Chromone derivatives exhibiting high absorbance values in the UVA/UVB region were synthesized, and their photoprotective properties were evaluated. Chromones were prepared according to known literature procedures and characterized by high resolution mass spectrometry, infrared (IR) and nuclear magnetic resonance (NMR) spectroscopy. The in vitro solar protection factor (SPF) was determined by the Mansur method and cytotoxicity was evaluated using the sulforhodamine B assay. Two of the chromones synthesized demonstrated suitable SPF values and displayed no cytotoxic effect towards MRC-5 human fibroblasts at the tested concentrations, indicating great potential for future in vivo assays and clinical trials. Finally, the lead compound was incorporated into a nanoemulsion. Nanoemulsions showed high droplet size homogeneity and excellent stability. Chromones bearing methoxy substituents were found to be the most promising compounds with ideal photoprotective properties desirable for utilization and incorporation in sunscreen formulations.

A series of drugs was investigated to determine structural, electronic and pharmacological properties, as well as the molecular affinity for the main protease of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The drugs were submitted to density functional theory calculations to optimize structures and predict binding preferences. The optimized geometries were used in molecular docking simulations. In the docking study, the receiver was considered rigid and the drugs flexible. The Lamarckian genetic algorithm with global search and Pseudo-Solis and Wets with local search were adopted for docking. Absorption, distribution, metabolism, excretion and toxicological properties were obtained from the Pre-ADMET online server. In this series, the antiviral atazanavir showed the potential to inhibit the main protease of SARS-CoV-2, based on the free binding energy, inhibition constant, binding interactions and its favorable pharmacological properties. Therefore, we recommend carrying out further studies with in vitro tests and subsequent clinical tests to analyze its effectiveness in the treatment of SARS-CoV-2.

The key step in the chemoenzymatic synthesis of apremilast was to produce the chiral alcohol (R)-1-(3-ethoxy-4-methoxyphenyl)-2-(methylsulfonyl)ethanol, (R)-3. Two enzymatic approaches were evaluated to obtain (R)-3, one using ketoreductases and the other lipases. Bioreduction of 1-(3-ethoxy-4-methoxyphenyl)-2-(methylsulfonyl)ethanone (2), using ketoreductase KRED-P2-D12, led to (R)-3 with 48% conversion and 93% enantiomeric excess (ee). Kinetic resolution of rac-1-(3-ethoxy-4-methoxyphenyl)-2-(methylsulfonyl)ethyl acetate (rac-4), via hydrolysis reaction, with 20% of n-butanol, catalyzed by lipase from Aspergillus niger yielded (R)-3 with > 99% ee, 50% conversion and E-value (enantiomeric ratio) > 200. The reaction between enantiomerically pure (R)-3 and 4-acetylamino-isoindol-1,3-dione (8) afforded apremilast in 65% yield and 67% ee.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract Despite the great diversity, economic and environmental importance of limnic mollusks, the group is still little investigated in the Brazilian semiarid region, especially in the state of Piauí. Thus, this work aims to gather information on the diversity of limnic mollusks in the Brazilian Northeast and the semiarid region, including new records for the state of Piauí, northeastern Brazil. For this, collections in the urban perimeter of the Itaim and Guaribas Rivers, municipalities of Itainópolis, and Picos, respectively, between October 2017 and September 2019. Besides, a literature review was conducted on studies of malacofauna in the Northeast and the Brazilian semiarid region. A total of 11 species from the classes Gastropoda and Bivalvia were collected on the rivers of Piauí, being the first record of Uncancylus concentricus for the Northeast region. For lotic environments in the state of Piauí, Drepanotrema schubarti, Pomacea canaliculata, Pisidium dorbignyi and Stenophysa marmorata were registered for the first time. Then, it was possible to verify that the Northeast of Brazil has 71 species registered, of which 34 occur in the semiarid. The diversity of species surveyed and the first record of the occurrence of several of them for the semiarid region demonstrate the importance of expanding studies involving this group to other areas. The present study is the first to gather and increase the information available on the fauna of limnic mollusks for the Northeast and Brazilian semiarid regions. The information gathered here may contribute to future research and to the species conservation and the environments they occupy, especially considering the presence of non- native and vector species.

