Abstract INTRODUCTION: Brazil ranks 5th in the number of deaths due to road injuries. This study aimed to analyze mortality and disabilities resulting from road injuries in Brazil, and to assess the Sustainable Development Goals (SDG) target of reducing deaths due to road injuries by 50% by 2030. METHODS: This descriptive and exploratory study used the estimates from the Global Burden of Disease 2019: indicators of mortality, premature deaths, and disabilities according to sex, age group, and type of transport for 1990, 2015, and 2019. Time trends in mortality rates from 1990 to 2019 were assessed, and a projection for 2030 was calculated, applying a linear regression model. RESULTS: Deaths due to road injuries were 44,236 in 1990, and 44,529 in 2019, representing a 43% reduction in mortality rates. The highest rates were in the North, Northeast, and Midwest regions of Brazil, in males and young adults. A 77% reduction was observed in mortality rates for pedestrians and an increase of 53% for motorcyclists and of 54% for cyclists during the period. In terms of motorcycle road injuries, the mortality rate for men increased from 7.3/100,000 (1990) to 11.7/100,000 inhabitants (2019). The rates of premature deaths and disabilities were also higher for men when compared to women. Amputations, fractures, spinal cord injuries, and head trauma were the main types of road injuries. The projections for 2030 show that Brazil might not reach the SDG target. CONCLUSIONS: Despite the decline in mortality rates, the 2030 Agenda’s target might not be achieved.

RESUMO Racional: O II Consenso Brasileiro de Câncer Gástrico da Associação Brasileira de Câncer Gástrico ABCG (Parte 1) foi recentemente publicado. Nesta ocasião inúmeros especialistas que atuam no tratamento desta doença expressaram suas opiniões diante declarações apresentadas. Objetivo: Apresentar as Diretrizes da ABCG (Parte 2) quanto às indicações de tratamento cirúrgico, técnicas operatórias, extensão de ressecção e terapia combinada. Métodos: Para formulação destas diretrizes os autores realizaram extensa e atual revisão referente a cada declaração presente no II Consenso, utilizando as bases Medline/PubMed, Cochrane Library e SciELO, inicialmente com os seguintes descritores: câncer gástrico, gastrectomia, linfadenectomia, terapia combinada. Ainda, cada declaração foi classificada de acordo com o nível de evidência e grau de recomendação. Resultados: Das 43 declarações presentes neste estudo, 11 (25,6%) foram classificadas com nível de evidência A, 20 (46,5%) B e 12 (27,9%) C. Quanto ao grau de recomendação, 18 (41,9%) declarações obtiveram grau de recomendação 1, 14 (32,6%) 2a, 10 (23,3%) 2b e um (2,3%) 3. Conclusão: O complemento das diretrizes aqui presentes possibilita que cirurgiões e oncologistas que atuam no combate ao câncer gástrico possam oferecer o melhor tratamento possível, de acordo com as condições locais disponíveis.
ABSTRACT Background : The II Brazilian Consensus on Gastric Cancer of the Brazilian Gastric Cancer Association BGCA (Part 1) was recently published. On this occasion, countless specialists working in the treatment of this disease expressed their opinion in the face of the statements presented. Aim : To present the BGCA Guidelines (Part 2) regarding indications for surgical treatment, operative techniques, extension of resection and multimodal treatment. Methods: To formulate these guidelines, the authors carried out an extensive and current review regarding each declaration present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases initially with the following descriptors: gastric cancer, gastrectomy, lymphadenectomy, multimodal treatment. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results : Of the 43 statements present in this study, 11 (25,6%) were classified with level of evidence A, 20 (46,5%) B and 12 (27,9%) C. Regarding the degree of recommendation, 18 (41,9%) statements obtained grade of recommendation 1, 14 (32,6%) 2a, 10 (23,3%) 2b e one (2,3%) 3. Conclusion : The guidelines complement of the guidelines presented here allows surgeons and oncologists who work to combat gastric cancer to offer the best possible treatment, according to the local conditions available.

Ainda, cada declaração foi classificada de acordo com o nível de evidência e grau de recomendação. Resultados: Das 24 declarações, duas (8,3%) foram classificadas com nível de evidência A, 11 (45,8%) B e 11 (45,8%) C. Quanto ao grau de recomendação, seis (25%) declarações obtiveram grau de recomendação 1, nove (37,5%) grau 2a, seis (25%) 2b e três (12,5%) 3. Conclusão: As diretrizes aqui presentes têm a finalidade de auxiliar os profissionais que atuam no combate ao câncer gástrico com informações relevantes e atuais, permitindo que sejam aplicadas na prática médica diária.
ABSTRACT Background: The II Brazilian Consensus on Gastric Cancer by the Brazilian Gastric Cancer Association (ABCG) was recently published. On this occasion, several experts in gastric cancer expressed their opinion before the statements presented. Aim: To present the ABCG Guidelines (part 1) regarding the diagnosis, staging, endoscopic treatment and follow-up of gastric cancer patients. Methods: To forge these Guidelines, the authors carried out an extensive and current review regarding each statement present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases with the following descriptors: gastric cancer, staging, endoscopic treatment and follow-up. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results: Of the 24 statements, two (8.3%) were classified with level of evidence A, 11 (45.8%) with B and 11 (45.8%) with C. As for the degree of recommendation, six (25%) statements obtained grade of recommendation 1, nine (37.5%) recommendation 2a, six (25%) 2b and three (12.5%) grade 3. Conclusion: The guidelines presented here are intended to assist professionals working in the fight against gastric cancer with relevant and current information, granting them to be applied in the daily medical practice.

