Abstract The present study aims to evaluate the effect of different glide path instruments on the cyclic fatigue resistance of reciprocating endodontic instruments after three uses in mandibular molars. Eighteen Wave One Gold Primary reciprocating instruments were selected and randomly divided into three groups according to the glide path instrument: G1 - manual file K #15, G2 - Wave One Glider reciprocating instrument, and G3 (control group) - glide path was not performed. The reciprocating instruments were tested on mandibular molars and subdivided into three other groups: a new instrument, an instrument with a previous single-use, and an instrument with two previous uses. After the endodontic instrumentation, the instruments were subjected to the cyclic fatigue resistance test using an appropriate tool. The data were submitted to the Shapiro-Wilk test, and subsequently the Kruskal-Wallis test with a significance level of 5%. The results showed no statistical difference between the groups. Thus, it was concluded that the creation of a glide path did not affect the cyclic fatigue resistance of the reciprocating instrument. In addition, the reuse of final preparation instruments up to two times proved to be safe since no fractures were observed in the tested instruments.

Resumo O objetivo do presente estudo foi avaliar a relação de diferentes instrumentos de glide path na resistência à fadiga cíclica de instrumentos endodônticos reciprocantes de preparo final após três utilizações em molares inferiores. Foram selecionados 18 instrumentos reciprocantes Wave One Gold Primary e divididos de maneira randomizada em três grupos: G1 - glide path utilizado foi uma lima manual K #15, G2 - glide path utilizado foi um instrumento reciprocante Wave One Glider, e G3 (controle) - sem utilização de glide path. Os instrumentos foram testados em molares inferiores e subdivididos em outros três grupos: instrumento novo, instrumento com um uso prévio e instrumento com dois usos prévios. Após os testes, os instrumentos foram submetidos ao teste de resistência à fadiga cíclica por meio de uma ferramenta própria para esse uso. Os dados encontrados foram submetidos ao teste de Shapiro-Wilk para avaliar a normalidade na distribuição da amostra, e posteriormente ao teste de Kruskal-Wallis com nível de significância de 5%. Os resultados demonstraram não haver diferença estatística entre os grupos testados. Com isso, concluiu-se que a criação de um glide path não possuiu efeito na resistência à fadiga cíclica do instrumento reciprocante de preparo final. O instrumento reciprocante de preparo final foi utilizado, e posteriormente reutilizado de maneira segura em três dentes posteriores, visto a não ocorrência de fratura em nenhum dos grupos testados.

ABSTRACT This cross-sectional seroepidemiological survey presents the seroprevalence of SARS-CoV-2 in a population living in 15 Long-Term Care Facilities (LTCFs), after two intra-institutional outbreaks of COVID-19 in the city of Botucatu, Sao Paulo State, Brazil. Residents were invited to participate in the serological survey performed in June and July 2020. Sociodemographic and clinical characterization of the participants as well as the LTCF profile were recorded. Blood samples were collected, processed and serum samples were tested using the rapid One Step COVID-19 immunochromatography test to detect IgM and IgG anti-SARS-CoV-2. Among 209 residents, the median of age was 81 years old, 135 (64.6%) were female and 171 (81.8%) self-referred as being white. An overall seroprevalence of 11.5% (95% CI: 7.5% – 16.6%) was found. The highest seroprevalences of 100% and 76.9% were observed in LTCFs that had experienced COVID-19 outbreaks. Most residents with positive immunochromatography tests (70.8%) referred previous contact with a confirmed COVID-19 case. Although there was a relatively low seroprevalence of COVID-19 in the total number of elderly people, this population is highly vulnerable and LTCFs are environments at higher risk for COVID-19 dissemination. A well-established test for COVID-19 policies, the adequate characterization of the level of interaction between residents and the healthcare provider team and the level of complexity of care are crucial to monitor and control the transmission of SARS-CoV-2 in these institutions.

