OBJETIVOS: o presente estudo teve o objetivo de avaliar as alterações dentárias e periodontais decorrentes da Expansão Rápida da Maxila Assistida Cirurgicamente (ERMAC). MÉTODOS: foram obtidos os modelos de gesso de 18 pacientes (média de idade de 23,3 anos), ao início (T1), 3 meses após a ERMAC (T2) e 6 meses após a expansão (T3). Os modelos foram digitalizados (Scanner 3D 3Shape D-250) e mensuraram-se as distâncias transversais, bem como a inclinação e a altura da coroa clínica dos dentes posteriores. Para análise dos resultados, aplicou-se a análise de Variância e o teste de Tukey. RESULTADOS: nas distâncias transversais, observou-se um aumento de T1 para T2 e uma manutenção de T2 para T3. As inclinações dentárias demonstraram diferenças estatisticamente significativas em alguns dentes; porém, numericamente tenderam a um aumento de T1 para T2 e a uma diminuição de T2 para T3. Não se observou diferença estatisticamente significativa na altura da coroa clínica, exceto nos primeiros e segundos molares do lado esquerdo, porém, clinicamente irrelevante. CONCLUSÕES: a ERMAC demonstrou ser um procedimento efetivo e estável, com mínima repercussão periodontal.

OBJECTIVE: The present study assessed the maxillary dental arch changes produced by surgically assisted rapid maxillary expansion (SARME). METHODS: Dental casts from 18 patients (mean age of 23.3 years) were obtained at treatment onset (T1), three months after SARME (T2) and 6 months after expansion (T3). The casts were scanned in a 3D scanner (D-250, 3Shape, Copenhagen, Denmark). Maxillary dental arch width, dental crown tipping and height were measured and assessed by ANOVA and Tukey's test. RESULTS: Increased transversal widths from T1 and T2 and the maintenance of these values from T2 and T3 were observed. Buccal teeth tipping also showed statistically significant differences, with an increase in all teeth from T1 to T2 and a decrease from T2 to T3. No statistically significant difference was found for dental crown height, except for left first and second molars, although clinically irrelevant. CONCLUSION: SARME proved to be an effective and stable procedure, with minimum periodontal hazards.

INTRODUÇÃO: a principal limitação do tratamento interceptivo das más oclusões de Classe III está no prognóstico. Os procedimentos interceptivos de expansão rápida da maxila e de tração reversa, adotados ao início da dentição mista, são capazes de propiciar sobrecorreção imediata e manutenção da morfologia facial e oclusal por alguns anos. Pacientes que, ao final do crescimento, apresentam, no mínimo, faces aceitáveis, são candidatos ao tratamento ortodôntico compensatório, ao passo que aqueles com comprometimento facial deveriam ser submetidos a tratamento ortodôntico descompensatório para cirurgia ortognática. OBJETIVO: investigar variáveis cefalométricas preditoras dos resultados do tratamento ortopédico com expansão rápida da maxila e tração reversa (ERM e TM). MÉTODOS: uma avaliação cefalométrica foi aplicada, ao início do tratamento, em 26 crianças com má oclusão de Classe III (média de idade de 8 anos e 4 meses). Após um período médio de 6 anos e 10 meses, ao final do crescimento pubertário. sob o crivo de uma análise facial subjetiva, foram constituídos dois grupos, sendo um grupo de sucesso (21 pacientes) e um grupo de insucesso (5 pacientes). Análise discriminante foi aplicada aos valores cefalométricos ao início do tratamento, por meio do procedimento stepwise, assim, identificamos duas variáveis preditoras. RESULTADOS: o tratamento ortopédico de uma má oclusão Classe III pode ter prognóstico desfavorável ao final do crescimento quando, nos registros cefalométricos iniciais, for observada uma altura facial anteroinferior aumentada (AFAI) associada a uma diminuição do ângulo entre o eixo condilar e o plano mandibular (CondAx.PM). CONCLUSÃO: os resultados para o final de crescimento de um tratamento com ERM e TM, para cada novo paciente com má oclusão de Classe III, poderiam ser previstos com uma probabilidade de acerto de 88,5%.

