RESUMO: Revestimento comestível de alginato de sódio incorporado com própolis marrom (2,5%, 5%, 10% e 15%) foi aplicado em grãos de pimenta-do-reino para melhorar a qualidade microbiológica ao longo de 30 dias. Análise de cromatografia gasosa associada à espectrometria de massa identificou 29 metabólitos no extrato, principalmente compostos terpênicos (51,74%), compostos fenólicos (25,83%) e flavonóides (14,48%). A própolis marrom apresentou maior atividade antibacteriana para bactérias Gram-positivas (CIM de 0,1 a 0,5 mg.mL-1) e menor atividade para Escherichia coli (CIM 18 mg.mL-1). Um aumento em 5% no revestimento da própolis reduziu a contagem de Bacillus cereus em sete vezes, 9,4% para Staphylococcus aureus e 5,4% para bactérias mesófilas. O revestimento comestível de alginato de sódio e própolis marrom foi eficaz na redução microbiana da pimenta-do-reino, em que a concentração de 15% de própolis garantiu a qualidade microbiológica da especiaria até 20 dias.

ABSTRACT: An edible coating of sodium alginate incorporated with brown propolis (2.5%, 5%, 10%, and 15%) was applied to black pepper grains to improve microbiological quality over 30 days. Gas chromatography coupled with mass spectrometry identified 29 metabolites in the extract, mainly terpene compounds (51.74%), phenolic compounds (25.83%), and flavonoids (14.48%). Brown propolis showed greater antibacterial activity for Gram-positive bacteria (MIC from 0.1 to 0.5 mg.mL-1) and lower activity for Escherichiacoli (MIC 18 mg.mL-1). A 5% increase in propolis in the coating reduced Bacilluscereus counts by 7-fold, 9.4% for Staphylococcusaureus, and 5.4% for mesophilic bacteria. The edible sodium alginate coating containing brown propolis was effective in reducing microbes on black pepper, with a concentration of 15% propolis assuring the microbiological quality of the spice after 20 days.

ABSTRACT Background: We aimed to evaluate the costs of GenoType® MTBDRplus and MTBDRsl incurred during the diagnosis of first- and second-line drug-resistant tuberculosis (TB) in São Paulo, Brazil. Methods: Mean and activity-based costs of GenoType® were calculated in a referral laboratory for TB in Brazil. Results: The mean cost value and activity-based cost of GenoType® MTBDRplus were USD 19.78 and USD 35.80 and those of MTBDRsl were USD 54.25 and USD 41.85, respectively. Conclusions: The cost of GenoType® MTBDRplus was reduced owing to the high number of examinations performed and work optimization.

Abstract Objective To determine the association between nutritional status according to different cutoff points for body mass index (BMI) and the occurrence of noncommunicable chronic diseases (NCD) in older people. Methods A cross-sectional study of 365 older people was conducted using a questionnaire collecting information on health variables, body weight and height measurements. The association between classification of nutritional status using BMI and occurrence of NCD was estimated by the Odds Ratio (OR) and 95% Confidence Intervals (95%CI). Results As measured by the different BMI classification criteria, nutritional status varied: for normal weight (24.9-32.3%), excess weight (57.3-73.2%) and for underweight (1.9-15.3%). According to the Lipschitz classification, underweight was associated with lower occurrence of osteoarticular diseases (OR=0.38; 95%CI: 0.15-0.93) and cardiometabolic diseases (OR=0.42; 95%CI: 0.19- 0.94); while overweight was associated with higher occurrence of cardiometabolic diseases (OR=2.26; 95%CI: 1.30-3.93). According to the World Health Organization criteria, underweight was associated with lower occurrence of cardiometabolic diseases (OR=0.09; 95%CI: 0.01-0.61), overweight with lower occurrence of neuropsychological diseases (OR=0. 47; 95%CI: 0.26-0.87), while obese status was associated with higher occurrence of osteoarticular (OR=1.95; 95%CI: 1.08-3.52) and cardiometabolic (OR=3.02; 95%CI: 1.54-5.93) diseases. According to the Pan American Health Organization criteria, underweight was associated with lower occurrence of cardiometabolic diseases (OR=0.45; 95%CI: 0.22-0.91) and obese status with higher occurrence of osteoarticular (OR=1, 91; 95%CI: 1.16-3.15), cardiometabolic (OR=2.58; 95%CI: 1.36-4.85) and respiratory (OR=1.96; 95%CI: 1.16-3.16) diseases. Conclusion The classification criteria for BMI were negatively (underweight) and positively (excess weight) associated with the occurrence of NCD, exhibiting a stronger association when the overweight classification was differentiated from the obese classification.

