Is Age Associated with Complications of Atrial Fibrillation Catheter Ablation?

Abstract Background Atrial fibrillation (AF) is the most frequent arrhythmia, and its prevalence increases with age. The management of AF in the elderly is challenging, as it is normally associated with comorbidities and frailty. AF catheter ablation (CA) is a safe and superior alternative to antiarrhythmic drugs (AADs) for the maintenance of sinus rhythm. Objectives To evaluate the rate of complications associated with CA for AF across different age groups. Methods A retrospective analysis of 219 patients who underwent CA for AF between 2016 and 2020 were divided into 3 age groups: less than 60 years, 60 to 70 years, and > 70 years. All the included patients underwent radiofrequency ablation using an electroanatomic mapping system. Categorical variables were evaluated with chi-square and Fisher’s test, and continuous variables were evaluated by Kruskal-Wallis and post-hoc Tamhane’s T2. P values less than 0.05 were considered significant. Results We found an overall total complication rate of 4.6%. The total complication rate was 3.3% in patients < 60 years of age, 5.7% in patients between 60 and 70 years, and 5.2% in patients > 70 years (p = 0.742). No deaths occurred. Conclusion There was no significant difference in the AF CA-related complications when comparing the patients by age group.

ABSTRACT Cystic fibrosis (CF) is a genetic disease that results in dysfunction of the CF transmembrane conductance regulator (CFTR) protein, which is a chloride and bicarbonate channel expressed in the apical portion of epithelial cells of various organs. Dysfunction of that protein results in diverse clinical manifestations, primarily involving the respiratory and gastrointestinal systems, impairing quality of life and reducing life expectancy. Although CF is still an incurable pathology, the therapeutic and prognostic perspectives are now totally different and much more favorable. The purpose of these guidelines is to define evidence-based recommendations regarding the use of pharmacological agents in the treatment of the pulmonary symptoms of CF in Brazil. Questions in the Patients of interest, Intervention to be studied, Comparison of interventions, and Outcome of interest (PICO) format were employed to address aspects related to the use of modulators of this protein (ivacaftor, lumacaftor+ivacaftor, and tezacaftor+ivacaftor), use of dornase alfa, eradication therapy and chronic suppression of Pseudomonas aeruginosa, and eradication of methicillin-resistant Staphylococcus aureus and Burkholderia cepacia complex. To formulate the PICO questions, a group of Brazilian specialists was assembled and a systematic review was carried out on the themes, with meta-analysis when applicable. The results obtained were analyzed in terms of the strength of the evidence compiled, the recommendations being devised by employing the GRADE approach. We believe that these guidelines represent a major advance to be incorporated into the approach to patients with CF, mainly aiming to favor the management of the disease, and could become an auxiliary tool in the definition of public policies related to CF.
RESUMO A fibrose cística (FC) é uma doença genética que resulta em disfunção da proteína reguladora de condutância transmembrana da FC (CFTR), que é um canal de cloro e bicarbonato expresso na porção apical de células epiteliais de diversos órgãos. A disfunção dessa proteína resulta em manifestações clínicas diversas, envolvendo primariamente os sistemas respiratório e gastrointestinal com redução da qualidade e expectativa de vida. A FC ainda é uma patologia incurável, porém o horizonte terapêutico e prognóstico é hoje totalmente distinto e muito mais favorável. O objetivo destas diretrizes foi definir recomendações brasileiras baseadas em evidências em relação ao emprego de agentes farmacológicos no tratamento pulmonar da FC. As perguntas PICO (acrônimo baseado em perguntas referentes aos Pacientes de interesse, Intervenção a ser estudada, Comparação da intervenção e Outcome [desfecho] de interesse) abordaram aspectos relativos ao uso de moduladores de CFTR (ivacaftor, lumacaftor + ivacaftor e tezacaftor + ivacaftor), uso de dornase alfa, terapia de erradicação e supressão crônica de Pseudomonas aeruginosa, e erradicação de Staphylococcus aureus resistente a meticilina e do complexo Burkholderia cepacia. Para a formulação das perguntas, um grupo de especialistas brasileiros foi reunido e realizou-se uma revisão sistemática sobre os temas, com meta-análise quando aplicável. Os resultados encontrados foram analisados quanto à força das evidências compiladas, sendo concebidas recomendações seguindo a metodologia GRADE. Os autores acreditam que o presente documento represente um importante avanço a ser incorporado na abordagem de pacientes com FC, objetivando principalmente favorecer seu manejo, podendo se tornar uma ferramenta auxiliar na definição de políticas públicas relacionadas à FC.