Resumo Este estudo verificou o grau de saturação de oxigênio (SaO2) pulpar antes, durante e após o clareamento dental caseiro em incisivos centrais superiores hígidos. O nível de SaO2 foi verificado em 136 incisivos centrais superiores hígidos usando oxímetro de pulso. A técnica de clareamento empregou peróxido de carbamida 10% em moldeira individual por quatro horas diárias durante 14 dias. Os níveis de SaO2 foram analisados antes do clareamento (T0), imediatamente após a primeira sessão (T1), no sétimo dia de tratamento (T2), no décimo quinto dia (um dia após a última sessão) (T3) e 30 dias após o término do clareamento dental (T4). A análise estatística utilizou o modelo de equações de estimações generalizadas (GEE), teste t de Student (p<0,05) e correlação de Pearson. Os níveis médios de SaO2 pulpar foram 85,1% em T0, 84,9% em T1, 84,7% em T2, 84,3% em T3 e 85,0% em T4. Foi observada uma redução gradual dos níveis de SaO2, com diferenças significantes (p<0,001) durante o clareamento dental caseiro. No entanto, 30 dias após o término do clareamento dental, houve retorno aos valores iniciais. O clareamento dental caseiro provocou uma diminuição transitória reversível no grau de SaO2 pulpar.

Abstract The present study assessed oxygen saturation (SaO2) levels before, during, and after at-home bleaching treatment in the pulps of healthy maxillary central incisors. SaO2 levels were measured in 136 healthy maxillary central incisors using a pulse oximeter. The bleaching protocol consisted of 10% carbamide peroxide gel placed in individual trays and used for four hours daily for 14 days. SaO2 levels were assessed before bleaching (T0), immediately after the first session (T1), on the 7th day of treatment (T2), on the 15th day (the day following the last session) (T3), and 30 days after completion of the bleaching protocol (T4). Data were statistically analyzed using generalized estimating equations (GEE), Student’s t test (p<0.05) and Pearson’s correlation. Mean pulp SaO2 levels were 85.1% at T0, 84.9% at T1, 84.7% at T2, 84.3% at T3, and 85.0% at T4. Gradual reductions in SaO2 levels were observed, with significant differences (p<0.001) during the course of home bleaching treatment. However, 30 days after the end of the bleaching protocol, SaO2 levels returned to baseline levels. Home bleaching caused a reversible transient decrease in SaO2 levels in the pulps.

OBJECTIVE: The objective of this work was to evaluate, using radiographic images, the behavior of four materials used to repair root perforations in dogs' teeth. Material and METHODS: Second and third premolars of 6 dogs were used. The 48 teeth were randomly divided into 4 groups (n=12) and the perforations were sealed with one of the following materials: MTA, AH Plus, Vitremer and gutta-percha. Dogs were submitted to general anesthesia, teeth were radiographed and pulp was accessed. Perforations were done, at the maximum curve of the pulp floor, sealed and the accessed coronal cavity was filled with glass ionomer cement (Vidrion R). After 90 days, the dogs were sacrificed and the last x-ray image was taken. Images were analyzed for the presence/absence of periodontal lesions at the perforation region. Data were analyzed statistically by chi-square test at 5% significance level. RESULTS: There were no statistically significant differences (p>0.05) among AH Plus, Vitremer and gutta-percha groups. MTA produced the smallest number of periodontal lesions (p<0.05). CONCLUSIONS: It may be concluded that none of the tested materials was able to preserve the integrity of the periodontal tissues in the furcation region, and the use of MTA resulted in the least formation of adjacent periodontal bone lesions revealed by the radiographic comparisons.

This work aimed at studying the effects of different sources and cytokinin concentrations, as well as explants orientation and time of their permanence in shoots induction medium to obtain high multiplication rate. Four experiments were carried out to evaluate the sprout percentage mean, shoots number per explant, elongated shoots percentage and shoots length. The treatments with 2iP and zeatin did not promote in vitro plant multiplication. When different BAP concentrations were tested, an increase in sprout percentage was obtained using BAP concentration up to 3.66 mg L-1 (80%), and for shoots number per explant a linear behavior was observed, showing a mean of four shoots per explant with 4.0 mg L-1 BAP. For the 'Flordaguard' rootstocks, the explants orientation did not alter the potential in vitro multiplication and the shoots growth. However, the explants permanence in the shoots induction medium for 40 days provided the best results for shoots growth.

