Hematological problems are associated with Coronavirus disease 2019 (COVID-19). Respiratory impairment is the higher point studied, although without experimental studies related to the oxygen transport performed by erythrocytes. Therefore, we decided to investigate if erythrocytes from COVID-19 patients have their functionality changed. The case-control study included hospitalized patients with a positive real-time polymerase chain reaction (RT-PCR) result admitted to University Hospital. Volunteers (negative RT-PCR results) were recruited as a control group. Thus, we assessed different erythrocytes parameters, oxidative stress markers, and biophysical studies using whole blood and isolated hemoglobin. We found a decrease of 51% in oxygen uptake and reduced antioxidant enzyme activity in COVID-19 patients compared to controls. Raman spectrometry showed structural changes in the hemoglobin and lipids of the erythrocytes from COVID-19 patients; thus, these results were consolidated with an increase in Young’s modulus in erythrocytes followed by morphology changes. Besides, isolated hemoglobin from COVID-19 patients has a distinct interaction profile using a ligand model compared to the control. COVID-19 leads to structural, functional, and morphological damage to human erythrocytes. Our data showed structural and molecular changes and induction of oxidative stress in erythrocytes by COVID-19, a new perspective on the contribution of erythrocytes to a respiratory commitment in COVID-19.

ABSTRACT Objective: The risk of malignancy and diagnostic accuracy of fine-needle aspiration biopsy (FNAB) of thyroid nodules (TN) with diameters ≥ 3-4 cm remains controversial. However, some groups have indicated surgical treatment in these patients regardless of the FNAB results. We aimed to evaluate the diagnostic accuracy of the FNAB in systematically resected ≥4 cm TN and if the risk of malignancy is higher in these patients. Subjects and methods: We retrospectively evaluated 138 patients (142 nodules) with TN with diameters ≥4 cm who underwent thyroidectomy. Results: The FNAB results were nondiagnostic/unsatisfactory (ND/UNS) in 2.1% of the cases and benign in 51.4%. They indicated atypia of undetermined significance/follicular lesion of undetermined significance (AUS/FLUS) in 23.9% of cases, follicular neoplasia/suspicious for a follicular neoplasm (FN/SFN) in 9.2%, suspicion of malignancy (SUS) in 8.5%, and malignant in 4.9%. The histopathological analysis after thyroidectomy revealed a thyroid cancer rate of 100% in the FNABs classified as malignant, 33.3% in SUS cases, 7.7% in FN/SFN, 17.6% in AUS/FLUS, and 4.1% in benign FNABs. None of the ND/UNS FNABs were malignant. The global malignancy diagnosis was 14.8% (n = 21). However, the rate of false negatives for FNAB was low (4.1%). Conclusion: We showed that the risk of malignancy in nodules with diameters ≥4 cm was higher compared to the risk of thyroid cancer in TN in general. However, we found a low rate of false-negative cytological results; therefore, our data do not justify the orientation of routine resection for these larger nodules.