Abstract Simulating a bacterial-induced pulpitis environment in vitro may contribute to exploring mechanisms and bioactive molecules to counteract these adverse effects. Objective To investigate the chronic exposure of human dental pulp cells (HDPCs) to lipopolysaccharides (LPS) aiming to establish a cell culture protocol to simulate the impaired odontogenic potential under pulpitis conditions. Methodology HDPCs were isolated from four healthy molars of different donors and seeded in culture plates in a growth medium. After 24 h, the medium was changed to an odontogenic differentiation medium (DM) supplemented or not with E. coli LPS (0 - control, 0.1, 1, or 10 µg/mL) (n=8). The medium was renewed every two days for up to seven days, then replaced with LPS-free DM for up to 21 days. The activation of NF-κB and F-actin expression were assessed (immunofluorescence) after one and seven days. On day 7, cells were evaluated for both the gene expression (RT-qPCR) of odontogenic markers (COL1A1, ALPL, DSPP, and DMP1) and cytokines (TNF, IL1B, IL8, and IL6) and the production of reactive nitrogen (Griess) and oxygen species (Carboxy-H2DCFDA). Cell viability (alamarBlue) was evaluated weekly, and mineralization was assessed (Alizarin Red) at 14 and 21 days. Data were analyzed with ANOVA and post-hoc tests (α=5%). Results After one and seven days of exposure to LPS, NF-κB was activated in a dose-dependent fashion. LPS at 1 and 10 µg/mL concentrations down-regulated the gene expression of odontogenic markers and up-regulated cytokines. LPS at 10 µg/mL increased both the production of reactive nitrogen and oxygen species. LPS decreased cell viability seven days after the end of exposure. LPS at 1 and 10 µg/mL decreased hDPCs mineralization in a dose-dependent fashion. Conclusion The exposure to 10 µg/mL LPS for seven days creates an inflammatory environment that is able to impair by more than half the odontogenic potential of HDPCs in vitro, simulating a pulpitis-like condition.

Abstract An arteriovenous fistula (AVF) is an uncommon sequela of spontaneous arterial aneurysm rupture into the adjacent venous system. We describe the case of a 74-year-old patient who underwent endovascular treatment of a right iliac AVF caused by a ruptured common iliac artery (CIA) aneurysm and a distal left CIA aneurysm. Surgery preserved the lumbar and inferior mesenteric arteries because of the need to simultaneously exclude the hypogastric arteries. Dynamic fluid balance phenomena provoked by closure of the AVF are described. The patient had a benign postoperative course with normalization of the severe hemodynamic changes presented prior to the intervention and resolution of respiratory symptoms attributed to pulmonary arterial hypertension.
Resumo A fístula arteriovenosa (FAV) é uma sequela incomum de rotura espontânea de aneurisma arterial dentro do sistema venoso adjacente. Descrevemos, em um paciente de 74 anos, o tratamento endovascular de FAV ilíaca à direita por aneurisma de artéria ilíaca comum (AIC) roto associado a aneurisma de AIC distal à esquerda, em cuja cirurgia foram preservadas as artérias lombares e mesentérica inferior por necessidade de excluir simultaneamente as artérias hipogástricas. Foram demonstrados os fenômenos na dinâmica do balanço hídrico ocorridos em decorrência da interrupção da FAV. A evolução do paciente foi benigna, com normalização das graves alterações hemodinâmicas que apresentava e com desaparecimento dos sintomas respiratórios atribuídos à hipertensão arterial pulmonar.

