A new freshwater crayfish species of Parastacus Huxley, 1879 (Malacostraca: Decapoda: Parastacidae) from southern Brazil

Abstract The present paper describes a new species of freshwater crayfish of the genus Parastacus Huxley, 1879 from the municipality of Amaral Ferrador in the state of Rio Grande do Sul, southern Brazil, evaluates its conservation status according to the IUCN Red List Criteria (sub-criterion B1), and updates the identification key for the genus. Parastacus longidactylus sp. n. differs from all analyzed species by the large eyes, the prominent and wide rostral carinae, surpassing the rostral basis and extending to the proximal third of the postorbital carinae, postorbital carinae and rostral carinae prominent and with similar lengths, chelipeds carpus dorsomedial surface not divided longitudinally by groove and long dactylus, surpassing the fixed finger. The possible extent of occurrence was estimated to be 6,968 km² and the species was recorded in only one location in the Camaquã River basin. The main threats to the region are the replacement of native vegetation by crops and livestock, water pollution and soil erosion. However, because there is only one known location where Parastacus longidactylus sp. n. occurs we suggest classifying the species as Data Deficient.

Abstract Pockmarks are circular or elliptical structures formed at the seabed by the expulsion of gas from the subsurface. They are widely distributed along the continental margin off southeastern Brazil and can be over a kilometer wide and 100 meters deep. However, studies concerning the organic characteristics of these pockmark areas are scarce. This study sought to evaluate the organic composition of the sedimentary matter in pockmark areas located in the continental slope region of the southern Brazilian coast. Hydrocarbons, sterols, long-chain alcohols, stable isotopes of C and N, total organic carbon, and total nitrogen were assessed to provide an organic molecular characterization of the pockmarks located in the Southwestern Atlantic Ocean. These compounds did not reflect the organic characteristics of the scape of fluids that generate pockmark structures.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract Cymothoa ianuariiSchioedte & Meinert, 1884 is rediscovered almost 136 years after its original description and redescribed from specimens collected in the state of São Paulo. This species is mainly characterized by adult females with cephalon with conspicuous eyes and not deeply immersed in pereonite 1, pereonites 5-6 much wider than 4, pleotelson twice as wide as long and pleopods 1-5 decreasing in size; Cymothoa excisaPerty, 1833 and C. oestrum (Linnaeus, 1758) are recorded from the state of Bahia. Illustrations and an updated distribution map for these species in Brazil are provided.

Radiomic analysis of MRI to Predict Sustained Complete Response after Radiofrequency Ablation in Patients with Hepatocellular Carcinoma - A Pilot Study

OBJECTIVES: To investigate whether quantitative textural features, extracted from pretreatment MRI, can predict sustained complete response to radiofrequency ablation (RFA) in patients with hepatocellular carcinoma (HCC). METHODS: In this IRB-approved study, patients were selected from a maintained six-year database of consecutive patients who underwent both pretreatment MRI imaging with a probable or definitive imaging diagnosis of HCC (LI-RADS 4 or 5) and loco-regional treatment with RFA. An experienced radiologist manually segmented the hepatic nodules in MRI arterial and equilibrium phases to obtain the volume of interest (VOI) for extraction of 107 quantitative textural features, including shape and first- and second-order features. Statistical analysis was performed to evaluate associations between textural features and complete response. RESULTS: The study consisted of 34 patients with 51 treated hepatic nodules. Sustained complete response was achieved by 6 patients (4 with single nodule and 2 with multiple nodules). Of the 107 features from the arterial and equilibrium phases, 20 (18%) and 25 (23%) achieved AUC >0.7, respectively. The three best performing features were found in the equilibrium phase: Dependence Non-Uniformity Normalized and Dependence Variance (both GLDM class, with AUC of 0.78 and 0.76, respectively) and Maximum Probability (GLCM class, AUC of 0.76). CONCLUSIONS: This pilot study demonstrates that a radiomic analysis of pre-treatment MRI might be useful in identifying patients with HCC who are most likely to have a sustained complete response to RFA. Second-order features (GLDM and GLCM) extracted from equilibrium phase obtained highest discriminatory performance.

Abstract Prosthesis-patient mismatch (PPM) after surgical aortic valve replacement (SAVR) is an issue that has been overlooked (not to say neglected). Cardiac surgeons must bear in mind that this is a real problem that we must tackle. The purpose of this paper is to be a wake-up call to the surgical community by giving a brief overview of what PPM is, its incidence and impact on the outcomes. We also discuss the increasing role played by imaging for predicting and assessing PPM after SAVR (with which surgeons must become more acquainted) and, finally, we present some options to avoid PPM after the surgical procedure.

