Abstract Background Computerized cognitive training programs may have benefited the self-assessment of memory, quality of life, and mood among older adults during the coronavirus disease 2019 (COVID-19) pandemic. Objective To determine the subjective impacts of computerized cognitive training on mood, frequency of forgetfulness, memory complaints, and quality of life in the elderly using an online platform. Methods In total, 66 elderly participants of USP 60 +, a program for the elderly offered by Universidade de São Paulo, who voluntarily enrolled in the study were selected and randomized with an allocation ratio of 1:1 into 2 groups: the training group (n = 33) and the control group (n = 33). After signing the free and informed consent form, they answered a protocol which included a sociodemographic questionnaire, the Memory Complaints Questionnaire (MAC-Q), the McNair and Kahn’s Frequency of Forgetfulness Scale, the Geriatric Depression Scale (GDS-15), the Geriatric Anxiety Inventory (GAI), and the Control, Autonomy, Self-Realization, and Pleasure (CASP-19) questionnaire. The training cognitive game platform aimed to stimulate various cognitive aspects, including memory, attention, language, executive functions (reasoning, logical thinking), and visual and spatial skills. Results The participants of the training group showed a reduction in the MAC-Q, MacNair and Kahn, and GAI scores in the pre- and posttest comparison. Significant differences were identified between the groups regarding the total scores of the MAC-Q in the post-test, which was also evidenced by the logistic regression. Conclusion Participation in a computerized cognitive intervention promoted reductions in memory complaints, frequency of forgetfulness, and anxiety symptoms, in addition to improving self-reported quality of life.

Resumo Antecedentes Programas informatizados de treinamento cognitivo podem ter beneficiado a autoavaliação da memória, a qualidade de vida e o humor entre os idosos durante a pandemia de doença do coronavírus 2019 (coronavirus disease 2019, COVID-19, em inglês). Objetivo Determinar os impactos subjetivos do treinamento cognitivo computadorizado no humor, na frequência de esquecimento, nas queixas de memória, e na qualidade de vida em idosos utilizando uma plataforma online. Métodos Ao todo, 66 idosos participantes do programa USP 60 +, oferecido à terceira idade pela Universidade de São Paulo, e inscritos voluntariamente no estudo, foram selecionados e randomizados em uma razão de 1:1 em 2 grupos: grupo treinamento (n = 33) e grupo controle (n = 33). Após assinarem o termo de consentimento livre e esclarecido, os participantes responderam a um protocolo que incluía um questionário sociodemográfico, o Questionário de Queixas de Memória (Memory Complaints Questionnaire, MAC-Q), A Escala de Frequência de Esquecimento de McNair e Kahn, a Escala de Depressão Geriátrica (Geriatric Depression Scale, GDS-15), o Inventário de Ansiedade Geriátrica (Geriatric Anxiety Inventory, GAI), e –o questionário de Controle, Autonomia, Autorrealização e Prazer (Control, Autonomy, Self-Realization, and Pleasure, CASP-19). A plataforma de jogos cognitivos de treinamento visou estimular diversos aspectos cognitivos, incluindo memória, atenção, linguagem, funções executivas (raciocínio, raciocínio lógico) e habilidades visuais e espaciais. Resultados Na comparação pré e pós-teste, os participantes do grupo de treinamento apresentaram redução nas pontuações do MAC-Q, da escala McNair e Kahn e do GAI. Diferenças significativas entre os grupos quanto às pontuações totais da escala MAC-Q no pós-teste também foram evidenciadas pela regressão logística. Conclusão A participação em uma intervenção cognitiva computadorizada promoveu reduções nas queixas de memória, frequência de esquecimento e sintomas de ansiedade, além de melhorar a qualidade de vida autorrelatada.

RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. Este estudo pode contribuir para embasar políticas públicas que reforcem a importância da suspensão das aulas presenciais e da necessidade de estratégias de medidas de proteção e acompanhamento do status sorológico daqueles que ainda não foram contemplados no calendário vacinal.

