The present study aimed to propose a new cause of odorous compounds release, i.e., the presence of antibiotics in water bodies and its toxicity to cyanobacteria, known to be the main producer of geosmin (GEO) and 2-methylisoborneol (2-MIB). Therefore, a literature review was carried out regarding the problems caused by antibiotics in aquatic environment, including cyanobacterial blooms and GEO and 2-MIB release. In addition, a bibliometric analysis was performed using the VOSviewer software based on the results obtained from the Web of Science (WOS) database. This review aims to build a scientific understanding of the problem, presenting interesting points that converge with the proposed association. It is worth mentioning that no work has been found in the literature that has proposed this relationship. Thus, based on the bibliographic survey, observations and information acquired in recent years about cyanobacterial blooms and environmental contamination by pharmaceutical drugs, one of the main causes of an earthy and musty flavour and odour in a drinking water supply is the toxicity imposed by the presence of antibiotics in aquatic environments on cyanobacteria.

Abstract Objective The outbreak of the COVID-19 pandemic had a considerable impact on the healthcare access, treatment, and follow-up of oncologic patients. The aim of this study was to evaluate how the COVID-19 pandemic has affected consultation and follow-up demand as well as treatment volume at Brazilian Head and Neck Surgery centers. Methods An anonymous online questionnaire was used for collection of data across all Brazilian Head and Neck Surgery Centers across a 3-month period (April‒June 2021). This information included the characteristics of each center, and the perceived self-reported impact of the COVID-19 pandemic on academic activities, residency training, and the diagnosis, treatment, and follow-up of patients with Head and Neck diseases between 2019 and 2020. Results The response rate across the 40 registered Brazilian Head and Neck Surgery Centers was 47.5% (n = 19). The data showed a significant reduction in the total number of consultations (24.8%) and number of attending patients (20.2%) between 2019 and 2020. The total number of diagnostic exams (31.6%) and surgical procedures (13.0%) conducted over this period also decreased significantly. Conclusions The COVID-19 pandemic had a significant national impact on Brazilian Head and Neck Surgery Centers. Future studies should examine the long-term effects of the pandemic on cancer treatment. Level of evidence Evidence from a single descriptive study.

Abstract Glyphosate affects ecosystems due to exposure of non-target crops and is a persistent contaminant at low concentrations. The application of palygorskite to reduce glyphosate contamination in Brazil is an environmentally friendly way to remediate impacted areas. This research evaluated palygorskite application for glyphosate adsorption present in a synthetic effluent. Palygorskite samples were ore dressed by wet granulometric classification and wet magnetic separation and submitted to organophilization with cetyltrimethyl ammonium bromide solutions at concentrations of 0.01, 0.1, 1.0 and 1.5% (w/w), and the organo-palygorskite was applied in glyphosate adsorption tests. After ore dressing there was an increase in its cation exchange capacity from 19 to 41 meq 100 g-1 and the surface area was 149 g m-2. The 1.0% organo-palygorskite adsorbed about 86% of glyphosate from synthetic aqueous effluent. Based on this high adsorption yield, the organo-palygorskite is a potential adsorbent for remediation of effluents containing the toxic herbicide glyphosate.

Abstract Studying a watershed in the Amazon region requires understanding that this region has national and global relevance. The fact that the study area is located in the so-called arc of deforestation raises the need to understand its context, its vulnerabilities, and its physical, environmental, and anthropic components. This research assesses the environmental vulnerability of the Buriticupu River Watershed in Maranhão State, Brazil. The aim is to foster improvements that may contribute to environmental planning and, specifically, to water resource management. For this purpose, the authors used the Geographic Information System environment to work with Map Algebra, interrelating primary and secondary data on geology, relief, pedology, and land use and land cover to generate synthesis cartography. The results pointed to areas with predominantly medium vulnerability. However, the analyses highlight alluvial plains, which became the most fragile areas from an environmental point of view. This is mainly because these areas comprise water-saturated soils (gleysols), unconsolidated deposits of sand and silt (alluvial deposits), and extensive humid vegetation, characteristics of igapó and floodplain forests. This research generated a document that applies is applicable to the study area. It also provided, in a systemic and integrated way, a product that will help in planning, which is very important for the creation of a watershed committee, something necessary and still embryonic in Maranhão State, Brazil.