Esta pesquisa estudou a flexibilidade de instrumentos do Sistema K3 (conicidade 0.04) e do Sistema ProTaper Universal, por meio de um teste mecânico, quando os instrumentos eram novos e após 5 usos em canais simulados. Cinco jogos de instrumentos de cada sistema foram testados, como segue: grupo I - K3 (15, 20, 25, 30, 35, 40 e 45), e grupo II - ProTaper Universal (S1, S2, F1, F2, F3, F4 e F5). Cada jogo de instrumento foi usado para preparar um canal simulado, e o mesmo jogo de instrumento foi usado 5 vezes (50 canais). O número de cada subgrupo representou o número de usos: 0 (controle), 1, 3 e 5 usos. Antes do uso e depois de cada uso os instrumentos foram submetidos a um teste mecânico de flexibilidade realizado na máquina de ensaios universais Versat 502. A interação entre o instrumento e o número de usos foi analisada pelo ANOVA e pelo teste de Tukey ao nível de significância 5%. Instrumentos de ambos os sistemas apresentaram menor flexibilidade após o terceiro uso quando comparada com a flexibilidade após os usos 0 e 1 (p<0,05), e mantiveram a mesma flexibilidade após o quinto uso. A flexibilidade dos instrumentos do sistema K3 diminuiu conforme o aumento de diâmetro, independente do número de usos. Entre os instrumentos do sistema ProTaper Universal, os shaping apresentaram maior flexibilidade que os finishing. F2 e F3 foram os menos flexíveis e F4 e F5 apresentaram valores de flexibilidade similares ao F1.

This study used a mechanical test to evaluate the flexibility of instruments from the K3 (conicity 0.04) and the ProTaper Universal systems when they were new and after 5 uses in simulated canals. Five sets of instruments of each system were tested: K3 (15, 20, 25, 30, 35, 40 and 45) and ProTaper Universal (S1, S2, F1, F2, F3, F4 and F5). Each set of instruments was used to prepare a simulated canal and the same set of instruments was used 5 times (50 canals). The number of each subgroup represented the number of uses: 0 (control), 1, 3 and 5 uses. Before and after each use, the instruments were submitted to a mechanical flexibility test performed in a Versat 502 universal testing machine. Interactions between the instrument and the number of uses were analyzed by ANOVA and Tukey's test at a 5% level of significance. Instruments from both systems presented lower flexibility after the third use compared to the flexibility obtained after uses 0 and 1 (p<0.05), and maintained the same flexibility after the fifth use. The flexibility of instruments from the K3 system decreased with the increase of diameter, irrespective of the number of uses. Among the instruments from the ProTaper Universal system, the shaping files presented greater flexibility than the finishing files. F2 and F3 were the least flexible instruments, and F4 and F5 presented flexibility values similar to those of F1.

Este estudo utilizou a tomografia computadorizada (TC) para comparar o transporte do canal radicular nos canais mésio-vestibulares de molares superiores preparados por diferentes técnicas. Sessenta molares superiores foram divididos em 3 grupos (n=20) de acordo com a técnica utilizada para o preparo do canal radicular: instrumentação manual com limas K-Flexofile, limas K-Flexofile acopladas a um sistema oscilatório e sistema rotatório ProTaper. Imagens de TC pré e pós-instrumentação foram obtidas 3 mm aquém do forame apical e superpostas para comparar o transporte do canal. Os dados foram analisados estatisticamente pela Análise de Variância (ANOVA) e Teste de Tukey utilizando o software SPSS (α=0,05). Na direção vestibular, a técnica manual produziu significantemente menos transporte do canal radicular do que a técnica oscilatória (p<0,05) e ambas foram similares a técnica rotatória (p>0,05). Nas direções distal e disto-palatina, a técnica oscilatória produziu mais transporte do canal radicular (p<0,05). Na direção mésio-palatina, a técnica oscilatória produziu mais transporte do canal radicular do que a técnica manual (p<0,05), sendo que ambas foram similares à técnica rotatória (p>0,05). Em conclusão, todas as técnicas produziram transporte do canal radicular e a técnica oscilatória produziu os maiores desgastes de dentina na direção interna da curvatura.