ABSTRACT Prolactinomas are the most common pituitary adenomas (approximately 40% of cases), and they represent an important cause of hypogonadism and infertility in both sexes. The magnitude of prolactin (PRL) elevation can be useful in determining the etiology of hyperprolactinemia. Indeed, PRL levels > 250 ng/mL are highly suggestive of the presence of a prolactinoma. In contrast, most patients with stalk dysfunction, drug-induced hyperprolactinemia or systemic diseases present with PRL levels < 100 ng/mL. However, exceptions to these rules are not rare. On the other hand, among patients with macroprolactinomas (MACs), artificially low PRL levels may result from the so-called “hook effect”. Patients harboring cystic MACs may also present with a mild PRL elevation. The screening for macroprolactin is mostly indicated for asymptomatic patients and those with apparent idiopathic hyperprolactinemia. Dopamine agonists (DAs) are the treatment of choice for prolactinomas, particularly cabergoline, which is more effective and better tolerated than bromocriptine. After 2 years of successful treatment, DA withdrawal should be considered in all cases of microprolactinomas and in selected cases of MACs. In this publication, the goal of the Neuroendocrinology Department of the Brazilian Society of Endocrinology and Metabolism (SBEM) is to provide a review of the diagnosis and treatment of hyperprolactinemia and prolactinomas, emphasizing controversial issues regarding these topics. This review is based on data published in the literature and the authors' experience.

Resumo O pseudoaneurisma decorrente de pancreatite crônica consiste em complicação rara, porém com alta taxa de mortalidade. Está etiologicamente associado à pancreatite crônica, e seu diagnóstico é feito mais comumente após ruptura, manifestando-se através de sinais clínicos de hemorragia aguda. A tomografia computadorizada desempenha papel importante no diagnóstico; contudo, a angiografia por subtração digital mantém-se como método padrão-ouro para confirmação diagnóstica e direcionamento do tratamento. O presente artigo relata dois casos de pseudoaneurisma em pacientes com pancreatite crônica alcoólica, sendo um da artéria esplênica e outro da artéria gastroduodenal, complicados com sangramento torácico e abdominal respectivamente. Ambos foram submetidos a tratamento endovascular minimamente invasivo com sucesso, através de implante de molas e de stent-grafts.
Abstract Pseudoaneurysm secondary to chronic pancreatitis is a rare complication, but one with a high mortality rate. It is etiologically associated with chronic pancreatitis, and most diagnoses are made after rupture, which manifests with clinical signs of acute hemorrhage. Computed tomography plays an important role in diagnosis, but digital subtraction angiography remains the gold-standard method for diagnostic confirmation and for treatment planning. This article describes two cases of pseudoaneurysm in patients with chronic alcoholic pancreatitis; one involving the splenic artery and the other the gastroduodenal artery, complicated by thoracic and abdominal bleeding respectively. Both were successfully treated, using minimally invasive endovascular methods to implant coils and stent-grafts.

New endemic species of freshwater crayfish Parastacus Huxley, 1879 (Crustacea: Decapoda: Parastacidae) from the Atlantic forest in southern Brazil

Abstract Two new species of Parastacus Huxley, 1879 are described from the Atlantic Forest in southern Brazil: Parastacus buckupi sp. nov. from a highland stream in the hydrographic basin Maquiné river, Maquiné, state of Rio Grande do Sul; and Parastacus pilicarpus sp. nov. from a first order stream in the hydrographic basin Araranguá River, Morro Grande, state of Santa Catarina. Parastacus buckupi sp. nov. is distinguishable from all other Parastacus species via its large and globose chelipeds, by the presence of tufts of long and simple setae in the cutting edge of cheliped fingers, which are more abundant proximally and by possessing a telson with an acute distal margin. Parastacus pilicarpus sp. nov. is diagnosed mainly by the distal portion of the carpus which possess tufts of long, simple setae, and by having an epistome whose anteromedian lobe is heptagonal and as long as it is wide. Finally, P. pilicarpus antennal scales lateral margin terminates in a very strong terminal spine and mandibles incisive lobe has nine teeth, with the first tooth from the anterior margin being the largest. The conservation status based on the sub-criterion B1 of IUCN Red List allowed us to classify these species as “ENDANGERED” and “DATA DEFICIENT”, respectively.

