RESUMO Racional: Avaliações pós-operatórias têm se tornado necessárias em ciruriga bariátrica considerando o grande número de operações e o tempo decorrido de sua realização. Objetivo: Avaliar o perfil psicológico de pacientes de um serviço público de cirurgia bariátrica antes e após o procedimento. Métodos: Foram avaliados no total 281 pacientes. Destes, 109 completaram as avaliações antes (T0) e até 23 meses após a operação (T1); 128 completaram as avaliações em T0 e entre 24 meses e 59 meses após a operação (T2); e 44 completaram as avaliações em T0 e 60 meses após a operação (T3). Foram utilizados entrevista semi-estruturada, Inventário Beck de Depressão (BDI), Inventário Beck de Ansiedade (BAI) e Escala de Compulsção Alimentar Periódica (ECAP). Resultados: Observou-se maior prevalência de mulheres (83%), pacientes com menos de 12 anos de escolaridade (83%) e pacientes que tinham um companheiro(a) (64%). Ao analisar todos os tempos de avaliação, observou-se que, com relação à ansiedade, depressão e compulsão alimentar, houve redução de todos os sintomas em T1, apontando para melhorias significativas nos primeiros 23 meses após a operação. Já em T2 e T3 observou-se aumento de todos os indicadores de ansiedade, depressão e compulsão alimentar, apontando para o impacto transitório da perda de peso alcançada pela cirurgia bariátrica nesses sintomas. Conclusões: Este estudo mostra a importância de avaliações psicológicas contínuas e a necessidade de intervenções multiprofissionais apropriadas para pacientes submetidos à cirurgia bariátrica, mesmo após a perda de peso.

ABSTRACT Background: As the number of surgeries increases and the elapsed time of the realization increases as well, the postoperative evaluations would become increasingly necessary. Aim: To assess the psychological profile before and after surgery. Methods: Were evaluated 281 patients from the public service of bariatric surgery. In this study, 109 patients completed the evaluations before surgery (T0) and up to 23 months after surgery (T1); 128 completed the evaluations in T0 and between 24 months and 59 months after surgery (T2); and 44 completed the evaluations in T0 and 60 months after surgery (T3). A semi-structured interview, the Beck Depression Inventory (BDI), Beck Anxiety (BAI), and the Binge Eating Scale (BES) were used. Results: There was a higher prevalence of female (83%), patients with less than 12 years of education (83%), and patients who have a partner (64%). Analyzing all times of evaluation, regarding anxiety, depression, and binge eating, there was a reduction in all symptoms in T1, pointing to significant improvements in the first 23 months after surgery. Already, in T2 and T3, there was an increase in all indicators of anxiety, depression, and binge eating pointing to the transient impact of weight loss or bariatric surgery on these symptoms. Conclusions: This study shows the importance of the continuous psychological evaluation and needs for the appropriate interventions for these patients who have undergone bariatric surgery, even after weight loss.

RESUMO Racional: O psicólogo que atua na área da cirurgia bariátrica tem papel de acolher, avaliar, preparar e conscientizar o paciente que vai ser submetido ao procedimento cirúrgico. A avaliação psicológica reveste-se de importância na medida em que obtém dados sobre história pessoal e familiar, além do rastreio de possíveis alterações psicopatológicas. Objetivo: Analisar as avaliações psicológicas dos pacientes de um serviço de cirurgia bariátrica de um hospital público, buscando traçar o perfil psicológico dos pacientes deste serviço. Métodos: Foram coletados dados de 827 pacientes entre 2001 e 2015, utilizando-se dados de uma entrevista, Inventário Beck de Depressão (BDI), Inventário Beck de Ansiedade (BAI) e Escala de Compulsão Alimentar Periódica (ECAP). Resultados: A idade média dos pacientes antes da operação foi 39 anos+10 e o IMC médio foi 51 kg/m²+7, sendo a maioria (81%) do gênero feminino. A pontuação média no BDI foi 14,8+8 e as mulheres tiveram pontuação significativamente maior que os homens. No BAI a pontuação média foi 11+ 8 e no ECAP 14+ 8, ambos sem diferença entre os grupos. Conclusão: A caracterização psicossocial dos pacientes avaliados aponta para a presença significativa de indicadores de depressão, com baixos níveis de ansiedade e de compulsão alimentar.

