OBJETIVO: Identificar a inserção ulnar do ligamento colateral lateral ulnar (LCLU) com o uso da ponta do olécrano e da cabeça do rádio como parâmetros fixos para orientar a cirurgia de reconstrução ligamentar. MÉTODOS: Foram dissecados 13 cotovelos de cadáveres adultos frescos para estudo do LCLU. Com paquímetro digital mediram-se as distâncias entre as inserções proximal e distal do LCLU na ulna (footprint), entre a cabeça do rádio e a zona do footprint e entre o olécrano e a zona do footprint. RESULTADOS: A distância média da cabeça do rádio ao ponto de inserção proximal e distal do LCLU foi de 13,6 e 22,99 mm, respectivamente, da ponta do olécrano à inserção proximal e distal do LCLU foi 38,25 e 47,6 mm, respectivamente, e o comprimento médio do footprint do LCLU foi de 9,35 mm. CONCLUSÕES: A inserção do LCLU tem um footprint amplo com média de 9,3 mm (7,5-11 mm). O ponto médio de inserção ulnar situa-se a 18,2 mm da cabeça do rádio e a 42,9 mm da ponta do olécrano.

INTRODUCTION: The purpose of this study was to indentify the ulnar insertion of the LUCL using the olecranon tip and the radial head as parameters to guide the ligament reconstruction surgery. METHODS: Thirteen elbows of eight fresh cadavers were dissected for the study of the LUCL. The distances between the proximal and distal insertion of the LUCL (footprint), between the radial head and the footprint and between the olecranon tip and the footprint were measure with a digital pachimeter. RESULTS: The average distance from the radial head to the proximal and distal ulnar insertion of the LUCL was 13.6 and 22.99 mm, respectively. The average distance between the olecranon tip and the proximal and distal ulnar insertion of the LUCL was 38.25 and 47.6 respectively. The mean length of the LUCL footprint was 9.35 mm. CONCLUSIONS: The LUCL insertion has a wide footprint with average 9.3 mm (7.5-11 mm). Ulnar insertion half point be located at 18.2 mm of the radial head and at 42.9 mm of olecranon tip.

OBJETIVO: Avaliar os resultados do tratamento cirúrgico de 84 luxações acromioclaviculares agudas com a utilização da via de acesso posterossuperior do ombro. MÉTODOS: Foram avaliados 84 casos de luxações acromioclaviculares agudas grau III da classificação de Allman-Tossy operados de novembro de 2002 a maio de 2010. A média de idade dos pacientes foi de 34 anos. O diagnóstico foi realizado por avaliação clínica e radiográfica. Os pacientes foram operados pela mesma equipe cirúrgica em até três semanas da data do trauma realizando-se a via de acesso posterossuperior do ombro com acesso ao topo da base do processo coracoide para colocação de duas âncoras utilizadas na redução da luxação. O seguimento mínimo foi de 12 meses. A avaliação clínica-radiográfica pós-operatória foi realizada pelos critérios de Karlsson modificados e do escore da Universidade da Califórnia em Los Angeles (UCLA). RESULTADOS: Dos 84 pacientes operados, 92,8% apresentavam resultados bons ou excelentes e 7,2% de resultados regulares ou fracos pelo escore de avaliação da UCLA. Pelos critérios de Karlsson modificados 76,2% foram avaliados como grau A, 17,9% como grau B e 5,9% como grau C. CONCLUSÃO: A VIA de acesso posterossuperior do ombro é uma nova opção para acesso ao processo coracoide e tratamento das luxações acromioclaviculares, com resultados clínicos e radiográficos equivalentes aos da literatura.