Abstract Simultaneous analysis studies of several agronomic traits in cultivated plants make it possible to identify phenotypic and genotypic differences due to environmental variations, such as altitude. Therefore, the objective was to evaluate, through multivariate analysis of agronomic, physicochemical and physiological characters, passion fruit hybrids cultivated in different environments. The hybrids used were Gigante Amarelo, Rubi do Cerrado and Sol do Cerrado, cultivated in the southern region of Espírito Santo in four municipalities/environments: Marataízes (41 m), Jerônimo Monteiro (104 m), Alegre (711 m), and Ibitirama (1016 m). The agronomic characters of the plants, the physical-chemical characteristics of the fruits and the physical, biochemical and physiological qualities of the seeds were analyzed. The Singh method was used to determine the most important differentiating characters between hybrids growing in different environments. Based on these characters, a dissimilarity matrix was generated and a principal coordinate analysis was performed. It was observed that the pulp yield was influenced by altitude. The three hybrids showed greater performance in terms of agronomic characters at altitude (41 m) than at altitude (104 m). The Sol do Cerrado hybrid showed high performance in the physical-chemical characteristics of the fruits at altitude (104 m).
Resumo Estudos de análise simultânea de diversos caracteres agronômicos em plantas cultivadas permitem identificar diferenças fenotípicas e genotípicas devido a variações ambientais, como a altitude. Diante disso, objetivou-se avaliar através da análise multivariada de caracteres agronômicos, fisico-químicos e fisiológicos, híbridos de maracujazeiros cultivados em diferentes ambientes. Os híbridos utilizados foram Gigante Amarelo, Rubi do Cerrado e Sol do Cerrado, cultivados na região sul do Espírito Santo em quatro municípios/ambientes: Marataízes (41 m), Jerônimo Monteiro (104 m), Alegre (711 m), e Ibitirama (1016 m). Foram analisados os caracteres agronômicos das plantas, as características físico-químicas dos frutos e as qualidades físicas, bioquímicas e fisiológicas das sementes. O método Singh foi usado para determinar os caracteres diferenciadores mais importantes entre híbridos crescendo em diferentes ambientes. Com base nesses caracteres, uma matriz de dissimilaridade foi gerada e uma análise de coordenadas principais foi realizada. Observou-se que o rendimento de polpa foi influenciado pela altitude. Os três híbridos apresentaram maior desempenho em termos de caracteres agronômicos na altitude (41 m) do que na altitude (104 m). O híbrido Sol do Cerrado apresentou alto desempenho nas características físico-químicas dos frutos na altitude (104 m).

Resumo Este estudo objetivou avaliar a capacidade de degradar a matéria orgânica pela macrofauna edáfica (minhocas), a relação carbono/nitrogênio (C/N) e o potencial hidrogeniônico (pH), durante o processo da vermicompostagem em diferentes resíduos orgânicos. Os tratamentos foram constituídos por resíduos orgânicos de origem animal (esterco bovino, ovino e equino) e vegetal (resíduo de erva-mate e borra-de-café), os quais foram acondicionados em vasos plásticos com capacidade de 10 litros, compondo cinco tratamentos em delineamento experimental inteiramente casualisado, com cinco repetições. Foram inoculadas 150 minhocas da espécie Eisenia foetida, em cada recipiente. Após 87 dias, foi realizada a avaliação da multiplicação das minhocas, através da sua contagem manual e seus casulos. Os resíduos foram submetidos, ao inicio e ao final do experimento, a análises de umidade a 60 °C, pH, densidade volumétrica, análise química de macronutrientes e relação C/N. Houve uma dominância de minhocas e casulos no processo da vermicompostagem nos resíduos de esterco ovino e erva-mate. Observou-se para os macronutrientes (P, K e Mg) e para a relação C/N uma maior quantidade nos resíduos vegetais, enquanto que para N valores maiores foram encontrados nos tratamentos esterco ovino e borra-de-café, e para Ca o maior valor entre os tratamentos foi observado no tratamento com borra de café ao final e o menor valor no inicio. Os resultados obtidos neste estudo demonstram a importância da macrofauna edáfica para o processo da vermicompostagem por possibilitar maiores informações sobre sua influência na continuidade dos processos de decomposição da matéria orgânica.
Abstract The objective of this study was to evaluate the ability to degrade organic matter by edaphic macrofauna (worms), carbon/nitrogen (C/N) ratio and hydrogenation potential (pH) during the vermicomposting process in different organic residues. The treatments were constituted by organic residues of animal origin (bovine, ovine and equine manure) and vegetable (herb-checkmate and coffee drag), which were conditioned in plastic pots with a capacity of 10 liters, comprising five treatments in a completely randomized experimental design, with five replications. Were inoculated 150 earthworms of the species Eisenia foetida, into each plot. After 87 days, the evaluation of the multiplication of the earthworms was carried out, through its manual count and its cocoons. At the beginning and at the end of the experiment, the samples were submitted to analysis of humidity at 60 °C, pH, volumetric density, chemical analysis of macronutrients and C/N ratio. There was a dominance of worms and cocoons in the process of vermicomposting in the residues of ovine manure and herb-checkmate. The macronutrients (P, K and Mg) and C/N ratio were higher in the vegetal residues, while for N higher values were found in ovine manure and coffee drag treatments, and for Ca higher value among treatments was observed in the coffee drag treatment at the end and the lowest value at initiation. The results obtained in this study demonstrate the importance of the edaphic macrofauna to the vermicomposting process, since it allows more information about its influence on the continuity of soil organic matter decomposition processes.

