Sintomas Cardiopulmonares Pós-COVID-19: Preditores e Características de Imagem de Pacientes após a Alta Hospitalar

Abstract Background Most of the evidence about the impact of the post-acute COVID-19 Syndrome (PACS) reports individual symptoms without correlations with related imaging. Objectives To evaluate cardiopulmonary symptoms, their predictors and related images in COVID-19 patients discharged from hospital. Methods Consecutive patients who survived COVID-19 were contacted 90 days after discharge. The Clinic Outcome Team structured a questionnaire evaluating symptoms and clinical status (blinded for hospitalization data). A multivariate analysis was performed to address the course of COVID-19, comorbidities, anxiety, depression, and post-traumatic stress during hospitalization, and cardiac rehabilitation after discharge. The significance level was set at 5%. Results A total of 480 discharged patients with COVID-19 (age: 59±14 years, 67.5% males) were included; 22.3% required mechanical ventilation. The prevalence of patients with PACS-related cardiopulmonary symptoms (dyspnea, tiredness/fatigue, cough, and chest discomfort) was 16.3%. Several parameters of chest computed tomography and echocardiogram were similar in patients with and without cardiopulmonary symptoms. The multivariate analysis showed that PACS-related cardiopulmonary-symptoms were independently related to female sex (OR 3.023; 95% CI 1.319-6.929), in-hospital deep venous thrombosis (OR 13.689; 95% CI 1.069-175.304), elevated troponin I (OR 1.355; 95% CI 1.048-1.751) and C-reactive protein during hospitalization (OR 1.060; 95% CI 1.023-1.097) and depression (OR 6.110; 95% CI 2.254-16.558). Conclusion PACS-related cardiopulmonary symptoms 90 days post-discharge are common and multifactorial. Beyond thrombotic and markers of inflammation/myocardial injury during hospitalization, female sex and depression were independently associated with cardiopulmonary-related PACS. These results highlighted the need for a multifaceted approach targeting susceptible patients.
Resumo Fundamento A maioria da evidência sobre o impacto da síndrome COVID pós-aguda (PACS, do inglês, post-acute COVID-19 syndrome) descreve sintomas individuais sem correlacioná-los com exames de imagens. Objetivos Avaliar sintomas cardiopulmonares, seus preditores e imagens relacionadas em pacientes com COVID-19 após alta hospitalar. Métodos Pacientes consecutivos, que sobreviveram à COVID-19, foram contatados 90 dias após a alta hospitalar. A equipe de desfechos clínicos (cega quanto aos dados durante a internação) elaborou um questionário estruturado avaliando sintomas e estado clínico. Uma análise multivariada foi realizada abordando a evolução da COVID-19, comorbidades, ansiedade, depressão, e estresse pós-traumático durante a internação, e reabilitação cardíaca após a alta. O nível de significância usado nas análises foi de 5%. Resultados Foram incluídos 480 pacientes (idade 59±14 anos, 67,5% do sexo masculino) que receberam alta hospitalar por COVID-19; 22,3% necessitaram de ventilação mecânica. A prevalência de pacientes com sintomas cardiopulmonares relacionados à PACS (dispneia, cansaço/fadiga, tosse e desconforto no peito) foi de 16,3%. Vários parâmetros de tomografia computadorizada do tórax e de ecocardiograma foram similares entre os pacientes com e sem sintomas cardiopulmonares. A análise multivariada mostrou que sintomas cardiopulmonares foram relacionados de maneira independente com sexo feminino (OR 3,023; IC95% 1,319-6,929), trombose venosa profunda durante a internação (OR 13,689; IC95% 1,069-175,304), nível elevado de troponina (OR 1,355; IC95% 1,048-1,751) e de proteína C reativa durante a internação (OR 1,060; IC95% 1,023-1,097) e depressão (OR 6,110; IC95% 2,254-16,558). Conclusão Os sintomas cardiopulmonares relacionados à PACS 90 dias após a alta hospitalar são comuns e multifatoriais. Além dos marcadores trombóticos, inflamatórios e de lesão miocárdica durante a internação, sexo feminino e depressão foram associados independentemente com sintomas cardiopulmonares relacionados à PACS. Esses resultados destacaram a necessidade de uma abordagem multifacetada direcionada a pacientes susceptíveis.