The production of fatty acid methyl esther (FAME) via transesterification was studied, assessing the influence of CaO heterogeneous catalysts obtained from four different fishery residues: sururu, crab, clam and mussel. Characterization and properties of the residues were obtained via thermogravimetric analysis, X-ray diffraction, X-ray fluorescence, Fourier transform infrared (FTIR) spectra, nitrogen adsorption/desorption, chemical composition and scanning electron microscopy (SEM). Catalytic activities and reaction kinetics of FAME synthesis from the transesterification of soybean oil were performed. FAME yield was determined by 1H nuclear magnetic resonance (NMR). A higher efficiency and reaction rate were observed for the catalysts obtained from the sururu residues with 93.7% FAME yield after 3.5 h of reaction at the first usage, reducing only to 91.0% after four consecutive cycles of reuse. The best activities were assigned to the presence of SrO, to smaller particle size, higher pore volumes and the higher Ca leaching, yielding Ca-diglyceroxide which is an important active phase for transesterification.

ABSTRACT The excessive sun exposure, coupled with lack of sun protection represents one of the biggest risks to the occurrence of skin cancer and photoaging. Recent strategies for photoprotection have included the incorporation of natural antioxidant and anti-inflammatory compounds, into sunscreens, and the oral administration of natural antioxidant extracts. In this work, we use Brazilian red propolis extract because its antioxidant and anti-inflammatory activities. The aim of this work was to evaluate the sun protection factor and antioxidant activity of different hydroalcoholic extracts of red propolis (70% and 75%) prepared from lyophilized red propolis at room and high temperatures. The sun protection factor in vitro was determined by a spectrophotometric method developed by Mansur. The hydroalcoholic extracts of red propolis incorporated with Filter UVA-UVB 5% Gel Permulem TR-1 presented absorption in the UVB region. Also, the in vitro capacity of the hydroalcoholic extracts of red propolis to increase photoprotective activity of Filter UVA-UVB 5% Gel was evaluated. The hydroalcoholic extracts of red propolis incorporated presented higher values of sun protection factor and showed synergism in the photoprotective activity of Filter UVA-UVB 5% Gel Permulem TR-1. The antioxidant activity and sun protection factor are correlated with total phenolics content of the extracts and the hydroalcoholic extract of red propolis 75% at room temperature was choosen. The formulation developed with Filter UVA-UVB 5% Gel Permulem TR-1 with this hydroalcoholic extract of red propolis showed safe to be applied on the skin according HET-CAM test. Suggests indication of hydroalcoholic extract of red propolis (75% – room temperature) associated to photoprotective formulations for use in photoprotective products.

RESUMO O objetivo com este estudo foi avaliar a utilização de silagens de diferentes cultivares de sorgo através do consumo, digestibilidade dos nutrientes e desempenho de ovinos. O experimento foi conduzido na Estação Experimental de Pendência, da Empresa Estadual de Pesquisa Agropecuária da Paraíba S.A. Para o experimento, foram utilizados trinta e cinco carneiros, sem raça definida, com peso vivo médio de 17,7 kg, distribuídos em um delineamento em blocos casualizados, com cinco tratamentos e sete repetições para determinar o consumo e desempenho; e 25 carneiros não castrados, sem raça definida, distribuídos em um delineamento inteiramente casualizado, com cinco tratamentos e cinco repetições, para determinar a digestibilidade dos nutrientes. O consumo médio diário de matéria seca foi, em média, 994,66 g. Somente os consumos médios de extrato etéreo, fibra em detergente neutro e carboidratos não-fibrosos, foram afetados pelas dietas. O ganho de peso (GP), conversão alimentar e eficiência alimentar dos animais que receberam as dietas não foram afetados pelos diferentes cultivares utilizados. Silagens de sorgo BRS 655, 610, 800, 810 e Ponta Negra promoveram consumo e digestibilidade de nutrientes semelhantes. A utilização de silagens de diferentes cultivares de sorgo (BRS 610, BRS 655, BRS 800, BRS 810 e Ponta Negra) em dietas para ovinos em confinamento promove desempenho e consumo similar.