INTRODUCTION: Prognosis is the main limitation of interceptive treatment of Class III malocclusions. The interceptive procedures of rapid maxillary expansion (RME) and face mask therapy performed in early mixed dentition are capable of achieving immediate overcorrection and maintenance of facial and occlusal morphology for a few years. Individuals presenting minimal acceptable faces at growth completion are potential candidates for compensatory orthodontic treatment, while those with facial involvement should be submitted to orthodontic decompensation for orthognathic surgery. OBJECTIVES: To investigate cephalometric variables that might predict the outcomes of orthopedic treatment with RME and face mask therapy (FM). METHODS: Cephalometric analysis of 26 Class III patients (mean age of 8 years and 4 months) was performed at treatment onset and after a mean period of 6 years and 10 months at pubertal growth completion, including a subjective facial analysis. Patients was divided into two groups: success group (21 individuals) and failure group (5 individuals). Discriminant analysis was applied to the cephalometric values at treatment onset. Two predictor variables were found by stepwise procedure. RESULTS: Orthopedic treatment of Class III malocclusion may have unfavorable prognosis at growth completion whenever initial cephalometric analysis reveals increased lower anterior facial height (LAFH) combined with reduced angle between the condylar axis and the mandibular plane (CondAx.MP). CONCLUSION: The results of treatment with RME and face mask therapy at growth completion in Class III patients could be predicted with a probability of 88.5%.

INTRODUÇÃO: esse estudo foi realizado com o propósito de adaptar os métodos para avaliação da maturação óssea por meio das vértebras cervicais na curva do Surto de Crescimento Puberal (SCP)de Martins e Sakima, assim como testar a confiabilidade e a concordância existente entre eles e o método para a radiografia de mão e punho, quando comparados 2 a 2, e entre todos, conjuntamente. MÉTODOS: a amostra constou de 72 radiografias, sendo 36 telerradiografias em norma lateral da cabeça e 36 radiografias de mão e punho, de 36 indivíduos com Síndrome de Down (SD), sendo 13 do sexo feminino e 23 do masculino, na faixa etária entre oito anos e seis meses até 18 anos e sete meses, com média de 13 anos e dez meses. CONCLUSÃO: de acordo com os resultados obtidos, concluímos que as adaptações dos métodos para a avaliação da maturação óssea por meio das vértebras cervicais na curva de SPC de Martins e Sakima resultaram em uma ferramenta prática e útil na determinação do estágio de crescimento e desenvolvimento dos indivíduos. Os estágios de maturação avaliados por meio das vértebras cervicais e os centros de ossificação observados nas radiografias de mão e punho foram considerados confiáveis, com excelente grau de concordância entre os métodos de diversos outros autores, e uma concordância de razoável a boa entre os três métodos para avaliação da maturação óssea quando comparados conjuntamente, sendo estatisticamente significativa.

INTRODUCTION: This study was conducted with the aim of adapting the methods developed by Martins and Sakima to assess skeletal maturation by cervical vertebrae in the pubertal growth spurt (PGS) curve. It also aimed to test the reliability and agreement between those methods and the method of hand and wrist radiograph when compared two by two and all together. METHODS: The sample comprised 72 radiographs, with 36 lateral radiographs of the head and 36 hand-wrist radiographs of 36 subjects with Down's syndrome (DS), 13 female and 23 male, aged between 8 years and 6 months and 18 years and 7 months, with an average age of 13 years and 10 months. RESULTS AND CONCLUSIONS: Results revealed that adapting the methods developed by Martins and Sakima to assess skeletal maturation by cervical vertebrae in the curve of PGS is practical and useful in determining the stage of growth and development of individuals. The stages of maturation evaluated by cervical vertebrae and ossification centers observed in radiographs of the hand and wrist were considered reliable, with excellent level of agreement between the methods by Hassel and Farman as well as Baccetti, Franchi and McNamara Jr and Martins and Sakima. Additionally, results revealed an agreement that ranged between reasonable to good for the three methods used to assess the skeletal maturation, showing statistical significance.