RESUMO Racional: Desde a publicação do primeiro Consenso Brasileiro sobre Câncer Gástrico em 2012 realizado pela Associação Brasileira de Câncer Gástrico (ABCG), novos conceitos sobre o diagnóstico, estadiamento, tratamento e seguimento foram incorporados. Objetivo: Promover uma atualização aos profissionais que atuam no combate ao câncer gástrico (CG) e fornecer diretrizes quanto ao manejo dos pacientes portadores desta afecção. Métodos: Cinquenta e nove especialistas responderam 67 declarações sobre o diagnóstico, estadiamento, tratamento e prognóstico do CG com cinco alternativas possíveis: 1) concordo plenamente; 2) concordo parcialmente; 3) indeciso; 4) discordo e 5) discordo fortemente. Foi considerado consenso a concordância de pelo menos 80% da soma das respostas “concordo plenamente” e “concordo parcialmente”. Este artigo apresenta apenas as respostas dos especialistas participantes. Os comentários sobre cada declaração, assim como uma revisão da literatura serão apresentados em publicações futuras. Resultados: Das 67 declarações, houve consenso em 50 (74%). Em 10 declarações, houve concordância de 100%. Conclusão: O tratamento do câncer gástrico evoluiu consideravelmente nos últimos anos. Este consenso reúne princípios consolidados nas últimas décadas, novos conhecimentos adquiridos recentemente, assim como perspectivas promissoras sobre o manejo desta doença.
ABSTRACT Background: Since the publication of the first Brazilian Consensus on Gastric Cancer (GC) in 2012 carried out by the Brazilian Gastric Cancer Association, new concepts on diagnosis, staging, treatment and follow-up have been incorporated. Aim: This new consensus is to promote an update to professionals working in the fight against GC and to provide guidelines for the management of patients with this condition. Methods: Fifty-nine experts answered 67 statements regarding the diagnosis, staging, treatment and prognosis of GC with five possible alternatives: 1) fully agree; 2) partially agree; 3) undecided; 4) disagree and 5) strongly disagree A consensus was adopted when at least 80% of the sum of the answers “fully agree” and “partially agree” was reached. This article presents only the responses of the participating experts. Comments on each statement, as well as a literature review, will be presented in future publications. Results: Of the 67 statements, there was consensus in 50 (74%). In 10 declarations, there was 100% agreement. Conclusion: The gastric cancer treatment has evolved considerably in recent years. This consensus gathers consolidated principles in the last decades, new knowledge acquired recently, as well as promising perspectives on the management of this disease.

AN APPROACH TO THE KINETICS AND THERMODYNAMICS OF ELEMENTARY CHEMICAL REACTIONS USING A STOCHASTIC MODEL

A mathematical model is proposed to investigate the kinetics and equilibrium of homogeneous elementary first- and second-order chemical reactions. The dynamics is defined by the Monte Carlo method (MCM), with the Metropolis update, and the Ehrenfest urn model (EUM). MCM is an important step that accesses the kinetic and thermodynamic properties of the system, while the EUM defines the orders of the reactions studied in this work. The main parameters, such as temperature, the activation energy and the steric factor, were taken into account in the calculation of the transition probabilities between the reactants and products. It is thus possible to reproduce the kinetic profiles of the reactions and to evaluate the influence of temperature and the steric factor. Furthermore, it is possible to simulate the behaviour of the system by modifying the activation energy barrier, thus simulating a catalytic process. The effect of the addition of molecules was also investigated, with the system returning to a thermodynamic equilibrium condition after partial consumption of the added reactant, as predicted by Le Chatelier’s principle. All the simulated data are in agreement with the theoretical results present in physical chemistry textbooks.