Abstract This study aimed to assess the cortisol, body and reproductive development of prepubertal Holstein and Holstein-Gir ¾ heifers at 27 months of age maintained in an integrated livestock-forest (ILF) system for 60 summer days compared to the monoculture system in full sun (FS). The ILF system promoted changes (P=0.02) in the cortisol levels of Holstein-Gir ¾ heifers and did not affect weight gain in any of the breed groups studied. Animals in ILF system presented a lower (P=0.006) vulvar development for the rima height parameter and similar for the vulva width parameter. The ovarian follicular population of Holstein-Gir ¾ heifers in the ILF system was lower (P=0.004); however, for the Holstein heifers, no statistical difference was found, and numbers were higher (P=0.08) in the ILF system. None of the other ovarian parameters studied had any changes, and we also found important racial differences. Weight gain (P=0.003), vulvar development (P<0.001), and mean follicular size (P=0.008) were higher in the Holstein-Gir ¾ animals. Based on such results, the effect of the ILF system at 27 months of age on stress and reproductive parameters in the Holstein breed is considered positive, although negative effects have been detected on reproductive parameters in the Holstein-Gir ¾ breed.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
nbsp;
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Abstract Objective: To determine the prevalence of vitamin A deficiency (VAD) and serum concentrations of retinol, correlating them with IGF-1 concentrations in preschoolers with DS. Methods: Cross-sectional study was conducted on 47 children with DS aged 24 to 72 months, in Ribeirão Preto, Brazil. VAD was determined by the relative dose-response (RDR) test. Retinol serum concentration ≤ 0.70 μmol/L and IGF-1 serum concentration below the 3rd percentile for sex and age were considered to represent deficiency. C-reactive protein (CRP) was determined at the beginning of the study. Weight, height, and information about fever and/or diarrhea were obtained at the beginning of the study. Results: VAD prevalence was 25.5% (12/47), and 74.5% (35/47) of the children had deficient retinol before the intervention. CRP was not associated with VAD. Mean IGF-1 were 103.5 ng/mL (SD = 913) for the group with VAD and 116.3 ng/mL (SD = 54.9) for the group with no VAD (p-value = 0.85); 8.5% (4/47) of the children showed deficient IGF-1, but without VAD. No association was observed between VAD and IGF-1 deficiency. A moderate positive correlation was observed between pre-intervention retinol and IGF-1 (ρ = 0.37; p-value = 0.01). Conclusion: a high prevalence of VAD and deficient retinol was observed and there was a positive correlation between serum retinol and IGF-1.

ABSTRACT The objective of this work was to investigate the adsorptive potential of a previously benefited and characterized brazilian montmorillonite sample, regarding amoxicillin trihydrate removal, a zwitterionic species, present in aqueous solutions. 500 mg capsule of antibiotic was properly characterized by XRD, FTIR, SEM/EDS and surface charge measurement through Zeta potential. Amoxicillin aqueous solution and supernatants from adsorption essays were characterized by Molecular Absorption Spectrophotometry in Ultraviolet-Visible Region (UV/VIS), from 200 to 400 nm. Three investigative essays were performed, varying montmorillonite mass (0.050, 0.525 and 1.000 g) and contact time between substances (10, 95 and 180 min). Amoxicillin sample is crystalline, morphologically prismatic, essentially composed of carbon, nitrogen, sulfur, hydrogen and oxygen. Its zwitterionic character was confirmed by Zeta potential, presenting positive (pH 2 to 4), relatively neutral (pH 4 to 6), and negative (above pH 6) surface charges. Test results indicated that the highest removals (21.4 and 41.8 %) were for the largest clay mineral masses (0.525 and 1.000 g, respectively), with pHs close to amoxicillin pka1 (2.68). Larger clay mineral masses have more adsorbent sites, which, associated with components opposite charges in desired pH range, promote a greater interaction. Thus, brazilian montmorillonite can promote amoxicillin removal from aqueous solutions.