Upconversion nanoparticles (UCNPs) are suitable materials for bioapplications due to their ability to emit visible light under near infrared (NIR) excitation, in the biological transparency range. Polycaprolactone(PCL)-based scaffolds are widely used in tissue engineering in combination with inorganic compounds to improve bioactivity and osteoconductive properties. This work proposes a 3D printed composite scaffold with upconversion property aiming at biomedical applications in therapy-stimulated bone repair and photodynamic therapy (PDT). The system combines PCL polymer, UCNPs-apatite and a PDT photosensitizer. Thermal and rheological behaviors of the composite were similar to pure PCL polymer. Mechanical properties were improved by adding UCNPs-apatite. The 3D printable composite presented upconversion property and potential for PDT application, which was demonstrated by singlet oxygen generation under 980 nm excitation. Cytotoxicity, genotoxicity and mutagenicity assays indicated no toxicological effects at low concentrations of rare earth elements. Taken together, a potential multifunctional material is proposed for biomedical applications.

Abstract: We have identified specimens of the leafhopper Gypona cerea DeLong & Freytag (Cicadellidae: Iassinae: Gyponini) collected from a crop of dwarf cashew (Anacardium occidentale Linnaeus 1753) located in Canto do Buriti, Piauí, Northeastern Brazil. The damage caused to the crop included inflorescences covered with sticky exudates, presence of sooty mold, and yellowing and drying of new shoots and inflorescences. Such documentation serves as a warning about possible outbreaks of infestation in the region and the likelihood of its expansion to other cashew producing areas.

ABSTRACT Prolactinomas are the most common pituitary adenomas (approximately 40% of cases), and they represent an important cause of hypogonadism and infertility in both sexes. The magnitude of prolactin (PRL) elevation can be useful in determining the etiology of hyperprolactinemia. Indeed, PRL levels > 250 ng/mL are highly suggestive of the presence of a prolactinoma. In contrast, most patients with stalk dysfunction, drug-induced hyperprolactinemia or systemic diseases present with PRL levels < 100 ng/mL. However, exceptions to these rules are not rare. On the other hand, among patients with macroprolactinomas (MACs), artificially low PRL levels may result from the so-called “hook effect”. Patients harboring cystic MACs may also present with a mild PRL elevation. The screening for macroprolactin is mostly indicated for asymptomatic patients and those with apparent idiopathic hyperprolactinemia. Dopamine agonists (DAs) are the treatment of choice for prolactinomas, particularly cabergoline, which is more effective and better tolerated than bromocriptine. After 2 years of successful treatment, DA withdrawal should be considered in all cases of microprolactinomas and in selected cases of MACs. In this publication, the goal of the Neuroendocrinology Department of the Brazilian Society of Endocrinology and Metabolism (SBEM) is to provide a review of the diagnosis and treatment of hyperprolactinemia and prolactinomas, emphasizing controversial issues regarding these topics. This review is based on data published in the literature and the authors' experience.

In this study, a photoelectroanalytical sensor for determination of adrenaline based on deoxyribonucleic acid (DNA) and anatase titanium dioxide (TiO2) nanoparticles sensitized with bis(ethylenedithio)tetrathiafulvalene (BEDT-TTF) was developed, which we henceforward call BEDT-TTF/DNA/TiO2/ITO. The photoelectroanalytical sensor showed high photocurrent to adrenaline under visible light emitting diode (LED) light irradiation in comparison to each component of the composite material. Under optimized conditions, the BEDT-TTF/DNA/TiO2/ITO sensor shows a linear response range from 10 nmol L-1 up to 100 µmol L-1 with a sensitivity of 8.1 nA L µmol-1 and limit of detection of 1 nmol L-1 for the detection of adrenaline. The photoelectrochemical sensor showed high photocurrent to adrenaline in comparison to photocurrent response to ascorbic acid and uric acid. The BEDT-TTF/DNA/TiO2/ITO photoelectrochemical sensor was successfully applied to urine samples, with recovery values between 96 and 106%.

Can chest high-resolution computed tomography findings diagnose pulmonary alveolar microlithiasis?