Resumo Objetivo Verificar a associação entre estado nutricional segundo diferentes pontos de corte para índice de massa corporal (IMC) e ocorrência de doenças crônicas não transmissíveis (DCNT) em idosos. Métodos Estudo transversal, realizado com 365 idosos de ambos os sexos, utilizando-se um questionário contendo variáveis sobre saúde e medidas de peso e estatura. A associação entre classificação do estado nutricional pelo IMC e ocorrência de DCNT foi estimada pelo Odds Ratio (OR) e Intervalos de 95% de Confiança (IC95%). Resultados A partir dos critérios de classificação do IMC, o estado nutricional variou entre: adequado (24,9%-32,3%), excesso de peso (57,3%-73,2%) e baixo peso (1,9%-15,3%). Segundo a classificação do estado nutricional por Lipschitz, o baixo peso se associou à menor ocorrência de doenças osteoarticulares (OR=0,38; IC95%:0,15-0,93) e cardiometabólicas (OR=0,42; IC95%:0,19-0,94); o sobrepeso se associou à maior ocorrência de doenças cardiometabólicas (OR=2,26; IC95%:1,30-3,93). Segundo critérios da Organização Mundial de Saúde, o baixo peso se associou à menor ocorrência de doenças cardiometabólicas (OR=0,09; IC95%:0,01-0,61), sobrepeso à menor ocorrência de doenças neuropsicológicas (OR=0,47; IC95%:0,26-0,87), obesidade à maior ocorrência de doenças osteoarticulares (OR=1,95; IC95%:1,08-3,52) e cardiometabólicas (OR=3,02; IC95%: 1,54-5,93). Pelos critérios da Organização Pan-Americana da Saúde, o baixo peso se associou à menor ocorrência de doenças cardiometabólicas (OR=0,45; IC95%:0,22-0,91), obesidade à maior ocorrência de doenças osteoarticulares (OR=1,91; IC95%:1,16-3,15), cardiometabólicas (OR=2,58; IC95%:1,36-4,85) e respiratórias (OR=1,96; IC95%:1,16-3,16). Conclusão Os critérios de classificação do IMC associaram-se negativamente (baixo peso) e positivamente (excesso de peso) à ocorrência de DCNT, verificando-se maior força na associação quando se diferenciou sobrepeso de obesidade.

ABSTRACT Hand-foot-and-mouth disease (HFMD) is a highly contagious viral disease commonly associated to Enteroviruses (EV). During 2018, Brazil faced massive HFMD outbreaks spread across the country. This study aimed to characterize the EV responsible for the HFMD outbreak that occurred in Paraiba State, Brazilian Northeastern region, in 2018, followed by a phylogenetic analysis to detail information on its genetic diversity. A total of 49 serum samples (one from each patient) collected from children ≤ 15 years old, clinically diagnosed with HFMD were tested for EV using conventional RT-PCR and RT-qPCR. EV infection was confirmed in 71.4% (35/49) of samples. The mean and median ages were 1.83 years and one year old, respectively. Twenty-two EV-positive samples were successfully sequenced and classified as EV-A species; 13 samples were also identified with the CV-A6 genotype. The phylogenetic analysis (VP1 region) of three samples revealed that the detected CV-A6 strains belonged to sub-lineage D3. The CV-A6 strains detected here clustered with strains from South America, Europe and West Asia strains that were also involved in HFMD cases during the 2017-2018 seasons, in addition to the previously detected Brazilian CV-A6 strains from 2012 to 2017, suggesting a global co-circulation of a set of different CV-A6 strains introduced in the country at different times. The growing circulation of the emerging CV-A6 associated with HFMD, together with the detection of more severe cases worldwide, suggests the need for a more intense surveillance system of HFMD in Brazil. In addition, this investigation was performed exclusively on serum samples, and the analysis of whole blood samples should be considered and could have shown advantages when employed in the diagnosis of enteroviral HFMD outbreaks.