Abstract INTRODUCTION Brazil has the largest number of snakebite cases in South America, of which the large majority is concentrated in the Midwest and North. METHODS In this descriptive observational study, we assessed the epidemiological and clinical snakebite cases referred to the Centro de Medicina Tropical de Rondônia from September 2008 to September 2010. RESULTS We followed up 92 cases from admission until discharge, namely 81 (88%) men and 11 (12%) women, with a mean age of 37 years, and mainly from rural areas (91.3%). The snakebites occurred while performing work activities (63%) during the Amazon rainy season (78.3%). The vast majority of individuals presented from the Porto Velho microregion (84.7%). Approximately 95.6% of the snakebites were caused by snakes of the genus Bothrops, followed by two lachetics and two elapidics cases. Surgery was performed in 10 cases (9 fasciotomies in the lower limb and 1 amputation). No deaths were reported in this study, but 4 cases (4.3%) developed sequelae in the lower limb. CONCLUSIONS This study can contribute to a better understanding of envenomation in the state of Rondônia and thus can be useful for identifying real conditions that can increase the incidence of snakebites in this region. Moreover, the study results can serve as a basis for improving educational campaigns designed to prevent these types of snakebites, as well as for preserving snakes.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Several reports describing Superconducting Fault Current Limiter (SFCL) containing members of the bismuth or yttrium ceramics were already described. However, none of these included the mercury and samarium cuprates. Consequently, we have conducted a study of a resistive-type superconductor fault current limiter based on Hg0.82Re0.18Ba2Ca2Cu3O8+d samples and SmBa2Cu3O7-d ceramics. All polycrystalline samples were prepared by solid-state reaction method using commercial oxide and carbonate powders. The superconducting ceramic with 2.4 × 2.6 × 6.0 mm³ dimensions was set up such that the electrical current flew through the area of 2.6 × 6.0 mm². All measurements were done at 77K and without applied magnetic field (Happ = 0). In the case mercury sample, the fault current of 16.1 Apeak was reduced to 8.1 Apeak by the superconducting element and sustained for 100 ms. The prospective/limited of current ratio observed in this experiment was ~ 2.0, as considered a 2.4 mm pellet thickness. When the test was realized with the samarium sample, the prospective/limited current ratio observed was approximately 1.2, for same ceramic thickness. The tests confirmed the capability of the resistive type SFCL to limit the fault current. These results open the possibility of future investigations into SFCL devices based on those superconducting ceramics in low power electronics and electrical motor.

By using the Monte Carlo simulation platform with probabilistic mathematical functions of the Boltzmann type, <img src="/img/revistas/qn/v36n5/a20res.jpg" align="absmiddle"/>, having activation energy and temperature as parameters, it was possible to assess important dynamic aspects of homogeneous chemical reactions of the types A → B and A <img src="/img/revistas/qn/v36n5/20s01.jpg" align="absmiddle"/> B. The protocol proved a useful tool in work with the basic concepts of Kinetics and Thermodynamics allowing its application both in class activities and for assisting experimental procedures.

The synthesis of fine chemicals intermediates using Friedel-Crafts acylation is one of the most important methods in chemical technology. In this work, the acylation of 2-methoxynaphthalene with acetic anhydride using a silica-supported dodecatungstophosphoric acid catalyst (HPW/SiO2) and acetonitrila as solvent was studied, showing that this reaction is a feasible alternative to produce intermediaries to replace the current methods of production. The reactions using acetonitrile solvent showed yields greater than or equal to the reactions using traditional solvents such as nitrobenzene and dichloroethane. Finally, the modified Eley-Rideal mechanism was proposed to elucidate the experimental data obtained.

