RESUMO O objetivo foi avaliar a relação espacial dos indicadores de saneamento (acesso à água e ao esgoto), considerando a modelagem da concessão da Companhia Estadual de Águas e Esgotos do Rio de Janeiro (Cedae), e a sua possível relação com a incidência da Covid-19, nos bairros com e sem áreas de favelas. Os dados de casos confirmados da Covid-19 analisados foram obtidos no Painel Saúde do Rio de Janeiro. Foi considerado o fracionamento do município em quatro blocos regionais. Nos 163 bairros da cidade, a taxa de incidência média foi de 9,78 casos/1.000 hab. Os bairros com as menores taxa de incidência foram aqueles com predominância de aglomerados subnormais (favelas), baixa renda per capita e cobertura de saneamento, sugerindo maior lacuna de testagem e subnotificação de casos. Reduzir a inequidade e o racismo estrutural deveria ser prioritário. A sindemia da Covid-19 contribuiu fortemente para perdas socioeconômicas e de saúde pública significativas. Em um cenário de reconstrução, é imprescindível retomar o compromisso para com os Objetivos de Desenvolvimento Sustentável da Agenda 2030 das Nações Unidas.

Abstract The aim of this study was to investigate the segregation patterns of molar incisor hypomineralization (MIH) in families, given the evidence that its etiology is influenced by genetics. Clinically, MIH may be detected in parents and/or siblings of MIH-affected children. Our study included children with at least one first permanent molar affected by MIH (proband) and their first-degree relatives (parents and siblings). The participants were examined clinically to detect MIH, according to the European Academy of Paediatric Dentistry criteria (2003). A total of 101 nuclear families (391 individuals) were studied. Proband diagnosis was followed by MIH classification of the subject, his parents and siblings, as affected, unaffected, or unknown. Segregation analysis was performed using the multivariate logistic regression model of the Statistical Analysis for Genetic Epidemiology package, and segregation models (general transmission, environmental, major gene, dominant, codominant and recessive models). The Akaike information criterion (AIC) was used to evaluate the most parsimonious model. In all, 130 affected individuals, 165 unaffected individuals, and 96 unknown individuals were studied. Severe MIH was found in 50.7% of the cases. A segregation analysis performed for MIH revealed the following different models: environmental and dominance (p = 0.05), major gene (p = 0.04), codominant (p = 0.15) and recessive models (p = 0.03). According to the AIC values, the codominant model was the most parsimonious (AIC = 308.36). Our results suggest that the codominant model could be the most likely for inheriting MIH. This result strengthens the evidence that genetic factors, such as multifactorial complex defect, influence MIH.

Resumo Objetivo: avaliar, em condições simuladas de campo, a eficácia do pyriproxyfen (hormônio juvenil), do novaluron (inibidor de quitina) e do spinosad (biolarvicida) no controle do Aedes aegypti. Métodos: exposição periódica de larvas de Ae. aeypti obtidas em Itabuna a recipientes tratados com os larvicidas, e comparação do efeito residual do tratamento com cepa Rockfeller. Resultados: o efeito inibitório na emergência de adultos, após 60 dias, foi de 89,5% spinosad, 96,5% novaluron e 75,4% pyriproxifen para larvas de Itabuna, não havendo diferença estatística (p=0,412) entre os tratamentos; spinosad e novaluron apresentaram maior percentual de mortalidade na fase larval, 98,8% e 97,9% respectivamente; pyriproxifen apresentou mortalidade maior na fase pupal, 95,1%. Conclusão: os três larvicidas apresentaram controle semelhante; no entanto, o pyriproxifen pode deixar a falsa impressão de positividade dos criadouros, por agir em fase pupal, comprometendo os indicadores de infestação que são parâmetros estratégicos para as ações de controle.

Abstract Objective: to evaluate, under simulated field conditions, the efficacy of pyriproxyfen (juvenile hormone), novaluron (chitin inhibitor) and spinosad (biolarvicide) in controlling Aedes aegypti. Methods: periodic exposition of Ae. aegypti larvae collected in Itabuna, BA, Brazil, to recipients treated with larvicides and comparison of residual effect of treatment with the Rockefeller strain. Results: the inhibitory effect on adult emergence after 60 days was spinosad 89.5%, novaluron 96.5% and pyriproxifen 75.4% for Itabuna larvae, with no statistical difference (p=0.412) between treatments; spinosad and novaluron had a higher percentage of mortality in the larval stage, 98.8% and 97.9% respectively; pyriproxyfen showed higher mortality (95.1%) in the pupal stage. Conclusion: the three larvicides demonstrated similar control; however, pyriprofyxen might give a false impression of breeding ground positivity as it acts at the pupal stage, compromising the indicators of infestation that are strategic parameters for control actions.