ABSTRACT OBJECTIVE To estimate seroprevalence of SARS-CoV-2 antibodies in schoolchildren aged 4 to 14 years living in the city of São Paulo, according to clinical, demographic, epidemiological, and social variables, during the school closure period as a measure against covid-19 spread. METHODS A serological survey was made in September 2020 with a random sample stratified by school system (municipal public, state public and private) type. A venous blood sample was collected using the Wondfo SARS-CoV-2 Antibody Test (lateral flow method) for detection of total SARS-CoV-2 virus antibodies. Semi-structured questionnaires were applied to collect clinical, demographic, social, and epidemiological data. RESULTS Seroprevalence of SARS-CoV-2 antibodies in schoolchildren was of 16.6% (95%CI 15.4–17.8). The study found higher seroprevalence in the municipal (18.5%; 95%CI 16.6–20.6) and state (16.2%; 95%CI 14.4–18.2) public school systems compared to the private school system (11.7; 95%CI 10.0–13.7), among black and brown students (18.4%; 95%CI 16.8–20.2) and in the most vulnerable social stratum (18.5 %;95%CI 16.9–20.2). Lower seroprevalence was identified in schoolchildren who reported following the recommended protective measures against covid-19. CONCLUSION Seroprevalence of SARS-CoV-2 antibodies is found mainly in the most socially vulnerable schoolchildren. This study can contribute to support public policies that reinforce the importance of suspending face-to-face classes and developing strategies aimed at protective measures and monitoring of the serological status of those who have not yet been included in the vaccination schedule.

Resumo O tétano é uma doença grave e rara em pequenos animais, de caráter agudo e mediada pela ação da neurotoxina tetanospasmina, proveniente do bacilo Clostridium tetani. Neste relato descreve-se um caso de tétano em um canino, com ênfase em seus aspectos clínicos e terapêuticos, além da sua recuperação clínica. Foi atendido no Hospital Veterinário da Universidade de Passo Fundo, um canino, fêmea, da raça Pitbull, com cinco meses, em status epilepticus. Após a estabilização da paciente, constataram-se animal em posição de cavalete, com paralisia espástica generalizada, risus sardonicus, trismo, cauda e orelhas eretas e dificuldade em expandir o tórax. O diagnóstico presuntivo de tétano foi firmado a partir da anamnese, sinais clínicos e achados laboratoriais. O tratamento suporte estabelecido incluiu reposição hidroeletrolítica, soro antitetânico, antibioticoterapia, analgesia, fármacos miorrelaxantes e anticonvulsivantes. O manejo intensivo da paciente foi realizado com sondagem uretral e nasoesofágica, trocas de decúbito a cada duas horas e diminuição de estímulos ambientais. A fisioterapia e a acupuntura também foram utilizadas para complementar o tratamento e acelerar a recuperação. A paciente recebeu alta médica um mês após o início do tratamento, apresentando evolução satisfatória. No presente caso, o exame clínico em associação aos sinais clínicos característicos da doença, acrescidos da anamnese minuciosa foram fundamentais ao diagnóstico presuntivo de tétano. Além disso, o manejo intensivo, o tratamento medicamentoso, assim como a realização de fisioterapia e acupuntura, possibilitaram a evolução do quadro clínico à cura.

Abstract Tetanus is a serious and rare disease in small animals, of an acute nature and mediated by the action of the neurotoxin tetanospasmin, from the bacillus Clostridium tetani. This report describes a case of tetanus in a canine, with emphasis on its clinical and therapeutic aspects, in addition to its clinical recovery. A canine, female, Pitbull breed, five months old, in status epilepticus, was attended at the Veterinary Hospital of the University of Passo Fundo. After stabilizing the patient, the animal was found in a trestle position, with generalized spastic paralysis, risus sardonicus, trismus, erect tail and ears, and difficulty in expanding the thorax. The presumptive diagnosis of tetanus was established based on the anamnesis, clinical signs, and laboratory findings. The established supportive treatment included hydro electrolytic replacement, antitetanus serum, antibiotic therapy, analgesia, myorelaxant and anticonvulsant drugs. The patient's intensive management was carried out with urethral and gastroesophageal probing, changes of position every two hours, and reduction of environmental stimuli. Physiotherapy and acupuncture were also used to complement the treatment and accelerate recovery. The patient was discharged one month after the beginning of the treatment, presenting satisfactory evolution. In the present case, the clinical examination in association with the characteristic clinical signs of the disease, added to the detailed anamnesis, was essential for the presumptive diagnosis of tetanus. In addition, intensive management, drug treatment, physiotherapy, and acupuncture enabled the evolution of the clinical condition to cure.

