Conservative management of newborns with 35 weeks or more of gestational age at risk for early-onset sepsis: a Brazilian cohort study

Abstract Objective: To evaluate the conservative management of newborns born at ≥35 weeks of gestational age, at risk for early-onset neonatal sepsis (EOS). Methods: Retrospective, analytic cohort study (2016 to 2019), including newborns ≥35 weeks of gestational at risk of EOS, asymptomatic at birth, managed conservatively in full rooming-in: serial physical examination and clinical observation for at least 48 h. They were classified into three groups, according to the clinical course: asymptomatic (group A), symptomatic for other reasons (group B), and with sepsis (group C). Risk factors, clinical signs and differential diagnoses of sepsis, length of stay, and discharge conditions were evaluated. Results: The authors evaluated 769 asymptomatic newborns at risk of EOS. (mean birth weight 2999 ± 485 g and gestational age 37.6 ± 1.7 weeks, respectively) corresponding to 12.2% of rooming-in admissions. The most prevalent risk factors were colonization by Group B Streptococcus (29%), prolonged rupture membrane duration (21.9%) and preterm labor (21.4%). Most of all of them (53.9%) remained asymptomatic (group A). Group B corresponded for 45.3%, and the most common clinical signs were hypothermia (24.5%), tremors (8.7%) and vomiting (8%). Environmental dysthermia (50.7%), prematurity (20.0%), and feeding intolerance (15.7%) were common in Group B. Laboratory tests were performed in 3.5%. Five patients (one confirmed) comprised group C (0.8/1,000 live births). There were no deaths. The median length of stay was 64 h (IQR 50-93). Conclusion: The rate of clinical/confirmed EOS was low. Most of the symptomatic patients only needed clinical evaluation to rule out sepsis. Management was shown to be safe.

Stabilization of β-Galactosidase Encapsulated in Pectin-Alginate Hydrogel and Hydrolysis of Whey Lactose and Whole Milk

The operational conditions of β-galactosidase encapsulated in pectin-alginate hydrogel were investigated as well as its catalytic activity in the hydrolysis of lactose from whey and whole milk. β-Galactosidase encapsulated in pectin-alginate hydrogel showed the best results to yields and diffusional effect and pH range. Thermostability showed considerable income gain for half-life (1.7-fold), activation energy of denaturation (31.6-fold), activation of denaturation (217.3-fold at 40 ºC; 266.8-fold at 50 ºC; 345.6-fold at 60 ºC), and entropy of activation of denaturation (-42.0 J mol 1 K-1). Reusability of encapsulated β-galactosidase was observed in 8-cycles for the milk whey lactose hydrolysis (51.9%) and 7-cycles for whole milk (55.6%), and the lactose hydrolysis was observed in 13% in milk whey and 10.3% in whole milk after 10 cycles. These results revealed an industrial application potential of β-galactosidase encapsulated in pectin-alginate hydrogel for lactose hydrolysis of milk whey and whole milk processes.

Amides have been recognized as potent insecticidal natural products but, despite their variety of targets and mechanisms of action, their metabolic fate in insects is virtually unknown. The currently accepted hypothesis is that specialist herbivores are capable of biotransforming xenobiotics rendering them more polar and excretable while generalist insects do not have comparable capacity. The leaves from Piper tuberculatum, rich in insecticide amides, were offered to two insect species found on Piper leaves under natural conditions and also to four generalist grasshoppers in order to compare their capacity of biotransforming xenobiotics. The amides 1-7 were identified in the P. tuberculatum leaves and their corresponding carboxylic acids 8-13 were detected in frass samples of two host insects suggesting that these species promote the amides hydrolysis. The four generalist grasshoppers when offered P. tuberculatum leaves, starved to death after 72 h, indicating a strong antifeedant activity of P. tuberculatum leaves.