Self-nanoemulsion drug delivery systems (SNEDDS) represent a potential carrier to improve solubility and bioavailability of health care products. In this sense, copaiba oil (CO) loaded on cosurfactant-free SNEDDS formulations (CO-SNEDDS) were obtained by using phase diagram performed on different concentrations of constituents (oil, surfactant and water). The CO-SNEDDS are composed of 1% (m/m) of oil phase, 20% (m/m) of Tween 80, and 79% (m/m) of double-distilled water. Specifically, varying the oil phase to 1% of CO affords the so-called SNECO system, and 1% of CO blended on a 1:1 ratio with coconut oil or sunflower oil affords, respectively, the SNECO-COO and SNECO-SO derivative systems. CO sample was authenticated by gas chromatography equipped with a flame ionization detector and the CO-SNEDDS samples were characterized by droplet particle size, zeta potential, polarized light microscopy, pH, conductivity, refractive index, rheological and surface tension analyses. According to the antioxidant tests reducing power and hydroxyl inhibiting formation, the findings displayed 47.89 and 72.92% for SNECO, 46.48 and 84.11% for SNECO-COO, and 52.46 and 74.48% for SNECO-SO, respectively. From now on CO-SNEDDS based systems become available as promising targets for novel medicinal investigations and also could be undertaken for the oral administration of poorly water soluble drugs.

RESUMO A técnica de substituição de açúcar em dois estágios (D3S) foi utilizada para induzir a troca de açúcares na manga. Esta técnica envolve dois estágios, nos quais os açúcares de alto teor calórico são parcialmente removidos das amostras de frutas no primeiro estágio e, no segundo, açúcar de baixa caloria é incorporado à manga. Ondas ultrassônicas podem ser aplicadas em um ou ambos estágios e seu uso também foi avaliado neste trabalho. Os resultados indicaram que submeter as amostras às ondas ultrassônicas (25 kHz) nos dois estágios e sua imersão em solução a base Stevia (250 ou 500 g kg-1) no segundo estágio por 10, 20 e 30 min de processamento resultou em maiores perdas de água durante o processo, enquanto maiores valores de ganho de sólidos foram obtidos pela aplicação do ultrassom apenas no primeiro estágio. As amostras também foram avaliadas em relação a alguns parâmetros de qualidade. O uso desta técnica resultou em amostras com maiores valores de fenólicos totais e em mudanças nos parâmetros de cor (L*, a* e b*). Quando as amostras foram submetidas às ondas ultrassônicas em ambos estágios, uma maior retenção de carotenoides foi observada.
ABSTRACT The dual-stage sugar substitution technique (D3S) was used to induce sugar replacement in mango. It involved two stages, in which high-calorie sugars were partially removed from the fruit samples in the first stage and, in the second one, low-calorie sugar was incorporated into the mango. Ultrasonic waves can be applied in one or both stages and their use was also evaluated in this study. Results showed that submitting samples to ultrasonic waves (25 kHz) in both stages and their immersion in Stevia-based solution (250 or 500 g kg-1) in the second stage for 10, 20 and 30 min of processing gave higher water loss during the process, while greater solids gain could be achieved by applying ultrasound only in the first stage. Samples were also evaluated in terms of some quality parameters. The use of this technique resulted in samples with higher values of total phenolic content and changes in color parameters (L*, a* and b*). When samples were subjected to ultrasonic waves in both stages, a higher carotenoid retention was observed.

ABSTRACT The aim of this study was to evaluate the efficacy and safety of several anesthetics in the brachyuran crab Neohelice granulata, an emergent experimental model. The essential oils (EOs) of Lippia alba, Aloysia tryphilla, and Melaleuca alternifolia (tea tree oil; TTO), the isolated compounds eugenol, menthol, terpinen-4-ol, and the nanoencapsulated form of TTO, were administered in one or more of the following ways: added to the water (immersion), through an arthrodial membrane (injected), or by oral gavage. Unexpectedly, most EOs did not produce an anesthetic effect after immersion. Only TTO and eugenol induced anesthesia by immersion, with very long induction and recovery times compared to anesthesia of other crustaceans. However, a good anesthetic effect was observed with the injection of terpinen-4-ol and nanoencapsulated TTO in N. granulata; both demonstrated ideal induction and recovery times. These substances appear to be promising anesthetic alternatives for crustaceans.