ABSTRACT. Dementias secondary to cardiovascular diseases are the second cause of neurogenerative diseases. These conditions can be prevented by controlling risk factors, and it is necessary to observe the relationship between chronic diseases. Objective: to know the influence of chronic non-communicable diseases on cognition and depressive symptoms in the elderly, amid the COVID-19 pandemic. Methods: 578 older adults were evaluated using a sociodemographic questionnaire, the Brazilian Telephone version of the Mini Mental State Examination (Braztel-MMSE), the Geriatric Depression Scale (GDS-15) and an open questionnaire related to NCDs. Results: the association of Non-Communicable Diseases (NCD) with age, depressive symptoms and schooling was confirmed. Conclusion: no association with cognitive decline was evident due to the relationship of high schooling of participants and control of NCDs.

RESUMO. As demências secundárias às doenças cardiovasculares são a segunda causa de doenças neurodegenerativas. Essas condições podem ser prevenidas pelo controle de fatores de risco, sendo necessário observar a relação entre doenças crônicas. Objetivo: Conhecer a influência das doenças crônicas não transmissíveis (DCNT) na cognição e nos sintomas depressivos em pessoas idosas, em meio à pandemia da COVID-19. Métodos: Foram avaliados 578 idosos utilizando um questionário sociodemográfico, o Brazilian telephone version of the Mini Mental State Examination (Braztel-MMSE), a Escala de Depressão Geriátrica (GDS-15) e um questionário aberto relacionado às DCNT. Resultados: Foi confirmada a associação das DCNT crônicas com idade, sintomas depressivos e escolaridade. Conclusão: Nenhuma associação com declínio cognitivo foi evidente em razão da relação da alta escolaridade dos participantes com o controle das DCNT.

Abstract In this article we aim to address the Malvinas issue in its dimensión of school content for Primary Education. With this objective, we analyzed the different curricular documents of the City of Buenos Aires, from the moment in which the Malvinas Islands were thegeographical space of the war episode until today. The analysis and historical journey begin with the Curriculum Design dictated and applied in 1981 by the last Civic- Military Dictatorship, the Curriculum Design that is in force, and attached Documents. Determining whatplace the Malvinas question occupies as a content of the Social Sciences, its pre-eminence as ephemeris, what changes have occurred in the didactic proposals during the last 40 years and what is happening today, after ideological de- Malvinization attempts and re-Malvinization attempts is an arduous task. It requires focusing on the times of war, as well as in the democratic years to elucidate how the written letter impacted Malvina's school history.

Resumen En este artículo pretendemos abordar la cuestión Malvinas en su dimensión de contenido escolar para la Educación Primaria. con este objeto analizamos los diferentes documentos curriculares de la Ciudad de Buenos Aires desde el momento en que las Islas Malvinas fueron el espacio geográfico del episodio bélico hasta la actualidad. Iniciamos el análisis y recorrido histórico con el Diseño Curricular dictado y aplicado en 1981 por la última Dictadura Cívico Militar, para luego continuar con el Diseño Curricular que se encuentran en vigencia, así como con sus Documentos anexos y la Ley Nacional de Educación de 2006. Determinar qué lugar ocupa la cuestión Malvinas como contenido de las Ciencias Sociales, su preeminencia como efemérides, cuáles fueron los cambios atravesados en las propuestas didácticas durante los últimos 40 años y qué sucede hoy, luego de intentos de desmalvinización ideológica e intentos de remalvinización, es una tarea ardua. Requiere del análisis de la normativa, haciendo foco en los tiempos de guerra, como en los años democráticos con el fin de elucidar cómo la letra escrita impactó en la historia escolar de Malvinas.

Abstract Efavirenz is one of the most commonly used drugs in HIV therapy. However the low water solubility tends to result in low bioavailability. Drug nanocrystals, should enhance the dissolution and consequently bioavailability. The aim of the present study was to obtain EFV nanocrystals prepared by an antisolvent technique and to further observe possible effect, on the resulting material, due to altering crystallization parameters. A solution containing EFV and a suitable solvent was added to an aqueous solution of particle stabilizers, under high shear agitation. Experimental conditions such as solvent/antisolvent ratio; drug load; solvent supersaturation; change of stabilizer; addition of milling step and solvents of different polarities were evaluated. Suspensions were characterized by particle size and zeta potential. After freeze- dried and the resulting powder was characterized by PXRD, infrared spectroscopy and SEM. Also dissolution profiles were obtained. Many alterations were not effective for enhancing EFV dissolution; some changes did not even produced nanosuspensions while other generated a different solid phase from the polymorph of raw material. Nevertheless reducing EFV load produced enhancement on dissolution profile. The most important modification was adding a milling step after precipitation. The resulting suspension was more uniform and the powder presented grater enhancement of dissolution efficacy.