SUMMARY The aim of this study was to evaluate the use of silages of different sorghum cultivars through digestibility and nutrient intake and performance of sheep. The experiment was conducted at the Pendência Experimental Station, of the State Company for Agricultural Research of Paraíba S.A. For the experiment, thirty-five rams were used, without defined breed, with average live weight of 17.7 kg, distributed in a completely randomized block design, with five diets and seven replicates to determine intake and performance; and 25 non-castrated rams, without defined breed, distributed in a completely randomized design with five diets and five replicates, to determine the digestibility of nutrients. The average daily intake of dry matter was, on average, 994.66 g. Only the average intakes of ether extract, neutral detergent fiber and non-fibrous carbohydrates, were affected by the diets. The weight gain (WG), feed conversion and feed efficiency of the animals receiving the diets suffered no effect. Silages of BRS 655, 610, 800, 810 and Ponta Negra provided similar intake and digestibility of nutrients. The use of silages of different sorghum cultivars (BRS 610, BRS 655, BRS 800, BRS 810 and Ponta Negra) in diets for confined sheep provide similar performance and intakes.

Resumo Avaliou-se a relação anatômica entre dentes posteriores e o soalho do seio maxilar (SSM) por meio da tomografia computadorizada de feixe cônico (TCFC) em 202 exames. A distância entre os ápices radiculares e o SSM, bem como a espessura do osso cortical do SSM próximo dos ápices radiculares e áreas de bifurcação foram medidas. As relações verticais e horizontais do SSM com as raízes dos molares foram classificados em categorias adaptadas a partir dos critérios propostos pelo Kwak et al. (14). A menor distância entre o SSM e os ápices dentários foi observada na raiz mesiovestibular do segundo molar (0,36±1,17 mm) e na raiz palatina do primeiro molar (0,45±1,10 mm), e a maior na raiz vestibular do primeiro pré-molar (5,47±4.43 mm). Diferenças significantes foram observadas entre a distância do SSM e os ápices dentários de primeiros e segundos pré-molares unirradiculares. A espessura da cortical óssea variou de 0,65±0,41 mm na região da raiz mesiovestibular do segundo molar a 1,28±0,42 na raiz vestibular do primeiro pré-molar. As relações vertical e horizontal mais prevalentes foram do tipo II e 2H, respectivamente. As raízes dos molares superiores apresentaram maior proximidade com o SSM. A espessura da cortical óssea do SSM nas regiões mais próximas dos ápices e área de furca foi similar apenas para os pré-molares.
Abstract This study evaluated the anatomical relationship between posterior teeth root apices and maxillary sinus floor (MSF) on 202 cone beam computed tomography (CBCT) exams. The distance between the root apices and the MSF, as well as the MSF thickness of the cortical bone closest to root apices and furcation regions were measured. The vertical and horizontal relationships of the MSF with the molar roots were classified into categories adapted from the criteria proposed by Kwak et al. (14). The shortest distances between MSF and the root apices were observed in the mesiobuccal root of the second molar (0.36±1.17 mm) and the palatal root of the first molar (0.45±1.10 mm) and the widest in buccal roots of the first premolars (5.47±4.43 mm). Significant differences were observed between the distance of MSF to the root apices of single-rooted first and second premolars. The cortical thickness ranged from 0.65±0.41 mm over the mesiobuccal root of the second molar to 1.28±0.42 mm over the buccal root of the first premolar. The most observed vertical and horizontal relationships were type II and 2H, respectively. The maxillary molar roots showed greater proximity to the MSF. The thickness of the MSF cortical bone closest to the apices and furcation regions was found to be similar only for premolars.