Resumo: Introdução: A comunicação é uma competência essencial para o(a) médico(a) e outras categorias profissionais, e deve ser desenvolvida durante sua formação profissional. A elaboração de um projeto de comunicação, incluindo um consenso brasileiro, visou subsidiar as escolas médicas a preparar os estudantes de Medicina para se comunicarem efetivamente com os(as) cidadãos/cidadãs brasileiros(as), de características plurais intra e inter-regionais, pautando-se no profissionalismo e nos princípios do Sistema Único de Saúde (SUS). Objetivo: Este manuscrito apresenta o consenso para o ensino de comunicação nas escolas médicas brasileiras. Método: O consenso foi construído colaborativamente com 276 participantes, experts em comunicação, docentes, profissionais de saúde e discentes, de 126 escolas médicas e cinco instituições de saúde, ao longo de nove encontros presenciais em congressos e de encontros virtuais quinzenais ou mensais. Nos encontros, compartilharam-se as experiências dos participantes e o material bibliográfico, incluindo os consensos internacionais, e apresentou-se o consenso em construção, com discussão em grupos para elencar novos componentes para o consenso brasileiro, seguida por debate com todos para pactuá-los. A versão final foi aprovada em reunião virtual, com convite a todos(as) os(as) participantes em julho de 2021. Após submissão, diversas alterações foram requeridas, o que demandou novos encontros para revisão da versão final do consenso. Resultado: O consenso tem como pressupostos que a comunicação deve ser centrada nas relações, pautada nos princípios do SUS, na participação social e no profissionalismo, e embasada nas Diretrizes Curriculares Nacionais do curso de graduação em Medicina, em referenciais teóricos e nas evidências científicas. São descritos objetivos específicos para desenvolver a competência em comunicação nos estudantes, abrangendo: fundamentos teóricos; busca e avaliação crítica da literatura; elaboração e redação de documentos; comunicação intrapessoal e interpessoal no ambiente acadêmico-científico, na atenção à saúde em diversos contextos clínicos e na gestão em saúde. Recomenda-se a inserção curricular da comunicação do início ao final do curso, integrada a outros conteúdos e áreas de saber. Conclusão: Espera-se que esse consenso contribua para a revisão ou implementação da comunicação nos currículos das escolas médicas brasileiras.

Abstract: Introduction: Communication is an essential competence for the physician and other professional categories, and must be developed their professional training. The creation of a communication project including a Brazilian consensus aimed to subsidize medical schools in preparing medical students to communicate effectively with Brazilian citizens, with plural intra and inter-regional characteristics, based on the professionalism and the Brazilian Unified System (SUS) principles. Objective: The objective of this manuscript is to present the consensus for the teaching of communication in Brazilian medical schools. Method: The consensus was built collaboratively with 276 participants, experts in communication, faculty, health professionals and students from 126 medical schools and five health institutions in face-to-face conference meetings and biweekly or monthly virtual meetings. In the meetings, the participants’ experiences and bibliographic material were shared, including international consensuses, and the consensus under construction was presented, with group discussion to list new components for the Brazilian consensus, followed by debate with everyone, to agree on them. The final version was approved in a virtual meeting with invitation to all participants in July 2021. After the submission, several changes were required, which demanded new meetings to review the consensus final version. Result: The consensus is based on assumptions that communication should be relationship-centered, embedded on professionalism, grounded on the SUS principles and social participation, and based on the National Guidelines for the undergraduate medical course, theoretical references and scientific evidence. Specific objectives to develop communication competence in the students are described, covering: theoretical foundations; literature search and its critical evaluation; documents drafting and editing; intrapersonal and interpersonal communication in the academicscientific environment, in health care and in health management; and, communication in diverse clinical contexts. The inclusion of communication in the curriculum is recommended from the beginning to the end of the course, integrated with other contents and areas of knowledge. Conclusion: It is expected that this consensus contributes the review or implementation of communication in Brazilian medical schools’ curricula.

