary anterior teeth, through an interdisciplinary approach. The diagnostic wax-up was essential to predict the outcome; guide the periodontal surgery; serve as a reference for dental preparations, and restore the dynamics of jaw movements. After the adhesive cementation, a protective occlusal splint for use during sleep was introduced. The authors concluded that careful diagnosis and planning was critical to outcome of this case, and that the quality and longevity of restorative treatment depended on the respect for biomechanical, occlusal, and aesthetic concepts.

RESUMO Objetivo: Analisar, por meio do método dos elementos finitos, a distribuição de tensões em elemento dental pré-molar inferior unirradicular restaurado de duas maneiras distintas. Métodos: A simulação laboratorial considerou as seguintes situações: 1) pré-molar restaurado com núcleo metálico fundido em liga áurea e coroa em cerâmica; 2) pré-molar restaurado com pino de fibra de vidro, núcleo de preenchimento em resina composta e coroa em cerâmica. Para isso foi utilizado um modelo bidimensional de um pré-molar com aplicação de carga de 300N nas direções axial e oblíqua com uso de programa ANSYS para Windows. Resultados: Diante da carga axial, foi observada uma maior concentração de tensões na região do núcleo fundido (166 Mpa). Na simulação de carga oblíqua, observou-se uma concentração de tensões na parede radicular vestibular, interna e externamente, no dente restaurado com núcleo fundido (222 Mpa). Nas duas situações em estudo, o pino de fibra de vidro e núcleo de preenchimento dissipou, com maior uniformidade, a tensão recebida (55 a 111 Mpa). Conclusão: Esses dados sugerem que o uso de pino de fibra pré-fabricado e núcleo de preenchimento em resina composta pode ser mais vantajoso que a indicação de núcleo fundido, sob o ponto-de-vista biomecânico, em dente pré-molar inferior unirradicular.

ABSTRACT Objective: The objective of this study by the finite element method was to analyze the stress distribution on a single-rooted mandibular premolar tooth restored in two different ways. Methods: Laboratory simulation considered the following situations: 1) premolar restored with gold cast post and ceramic crown; 2) premolar restored with glass fiber post, composite resin core and ceramic crown. For this we used a two-dimensional model of a premolar with load application of 300N in the axial and oblique directions within the ANSYS program for Windows. Results: Under the axial load, a higher concentration of stresses was observed around the cast post (166 MPa). In the oblique load simulation, there was a concentration of stresses on the buccal root wall, internally and externally, in the tooth restored with cast post (222 MPa). In both load directions, the glass fiber post dissipated the stress received with greater uniformity (55 to 111 MPa). Conclusion: These data suggested that from the biomechanical point of view, the use of a glass fiber post and composite resin core could be more advantageous than the cast post technique for a mandibular single-rooted premolar tooth.

Resumo Este estudo avaliou a eficácia de soluções higienizadoras na remoção do biofilme de dentadura por meio de ensaio clínico randomizado cruzado. Trinta e dois pacientes desdentados foram instruídos a escovar suas dentaduras (escova específica e sabão líquido) três vezes ao dia (após café da manhã, almoço e jantar) e imergi-las (≥ 8 horas) em: (C) controle - água; (PA): peróxido alcalino; ou (HS) hipoclorito de sódio a 0,5%. Cada solução foi usada por 21 dias (três ciclos alternados de 7 dias). Ao final de cada ciclo, a superfície interna da dentadura maxilar foi evidenciada (vermelho neutro 1%) e fotografada (HX1- Sony). As áreas (total e corada com biofilme) foram medidas (software Image Tool), e a porcentagem de biofilme calculada como a relação entre a área do biofilme multiplicado por 100 e área da superfície total da base interna da dentadura. Os dados foram comparados por meio de equações de estimação generalizadas (α=5%) e comparações múltiplas (Bonferroni - α=1,67%). A imersão em HS reduziu o biofilme (%) (8,3 ± 13,3B) em comparação com C (18,2 ± 14,9A) e PA (18,2 ± 16,6A). A solução de hipoclorito de sódio a 0,5% foi a mais eficaz na remoção do biofilme. Peróxidos alcalinos podem não levar a maior remoção do biofilme em pacientes com hábitos adequados de manutenção de dentadura.

Abstract This study evaluated the efficacy of cleanser solutions on denture biofilm removal by a crossover randomized clinical trial. Thirty two edentulous patients were instructed to brush their dentures (specific brush and liquid soap) three times a day (after breakfast, lunch and dinner) and to soak them (≥ 8 h) in: (C) control -water; (AP): alkaline peroxide; or (SH) 0.5% sodium hypochlorite. Each solution was used for 21 days (three cycles of 7 days). At the end of each cycle, the inner surfaces of maxillary dentures were disclosed (1% neutral red) and photographed (HX1 - Sony). Areas (total and stained biofilm) were measured (Image Tool software) and the percentage of biofilm calculated as the ratio between the area of the biofilm multiplied by 100 and total surface area of the internal base of the denture. Data were compared by means of generalized estimating equation (α=5%) and multiple comparisons (Bonferroni; α=1.67%). Immersion in SH reduced biofilm (%) (8.3 ± 13.3B) compared to C (18.2 ± 14.9A) and AP (18.2 ± 16.6A). The 0.5% sodium hypochlorite solution was the most efficacious for biofilm removal. Alkaline peroxides may not lead to further biofilm removal in patients with adequate denture maintenance habits.

