OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

Abstract Crustacean decapod larvae have developed several adaptations to maximize their survival, including swimming behavior. Swimming improves larvae dispersal and contributes to their response to environmental stresses, including variations in abiotic factors, presence of predators, and search for food. Data on larval behavior can also assist in studies on conservation and in species management in aquaculture. Unfortunately, swimming behavior remains unstudied in many species. The present work aims to analyze the swimming behavior in newly hatched larvae of six common decapod crustaceans in coastal Southeast Brazil (Macrobrachium acanthurus, Macrobrachium olfersii, Emerita brasiliensis, Ucides cordatus, Minuca rapax, and Callinectes danae). The analysis included tests with larval responses to four isolated stimuli: gravity (geotaxis), vertical salinity gradients, low-pressure zones, and presence of the planktivorous fish, Atherinella brasiliensis (only for C. danae and M. rapax). The first two and the last two stimuli were used as indicators of dispersal and defensive behaviors, respectively. All larvae preferred water depth with higher salinity levels. Additionally, negative geotaxis was observed in M. olfersii, M. acanthurus, U. cordatus, and C. danae, being stronger in the last. Both Macrobrachium larvae responded to low-pressure zones, promptly moving away. Callinectes danae and M. rapax larvae avoided contact with the predatory fish by creating avoidance zones around it, even in the absence of light. This is an active defensive behavior that has never been reported for the newly hatched larvae of these two species.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. ClinicalTrials.gov: NCT04378582. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

RESUMO: Problemas causados pelos nematoides-das-galhas, pertencentes ao gênero Meloidogyne podem ser controlados com a introdução de genes de resistência em cultivares comerciais de tomateiro. No entanto, plantas do híbrido ‘Heinz 9992’ adequadas ao processamento industrial, portadoras do gene Mi, foram encontradas com suas raízes severamente infectadas, em um cultivo experimental no município de Morrinhos (GO) com sintomas típicos de galhas causadas por Meloidogyne spp. Após dissecar as galhas presentes nas amostras de raízes, as fêmeas do nematoide foram retiradas para a análise do padrão perineal e a caracterização bioquímica por eletroforese de isoenzimas, além do teste de patogenicidade. O patógeno em estudo foi identificado como Meloidogyne incognita. O teste de patogenicidade confirmou que ocorreu a suplantação de resistência do híbrido de tomateiro, portador do gene Mi, por esta população virulenta de M. incognita.
ABSTRACT: Problems with root-knot nematodes, caused by genus Meloidogyne, can be controlled through the introduction of resistance genes in commercial varieties of tomatoes. However, plants of the hybrid ‘Heinz 9992’ for industrial processing, carrying the Mi gene, were reported with their roots severely infected in experimental cultivation in Goiás State of Brazil with symptoms typical of galls caused by Meloidogyne spp. After dissecting the galls present in the root samples, the females of the nematodes were removed and afterwards, performed the analysis of the perineal pattern and the biochemical characterization by electrophoresis of isoenzymes, besides the pathogenicity test. The pathogen analyzed was identified as Meloidogyne incognita. The pathogenicity test confirmed the supplanting resistance of the tomato hybrid with the Mi gene by this virulent population of M. incognita.