Resumo Objetivo: Analisar, retrospectivamente, as tomografias computadorizadas de alta resolução (TCAR) de pacientes com microlitíase alveolar pulmonar, a fim de avaliar a frequência dos padrões tomográficos e sua distribuição no parênquima pulmonar. Materiais e Métodos: O estudo incluiu 13 pacientes (9 femininos e 4 masculinos) com idades variando de 9 a 59 anos (média de 34,5 anos). Os exames foram avaliados por dois observadores, de modo independente, e os casos discordantes, resolvidos por consenso. O critério de inclusão foi a presença de anormalidades na TCAR típicas de microlitíase alveolar pulmonar, o que prescinde a necessidade de exame histopatológico. Entretanto, em 6 casos foram realizadas biópsias pulmonares. Resultados: Os achados tomográficos predominantes foram opacidades em vidro fosco e pequenos nódulos parenquimatosos, presentes em 100% dos casos, seguidos de pequenos nódulos subpleurais (92,3%), cistos subpleurais (84,6%), calcificações lineares subpleurais (69,2%), padrão de pavimentação em mosaico (69,2%), fissura nodular (53,8%), calcificação ao longo dos septos interlobulares (46,2%) e consolidações densas (46,2%). Conclusão: Quanto à distribuição dos achados, houve acometimento preferencial dos terços inferiores. Não foi observado predomínio de distribuição dos sentidos axial e anteroposterior
Abstract Objective: The present study was aimed at retrospectively reviewing high-resolution computed tomography (HRCT) findings in patients with pulmonary alveolar microlithiasis in order to evaluate the frequency of tomographic findings and their distribution in the lung parenchyma. Materials and Methods: Thirteen patients (9 females and 4 males; age, 9 to 59 years; mean age, 34.5 years) were included in the present study. The HRCT images were independently evaluated by two observers whose decisions were made by consensus. The inclusion criterion was the presence of abnormalities typical of pulmonary alveolar microlithiasis at HRCT, which precludes lung biopsy. However, in 6 cases lung biopsy was performed. Results: Ground-glass opacities and small parenchymal nodules were the predominant tomographic findings, present in 100% of cases, followed by small subpleural nodules (92.3%), subpleural cysts (84.6%), subpleural linear calcifications (69.2%), crazy-paving pattern (69.2%), fissure nodularity (53.8%), calcification along interlobular septa (46.2%) and dense consolidation (46.2%). Conclusion: As regards distribution of the lesions, there was preferential involvement of the lower third of the lungs. No predominance of distribution in axial and anteroposterior directions was observed.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Hyponatremia in visceral leishmaniasis

Existem poucos relatos relacionando hiponatremia com a leshmaniose visceral (calazar). Este é um estudo de 55 pacientes portadores de calazar e um grupo controle de 20 indivíduos normais. Hiponatremia e hipo-osmolalidade sérica foram detectados em 100% dos pacientes portadores de calazar. A presença de alta osmolalidade da primeira urina da manhã (750,0 ± 52,0 vs. 894,5 ± 30 mOsm/Kg H2O, p < 0,05) e da urina de 24h (426,0 ± 167,0 vs. 514,6 ± 132,0 mOsm/Kg H2O, p < 0,05), demonstraram a presença de persistente secreção de hormônio antidiurético. A concentração de sódio urinário foi elevada (82,3 ± 44,2 vs. 110,3 ± 34,7 mEq/L, p < 0,05). Hipouricemia ocorreu em 61,8% dos pacientes e aumento da fração de excreção urinária de ácido úrico foi detectada em 74,5% dos casos. Aumento da velocidade de filtração glomerular estava presente em 25,4% dos pacientes. Não havia evidência clínica de depleção de volume extracelular. Valores normais de ADH plasmático foram observados nos pacientes com calazar. Não foi detectada disfunção renal ou endócrina. É provável, que a maioria dos pacientes com calazar apresente uma síndrome de secreção inapropriada de hormônio antidiurético.
There are few reports linking hyponatremia and visceral leishmaniasis (kala-azar). This is a study of 55 consecutive kala-azar patients and 20 normal individuals as a control group. Hyponatremia and serum hypo-osmolality were detected in 100% of kala-azar patients. High first morning urine osmolality (750.0 ± 52.0 vs. 894.5 ± 30.0mOsm/kg H2O, p < 0.05), and high 24-hour urine osmolality (426.0 ± 167.0 vs. 514.6 ± 132.0 mOsm/kg H2O, p < 0.05) demonstrated persistent antidiuretic hormone secretion. Urinary sodium was high (82.3 ± 44.2 vs.110.3 ± 34.7 mEq/L, p < 0.05). Low seric uric acid occurred in 61.8% of patients and increased fractional urinary uric acid excretion was detected in 74.5% of them. Increased glomerular filtration rate was present in 25.4% of patients. There was no evidence of extracellular volume depletion. Normal plasma ADH levels were observed in kala-azar patients. No endocrine or renal dysfunction was detected. It is possible that most hyponatremic kala-azar patients present the syndrome of inappropriate antidiuretic hormone secretion.