The aim of the present study is to assess the potential of ethanol deriving from the mesocarp of babassu coconut to be used in fuel cells. Babassu ethanol was generated through hydrolysis and fermentation processes. The Pt, PtRh and PtRu electrodes were prepared in carbon Vulcan XC72R through the reduction method and applied as electrocatalysts in ethanol oxidation reaction. X-Ray diffraction (XRD), energy-dispersive X-ray (EDX), stripping CO, cyclic voltammetry, chronoamperometry, and online differential electrochemical mass spectrometry (DEMS) were used to characterize the synthesized eletrocatalysts. The electrocatalyst Pt80Ru20/C presented larger active area and higher catalytic activity than other studied materials. The current efficiency of CO2 production rated less than 1% in all studied electrocatalysts, thus showing that babassu ethanol oxidation produces less pollutants than the commercial ethanol.

Resumo O presente estudo avalia a influência da anestesia sobre a fauna parasitária de monogeneas em peixes, sua intensidade e sua viabilidade. Dois experimentos foram realizados: Avaliação de um método anestésico por aspersão eugenol diretamente nas brânquias e avaliação da motilidade das monogeneas e sua viabilidade; e Comparação entre imersão e aspersão diretamente nas brânquias com benzocaína e eugenol, seguido de avaliação sobre a intensidade parasitária. Os resultados sugerem que a aspersão do anestésico não interferiu na motilidade, morfologia, superfície corporal e integridade do parasita, analisadas pelo método de fluorescência. A intensidade de monogenéticos nas brânquias foi menor nos peixes anestesiados pelo método de imersão em comparação com o método de aspersão e o grupo controle. O método de anestesia por aspersão nas brânquias pode ser utilizado em estudos parasitológicos.
Abstract The present study evaluates the influence of anesthesia on the parasitic fauna of monogenea fish parasites, as its intensity and viability. Two experiments were conducted: Evaluation of an anesthetic method by sprinkling eugenol directly on gills and evaluation of monogenea motility and viability; Comparison of immersion and directly sprinkling on the gills with benzocaine and eugenol followed by evaluation on parasite intensity. The results suggest that the anesthetic sprinkling didn’t interfere in the parasite motility, morphology and body surface integrity analyzed by fluorescence method. The monogenean intensity in the gills was lower in fish anesthetized by immersion method compared to the sprinkling method and the control group. This method of anesthesia can be used in parasitological studies.

ABSTRACT Objectives The presence of thyroglobulin (Tg) in needle washouts of fine needle aspiration biopsy (Tg-FNAB) in neck lymph nodes (LNs) suspected of metastasis has become a cornerstone in the follow-up of patients with papillary thyroid carcinoma (PTC). However, there are limited data regarding the measurement of anti-Tg antibodies in these washouts (TgAb-FNAB), and it is not clear whether these antibodies interfere with the assessment of Tg-FNAB or whether there are other factors that would more consistently justify the finding of low Tg-FNAB in metastatic LNs. Materials and methods We investigated 232 FNAB samples obtained from suspicious neck LNs of 144 PTC patients. These samples were divided according to the patient’s serum TgAb status: sTgAb- (n = 203 samples) and sTgAb+ (n = 29). The TgAb-FNAB levels were measured using two different assays. Tg-FNAB was also measured using two assays when low levels (< 10 ng/mL) were identified in the first assay of the metastatic LNs from the sTgAb+ samples. Results The TgAb-FNAB results were negative in both assays in all samples. Low levels of Tg-FNAB were identified in 11/16 of the metastatic LNs of the sTgAb+ patients and 16/63 of the sTgAb- patients (p < 0.05) using assay 1. The measurement of the Tg-FNAB levels using assay 2 indicated additional metastases in 5 LNs of the sTgAb+ patients. Conclusions Factors other than the presence of TgAb-FNAB may contribute to the higher number of metastatic LNs with undetectable Tg-FNAB in the sTgAb+ group. In addition, the measurement of Tg-FNAB using different assays was useful to enhance the diagnosis of metastatic LNs, particularly when cytological and Tg-FNAB results are discordant.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