Resumo: O objetivo deste estudo foi avaliar a frequência de raízes, canais radiculares e forames apicais em dentes permanentes humanos por meio de imagens de tomografia computadorizada de feixe cônico (TCFC). Imagens de TCFC de 1.400 dentes de um banco de dados avaliadas anteriormente (1) foram usadas para determinar a frequência do número de raízes, canais radiculares e forames apicais. Em todos os dentes foi realizada avaliação por visualização em planos sagital, axial e coronal. Foram feitas navegações em cortes axiais de 0,1 mm/0,1 mm em sentido cervical para a direção apical, bem como de apical para cervical. Todas as imagens foram avaliadas por dois examinadores. Os dados estatísticos foram analisados, incluindo distribuição de frequência e tabulação cruzada. A maior frequência de quatro canais radiculares e quatro forames apicais foi encontrada em primeiros molares superiores (76%, 33%), seguido de segundos molares superiores (41%, 25%). A frequência de quatro canais radiculares nos primeiros molares inferiores foi de 51%. Primeiros pré-molares inferiores apresentaram dois canais radiculares e dois forames apicais em 29% e 20%, respectivamente. Os incisivos centrais e laterais inferiores e caninos apresentaram dois canais radiculares em 35%, 42% e 22% dos casos, respectivamente. A estratégia de navegação em imagens de TCCB favorece uma melhor identificação de frequência, posição das raízes, canais radiculares e forames apicais em dentes permanentes humanos..
Abstract: The aim of this study was to evaluate the frequency of roots, root canals and apical foramina in human permanent teeth using cone beam computed tomography (CBCT). CBCT images of 1,400 teeth from database previously evaluated were used to determine the frequency of number of roots, root canals and apical foramina. All teeth were evaluated by preview of the planes sagittal, axial, and coronal. Navigation in axial slices of 0.1 mm/0.1 mm followed the coronal to apical direction, as well as the apical to coronal direction. Two examiners assessed all CBCT images. Statistical data were analyzed including frequency distribution and cross-tabulation. The highest frequency of four root canals and four apical foramina was found in maxillary first molars (76%, 33%, respectively), followed by maxillary second molars (41%, 25%, respectively). The frequency of four root canals in mandibular first molars was 51%. Mandibular first premolars had two root canals and two apical foramina in 29% and 20% of the cases, respectively. Mandibular central and lateral incisors and canines presented two root canals in 35%, 42% and 22% of the cases, respectively. The navigation strategy in CBCT images favors a better identification of frequency and position of roots, root canals and apical foramina in human permanent teeth.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

INTRODUÇÃO: A conformabilidade do stent, definida como a adaptação da prótese à forma natural do vaso, é a principal responsável pelas alterações geométricas produzidas após o implante do dispositivo, sendo influenciada pelo material e pelo desenho do stent. Ela pode ser aferida medindo-se as modificações da curvatura e a angulação do segmento tratado após o implante do stent. O objetivo deste estudo foi comparar as mudanças na angulação coronária após implante do suporte vascular bioabsorvível (SVB) e das plataformas metálicas de cromo-cobalto e aço inoxidável, utilizadas em stents farmacológicos de segunda geração. MÉTODOS: Foram incluídos, nesta análise retrospectiva, 50 pacientes com lesões únicas, de novo, em coronárias nativas, com diâmetro entre 2,5 e 3,5 mm e extensão de até 23 mm. Vinte e cinco pacientes foram tratados com SVB e 25 pacientes com plataformas metálicas de cromo-cobalto (n = 12) ou de aço inoxidável (n = 13). A angulação foi medida usando um software de angiografia coronária quantitativa dedicado. RESULTADOS: A angulação do vaso modificou-se significativamente após o implante dos dispositivos. No grupo submetido ao implante de plataformas metálicas, houve maior modificação do ângulo coronário quando comparado ao tratado com SVB (41,6% vs. 14,9%; P < 0,01). Quando observado o comportamento do SVB e das diferentes plataformas metálicas, a alteração do ângulo coronário foi maior para as plataformas de aço inoxidável, seguida das plataformas de cromo-cobalto e os SVB (53,7% vs. 28,5% vs. 14,9%; P < 0,01). CONCLUSÕES: Nesta avaliação preliminar, o SVB produziu uma menor mudança da angulação coronária. O impacto clínico deste achado necessita ser investigado prospectivamente em uma coorte maior e mais complexa.
