A cardiomiopatia hipertrófica (CMH) é uma forma de doença do músculo cardíaco de causa genética, caracterizada pela hipertrofia das paredes ventriculares. O diagnóstico requer detecção por métodos de imagem (Ecocardiograma ou Ressonância Magnética Cardíaca) de qualquer segmento da parede do ventrículo esquerdo com espessura > 15 mm, sem outra causa provável. A análise genética permite identificar mutações de genes codificantes de diferentes estruturas do sarcômero responsáveis pelo desenvolvimento da CMH em cerca de 60% dos casos, permitindo o rastreio de familiares e aconselhamento genético, como parte importante do manejo dos pacientes e familiares. Vários conceitos sobre a CMH foram recentemente revistos, incluindo sua prevalência de 1 em 250 indivíduos, não sendo, portanto, uma doença rara, mas subdiagnosticada. A vasta maioria dos pacientes é assintomática. Naqueles sintomáticos, a obstrução do trato de saída do ventrículo esquerdo (OTSVE) é o principal distúrbio responsável pelos sintomas, devendo-se investigar a sua presença em todos os casos. Naqueles em que o ecocardiograma em repouso ou com Manobra de Valsalva não detecta gradiente intraventricular significativo (> 30 mmHg), devem ser submetidos à ecocardiografia com esforço físico para detecção da OTSVE. Pacientes com sintomas limitantes e grave OTSVE, refratários ao uso de betabloqueadores e verapamil, devem receber terapias de redução septal ou uso de novas drogas inibidoras da miosina cardíaca. Por fim, os pacientes adequadamente identificados com risco aumentado de morta súbita podem receber medida profilática com implante de cardiodesfibrilador implantável (CDI).
La miocardiopatía hipertrófica (MCH) es una forma de enfermedad cardíaca de origen genético, caracterizada por el engrosamiento de las paredes ventriculares. El diagnóstico requiere la detección mediante métodos de imagen (Ecocardiograma o Resonancia Magnética Cardíaca) que muestren algún segmento de la pared ventricular izquierda con un grosor > 15 mm, sin otra causa probable. El análisis genético permite identificar mutaciones en genes que codifican diferentes estructuras del sarcómero responsables del desarrollo de la MCH en aproximadamente el 60% de los casos, lo que permite el tamizaje de familiares y el asesoramiento genético, como parte importante del manejo de pacientes y familiares. Varios conceptos sobre la MCH han sido revisados recientemente, incluida su prevalencia de 1 entre 250 individuos, por lo tanto, no es una enfermedad rara, sino subdiagnosticada. La gran mayoría de los pacientes son asintomáticos. En los casos sintomáticos, la obstrucción del tracto de salida ventricular izquierdo (TSVI) es el trastorno principal responsable de los síntomas, y su presencia debe investigarse en todos los casos. En aquellos en los que el ecocardiograma en reposo o la maniobra de Valsalva no detecta un gradiente intraventricular significativo (> 30 mmHg), deben someterse a ecocardiografía de esfuerzo para detectar la obstrucción del TSVI. Los pacientes con síntomas limitantes y obstrucción grave del TSVI, refractarios al uso de betabloqueantes y verapamilo, deben recibir terapias de reducción septal o usar nuevos medicamentos inhibidores de la miosina cardíaca. Finalmente, los pacientes adecuadamente identificados con un riesgo aumentado de muerte súbita pueden recibir medidas profilácticas con el implante de un cardioversor-desfibrilador implantable (CDI).
Hypertrophic cardiomyopathy (HCM) is a form of genetically caused heart muscle disease, characterized by the thickening of the ventricular walls. Diagnosis requires detection through imaging methods (Echocardiogram or Cardiac Magnetic Resonance) showing any segment of the left ventricular wall with a thickness > 15 mm, without any other probable cause. Genetic analysis allows the identification of mutations in genes encoding different structures of the sarcomere responsible for the development of HCM in about 60% of cases, enabling screening of family members and genetic counseling, as an important part of patient and family management. Several concepts about HCM have recently been reviewed, including its prevalence of 1 in 250 individuals, hence not a rare but rather underdiagnosed disease. The vast majority of patients are asymptomatic. In symptomatic cases, obstruction of the left ventricular outflow tract (LVOT) is the primary disorder responsible for symptoms, and its presence should be investigated in all cases. In those where resting echocardiogram or Valsalva maneuver does not detect significant intraventricular gradient (> 30 mmHg), they should undergo stress echocardiography to detect LVOT obstruction. Patients with limiting symptoms and severe LVOT obstruction, refractory to beta-blockers and verapamil, should receive septal reduction therapies or use new drugs inhibiting cardiac myosin. Finally, appropriately identified patients at increased risk of sudden death may receive prophylactic measure with implantable cardioverter-defibrillator (ICD) implantation.