This study used computed tomography (CT) to compare apical canal transportation in mesiobuccal canals of maxillary molars prepared with different techniques. Sixty teeth were assigned to 3 groups (n=20), according to the technique used for root canal instrumentation: hand instrumentation with K-Flexofiles, K-Flexofiles activated by an oscillatory system and ProTaper NiTi rotary system. Pre and post-instrumentation CT images were obtained 3 mm short of the apical foramen and were superimposed to compare canal transportation. Data were analyzed statistically by ANOVA and Tukey's test using the SPSS software (α=0.05). In the buccal direction, the manual technique produced significantly less canal transportation than the oscillatory technique (p<0.05) and both were similar to the rotary technique (p>0.05). In the distal and distopalatal directions, the oscillatory technique produced more canal transportation (p<0.05). In the mesiopalatal direction, the oscillatory technique produced more canal transportation than the manual technique (p<0.05), and both were similar to the rotary technique (p>0.05). In conclusion, all techniques produced canal transportation, and the oscillatory technique produced the greatest removal of root dentin toward the innerside of the root curvature.

OBJECTIVE: This study investigated the influence of the last apical instrument of the ProTaper system with and without 2.5% sodium hypochlorite for cleaning mandibular central incisors. MATERIAL AND METHODS: Thirty two mandibular central incisors were divided into six study groups: Group I - F1 instrument with 2.5% sodium hypochlorite; Group II - F1 and F2 with 2.5% sodium hypochlorite; Group III - F1, F2 and F3 with 2.5% sodium hypochlorite; Group IV - F1 with distilled water; Group V - F1 and F2 with distilled water; Group VI - F1, F2 and F3 with distilled water. The two remaining teeth comprised the negative control group. The specimens were prepared following the principles of the technique suggested by the manufacturer and then submitted to histological preparation and morphometric analysis. Data were analyzed statistically by the Kruskal Wallis test at 1% significance level. RESULTS: There was statistically significant difference (p<0.01) between all study groups, except between Groups I and VI. CONCLUSIONS: It was concluded that no technique allowed complete cleaning of the root canals. However, the technique of finishing preparation of the apical third with the F3 instrument with 2.5% sodium hypochlorite irrigation was the most effective.

Coronal leakage is an important factor affecting the outcome of endodontic therapy. This study evaluated the microbial leakage (ML) and the apical inflammatory response (AIR) in dog's teeth after root canal filling with three endodontic sealers, post preparation and exposure to the oral environment, testing the hypothesis that there is a positive correlation between these two histological parameters (ML and AIR). Sixty-four root canals of 8 mongrel dogs were cleaned, shaped and randomly distributed into groups according to the sealer to be used: Sealer 26 (n=18); AH Plus (n=18); RoekoSeal (n=19); no sealer - control group (n=9). Root canals were filled by the lateral condensation technique. Post space preparation left 4 mm of filling material in the apical root third, and specimens were exposed to the oral environment for 90 days. The dogs were killed and jaw blocks were histologically processed using Brown & Brenn and HE staining techniques. ML and AIR were scored from 1 to 4. Results were analyzed statistically using ANOVA, Duncan's post-hoc test and Spearman's correlation. ML and AIR score means were: Sealer 26 - 2.44±0.98 and 2.50±0.70; AH Plus - 2.50±0.78 and 2.22±0.54; RoekoSeal - 1.84±0.95 and 2.63±0.83; Control - 2.56±1.23 and 3.11±0.60. Statistically significant differences in AIR scores were found between the AH Plus and control groups (p<0.05). Although RoekoSeal had the lowest ML means, and AH Plus, the lowest AIR means after 90-day exposure to the oral environment, no statistically significant differences were found between the three sealers under study, and no correlation was found between ML and AIR.

Este estudo in vitro avaliou comparativamente, por meio de tomografia computadorizada (TC), a quantidade de dentina removida das paredes do canal radicular utilizando-se as técnicas de preparo manual e automatizada de instrumentação rotatória. Foram selecionados 40 incisivos inferiores com coroa dental e um único canal. Os dentes foram divididos aleatoriamente em 2 grupos, de acordo com a técnica empregada para o preparo dos canais radiculares: Grupo I - técnica manual com limas manuais de aço inoxidável e Grupo II - técnica automatizada de movimentos rotatórios RaCe com limas de níquel-titânio. Em cada dente, procedeu-se à mensuração (em mm) da espessura da dentina radicular nas faces vestibular, lingual, mesial e distal dos terços apical, médio e cervical por meio de tomografia computadorizada multislice (Siemens Emotion, Duo). Os dados coletados foram armazenados utilizando-se os softwares SPSS versão 11.5 e SigmaPlot 2001 v. 7.101 (SPSS Inc.). Após a abertura coronária, o comprimento de trabalho foi estabelecido, os canais radiculares foram instrumentados e novas tomadas de tomografia computadorizada foram realizadas para mensuração da espessura das paredes dentinárias radiculares. Os dados obtidos antes e após o preparo biomecânico dos canais foram comparados. Os resultados foram estatisticamente por meio de análise de variância (ANOVA) com localização de diferenças post hoc de Tukey (p=0,05). Com base nos achados desse estudo, pode-se concluir que: com relação à remoção de dentina das paredes dos canais radiculares durante a instrumentação nenhuma das técnicas avaliadas pode ser apontada como mais efetiva que a outra.