ABSTRACT Background: The burden of dengue in Brazil is poorly documented and is based on data from the public health care setting. This study estimated the prevalence and costs of dengue management in the private health care system in Brazil from 2015 to 2020 using a large claims database from Orizon. Methods: We selected claims with dengue ICD codes (ICD-10 A90 or A91) from January 2015 to December 2020. Prevalence was estimated based on the population enrolled in health insurance plans in the given year. Costs were adjusted for the inflation up to December 2021 and evaluated by measures of central tendency and dispersion. Results: A total of 63,882 unique beneficiaries were included, with a total of 64,186 dengue cases. The year with the highest prevalence was 2015 (1.6% of patients who used health plans), and there was also an increase in cases in 2016 and 2019. The median cost per hospitalization in 2015 was US$486.17, and in 2020, it reached US$696.72. The median cost of a case seen at an emergency room ranged from US$ 97.78 in 2015 to US$ 118.16 in 2017. Conclusions: The estimated prevalence of dengue in this population of private health-insured patients followed the epidemiological trends of the general population in Brazil, with the highest rates in 2015, 2016, and 2019. The cost of dengue management has increased in the private health care setting over the years.

Abstract Probiotic starter culture does not only improve the safety and shelf-life of products but also extends health benefits to the consumer. This study investigated the probiotic potential of a commercial starter culture used in the fermentation of meat sausages. The starter culture tested, composed of Lactobacillus sakei, Staphylococcus xylosus, and Staphylococcus carnosus, was evaluated for resistance to antimicrobials, low pH values and bile salts; production of gas and capsules; acidification capacity; and growth after exposure to different pH values, temperatures, and curing salts. The antagonistic capacity was also assessed against Escherichia coli ATCC25922, Salmonella Enteritidis ATCC13076, Vibrio parahaemolyticus, Staphylococcus aureus ATCC43300, Enterococcus faecalis ATCC29212, and Listeria monocytogenes CERELA. The starter culture was susceptible to all tested antimicrobials and strongly inhibited pathogenic strains, with inhibition halos diameters > 30 mm. The culture was resistant to all concentrations of bile salts tested, did not produce gas or capsules, and could grow within a temperature range of 15 °C to 35 °C in saline medium containing healing salts (nitrite/nitrate). Although, the inability of the culture to withstand low pH, indicating intolerance to stomach acidity, limits its use as a live probiotic, beneficial health effects may be derived from the inactivated culture.

Abstract INTRODUCTION: The intensification of research and innovation with the creation of networks of rapid and effective molecular tests as strategies for the end of tuberculosis are essential to avoid late diagnosis and for the eradication of the disease. We aimed to evaluate the cost-effectiveness of Xpert®MTB/RIF (Xpert) in the diagnosis of drug-resistant tuberculosis in reference units, in scenarios with and without subsidies, and the respective cost adjustment for today. METHODS: The analyses were performed considering as criterion of effectiveness, negative culture or clinical improvement in the sixth month of follow-up. The comparison was performed using two diagnostic strategies for the drug susceptibility test (DST), BactecTMMGITTM960 System, versus Xpert. The cost effectiveness and incremental cost-effectiveness ratio (ICER) were calculated and dollar-corrected for American inflation (US$ 1.00 = R$ 5,29). RESULTS: Subsidized Xpert had the lowest cost of US$ 33.48 (R$67,52) and the highest incremental average efficiency (13.57), thus being a dominated analysis. After the inflation was calculated, the mean cost was DST-MGIT=US$ 74.85 (R$ 396,73) and Xpert = US$ 37.33 (R$197,86) with subsidies. CONCLUSIONS: The Xpert in the diagnosis of TB-DR in these reference units was cost-effective with subsidies. In the absence of a subsidy, Xpert in TB-DR is not characterized as cost effective. This factor reveals the vulnerability of countries dependent on international organizations’ subsidy policies.