Resumo Objetivo: Investigar se há associação entre níveis de senso de coerência e desmame precoce. Método: O presente estudo tem natureza quantitativa e delineamento transversal. Foram investigados fatores associados ao desmame precoce (interrupção da oferta do leite materno à criança) em uma amostra de 425 mulheres com idade superior a 18 anos, sem distinção de etnia ou classe social, mães de crianças com até 36 meses, que não fossem gêmeos ou apresentassem deficiências sensoriais ou motoras. Foi feito o teste de qui-quadrado, com nível de significância de 5%, para testar a associação entre a variável dependente (desmame precoce) e as variáveis independentes (socioeconômicas, demográficas e nível de senso de coerência). As variáveis que apresentaram p ≤ 0,20 foram testadas no modelo de regressão logística múltipla. Os odds ratio e os respectivos intervalos de 95% de confiança foram estimados. Todos os testes estatísticos foram feitos com o programa SAS 9.2. Resultados: Os resultados apontaram que mães com maior senso de coerência possuem 1,82 vez mais chance de manter o aleitamento por mais tempo (p = 0,02). Conclusões: A identificação de mães com baixo senso de coerência permite a intervenção precoce dos profissionais de saúde, contribui para diminuir as taxas de desmame precoce na população.
Abstract Objective: To investigate whether there is an association between sense of coherence levels and early weaning. Method: This study had a quantitative nature and a cross-sectional design. Factors associated with early weaning (interruption of maternal breastfeeding) were investigated in a sample of 425 women older than 18 years, mothers of children up to 36 months of age who were not twins, with no sensory or motor deficiencies, without distinction of ethnicity or social class. The chi-squared test, with a significance level of 5%, was used to evaluate the association between the dependent variable (early weaning) and the independent variables (socioeconomic, demographic factors and sense of coherence level). Variables with p ≤ 0.20 were tested by the multiple logistic regression model. Odds ratio and the respective 95% confidence intervals were estimated. All statistical tests were performed using the SAS 9.2 software. Results: The results showed that mothers with greater sense of coherence were 1.82 times more likely to maintain breastfeeding for longer periods (p = 0.02). Conclusions: The identification of mothers with low sense of coherence allows the early intervention of health professionals, contributing to decrease the rates of early weaning in the population.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

OBJECTIVE: Non-pituitary tumors have been reported in a subset of patients harboring germline mutations in the aryl hydrocarbon receptor-interacting protein (AIP) gene. However, no detailed investigations of non-pituitary tumors of AIP-mutated patients have been reported so far. PATIENTS: We examined a MEN1- and p53-negative mother-daughter pair with acromegaly due to somatotropinoma. Subsequently, the mother developed a large virilizing adrenocortical carcinoma and a grade II B-cell non-Hodgkin's lymphoma. DESIGN: Mutational analysis was performed by automated sequencing. Loss-of-heterozygosity (LOH) analysis was carried out by sequencing and microsatellite analysis. AIP expression was assessed through quantitative PCR (qPCR) and immunohistochemistry. RESULTS: The functional inactivating mutation c.241C>T (R81X), which blocks the AIP protein from interacting with phosphodiesterase 4A (PDE4A), was identified in the heterozygous state in the leukocyte DNA of both patients. Analyzing the tumoral DNA revealed that the AIP wild-type allele was lost in the daughter's somatotropinoma and the mother's adrenocortical carcinoma. Both tumors displayed low AIP protein expression levels. Low AIP gene expression was confirmed by qPCR in the adrenocortical carcinoma. No evidence of LOH was observed in the DNA sample from the mother's B-cell lymphoma, and this tumor displayed normal AIP immunostaining. CONCLUSIONS: Our study presents the first molecular analysis of non-pituitary tumors in AIP-mutated patients. The finding of AIP inactivation in the adrenocortical tumor suggests that further investigation of the potential role of this recently identified tumor suppressor gene in non-pituitary tumors, mainly in those tumors in which the cAMP and the 11q13 locus are implicated, is likely to be worthwhile.

Resumo: O principal objetivo deste trabalho foi realizar a simulação computacional no software CadWind® de uma bobinagem filamentar para um vaso de pressão aplicado a motor foguete A simulação realizada foi de uma bobinagem helicoidal não-geodésica, variando o ângulo de bobinagem na parte cilíndrica do vaso de pressão, devido às aberturas polares serem diferentes e vaso comprido. Também foram realizadas simulações variando o padrão de bobinagem. Os resultados indicam que um padrão de bobinagem mais elevado proporciona um maior ancoramento e distribuição das fibras nos domos.
Abstract: The main objective of this work was to accomplish the computational simulation in the software CadWind® of a filament winding for an applied pressure vase to motor rocket. The adopted simulation was of a non-geodesic helical winding, varying the winding angle in the cylindrical part of the pressure vessel, due the polar holes be different and the vessel long. Also simulations were accomplished varying the winding pattern. The results indicate that a more substantial winding pattern provides increased fiber deposition and distribution on the outer region of the domes.