A forma hereditária do carcinoma medular da tiróide pode ocorrer de modo isolado, o carcinoma medular da tiróide familiar (FMTC), ou como parte das neoplasias endócrinas múltiplas tipo 2A (MEN2A) e 2B (MEN2B). MEN2B é uma síndrome rara e seu fenótipo é usualmente, mas nem sempre, notado pelo médico. Na infância, nenhuma das características de MEN2B estão presentes, exceto pela disfunção gastrintestinal precoce, causada pelos neuromas intestinais. Quando disponível, a análise genética confirma o diagnóstico e orienta a avaliação pré-operatória e extensão da cirurgia. Neste artigo, apresentamos quatro casos de MEN2B, nos quais o diagnóstico tardio teve impacto significativo na evolução clínica e, potencialmente, na mortalidade em geral. APRESENTAÇÃO DOS CASOS: Apresentamos quatro casos, dois homens e duas mulheres, com diferenças em seus fenotipos e com diagnóstico tardio. O primeiro caso tem história de obstrução gastrintestinal importante em que foi necessária cirurgia dois dias após o nascimento. O segundo paciente apresentava nódulos na mucosa oral e constipação desde a infância. O terceiro referia história de constipação desde o nascimento até 5 meses de idade. O quarto tinha história de constipação intestinal desde a infância. DISCUSSÃO: Novos conceitos emergiram desde que o oncogene RET foi identificado, em 1993, como o gene responsável pelo carcinoma medular da tiróide hereditário. A maioria dos indivíduos apresenta a mutação M918T no éxon 16 do RET, enquanto poucos casos apresentam a mutação A883F ou a associação de V804M com E805K, Y806C ou S904C. O consenso recomenda a tiroidectomia total com dissecção dos linfonodos no compartimento central até os 6 meses após o nascimento. O diagnóstico rápido e preciso é essencial para o atingir os objetivos. CONCLUSÃO: O diagnóstico precoce de MEN2B é muito importante para propiciar a investigação apropriada de doenças associadas e para permitir aconselhamento e rastreamento dos parentes para uma mutação do RET. Pacientes com MEN2B, que apresentam frequentemente achados típicos ao exame físico, podem não ser reconhecidos e seguidos como casos esporádicos. Por causa disso, todos os casos de neoplasia endócrina múltipla devem ser avaliados pelo teste genético para mutações do RET.
BACKGROUND: The hereditary form of medullary thyroid carcinoma may occur isolated as a familial medullary thyroid carcinoma (FMTC) or as part of Multiple Endocrine Neoplasia 2A (MEN2A) and 2B (MEN2B). MEN2B is a rare syndrome, its phenotype may usually, but not always, be noted by the physician. In the infant none of the MEN2B characteristics are present, except by early gastrointestinal dysfunction caused by intestinal neuromas. When available, genetic analysis confirms the diagnosis and guides pre-operative evaluation and extent of surgery. Here we report four cases of MEN2B in which the late diagnosis had a significant impact in clinical evolution and, potentially, in overall survival. CASE REPORT: We report four cases, 2 men and 2 women, with differences in their phenotypes and with a late diagnosis. The first case has a history of severe gastrointestinal obstruction requiring a surgery intervention two days after his birth. The second told had nodules in the oral mucosa and constipation since childhood. The third case referred a history of constipation from birth until 5 months of life. The fourth has had a history of chronic constipation since childhood. DISCUSSION: New concepts have emerged since the RET oncogene was identified in 1993 as the responsible gene for hereditary medullary thyroid carcinoma. The majority of MEN2B individuals have M918T mutation in the exon 16 of RET, with a few cases having a mutation A883F or the association of V804M with E805K, Y806C or S904C mutations. The consensus classifies the RET mutation in codon 918 as of highest risk and recommends total thyroidectomy and central lymph node dissection until 6 months after birth. A fast and precise diagnosis is essential to reach these goals. The identification of early manifestations such as intestinal ganglioneuromatosis and oral mucosal neuromas should prompt the physician to initiate an investigation for multiple endocrine neoplasia type 2B. CONCLUSION: The diagnosis of MEN2B is very important to allow appropriate investigation of associated diseases and to allow counseling and appropriate screening of relatives for a RET mutation. Even patients with MEN2B, which often have typical physical features, may not be properly recognized and be followed as a sporadic case. Based on this, all suspicious cases of multiple endocrine neoplasia should undergo a molecular genetic test.