BACKGROUND: The conformability of the stent, defined as the adaptation of the prosthesis to the natural shape of the vessel, is the major cause of geometrical changes after stenting and is influenced by the stent material and design. It may be assessed by measuring changes in the curvature and the angulation of the treated segment after stent implantation. The objective of this study was to compare changes in coronary angulation after implantation of the bioresorbable vascular scaffold (BVS) and cobalt-chromium and stainless steel metal platforms used in second-generation drug-eluting stents. METHODS: In this retrospective analysis, 50 patients with single de novo lesions in native coronary arteries and diameter between 2.5 and 3.5 mm and length up to 23 mm were included. Twenty-five patients were treated with BVS and 25 patients were treated with cobalt-chromium (n = 12) or stainless steel (n = 13) platforms. Angulation was measured using a dedicated quantitative angiography analysis software. RESULTS: Vessel angulation significantly changed after device implantation. In the group submitted to the implantation of metal platforms there was greater coronary angulation change when compared to the group treated with BVS (41.6% vs. 14.9%; P < 0.01). When we analyzed the performance of the BVS and the different metal platforms, coronary angulation change was greater for the stainless steel platforms, followed by cobalt-chromium platforms and the BVS (53.7% vs. 28.5% vs. 14.9%; P < 0.01). CONCLUSIONS: In this preliminary assessment, the BVS produced a smaller coronary angulation change. The clinical impact of this finding must be prospectively investigated in a larger and more complex cohort.

INTRODUÇÃO: Suportes vasculares bioabsorvíveis (SVB) têm sido desenvolvidos como forma de fornecer sustentação à parede do vaso enquanto ocorre o processo de cicatrização, após a intervenção coronária percutânea (ICP), sendo absorvido posteriormente. Pelo fato da plataforma ser de material polimérico, existe preocupação em relação à retração aguda do dispositivo. Avaliamos aqui a retração aguda do SVB com a de dois diferentes stents farmacológicos metálicos. MÉTODOS: Foram incluídos 50 pacientes com lesões não complexas. Dentre esses pacientes, 25 foram tratados com SVB e comparados a outros 25 pacientes tratados com stent de cromo-cobalto eluidor de everolimus (EES; n = 12) ou stent de aço inoxidável eluidor de biolimus (BES; n = 13). A retração aguda foi definida como a diferença entre o diâmetro médio do balão durante a pressão máxima de inflação (X) e o diâmetro médio do stent após o esvaziamento do balão (Y). A porcentagem de retração aguda foi definida como (X - Y)/X. RESULTADOS: Não houve diferença significativa em relação às características clínicas e angiográficas basais. O ganho luminal agudo foi menor com o SVB comparado ao EES e ao BES (1,51 ± 0,41 mm vs. 1,76 ± 0,28 mm vs. 1,9 ± 0,42 mm; P = 0,02). A retração aguda foi de 0,21 ± 0,13 mm vs. 0,15 ± 0,08 mm vs. 0,14 ± 0,08 mm (P = 0,21), e o porcentual de retração aguda foi de 7,0 ± 4,6% vs. 5,0 ± 2,2% vs. 5,7 ± 4,1% (P = 0,16). CONCLUSÕES: O SVB demonstrou ter retração aguda ligeiramente maior, embora não significativa, que os stents metálicos farmacológicos de segunda geração.