Abstract Background: The psychologist who works in bariatric surgery has a role to receive, evaluate, prepare and educate the patient who will undergo the surgical procedure. Psychological evaluation becomes important in so far as allows us to obtain data on personal and familiar history and allow tracing of possible psychopathology. Aim: To collect data on psychological evaluations of patients in a bariatric surgery service of a public hospital in order to describe the psychological profile of patients in this service. Method: Data were collected from 827 patients between 2001 and 2015, using data from an interview, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and Binge Eating Scale (BES). Results: The mean age of patients before surgery was 39 years+/- 10, the mean BMI was 51 kg/m²+7, and most patients (81%) were female. The average score on the BDI was 14.8+8 and women had significantly higher scores than men. On the BAI the average score was 11+8 and on the ECAP was 14+8, both with no difference between groups. Conclusions: Psychosocial characteristics of the patients points to the significant presence of indicators of depression, with low levels of anxiety and binge eating.

Objetivou-se investigar, através da Escala de Desenhos de Silhuetas, a percepção de tamanho e forma corporal de 150 mulheres, divididas com base no índice de massa corporal (IMC) em cinco grupos: não-obesidade, sobrepeso e obesidade graus I, II e III. As escolhas foram classificadas em representações de tamanho e forma corporal normal, real e ideal. Evidenciou-se que as escolhas de homem e mulher de tamanhos normais, nos cinco grupos, foram associadas a representações de baixo peso. Na auto-avaliação de tamanho e forma corporal real, notou-se ampla distribuição de escolhas, sendo que, com exceção do grupo com sobrepeso, a maioria das mulheres dos demais grupos fez escolhas compatíveis com seu IMC. O tamanho e forma corporal ideal foram associados a figuras representativas de baixo peso. Os dados apontam para a relevância das diferenças entre adequação da percepção real e ideal, sinalizando a direção das dificuldades relativas à autopercepção corporal.

The objective of the present study was to investigate women's size and shape body perception using the Nine-figure Outline Scale. A total of 150 women were divided into five groups according to body mass index (BMI): non-obese, overweight and grade I, II and III obese women. The options were classified in representations of normal, real and ideal body size and shape. It was evident that the men and women's choice for normal sizes, in the five groups, were predominantly connected to the representation of low body weight. In the self-assessment of the real body size and shape, it was noticed a broad distribution of choices, although with the exception of the pre-obesity group, the majority of women from the other groups made compatible choices with their BMI. The ideal body size and shape was associated with representative figures of low body weight. The data indicate the relevance of the differences between the adequacy of real and ideal perception, pointing at the difficulties related to body self-perception.

Objetiva-se avaliar a auto-imagem de 30 mulheres com obesidade mórbida, comparativamente a 30 mulheres não obesas. Procedeu-se à avaliação individual, utilizando-se o Desenho da Figura Humana (DFH) e entrevista complementar. Quanto aos aspectos gerais do desenho, os grupos diferiram significativamente em relação aos índices qualidade do grafismo, localização na página e temática, caracterizando predomínio de sentimentos de inadequação por parte das obesas. Quanto aos índices relativos ao tamanho do desenho, proporção, representação do tronco e tamanho de diferentes partes do corpo, os grupos diferiram entre si, sugerindo presença de indicadores de depreciação e distorção da imagem corporal entre as obesas. Observa-se que as mulheres obesas apresentaram dificuldade de expressar, simbolicamente, sua vivência corporal, sugerindo a presença de indicadores de sentimentos de inferioridade, descontentamento e preocupação com o corpo e a beleza.