INTRODUÇÃO: a Ortodontia passa, como toda ciência, por constantes evoluções tecnológicas que buscam aumentar a efetividade da abordagem terapêutica, visando a diminuição do tempo de tratamento, o aumento do conforto para os pacientes, bem como a obtenção da tão almejada, e pouco alcançada, estabilidade em longo prazo. O estágio atual de desenvolvimento tecnológico da Ortodontia representa, ao que tudo indica, uma fase de transição entre os sistemas convencionais de ligação (com módulos elásticos) e os chamados autoligáveis. As evidências científicas nem sempre consubstanciam a clara percepção clínica das vantagens desse sistema, no que diz respeito a um menor tempo de alinhamento e nivelamento, uma relativa simplificação técnica, maior conforto para os pacientes, além do aumento da capacidade de tratamento sem extrações - embora essa indicação esteja mais ligada à avaliação do padrão morfológico facial, e menos a qualquer escolha técnica. Desde um passado recente e não menos brilhante, a Ortodontia vem utilizando a individualização de braquetes para tratamentos compensatórios, buscando aumentar a efetividade da abordagem terapêutica, com menores custos biológicos e menor tempo de tratamento. OBJETIVO: o presente artigo tem como objetivo apresentar um protocolo bem definido de melhor aproveitamento dessa fase de transição tecnológica, buscando explorar o que cada sistema tem de melhor, principalmente sob a óptica da redução do tempo de tratamento e aumento da capacidade de movimentação dentária compensatória em pacientes adultos. Especificamente, serão abordadas as más oclusões de Classe III compensáveis, usando o sistema de braquetes autoligáveis onde se deseja maior capacidade de movimento expansivo e protrusivo (arcada superior) e braquetes convencionais Prescrição III Capelozza(r) onde a manutenção da forma com mínima mudança (arcada inferior) é imprescindível para a obtenção dos resultados almejados.

INTRODUCTION: Orthodontics, just as any other science, has undergone advances in technology that aim at improving treatment efficacy with a view to reducing treatment time, providing patients with comfort, and achieving the expected, yet hardly attained long-term stability. The current advances in orthodontic technology seem to represent a period of transition between conventional brackets (with elastic ligatures) and self-ligating brackets systems. Scientific evidence does not always confirm the clear clinical advantages of the self-ligating system, particularly with regard to reduced time required for alignment and leveling (a relatively simple protocol), greater comfort for patients, and higher chances of performing treatment without extractions - even though the number of extractions is more closely related to patient's facial morphological pattern, regardless of the technique of choice. Orthodontics has recently and brilliantly used bracket individualization in compensatory treatment with a view to improving treatment efficacy with lower biological costs and reduced treatment time. OBJECTIVE: This paper aims at presenting a well-defined protocol employed to produce a better treatment performance during this period of technological transition. It explores the advantages of each system, particularly with regards to reduced treatment time and increased compensatory tooth movement in adult patients. It particularly addresses compensable Class III malocclusions, comparing the system of self-ligating brackets, with which greater expansive and protrusive tooth movement (maxillary arch) is expected, with conventional brackets Capelozza Prescription III, with which maintaining the original form of the arch (mandibular arch) with as little changes as possible is key to yield the desired results.

O objetivo deste estudo foi avaliar se a inclinação da cabeça para cima ou para baixo interfere na determinação do estágio de crescimento por meio do Índice de maturação das vértebras cervicais (IMVC), verificando a acurácia deste método de diagnóstico, quando as radiografias são tomadas com erros. Quarenta e nove pacientes, 26 do gênero feminino e 23 do masculino, entre 9 e 15 anos de idade, foram submetidos a 3 telerradiografias em norma lateral: posição natural de cabeça (PNC), cabeça inclinada 15° para cima (PNC-alta), e 15° para baixo (PNC-baixa). Três examinadores avaliaram o IMVC nas 147 telerradiografias. A concordância entre os examinadores foi alta na avaliação das telerradiografias obtidas em PNC. O teste Kappa revelou concordância moderada a substancial entre PNC e PNC-alta e entre PNC e PNC-baixa. Houve concordância significante entre PNC-alta e PNC-baixa. Pode-se concluir que a avaliação do IMVC em telerradiografias obtidas em PNC difere em comparação com as radiografias tomadas com inclinações. Tanto PNC-alta quanto PNC-baixa demonstraram maior discordância na interpretação entre os examinadores, uma vez que o método de avaliação não foi preconizado para telerradiografias com erros de posicionamento.

The aim of this study was to evaluate if upward or downward head inclination might interfere with determination of the growth stage, using cervical vertebrae maturation index (CVMI), in order to verify the accuracy of such diagnosis when radiographs are taken with errors. Forty-nine patients, 26 females and 23 males, aged 9 to 15 years, were submitted to 3 lateral cephalograms: normal (NHP), with 15° upward head inclination (NHP-Up), and with 15° downward head inclination (NHP-Down). Three examiners evaluated the CVMI on the 147 cephalograms. The agreement among examiners was higher in the evaluation of cephalograms taken in NHP. The weighted Kappa test revealed moderate to substantial agreement between NHP and NHP-Up and between NHP and NHP-Down. There was greater agreement between NHP-Up and NHP-Down. It may be concluded that the evaluation of the CVMI on cephalograms in NHP is different as compared with radiographs taken with inclinations. Both NHP-Up and NHP-Down exhibited greater disagreement in the interpretation among examiners, since the evaluation method was not designed for cephalograms with positioning errors.