As respostas fisiológicas e perceptuais verificadas durante a caminhada vêm sendo amplamente pesquisadas; contudo, poucas investigações buscaram comparar essas respostas em grupos de diferentes índices de massa corporal durante caminhada em ritmo autosselecionado. O objetivo do presente estudo foi comparar as respostas fisiológicas e perceptuais entre mulheres com peso normal (PN), sobrepeso (SP) e obesas (OB) durante a caminhada em ritmo autosselecionado. Participaram 66 mulheres sedentárias, com idade entre 20 e 45 anos, designadas em três grupos de acordo com o seu índice de massa corporal: (i) PN (18,5 - 24,9kg·m-2, n = 22); (ii) SP (25,0 - 29,9kg·m-2, n = 22); e (iii) OB (> 30,0kg·m-2, n = 22). Cada participante realizou uma sessão de familiarização e duas sessões experimentais (teste incremental máximo e teste de 20 minutos de caminhada em ritmo autosselecionado), a fim de determinar as respostas fisiológicas (frequência cardíaca, FC; e consumo de oxigênio, VO2) e perceptuais (percepção subjetiva de esforço, PSE) de cada sessão. A ANOVA one-way foi empregada para comparar as respostas fisiológicas e perceptuais obtidas durante a caminhada em ritmo autosselecionado entre os três protocolos experimentais, empregando-se o teste post hoc de Tukey com p < 0,05. Embora os sujeitos com obesidade tenham registrado menor velocidade de caminhada (1,30 ± 0,13m·s-1) comparativamente aos com peso normal e sobrepeso (1,69 ± 0,11 e 1,58 ± 0,22m·s-1, respectivamente) (p < 0,05), eles apresentaram similar resposta fisiológica (58,7 ± 8,6; 63,1 ± 12,6; 64,8 ± 11,1 %VO2 e 75,1 ± 7,8; 78,8 ± 8,5; 78,7 ± 7,2 %FCmax, para os grupos normal, sobrepeso e obeso, respectivamente). Além disso, as respostas perceptuais também não diferiram entre os grupos experimentais (11,6 ± 1,6; 11,7 ± 1,6; 12,2 ± 1,8, respectivamente). Desse modo, pode-se concluir que, independente do IMC, mulheres adultas apresentam respostas fisiológicas e perceptuais similares durante caminhada em ritmo autosselecionado, mesmo caminhando em velocidades diferentes.
Physiological and perceived responses verified during gait have been widely studied; however, few investigations have tried to compare these responses in groups of different body mass indices during gait in self-selected rhythm. The aim of the present study was to compare the physiological and perceived responses among normative (N), overweighed (OW) and obese (OB) women during gait in self-selected rhythm. Sixty-six sedentary women, aged between 20 and 45 years and divided in three groups according to their body mass indices participated in the study: (i) N (18.5 - 24.9kg·m-2, n = 22); (ii) OW (25.0 - 29.9kg·m-2, n = 22) and (iii) OB (> 30.0kg·m-2, n = 22). Each participant performed a familiarization session and two experimental sessions (maximal incremental test and 20m-walking test in self-selected rhythm) to determine the physiological (heart rate - HR and oxygen consumption - O2) and perceived responses (perceived exertion - PE) of each session. One-way ANOVA was applied to compare the physiological and perceived responses obtained during gait in self-selected rhythm among the three experimental protocols, applying the Tukey's post hoc test with p < 0.05. Although the obese subjects have registered lower walking velocity (1.30 ± 0.13m·s-1) compared with the normative and overweighed ones (1.69 ± 0.11 and 1.58 ± 0.22m·s-1, respectively) (p < 0.05), they presented similar physiological response ((58.7 ± 8.6; 63.1 ± 12.6; 64.8 ± 11.1 %O2 and 75.1 ± 7.8; 78.8 ± 8.5; 78.7 ± 7.2 %HRmax, for the normative, overweighed and obese groups, respectively). Moreover, the perceived responses did not differ between experimental groups (11.6 ± 1.6; 11.7 ± 1.6; 12.2 ± 1.8, respectively). Therefore, it can be concluded that regardless of the BMI, adult women present similar physiological and perceived responses during gait in self-selected rhythm even when walking in different velocities.