Abstract Background: The association of scaffolds to repair extensive bone defects can contribute to their evolution and morphophysiological recomposition. The incorporation of particulate biomaterials into three-dimensional fibrin bioproducts together with photobiomodulation therapy (PBM) has potential and can improve regenerative medicine procedures. The objective of this experiment was to evaluate the effects of PBM therapy on critical size defects filled with xenogenic bone substitute associated with fibrin biopolymer. Methods: A critical defect of 8 mm was performed in 36 Wistar male adult rats that were divided into four groups. Groups BC and BC-PBM were defined as controls with defects filled by a clot (without or with PBM, respectively) and groups XS and XS-PBM that comprised those filled with biocomplex Bio-OssTM in association with fibrin biopolymer. PBM was applied immediately after the surgery and three times a week every other day, with the parameters: wavelength of 830 nm, energy density 6.2 J/cm2, output power 30 mW, beam area of 0.116 cm2, irradiance 0.258,62 W/cm2, energy/point 0.72 J, total energy 2.88 J. Fourteen and 42 days after the surgery, animals were euthanatized and subjected to microtomography, qualitative and quantitative histological analysis. Results: The BC-PBM and XS-PBM groups had a similar evolution in the tissue repair process, with a higher density of the volume of new formed bone in relation to the groups without PBM (p = 0.04086; p = 0.07093, respectively). Intense vascular proliferation and bone deposition around the biomaterial particles were observed in the animals of the groups in which biocomplex was applied (XS and XS-PBM). Conclusion: PBM therapy allowed an improvement in the formation of new bone, with a more organized deposition of collagen fibers in the defect area. Biocomplex favored the insertion and permanence of the particulate material in bone defects, creating a favorable microenvironment for accelerate repair process.

ABSTRACT Temperature is one of the most dominant environmental factor influencing the biology and performance of aquatic ectotherms in the wild and in culture conditions. Thus, the aims of the present study were to investigate thermoregulatory behavior, thermal tolerance and active metabolic scope in Macrobrachium tenellum. To fulfill our goal, we measure, the preferred temperature, critical threshold limits, thermal window width, oxygen consumption routine rate and active metabolic scope in the prawn M. tenellum acclimated to 20, 23, 26, 29 and 32°C. The preferred temperature obtained by the graphic acute method was 28.5°C. Acclimation temperature significantly affected the thermal tolerance which increased with the acclimation temperature. The scope for thermal tolerance had an interval of 25.3 to 27.7°C. The thermal window calculated for M. tenellum was 325°C2. The acclimation response ratios had an interval of 0.42-0.50. These values allow us to characterize these species as inhabitants of subtropical and tropical regions. The oxygen consumption routine rates increased as the acclimation temperature increased from 20 to 32°C. The range of temperature coefficient (Q10) between 29-32°C was the lowest, at 1.98. The active metabolic scope for prawns was the lowest for organisms acclimated to 20 and 32°C and the highest value was obtained at 29°C. The results obtained in the present study are important to determine the optimum conditions in which M. tenellum needs to live in the natural environment and may partially explain their wide distribution pattern along the Mexican Pacific Ocean littoral.

Abstract Four bisabolanes 1–4, including perezone (1) and triacetyl perezone (2), were isolated through a bioassay-guided fractionation of the extract obtained from the Caribbean gorgonian coral Pseudopterogorgia rigida collected during an expedition cruise to the Bahamas. All isolated compounds showed to be cytotoxic toward panel of four human tumor cell lines, as quantified by the MTT assay after 72 h incubation. Perezone (1), the most active one, was further analyzed, showing to be cytotoxic, but not selective, in a 12-cell line panel comprising tumor and non-tumor, as well as human and murine cells. Additionally, 1 was assayed for cytotoxicity against HL-60 leukemic cells. Pre-treatment with an acute free radical scavenger (L-NAC) before exposure of cells to perezone virtually eliminated the generation of intracellular ROS and lessened its severe cytotoxicity. The protective effect delivered by L-NAC evidences that the mechanism of perezone-induced cytotoxicity is partially associated to production of ROS and a consequent induction of oxidative stress.