Laryngotracheobronchial papillomatosis: chest CT findings

RESUMO Objetivo: Analisar os achados em TCs de tórax em 16 pacientes (8 homens e 8 mulheres) com papilomatose laringotraqueobrônquica. Métodos: Estudo retrospectivo que incluiu pacientes com idade variando de 2 a 72 anos. As imagens de TC foram avaliadas por dois observadores, de forma independente, e os casos discordantes foram resolvidos por consenso. Os critérios de inclusão foram presença de anormalidades na TC, e o diagnóstico foi confirmado por exame anatomopatológico das lesões papilomatosas. Resultados: Os sintomas mais frequentes foram rouquidão, tosse, dispneia e infecções respiratórias de repetição. Os principais achados na TC foram formações nodulares na traqueia, nódulos sólidos e/ou escavados no parênquima pulmonar, aprisionamento aéreo, massas e consolidação. Formações nodulares na traqueia foram observadas em 14 pacientes. Somente 2 pacientes apresentaram lesões parenquimatosas pulmonares na ausência de acometimento traqueal, e 1 paciente não apresentou disseminação pulmonar da doença, somente comprometimento de via aérea. Nódulos sólidos no parênquima pulmonar foram observados em 14 pacientes (87,5%) e nódulos escavados em 13 (81,2%). Massas foram observadas em 6 pacientes (37,5%); aprisionamento aéreo, em 3 (18,7%); consolidação, em 3 (18,7%); e derrame pleural, em 1 (6,3%). O comprometimento pulmonar foi bilateral em todos os pacientes. Conclusões: Os achados tomográficos mais frequentes foram formações nodulares na traqueia, nódulos sólidos e/ou escavados no parênquima pulmonar e massas parenquimatosas. A transformação maligna das lesões foi observada em 5 casos.
ABSTRACT To evaluate the findings on chest CTs in 16 patients (8 men and 8 women) with laryngotracheobronchial papillomatosis. Methods: This was a retrospective study involving patients ranging from 2 to 72 years of age. The evaluation of the CT scans was independently performed by two observers, and discordant results were resolved by consensus. The inclusion criteria were presence of abnormalities on the CT scans, and the diagnosis was confirmed by anatomopathological examination of the papillomatous lesions. Results: The most common symptoms were hoarseness, cough, dyspnea, and recurrent respiratory infections. The major CT findings were nodular formations in the trachea, solid or cavitated nodules in the lung parenchyma, air trapping, masses, and consolidation. Nodular formations in the trachea were observed in 14 patients (87.5%). Only 2 patients had lesions in lung parenchyma without tracheal involvement. Only 1 patient had no pulmonary dissemination of the disease, showing airway involvement only. Solid and cavitated lung nodules were observed in 14 patients (87.5%) and 13 (81.2%), respectively. Masses were observed in 6 patients (37.5%); air trapping, in 3 (18.7%); consolidation in 3 (18.7%); and pleural effusion, in 1 (6.3%). Pulmonary involvement was bilateral in all cases. Conclusions: The most common tomography findings were nodular formations in the trachea, as well as solid or cavitated nodules and masses in the lung parenchyma. Malignant transformation of the lesions was observed in 5 cases.

A new species of Parastacus Huxley, 1879 (Crustacea, Decapoda, Parastacidae) from a swamp forest in southern Brazil

Abstract In this contribution we describe a new species of burrowing crayfish of the genus Parastacus Huxley, 1879 from a swamp forest in southern Brazil and determine its conservation status. The distinction of the new species is based on morphology and the mitochondrial DNA marker 16S rRNA. The extinction risk was assessed according to the sub-criterion B1 of IUCN that estimates the Extent of Occurrence (EOO). Parastacus tuerkayi sp. nov. is morphologically distinguishable from all species of Parastacus by having three lines of verrucous tubercles on the dorsomesial margin of the cheliped propodus and a suborbital angle exceeding 90°. The EOO comprises 647,674 km², and the species is classified as “endangered”. Phylogenetic relationships indicate the distinct position of this new species in relation to the already described species.