BACKGROUND: Bioresorbable vascular scaffolds (BVS) have been developed to provide support to the vessel wall during the healing process after percutaneous coronary intervention (PCI), being reabsorbed afterwards. Because the scaffold is made of polymeric material, there is a concern regarding the acute recoil of the device. We compared the BVS acute recoil with that of two different metallic drug-eluting stents. METHODS: Fifty patients with non-complex lesions were included. Twenty-five of these patients were treated with a BVS who were compared to 25 patients treated with a cobalt-chromium everolimus-eluting stent (EES, n = 12) or a stainless steel biolimus-eluting stent (BES, n = 13). Acute recoil was defined as the difference between the mean diameter of the balloon during maximum inflation pressure (X) and the mean diameter of the stent immediately after balloon deflation (Y). The percentage of acute recoil was defined as (X-Y)/X. RESULTS: There was no significant difference in the baseline clinical and angiographic characteristics. Acute luminal gain was lower with BVS compared to EES and BES (1.51 ± 0.41 mm vs. 1.76 ± 0.28 mm vs. 1.9 ± 0.42 mm, P = 0.02). Acute recoil was 0.21 ± 0.13 mm vs. 0.15 ± 0.08 mm vs. 0.14 ± 0.08 mm (P = 0.21) and the percentage of acute recoil was 7.0 ± 4.6% vs. 5.0 ± 2.2% vs. 5.7 ± 4.1% (P = 0.16). CONCLUSIONS: BVS presented a slightly higher, although not significant, acute recoil than the two second-generation metallic drug-eluting stents.

INTRODUÇÃO: A revascularização percutânea de diabéticos é frequente e a utilização de stents farmacológicos (SF) é desejável, pois estes reduzem a reestenose e a necessidade de nova revascularização. O objetivo desse estudo foi comparar os resultados clínicos de longo prazo entre diabéticos tratados com e sem SF. MÉTODOS: Analisou-se uma coorte consecutiva de diabéticos submetidos à intervenção coronária percutânea (ICP) entre janeiro de 2009 e dezembro de 2012, em hospital terciário da rede pública. Esses pacientes foram acompanhados prospectivamente. RESULTADOS: Avaliamos 939 diabéticos, sendo 359 (38,3%) tratados com SF e 580 (61,7%) tratados com stents não farmacológicos (SNF). A taxa de eventos cardiovasculares adversos maiores (ECAM) em 12,6 ± 3,4 meses foi maior no grupo SNF (9,5% vs. 14,8%; risco relativo - RR = 1,56; intervalo de confiança de 95% - IC 95% 1,07-2,27; P = 0,02), assim como o óbito (2,8% vs. 6,7%; RR = 2,41; IC 95% 1,22-4,77; P < 0,01) e a revascularização do vaso alvo (3,9% vs. 7,2%; RR = 1,85; IC 95% 1,03-3,35; P = 0,04). Não foram observadas diferenças na incidência de infarto do miocárdio (1,7% vs. 0,5%; RR = 0,30; IC 95% 0,07-1,23; P = 0,08) ou acidente vascular encefálico (1,1% vs. 0,2%; RR = 0,15; IC 95% 0,01-1,37; P = 0,07). A análise multivariada revelou que a doença renal crônica (RR = 2,05; IC 95% 1,40-2,98; P < 0,01) e a síndrome coronária aguda (RR = 2,08; IC 95% 1,42-3,02; P < 0,01) foram os únicos preditores independentes de ECAM. CONCLUSÕES: Em pacientes diabéticos não selecionados, a evolução clínica tardia foi pior para os tratados com SNF. Após o ajuste das variáveis de confusão, o uso de SF não se mostrou preditor independente da redução de ECAM.
BACKGROUND: Percutaneous revascularization in diabetic is frequent and the use of drug-eluting stents (DES) is desirable, reducing restenosis and the need for repeat revascularization. The objective of this study was to compare the long-term outcomes of diabetic patients treated with and without DES. METHODS: A consecutive cohort of diabetic patients undergoing percutaneous coronary intervention (PCI) between 01/2009 and 12/2012 in a public tertiary hospital was prospectively followed-up. RESULTS: Nine hundred and thirty-nine diabetic patients (38.3%) treated with DES and 580 (61.7%) treated with bare metal stents (BMS) were evaluated. The rate of major adverse cardiac events (MACE) in 12.6 ± 3.4 months was greater in the BMS group (9.5% vs. 14.8%; RR, 1.56; 95% CI, 1.07-2.27; P = 0.02), as well as death (2.8% vs. 6.7%; RR, 2.41; 95% CI, 1.22-4.77; P < 0.01), and target vessel revascularization (3.9% vs. 7.2%; RR, 1.85; 95% CI, 1.03-3.35; P = 0.04). There were no differences in the incidence of myocardial infarction (1.7% vs. 0.5%; RR, 0.30; 95% CI, 0.07-1.23; P = 0.08) or stroke (1.1% vs. 0.2%; RR, 0.15; 95% CI, 0.01-1.37; P = 0.07). Multivariate analysis indicated that chronic kidney disease (RR, 2.05; 95% CI, 1.40-2.98; P < 0.01) and acute coronary syndrome (RR, 2.08; 95% CI 1.42-3.02; P < 0.01) were the only independent predictors of MACE. CONCLUSIONS: In non-selected diabetic patients the long-term clinical outcome was worse for patients treated with BMS. After adjusting for confounding variables, the use of DES was not an independent predictor of reduced MACE.

