Objective: to compare the size of the osseous triangular area, adjacent to lumbar pedicle screws, between to insertion techniques. Method: in 50 cases (27 males - 23 females) with a median age of 54.4 years, magnetic resonance images of normal lumbar spines were obtained. Axial sections through the pedicles of the vertebrae L4 were studied with the following protocol: FSE, T2 weighted images (TE:130 TR:1740), nex 2, 2 mm slices, FOV: 256x256, with a Signa Infinity equipment, GE 1.5 T. We draw lines centred in both pedicles following 2 angles: angle 1 of 18º between the base of the superior facet to the anterior third of the vertebral body and angle 2 of 38º between the union of the transverse process with the base of the superior facet and the anterior third of the vertebral body. The triangular areas adjacent to the lines were measured (mm) and compared. The statistical analysis was performed with ANOVA for repeated measurements and with the Scheffe test for ad hoc comparisons. Results: the averages of areas for angle 1 were: right 63.86±1.77 and left 64.06±1.54; the differences were not statistically significative (p=0.906). The averages of areas for angle 2 were: right 154.36±1.87 and left 154.54±1.81; the differences were not statistically significative (p=0.929). The differences found in the size of the right and left areas at L4, between the two insertion techniques, were very significative (p<0.0001). Conclusion: the sizes of the triangular areas adjacent to the lines following the angle 2 were larger than those adjacent to the lines following the angle 1 and their differences were statistically significative.
Objetivo. Comparar la magnitud del área triangular, adyacente a los tornillos pediculares, entre dos técnicas de inserción a nivel lumbar. Método. En 50 pacientes (27 varones y 23 mujeres) con una edad media de 54,5 años se obtuvieron imágenes por resonancia magnética de columnas no operadas, sin patología y alineadas, estudiándose la vértebra L4 con cortes perpendiculares al muro posterior, a nivel de los pedículos, utilizando el siguiente protocolo: FSE, potenciadas en T2 (TE:130 TR:1740), nex 2, espesor 2 mm, FOV: 256 x 256, con equipo Signa Infinity, GE 1,5 T ; las imágenes obtenidas fueron grabadas en disco óptico en sistema DICOM y transferidas a Workstation Sun Microsystems para su análisis mediante software Functool 2. Se trazaron líneas centradas en ambos pedículos siguiendo 2 ángulos (ANG): ANG 1 a 18°, desde la entrada a la cortical de las facetas articulares posteriores hasta el tercio anterior del cuerpo vertebral y ANG 2 a 38º, desde la entrada en la unión de la apófisis transversa con la articulación interapofisaria. Luego se midieron las áreas triangulares (mm2) resultantes de la proyección de las líneas sobre los cuerpos vertebrales. Para el análisis estadístico se emplearon: el ANOVA para medidas repetidas y el test de Scheffe para las comparaciones Post Hoc. Resultados. Los promedios del área en ANG 1 fueron: derecho 63,86 ± 1,77 e izquierdo 64,06 ± 1,54; las diferencias no fueron significativas (p=0,906). Los promedios del área en ANG 2 fueron: derecho 154,36 ± 1,87 e izquierdo 154,54 ± 1,81; las diferencias no fueron significativas (p=0,929). Las diferencias encontradas entre las magnitudes de las áreas derecha e izquierda a nivel L4, entre las dos técnicas de inserción, fueron muy significativas (p<0,0001). Conclusión. Las magnitudes de las áreas triangulares adyacentes a los tornillos con la técnica de inserción 2 fueron mayores que con la técnica 1 y sus diferencias fueron estadísticamente significativas.