Resumo Até 2010, o conhecimento sobre a diversidade de fungos do Brasil estava registrado em publicações esparsas de taxonomia e ecologia e em algumas poucas listas de espécies. Com a publicação do Catálogo de Plantas e Fungos do Brasil, e a disponibilização da lista online, tem sido possível agregar o conhecimento disperso. A versão ora apresentada acrescenta 2.111 nomes de espécies aos 3.608 listados em 2010. São citadas 5.719 espécies de fungos distribuídas em 1.246 gêneros, 102 ordens e 13 divisões, consistindo em considerável aumento em relação a 2010, quando estavam registrados 924 gêneros e 78 ordens. Predominam os Basidiomycota (2.741 espécies, em 22 ordens) e Ascomycota (1.881 espécies, em 41 ordens). A Mata Atlântica possui a maior quantidade de registros, com 3.017 espécies, seguido pela Amazonia (1.050), Caatinga (999), Cerrado (638) e Pampa e Pantanal com 84 e 35 espécies, respectivamente. A região Nordeste tem a maior riqueza (2.617 especies), seguida pelo Sudeste (2.252), Sul (1.995), Norte (1.301) e Centro Oeste (488 espécies). Em relação aos Estados da Federação, São Paulo (1.846 espécies), Pernambuco (1.611) e Rio Grande do Sul (1.377) são os mais diversos.
Abstract Knowledge about the Brazilian fungal diversity was, until 2010, recorded in few taxonomy and ecology publications, as well as in a handful of species lists. With the publication of the Catálogo de Plantas e Fungos do Brasil and the continued availability of an online list, it has been possible to aggregate this dispersed knowledge. The version presented here adds 2,111 species names to the 3,608 listed in 2010. A total of 5,719 species of fungi distributed in 1,246 genera, 102 orders and 13 phyla represents a considerable increase over the last five years, when only 924 genera and 78 orders were registered. Basidiomycota (2,741 species in 22 orders) and Ascomycota (1,881 species in 41 orders) predominate over other groups. The Atlantic Rainforest has the largest number of records, with 3,017 species, followed by Amazon Rainforest (1,050), Caatinga (999), Cerrado (638) and Pampa and Pantanal with 84 and 35 species, respectively. The Northeast region has the greatest richness (2,617 species), followed by Southeast (2,252), South (1,995), North (1,301) and Central-West (488 species). Regarding the States of the Federation, São Paulo with 1,846 species, Pernambuco with 1,611 and Rio Grande do Sul with 1,377 species are the most diverse.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Optimization method for sequential determination of Cu and Fe in airborne particulate matter collected on glass fiber filters by slurry sampling FAAS

Um método para determinação sequencial de Cu e Fe em material particulado atmosférico (APM) coletado em filtro de fibra de vidro por amostragem em suspensão e espectrometria de absorção atômica com chama é proposto. A otimização do método foi realizada utilizando-se uma massa de amostra seca em pó (filtro + APM) de 50 mg, dispersa em solução de ácido nítrico 0,5 mol L-1 e tratada por 10 min em banho ultrassônico. Os limites de detecção (n = 10) foram 4 e 14 µg g-1 para Cu e Fe no filtro, respectivamente. A repetitividade expressa como desvio padrão relativo foi melhor do que 7 e 8% (n = 3) para Cu e Fe, respectivamente. A exatidão do método foi confirmada analisando-se material de referência certificado de cinzas atmosféricas (BCR 176R). Os resultados obtidos para seis amostras de APM utilizando o método proposto não apresentaram diferenças significativas em comparação com os valores obtidos usando o método de extração ácida e determinação por espectrometria de massas com plasma indutivamente acoplado. O método analítico mostrou-se simples, rápido e confiável.
A slurry sampling flame atomic absorption spectrometric method for sequential determination of Cu and Fe in airborne particulate matter (APM) collected on glass fiber filters is proposed. The method optimization was carried out using a dry powdered sample (filter + APM) mass of 50 mg, dispersed in 0.5 mol L-1 nitric acid solution and under ultrasonic bath for 10 min. The limits of detection (n = 10) were 4 and 14 µg g-1 for Cu and Fe on the filter, respectively. The repeatability expressed as the relative standard deviation was better than 7 and 8% (n = 3) for Cu and Fe, respectively. The accuracy of the method was confirmed by analysis of the fly ash certified reference material (BCR 176R). The results obtained for six samples of APM using the proposed method showed no significant differences with those obtained after microwave-assisted acid extraction and determination by inductively coupled plasma mass spectrometry. The analytical method proved to be simple, fast and reliable.