RESUMO Esta pesquisa filia-se aos postulados do Círculo de Bakhtin no que se refere ao dialogismo da linguagem, em especial, à responsividade ativa atribuída a todo discurso que se faz em resposta a outros. Situada no âmbito da Análise Dialógica do Discurso, propomos uma interface com alguns elementos da Gramática Discursivo-Funcional para a análise linguística dos enunciados. Nosso objetivo é avaliar como a atitude responsiva se instaura no discurso do Governo do Estado de São Paulo em relação a declarações do governo federal, de modo a construir uma política institucional em tempos de pandemia da covid-19. Por meio da análise de dois vídeos, constatamos que a atitude responsiva que o Governo de São Paulo procura demonstrar não configura, necessariamente, atitudes de responsabilidades perante os menos favorecidos, entretanto, alcança seu objetivo, que é contrapor o discurso do governo federal para orientar as atitudes da população no cenário pandêmico.

ABSTRACT This research is in line with the postulates of the Bakhtin Circle regarding the dialogism of language, particularly, the active responsiveness assigned to any utterance in response to others. Within the scope of Dialogic Discourse Analysis, we propose an interface with some elements of the Functional Discourse Grammar for linguistic analysis of utterances. Our objective is to evaluate how the responsive attitude is established in the discourse of the Government of São Paulo in regard to statements by the Federal Government, in order to build an institutional policy in times of covid-19 pandemic. Through the analysis of two videos, we have found that the responsive attitude which the Government of the State São Paulo seeks to demonstrate does not necessarily configure attitudes of responsibility towards the less fortunate. However, it reaches its objective, which is to oppose to the Federal Government’s discourse guiding the attitudes of the population in the pandemic scenario.

ABSTRACT Objective: Permanent access in the form of a fistula is the preferred form of vascular access for most pediatric patients on maintenance hemodialysis (HD) therapy; however, the technical aspects of the procedure that are unique to the pediatric population, the expectation of a short waiting time for kidney transplantation and the need to cannulate every other day (with the pain and fear associated with it) limit its use. Our objective was to analyze the long-term outcomes of pediatric arteriovenous fistulas in our institution. Methods: A retrospective review was performed of all arteriovenous fistula (AVF) created in a HD population aged 0 to 18 years at a single institution from 2007 to 2019. Data abstracted included age, weight, etiology of renal failure, time on dialysis, central venous catheter history and transplantation history. Data were analyzed to determine the primary and secondary patency. Results: During the study period, 19 AVFs were performed in 16 patients, of whom 9 patients (56.3%) were male. Mean patient age was 12.3 years (range 5-17 years), and mean weight was 38.3kg (range 12-83kg). At the time of AVF creation, 9 patients were on dialysis and 7 patients had a central venous catheter (CVC), with a median length of CVC dependence of 10 months. Procedures performed included 4 radiocephalic fistulas, 11 brachiocephalic fistulas and 4 brachiobasilic. Five accesses failed to mature (26.3%). Mean follow-up was 6 years. The 2-year primary and secondary patency rates were 92.3% and 100%, respectively. The 4-year primary and secondary patency rates were 76.9% and 100%, respectively. No thrombosis was documented during follow-up. During the postoperative period, 10 patients (62.5%) received a kidney transplant, in a mean time of 23 months. Conclusions: AVFs demonstrate excellent long-term patency in pediatric HD patients. No significant complications were reported and no thrombosis occurred.

Chromone derivatives exhibiting high absorbance values in the UVA/UVB region were synthesized, and their photoprotective properties were evaluated. Chromones were prepared according to known literature procedures and characterized by high resolution mass spectrometry, infrared (IR) and nuclear magnetic resonance (NMR) spectroscopy. The in vitro solar protection factor (SPF) was determined by the Mansur method and cytotoxicity was evaluated using the sulforhodamine B assay. Two of the chromones synthesized demonstrated suitable SPF values and displayed no cytotoxic effect towards MRC-5 human fibroblasts at the tested concentrations, indicating great potential for future in vivo assays and clinical trials. Finally, the lead compound was incorporated into a nanoemulsion. Nanoemulsions showed high droplet size homogeneity and excellent stability. Chromones bearing methoxy substituents were found to be the most promising compounds with ideal photoprotective properties desirable for utilization and incorporation in sunscreen formulations.