INTRODUÇÃO: A valvoplastia aórtica por cateter-balão em neonatos e lactentes ainda é um procedimento discutido na literatura. Neste artigo são relatados os resultados imediatos desse procedimento realizado pela via carotídea. MÉTODO: Estudo longitudinal de uma coorte de pacientes com estenose valvar aórtica grave ou crítica congênita com até seis meses de idade, nos quais foi realizada valvuloplastia aórtica com cateter-balão no período de 1997 a 2008. RESULTADOS: Entre março de 1997 e março de 2008, foram realizados 25 procedimentos em 24 pacientes (79% do sexo masculino), com média de idade de 1,4 ± 1 mês (0,1-4 meses) e média de peso de 3,7 ± 1,2 kg (1,1-5,2 kg). A relação balão-anel foi de 0,93 ± 0,1. Óbitos relacionados ao procedimento ocorreram em 4 pacientes (17%), três por falência ventricular esquerda e um por dissecção do seio de Valsalva da artéria coronária esquerda durante o posicionamento do guia e introdutor antes da realização da valvoplastia. O gradiente sistólico máximo transvalvar à ecocardiografia caiu de 86 ± 25 mmHg para 36 ± 15 mmHg, enquanto o gradiente pico-a-pico caiu de 61 ± 36 mmHg para 29 ± 17 mmHg após o procedimento. Melhora significativa da função ventricular foi observada nos pacientes com disfunção prévia. Houve aparecimento ou piora de insuficiência aórtica em 10 pacientes, não sendo necessária intervenção cirúrgica imediata. Fibrilação ou taquicardia ventricular foi observada em 4 pacientes. No seguimento de 7 pacientes, 2 necessitaram de cirurgia valvar. CONCLUSÕES: Embora ainda seja um procedimento associado a morbidade e mortalidade significativas, a valvoplastia aórtica é eficaz na redução do gradiente transvalvar e na preservação da função ventricular e é capaz de postergar a intervenção cirúrgica.
BACKGROUND: Balloon aortic valvuloplasty still remains a debatable procedure in the literature. In this paper, we report the immediate results of this procedure using the carotid access. METHOD: Longitudinal study of a cohort of patients with severe or critical congenital aortic valvular stenosis with up to six months of age treated with balloon aortic valvuloplasty from 1997 to 2008. RESULTS: Between March 1997 and March 2008, 25 procedures were performed in 24 patients (79% males) with mean age of 1.4 ± 1.0 months (0.1-4 months) and mean weight of 3.7 ± 1.2 kg (1.1-5.2 kg). The balloon-annulus ratio was 0.93 ± 0.1. There were 4 procedure-related deaths (17%), three of them due to left ventricular dysfunction and one due to the dissection of the left sinus of Valsalva during positioning of the guide wire and sheath prior valvuloplasty. The peak transvalvar systolic gradient at echocardiography decreased from 86 ± 25 mmHg to 36 ± 15 mmHg whereas the peak-to-peak gradient decreased from 61 ± 36 mmHg to 29 ± 17 mmHg after the procedure. There was significant improvement of ventricular function in patients with previous dysfunction. There was worsening or de-novo aortic failure in 10 patients, which did not require immediate surgical intervention. Ventricular tachycardia or ventricular fibrillation was observed in 4 patients. During follow-up, valve surgery was required in 2 of 7 patients. CONCLUSIONS: Although still associated to significant morbidity and mortality, balloon aortic valvuloplasty is effective in reducing transvalvar gradient and in preserving left ventricular function and may delay the need for surgical intervention.

INTRODUÇÃO: O acúmulo de conhecimentos tem sobrecarregado o conteúdo curricular do curso de graduação médica. As escolas médicas devem estar preocupadas em definir um conteúdo temático mínimo, relacionado com os transtornos mais frequentes; buscamos sugerir o conteúdo temático mínimo, para o curso de Neurologia na graduação médica. MÉTODO: 1. Identificamos os locais de trabalho dos médicos jovens, fora do Hospital Escola(HE) pela análise de resposta às cartas enviadas a 6415 médicos residentes (MR) do Estado de São Paulo e de 201 entrevistas pessoais com MR; 2. Verificamos quais os transtornos neuro-psiquiátricos (TNP) mais frequentes na população, através da análise dos registros diagnósticos de pacientes que procuraram o Pronto Socorro (PS) de três instituições: Municipal de Taubaté-SP, Municipal de São José dos Campos-SP e Faculdade de Medicina da Santa Casa de Misericórdia de São Paulo-SP. RESULTADOS: 1. Os MR são jovens e, fora do HE, trabalham em PS. 2.Os diagnósticos mais frequentes nas três instituições foram: alcoolismo, cefaléia, coma, demência, desmaio, doença encéfalo-vascular, epilepsia, hemiplegias ou paraplegias, meningite, neuropatia periférica, paralisia facial, tontura, transtornos psíquicos e traumatismo craniencefálico. CONCLUSÃO: Estes diagnósticos mais frequentes são os temas relevantes para o conteúdo programático do curso de Neurologia na graduação médica.
INTRODUCTION: Knowledge accumulation is overfilling the thematic content of medical graduation. Medical Schools must be alert to define a minimal content related with the most frequent disturbs. We intent to suggest topics for the minimal content, to the program of Neurology in medical graduation. METHOD: 1. To identify the places where young doctors are working outside the School Hospital (SH): we analysed the answers of the letters sent to 6415 resident - doctors (RD) in São Paulo's State and we made personal interview with 201 RD. 2. To verify the most frequent neuropsichiatric disturbs (ND) in the population: we made the analysis of the patient's diagnostic records in emergency room of three institutions: Municipal of Taubaté-SP, Municipal of São José dos Campos-SP and Faculty of Medicine of São Paulo Santa Casa de Misericórdia. RESULTS: 1. The RD are young and, outside the SH, they work in other emergency services. 2. The most frequent diagnosis in institutions were listed: alcoholism, cerebrovascular disease, coma, cranial trauma, dementia, dizziness, epilepsy, facial paralysis, faint, headache, hemiplegia or paraplegia, meningitis, others paralysis, periferical neuropathy and psychiatric disordes. CONCLUSIONS: Those diagnostics most frequent are relevant topics of the programmatic content to the program of Neurology in medical graduation.