FUNDAMENTO: O envelhecimento e a aterosclerose estão relacionados à hipertensão renovascular em indivíduos idosos. Independentemente das comorbidades, a estenose de artéria renal é, por si só, importante causa de morbidade e mortalidade cardiovascular. OBJETIVO: Definir a sensibilidade, a especificidade, o valor preditivo positivo e o valor preditivo negativo dos exames não invasivos utilizados no diagnóstico de estenose da artéria renal. MÉTODOS: Um grupo de 61 pacientes recrutados permitiram a análise de 122 artérias e a definição de sensibilidade, especificidade e da contribuição relativa de cada exame realizado (Doppler, cintilografia e angiotomografia, comparados a arteriografia renal). RESULTADOS: A média das idades foi de 65,43 (desvio padrão: 8,7) anos. Das variáveis relacionadas à população do estudo e comparadas à arteriografia, duas estiveram correlacionadas à estenose da artéria renal, à disfunção renal e aos triglicerídeos. A mediana do ritmo de filtração glomerular foi de 52,8 mL/min/m². O Doppler identificou sensibilidade de 82,90%, especificidade de 70%, valor preditivo positivo de 85% e valor preditivo negativo de 66,70%. Para a tomografia, encontraram-se sensibilidade de 66,70%, especificidade de 80%, valor preditivo positivo de 87,50% e valor preditivo negativo de 55,20%. Esses achados permitiram identificar os exames que melhor detectavam a estenose. CONCLUSÃO: A tomografia e o Doppler mostraram qualidade e grande possibilidade no diagnóstico de estenose da artéria renal, com vantagem para o segundo, pois não há necessidade do uso de meio de contraste na avaliação de uma doença que, frequentemente, ocorre em diabéticos e associa-se à disfunção renal e à disfunção ventricular esquerda grave.
BACKGROUND: Aging and atherosclerosis are related to renovascular hypertension in elderly individuals. Regardless of comorbidities, renal artery stenosis is itself an important cause of cardiovascular morbidity and mortality. OBJECTIVE: To define the sensitivity, specificity, positive predictive value, and negative predictive value of noninvasive imaging tests used in the diagnosis of renal artery stenosis. METHODS: In a group of 61 patients recruited, 122 arteries were analized, thus permitting the definition of sensitivity, specificity, and the relative contribution of each imaging study performed (Doppler, scintigraphy and computed tomographic angiography in comparison to renal arteriography). RESULTS: The mean age was 65.43 years (standard deviation: 8.7). Of the variables related to the study population that were compared to arteriography, two correlated with renal artery stenosis, renal dysfunction and triglycerides. The median glomerular filtration rate was 52.8 mL/min/m². Doppler showed sensitivity of 82.90%, specificity of 70%, a positive predictive value of 85% and negative predictive value of 66.70%. For tomography, sensitivity was 66.70%, specificity 80%, positive predictive value 87.50% and negative predictive value 55.20%. With these findings, we could identify the imaging tests that best detected stenosis. CONCLUSION: Tomography and Doppler showed good quality and efficacy in the diagnosis of renal artery stenosis, with Doppler having the advantage of not requiring the use of contrast medium for the assessment of a disease that is common in diabetics and is associated with renal dysfunction and severe left ventricular dysfunction.