Resumo Este estudo avaliou a resistência à fratura de dentes imaturos simulados reforçados com cimento de aluminato de cálcio (CAC) ou trióxido agregado mineral (MTA) contendo nanopartículas de carbonato de cálcio (nano-CaCO3). O arranjo microestrutural dos cimentos e sua constituição química também foram avaliados. Quarenta e oito caninos simulando dentes imaturos foram distribuídos em 6 grupos (n=8): Controle negativo - sem plug apical ou obturação do canal radicular; CAC - plug apical com CAC; CAC/nano-CaCO3 - plug apical com CAC + 5% nano-CaCO3; MTA - plug apical com MTA; MTA/nano-CaCO3 - plug apical com MTA + 5% nano-CaCO3; e Controle positivo - obturação dos canais radiculares com MTA. A resistência à fratura foi avaliada em máquina universal de ensaios. Amostras dos cimentos foram analisadas em Microscópio Eletrônico de Varredura (MEV) para determinar seu arranjo microestrutural. A análise química dos cimentos foi realizada por Espectroscopia de Energia Dispersiva de Raio-X (EDS). A resistência à fratura de CAC/nano-CaCO3 foi significativamente maior do que o controle negativo (p<0,05). Não houve diferença significativa entre os outros grupos (p>0,05). Ambos os cimentos apresentaram microestrutura mais regular com a adição de nano-CaCO3. As amostras de MTA apresentaram mais cálcio disponível em formas solúveis do que CAC. A adição de nano-CaCO3 ao CAC aumentou a resistência à fratura dos dentes em comparação aos dentes não reforçados. A microestrutura de ambos os cimentos contendo nano-CaCO3 foi semelhante, com uma distribuição mais homogênea de cristais de formato lamelar e prismático. MTA apresentou mais cálcio disponível nas formas solúveis do que CAC.

Abstract This study assessed the fracture resistance of simulated immature teeth reinforced with calcium aluminate cement (CAC) or mineral trioxide aggregate (MTA) containing calcium carbonate nanoparticles (nano-CaCO3). The microstructural arrangement of the cements and their chemical constitution were also evaluated. Forty-eight canines simulating immature teeth were distributed into 6 groups (n=8): Negative control - no apical plug or root canal filling; CAC - apical plug with CAC; CAC/nano-CaCO3 - apical plug with CAC+5% nano-CaCO3; MTA - apical plug with MTA; MTA/nano-CaCO3 - apical plug with MTA+5% nano-CaCO3; and Positive control - root canal filling with MTA. The fracture resistance was evaluated in a universal testing machine. Samples of the cements were analyzed under Scanning Electron Microscope (SEM) to determine their microstructural arrangement. Chemical analysis of the cements was performed by Energy Dispersive X-ray Spectroscopy (EDS). The fracture resistance of CAC/nano-CaCO3 was significantly higher than the negative control (p<0.05). There was no significant difference among the other groups (p>0.05). Both cements had a more regular microstructure with the addition of nano-CaCO3. MTA samples had more calcium available in soluble forms than CAC. The addition of nano-CaCO3 to CAC increased the fracture resistance of teeth in comparison with the non-reinforced teeth. The microstructure of both cements containing nano-CaCO3 was similar, with a more homogeneous distribution of lamellar- and prismatic-shaped crystals. MTA had more calcium available in soluble forms than CAC.

