ABSTRACT BACKGROUND: The intensity of the thromboprophylaxis needed as a potential factor for preventing inpatient mortality due to coronavirus disease-19 (COVID-19) remains unclear. OBJECTIVE: To explore the association between anticoagulation intensity and COVID-19 survival. DESIGN AND SETTING: Retrospective observational study in a tertiary-level hospital in Spain. METHODS: Low-molecular-weight heparin (LMWH) status was ascertained based on prescription at admission. To control for immortal time bias, anticoagulant use was analyzed as a time-dependent variable. RESULTS: 690 patients were included (median age, 72 years). LMWH was administered to 615 patients, starting from hospital admission (89.1%). 410 (66.7%) received prophylactic-dose LMWH; 120 (19.5%), therapeutic-dose LMWH; and another 85 (13.8%) who presented respiratory failure, high D-dimer levels (> 3 mg/l) and non-worsening of inflammation markers received prophylaxis of intermediate-dose LMWH. The overall inpatient-mortality rate was 38.5%. The anticoagulant nonuser group presented higher mortality risk than each of the following groups: any LMWH users (HR 2.1; 95% CI: 1.40-3.15); the prophylactic-dose heparin group (HR 2.39; 95% CI, 1.57-3.64); and the users of heparin dose according to biomarkers (HR 6.52; 95% CI, 2.95-14.41). 3.4% of the patients experienced major hemorrhage. 2.8% of the patients developed an episode of thromboembolism. CONCLUSIONS: This observational study showed that LMWH administered at the time of admission was associated with lower mortality among unselected adult COVID-19 inpatients. The magnitude of the benefit may have been greatest for the intermediate-dose subgroup. Randomized controlled trials to assess the benefit of heparin within different therapeutic regimes for COVID-19 patients are required.

Bridging the Gap Between Model-Based Design and Reliable Implementation of Feedback-Based Biocircuits: A Systems Inverse Problem Approach

Abstract: Our concern is the tuning of mathematical models describing rationally designed genetic biocir-cuits. Based on a deterministic lumped continuous-time approach, we propose a tuning methodology combining both exact algebraic parameter reconstruction and nonlinear parameter estimation of a given model supporting the design of a specific genetic biocircuit, i.e., we bridge the gap between model-based design and implementation as the solution of a systems inverse problem. As a proof of concept, our proposal is constrained to cyclic feedback systems characterizing synthesized transcriptional networks conditioned to display sustained oscillatory behavior. Our proposed methodology is illustrated via computer–based simu-lations involving the tuning of a state–based model describing a well–know cyclic feedback biocircuit: the celebrated repressilator. Tuning in our case is conceived as a procedure to adjust the parameter values of the mathematical model taking into account for this the actual behavior observed from the corresponding synthesized biocircuit.

El cáncer de mama en hombres es una enfermedad poco frecuente, en Colombia la incidencia estimada anual es de 1 caso por cada 1.000.000 de habitantes, lo cual es significativamente menor que en otros países. Objetivo: Describir la presentación clínica, histopatológica, tratamiento inicial y recaídas de pacientes de sexo masculino con diagnóstico de cáncer de mama en el Instituto Nacional de Cancerología entre el periodo 1996 a 2011. Resultados: La población del estudio fue de 27 pacientes. Los principales síntomas reportados por los pacientes fueron masa y dolor. La mayoría de los pacientes (89%) presentaban un estadio clínico localmente avanzado (IIIB y IV). El tipo histológico ductal NOS fue el más predominante (82%). Se encontró resultado de receptores hormonales en 22 de los 27 pacientes, de los cuales 19 (86%) eran positivos. El porcentaje de pacientes llevados a cirugía fue del 71% (20 pacientes), la técnica quirúrgica utilizada en 17 de los 20 pacientes fue la mastectomía radical modificada. En todos los pacientes sometidos a tratamiento quirúrgico fue realizado vaciamiento axilar y resección del CAP. La mediana de seguimiento de los pacientes fue de 8,9 meses. Dos pacientes presentaron recaída sistémica. Ninguno de los pacientes presentó recaída locorregional. Conclusión: Presentamos una serie de pacientes con cáncer de mama en hombres, lo cual es una entidad de baja incidencia. Nuestros pacientes se presentaron principalmente en estadio avanzado, en una proporción mayor a lo descrito en otras series latinoamericanas. Aunque la biología tumoral encontrada representa factores de buen pronóstico, no concuerda con el hecho de que la mayoría de tumores sean avanzados.
Breast cancer in men is a rare disease in Colombia, with an estimated annual incidence of 1 case per 1,000,000 habitants, which is significantly lower than in other countries. Objective: To describe the clinical presentation, histopathology, initial treatment and relapse of male patients diagnosed with breast cancer at the National Cancer Institute between 1996 and 2011. Results: The study population was 27 patients. The main symptoms reported by patients were mass and pain. Most patients (89%) had locally advanced clinical stage (IIIB and IV). NOS ductal histological type was the most prevalent (82%). Hormone receptor was found in 22 of 27 patients, of whom 19 (86%) were positive. The percentage of patients undergoing surgery was 71% (20 patients), and the surgical technique used in 17 of the 20 patients was modified radical mastectomy. Axillary dissection and resection of the CAP was performed on all patients who underwent surgical treatment. The median follow up of patients was 8.9 months. Two patients had systemic relapse. None of the patients had locoregional relapse. Conclusion: A case series is presented of male patients with breast cancer, which is a disease of low incidence. Our patients are mainly presented in advanced stages, a greater proportion than that described in other Latin American series. Although tumour biology found good prognostic factors, it is not consistent with the fact that most tumours are advanced.