FUNDAMENTO: No Brasil, a prevalência de hipertensão arterial (HA) e seus fatores de risco são pouco conhecidos nas regiões menos desenvolvidas. OBJETIVO: Estimar a prevalência da hipertensão arterial na população > 18 anos em São Luís - MA e fatores associados, de acordo com os critérios do Seventh Report of the Joint National Committee (JNC 7). MÉTODOS: Realizou-se estudo transversal em São Luís - MA, de fevereiro a março de 2003, em 835 pessoas com idade > 18 anos que responderam a um questionário estruturado em domicílio. Foram medidos pressão arterial (PA), peso, altura e circunferência abdominal. Avaliaram-se outros fatores de risco para doença cardiovascular. Na identificação dos fatores associados à HA foi utilizado o modelo de regressão de Poisson, com estimativa da razão de prevalências (RP) e seu respectivo intervalo de confiança de 95%. RESULTADOS: A idade variou entre 18 e 94 anos (média de 39,4 anos), sendo 293 (35,1%) pessoas normotensas e 313 (37,5%) pré-hipertensas. A prevalência de HA foi de 27,4% (IC95% 24,4% a 30,6%), maior no sexo masculino (32,1%) que no feminino (24,2%). Na análise ajustada permaneceram independentemente associados à HA: sexo masculino (RP 1,52 IC95% 1,25-1,84), idade > 30 anos, sendo RP=6,65, IC95% 4,40-10,05 para idade > 60 anos, sobrepeso (RP 2,09 IC95% 1,64-2,68), obesidade (RP 2,68 IC95% 2,03-3,53) e diabete (RP 1,56 IC95% 1,24-1,97). CONCLUSÃO: Os resultados sugerem a necessidade de controle do sobrepeso, obesidade e diabete, sobretudo em mulheres e pessoas com idade maior ou igual a 30 anos para a redução da prevalência da hipertensão arterial.
BACKGROUND: Little is known about the prevalence of arterial hypertension (AH) and its risk factors in the less developed regions of Brazil. OBJECTIVE: To estimate the prevalence of arterial hypertension and its associated factors in the population > 18 years in São Luís, state of Maranhão according to the Seventh Report of the Joint National Committee (JNC 7) criteria. METHODS: A cross-sectional study was conducted in São Luís, MA, from February to March 2003, with 835 individuals >18 years who completed a structured household questionnaire. Measurements of arterial pressure (AP), weight, height and waist circumference were taken, and other risk factors for cardiovascular disease were assessed. The Poisson regression method was used for the identification of factors associated with AH, with an estimate of the prevalence ratio (PR) and its corresponding 95% confidence interval. RESULTS: Age varied from 18 and 94 years (mean age was 39.4 years), 293 (35.1%) individuals were normotensive and 313 (37.5%) were pre-hypertensive. The AH prevalence was 27.4% (95% CI - 24.4% to 30.6%), and it was higher among men (32.1%) than among women (24.2%). In the adjusted analysis, the following remained independently associated with AH: male gender (PR 1.52, 95% CI, 1.25-1.84), age > 30 years, with PR=6.65, 95% CI, 4.40-10.05 for >60 years of age, overweight (PR 2.09 95% CI 1.64-2.68), obesity (PR 2.68, 95% CI, 2.03-3.53) and diabete (PR 1.56, 95% CI, 1.24-1.97). CONCLUSION: These findings suggest the need to control overweight, obesity and diabete, especially among women and individuals > 30 years of age in order to reduce the prevalence of arterial hypertension.