RESUMO As práticas discursivas na contemporaneidade constituem-se de relações dialógicas cada vez mais inter/multi/transculturais e multissemióticas devido aos processos de globalização e, sobretudo, pelos avanços das Tecnologias Digitais da Informação e Comunicação (TDIC). Desse modo, emergem outros (multi)letramentos. No entanto, há ainda diversas lacunas que inviabilizam espaços de (multi)letramentos na educação formal do Brasil, entre elas a formação de professores. Nesse sentido, o objetivo desta pesquisa é avaliar em que medida o Projeto Político Curricular (PPC) do curso de Letras Inglês da Universidade Federal do Acre (UFAC), campus Floresta, possibilita (ou não) aos discentes o desenvolvimento da curadoria de sentidos em (multi)letramentos digitais, de maneira crítica, ética e estética para a prática docente. Para isso, sob a ótica da Linguística Aplicada in/transdisciplinar (MOITA LOPES, 2006; ROJO, 2006), avaliamos sete excertos do PPC em foco, relacionados às TDIC. Por meio das análises, de cunho qualitativo, constatamos que a curadoria de sentidos em (multi)letramentos digitais está presente na política de formação de professores do referido curso de Letras, entretanto; as ementas das disciplinas relacionadas aos gêneros discursivos do campo acadêmico e do campo artístico-literário são incipientes às demandas das práticas discursivas emergentes.

ABSTRACT Contemporary discursive practices are constituted by increasingly inter/multi/transcultural and multisemiotic dialogic relations due to the processes of globalization and, above all, by the advances in digital Information and Communication Technologies. In this context, other (multi) literacies emerge. On the other hand, there are still several gaps that preclude spaces for (multi)literacies in formal education in Brazil, including teacher education. In this sense, the aim of this research is to evaluate to what extent the “Curriculum” [Projeto Político Curricular - PPC] of the undergraduate course in English Language and Literature Teaching [Letras Inglês] at the Federal University of Acre [Universidade Federal do Acre - UFAC], campus Floresta, allows (or not) the development of sense “curation” in digital multiliteracies, in the critical, ethical, and aesthetic ways of English Language Teaching (ELT) practice. In order to do so, from the perspective of in/transdisciplinary Applied Linguistics (MOITA LOPES, 2006; ROJO, 2006), we evaluated seven excerpts of the PPC in focus, related to Digital Technologies. Through qualitative analysis, we found that the sense “curation” in digital multiliteracies is present in the teacher education policies of the mentioned course; however, summaries of disciplines related to speech genres from academic and artistic-literary fields are incipient to the demands of emerging discursive practices.

Resumo Introdução: A isquemia mesentérica crónica como resultado de uma disfunção do fluxo sanguíneo nos órgãos viscerais é caracterizada por um curso clínico insidioso e, portanto, é frequentemente uma doença subestimada e subtratada. A história natural é progressiva, causada pelo desenvolvimento da aterosclerose numa população envelhecida e com múltiplas comorbilidades. Devido à extensa colateralidade, as lesões estenóticas difusas podem permanecer assintomáticas por muito tempo e manifestarem-se apenas quando ocorre um evento clínico ou cirúrgico agudo. Nestes casos, a suspeita clínica é fundamental para se chegar a um diagnóstico atempado de forma a preservar a qualidade de vida do doente e, principalmente, assegurar a sua sobrevida. Caso-clínico 1: Mulher de 48 anos de idade com antecedentes de tabagismo recorre ao serviço de urgência por dor abdominal hipogástrica, náuseas e vómitos. Foi realizado estudo por angio-tomografia computorizada (ATC) que demonstrou oclusão da artéria mesentérica superior, e estenose significativa do tronco celíaco e espessamento de ansas do intestino delgado sugestivo de sofrimento isquémico. Foi realizado uma trombectomia da artéria mesentérica superior (AMS) e stenting retrógrado do seu ostio seguido posteriormente de enterectomia extensa na laparotomia das 24 horas. Após alta hospitalar, a doente apresentou novo quadro abdominal tendo sido documentada a oclusão do stent na AMS. Foi submetida a um bypass ílio-hepático com bom resultado. Caso-clínico 2: Homem de 76 anos de idade previamente submetido a um bypass sequencial axilo-femoral e femoropopliteo direitos para tratamento de isquemia crónica com ameaça de membro. O doente recorreu ao serviço de urgência com dor abdominal, vómitos e obstipação. Foi realizado o diagnóstico de um quadro suboclusivo tendo realizado estudo por tomografia computorizada (TC) onde se observou aderência de ansas do íleon ao cego que se encontrava espessado; o tronco celíaco e AMS apresentavam estenoses suboclusivas. O doente foi submetido a um stenting primário do tronco celíaco com stent expansível em balão. Após a revascularização celíaca foi realizada uma colectomia direita com duas enterectomias segmentares. Conclusão: A isquemia mesentérica é uma patologia grave muitas vezes subdiagnosticada por falta de atenção para a doença por parte da generalidade dos médicos bem como pelos sintomas inespecíficos. A intervenção vascular visa impedir a necrose intestinal que pode mesmo levar à morte do doente. O diagnóstico atempado é assim fundamental e a revascularização deve ser realizada antes ou no mesmo tempo da cirurgia intestinal. A estreita colaboração entre as equipas de cirurgia geral e vascular é de essencial importância para o sucesso destes casos.