OBJECTIVE: To identify the prevalence and factors associated with cervical human papillomavirus infection in women with systemic lupus erythematosus METHODS: This cross-sectional study collected traditional and systemic lupus erythematosus-related disease risk factors, including conventional and biologic therapies. A gynecological evaluation and cervical cytology screen were performed. Human papillomavirus detection and genotyping were undertaken by PCR and linear array assay. RESULTS: A total of 148 patients were included, with a mean age and disease duration of 42.5±11.8 years and 9.7±5.3 years, respectively. The prevalence of squamous intraepithelial lesions was 6.8%. The prevalence of human papillomavirus infection was 29%, with human papillomavirus subtype 59 being the most frequent. Patients with human papillomavirus were younger than those without the infection (38.2±11.2 vs. 44.2±11.5 years, respectively; p = 0.05), and patients with the virus had higher daily prednisone doses (12.8±6.8 vs. 9.7±6.7 mg, respectively; p = 0.01) and cumulative glucocorticoid doses (14.2±9.8 vs. 9.7±7.3 g, respectively; p = 0.005) compared with patients without. Patients with human papillomavirus infection more frequently received rituximab than those without (20.9% vs. 8.5%, respectively; p = 0.03). In the multivariate analysis, only the cumulative glucocorticoid dose was associated with human papillomavirus infection. CONCLUSIONS: The cumulative glucocorticoid dose may increase the risk of human papillomavirus infection. Although rituximab administration was more frequent in patients with human papillomavirus infection, no association was found. Screening for human papillomavirus infection is recommended in women with systemic lupus erythematosus.

Non-bilayer phospholipid arrangements are three-dimensional structures that form when anionic phospholipids with an intermediate structure of the tubular hexagonal phase II are present in a bilayer of lipids. Antibodies that recognise these arrangements have been described in patients with antiphospholipid syndrome and/or systemic lupus erythematosus and in those with preeclampsia; these antibodies have also been documented in an experimental murine model of lupus, in which they are associated with immunopathology. Here, we demonstrate the presence of antibodies against non-bilayer phospholipid arrangements containing mycolic acids in the sera of lepromatous leprosy (LL) patients, but not those of healthy volunteers. The presence of antibodies that recognise these non-bilayer lipid arrangements may contribute to the hypergammaglobulinaemia observed in LL patients. We also found IgM and IgG anti-cardiolipin antibodies in 77% of the patients. This positive correlation between the anti-mycolic-non-bilayer arrangements and anti-cardiolipin antibodies suggests that both types of antibodies are produced by a common mechanism, as was demonstrated in the experimental murine model of lupus, in which there was a correlation between the anti-non-bilayer phospholipid arrangements and anti-cardiolipin antibodies. Antibodies to non-bilayer lipid arrangements may represent a previously unrecognised pathogenic mechanism in LL and the detection of these antibodies may be a tool for the early diagnosis of LL patients.

The lizard cerebral cortex as a model to study neuronal regeneration

O córtex cerebral de lagartos, uma área homóloga à fascia dentata hipocampal, exibe neurogênese pós-natal prolongada, isto é, o epêndima do córtex medial prolifera e dá origem a neurônios imaturos, que migram para a camada celular. Nesta camada, neurônios recrutados se diferenciam e dão origem a axônios, ricos em zinco, que se projetam para as demais áreas corticais, do que resulta um crescimento contínuo do córtex medial e sua projeção axonal. Isto acontece por toda a vida do lagarto, mesmo em animais adultos, o que permite uma de suas características mais importantes: a regeneração neuronal. Experimentos em nosso laboratório demonstraram que uma lesão química do córtex medial (afetando até 95% de seus neurônios) resulta em uma cascata de eventos: inicialmente, morte celular e retração dendro-dendrítica maciças, seguidas de proliferação neuroblasto-ependimária e subseqüente neo-histogênese e regeneração de um novo córtex medial, indistingüível de um normal, não danificado. Ao que saibamos, este é o único caso de regeneração de uma estrutura nervosa central por produção de novos neurônios e neo-histogênese em amniotas. Portanto, o córtex cerebral do lagarto é um bom modelo para estudar a regeneração de neurônios e os fatores complexos que regulam os seus eventos neurogenéticos, migratórios e de neo-sinaptogênese.
The medial cerebral cortex of lizards, an area homologous to the hippocampal fascia dentata, shows delayed postnatal neurogenesis, i.e., cells in the medial cortex ependyma proliferate and give rise to immature neurons, which migrate to the cell layer. There, recruited neurons differentiate and give rise to zinc containing axons directed to the rest of cortical areas, thus resulting in a continuous growth of the medial cortex and its zinc-enriched axonal projection. This happens along the lizard life span, even in adult lizards, thus allowing one of their most important characteristics: neuronal regeneration. Experiments in our laboratory have shown that chemical lesion of the medial cortex (affecting up to 95% of its neurons) results in a cascade of events: first, massive neuronal death and axonal-dendritic retraction and, secondly, triggered ependymal-neuroblast proliferation and subsequent neo-histogenesis and regeneration of an almost new medial cortex, indistinguishable from a normal undamaged one. This is the only case to our knowledge of the regeneration of an amniote central nervous centre by new neuron production and neo-histogenesis. Thus the lizard cerebral cortex is a good model to study neuronal regeneration and the complex factors that regulate its neurogenetic, migratory and neo-synaptogenetic events.
