RESUMO Introdução: As anomalias craniofaciais são identificadas usualmente pela sua aparência. Ao longo do tempo, diversas escalas e classificações foram propostas a partir de aspectos clínicos e anatômicos. Em 1976, Tessier fez uma associação entre os tecidos moles e o ósseo subjacente. Com esse conceito, criou um sistema numeral a partir da órbita em sentido horário de 0 - que chamou de linha zero, uma linha vertical da face - a 14. Rara e com múltiplas apresentações, sua condução segue um desafio mesmo para profissionais mais experientes. Relato de Caso: Paciente feminina, submetida a rinoplastia aberta estruturada aos 15 anos de idade para correção de nariz bífido, utilizando cartilagem costal e técnica de tongue-in-groove. Discussão: O nariz bífido é uma das principais apresentações da fissura 0. A rinoplastia aberta estruturada já foi aplicada em outros trabalhos com grande sucesso, sendo a técnica tongue-in-groove especialmente útil para projeção e rotação da ponta nasal. Conclusão: As anomalias craniofaciais variam em suas apresentações e cabe ao cirurgião plástico identificar os problemas e propor soluções terapêuticas que amenizem essas alterações. Seu tratamento irá necessitar de uma avaliação pré-operatória completa, planejamento cirúrgico cuidadoso e técnica cirúrgica meticulosa.

ABSTRACT Introduction: Craniofacial anomalies are usually identified by their appearance. Over time, several scales and classifications were proposed based on clinical and anatomical aspects. In 1976, Tessier made an association between soft tissue and underlying bone. With this concept, he created a numeral system starting from the clockwise orbit of 0 - which he called the zero line, a vertical line of the face - to 14. Rare and with multiple presentations, its conduction is a challenge even for more experienced professionals. Case Report: Female patient who underwent open structured rhinoplasty at the age of 15 to correct a bifid nose, using costal cartilage and the tongue-in-groove technique. Discussion: Bifid nose is one of the main presentations of cleft 0. Structured open rhinoplasty has already been successfully applied in other studies, and the tongue-in-groove technique is especially useful for the projection and rotation of the nasal tip. Conclusion: Craniofacial anomalies vary in their presentations, and it is up to the plastic surgeon to identify the problems and propose therapeutic solutions that alleviate these changes. Their treatment will require a thorough preoperative assessment, careful surgical planning, and meticulous surgical technique.

Abstract Objectives This analysis describes the protocol of a study with a case-cohort to design to prospectively evaluate the incidence of subclinical atherosclerosis and Cardiovascular Disease (CVD) in Chronic Inflammatory Disease (CID) participants compared to non-diseased ones. Methods A high-risk group for CID was defined based on data collected in all visits on self-reported medical diagnosis, use of medicines, and levels of high-sensitivity C-Reactive Protein >10 mg/L. The comparison group is the Aleatory Cohort Sample (ACS): a group with 10% of participants selected at baseline who represent the entire cohort. In both groups, specific biomarkers for DIC, markers of subclinical atherosclerosis, and CVD morbimortality will be tested using weighted Cox. Results The high-risk group (n = 2,949; aged 53.6 ± 9.2; 65.5% women) and the ACS (n=1543; 52.2±8.8; 54.1% women) were identified. Beyond being older and mostly women, participants in the high-risk group present low average income (29.1% vs. 24.8%, p < 0.0001), higher BMI (Kg/m2) (28.1 vs. 26.9, p < 0.0001), higher waist circumference (cm) (93.3 vs. 91, p < 0.0001), higher frequencies of hypertension (40.2% vs. 34.5%, p < 0.0001), diabetes (20.7% vs. 17%, p = 0.003) depression (5.8% vs. 3.9%, p = 0.007) and higher levels of GlycA a new inflammatory marker (p < 0.0001) compared to the ACS. Conclusions The high-risk group selected mostly women, older, lower-income/education, higher BMI, waist circumference, and of hypertension, diabetes, depression, and higher levels of GlycA when compared to the ACS. The strategy chosen to define the high-risk group seems adequate given that multiple sociodemographic and clinical characteristics are compatible with CID.