The organophosphate temephos has been the main insecticide used against larvae of the dengue and yellow fever mosquito (Aedes aegypti) in Brazil since the mid-1980s. Reports of resistance date back to 1995; however, no systematic reports of widespread temephos resistance have occurred to date. As resistance investigation is paramount for strategic decision-making by health officials, our objective here was to investigate the spatial and temporal spread of temephos resistance in Ae. aegypti in Brazil for the last 12 years using discriminating temephos concentrations and the bioassay protocols of the World Health Organization. The mortality results obtained were subjected to spatial analysis for distance interpolation using semi-variance models to generate maps that depict the spread of temephos resistance in Brazil since 1999. The problem has been expanding. Since 2002-2003, approximately half the country has exhibited mosquito populations resistant to temephos. The frequency of temephos resistance and, likely, control failures, which start when the insecticide mortality level drops below 80%, has increased even further since 2004. Few parts of Brazil are able to achieve the target 80% efficacy threshold by 2010/2011, resulting in a significant risk of control failure by temephos in most of the country. The widespread resistance to temephos in Brazilian Ae. aegypti populations greatly compromise effective mosquito control efforts using this insecticide and indicates the urgent need to identify alternative insecticides aided by the preventive elimination of potential mosquito breeding sites.

CONTEXTO: As infecções do sítio cirúrgico constituem risco inerente ao ato operatório, principalmente após cirurgias digestivas. Ocorrem até 30 dias após a cirurgia, ou até 1 ano, se houver implante de prótese. O índice de risco de infecção cirúrgica (IRIC), metodologia NISS (National Nosocomial Infection Surveillance), é um método de avaliação do risco de infecções do sítio cirúrgico que considera o potencial de contaminação da cirurgia, o estado de saúde do paciente e o tempo cirúrgico. OBJETIVO: Avaliar a correlação entre a pontuação IRIC no primeiro dia de pós-operatório e o desenvolvimento da infecção do sítio cirúrgico até 30 dias de pós-operatório. Avaliou-se a infecção do sítio cirúrgico (NNIS) pós-operatória através de seguimento à internação e em ambiente de ambulatório. MÉTODOS: Foram acompanhados, prospectivamente, pacientes submetidos a cirurgias eletivas, limpas ou contaminadas, realizadas por via convencional em hospital universitário no período de junho de 2007 a agosto de 2008. A média de idade dos pacientes foi de 55,5 anos, 133 (65,5%) do sexo masculino; 120 (59,1%) submetidos a cirurgias limpas e 83 (40,9%) contaminadas. O índice global de infecções do sítio cirúrgico foi de 10,3%: 10 (8,3%) em procedimentos limpos e 11 (13,2%) em contaminados. Quatro (19,1%) das infecções do sítio cirúrgico foram diagnosticadas na internação e 17 (80,9%), no seguimento pós-alta. Doze (57,1%) das infecções do sítio cirúrgico foram superficiais: 2 (9,5%) profundas e 7 (33,3%) de sítio específico. Destas, 5 (6,6%) em pacientes classificados como IRIC 0 (76); 9 (15%) para IRIC 1 (60); 5 (9,1%) para IRIC 2 (55) e 2 (16,7%) (2) para IRIC 3. CONCLUSÃO: O índice global de infecções do sítio cirúrgico e sua incidência entre procedimentos contaminados estão dentro dos limites esperados. Avaliados pelo IRIC, os índices de infecções do sítio cirúrgico estão acima dos padrões esperados tanto para os procedimentos limpos quanto para os contaminados.

CONTEXT: Surgical site infections are a risk inherent to surgical procedures, especially after digestive surgeries. They occur up to 30 days after surgery, or up to a year later if a prosthesis is implanted. The Surgical-site Infection Risk Index (SIRI), NISS (National Nosocomial Infection Surveillance) methodology, is a method to evaluate the risk of surgical site infections, which takes into account the potential contamination of the surgery, the patient's health status and surgery duration. OBJECTIVES: To evaluate the correlation between the surgical-site infection risk index score on the 1st day postoperatively, and the development of surgical site infection up to 30 days postoperatively. METHODS: The postoperative surgical site infections (NNIS) was evaluated by following-up in hospital and as an outpatient. The patients followed prospectively were those submitted to elective surgeries, clean (hernioplasties) or contaminated (colorretal), performed by conventional approach at a university hospital, during the period from June 2007 to August 2008. The mean age of the patients was 55.5 years, 133 (65.5%) male; 120 (59.1%) submitted to clean surgeries and 83 (40.9%) contaminated. RESULTS: The global index of surgical site infections was 10.3%; 10 (8.3%) in clean procedures and 111(3.2%) in contaminated ones. Four (19.1%) of the surgical site infections were diagnosed at the time of hospitalization and 17 (80.9%) at post-discharge follow-up. Twelve (57.1%) of the surgical site infections were superficial, 2 (9.5%) deep and 7 (33.3%) at a specific site. Of these, 5 (6.6%) were in patients classified as SIRI 0 (76); 9 (15%) for SIRI 1 (60); 5 (9.1%) for SIRI 2 (55) and 2 (16.7%) for SIRI 3. CONCLUSION: The global index of surgical site infections and its incidence among contaminated procedures are within the expected limits. On the other hand according to SIRI, the surgical site infection indexes are above the expected standards both for the clean and for the contaminated procedures.