The aim of this study was to assess self-image in 30 morbid obese women compared to 30 non obese women. Individual evaluation was carried out using the Draw a Person Test and a interview. Regarding general aspects of the drawing the groups differed significantly in quality of graphism, location in the page and theme indexes, indicating prevalence of feelings of enadequacy in the obese group. With regard to size of drawing, proportion, trunk representation and different body parts size indexes, the groups differed significantly, suggesting depreciation and distortion of body image in the obese group. It was observed that obese women had difficulty in expressing way their corporal existence, in a symbolic suggesting inferiority feelings, discontent and worry about body and beauty.

Morbid obesity in women - Eating style end quality of life. Morbid obesity has been widely studied due to its high incidence and its bad consequences to health. Studies about obesity have been emphasived some important aspects such as eating styles and quality of life. This study aim to investigate the eating styles and the quality of life of women with morbid obesity, patients in treatment at the Ambulatory of Eating Disorders, University Hospital of the Faculty of Medicine of Ribeirão Preto, University of São Paulo. Sixty women were assessed, 30 obese (Body Mass Index - BMI > 40 kg/m²) and 30 nonobese (BMI 20 to 25 kg/m²). A semi-structured interview, the Dutch Eating Behaviour Questionnaire and the Short-form Health Survey: Medical Outcomes Study were used for the assessment, after translate to portuguese and adapted. The data were rated and quantified and the groups were statistically analyzed through the Tests of Mann-Whitney. The groups differed significantly as for restrained (p<0.001) and emotional eating (p<0.01), and did not differ as for external eating. The groups also differed in quality of life, concerning physical and social limitations (p<0.001), the presence of emotional indicators (p<0.001) and general health condition (p<0.001). Findings suggest that eating styles contribute to explain the difficult of eating control - but are not only factor in the obesity characterization. Moreover, morbidly obese women presented a quality of life more compromised and have an adequate perception of their physical, social and emotional limitations. That might favor a therapeutic approach to them

A obesidade mórbida tem sido amplamente estudada em função de sua incidência e por suas conseqüências à saúde. Estudos têm destacado como variáveis relevantes os estilos alimentares e a qualidade de vida. Objetiva-se investigar os estilos alimentares e a qualidade de vida de mulheres com obesidade mórbida, pacientes do Ambulatório de Distúrbios de Conduta Alimentar do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo. Foram sujeitos 60 mulheres, sendo 30 obesas (Índice de Massa Corporal - IMC>40 kg/m²) e 30 não-obesas (IMC entre 20 e 25 kg/m²). Procedeu-se à aplicação de uma entrevista semi-estruturada, do Questionário Holandês de Comportamento Alimentar e do Exame de Saúde após tradução para a língua portuguesa e adaptação dos mesmos. Os dados foram cotados e quantificados, procedendo-se à comparação dos grupos através do Teste de Mann-Whitney. Os grupos diferiram significativamente quanto às subescalas alimentação restrita (p<0.001) com valores inferiores para o grupo de obesas, e alimentação emocional (p<0.01) com valores superiores para o grupo de obesas, não tendo se diferenciado quanto à alimentação externa. Também diferiram quanto às limitações físicas e sociais (p<0.001), à presença de indicadores emocionais (p<0.001) e ao estado geral de saúde (p<0.001), em todas as categorias com valores superiores para o grupo de obesas, caracterizando maior dificuldade. Conclui-se que: a) os três estilos alimentares parecem contribuir para explicar a dificuldade quanto ao controle alimentar, sem contudo terem um caráter exclusivo na caracterização da obesidade; b) as mulheres morbidamente obesas apresentaram maior comprometimento na qualidade de vida, bem como percepção adequada quanto aos prejuízos relativos às condições de saúde, podendo ser este um indicador favorável à abordagem terapêutica