Abstract Introduction: Chronic Mesenteric Ischemia (CMI) resulting from an impaired blood flow of the splanchnic organs is characterized by an insidious clinical course and is often an underestimated and undertreated disease. The natural history is progressive, caused by atherosclerosis progression in a polymorbidity and aging society. Due to collateralization, diffuse stenotic lesions can remain asymptomatic for a long time and usually manifests when an acute medical or surgical event occurs. In those cases, the clinical suspicion is crucial to reach the diagnosis promptly, allowing to preserve the patients’ quality of life and, above all, the patients’ life. Clinical case 1: A 48-year-old woman with a history of smoking goes to the emergency department for hypogastric abdominal pain, nausea, and vomiting. A study was performed by computed tomography angiography (CTA) that demonstrated occlusion of the superior mesenteric artery (SMA) and significant stenosis of the celiac trunk associated with thickening of small bowel suggestive of ischemia. A thrombectomy of the superior mesenteric artery and retrograde stenting of the artery ostium was performed, followed by extensive enterectomy on 24-hour laparotomy. After hospital discharge, the patient had new abdominal complaints, and stent occlusion was documented in the SMA. She underwent an iliohepatic bypass with good results. Clinical case 2: A 76-year-old man previously submitted to a right axillofemoral and femoropopliteal sequential bypass to treat chronic limb-threatening ischemia (CLTI), went to the emergency department with complaints of abdominal pain, vomiting, and constipation. The diagnosis of a subocclusive intestinal syndrome was made, having performed a computed tomography (CT) scan where it was observed the Ileum adhered to the cecum, which was thickened; the celiac trunk and SMA had subocclusive stenosis. The patient underwent primary celiac trunk stenting with a balloon-expandable stent. After celiac artery revascularization, a right colectomy was performed with two segmental enterectomies. Conclusion: Mesenteric ischemia is a severe condition that is often underdiagnosed due to the lack of awareness of most physicians and nonspecific symptoms. The vascular intervention aims to prevent intestinal necrosis, which can lead to the patient’s death. Timely diagnosis is therefore essential, and revascularization must be performed before or at the same time as intestinal surgery. The close collaboration between the general and vascular surgery teams is essential to the success of these cases.

ABSTRACT OBJECTIVE To estimate the evolution of the prevalence of SARS-CoV-2 virus infection among residents aged 18 years or over in the municipality of São Paulo. METHODS This is a population-based household survey conducted every 15 days, between June and September 2020, and January and February 2021. In total, the study comprised 11 phases. The presence of antibodies against SARS-CoV-2 was identified in venous blood using a lateral flow test, Wondfo Biotech. In the last phase, the researchers combined it with an immunoenzymatic test, Euroimmun. The participants also answered a semi-structured questionnaire on sociodemographic and economic factors, and on social distancing measures. Prevalence estimates and the 95% confidence interval were estimated according to regions, Human Development Index, sex, age group, ethnicity, education, income, and variables associated with risk or prevention of infection. To compare the frequencies among the categories of each variable, the chi-square test with Rao-Scott correction was used, considering a significance level of 5%. RESULTS In total, 23,397 individuals were interviewed and had their samples collected. The estimated prevalence of antibodies against SARS-CoV-2 ranged from 9.7% (95%CI: 7.9–11.8%) to 25.0% (95%CI: 21.7–28.7). The prevalence of individuals with antibodies against the virus was higher among black and brown people, people with lower schooling and income, and among residents of regions with lower Human Development Index. The lowest prevalences were associated with recommended measures of disease protection. The proportion of asymptomatic infection was 45.1%. CONCLUSION The estimated prevalence of the infection was lower than the cumulative incidence variation, except for the last phase of the study. The differences in prevalence estimates observed among subpopulations showed social inequality as a risk of infection. The lower prevalence observed among those who could follow prevention measures reinforce the need to maintain social distancing measures as a way to prevent SARS-CoV-2 infection.