ABSTRACT The expression ‘you need to know to conserve’ is a well-known cliche among biologists. Documenting the richness of a group of organisms is the first step towards understanding biodiversity and preparing efficient conservation plans. In this context, many efforts have been made to quantify the number of species on Earth and estimate the number of species still unknown to science. A few countries have complete and integrated databases estimating the approximate number of species recorded for their territory, particularly in the Global South. In Brazil, a country of continental dimensions, revealing the richness of the second most diverse clade of invertebrates (=Mollusca) has been a goal of taxonomists. Recently, in an unprecedented, collective, and integrated effort among Brazilian malacologists, it was possible to estimate how many valid species of molluscs are there in Brazil. In this effort, more than 30 mollusc experts joined together to update the Taxonomic Catalogue of the Brazilian Fauna (TCBF), a governmental website that allows a quick and real-time updating of all Metazoan. So far, more than 5,000 updates have been made in TCBF, indicating the presence of 3,552 valid species of molluscs in Brazil, distributed among the main clades as follows: Caudofoveata (10 spp.), Solenogastres (6 spp.), Polyplacophora (35 spp.), Scaphopoda (43 spp.), Cephalopoda (92 spp.), Bivalvia (629 spp.) and Gastropoda (2,737 spp.). The present study, in addition to demonstrating for the first time the richness of Brazilian molluscs, also presents the state of the art of this important phylum of invertebrates highlighting its most representative and neglected groups.

Selection of natural hybrids of ‘Ubá’ mango from Zona da Mata, Minas Gerais state, Brazil

Abstract The ‘Ubá’ mango tree is found in all the cities of the Zona da Mata and its fruits are destined for both in natura consumption and for processing. However, there is a great heterogeneity among plants in the orchards, which contributes to low productivity. This study aimed to evaluate and select superior accessions of ‘Ubá’ mango trees collected in Ubá, Visconde do Rio Branco, and Viçosa, based on agronomic characteristics, aiming to produce high-quality fruits. The experiment was installed in Visconde do Rio Branco, MG. We evaluated 195 ‘Ubá’ mango tree accessions, in a completely randomized design, with four replications, of one tree each. The productions of the first five crops (mass and number of fruits, and alternate bearing index - per plant), and the fruit quality characteristics from the 2017-2018 crop (fruit, peel, seed and pulp masses; length, ventral and transversal diameters, peel and pulp color index, soluble solids content, titratable acidity, vitamin C and total carotenoids content of the pulp) were evaluated. Seventy-eight accessions of ‘Ubá’ mango trees were selected, 24 were based on accumulated production, over 200 kg of fruits per plant, and 59 accessions were based on fruit quality characteristics.
Resumo A mangueira ‘Ubá’ é encontrada em todos os municípios da Zona da Mata, e seus frutos são destinados tanto para consumo in natura como para o processamento. No entanto, há grande heterogeneidade entre plantas nos pomares, o que contribui para a baixa produtividade da cultura. O objetivo deste trabalho foi avaliar e selecionar acessos superiores de mangueiras ‘Ubá’ coletados em Ubá, Visconde do Rio Branco e Viçosa, com base em características agronômicas, visando à produção de frutos de qualidade. O experimento foi instalado em Visconde do Rio Branco-MG. Foram avaliados 195 acessos de mangueira ‘Ubá’, em delineamento inteiramente casualizado, com quatro repetições, de uma árvore cada. Foram avaliadas as produções das cinco primeiras safras (massa, número de frutos e índice de alternância de produção por planta) e as características de qualidade dos frutos, da safra 2017-2018 (massa do fruto, da casca, da semente e da polpa, comprimento, diâmetro ventral e transversal, índice de cor de casca e polpa, teor de sólidos solúveis, acidez titulável, vitamina C e teor de carotenoides totais da polpa). Foram pré-selecionados 78 acessos de mangueiras ‘Ubá’, sendo 24 com base em produção acumulada superior a 200 kg de frutos por planta e 59 acessos com base em características de qualidade dos frutos.