ABSTRACT Objective: Although some previous data have suggested a high iodine intake in Brazil, the prevalence of antithyroperoxidase antibodies (TPOAb) in the country is compatible with rates from countries with adequate iodine intake. This observation emphasizes the importance of knowing the incidence of TPOAb in Brazil. Materials and methods: This prospective analysis included euthyroid participants with negative TPOAb at baseline and a thyroid function assessment at a 4-year follow-up. TPOAb was measured by electrochemiluminescence and considered positive when titers were ≥34 IU/mL. TSH and free T4 (FT4) levels were determined by a third-generation immunoenzymatic assay. The incidence of TPOAb is expressed in percentage per year or as a cumulative incidence within the 4-year follow-up period. Results: Of 8,922 euthyroid participants (mean age 51.1 years; 50.9% women) with a negative TPOAb test at baseline, 130 presented incident TPOAb at the 4-year follow-up, yielding an annual incidence of TPOAb of 0.38%/year (95% confidence interval [95% CI], 0.37-0.39%/year) and a cumulative incidence over 4 years of 1.46% (95% CI, 1.21-1.71%). In men, the annual incidence was 0.32% (95% CI, 0.31-0.33%), and the cumulative incidence over 4 years was 1.23% (95% CI, 0.90-1.56%). In women, the annual incidence was 0.43%/year (95% CI, 0.42-0.44%/year) and the cumulative incidence over 4 years was 1.67% (95% CI, 1.30-2.04%). The only factor associated with incident TPOAb was the occurrence of thyroid diseases at follow-up. No differences in TPOAb incidence were detected across ELSA-Brasil research centers. Conclusion: Based on the results of this study, the incidence of TPOAb per year and at a 4-year follow-up period are compatible with those of a country with adequate iodine intake.

ABSTRACT Objective: To evaluate incidence of subclinical and overt hyperthyroidism and hypothyroidism. Subjects and methods: The Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) is a prospective cohort study of 15,105 civil servants, examined at baseline and over a 4-year follow-up. This analysis included 9,705 participants with normal thyroid function at baseline, follow-up information about thyroid function and with no report of using drugs that may interfere in the thyroid function. Thyroid function was defined by TSH/FT4 levels or routine use of thyroid hormones/anti-thyroid medications. Annual and cumulative (over 4-year) incidence rates were presented as percentages (95% Confidence Intervals). Results: The incidence of all overt and subclinical thyroid disease was 6.7% (1.73%/year): 0.19% for overt hyperthyroidism (0.048%/year), 0.54% for subclinical hyperthyroidism (0.14%/year), 1.98% for overt hypothyroidism (0.51%/year), and 3.99% for subclinical hypothyroidism (1.03%/year). The incidence of all thyroid diseases was higher in women, when compared to men, with a low women:men ratio (1.36). For Blacks the highest incidence was for overt hyperthyroidism, while for Whites, the highest incidence was for overt hypothyroidism. However, the highest incidence of overt hyperthyroidism was detected in Asian descendants. The presence of antithyroperoxidase antibodies at baseline was associated with higher incidence of overt thyroid diseases. Conclusion: These results showed a high incidence of hypothyroidism, which is compatible with a country with a more-than-adequate iodine intake. The low women:men ratio of the incidence of thyroid dysfunction highlights the importance of the diagnosis of thyroid diseases among men in Brazil.