This in vitro study compared, using computed tomography (CT), the amount of dentin removed from root canal walls by manual and mechanical rotary instrumentation techniques. Forty mandibular incisors with dental crown and a single canal were selected. The teeth were randomly assigned to two groups, according to the technique used for root canal preparation: Group I - manual instrumentation with stainless steel files; Group II - mechanical instrumentation with RaCe rotary nickel-titanium instruments. In each tooth, root dentin thickness of the buccal, lingual, mesial and distal surfaces in the apical, middle and cervical thirds of the canal was measured (in mm) using a multislice CT scanner (Siemens Emotion, Duo). Data were stored in the SPSS v. 11.5 and SigmaPlot 2001 v. 7.101 softwares. After crown opening, working length was determined, root canals were instrumented and new CT scans were taken for assessment of root dentin thickness. Pre- and post-instrumentation data were compared and analyzed statistically by ANOVA and Tukey's post-hoc test for significant differences (p=0.05). Based on the findings of this study, it may be concluded that regarding dentin removal from root canal walls during instrumentation, neither of the techniques can be considered more effective than the other.

OBJETIVO: determinar a influência de três sistemas distintos de instrumentos rotatórios para alargamento cervical na determinação do comprimento real de trabalho. MATERIAL E MÉTODOS: Trinta primeiros molares inferiores foram submetidos ao acesso endodôntico convencional e odontometria inicial pelo método visual, utilizando-se para irrigação/aspiração da câmara pulpar o hipoclorito de sódio a 5%. Procedeu-se a ampliação cervical dos canais mésio-vestibulares empregando-se diferentes alargadores cervicais. De acordo com o sistema de ampliação cervical empregado, as 30 amostras foram divididas aleatoriamente em 3 grupos de 10 dentes cada. No Grupo 1 as brocas de Gates-Glidden foram utilizadas para a ampliação cervical; no Grupo 2, Orifice Openers foram empregados; e, no Grupo 3, foi usado o sistema La Axxess. Foram utilizadas em ordem crescente, duas numerações do sistema rotatório correspondente a cada grupo e, após, realizou-se a odontometria final. A leitura das duas tomadas odontométricas foi realizada com paquímetro digital, em milímetros, considerando-se duas casas decimais para averiguar a provável discrepância entre ambas as tomadas. RESULTADOS: a análise de variância (ANOVA) e o teste de Tukey (p£ 0,05) demonstraram diferença estatística entre o Grupo 1 e os Grupos 2 e 3. CONCLUSÕES: todos os grupos apresentaram encurtamento do comprimento de trabalho após alargamento cervical; os grupos preparados com instrumentos La Axxes e Orifice Opener apresentaram os melhores resultados.

OBJECTIVE: to investigate the influence of three different rotary systems for cervical flaring on establishment of the real working length. MATERIAL AND METHODS: Thirty mandibular first molars were submitted to conventional endodontic access and initial working length measurement, followed by irrigation/suction of the pulp chamber with 5% sodium hypochlorite. Teeth were randomly divided into 3 groups (n=10) and cervical flaring of the mesiobuccal canals were performed using one of the following instrument systems: Group 1 - Gates-Glidden burs; Group 2 - Orifice Openers; Group 3 - La Axxess system. Two subsequent numbers of instruments of each rotary system were used and the final working length was recorded. A digital calyper was used to record the working length, in millimeters, to investigate a possible discrepancy between initial and final measurements. RESULTS: Analysis of variance (Anova) na Tukey test revealed statistical difference between Group 1 and Groups 2 and 3 (p£ 0.05). CONCLUSIONS: all groups presented shorter working length after cervical flaring; groups prepared with instruments La Axxes and Orifice Opener presented the best results among the systems studied.