Resumo Apesar das dificuldades encontradas em se realizar análises cromossômicas em peixes de pequeno porte, os estudos citogenéticos em Lebiasinidae vêm crescendo nos últimos anos e demonstrando padrões diferenciados na evolução cromossômica entre os membros da família. Nesse contexto, o gênero Lebiasina tem mostrado preservar sua macroestrutura cariotípica, composta por 2n = 36 cromossomos, enquanto os demais gêneros geralmente apresentam 2n maiores. Este estudo tem como foco a citogenética comparativa de três espécies de Lebiasina, sendo uma delas analisada pela primeira vez aqui, através do emprego de técnicas convencionais e moleculares. Os resultados obtidos reforçam a trajetória evolutiva diferenciada do gênero Lebiasina, ao mesmo tempo em que evidenciam as particularidades genômicas que acompanham a evolução de cada uma das espécies. Neste contexto, os componentes repetitivos do genoma tiveram um papel importante na caracterização particular de cada uma das espécies. Também, é notável que L. minuta e L. melanoguttata, duas espécies que ocorrem exclusivamente no território brasileiro, apresentam maior proximidade citogenética entre elas do que com a espécie irmã transandina, L. bimaculata.
Abstract Despite several difficulties in chromosomal analyses of small-sized fishes, the cytogenetics of the Lebiasinidae was largely improved in the last years, showing differential patterns in the chromosomal evolution inside the family. In this context, it has been shown that genus Lebiasina preserves its karyotypic macrostructure, composed of 2n = 36 chromosomes, whereas the other genera generally present higher 2n. This study focused on the comparative cytogenetics of three Lebiasina species, one of them analyzed here for the first time, using conventional and molecular procedures. The results reinforced the differentiated evolutionary path of the genus Lebiasina while, at the same time, highlighted the genomic particularities that have accompanied the evolution of each species. In this sense, the repetitive components of the genome played a significant role in the differentiation of each species. It is also notable that L. minuta and L. melanoguttata, the two species that occur exclusively in the Brazilian territory, show greater chromosomal similarities to each other than to the trans-Andean sister species, L. bimaculata.