ABSTRACT Introduction: Craniosynostoses are defined as premature fusions of one or more cranial sutures. They can be classified according to the sutures involved, correlated malformations and genetic alterations, and maybe syndromic or not. Its incidence is estimated at 1: 2,000-2,500 live births, with only 8% being syndromic. There are multiple surgical options for the correction of these malformations, and early referral to specialized centers for the treatment of these patients is necessary for a better therapeutic indication. The diagnosis is based on physical examination and computed tomography. The surgical treatment brings significant aesthetic and social improvements to patients. The objective is to analyze retrospectively patient data who underwent surgery to correct craniosynostosis at HC of UNESP-Botucatu. Methods: Retrospective analysis of the medical records of patients who underwent surgical correction of craniosynostosis between 2012 and 2017. Results: The prevalence of scaphocephaly and trigonocephaly was the same (38.5%). The mean age of the approach was 24 months; the mean surgical time was 3h48min, lower in scaphocephaly, 2h50min. All patients received blood transfusions in the perioperative period, with a mean percentage of 24.9% in relation to the preoperative blood volume. They underwent postoperative in a pediatric ICU. There were no deaths or complications, and the results were considered good by the team and family. Conclusion: Open surgical treatment of non-syndromic craniosynostosis is a safe procedure. Plastic surgery teams can achieve results comparable to those described in the literature, with low complication rates, good aesthetic results when performed correctly and with a multidisciplinary approach.

RESUMO Introdução: Craniossinostoses são definidas como fusões prematuras de uma ou mais suturas cranianas. Podem ser classificadas de acordo com as suturas envolvidas, malformações correlacionadas e alterações genéticas, podendo ser sindrômicas ou não. Sua incidência estimada em 1:2.000-2.500 nascidos vivos, sendo apenas 8% sindrômicas. Existem múltiplas opções cirúrgicas para a correção destas malformações, e o encaminhamento precoce para centros especializados no tratamento destes doentes é necessário para uma melhor indicação terapêutica. O diagnóstico é pautado no exame físico e tomografia computadorizada. O tratamento cirúrgico traz melhoras significativas estéticas e no convívio social dos pacientes. O objetivo é analisar os dados, retrospectivamente, dos pacientes submetidos à cirurgia para correção de craniossinostoses, no HC da UNESP-Botucatu. Métodos: Análise retrospectiva dos prontuários de pacientes submetidos à correção cirúrgica de craniossinostoses entre 2012 e 2017. Resultados: A prevalência de escafocefalia e trigonocefalia foi igual (38,5%), idade média de abordagem de 24 meses, o tempo cirúrgico médio foi de 3h48min, sendo menor nas escafocefalias, 2h50min. Todos os pacientes receberam transfusão de sangue no perioperatório com média percentual em relação ao volume sanguíneo pré-operatório de 24,9% e fizeram pós-operatório em UTI pediátrica. Não houve óbitos ou complicações, e os resultados tidos como bom pela equipe e familiares. Conclusão: O tratamento cirúrgico aberto das craniossinostoses não sindrômicas é um procedimento seguro. As equipes de cirurgia plástica conseguem alcançar resultados equiparáveis aos descritos na literatura, com baixas taxas de complicações, bom resultado estético, quando executado de maneira adequada, e com abordagem multidisciplinar.

O objetivo foi avaliar o potencial odontogênico de células indiferenciadas da polpa (OD-21) por meio de indução química in vitro. As células foram divididas em grupos: controle (OD-21), induzido (OD-21 em meio suplementado/OD-21 + OM), e células odontoblastóides (MDPC-23). Após 3, 7, 10 e 14 dias, avaliou-se proliferação e viabilidade celular, proteína total e fosfatase alcalina (ALP), mineralização, imunolocalização da proteína da matriz dentinária 1 (DMP1), ALP e osteopontina (OPN), assim como a expressão dos genes ALP, OSTERIX (Osx), DMP1 e fator de transcrição RUNX2 por PCR em tempo real. Os dados foram avaliados pelo teste de Kruskal-Wallis seguido pelo teste de Mann-Whitney U (p<0.05). Houve diminuição na proliferação celular em OD-21 + OM, com viabilidade celular similar em todos os grupos, exceto aos sete dias. O conteúdo de proteína total foi maior no grupo OD-21 + OM em todos os períodos; o mesmo ocorreu com a atividade de ALP quando comparada com o grupo OD-21, além de apresentar resultados similares ao grupo MDPC-23. A mineralização foi maior em OD-21 + OM quando comparada com o controle negativo. A imunolocalização demonstrou expressão de DMP1 e ALP em MDPC-23 e OD-21 + OM, enquanto que todos os grupos foram positivos para OPN. A expressão gênica de DMP1 e ALP foi maior nas culturas de MDPC-23, enquanto que a de RUNX2 foi menor para estas células e maior no controle negativo. A expressão de OSTERIX foi menor em OD-21 + OM quando comparada aos outros grupos. Sugere-se que as células indiferenciadas da polpa da linhagem OD-21 apresentam potencial odontogênico e poderiam ser usadas para a engenharia tecidual