OBJETIVO: esse estudo retrospectivo teve como objetivo avaliar, cefalometricamente, os efeitos esqueléticos, dentários e tegumentares decorrentes do tratamento com o Aparelho de Protração Mandibular em conjunto ao aparelho fixo em pacientes adultos para correção da má oclusão de Classe II. MÉTODOS: a amostra foi composta por telerradiografias pré- e pós-tratamento de nove adultos (idade inicial média de 22,48 anos), portadores de má oclusão de Classe II, divisão 1 bilateral. O teste t pareado (p < 0,05) foi empregado para comparação dos valores iniciais e finais. RESULTADOS: de acordo com o teste t, observou-se aumento da altura facial anteroinferior e da altura facial posterior. As alterações dentárias foram: extrusão dos incisivos superiores, inclinação para vestibular e protrusão dos incisivos inferiores; e mesialização e extrusão dos molares inferiores. Com relação ao componente tegumentar, houve aumento do ângulo nasolabial e retrusão do lábio superior. CONCLUSÃO: os efeitos do tratamento com o Aparelho de Protração Mandibular em conjunto com aparelho fixo em pacientes adultos para correção da má oclusão de Classe II foram direcionados, principalmente, à arcada inferior, com inclinação vestibular, protrusão e intrusão dos incisivos e mesialização e extrusão dos molares.

OBJECTIVE: This retrospective study aimed to conduct a cephalometric evaluation of the skeletal, dental and soft tissue effects resulting from treatment of adult patients presenting Class II malocclusion, performed with a Mandibular Protraction Appliance (MPA) combined with a fixed orthodontic appliance. METHODS: The sample consisted of teleradiographs obtained before and after treatment of 9 adult patients (initial mean age of 22.48 years) with bilateral Class II, division 1, malocclusion. Paired t test (p < 0.05) was applied to compare initial and final values. RESULTS: t test revealed an increase in anteroinferior facial height and posterior facial height. The dental changes include: extrusion of upper incisors, buccal inclination, protrusion of lower incisors, mesialization and extrusion of mandibular molars. Regarding the soft tissue component, there was an increase in nasolabial angle in addition to upper lip retrusion. CONCLUSIONS: The effects of treating Class II malocclusion adult patients, by means of using Mandibular Protraction Appliance (MPA) combined with a fixed appliance were mostly observed in the mandibular arch, and consisted of buccal inclination, protrusion and intrusion of incisors, and mesialization and extrusion of the molars.

INTRODUÇÃO: esse estudo avaliou as alterações das características do sorriso de pacientes com atresia maxilar submetidos à expansão rápida da maxila (ERM). MÉTODOS: a amostra consistiu de 81 fotografias extrabucais do sorriso máximo de 27 pacientes, com idade média de 10 anos, antes da expansão e aos três e seis meses após a fixação do parafuso expansor. As análises das fotografias foram realizadas por meio do programa Cef X 2001, e as seguintes medidas foram analisadas: dimensão transversal do sorriso, corredores bucais, quantidade de exposição dos incisivos superiores, exposição gengival dos incisivos superiores, altura do sorriso, espessuras dos lábios superior e inferior, simetria e arco do sorriso. As alterações no sorriso durante as diferentes fases foram avaliadas por meio da análise de variância (ANOVA), seguida pelo teste de comparações múltiplas de Bonferroni, com nível de significância de 5%. RESULTADOS: a ERM promoveu aumento estatisticamente significativo da dimensão transversal do sorriso e da quantidade de exposição dos incisivos centrais e laterais superiores; manutenção da simetria entre os lados direito e esquerdo e da falta de paralelismo entre a curvatura das bordas dos incisivos superiores e a do lábio inferior. CONCLUSÃO: a ERM beneficiou a estética do sorriso com o aumento da dimensão transversal do sorriso e da quantidade de exposição dos incisivos centrais e laterais superiores.