Genetic evaluation and selection of cocoa tree clones

RESUMO: O cultivo do cacau tem grande importância socioeconômica mundial. Os amêndoas de cacau são a matéria-prima essencial para a produção de chocolate. A variabilidade do cacaueiro avaliada representa apenas uma pequena fração da diversidade genética existente, principalmente na região amazônica. Além disso, apenas uma pequena parte dessa variabilidade foi explorada no melhoramento de cacau. Dessa forma, o presente estudo teve como objetivo realizar a avaliação genética e seleção de clones de cacau, com base em características morfo-agronômicas. Para isso, foram avaliados 145 clones, durante 37 colheitas, em quatro anos consecutivos. As seguintes características de frutos e sementes foram avaliadas: número total de frutos colhidos (NTFC), número total de frutos sadios (NTFS), peso de sementes úmidas dos frutos sadios (PSUFS), peso médio de sementes úmidas dos frutos sadios (PMSUFS), peso de sementes secas dos frutos sadios (PSSFS), peso médio de sementes secas dos frutos sadios (PMSSFS), porcentagem de frutos com vassoura-de-bruxa (PFVB), porcentagem de frutos com broca (PFBR), porcentagem de frutos com sementes germinadas (PFSG), número de ramos com vassoura-de-bruxa (NRVB) e número de almofadas florais com vassoura-de-bruxa (NAFVB). Diferenças significativas (P < 0,05) entre os clones foram observadas para todas as características, o que revela uma expressiva variabilidade e possibilidade de ganhos com a seleção. As maiores correlações significativas (P < 0,05) ocorreram entre os caracteres NTFC e NTFS (0,94), NTFS e PMSUFS (0,86) e, NTFS e PMSSFS (0,86). Com base no índice de seleção, os clones POUND 12 e CAB 12, 228, 253, 257, 258 e 422 foram considerados os mais adequados para a seleção.
ABSTRACT: The cultivation of cocoa is of great socio-economic importance worldwide. Cocoa beans are the essential raw material for chocolate production. The variability of cacao studied presents only a small fraction of the existing genetic diversity, mainly in the Amazon region. Furthermore, just a small part of this variability has been exploited in cocoa breeding. Thus, the present study processed the genetic evaluation and selection of cocoa clones, based on morpho-agronomical traits. For this, we evaluated 145 clones, during 37 harvests from, four consecutive years. The following traits were evaluated: total number of fruits collected (TNFC), total number of healthy fruits (TNHF), weight of wet seeds from healthy fruits (WWSHF), average weight of wet seeds per healthy fruit (AWWSHF), weight of dry seeds from healthy fruit (WDSHF), average weight of dry seeds per healthy fruit (AWDSHF), percentage of fruits with witches’ broom disease (PFWB), percentage of fruits with borer (PFBR), percentage of fruits with germinated seeds (PFGS), number of branches with witches’ broom disease (NBWB), and number of inflorescence with witches’ broom disease (NIWB). Significant differences (P < 0.05) among the clones were observed for all traits, which reveal an expressive variability and possibility of gains with selection. The highest significant correlations (P < 0.05) occurred between traits TNFC and TNHF (0.94), TNHF and AWWSHF (0.86), and TNHF and AWDSHF (0.86). Based on the selection index, the clones POUND 12 and CAB 12, 228, 253, 257, 258, and 422 were the most suitable for selection.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Abstract In canine visceral leishmaniasis, coinfections can aggravate the disease. Our aim was to investigate Brucella canis in dogs infected with Leishmania infantum. One hundred and six L. infantum-seropositive dogs were submitted to serology for B. canis, PCR for B. canis and L. infantum, and histopathological analysis of the genital tract. Anti-B. canis antibodies were detected in seven dogs whose clinical signs, L. infantum load and histological alterations were similar to those of seronegative animals. The circulation of anti-B. canis antibodies was low but demonstrates the exposure of dogs to this bacterium in a visceral leishmaniasis-endemic area.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Resumo A leishmaniose visceral canina (LVC) é uma doença zoonótica de alta letalidade causada por Leishmania infantum nas Américas. No cão infectado, as formas amastigotas são escassas no sangue, principalmente na fase tardia da doença. Este estudo teve como objetivo relatar um caso raro de amastigotas de L. infantum encontradas em neutrófilos do sangue periférico de um cão naturalmente infectado e terminal da LVC, descrevendo também seu estado clínico antes e após o tratamento com miltefosina a 2%. O cão, que se apresentou como polissintomático e com sinais e sintomas clássicos da LVC foi submetido aos seguintes testes: teste rápido Dual Path Platform (DPP), ELISA e exame parasitológico de sangue periférico. Os parâmetros hematológicos e bioquímicos foram obtidos antes e após o tratamento. Todos os testes diagnósticos foram positivos para LVC. A identificação de formas amastigotas de L. infantum, dentro de neutrófilos do sangue periférico foi confirmada por microscopia, e a espécie foi confirmada por análise molecular. Ao final do tratamento, não foi detectada parasitemia periférica, observando-se melhora dos parâmetros clínicos e laboratoriais. Por fim, esse achado atípico pode ser usado como exemplo para levantar discussões sobre o real papel imunológico dos neutrófilos nas fases tardias da LVC e suas implicações clínicas/terapêuticas.
Abstract Canine visceral leishmaniasis (CVL) is a zoonotic disease of high lethality caused by Leishmania infantum in the Americas. In the infected dog, the amastigotes are scarce in blood, especially in the late phase of the disease. This study aimed to report a rare case of L. infantum amastigotes found in neutrophils from peripheral blood of a naturally infected dog in terminal phase of CVL, also describing its clinical status before and after treatment with miltefosine 2%. The dog, which presented as polysymptomatic and with classical signs and symptoms of CVL was submitted to the following tests: Dual Path Platform (DPP) rapid test, ELISA and parasitological examination of peripheral blood. Hematological and biochemical parameters were obtained before and after treatment. All diagnostic tests were positive for CVL. The identification of L. infantum amastigotes inside neutrophils from peripheral blood was confirmed through microscopy, and the species was confirmed by molecular analysis. At the end of the treatment, peripheral parasitemia was not detected, and improvements were observed in clinical and laboratorial parameters. Finally, this atypical finding can be used as example to raise discussions about the real immunological role of neutrophils in late phases of CVL and its clinical/therapeutic implications.