OBJETIVO: Avaliar a geometria e a função do ventrículo esquerdo (VE) após a troca mitral com tração e fixação dos papilares, em portadores de insuficiência cardíaca terminal com insuficiência mitral secundária. MÉTODO: Dos 20 pacientes avaliados, 70% eram homens, com idade média de 50,2 anos e 55% recebiam inotrópicos. A fração de ejeção (FEVE) foi menor que 30% em todos; 85% estavam em classe funcional (CF) IV. Dezoito receberam próteses de pericárdio bovino e dois, mecânicas. Os períodos considerados foram: 3, 6, 12 e 18 meses. As variáveis consideradas: volume sistólico do VE (VS), a FEVE, os diâmetros sistólico e diastólico finais (DSF e DDF) e os volumes sistólico e diastólico finais (VSF e VDF). No estudo estatístico, empregou-se da análise de variância (AV) e o teste de Friedmann (F). A sobrevida foi aferida pelo método de Kaplan-Meyer. RESULTADOS: Dois (10%) faleceram no período imediato. A sobrevida no primeiro ano foi de 85%, no segundo, 44%, no terceiro, 44%, no quarto, 44% e no quinto, 44%. A comparação entre pré e 3 meses, empregando-se a AV, não revelou alteração significativa para o VS (p=0,086). Houve acréscimo da FEVE (p=0,008) e decréscimo do DDF (p=0,038); do DSF (p=0,008); do VDF (p=0,029) e do VSF (p=0,009). Os momentos pré, 3 e 6 meses, com o teste F, não revelaram alterações. Entre os momentos pré, 3 meses e final, empregando-se a AV, não houve significância. CONCLUSÃO: Há melhora da FEVE, dos VDF, VSF, DDF e DSF; até o terceiro mês. A partir de então, as variáveis permanecem estáveis.
OBJECTIVE: This study aimed at evaluating results of mitral valve replacement using a new technique of complete chordae tendineae adjustment for left ventricular remodeling. METHODS: Twenty end-stage idiopathic dilated cardiomyopathy patients with severe functional mitral valve regurgitation underwent mitral valve replacement. Seventeen (85%) were in functional class IV. Both anterior and posterior leaflets of the mitral valve were divided to obtain 4 pillars of chordae tendineae. These were displaced with traction toward the left atrium and anchored between the mitral annulus and a valvular prosthesis. To evaluate the left ventricular remodeling, Doppler echocardiography was performed. For statistical analysis, variance analysis and the Friedman's test were employed. RESULTS: Two (10%) early deaths occurred. Kaplan-Meyer showed survival at one year post-operative was 85%, 2 years it was 44%, at 3 years 44%, at 4 years 44% and at 5 years it was 44%. In the 48- and 54-month follow-ups, the McNemar test showed improvement in the functional class (p<0.001). In the third month of follow-up, variance analysis showed improvement in the ejection fraction (p=0.008) and reductions of the end diastolic diameter (p=0.038), end systolic diameter (p=0.008), end systolic volume (p=0.029) and end diastolic volume (p=0.009). No statistical differences were noted in the systolic volume. Comparing pre-operative and third- and six-month follow-ups, the Friedman test showed no statistical differences for all studied variables. Variance analyses between pre, three-month and final evaluations showed no significant differences. CONCLUSION: This technique of mitral valve replacement improved the left ventricle ejection fraction and decreased the end diastolic and systolic diameters and the end systolic diastolic volumes up to the third month of follow-up. From then on the variables stabilized.