The aim of this study was to evaluate the odontogenic potential of undifferentiated pulp cells (OD-21 cell line) through chemical stimuli in vitro. Cells were divided into uninduced cells (OD-21), induced cells (OD-21 cultured in supplemented medium/OD-21+OM) and odontoblast-like cells (MDPC-23 cell line). After 3, 7, 10 and 14 days of culture, it was evaluated: proliferation and cell viability, alkaline phosphatase activity, total protein content, mineralization, immunolocalization of dentin matrix acidic phosphoprotein 1 (DMP1), alkaline phosphatase (ALP) and osteopontin (OPN) and quantification of genes ALP, OSTERIX (Osx), DMP1 and runt-related transcription factor 2 (RUNX2) through real-time polymerase chain reaction (PCR). Data were analyzed by Kruskal-Wallis and Mann-Whitney U tests (p<0.05). There was a decrease in cell proliferation in OD-21 + OM, whereas cell viability was similar in all groups, except at 7 days. The amount of total protein was higher in group OD-21 + OM in all periods; the same occurred with ALP activity after 10 days when compared with OD-21, with no significant differences from the MDPC-23 group. Mineralization was higher in OD-21+OM when compared with the negative control. Immunolocalization demonstrated that DMP1 and ALP were highly expressed in MDPC-23 cells and OD-21 + OM cells, whereas OPN was high in all groups. Real-time PCR revealed that DMP1 and ALP expression was higher in MDPC-23 cell cultures, whereas RUNX2 was lower for these cells and higher for OD-21 negative control. Osx expression was lower for OD-21 + OM. These results suggest that OD-21 undifferentiated pulp cells have odontogenic potential and could be used in dental tissue engineering.

Symptomatic prostatic paracoccidioidomycosis (PCM) is a very rare condition; however, it may express as a typical benign prostatic hyperplasia or a simulating prostatic adenocarcinoma. This case report presents PCM mimicking prostatic adenocarcinoma. The purpose of this paper is to call the general physician's attention to this important differential diagnosis.

São apresentadas as características clínicas e evolutivas de sete pacientes (cinco masculinos e dois do sexo feminino), a maioria dos quais usuários de drogas ilícitas endovenosa, com paracoccidioidomicose associada à Síndrome da Imunodeficiência Adquirida (SIDA/AIDS). Em quatro pacientes a paracoccidioidomicose comprometia os pulmões isoladamente, nos demais a doença era generalizada com envolvimento cutâneo. Apenas dois pacientes eram procedentes recentes da zona rural. O que nos faz presumir que nos demais a paracoccidioidomicose doença resultou da reativação de focos latentes da infecção. Dado o papel da imunidade medida por células na defesa do hospedeiro contra o Paracoccidioides brasiliensis, é de se prever crescente ocorrência da associação paracoccidioidomicose -SIDA/AIDS nas áreas endêmicas para ambas as enfermidades.

We report the clinical findings and evolution of seven patients (five men and two women), the majority of them intravenous drug users, with paracoccidioidomycosis associated to acquired immunodeficiency syndrome (AIDS). In four of the patients the paracoccidioidomycosis was restricted to the lung and in the three others was generalized with cutaneous involvement. Only two of them had lived recently in rural area, an indication of the possible reactivation of latent focal infection in the other five patients. The recognition of the role of cell-mediated immunity in host defense against Paracoccidioides brasiliensis leds to the prediction of a growing occurrence of the paracoccidioidomycosis-AIDS association in areas that are endemic for these diseases.