Este estudo in vivo avaliou a capacidade de selamento de um cimento endodôntico resinoso (AH Plus) e um a base de óxido de zinco eugenol (Endofill), em dentes de cães, expostos ao meio bucal, por 45 e 90 dias. Foi realizado o tratamento endodôntico de 48 incisivos inferiores de 8 cães. Uma randomização estratificada determinou o tipo de cimento a ser usado em cada canal que foram tratados pela técnica de condensação lateral. Após a obturação, o excesso de material obturador, na porção cervical do canal radicular foi seccionado, restando o comprimento de 10 mm no interior do canal radicular. Os dentes foram selados, provisoriamente, com cimento de ionômero de vidro por 24 h. A seguir, o selamento coronário foi removido e os canais ficaram expostos ao meio bucal. Desta forma, delineou-se os seguintes grupos experimentais: A45- AH Plus por 45 dias; A90- AH Plus por 90 dias; E45- Endofill por 45 dias; e E90- Endofill por 90 dias (n=12). Passado o período experimental, os cães foram mortos e as mandíbulas removidas. Realizou-se a exodontia dos incisivos e as suas raízes foram impermeabilizadas com duas camadas de esmalte para unhas. Os dentes foram imersos em tinta nanquim por 96 h. Após realizou-se a diafanização dos espécimes e a infiltração de corante foi medida (em mm) com auxílio de uma lupa esteroscópica, em aumento de 10x. Os resultados foram analisados estatisticamente através da Análise de Variância de dois fatores e Teste de Comparações Múltiplas de Tukey (á = 0.05). O grupo E90 (2.03±0.94) apresentou uma infiltração média significativamente maior do que a ocorrida nos demais grupos (p<0.001). Os cimentos avaliados, nos dois períodos experimentais, não foram capazes de impedir a infiltração de corante.

This in vivo study evaluated the sealing ability of a resin-based sealer (AH Plus) and a zinc oxide-eugenol sealer (Endofill) in dogs' teeth, exposed to the oral environment for 45 and 90 days. Forty eight lower incisors from 8 dogs were endodonticaly treated. A stratified randomization determined the sealer use in each root canal. All canals were filled using the lateral condensation technique. The excess filling material at the cervical portion of the root canal was sectioned, leaving a 10-mm obturation length inside the canal. Teeth were provisionally sealed with glass ionomer cement for 24 h and the canals were exposed to the oral environment for either 45 or 90 days. Therefore, the experimental groups were as follows: A45- AH Plus for 45 days; A90- AH Plus for 90 days; E45- Endofill for 45 days; and E90- Endofill for 90 days (n=12). After the experimental period, the dogs were killed and the lower jaw was removed. The incisors were extracted and the roots were covered with two coats of nail varnish. The teeth were immersed in India ink for 96 h and submitted to diaphanization. Dye leakage (in mm) was measured using stereomicroscopy (10x magnification). The results were statistically analyzed using two-way ANOVA and Tukey test for multiple comparisons (á = 0.05). Group E90 (2.03±0.94) showed significantly higher mean leakage value than all other groups (p<0.001). None of the sealers, in both study conditions, were able to prevent dye leakage.