The aim of this study was evaluate the in vitro antifungal activity of crude extracts from Eugenia uniflora, Libidibia ferrea and Psidium guajava. The extracts were obtained by turbo-extraction using water (AQ) or acetone-water (AC-W) (7:3, v/v) as solvents and lyophilized to obtain the crude extracts (CE). The CE were characterized by UV-Vis, TLC and HPLC. The activity of CEs was investigated against clinical isolates of Candida spp. and the Minimum Inhibitory Concentration (MIC), MIC50 and MIC90 were determinated. The analysis by TLC showed that all CEs presented polyphenols (flavonoids and tannins). The CEs from E. uniflora showed higher amount of polyphenols (30.35 ± 2.15%, AC-W) and the HPLC analysis revealed the tannins in all extracts. The CEs of E. uniflora showed MIC range from 1.9 to 500.0 µg/mL, and lower values of MIC50 and MIC90 against non-albicans Candida isolates. Regarding L. ferrea and P. guajava, the results showing MIC from 3.9 to 1000.0 µg/mL (CE-AQ) against C. albicans. The results demonstrate antifungal performance from CE against various species of Candida spp., suggesting that the herbal species may be use as new potential antifungal agents. Additionally, the polyphenol content can play a pivotal role in the antifungal properties of CE.

ABSTRACT Background: Hepatitis C is an important health problem. In Brazil, 1-2 million people are infected. Despite this expressive number, and the availability of very successful treatment, many patients remained undiagnosed mainly because of the asymptomatic nature of the infection. Objectives: To describe epidemiological characteristics of HCV-infected patients seen at referral centers in Brazil, the source of referral, and the time spanned to reach a reference center, in order to improve the identification of undiagnosed patients. Methods: Multicenter observational, cross-sectional study carried out in 15 centers of Brazil, between January/2016 and June/2017. Data of patients with a confirmed diagnosis (anti-HCV and HCV-RNA) were collected by interview using standard questionnaires and by review of charts. Results: Two thousand patients were included; 55.1% were male, mean age 58 ± 11 years. Only 14.9% had higher education and 84.2% received up to five monthly minimum Brazilian wages (approximately US$260.00/month). The time between diagnosis and beginning of follow-up was 22.9 months. The most common reasons for testing were check-up (33.2%) and blood donation (19%). General practitioners diagnosed most of the patients (30.1%). Fibrosis stage was mainly evaluated by liver biopsy (61.5%) and 31.3% of the patients were cirrhotic at diagnosis. Conclusions: This multicenter Brazilian study showed that the mean time to reach a referral center for treatment was almost two years. Primary care physicians diagnoses most hepatitis C cases in the country. Population campaigns and medical education should be encouraged to intensify screening of asymptomatic individuals, considering the efficiency of check-ups in identifying new patients.

Abstract: Hymenaea courbaril has been used to treat different diseases, although its properties are yet to be scientifically validated. The objective of this study was to determine the cytotoxicity, genotoxicity, antigenotoxicity and antioxidant potentials of hydroethanolic extract from H. courbaril seeds. Therefore, for the cytotoxicity test an anti-melanoma assay was performed in B16F10 strain cells. The genotoxicity and antigenotoxicity was evaluated in bone marrow cells (Permit number: 002/2010) of mice, the antioxidant activity was determined by the DPPH test and the total flavonoid content was also determined. The hydroethanolic extract showed antigenotoxic effect and antioxidant activity. It was verified that total flavonoid content was 442.25±18.03 mg RE/g dry extract. HPLC-PAD chromatogram revealed presence of flavones as majority compound in evaluated extract. The results allowed us to also infer that the hydroethanolic extract from seeds shows cytotoxic activity against B16F10 melanoma cells line and it has dose-and-time-dependency.