INTRODUCTION: This study evaluated changes in the smile characteristics of patients with maxillary constriction submitted to rapid maxillary expansion (RME). METHODS: The sample consisted of 81 extraoral photographs of maximum smile of 27 patients with mean age of 10 years, before expansion and 3 and 6 months after fixation of the expanding screw. The photographs were analyzed on the software Cef X 2001, with achievement of the following measurements: Transverse smile area, buccal corridors, exposure of maxillary incisors, gingival exposure of maxillary incisors, smile height, upper and lower lip thickness, smile symmetry and smile arch. Statistical analysis was performed by analysis of variance (ANOVA), at a significance level of 5%. RESULTS: RME promoted statistically significant increase in the transverse smile dimension and exposure of maxillary central and lateral incisors; maintenance of right and left side smile symmetry and of the lack of parallelism between the curvature of the maxillary incisal edges and lower lip border. CONCLUSIONS: RME was beneficial for the smile esthetics with the increase of the transverse smile dimension and exposure of maxillary central and lateral incisors.

INTRODUÇÃO: a tração de dentes não irrompidos sempre foi considerada um procedimento de risco na prática ortodôntica. Com essa perspectiva, é indispensável a busca por eficiência nos procedimentos adotados para esse mister, o que justifica a proposta do presente artigo. Ao perfurar, vazamos uma estrutura natural, cuja restauração com material artificial é uma ação que está no escopo da área de maior evolução da Odontologia, não havendo justificativa para se evocar procedimentos de maior risco, como a colagem ou laçada com fios. OBJETIVO: este artigo objetiva apresentar protocolos para a Perfuração do Esmalte para o Tracionamento de Caninos (PETC), especificamente os caninos superiores, mais acometidos pelas anomalias de posicionamento, também denominadas disgenesias. Serão abordadas as vantagens e desvantagens da PETC em relação à Colagem de Acessório para o Tracionamento de Caninos (CATC), apresentando-se casos clínicos de diferentes níveis de complexidade, tendo como ponto de partida a literatura e a experiência clínica de 30 anos com elevado índice de sucesso.

INTRODUCTION: The management of unerupted teeth has always been considered as a challenging procedure in orthodontic practice. Within this perspective, the search for effectiveness in the adopted procedures for the management of unerupted teeth is essential, which explains the purpose of the present paper. When perforation is performed, a natural structure is transfixed, which may be restored with composite material and may dispense from risky procedures such as bonding attachment technique and lasso wire technique. OBJECTIVE: The present paper aims to present protocols for enamel perforation for canine traction (EPCT), specifically for maxillary canines, the most frequent teeth showing tooth position anomalies. In this paper, clinical cases with different degrees of complexities were illustrated, and, based upon the literature review and the 30 years of expertise with high rate of clinical success, advantages and disadvantages are discussed comparing EPCT and bonding attachment technique for canine traction (BACT).

OBJETIVO: avaliar a eficiência do laser diodo, infravermelho, na redução da dor no período pós-ativação da retração de caninos com molas fechadas de NiTi. MÉTODOS: doze pacientes que necessitavam de retração de caninos foram selecionados. Os caninos foram retraídos por meio de molas fechadas de NiTi, com força de 150g/lado. Um canino de cada paciente foi selecionado aleatoriamente para ser irradiado com laser, imediatamente após as ativações e depois de 3 e 7 dias. Os caninos homólogos foram utilizados como grupo controle e foi realizada somente a simulação de aplicação do laser. O laser irradiado foi o de diodo (ArGaAl), a um comprimento de onda de 780nm e uma potência de 20mW, densidade de energia na superfície do tecido alvo de 5J/cm², durante 10s por ponto, resultando numa energia de 0,2J por ponto e energia total (Et) de 2J. Para avaliação do efeito analgésico, foi utilizada a escala visual análoga (VAS), na qual os pacientes marcavam de 0 a 10, em consonância com a dor experimentada nos tempos de 12, 24, 48 e 72 horas pós ativação das molas e aplicação do laser.Todo o procedimento foi novamente realizado um mês depois, no momento da reativação da retração dos caninos. RESULTADOS E CONCLUSÕES: não houve diferença estatisticamente significativa entre os lados irradiado (GL) e controle (GC). Portanto, o laser de diodo infravermelho (780nm), no protocolo de aplicação utilizado, não foi eficiente em termos estatísticos para a diminuição da sensibilidade dolorosa provocada pela movimentação ortodôntica.