Avaliou-se a influência do pré-alargamento cervical na determinação do instrumento apical inicial em raízes mésio-vestibulares de molares superiores. Foram selecionados 50 primeiros molares superiores com grau de curvatura da raiz mésio-vestibular padronizado entre 10 e 15 graus. Concluída a cirurgia de acesso e remoção do tecido pulpar, determinou-se o comprimento de trabalho a 1mm do ápice. Os dentes foram divididos aleatoriamente em 5 grupos (n=10), de acordo com o tipo de alargamento realizado: Grupo 1: sem alargamento cervical; Grupo 2: brocas Gates-Glidden; Grupo 3: Orifice Opener; Grupo 4; ProTaper; Grupo 5: LA Axxess. Os canais foram explorados com lima do tipo K inserindo-se passivamente a lima 08 no comprimento de trabalho. A seguir, limas de maiores diâmetros foram sucessivamente introduzidas no canal radicular, até se obter a sensação de travamento no comprimento de trabalho. O diâmetro desse instrumento foi registrado, e este foi fixado em posição no canal com cianoacrilato de metila. Secções transversais realizadas no comprimento de trabalho foram observadas ao microscópio eletrônico de varredura e a discrepância entre o menor diâmetro do canal e o diâmetro do instrumento apical inicial foram calculados para cada amostra, por meio de um software. As médias de diferença entre esses diâmetros obtidas em cada grupo foram submetidas à análise estatística. A análise de variância indicou diferença estatisticamente significante entre os grupos (p<0,001). O teste complementar de Tukey evidenciou a maior discrepância para o grupo que não recebeu o pré-alargamento (média: 0,1543 mm ± 0,0216). As brocas LA Axxess promoveram a menor diferença entre o diâmetro anatômico no comprimento de trabalho e o instrumento apical inicial (média: 0,026 mm ± 0,0037). As brocas Gates-Glidden e os instrumentos Orifice Opener foram estatisticamente semelhantes (p>0,05) (média: 0,1167 mm ± 0,0231 e 0,1313 mm ± 0,0344 respectivamente) O pré-alargamento realizado com instrumentos ProTaper evidenciaram a média de 0,0567mm e desvio padrão 0,0354 para os valores de discrepância. Pode-se concluir que o pré-alargamento dos terços cervical e médio permitiu uma melhor determinação do instrumento apical inicial. O grupo no qual foram utilizados instrumentos LA Axxess refletiu com maior fidelidade o diâmetro anatômico no comprimento de trabalho em raízes mésio-vestibulares de primeiros molares.

The purpose of this study was to investigate the influence of cervical preflaring on determination of the initial apical file in mesiobuccal roots of maxillary molars. Fifty first molars with degree of curvature of the mesiobuccal root between 10° and 15° were utilized. After standard access opening and removal of pulp tissue, the working length (WL) was determined at 1 mm from the root apex. Five groups (n=10) were formed at random, according to the type of cervical preflaring performed. In group 1, the size of the initial apical file was determined without cervical preflaring. In groups 2 to 5, the cervical and middle thirds of the canals were preflared with Gates-Glidden drills, K3 Orifice Opener instruments, ProTaper instruments and LA Axxess burs, respectively. Canals were sized manually with K-files, starting with size 08 K-files, inserted passively up to the WL. File sizes were increased until a binding sensation was felt at the WL and the size of the file was recorded. The binding instruments were fixed into the canals at the WL with methylcyanacrylate. Cross-sections of the WL region were examined under scanning electron microscopy and the discrepancies between the canal diameter and the diameter of first file to bind at the WL were calculated using UTHSCSA ImageTool software. Data were analyzed statistically by ANOVA and multiple comparisons were done by Tukey's HSD post-hoc test. Significant differences (p<0.001) were found among the groups. The major discrepancy was observed for the group without preflaring (mean 0.1543 mm ± 0.0216). Cervical preflaring with LA Axxess burs produced the least discrepancies between the canal size and the diameter of the initial apical instrument (mean 0.026 mm ± 0.0037), followed by ProTaper files (mean 0.0567 mm ± 0.0354). Canals preflared with Gates-Glidden drills and K3 Orifice Opener instruments showed statistically similar discrepancy results to each other (p>0.05) (means 0.1167 mm ± 0.0231 and 0.1313 mm ± 0.0344, respectively). In conclusion, preflaring of the cervical and middle thirds improved the determination of the initial apical instrument. Canals preflared with LA Axxess burs showed a more accurate binding of the files to the anatomical diameter at the WL in the mesiobuccal roots of maxillary first molars.

O presente trabalho avaliou, in vitro, a presença de desvio apical no preparo de canais radiculares em raízes mésio-vestibulares de molares superiores humanos utilizando-se de peças automatizadas de giro contínuo, sistema Pow R (Moyco Union Broach) e giro alternado, sistema M4 (Kerr - USA). A análise da presença de desvios apicais foi realizada por meio de radiografias obtidas na plataforma radiográfica confeccionada para o experimento. Os resultados mostraram não haver diferença estatisticamente significante quanto à avaliação da presença de desvio entre os dois sistemas. Não houve correlação entre o grau de curvatura das raízes e o grau de desvio apical, no sistema de giro contínuo. No entanto, o sistema de giro alternado apresentou diferença estatística de correlação direta e grau médio.