RESUMO Mecanismos de regeneração natural são fundamentais para o restabelecimento das funções ecológicas de fragmentos florestais e de áreas degradadas. O presente trabalho teve como objetivo avaliar a chuva de sementes e o estabelecimento de plântulas de espécies arbóreas em Floresta Atlântica, localizada na região Nordeste do Brasil. Para amostragem da chuva de sementes foram alocados 60 coletores (área de 0,196 m2), ficando equidistantes, aproximadamente, de 50 m. As coletas foram realizadas com intervalo de 30 dias por um período de 12 meses. Para avaliar o estabelecimento de plântulas foram alocadas subparcelas ao lado dos coletores, totalizando 60 subparcelas de 0,25 m2 cada. Os indivíduos arbóreos identificados pelos diásporos coletados na chuva de sementes e pelas plântulas estabelecidas no fragmento foram contados e classificados quanto à síndrome de dispersão e grupo sucessional. A chuva de sementes foi representada por 60 morfoespécies e 20 famílias botânicas. O grupo ecológico e a síndrome de dispersão predominante foram de espécies secundárias iniciais e zoocórica, respectivamente. Em relação às plântulas estabelecidas foram contadas 122 plântulas, sendo Tapirira guianensis L. com maior número de plântulas. A chuva de sementes apresenta potencial para manutenção da dinâmica florestal por apresentar alta riqueza de espécies que compõe a atual vegetação, sendo composta, predominantemente, por espécies que produzem frutos abundantes com dispersão zoocórica que servem de alimento para fauna. Enquanto poucas sementes germinam para formar o banco de plântulas, a maioria está contribuindo para formação do banco de sementes.
ABSTRACT Mechanisms of natural regeneration are fundamental for ecological function restoration in forest fragments and degraded areas. The present work aimed to evaluate the seed rain and the establishment of seedlings of arboreal species in the Atlantic Forest, located in the northeast of Brazil. For sampling of the seed rain, 60 collectors were allocated (area of 0.196 m2), being equidistant, approximately, 50 m. The samples were collected at intervals of 30 days for a period of 12 months. To evaluate the establishment of seedlings, subplots were allocated next to the collectors, totaling 60 subplots of 0.25 m2 each. The arboreal individuals identified by the diaspores collected in the seed rain and by the seedlings established in the fragment were counted and classified for the dispersion syndrome and successional group. The seed rain was represented by 60 morphospecies and 20 botanical families. The ecological group and the predominant dispersion syndrome were of early secondary and zoocoric species, respectively. In relation to the established seedlings, 122 seedlings were counted, being Tapirira guianensis L. with higher number of seedlings. The rainfall of seeds presents potential for the maintenance of the forest dynamics due to the high species richness that compose the current vegetation. Being predominantly composed of species that produce abundant fruits with zoocoric dispersion that serve as food for fauna. While few seeds germinate to form the seedling bank, most of them are contributing to seed bank formation.

Support vector machine-based classification of neuroimages in Alzheimer’s disease: direct comparison of FDG-PET, rCBF-SPECT and MRI data acquired from the same individuals

Objective: To conduct the first support vector machine (SVM)-based study comparing the diagnostic accuracy of T1-weighted magnetic resonance imaging (T1-MRI), F-fluorodeoxyglucose-positron emission tomography (FDG-PET) and regional cerebral blood flow single-photon emission computed tomography (rCBF-SPECT) in Alzheimer’s disease (AD). Method: Brain T1-MRI, FDG-PET and rCBF-SPECT scans were acquired from a sample of mild AD patients (n=20) and healthy elderly controls (n=18). SVM-based diagnostic accuracy indices were calculated using whole-brain information and leave-one-out cross-validation. Results: The accuracy obtained using PET and SPECT data were similar. PET accuracy was 68∼71% and area under curve (AUC) 0.77∼0.81; SPECT accuracy was 68∼74% and AUC 0.75∼0.79, and both had better performance than analysis with T1-MRI data (accuracy of 58%, AUC 0.67). The addition of PET or SPECT to MRI produced higher accuracy indices (68∼74%; AUC: 0.74∼0.82) than T1-MRI alone, but these were not clearly superior to the isolated neurofunctional modalities. Conclusion: In line with previous evidence, FDG-PET and rCBF-SPECT more accurately identified patients with AD than T1-MRI, and the addition of either PET or SPECT to T1-MRI data yielded increased accuracy. The comparable SPECT and PET performances, directly demonstrated for the first time in the present study, support the view that rCBF-SPECT still has a role to play in AD diagnosis.