The infusion at 1.5% of yerba mate (Ilex paraguariensis A.St.-Hil.) (Ip) corresponds to "cooked mate" (MC), whose combination with milk (50:50 v/v) (M) and refined sugar (S) is the "mate cocido with milk", a popular food in southern South America offered to preschool and primary education students (3-13 years old) in Argentina. Another species of the genus, I. dumosa Reissek (Id), has low caffeine; appears in the market as an Id:Ip (70:30 w/w) mixture. The substitution of Ip by the mentioned mixture maintains analogous nutritional values and significantly lowers the caffeine levels (α=0.05); this allows for a higher consumption of the food during the day, without reaching the xanthine daily limits (40-100 mg day-1) for the age range, favoring the students intellectual and physical performance without adverse effects. The daily intake of a double serving (breakfast and afternoon snack) of MC+M+S provides high energy and is a source of carbohydrates, proteins, vitamins A, B1, B 2, B5, B6, and B12, biotin and essential minerals (Mn, P, Zn, Cu, Ca, Mg, Na, Cl, K, Fe and Al), with low cholesterol. The nutritional parameters are within international limits. Heavy metals do not make it to the infusion or they remain well below harmful values.
La infusión al 1,5% de "yerba mate" (Ilex paraguariensis A.St. -Hil.) (Ip) constituye el "mate cocido" (MC), que con leche (50:50 v/v) (M) y azúcar refinada (S) es el "mate cocido con leche", alimento popular en el Cono Sur de América ofrecido en la escuela a niños de educación inicial y primaria (3-13 años) en Argentina. Otra especie del género, I. dumosa Reissek (Id), tiene baja cafeína; se presenta en el mercado en mezcla con Ip (Id:Ip 70:30 p/p). Sustituyendo Ip por la mezcla, el MC+M+S mantiene valores nutricionales análogos con niveles significativamente bajos de cafeína (α=0.05); aún con consumos diarios altos de este alimento, no se alcanza los límites de xantinas establecidos para el rango etario (40-100 mg día-1), favoreciendo el desempeño intelectual y físico sin efectos adversos. La ingesta diaria de una doble porción (desayuno y merienda) de MC+M+S aporta altos valores energéticos y es fuente de carbohidratos, proteínas, vitaminas A, B1, B 2, B5, B6, y B12, biotina y minerales esenciales (Mn, P, Zn, Cu, Ca, Mg, Na, Cl, K, Fe y Al), con bajo colesterol. Los parámetros nutricionales están comprendidos dentro de límites internacionales. Los metales pesados no pasan a la infusión, o están muy por debajo de los valores peligrosos.

Fast detection of Mycobacterium tuberculosis in culture-positive sputum samples by nitrate reductase activity

ABSTRACT Microscopy and bacterial culture are the main tools in the diagnosis of tuberculosis. Since the slow growth of Mycobacterium tuberculosis impairs rapid diagnosis strategies, especially in countries where the latter are the only available resources, the ongoing development of new and inexpensive tools based on mycobacterial metabolism optimizing growth detection with preliminary identification is greatly welcome. When compared to the other species from the M. tuberculosis complex, M. tuberculosis is a strong nitrate reducer. Current assay compares the nitrate reductase activity of M. tuberculosis from pulmonary specimens cultivated in nitrate-supplemented media. Fifty-five sputum samples were decontaminated and inoculated in conventional (Middlebrook 7H9, Ogawa Kudoh-OK) and in nitrate-supplemented media (Middlebrook 7H9-N, Ogawa Kudoh-N). An aliquot from the media directly reacted with Griess reagent (7H9-N and OK-N) every five days, or transferred to a nitrate substrate solution (7H9, OK). Nitrate to nitrite reduction was considered positive, revealed by the pink color, indicating bacterial growth. As reference method, the Mycobacteria Growth Indicator Tube (MGIT) was used for sensitivity calculations and statistical analysis. 7H9-N and OK-N assays proved to perform better in detecting M. tuberculosis than conventional assays (7H9 and OK). Indeed, broth nitrate-supplemented medium (7H9-N) was comparable to MGIT to detect M. tuberculosis, except in growth detection time. Results show that 7H9-N may be used as an alternative tool particularly in low-income countries since it is a simple and cheap technique, and does not restrict diagnosis to single-source products.

This work demonstrates the application of batch-injection analysis (BIA) with amperometric detection on a multi-walled carbon nanotube (MWCNT) modified electrode for the sensitive determination of the antioxidant pyrogallol in biodiesel. Samples were diluted in electrolyte (0.1 mol L-1 HClO4) before injection using an electronic micropipette on the BIA cell. The size effect of MWCNTs on electrochemical oxidation of pyrogallol was investigated and 3.5-fold increase in the amperometric sensitivity using the modified electrode with MWCNTs of shorter dimensions was obtained. This result was in agreement with cyclic voltammetry and electrochemical impedance spectroscopy measurements. After optimization of BIA parameters for the amperometric detection on the MWCNT-modified electrode, the proposed method presented a linear range from 0.3 to 1000 µmol L-1, detection limit of 0.013 µmol L-1, precision of 3%, and sample throughput of 240 h-1. Spiked biodiesel samples were analyzed and recovery values between 95-99% were obtained.