The objective of this study was to evaluate the effect of different nutrient solutions and harvest date on production and nutritive quality of hydroponic forage wheat (Triticum aestivum L.). The experiment was conducted inside a protected environment at Fitotecnia Department of the Universidade Federal at Santa Maria, Brazil. The experimental design was a randomized complete block. Treatments consisted of two nutrient solutions (SIN: inorganic solution by Neves (2001), and SONC: organic solution of the vermicompost) and four harvesting dates (4, 8, 12, and 16 days after emergence), distributed in a 2x4 factorial scheme. It was observed interaction (P<0.05) between nutrient solution and harvest date for the variables plant height, dry weight (DW), crude protein (CP), soluble protein (SP), and neutral detergent fiber (NDF). SONC and SIN showed a decrease in DW values when delayed harvest dates. SONC presented an increase in the values of DW, CP, and SP, but a decrease in NDF in comparison with SIN in all harvest dates. It was observed an increase in acid detergent fiber (ADF), lignin (LIG), and cellulose (CEL) and a decrease in digestibility “in vitro” when delaying the harvesting dates. It was not observed statistical difference (P>0.05) in ADF, LIG, CEL, and digestibility “in vitro” among the solutions. The best quality characteristics (CP and digestibility “in vitro”) of hydroponic forage wheat were obtained when harvest was 8 to 12 days after sowing.
Com o objetivo de avaliar a produção e a qualidade nutricional da forragem hidropônica de trigo (Triticum aestivum L.) cultivado em ambiente protegido com soluções nutritivas inorgânicas e orgânicas parcialmente corrigidas, identificando a idade ideal de colheita, realizou-se um estudo no Departamento de Fitotecnia da Universidade Federal de Santa Maria. Adotou-se o delineamento experimental em blocos ao acaso, distribuídos em esquema fatorial 2x4 com duas soluções nutritivas: SIN: solução inorgânica segundo recomendação de Neves (2001) e SONC: solução orgânica parcialmente corrigida atendendo recomendação de Neves (2001) e quatro idades de colheita (4, 8, 12 e 16 dias). Foi observada significância (P<0,05) à interação soluções nutritivas x idades de colheita para as variáveis: estatura, fitomassa seca (FS), proteína bruta (PB), proteína solúvel (OS) e fibra em detergente neutro (FDN). Verificou-se para a SIN e SONC diminuição da FS, com o avanço da idade de colheita. A solução SONC proporcionou valores ligeiramente maiores de FS, PB e PS e menores de FDN em relação à SIN, em todas as idades de colheita. Não houve diferença significativa entre as soluções (P>0,05) para os teores de fibra em detergente ácido, lignina, celulose e digestibilidade “in vitro” da fitomassa seca. O aumento da idade de colheita promoveu maiores teores de FDA, lignina e celulose e menor digestibilidade “in vitro” da fitomassa seca (DIVFS). A idade de colheita ideal situa-se entre os 8-12 dias, pois neste período têm-se valores intermediários favorecendo o teor protéico sem comprometer a redução de FS e DIVFS devido ao maior teor fibras.

OBJETIVO: avaliar o efeito do tamoxifeno no perfil lipídico e renal de ratos controles e diabéticos. MÉTODOS: Foram utilizados 40 ratos fêmeas Wistar (180-220g peso corporal), divididos randomicamente em 4 grupos: C (n=10, receberam veículo), T (n=10, tratados com tamoxifeno, 0,3mg/kg/dia), D (n=10, diabéticos induzidos por estreptozotocina, 45mg/Kg) e DT (n=10, diabéticos tratados com tamoxifeno). Foram dosados os analitos, glicose, colesterol total, triglicérides, proteínas totais, albumina, uréia e creatinina utilizando Kits Labtest através do analisador Cobas Mira (Alemanha,1996). RESULTADOS: o grupo T apresentou diminuição do colesterol total e triglicérides em relação ao C, e o grupo D um aumento em relação aos demais. Para as proteínas totais foi observado um aumento no Grupo T em relação ao C. A albumina diminuiu nos grupos D e DT em relação aos grupos C e T. Nos níveis de uréia houve um aumento no grupo D e DT em relação aos grupos C e T. CONCLUSÃO: Em relação ao perfil lipídico foi constatado que durante o período de 60 dias o tratamento com tamoxifeno promoveu uma diminuição dos níveis séricos de colesterol e triglicérides, mesmo associado a condição de Diabetes mellitus.
OBJECTIVE: To evaluate the effect of tamoxifen in the lipidic and renal profile in diabetic and control rats. METHODS: 40 female Wistar rats had been used (180-220g corporal weight), divided randomly in 4 groups: C (n=10, received vehicle), T (n=10, treated with tamoxifen, 0.3mg/kg/day), D (n=10, diabetic induced by streptozotocin, 45mg/Kg) and DT (n=10, diabetic treated with tamoxifen). Glucose, total cholesterol, triglyceride, total proteins, albumin, urea and creatinin were analyzed using Labtest Kits through the Cobas Mira analyzer (Germany, 1996). RESULTS: Group T presented a reduction of total cholesterol and triglyceride in relation to C, and group D an increase in relation to the other ones. About the total proteins it was observed an increasing in Group T in relation to C. The albumin diminished in groups D and DT in relation to C and T. The urea levels increased in groups D and DT in relation to C and T. CONCLUSION: In relation to the lipidic profile it was evidenced that during the period of 60 days the treatment with tamoxifen promoted a reduction in cholesterol and triglyceride levels in the serum, even associated to the condition of Diabetes mellitus.