RESUMO Objetivo Analisar o efeito imediato da hidratação de superfície laríngea associado à técnica de vibração sonorizada de língua (TVSL) em cantores. Método Participaram 30 cantores, sem queixas vocais ou alterações laríngeas, divididos em grupo controle (GC) e experimental (GE). O GC realizou a TVSL por cinco minutos. O GE foi submetido à nebulização de 3 ml de solução salina seguido da TVSL por cinco minutos. Foram realizadas autoavaliação vocal, análise acústica e avaliação perceptivoauditiva nos momentos pré (PréTVSL) e pós (PTVSL) no GC e no momento pré (PréHTVSL), pós hidratação (PH) e pós hidratação+TVSL (PHTVSL) no GE. Na autoavaliação foram avaliados: qualidade, estabilidade, rouquidão e intensidade vocal. Os parâmetros acústicos analisados foram Frequência Fundamental; Jitter%; Shimmer%, Noise-to-harmonic Ratio e Cepstral Peak Prominence-Smoothed (CPPs). A avaliação perceptivoauditiva foi realizada por uma fonoaudióloga experiente. Resultados Na comparação dos resultados da autoavaliação, entre os grupos, observou-se melhora da percepção de estabilidade e intensidade vocal no PTVSL (GC) em relação ao PH (GE). Na comparação entre os momentos do GE houve diferença estatística na sensação de intensidade vocal, apontando melhor resultado para PHTVSL. Não houve diferença estatística entre os grupos investigados na avaliação perceptivoauditiva e na análise acústica. Conclusão A hidratação laríngea de superfície não potencializa o efeito da TVSL em cantores em condição natural de hidratação com uso de 3ml de nebulização. Para os profissionais da voz com grande demanda vocal, a hidratação de superfície pode ser introduzida durante a utilização da voz, para manutenção da qualidade vocal, sem perda de sua qualidade.

ABSTRACT Purpose To analyze the immediate effect of laryngeal surface hydration associated with the performance of Tongue Trills (TT) on singers. Methods Thirty singers without vocal complaints or laryngeal alterations divided into control (CG) and experimental (EG) groups. The CG performed the TT for five minutes. The EG was submitted a nebulization with 3 ml of saline solution followed by TT for five minutes. Voice self-assessment, acoustic analysis and perceptual assessment were performed at Pre (Pre TT) and post (PTT) moments in CG and pre (Pre TT), post hydration (PH) and post hydration + TT (PHTT) in GE. In the self-assessment were evaluated quality, stability, vocal intensity and hoarseness. There were extract the values of the Fundamental frequency; Jitter%; Shimmer%, Noise-to-harmonic Ratio e Cepstral Peak Prominence-Smoothed (CPPs) in the acoustic analyze. The perceptual evaluation was performed by an experienced speech therapist. Results Comparing the results of self-assessment between groups showed improvement in the perception of stability and vocal intensity in the PTT (CG) in relation to PH (EG). Comparison between the EG moments showed a statistical difference in the vocal intensity perception, indicating a better results for PHTT. There was no statistical difference between the groups investigated in the perceptual assessments and acoustic analysis. Conclusion Surface laryngeal hydration does not potentiate the effect of TT on naturally hydrated singers with 3ml nebulization. For voice professionals with high vocal demand, surface hydration can be introduced during voice use to maintain vocal quality.

RESUMO Objetivo Compreender as percepções de mulheres com doença falciforme (DF) sobre planejamento reprodutivo em serviço público de saúde. Método Estudo qualitativo onde foram entrevistadas 15 mulheres com DF atendidas em um hospital público em Recife, entre agosto/2018 e maio/2019. Foi empregada a técnica de análise de conteúdo temática de Bardin. Resultados As mulheres tinham entre 25 e 38 anos, eram casadas e de baixa escolaridade. Após a análise emergiram quatro categorias temáticas: informações sobre contracepção, uso de contraceptivos, sentimento sobre gestação e consulta ginecológica. Observou-se que a mulher com DF sofre influência de fatores como medo da gestação, falta de informação sobre planejamento reprodutivo, influência do parceiro e do profissional de saúde e dificuldades de acesso ao serviço de saúde. Considerações finais Os relatos demonstram que os profissionais de saúde precisam melhorar a forma como à informação chega à paciente de modo a oferecer uma prática profissional mais satisfatória.

ABSTRACT Objective To understand the perceptions of women with sickle cell disease (SCD) about reproductive planning in a public health service. Method This is a qualitative study conducted with 15 women with SCD attended at a public hospital in Recife, between August 2018 and May 2019. Bardin’s content analysis technique was used. Results The women were between 25 and 38 years old, married, and had low education. After the analysis, four thematic categories emerged: information about contraception, use of contraceptive methods, feelings about pregnancy, and gynecological consultation. It was found that women with SCD are influenced by factors such as fear of complications during pregnancy, lack of information on reproductive planning, partners and health professional opinion, and difficulties in health service access. Final considerations The reports demonstrate that health workers need to improve the way through which information reaches the patient, to offer a more satisfactory professional practice.

