Abstract Sandblasting is a common method to try to improve the Y-TZP/veneer bond strength of dental prostheses, however, it may put stress on zirconia surfaces and could accelerate the t→m phase transformation. Y-TZP sandblasting before sintering could be an alternative to improve surface roughness and bonding strength of veneering ceramic. Objectives. The aim of this study was to analyze the effect of Y-TZP pre-sintering sandblasting on surface roughness, phase transformation, and the Y-TZP/veneer shear bond strength. Material and Methods. The Y-TZP specimen surface underwent sandblasting with aluminum oxide (50 μm) pre-sintering (Z-PRE) and post-sintering (Z-POS). Z-CTR was not subjected to surface treatment. After ceramic veneer application, the specimens were subjected to shear bond testing. Surface roughness was analyzed by confocal microscopy. Y-TZP monoclinic and tetragonal phases were evaluated by micro-Raman spectroscopy. Shear bond strength and surface roughness data were analyzed by One-way ANOVA and Tukey tests (α=0.05). Differences in the wave numbers and the broadening bands of the Raman spectra were compared among groups. Results. Z-POS (9.73±5.36 MPa) and Z-PRE (7.94±2.52 MPa) showed the highest bond strength, significantly higher than that of Z-CTR (5.54±2.14 MPa). The Ra of Z-PRE (1.59±0.23 µm) was much greater and significantly different from that of Z-CTR (0.29±0.05 µm) and Z-POS (0.77±0.13 µm). All groups showed bands typical of the tetragonal (T) and monoclinic (M) phases. Y-TZP sandblasting before sintering resulted in rougher surfaces but did not increase the shear bond strength compared to post-sintering and increased surface defects. Conclusions. Surface treatment with Al3O2, regardless of the moment and application, improves the results of Y-TZP/veneer bonding and is not a specific cause of t→m transformation.

Molares superiores apresentam morfologias radiculares e canais radiculares variáveis. Este relato de caso descreve o manejo endodôntico de dois casos de canais mésio-centrais (MC) encontrados em molares superiores. Canais MC estavam presente em um primeiro molar superior com uma única raiz vestibular (Caso 1), e em um segundo molar com três raízes vestibulares (Caso 2). Uma avaliação da configuração interna destes dentes foi realizada usando tomografia computadorizada de feixe cônico (TCFC). Ampliação com microscópio operatório odontológico, lupas cirúrgicas e o uso de um explorador endodôntico possibilitaram a identificação dos orifícios dos canais MC. Os canais radiculares dos dois casos foram preparados e obturados. Imagens radiográficas pós-obturação revelaram quatro (um MC, dois mésio-vestibulares e um palatino) e cinco (um MC, dois mésio-vestibulares, um disto-palatino e um palatino) canais radiculares, que foram obturados nos Casos 1 e 2, respectivamente. Configurações complexas de canais em molares superiores, incluindo a presença de canais MC foram apresentadas. TCFC foi uma ferramenta útil no diagnóstico e propiciou uma descrição precisa destas variações anatômicas incomuns.

Maxillary molars present variable root canal and root morphologies. This report describes the endodontic management of two cases of midbuccal canals found in maxillary molars. Midbuccal canals were present in a maxillary first molar with a single buccal root (Case 1), and in a maxillary second molar with three buccal roots (Case 2). An assessment of the internal configuration of these teeth was performed using cone-beam computed tomography (CBCT). Magnification with a dental operating microscope, surgical loupes, and the use of an endodontic explorer enabled the identification of the midbuccal canal orifices. The root canals in both cases were chemomechanically prepared and filled. Postobturation radiographic images revealed four (one midbuccal, two mesiobuccal and one palatal) and five (one midbuccal, two mesiobuccal, one distobuccal and one palatal) root canals, which were filled in Cases 1 and 2, respectively. Complex canal configurations of maxillary molars including the presence of midbuccal canals were presented. CBCT was a valuable tool in this diagnosis, as it provided a precise description of these unusual anatomical variations.

RESUMO Objetivo: Avaliar o impacto de longo prazo (com seguimento mínimo de 2 anos) de polimorfismos em genes de citocinas em pares doador:receptor sobre os resultados do transplante. Métodos: Comparamos os polimorfismos genéticos das citocinas e os principais fatores de risco para o desenvolvimento de rejeição crônica em grupos pareados de pacientes transplantados renais com e sem nefropatia crônica do aloenxerto [CAN]. Resultados: A análise multivariada indicou que a presença do genótipo TT (códon 10) de alta produção do fator de crescimento transformador beta-1 (TGFB1) era protetor nos receptores (p=0,017), em contraste com o risco aumentado quando presente nas amostras de doadores (p=0,049). Por outro lado, no caso do interferon gama estudado, a maior frequência do alelo de alta produção foi protetora na análise do grupo de doadores (p=0,013), mas aumentava o risco de nefropatia crônica do aloenxerto quando presente nos receptores (p=0,036). Conclusão: Nossos resultados ressaltam a importância da genotipagem de TGFB1 também em doadores, e indicam que polimorfismos no gene desta citocina em células do doador podem contribuir no desenvolvimento da nefropatia crônica do aloenxerto.

ABSTRACT Objective: To assess the long-term impact (minimum of 3 years follow-up) of polymorphisms in cytokine genes in donor:recipient pairs on the results of the transplant. Methods: We compared genetic cytokine polymorphisms and the primary factors of risk for the development of chronic rejection in paired groups of renal transplant patients with and without chronic allograft nephropathy [CAN]. Results: Multivariate analysis indicated that the presence of the high-production TT genotype (codon 10) of the transforming growth factor beta-1 (TGFB1) was protective in receptors (p=0.017), contrasting with the increased risk when present in donor samples (p=0.049). On the other hand, in the case of the gamma interferon studied, the greater frequency of the high production allele was protective in the analysis of the donor group (p=0.013), increasing the risk of chronic nephropathy of the allograft when present in the recipients (p=0.036). Conclusion: Our results highlight the importance of TGFB1 genotyping in donors, and indicate that polymorphisms in the gene of this cytokine in donor cells might contribute to the development of chronic allograft nephropathy