Em 1978 Christopher T. Russell e Richard C. Elphic detectaram tubos de fluxo magnético na região de fronteira da magnetosfera terrestre por meio de dados observacionais obtidos pelos satélites ISEE-1 e −2, que foram denominados eventos de transferência de fluxo, ou simplesmente FTE (do inglês, flux transfer events). O vento solar, juntamente com o campo magnético interplanetário, oriundos do Sol, atingem a região de campo magnético terrestre, produzindo fenômenos como a reconexão magnética e os tubos de fluxo. Esse artigo apresenta uma introdução sobre a conexão Sol-Terra e o processo de reconexão magnética, e principalmente, os conceitos básicos sobre a geração e a formação dos tubos de fluxo magnético. Os modelos que descrevem o tipo de reconexão e as características dos tubos, como por exemplo Russell-Elphic, Lee-Fu e Southwood-Scholer, e as propriedades dos eventos FTE, são apresentados de forma clara e concisa. Por fim, o artigo trata de um exemplo de identificação dos tubos em simulação computacional, realizado nesse trabalho.
In 1978 Christopher T. Russell and Richard C. Elphic detected magnetic flux tubes in magnetosphere boundary region through observational data obtained from the satellites ISEE-1 and −2, which were named flux transfer events (FTE). The solar wind together with the interplanetary magnetic field, both originated in the Sun, reach the earth magnetic field region, producing the magnetic reconnection and the flux tubes phenomena. This paper presents an introduction on the Sun-Earth connection and the magnetic reconnection process, and mainly the basic concepts on the tubes generation and formation. The models which describe the type of reconnection and the tubes signatures, for example, Russell-Elphic, Lee-Fu, Southwood-Scholer, and the FTE event characteristics, are presented in a clear and concise way. Finally, this paper brings an example of the tubes identification in computing simulation, performed in this work.

RESUMO Racional: Uma vez que lesão biliar ocorreu, o reparo é feito por hepaticojejunostomia. O procedimento mais comum é efetuar dilatação com balão com sucesso de 70%. As taxas de sucesso utilizando stents biodegradáveis é de 85% a 95%. Stents biliares biodegradáveis devem mudar o tratamento desta complicação. Objetivo: Investigar o uso de stents biodegradáveis em um grupo de pacientes com estenose hepaticojejunal Métodos: Em estudo prospectivo 16 stents biodegradáveis foram colocados em 13 pacientes com estenose de hepaticojejunostomia secundárias usados para reparação do ductos biliares de lesão cirúrgica. A média de idade foi de 38,7 anos (23-67), nove pacientes eram homens e quatro mulheres. Todos os casos tiveram drenagem percutânea antes do momento da colocação de stent biodegradável. Resultados: Em um caso, haemobilia temporária estava presente com necessidade de transfusão de sangue. Em outro, dor após a colocação do stent necessitou de medicação intravenosa. Nos outros 11 pacientes, alta hospitalar foi na manhã seguinte após o procedimento. Durante o seguimento, nenhum apresentou sintomas durante os primeiros nove meses. Um paciente apresentou significativa elevação da fosfatase alcalina por recidiva da estenose. Um caso teve recorrência de colangite 11 meses após a colocação do stent. Continuaram assintomáticos 84,6% com média de acompanhamento de 20 meses. Conclusão: A colocação de stents biodegradáveis é técnica segura e viável. Não foram observadas restrições causadas pelo stent ou pela sua degradação. Stent pode substituir dilatação com balão na estenose de hepaticojejunostomia.
ABSTRACT Background: Once a biliary injury has occurred, repair is done by a hepaticojejunostomy. The most common procedure is to perform a dilatation with balloon with a success of 70 %. Success rates range using biodegradable stents is from 85% to 95%. Biodegradable biliary stents should change the treatment of this complication. Aim: To investigate the use of biodegradable stents in a group of patients with hepaticojejunonostomy strictures. Methods: In a prospective study 16 biodegradable stents were placed in 13 patients with hepaticojejunostomy strictures secondary to bile duct repair of a biliary surgical injury. Average age was 38.7 years (23-67), nine were female and four male. All cases had a percutaneous drainage before at the time of biodegradable stent placement. Results: In one case, temporary haemobilia was present requiring blood transfusion. In another, pain after stent placement required intravenous medication. In the other 11 patients, hospital discharge was the next morning following stent placement. During the patient´s follow-up, none presented symptoms during the first nine months. One patient presented significant alkaline phosphatase elevation and stricture recurrence was confirmed. One case had recurrence of cholangitis 11 months after the stent placement. 84.6% continued asymptomatic with a mean follow-up of 20 months. Conclusion: The placement of biodegradable stents is a safe and feasible technique. Was not observed strictures caused by the stent or its degradation. It could substitute balloon dilation in strictures of hepaticojejunostomy.