A potencialidade de um pólo de fruticultura irrigada no Norte Fluminense é enorme, dada a região apresentar muitas vantagens. Entretanto, a viabilidade de um sistema de produção dependerá do adequado manejo do mesmo, carecendo de análises econômicas dos insumos aplicados e dos resultados obtidos para estabelecer uma diretriz. Baseado nessas premissas, o objetivo deste trabalho foi avaliar o manejo da água a aplicar por meio de análises econômicas das funções de resposta (água x produção) e, ainda, considerando os diferentes mercados a atingir, nacional e internacional. Foi montado um experimento com diferentes lâminas de irrigação na cultura do mamão, com a cultivar "Improved Sunrise Solo 72/12", plantada em fileiras duplas, com espaçamento 3,6 x 2,0 x 2,0 m. Foi utilizado um sistema de irrigação por microaspersão com vazão e pressão de 9,7 x 10-3 L s-1 e 150 kPa, respectivamente. Os níveis de irrigação aplicados foram em função da evapotranspiração de referência (ET0), ou seja, 0; 40; 80; 120; 160; 200 e 240% da ETo. Os resultados encontrados, no período de 16 meses, permitiram estimar que as lâminas ótimas econômicas (precipitação + irrigação) foram de 2.818 e 2.832 mm, correspondendo a 45,4 e 35,5 t ha-1 de produtividade do mercado nacional e internacional, respectivamente.
The potentiality of irrigated horticulture pole in the Northern Fluminense is enormous, as the region presents a lot of advantages. However, the viability of the production system will depend on its appropriate management, lacking economical analyses of the applied inputs and results obtained to settle down a guideline. Based on those premises, this work aims to evaluate the appropriate water management to be applied through economical analyses of the answer functions (water x production) and, moreover, considering the different markets to be reached, mainly national and international. An experiment was set up with different irrigation sheets in papaya crop, with "Improved Sunrise Soil 72/12" cultivar, in double arrays with spacing of 3.6 x 2.0 x 2.0 m. A microsprinkler irrigation system was used, with flow and pressure equal to 9.7 x 10-3 L s-1 and 150 kPa, respectively. The applied irrigation levels, as function of the reference evapotranspiration (ET0), were 0; 40; 80; 120; 160; 200 and 240% of ET0. The obtained results, in the period of 16 months, allowed to estimate that the best economical dephts (rain + irrigation) were equal to 2,818 and 2,832 mm, corresponding to 45.4 and 35.5 t ha-1 of the national and international market production, respectively.

OBJETIVOS: O presente estudo visou a avaliar se o perímetro braquial poderia ser utilizado para a triagem de crianças obesas na idade pré-escolar. MÉTODOS: 1.090 crianças com idades entre 12 e 59,99 meses foram estudadas, obtendo-se informações pessoais, além de peso, estatura e perímetro braquial. Foram obtidas as sensibilidades e especificidades de diferentes escores z de perímetro braquial para idade e perímetro braquial para estatura, para a detecção de crianças obesas, definidas como aquelas que apresentaram escore z de peso para estatura acima de 2. RESULTADOS: Das 1.090 crianças avaliadas, 6,6% apresentaram escore z de peso para estatura acima de 2. O escore z de 0,7 do indicador perímetro braquial para idade determina a melhor associação entre sensibilidade (76,5%) e especificidade (77,9%), para a detecção das crianças obesas. O escore z de 0,6 do indicador perímetro braquial para estatura determina a melhor associação entre sensibilidade (79,4%) e especificidade (77,6%) para a detecção das crianças obesas. CONCLUSÕES: Não se observou vantagem do uso do perímetro braquial relacionado à estatura quando comparado à sua relação com a idade. Dessa forma, propõem-se pontos de corte, utilizando-se apenas o indicador perímetro braquial para idade que, pela sua facilidade e rapidez de obtenção em populações, é um método alternativo viável para triagem de crianças pré-escolares portadoras de obesidade.
OBJECTIVE: To evaluate if mid-upper arm circumference (MUAC) can be used for obesity screening in preschool children. METHODS: 1,090 children aged from 12 to 59.99 months were studied. Personal information, weight, height and MUAC were obtained. Sensitivity and specificity to detect obese children were calculated for MUAC to age and MUAC to height z scores. Obesity was defined as weight to height z score > 2. RESULTS: 6.6% of the children had a weight to height z score > 2. The best association between sensitivity (76.5%) and specificity (77.9%) to detect obese children for MUAC to age was obtained with a z score of 0.7. In terms of MUAC to height, the best association between sensitivity (79.4%) and specificity (77.6%) to detect obese children was obtained with a 0.6 score. CONCLUSIONS: MUAC to height z score was not advantageous for obesity screening when compared to MUAC to age z score. MUAC to age z score seems to be an adequate alternative method for obesity screening in preschool children.