OBJETIVO: Determinar a frequência dos achados tomográficos e sua distribuição no parênquima pulmonar de pacientes com pneumonia em organização. MÉTODOS: Estudo retrospectivo das imagens de TCAR de 36 pacientes adultos (26 mulheres e 10 homens) com pneumonia em organização confirmada por exame histopatológico. A faixa etária dos pacientes incluídos foi de 19 a 82 anos (média: 56,2 anos). As imagens foram avaliadas por dois observadores, de forma independente, e os casos discordantes foram resolvidos por consenso. RESULTADOS: O achado tomográfico mais comum foi o de opacidades em vidro fosco, presentes em 88,9% dos casos. O segundo achado mais comum foi o de consolidação (em 83,3% dos casos), seguido de opacidades peribroncovasculares (em 52,8%), reticulação (em 38,9%), bronquiectasias (em 33,3%), nódulos intersticiais (em 27,8%), espessamento de septos interlobulares (em 27,8%), padrão perilobular (em 22,2%), sinal do halo invertido (em 16,7%), nódulos do espaço aéreo (em 11,1%) e sinal do halo (em 8,3%). As lesões foram predominantemente bilaterais, e os terços médios e inferiores dos pulmões foram as regiões mais afetadas. CONCLUSÕES: Os achados mais frequentes foram opacidades em vidro fosco e consolidações, com distribuição predominantemente aleatória, embora tenham sido mais comuns nos terços médios e inferiores dos pulmões.
OBJECTIVE: To determine the frequency of HRCT findings and their distribution in the lung parenchyma of patients with organizing pneumonia. METHODS: This was a retrospective review of the HRCT scans of 36 adult patients (26 females and 10 males) with biopsy-proven organizing pneumonia. The patients were between 19 and 82 years of age (mean age, 56.2 years). The HRCT images were evaluated by two independent observers, discordant interpretations being resolved by consensus. RESULTS: The most common HRCT finding was that of ground-glass opacities, which were seen in 88.9% of the cases. The second most common finding was consolidation (in 83.3% of cases), followed by peribronchovascular opacities (in 52.8%), reticulation (in 38.9%), bronchiectasis (in 33.3%), interstitial nodules (in 27.8%), interlobular septal thickening (in 27.8%), perilobular pattern (in 22.2%), the reversed halo sign (in 16.7%), airspace nodules (in 11.1%), and the halo sign (in 8.3%). The lesions were predominantly bilateral, the middle and lower lung fields being the areas most commonly affected. CONCLUSIONS: Ground-glass opacities and consolidation were the most common findings, with a predominantly random distribution, although they were more common in the middle and lower thirds of the lungs.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Thoracic textilomas: CT findings

OBJETIVO: Analisar retrospectivamente os aspectos encontrados em TCs de tórax com textiloma torácico. MÉTODOS: Estudo retrospectivo de 16 pacientes (11 homens e 5 mulheres) com diagnóstico de textiloma torácico confirmado cirurgicamente. As TCs de tórax foram avaliadas, de modo independente, por dois observadores, e os casos discordantes foram resolvidos por consenso. RESULTADOS: Na maioria dos casos (62,5%), o fator causal foi a cirurgia cardíaca prévia. Os sintomas mais frequentes foram dor torácica (em 68,75%) e tosse (em 56,25%). Em todos os casos, o principal achado tomográfico foi de massa com contornos regulares e limites bem definidos ou parcialmente definidos. A localização dos textilomas ocorreu na mesma proporção no hemitórax direito e esquerdo, mas foi mais comum no terço inferior (em 56,25%). O tamanho das massas foi ≤ 10 cm e > 10 cm, respectivamente, em 10 (62,5%) e em 6 pacientes (37,5%). A maioria dos textilomas apresentou densidade heterogênea (81,25%), observando-se no seu interior calcificações, gás, marcador radiopaco ou material da compressa. A impregnação periférica da lesão foi observada em 12 (92,3%) dos 13 pacientes que receberam o meio de contraste. A cirurgia demonstrou acometimento do espaço pleural e pericárdico, respectivamente, em 14 (87,5%) e em 2 pacientes (12,5%). CONCLUSÕES: É importante reconhecer os principais aspectos tomográficos dos textilomas intratorácicos a fim de incluir essa possibilidade no diagnóstico diferencial em pacientes com dor torácica e tosse e história de cirurgia cardíaca ou torácica, contribuindo assim para o tratamento precoce dessa complicação cirúrgica.
OBJECTIVE: The aim of this study was to analyze chest CT scans of patients with thoracic textiloma. METHODS: This was a retrospective study of 16 patients (11 men and 5 women) with surgically confirmed thoracic textiloma. The chest CT scans of those patients were evaluated by two independent observers, and discordant results were resolved by consensus. RESULTS: The majority (62.5%) of the textilomas were caused by previous heart surgery. The most common symptoms were chest pain (in 68.75%) and cough (in 56.25%). In all cases, the main tomographic finding was a mass with regular contours and borders that were well-defined or partially defined. Half of the textilomas occurred in the right hemithorax and half occurred in the left. The majority (56.25%) were located in the lower third of the lung. The diameter of the mass was ≤ 10 cm in 10 cases (62.5%) and > 10 cm in the remaining 6 cases (37.5%). Most (81.25%) of the textilomas were heterogeneous in density, with signs of calcification, gas, radiopaque marker, or sponge-like material. Peripheral expansion of the mass was observed in 12 (92.3%) of the 13 patients in whom a contrast agent was used. Intraoperatively, pleural involvement was observed in 14 cases (87.5%) and pericardial involvement was observed in 2 (12.5%). CONCLUSIONS: It is important to recognize the main tomographic aspects of thoracic textilomas in order to include this possibility in the differential diagnosis of chest pain and cough in patients with a history of heart or thoracic surgery, thus promoting the early identification and treatment of this postoperative complication.