Abstract The host status of eighteen progenies (CRP 01-12 to CRP 16-12, CRP 19-12 and CRP 20-12) and two cultivars (FB 200 Yellow Master and FB 300 Araguari) of yellow passion fruit to Meloidogyne incognita race 2 was evaluated in a greenhouse experiment. The height and the biomass of the plants, besides the numbers of galls, eggs and second-stage juveniles in roots and soil were assessed after 70 days of cultivation of the genotypes and tomato (control) in plastic pots with 2 kg of soil infested with 5,000 eggs of the nematode. The reproduction factor of the pathogen (RF = Final population/Initial population) was calculated for classifying the genotypes as immune (RF = 0), resistant (RF < 1) or susceptible (RF ≥ 1). The cultivar FB 200 is resistant to M. incognita race 2 and can be cultivated in areas infested with this nematode.

Palicourea marcgravii é considerada a principal planta tóxica de interesse pecuário no Brasil, porém, até o momento, é conhecido apenas o quadro agudo da intoxicação. Esse estudo avaliou as alterações clínico-patológicas de 10 caprinos cronicamente intoxicados por P. marcgravii. Os animais receberam, diariamente, 0,2g/kg de peso corporal da planta fresca por 6 a 38 dias. Os caprinos apresentaram como principais sinais clínicos anorexia, apatia, taquicardia, arritmia, pulso venoso jugular positivo e decúbito. Nas necropsias, os achados macroscópicos foram hidropericárdio, palidez dos rins e do miocárdio, atrofia gelatinosa da gordura cardíaca, evidenciação do padrão lobular hepático e edema pulmonar. Os principais achados microscópicos foram tumefação e vacuolização de cardiomiócitos, necrose de fibras cardíacas e infiltrado inflamatório mononuclear no miocárdio. Nos rins foi encontrada marcante degeneração hidrópico-vacuolar difusa. Os achados demonstraram nos caprinos cronicamente intoxicados, quadro clínico-patológico com características distintas do observado na forma aguda da intoxicação por P. marcgravii. Essas observações comprovam o risco para caprinos da ingestão da planta, mesmo que em pequenas doses, resultando no surgimento de quadro clínico e graves lesões ainda pouco conhecidas, e que poderiam ser confundidas com outras enfermidades.
Palicourea marcgravii is considered the main toxic plant for livestock in Brazil, however, only the acute intoxication has been reported. This study reports the clinical and pathological changes of 10 goats chronically intoxicated by P. marcgravii. The animals received, daily, 0.2g/kg body weight of the fresh plant during 6 to 38 days. The main clinical signs were anorexia, lethargy, tachycardia, arrhythmia, positive jugular venous pulse and recumbency. At necropsies, the lesions were pale kidneys and myocardium, serous atrophy of cardiac fat, evident hepatic lobular pattern and pulmonary edema. The main microscopic findings were swelling and vacuolization of cardiomyocytes, cardiac necrosis and inflammatory mononuclear infiltrate of the myocardium. The kidneys showed diffuse vacuolar hydropic degeneration of the epithelium. These findings demonstrated that goats chronically intoxicated by P. marcgravii show distinct clinicopathologic features than those observed in the acute form of the poisoning. It is suggested that goats may be spontaneously become intoxicated by the ingestion of small repeated doses of the plant and that the poisoning can be confused with other diseases.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Capuchin monkeys, Sapajus Kerr, 1792, are known for their flexible behavior, including tool use, and their ability to survive in urban forests. We observed capuchin juveniles using wood as hammer and anvil and different materials as sponges (four tool-use events) in two geographically distinct urban populations in Brazil, in 2012: two in Goiânia, Central Brazil and two in Foz do Iguaçu, Southern Brazil. In Goiânia, a male used a detached tree branch as a hammer and a buttress root as an anvil to pound a seed of Terminalia Linnaeus. Another male used a small branch with leaves as a dipping tool to access water inside a tree trunk hole. In Foz do Iguaçu, the capuchins used a small branch and a piece of bread to obtain water by dipping them into tree trunk holes. This latter event might be interpreted as a case of self-control, with a familiar food item used as a tool to reach a resource that is difficult to access otherwise. Our observations contribute to the knowledge on the tool-kit of capuchins and we propose that these urban populations should be conserved for scientific evaluations of behavioral flexibility in non-human primates.