BACKGROUND Zika virus (ZIKV) is an emerging arbovirus associated with foetal malformations and neurological complications. The infection is usually associated with mild symptoms. The comparison between the allelic frequency of polymorphic genes in symptomatic infected individuals in the population can clarify the pathogenic mechanisms of ZIKV. During ZIKV infection, cytokines are produced and natural killer (NK) cells are recruited, whose activation depends on signaling pathways activated by specific receptors, such as killer cell immunoglobulin-like receptors (KIR). These molecules interact with human leukocyte antigen (HLA) class I ligands and are encoded by polymorphic genes. OBJECTIVES This study aimed to evaluate the frequency of allelic variants of the genes encoding the KIR receptors and their HLA class I ligands in 139 symptomatic ZIKV-patients and 170 controls negative for the virus, and to evaluate the role of these variants for ZIKV susceptibility. METHODS KIR and HLA class I genes were genotyped using the polymerase chain reaction-sequence specific oligonucleotide (PCR-SSO) technique. FINDINGS No significant differences in the frequency distribution of KIRs and KIR-HLA in patients compared to controls were observed. MAIN CONCLUSIONS KIR and its HLA ligands might play a minor role in ZIKV infection in the south and southeast Brazilian individuals.

Abstract Objectives To evaluate the impact of adherence to the cardiac surgical checklist on mortality at the teaching hospital. Methods A retrospective cohort study after the implementation of the cardiac surgical safety checklist in a reference hospital in Latin America. All patients undergoing coronary artery bypass surgery and/or heart valve surgery from 2013 to 2019 were analyzed. After the implementation of the project InCor-Checklist “Five steps to safe cardiac surgery” in 2015, the correlation between adherence and completeness of this instrument with surgical mortality was assessed. The EuroSCORE II was used as a reference to assess the risk of expected mortality for patients. Cross-sectional questionnaires were during the implementation of the InCor-Checklist. To perform the correlation, Pearson’s coefficient was calculated using R software. Results Since 2013, data from 8139 patients have been analyzed. The average annual mortality was 5.98%. In 2015, the instrument was used in only 58% of patients; in contrast, it was used in 100% of patients in 2019. There was a decrease in surgical mortality from 8.22% to 3.13% for the same group of procedures. The results indicate that the greater the checklist use, the lower the surgical mortality (r = 88.9%). In addition, the greater the InCor-Checklist completeness, the lower the surgical mortality (r = 94.1%). Conclusion In the formation of the surgical patient safety culture, the implementation and adherence to the InCor-Checklist “Five steps to safe cardiac surgery” was associated with decreased mortality after cardiac surgery. HIGHLIGHTS Checklists avoid human errors and are commonly used in high-reliability industries. The “InCor Checklist” was associated with decreased mortality over time. Adherence, completeness, and sustainability within public policies are necessary.

ABSTRACT Our previous work reported that palygorskite has potential for application as metal cations adsorbent due to its chemical and mineralogical properties. In this work, kinetic study and adsorption isotherms were performed in order to evaluate Pb (II) ions adsorption rate, maximum capacity and type of adsorption by using palygorskite as adsorbent. Adsorption tests were performed in batch, using pH of 5, 2 g of palygorskite, 40 mL of synthetic effluent solution and stirring for 1 hour. Kinetic experiments were performed using 34 mg L-1 of a lead synthetic effluent at same mass and pH conditions. Furthermore, after adsorption studies, the sample was characterized by X-ray fluorecence (XRF), Fourier Transform Infrared Spectroscopy (FTIR) and Scanning Electron Microscopy (SEM) coupled with Energy-dispersive X-ray spectroscopy (EDS) analysis, in order to verify and understand the interaction of lead ions in palygorskite. The results demonstrated that palygorskite presented an equilibrium time of 10 min with 99.14% of removal, following pseudo-second order kinetic. The maximum adsorption capacity was 21.65 mg g-1 and Gibbs’ adsorption-free energy was -21.39 KJ mol-1, with Langmuir model being the most suitable for adjustment of the data. Thus, its presence was confirmed by characterization techniques, indicating that the metal is distributed homogeneously on palygorskite surface, proving its efficiency as lead ions adsorbent.

Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.