RESUMEN Objetivo Comprender las percepciones de las mujeres con anemia de células falciformes (AF) sobre la planificación reproductiva en un servicio de salud pública. Método Estudio cualitativo donde se atendió a 15 mujeres con AF en un hospital público de Recife, entre agosto de 2018 y mayo de 2019. Se utilizó la técnica de análisis de contenido temático de Bardin. Resultados Las mujeres tenían entre 25 y 38 años, estaban casadas y tenían baja escolaridad. El análisis generó cuatro categorías temáticas: información sobre anticoncepción, uso de anticonceptivos, sentimientos sobre el embarazo y consulta ginecológica. Se observó que las mujeres con AF son influenciadas por factores como el miedo del embarazo, la falta de información sobre planificación reproductiva, la influencia de su pareja y del profesional de la salud, y dificultades para acceder al servicio de salud. Consideraciones finales Los informes demuestran que los profesionales de la salud necesitan mejorar la forma en que la información llega al paciente para ofrecer una práctica profesional más satisfactoria.

RESUMO O objetivo do estudo foi avaliar o histórico de quedas e investigar as condições de saúde e fatores de risco para sua ocorrência em idosos cadastrados nas unidades da Estratégia de Saúde da Família. Foram entrevistados 163 idosos cadastrados em dez unidades da Estratégia de Saúde da Família, por meio da aplicação de questionário estruturado para caracterização da presença de doenças autorreferidas, quedas nos últimos 12 meses, características da moradia e recebimento de visitas de profissionais de saúde. Além disso, os idosos foram submetidos a uma avaliação antropométrica. O índice de quedas foi de 31,3%, sendo que o gênero (p=0,032), o autorrelato de muitos móveis na residência (p=0,013) e a presença de doenças cardiometabólicas/respiratórias (p=0,024) são os fatores que se relacionam significantemente com a ocorrência de quedas no último ano. Os idosos do grupo quedas apresentaram maior índice de massa corporal (p=0,014) e maior número de doenças cardiometabólicas/respiratórias (p=0,015) e neuropsicológicas (p=0,049). Aproximadamente um terço dos idosos caíram no último ano, e a maior presença de quedas ocorreu em mulheres e em idosos que apresentaram maior presença de doenças cardiometabólicas/respiratórias e neuropsicológicas, além de maior índice de massa corporal e muitos móveis na residência.

ABSTRACT This study aimed to evaluate the history of falls and investigate health conditions and risk factors for falls in elderly subjects enrolled in the Family Health Strategy units. One hundred sixty-three elderly subjects registered in ten Family Health Strategy units were interviewed by applying a structured questionnaire to characterize living conditions, self-reported diseases, the occurrence of falls in the last 12 months, housing characteristics, and receiving visits from a health professional. In addition, the elderly underwent an anthropometric assessment. The results indicate a fall rate of 31.3%, with gender (p=0.032), self-report of how much furniture at home (p=0.013), and the presence of cardiometabolic/respiratory diseases (p=0.024) being the factors that are significantly related to the presence of falls in the last year. Elderly fallers had higher body mass index (p=0.014) and higher number of cardiometabolic/respiratory (p=0.015) and neuropsychological diseases (p=0.049). It can be concluded that approximately one-third of the elderly fell in the last year. A greater presence of falls occurs in women and the elderly with a higher presence of cardiometabolic/respiratory and neuropsychological diseases, in addition to higher body mass index and furniture at home.

RESUMO OBJETIVO: Descrever a mortalidade por hepatites B e C como causa básica no município de São Paulo, verificando a tendência dessas taxas, e avaliar a associação dessas doenças a outras no período de 2002 a 2016. MÉTODOS: Trata-se de um estudo de série temporal sobre mortalidade por hepatites B e C segundo sexo, com dados do Sistema de Informação de Mortalidade. A regressão de Prais Winsten foi usada na análise de tendência das taxas. RESULTADOS: Os achados do presente estudo mostraram tendência de declínio da mortalidade por hepatites B e C nos últimos anos, particularmente entre pessoas do sexo masculino. Essas infecções foram causas associadas importantes ao carcinoma de células hepáticas e ao HIV. Destaca-se a proporção de óbitos com menos de 70 anos de idade. CONCLUSÕES: O estudo fornece uma linha de base para pesquisas de tendência de mortalidade e de impacto de intervenções, visto o histórico de ampliação da detecção e oferta de tratamentos, incluindo os mais recentes antivirais no Brasil, desde 2015.