Objetivos: O tratamento com GH é ainda controverso, salvo na deficiência grave de GH (SGHD). O objetivo deste estudo foi comparar a resposta ao tratamento com GH em indivíduos com insensibilidade parcial ao GH (PGHIS) e na deficiência moderada do GH (MGHD) com SGHD.Sujeitos e métodos: Quinze pacientes com PGHIS, 11 com MGHD e 19 com SGHD, seguidos por mais de um ano no Sistema Único de Saúde, foram avaliados antropométrica e laboratorialmente, no início, com um ano de tratamento e na última avaliação (tempo máximo de dez anos na SGHD, quatro anos na MGHD e oito anos na PGHIS).Resultados: O escore de desvio-padrão (EDP) da estatura inicial foi menor nos indivíduos com SGHD do que naqueles com MGHD e PGHIS. Embora o aumento no EDP da estatura no primeiro ano em comparação com o inicial não fosse diferente entre os grupos, o EDP da altura no primeiro ano de tratamento permaneceu menor na SGHD que na MGHD. Não houve diferença no EDP da estatura na última avaliação entre os três grupos. O tratamento com GH, no período completo da observação, provocou uma tendência a menor aumento no EDP da estatura nos pacientes com PGHIS que naqueles com SGHD, entretanto aumentos semelhantes foram encontrados nos grupos MGHD e SGHD.Conclusão: O tratamento com GH aumentou a estatura nos indivíduos com PGHIS e produziu efeitos similares na estatura em MGHD e SGHD.
Objectives: GH therapy is still controversial, except in severe GH deficiency (SGHD). The objective of this study was to compare the response to growth hormone (GH) therapy in children with partial GH insensitivity (PGHIS) and mild GH deficiency (MGHD) with those with SGHD.Subjects and methods: Fifteen PGHIS, 11 MGHD, and 19 SGHD subjects, followed up for more than one year in the Brazilian public care service, were evaluated regarding anthropometric and laboratory data at the beginning of treatment, after one year (1 st year) on treatment, and at the last assessment (up to ten years in SGHD, up to four years in MGHD, and up to eight years in PGHIS).Results: Initial height standard deviation score (SDS) in SGHD was lower than in MGHD and PGHIS. Although the increase in 1 st year height SDS in comparison to initial height SDS was not different among the groups, height-SDS after the first year of treatment remained lower in SGHD than in MGHD. There was no difference in height-SDS at the last assessment of the children among the three groups. GH therapy, in the entire period of observation, caused a trend towards lower increase in height SDS in PGHIS than SGHD but similar increases were observed in MGHD and SGHD.Conclusion: GH therapy increases height in PGHIS and produces similar height effects in MGHD and SGHD.