OBJECTIVE: To evaluate the results from surgical treatment of 84 cases of acute acromioclavicular dislocation, using a posterosuperior access route. METHODS: Eighty-four cases of acute acromioclavicular dislocation (grade III in the Allman-Tossy classification) operated between November 2002 and May 2010 were evaluated. The patients' mean age was 34 years. The diagnoses were made using clinical and radiographic evaluations. The patients were operated by the same surgical team, within three weeks of the date of the trauma, using a posterosuperior approach to the shoulder to access the top of the base of the coracoid process for placement of two anchors, which were used in reducing the dislocation. The minimum follow-up was 12 months. The postoperative clinical-radiographic evaluation was done using the modified Karlsson criteria and the University of California at Los Angeles (UCLA) score. RESULTS: 92.8% of the 84 patients treated presented good or excellent results, and 7.2% presented fair or poor results, using the UCLA assessment score. According to the modified Karlsson criteria, 76.2% were assessed as grade A, 17.9% as grade B and 5.9% as grade C. CONCLUSION: The posterosuperior access route to the shoulder is a new option for accessing the coracoid process and treating acromioclavicular dislocation, with clinical and radiographic results equivalent to those in the literature.

BACKGROUND: A new fibrin adhesive made of buffalo plasma-derived fibrinogen and a thrombin-like snake venom enzyme, has been successfully used to immobilize free gingival grafts. This case series histologically compared sutured grafts (control group) with others immobilized by using the fibrin adhesive (experimental group). CASE DESCRIPTION: The grafts were placed in the contralateral mandibular bicuspids of 15 patients, so that each subject received one treatment of each type. Five biopsies of each group were collected at 7, 14 and 45 days of healing, which were histologically and morphometrically analyzed as regards the relative volume density of the different connective tissue components. RESULTS: The sites in the control group presented a higher inflammatory cell density at 7 days and a tendency towards a lower collagen density. In the experimental group, the grafts had an appearance of more advanced healing. Tissue maturity characteristics progressed until 14 and 45 days, but no difference between groups could be noted at these times. Conclusions: Within the limits of the present study, it may be suggested that the alternative fibrin adhesive tested could represent an alternative to sutures in gingival grafts procedures.

Este estudo consiste de uma análise morfométrica de diversas estruturas parenquimatosas da glândula submandibular de ratos submetidos ao alcoolismo crônico experimental e de ratos recuperados após 30 dias de desintoxicação. Foram utilizados 3 grupos (5 animais por grupo): Controle, Alcoolizado e Desintoxicado. O fato do álcool atuar nos tecidos corporais, leva-nos a acreditar que possam ocorrer alterações na morfologia da glândula submandibular de ratos submetidos ao alcoolismo crônico. De fato, após os processos de alcoolização e desintoxicação há ocorrência de diferenças estatisticamente significantes quando confrontamos os 3 grupos entre si (dois a dois) no que se refere à massa corpórea dos animais (g) e da glândula submandibular (mg), ocorrendo, normalmente, uma redução tanto do grupo alcoolizado quanto do grupo desintoxicado em relação ao grupo controle e um significante aumento das massas corpórea e glandular do grupo desintoxicado em relação ao grupo alcoolizado. Quanto ao volume absoluto das diversas estruturas glandulares analisadas constatou-se diferenças estatisticamente significantes nos compartimentos acinar e do ducto granuloso, onde os grupos alcoolizado e desintoxicado apresentaram menor volume absoluto em relação ao grupo controle. Quanto aos outros parâmetros estereológicos estudados não houve diferenças estatisticamente significantes entre os grupos e entre as diversas estruturas glandulares analisadas.

This work is a morphometric study of several parenquimatous structures of rats' submandibular gland which were submitted to experimental cronic alcoholism and others which were recovered after detoxication during 30 days. It was used 3 groups (5 rats in each group): Control, Alcoholic and Detoxicated. The fact of alcohol acts in body tissues, take us to believe that changes in the submandibular glands morphology of rats which were submitted to cronic alcoholism can happen. In fact, after alcoholisation and detoxication there is an occurence of significant statistically differences when we confront the 3 groups one another (two by two) in reference to the body weight of the animals (g) and the submandibular gland weight (mg). Usually, occurred a decrease in the alcoholic group and in the detoxicated group as well, towards the control group and a significant increase of the body and gland weight from the detoxicated group in relation to the alcoholic group. Regarding the total volume of the several gland structures evaluated, it was verified significant statistically differences in the acinar and granulous duct compartments, in which the alcoholic and the detoxicated groups presented a smaller total volume than the control group. About the other stereologic parameters evaluated, there was not significant statistically differences among the groups neither among the several gland structures evaluated.