OBJECTIVE: To evaluate the efficiency of infrared diode laser on pain reduction during canine initial retraction. METHODS: Twelve patients in need of canine retraction were selected. The canines were retracted with closed NiTi coil springs activated to 150g per side. One canine of each patient was randomly selected for laser irradiation immediately after activation and after 3 and 7 days. The contralateral canines were taken as control group and were submitted only to simulation of laser application. Diode laser (ArGaAl) was employed at wavelength of 780nm, power of 20mW and energy density in the target tissue of 5J/cm², for 10 seconds per point, delivering an energy of 0.2J per point and total energy (TE) of 2J. The analgesic effect was evaluated with aid of a visual analogue scale (VAS), on which the patients indicated 0 to 10 according to the pain experienced at 12, 24, 48 and 72 hours after coil spring activation and laser application. The procedure was repeated after one month, upon reactivation of canine retraction. RESULTS AND CONCLUSIONS: There was no statistically significant difference between irradiated side (LG) and control side (CG). Thus, utilization of infrared diode laser (780nm) according to the present protocol was not statistically effective to reduce pain sensitivity caused by orthodontic movement.

The aim of this study was to evaluate the shear bond strength of brackets bonded with different restorative systems and compare it with that afforded by an established orthodontic bonding system. Seventy human bicuspids were used, divided into five different groups with 14 teeth each. Whereas a specific orthodontic bonding resin (TransbondTM XT) was used in the control group, the restorative systems Charisma, Tetric Ceram, TPH Spectrum and Z100 were used in the other four groups. Seven days after bonding the brackets to the samples, shear forces were applied under pressure in a universal testing machine. The data collected was evaluated using the ANOVA test and, when a difference was identified, the Tukey test was applied. A 5% level of significance was adopted. The mean results of the shear bond strength tests were as follows: Group 1 (Charisma), 14.98 MPa; Group 2 (Tetric Ceram), 15.16 MPa; Group 3 (TPH), 17.70 MPa; Group 4 (Z100), 13.91 MPa; and Group 5 or control group (TransbondTM XT), 17.15 MPa. No statistically significant difference was found among the groups. It was concluded that all tested resins have sufficient bond strength to be recommended for bonding orthodontic brackets.

Effective orthodontic anchorage may be obtained by miniimplants inserted into the maxillary bones. However, the risk of miniimplant failure is one of the most important issues, especially the rupture of its structure referred to as fracture, mainly due to metal deficiencies. This study analyzed the internal microstructure of orthodontic mini-implants, ascertaining the composition of the metal to detect possible discontinuities from the surface to the core of the screws. Eighteen samples of mini-implants, of 3 different brands, were obtained. The samples were cold-embedded in methyl methacrylate polymer, and were sectioned both longitudinally (3 samples of each brand) and transversely (the other 3 screws of each brand). After preparation, the samples were observed using a light microscope at up to 2,000 x magnification. The results showed that the mini-implants thus analyzed were composed of an Alpha-Beta globular phase of titanium alloy, patterns A1 and A9 (in accordance with the "Technical Committee of European Titanium Producers"). The miniimplants did not present any defects such as bubbles, imperfections or fissures, in either longitudinal or transverse sections, in their internal microstructure. All samples met the requirements of international norms. Orthodontists must be aware of the metal composition and internal microstructure of mini-implants, to decrease the risk of fractures.

OBJETIVO: avaliar as possíveis alterações e a estabilidade dentária e esquelética no sentido transversal, bem como as possíveis alterações verticais da face (AFAI), produzidas pela Expansão Rápida da Maxila Assistida Cirurgicamente (ERMAC). MÉTODOS: a amostra selecionada para este estudo retrospectivo foi composta por 60 telerradiografias em norma frontal, de 15 pacientes, sendo 6 do gênero masculino e 9 do gênero feminino, com média de idades de 23 anos e 3 meses. O disjuntor Hyrax foi instalado e o procedimento cirúrgico adotado envolveu a separação da sutura palatina mediana e não-abordagem da sutura pterigomaxilar. A ativação foi realizada do terceiro dias após a cirurgia até o término da expansão, determinada por critérios clínicos. Todos os pacientes foram radiografados nas fases pré-expansão (T1); pós-expansão imediata (T2); 3 meses pós-expansão, com o próprio disjuntor como contenção (T3); e 6 meses pós-expansão, com a placa removível de acrílico como contenção (T4). Medidas lineares foram obtidas a partir dos traçados cefalométricos gerados por um programa computadorizado (Radiocef Studio 2) e analisadas estatisticamente pelos testes de variância (ANOVA) e Tukey ao nível de 5% de significância. RESULTADOS E CONCLUSÕES: concluiu-se que a ERMAC produziu aumentos estatisticamente significativos da cavidade nasal, da largura maxilar e da distância intermolares superiores, de T1 para T2, os quais se mantiveram em T3 e T4. A largura facial e as distâncias intermolares inferiores não apresentaram alterações após a ERMAC. Avaliando o comportamento vertical da face, notou-se um aumento da AFAI nas fases T1 para T2, que diminuiu após a contenção de 3 meses (T3) e permaneceu estável em T4, embora aumentada se comparada com T1.