As distribuições de Co, Cr, Cu, Mn, Ni, Pb, Fe e Al foram investigadas em sedimentos superficiais coletados em 19 sítios da Bacia Hidrográfica do Rio Sergipe, Nordeste do Brasil. Foi definida uma base geoquímica para a região (RGB) usando o ferro como elemento de referência. Fatores de enriquecimento (EF) e índice de geoacumulação (Igeo) foram empregados para determinar a contribuição por origem antropogênica. Valores de EF mostraram que os sedimentos dos sítios 1, 5 e 13 podem ser considerados contaminados por Pb, Cr e Cu, respectivamente. Valores de Igeo mostraram que o sítio 13 pode ser considerado contaminado. Há uma grande predominância nos sedimentos de metais de origem natural. A possível toxicidade foi avaliada comparando com os valores PEC-TEC dos guias de qualidade de sedimentos (SQG). Análise de componentes principais (PCA) separou claramente os pontos em dois grupos e a análise de agrupamento hierárquico (HCA) confirmou as interpretações feitas pela PCA.
The distributions of Co, Cr, Cu, Mn, Ni, Pb, Fe and Al were investigated in surface sediments collected at 19 sites in the Sergipe River Hydrographic Basin of Northeast Brazil. A regional geochemical baseline (RGB) was defined using iron as a reference element. Enrichment factors (EF) and geoaccumulation indices (Igeo) were used to determine the extent of anthropogenic metal pollution. EF values showed that sediments from sites 1, 5 and 13 could be considered contaminated by Pb, Cr and Cu, respectively. Igeo values showed that only site 13 could be considered contaminated. For other sites, results indicated that naturally occurring metals predominated in the sediments. Possible toxicity related to these metals was examined using the comparing sediment chemical data with sediment quality guidelines (SQG) PEC-TEC values. Principal component analysis (PCA) clearly separated the sites into two groups and hierarchical cluster analysis (HCA) confirmed the interpretations made from the PCA results.

Superação da dureza de sementes e frutos de Stylosanthes scabra J. Vogel e seu efeito na germinação

O presente trabalho foi desenvolvido com a finalidade de avaliar diferentes métodos de superação da dureza das sementes e dos frutos, separadamente, de Stylosanthes scabra. Os métodos avaliados foram: testemunha, calor seco a 85ºC durante 10, 12 e 14 horas, ácido sulfúrico concentrado por cinco e dez minutos e imersão em água fervendo durante um minuto. No teste de germinação, registraram-se as porcentagens de germinação (plântulas normais), de plântulas anormais, de sementes duras e de sementes mortas. Concluiu-se que o melhor tratamento foi a escarificação das sementes com o ácido sulfúrico, independente do tempo de imersão; para escarificação dos frutos, um tempo de imersão superior a 10 minutos deve merecer posteriores estudos. O calor seco não superou o problema da impermeabilidade da cobertura protetora e a água fervendo causou a morte de praticamente todas as sementes.
This study was carried out to evaluate methods of breaking Stylosanthes scabra seed and fruit hardness, separately. The methods evaluated were: control dry heat at 85ºC for 10, 12, and 14 hours, immersion in concentrated sulphuric acid for five and 10 minutes, and immersion in boiling water for one minute. The germination test consisted of evaluating normal seedlings (germination percentage), abnormal seedlings, hard seeds and dead seeds. It was concluded that the best treatment was seed scarification with concentrated sulfuric acid for five and 10 minutes. In relation to fruit scarification, an immersion time longer than 10 minutes deserves further study. The dry heat treatment did not overcome the impermeabily of the seed covering layers. Boiling water led to the death of more than 90% of the seeds.