Experiência em pacientes com suspeita de hepatopatia crônica e contra-indicação para biópsia hepática percutânea utilizando a agulha de Ross modificada

OBJETIVOS: O trabalho visou introduzir em nosso meio a técnica de biópsia hepática transjugular orientada por métodos radiológicos para a obtenção do diagnóstico histológico em pacientes clínica e laboratorialmente diagnosticados com hepatopatia crônica e com pelo menos uma das contra-indicações à biópsia hepática percutânea: coagulopatia, ascite maciça e anemia crônica acentuada com insuficiência renal crônica ou obesidade mórbida. MÉTODOS: Biópsia hepática aspirativa foi obtida com agulha de Ross modificada, através da punção da veia jugular interna com cateterização da veia hepática direita sob controle fluoroscópico. RESULTADOS: Trinta e nove pacientes foram estudados, obtendo-se tecido hepático em 32 (82%) sendo satisfatório para diagnóstico histológico em 25 (64,1%), com concordância entre o diagnóstico pré e pós-biópsia em 11 (28,2%) e discordância em 14 pacientes (35,9%). O procedimento foi bem tolerado pela maioria dos pacientes; contudo um paciente apresentou sangramento retroperitoneal, necessitando cirurgia imediata para controle da hemorragia. CONCLUSÕES: A biópsia hepática transjugular é um método diagnóstico útil para o estudo histopatológico na suspeita de hepatopatia crônica com contra-indicações a biópsia hepática percutânea. Em nossa série foi obtido diagnóstico histopatológico em 64,1% dos indivíduos estudados. Pacientes com suspeita clínica de cirrose tiveram baixo diagnóstico histopatológico (50%) quando comparados com indivíduos sem essa suspeita (78,9%). O método é relativamente complexo, podendo apresentar complicações graves, e deve ser realizado em centros de excelência em radiologia e hepatologia.
PURPOSE: This work aimed at introducing the transjugular liver biopsy technique oriented by radiologic methods in our hospitals. This technique is indicated for obtaining histopathologic diagnosis in patients clinically and laboratorially suspected of having chronic liver disease with contraindications to percutaneous liver biopsy. METHODS: Liver biopsy was obtained with a modified Ross needle through the right jugular vein and right hepatic vein under fluoroscopic control. RESULTS: Transjugular liver biopsy was attempted for 39 patients, liver tissue obtained en 32 and histopathologic diagnosis in 25 (64.1%). In 11 patients (28.2%) there was agreement between the diagnoses established before and after biopsy, however, in 14 patients (35.9%), there was disagreement. The yield of diagnosis was low when patients were suspected for cirrhosis. The procedure was well tolerated by the majority of patients. Nonetheless, 1 presented intra-abdominal bleeding and required immediate surgery to control retroperitoneal hemorrhage. CONCLUSIONS: Transjugular liver biopsy is useful for the histopathologic diagnosis of patients with chronic liver diseases whenever the percutaneous route is contraindicated. In this series we obtained histopathologic diagnosis for 64,1% of the subjects studied. Patients suspected of having cirrhosis had a low yield of histopathologic diagnosis (50%) when compared to subjects without clinical evidence for crrhosis (78,9%). The technique is rather complex, and can cause serious complications. This, it should be performed in reference centers in radiology and hepatology.

Os autores relatam um caso de um paciente do sexo masculino com 12 anos de idade e que desde os 10 anos apresenta oftalmoplegia progressiva completa associada a ptose palpebral. O exame eletromiográfico mostrou alterações de padrão miopático em músculos extrínsecos oculares e de membros. A case of progressive ophtalmoplegia and ptosis afecting a 12 year old boy is reported. A miopatk pattern was detected by the electromyography, the cerebrospinal fluid showed an increase of the protein content and retinal pigmentary degeneration was detected. This findings call the attention for a more widespread degenerative disorders afecting not only the muscular sistem but other organs as well.