AIM: This study evaluated the stability of the dental and skeletal changes produced by the Surgically Assisted Rapid Maxillary Expansion (SARME) in transversal and vertical dimensions. METHODS: The sample selected for this retrospective study was comprised by 60 posteroanterior cephalograms, of 15 patients (6 males and 9 females), with mean age of 23.3 years. An Hyrax appliance was used and the surgical procedure was characterized by the midsagittal osteotomy and non approach of pterygopalatine suture. The beginning of activation occurred in the third postoperative day, being that the limits for the expansion were determined by eminently clinical criteria. All the patients were radiographed in the following phases: pre-expansion (T1), immediate post-expansion (T2), three months post-expansion (with the Hyrax appliance as retention) (T3), and six months post-expansion (with the acrylic removable plate as retention) (T4). Linear measurements were obtained from the cephalograms tracings generated by a computerized program (Radiocef Studio 2) and statistically analyzed by the variance test (ANOVA) and Tukey at the 5% level of significance. RESULTS AND CONCLUSION: It was concluded that SARME produced a statistically significant increase of nasal cavity, maxillary width, upper intermolars distance, from T1 to T2, which were maintained in T3 and T4. The facial width and the lower intermolars distance did not presented changes after the SARME. Evaluating the vertical behavior of the face, it was observed an increase of anterior inferior facial height in phases T1 to T2 that decreased after three months of retention (T3) and maintained stable in T4, although increased if compared with T1.

INTRODUÇÃO: o posicionamento da maxila no esqueleto craniofacial tem sido motivo de investigação por diversos autores. Traduzindo suas idéias por meio de medidas lineares ou angulares, tais autores definiram o que consideraram como a posição "ideal", "normal" e, ainda, "aceitável" ou "inaceitável" da maxila, relacionando-a, na maioria das vezes, com a base do crânio. A partir da avaliação de indivíduos com oclusão considerada normal e com bom equilíbrio facial, eram calculados valores médios e desvios-padrão de determinadas medidas, os quais eram tomados como parâmetros para avaliações cefalométricas de pacientes distintos. OBJETIVOS: diante das divergências de opiniões encontradas na literatura, a proposta do presente estudo foi avaliar o posicionamento da maxila nos sentidos vertical e ântero-posterior, além da sua inclinação, numa amostra de 94 indivíduos com oclusão normal e equilíbrio facial. MÉTODOS: foram determinadas correlações entre medidas do próprio indivíduo: OPI-N com OPI-ENA e N-ENA com ENA-ENP. CONCLUSÕES: a partir dos fortes índices de correlação encontrados, concluiu-se que a medida OPI-N pode ser tomada como referência para determinação de OPI-ENA, da mesma forma que ENA-ENP pode ser considerada para determinação de N-ENA, definindo, respectivamente, a posição da maxila nos sentidos sagital e vertical. A inclinação da maxila, representada aqui pelo ângulo OPI.ENA.ENP, teve valor médio estatisticamente próximo a 0º (zero grau), indicando forte tendência do prolongamento posterior do plano palatino (ENA-ENP) tangenciar a base posterior do crânio (ponto OPI), o que se revela como uma importante característica de indivíduos com oclusão normal e equilíbrio facial.