The present study conducted an in vitro evaluation of the presence of apical deviation on the root canal preparation of the mesiobuccal roots of human maxillary molars employing automated handpieces with continuous motion, the Pow R system (Moyco Union Broach), and alternate motion, the M4 system (Kerr). Analysis of the presence of apical deviation was carried out by means of radiographs obtained on a radiographic desk fabricated for the study. The results demonstrated the lack of statistically significant differences between the two systems as to the evaluation of the presence of apical deviation. There was no correlation between the degree of curvature of the roots and the degree of apical deviation for the continuous motion system. However, there was a statistic difference for the alternate motion system, with a direct correlation of moderate degree.

O objetivo deste trabalho foi avaliar o deslocamento apical produzido por diferentes instrumentos de níquel-titânio acionadas a motor testando a hipótese de que os sistemas rotatórios que usam instrumentos de níquel-titânio produzem valores médios de deslocamento apical menores que as limas manuais de aço inox. Foram utilizados 100 primeiros molares permanentes superiores, cujas raízes mesiovestibulares foram separadas e incluídas em blocos de resina autopolimerizável. Os corpos de prova foram divididos aleatoriamente em 5 grupos e os canais radiculares preparados com instrumentos de níquel-titânio: Grupo 1 - sistema Quantec 2000 (Analytic Endodontics, México); Grupo 2 - Pro-File T.0.04 (Dentsply/Maillefer, Suíça); Grupo 3 - Pro-File Série 29 T.0.04 (Dentsply Tulsa, Suíça); Grupo 4 - Pow-R T.0.02 (Moyco-Union Broach, EUA); e Grupo 5 (limas manuais) - limas manuais de aço inox Flexofile (Dentsply/Maillefer, Suíça). Todos os canais mesiovestibulares foram submetidos a um preparo cervical prévio utilizando-se instrumentos Orifice Shaper números 1, 2, 3 e 4 (Dentsply/Maillefer, Suíça). Realizada a odontometria, o restante do canal radicular foi modelado a partir dos instrumentos do menor para o maior calibre de tal ordem que o instrumento final de preparo correspondeu ao número 9 para o Quantec, número 06 para o Pro-File Série 29 e o número 35 para os demais grupos. Nos grupos de 1 a 4 foi utilizado motor elétrico com contra-ângulo de redução de 16:1 numa velocidade de 350 rpm. No grupo 5 o preparo foi realizado manualmente. A análise dos resultados (ANOVA), obtidos pela sobreposição radiográfica numa plataforma especifica, demonstraram que todos os grupos apresentaram deslocamentos apicais, sendo que o menor valor médio foi verificado no grupo 4 e os maiores valores médios foram observados nos grupos 2 e 3 (p<0,001).

The aim of this study was to evaluate the apical displacement produced by different rotary nickel-titanium instruments, testing the hypothesis that rotary systems with nickel-titanium instruments produce lower mean values of apical displacement than stainless steel hand instruments. A total of 100 maxillary permanent first molars were selected for the study. The mesiobuccal roots were sectioned at the top cervical third and embedded in blocks of self-curing resin. The specimens were randomly divided into 5 groups and the root canals were prepared using the following nickel-titanium instruments: Group 1 - Quantec system 2000 (Analytic Endodontics, Mexico); Group 2 - Pro-File T.0.04 (Dentsply/Maillefer, Switzerland); Group 3 - Pro-File Series 29 T.0.04 (Dentsply Tulsa, Switzerland); Group 4 - Pow-R T.0.02 (Moyco-Union Broach, USA). Specimens in Group 5 were prepared using stainless steel hand instruments Flexofile (Dentsply/Maillefer, Switzerland). All root canals were previously submitted to cervical preparation using Orifice Shaper instruments #1, 2, 3 and 4 (Dentsply/Maillefer, Switzerland). After odontometry, the remaining root canal was shaped employing increasingly larger instruments, so that the final instrument corresponded to Quantec #9, Pro-File Series 29 #6, and #35 for the other groups. Specimens in Groups 1 to 4 were prepared using an electric handpiece with 16:1 reduction at 350 rpm. The specimens in Group 5 were manually prepared. Apical displacement was measured and recorded by means of radiographic superimposition on a specific desk. Statistical analysis (ANOVA) of the results revealed that all groups presented apical displacements. Considering only the nickel-titanium instruments, Group 4 showed the lowest mean value while Groups 2 and 3 produced the highest mean apical displacement values (p<0.001).