ABSTRACT OBJECTIVE: To describe mortality due to hepatitis B and C as underlying cause in the municipality of São Paulo, verifying the trend of these rates, and to assess the association of these diseases with others, from 2002 to 2016. METHODS: This is a time series study on mortality due to hepatitis B and C according to sex, with data from the Sistema de Informação de Mortalidade (SIM – Mortality Information Sistem). Prais Winsten regression was used in rate trend analysis. RESULTS: The findings of this study showed a trend of decline of mortality from hepatitis B and C in recent years, particularly among males. These infections were important associated causes of liver cell carcinoma and HIV. The proportion of deaths under 70 years of age stands out. CONCLUSIONS: The study provides a baseline for research on mortality trend and the impact of interventions, given the history of expanded detection and supply of treatments, including the most recent antivirals in Brazil, since 2015.

RESUMO OBJETIVO: Descrever a mortalidade por hepatites B e C como causa básica no município de São Paulo, verificando a tendência dessas taxas, e avaliar a associação dessas doenças a outras no período de 2002 a 2016. MÉTODOS: Trata-se de um estudo de série temporal sobre mortalidade por hepatites B e C segundo sexo, com dados do Sistema de Informação de Mortalidade. A regressão de Prais Winsten foi usada na análise de tendência das taxas. RESULTADOS: Os achados do presente estudo mostraram tendência de declínio da mortalidade por hepatites B e C nos últimos anos, particularmente entre pessoas do sexo masculino. Essas infecções foram causas associadas importantes ao carcinoma de células hepáticas e ao HIV. Destaca-se a proporção de óbitos com menos de 70 anos de idade. RESUMO: Este estudo teve como objetivo avaliar a qualidade microbiológica e a veiculação de bactérias multirresistentes em diferentes especiarias comercializadas em feiras livres nos municípios do Recôncavo Baiano. Foram analisadas amostras de pimenta-do-reino, orégano e canela durante seis meses e pesquisados coliformes a 45 °C, Staphylococcus spp., Staphylococcus aureus, Bacillus spp., Bacillus cereus, Escherichia coli e Salmonella spp. A contaminação nas amostras de pimenta-do-reino (log 4,66 UFC g-1) foi maior (P>0,05), quando comparado com as amostras de canela (log 2,55 UFC g-1) e orégano (log 2,49 UFC g-1), principalmente para B. cereus. E. coli (89%) e Salmonella spp. (67%) foram isoladas apenas na pimenta-do-reino. B. cereus e S. aureus apresentaram maior resistência aos β-lactâmicos (penicilina, oxacilina e cefepime), com cerca de 40% das cepas com índice MAR de 0,33 (resistência a 3 antimicrobianos). E. coli foi mais resistente a ampicilina e Salmonella spp. ao ácido nalidíxico, ampicilina e ceftriaxona. Salmonella spp. apresentou índice MAR variando de 0,16 a 0,91 (até 11 antimicrobianos), e E. coli até 0,58 (7 antimicrobianos). Com isso, as especiarias comercializadas nas feiras livres do Recôncavo Baiano apresentam baixa qualidade microbiológica, com presença de patógenos e elevada resistência a antimicrobianos comumente usados no tratamento de enfermidades transmitidas por alimentos.