INTRODUCTION: The position of the maxilla in the skull is subject of investigation by many authors. Transmitting their ideas trough linear and angular measurements, they defined what is considered as "ideal", "normal" and "acceptable" or "unacceptable" position of the maxilla, relating it in most of the time with the cranial base. By the study of subjects with considered "normal" occlusion and good facial balance, it was calculated mean values and standard deviations of some measurements, which were considered as parameters to evaluate all kinds of malocclusions. AIM: Considering the divergences found in the literature, the aim of the present study was to evaluate the vertical and horizontal position of the maxilla and its inclination, in 94 subjects with normal occlusion and good facial balance. METHODS: Considering this, it was determined some correlations between measurements of the own subjects: OPI-N with OPI-ENA and N-ENA with ENA-ENP. CONCLUSIONS: By the strong index of correlation found between these measurements, it was concluded that OPI-N can be take as a reference to establishment of OPI-ENA. In addition, ENA-ENP can be considered a reference to the distance N-ENA, so it is possible to determine respectively the position of the maxilla on the horizontal and vertical planes. The inclination of the maxilla, defined by the angle OPI.ENA.ENP, showed a mean value statistically very close of 0º (zero), indicating a strong tendency of the posterior extension of the palatal plane (ENA-ENP) to touch the most posterior and inferior region of the occipital bone (OPI point). This was considered an important characteristic of subjects with normal occlusion and good facial balance.

INTRODUÇÃO: dentro da ciência Bioprogressiva, o arco utilidade, juntamente com o quadri-hélice, é o dispositivo mais conhecido e utilizado para o tratamento de inúmeras más oclusões. Com um desenho ao mesmo tempo simples e original, o arco utilidade tornou-se parte integrante do arsenal ortodôntico utilizado nos períodos das dentaduras mista e permanente. O nome do arco é uma referência à grande versatilidade que este dispositivo oferece, e devido aos vários efeitos que o mesmo pode promover durante o tratamento. OBJETIVO: este artigo destina-se a discutir a fabricação, a ativação e o uso clínico do arco utilidade de Ricketts no tratamento ortodôntico, além de descrever alguns dos princípios da técnica ortodôntica Bioprogressiva. CONCLUSÃO: pode-se concluir que o arco utilidade é um aparelho extremamente versátil, com inúmeras possibilidades clínicas, sendo capaz de intruir, extruir, avançar ou retrair os incisivos superiores e inferiores; manter, diminuir ou aumentar o espaço no arco inferior; além de ancorar os molares inferiores. Entretanto, uma das maiores vantagens de seu uso consiste na correção de más oclusões do tipo Classe II, com mordida profunda.

INTRODUCTION: The utility archwire, along with the quad-helix, is the most widely known and used device within the Bioprogressive technique for the treatment of several malocclusions. Featuring a simple yet original design, the utility archwire has become an integral part of the orthodontic arsenal, with applications in the periods of mixed and permanent dentition. The name of this archwire refers to its great versatility, and to the multiple effects it can have during treatment. OBJECTIVE: This article aims to discuss the production, application and clinical use of the Ricketts utility archwire in orthodontic treatments, as well as to describe some of the basics aspects of the Bioprogressive orthodontic technique. CONCLUSION: It can be concluded that the utility archwire is an extremely versatile appliance with innumerable clinical applications - able to intrude, extrude, advance or retract maxillary or mandibular incisors; maintain, diminish or increase space in the lower arch, in addition to anchoring mandibular molars. Nevertheless, one of the most significant advantages of its use is in the correction of Class II malocclusions, with overbite.

OBJETIVO: esse trabalho teve como objetivo principal analisar a literatura relacionada a um relevante aspecto clínico, a configuração do arco dentário, salientando a importância da manutenção da forma original do arco dentário para se obter a estabilidade do tratamento ortodôntico, bem como avaliar os métodos utilizados para a determinação da forma do arco e os diagramas de forma propostos por diversos autores. RESULTADOS E CONCLUSÕES: constatou-se uma controvérsia quanto à indicação de uma forma ideal média e a seleção de uma forma individualizada a partir de diagramas que forneçam configurações variadas para o arco dentário. Entretanto, observou-se um progresso nas metodologias aplicadas nos recentes estudos, o que possivelmente fornecerá precisão nos próximos resultados.

AIM: The main purposes of this study were to analyze the literature related to a relevant clinic aspect, the dental arch form, emphasizing the importance of maintaining the original form in order to obtain stability after the orthodontic treatment, as well as to evaluate the methods used for establishing the dental arch form and the arch form diagrams proposed by many authors. RESULTS AND CONCLUSIONS: It was verified that there is some controversy regarding the indication of an ideal average form and the selection of an individualized form from diagrams which provide varied formats for the dental arch. However, some progress was observed in the methodologies applied in recent studies, which will possibly provide more accuracy in arch form determination in the near future.