Fluorinated denture base acrylic resins can present more stable physical properties when compared with conventional polymers. This study evaluated the incorporation of a fluoroalkyl methacrylate (FMA) mixture in a denture base material and its effect on roughness and flexural strength. A swelling behavior assessment of acrylic resin specimens (n=3, per substance) after 12 h of FMA or methyl methacrylate (MMA) immersion was conducted to determine the solvent properties. Rectangular specimens (n=30) were allocated to three groups, according to the concentration of FMA substituted into the monomer component of a heat-polymerized acrylic resin (Lucitone 550), as follows: 0% (control), 10% and 20% (v/v). Acrylic resin mixed with concentrations of 25% or more did not reach the dough stage and was not viable. The surface roughness and flexural strength of the specimens were tested. Variables were analyzed by ANOVA and Tukey's test (a=0.05). Immersion in FMA produced negligible swelling, and MMA produced obvious swelling and dissolution of the specimens. Surface roughness at concentrations of 0%, 10% and 20% were: 0.25 ± 0.04, 0.24 ± 0.04, 0.22 ± 0.03 mm (F=1.78; p=0.189, not significant). Significant differences were found for flexural strength (F=15.92; p<0.001) and modulus of elasticity (F=7.67; p=0.002), with the following results: 96 ± 6, 82 ± 5, 84 ± 6 MPa, and 2,717 ± 79, 2,558 ± 128, 2574 ± 87 MPa, respectively. The solvent properties of FMA against acrylic resin are weak, which would explain why concentrations over 20% were not viable. Surface changes were not detected after the incorporation of FMA in the denture base acrylic resin tested. The addition of FMA into denture base resin may lower the flexural strength and modulus of elasticity, regardless of the tested concentration.

Este estudo avaliou o efeito do tipo de resina de base (termo-polimerizada e polimerizada por microondas), tratamento da superfície de união (com e sem aplicação de metil metacrilato) e termociclagem sobre a resistência adesiva à microtração (RAµT) do dente artificial Biotone. A superfície cervical dos dentes foi desgastada para se obter superfícies planas, as quais foram unidas a cada uma das resinas, constituindo os seguintes grupos (n=6): (G1a) Clássico/com MMA; (G1b) Clássico/sem MMA; (G2a) OndaCryl/com MMA; (G2b) OndaCryl/sem MMA. Corpos-de-prova retangulares com área transversal de 1 mm² foram preparados. Metade dos espécimes em cada grupo foi termociclado (5000 ciclos de 4ºC a 60ºC). O teste de resistência adesiva foi realizado numa máquina de ensaios universal (velocidade da carga: 1 mm/min). Os dados foram analisados estatisticamente por meio da ANOVA para 3 fatores (α=0,05). Os valores médios de RAµT e desvios-padrão foram: Termociclagem - G1a: 41,00 (14,00); G1b: 31,00 (17,00); G2a: 50,00 (27,00); G2b: 40,00 (18,00); Sem termociclagem - G1a: 37,00 (14,00); G1b: 43,00 (25,00); G2a: 43,00 (14,00); G2b: 40,00 (27,00). A RAµT entre os dentes artificiais e as resinas acrílicas para base de prótese não foi influenciada pelos fatores resina acrílica, tratamento de superfície ou termociclagem.

This study evaluated the effect of denture base polymer type (heat- and microwave-polymerized), ridge lap surface treatment (with and without methyl methacrylate-MMA etching) and thermocycling on the microtensile bond strength (µTBS) of Biotone acrylic teeth. Flat-ground, ridge-lap surface of posterior artificial teeth were bonded to cylinders of each denture base resin, resulting in the following groups (n=6): G1a - Clássico/with MMA etching; G1b - Clássico/without MMA etching; G2a - OndaCryl/with MMA etching; G2b - OndaCryl/without MMA etching. Rectangular bar specimens with a cross-sectional area of 1 mm² were prepared. Half of the bars in each group were thermocycled (5,000 cycles between 4ºC and 60ºC). µTBS testing was performed in an universal testing machine at a crosshead speed of 0.5 mm/min. Data were analyzed statistically by three-way ANOVA (α=0.05). There was no statisticaly significant difference (p>0.05) for the factors (resin, surface treatment,and thermocycling) or their interactions. The mean µTBS values (MPa) and standard deviations were as follows: Thermocycling - G1a: 41.00 (14.00); G1b: 31.00 (17.00); G2a: 50.00 (27.00); G2b: 40.00 (18.00); No thermocycling - G1a: 37.00 (14.00); G1b: 43.00 (25.00); G2a: 43.00 (14.00); G2b: 40.00 (27.00). The µTBS of Biotone artificial teeth to the denture base acrylic resins was not influenced by the polymer type, surface treatment or thermocycling.