ABSTRACT: This study aimed to evaluate the microbiological quality and the transmission of multidrug-resistant bacteria in different spices sold in town fairs (local food markets) in the municipalities of Recôncavo Baiano. Samples of black pepper, oregano, and cinnamon were collected over a period of six months and investigated for coliforms at 45 °C, Staphylococcus spp., Staphylococcus aureus, Bacillus spp., Bacillus cereus, Escherichia coli and Salmonella spp. The contamination in the black pepper samples (log 4.66 CFU g-1) was higher (P>0.05), than those of cinnamon (log 2.55 CFU g-1) and oregano (log 2.49 CFU g-1), particularly for B. cereus. E. coli (89%) and Salmonella spp. (67%) were isolated only from black pepper. B. cereus and S. aureus showed greater resistance to β-lactams (penicillin, oxacillin, and cefepime), with approximately 40% of the strains with a multiple antimicrobial resistance (MAR) index of 0.33 (i.e., resistant to three antimicrobials). E. coli was more resistant to ampicillin and Salmonella spp. to nalidixic acid, ampicillin, and ceftriaxone. Salmonella spp. had a MAR index ranging from 0.16 to 0.91 (i.e, resistant to up to 11 antimicrobials), and E. coli to up to 0.58 (i.e., resistant to 7 antimicrobials). In conclusion, the spices sold in the town fairs of Recôncavo Baiano are of low microbiological quality, with the presence of pathogens, of which some display high resistance to antimicrobials that are commonly used for treating foodborne illnesses.

Resumo Objetivo: descrever características sociodemográficas e assistenciais de mulheres que morreram por causa materna em Recife, Pernambuco, Brasil. Métodos: estudo descritivo utilizando o Sistema de Informações sobre Mortalidade, fichas de investigação e fichas-síntese de óbitos maternos, precoces e tardios, ocorridos entre 2006 e 2017, com evitabilidade avaliada pelo Comitê Municipal de Mortalidade Materna. Resultados: identificaram-se 171 óbitos, 133 no puerpério; a maior parte dos óbitos ocorreu em negras (68,4%), sem companheiro (60,2%), acompanhadas com atendimento pré-natal (77,2%), de parto em maternidades/hospitais (97,1%), assistidas por obstetras (82,6%); das mulheres com complicações puerperais, 10,4% não tiveram assistência; óbitos evitáveis/provavelmente evitáveis corresponderam a 81,9%, por causas indiretas (n=80) e diretas (n=79). Conclusão: as mortes ocorreram principalmente no puerpério e em negras; falhas assistenciais foram frequentes; é necessária melhor vigilância e acompanhamento dos serviços de saúde no período gravídico-puerperal, em Recife.

Abstract Objective: to describe the sociodemographic and health care characteristics of women dying due to maternal causes in Recife, Pernambuco, Brazil. Methods: this was a descriptive study using the Mortality Information System, case investigation sheets and summary sheets of early and late maternal deaths occurring between 2006 and 2017, with avoidability assessed by the Municipal Maternal Mortality Committee. Results: we identified 171 deaths, of which 133 were in the puerperium; most deaths occurred among Black women (68.4%), women without partners (60.2%), women who had prenatal care (77.2%), during maternity hospital/general hospital delivery (97.1%), women attended to by obstetricians (82.6%);10.4% of women with puerperal complications had no health care; avoidable/probably avoidable deaths corresponded to 81.9%, for indirect causes (n=80), and direct causes (n=79). Conclusion: deaths occurred mainly in the postpartum period, among Black women; care failures were frequent; improved health service surveillance and follow-up is needed in the pregnancy-puerperal period, in Recife.

Resumen Objetivo: describir características sociodemográficas y asistenciales de mujeres que murieron por causa materna en Recife, Pernambuco, Brasil. Métodos: estudio descriptivo utilizando el Sistema de Informaciones sobre Mortalidad, fichas de investigación y síntesis de muertes maternas, tempranas y tardías, entre 2006 y 2017, con evaluación de la evitabilidad por el Comité Municipal de la Mortalidad Materna. Resultados: se identificaron 171 óbitos maternos, 133 en el puerperio; la mayoría de las muertes ocurrió en negras (68,4%), sin compañero (60,2%), acompañadas con atención prenatal (77,2%), de parto en maternidades/hospitales (97,1%), asistidas por obstetras (82,6%); de las mujeres con complicaciones puerperales, el 10,4% no tuvo asistencia; muertes evitables/probablemente evitables correspondieron al 81,9%, por causas indirectas (n=80) y directas (n=79). Conclusión: las muertes ocurrieron principalmente en el período del puerperio y en mujeres negras, con frecuentes fallas en la atención; se requiere una mayor vigilancia y acompañamiento de los servicios de salud en el período de embarazo-puerperio, en Recife.
