Introducción: En Colombia se comercializan diferentes moléculas de Remifentanil que nunca han sido comparadas en un entorno clínico. Objetivo: El objetivo de este estudio fue investigar el perfil farmacodinámico de la molécula innovadora de Remifentanil (grupo O: Glaxo SmithKline Manufacturing S.P.A.) y dos moléculas genéricas (grupo A: Laboratorios Chalver de Colombia S.A. y grupo B: Instituto Biológico Contemporáneo, Argentina) registradas en Colombia. Métodos: Se llevó a cabo un experimento clínico doble ciego, aleatorizado, controlado. Se comparó la molécula original de Remifentanil (grupo O, n = 29) frente a las dos moléculas genéricas (grupo A, n = 29; grupo B, n = 32) durante la inducción anestésica e intubación oro-traqueal de pacientes adultos ASA I sin predictores de vía aérea difícil. Se evaluaron las dosis 6, 8 y 10 ng/ml (TCI, Target Controlled Infusion) con el modelo de Minto. La inducción se complementó con Propofol 5 mcg/ml (TCI) con modelo de Schneider y Rocuronio 0.6mg/kg. El desenlace primario se evaluó como las diferencias en la presión arterial media y en la frecuencia cardiaca preintubación (momento en que se alcanza la concentración objetivo en sitio efecto) y posintubación (máximo valor alcanzado en 5 minutos). Resultados: Se observó similitud en el perfil farmacodinámico de las moléculas de Remifentanil estudiadas. Las diferencias en el cambio de frecuencia cardiaca fue de 1.27 (IC 95% -3.11;5.67) con la molécula A y 1.40 (IC 95% -2.65;5.46) con la molécula B frente a la molécula O (latidos/minuto). Las diferencias en el cambio de presión arterial media fue de 1 (CI 95% -4.81;6.81) para la molécula A y 1.82 (IC 95% -4.08;7.74) para la molécula B frente a la molécula O (mmHg). Hubo un caso de hipotensión arterial en cada grupo. Conclusión: Los resultados sugieren que desde un punto de vista farmacodinámico las moléculas innovadora y genéricas de Remifentanil son similares para la laringo-scopia/intubación con dosis TCI de 6, 8 y 10 ng/ml.

Introduction: Several remifentanil products are commercialized in Colombia though they have never been compared in a clinical setting. Objective: The aim of this study was to investigate the pharmacodynamic profile of the branded remifentanil molecule (group O: Glaxo SmithKline Manufacturing S.P.A.) and two unbranded molecules (group A: Laboratorios Chalver de Colombia S.A. and group B: Instituto Biológico Contemporáneo, Argentina) registered in Colombia. Methods: We carried out a double-blind, randomized, controlled trial. The branded molecule of remifentanil (group O, n = 29) was compared with the two unbranded molecules (group A, n = 29; group B, n = 32) during anesthetic induction and tracheal intubation in adult patients ASA I without predictors for difficult airway. The target controlled infusion (TCI) doses evaluated were 6, 8 and 10ng/ml with the Minto model. Induction was complemented with propofol 5 mcg/ml (TCI) with the Schneider model and rocuronium 0.6 mg/kg. The primary outcome was the difference between preintubation (TCI equilirium) and postintubation (maximum measurement within 5 min) mean arterial pressure and heart rate. Results: A similar pharmacodynamic profile was observed in all of the studied remifentanil molecules. The differences in the change in heart rate were 1.27 (95% CI -3.11;5.67) with molecule A and 1.40 (95% CI -2.65;5.46) with molecule B compared to molecule O (beats/min). The differences in the change in mean arterial pressure were 1 (95% CI -4.81;6.81) with molecule A and 1.82 (95% CI -4.08;7.74) with molecule B compared to molecule O (mmHg). There was one case of arterial hypotension in each group. Conclusion: The results suggest that, from a pharmacodynamic point of view, branded and unbranded remifentanil molecules are similar for laryngoscopy/intubation with TCI doses 6, 8 and 10ng/ml.

Objectives To evaluate the PSA in a large population of Brazilian men undergone to check up, and correlate the PSA cutoffs with prostate size and urinary symptoms. Materials and Methods This is a cross sectional study performed with men between 40 and 70 years undergone to check-up. All men were undergone to urological evaluation, digital rectal examination, prostate-specific antigen, and ultrasonography The exclusion criteria were men who used testosterone in the last six months, or who were using 5 alpha-reductase inhibitors. Results A total of 5015 men with an average age of 49.0 years completed the study. Most men were white and asymptomatic. The PSA in the three different aging groups were 0.9 ± 0.7ng/dL for men between 40 and 50; 1.2 ± 0.5ng/dL for men between 50 and 60; and 1.7 ± 1.5ng/dL for men greater than 60 years (p=0.001). A total of 192 men had PSA between 2.5 and 4ng/ml. From these men 130 were undergone to prostate biopsy. The predictive positive value of biopsy was 25% (32/130). In the same way, 100 patients had PSA >4ng/mL. From these men, 80 were undergone to prostate biopsy. In this group, the predictive positive value of biopsy was 40% (32/100). The Gleason score was 6 in 19 men (60%), 7 in 10 men (31%) and 8 in 3 men (9%). Conclusions The PSA level of Brazilian men undergone to check up was low. There was a positive correlation with aging, IPSS and prostate size.

Introducción: La tromboelastografía (TEG) es un método para valorar las características de la formación y destrucción del coágulo. Una variedad de aplicaciones han sido sugeridas en la literatura. Objetivo: Proporcionar un resumen acerca del conocimiento actual de las aplicaciones de la TEG. Métodos: Se realizó una búsqueda en la base de datos PubMed hasta julio de 2012 con el término «Thrombelastography [MeSH Terms]». Se analizaron artículos de estudios retrospectivos y prospectivos, revisiones y guías conteniendo información acerca de las aplicaciones de la TEG escritos en inglés y español. Resultados: La búsqueda arrojó 3.139 artículos desde 1962. Se clasificaron en 8 categorías: 862 (27,6%) asociados a enfermedades no quirúrgicas, 294 (9,4%) a trasplante hepático, 711 (22,6%) a investigación básica, 174 (5,5%) a obstetricia, 228 (7,3%) a cirugía cardiovascular, 177 (5,6%) a otras cirugías, 234 (7,4%) a técnicas anestésicas y 459 (14,6%) a fármacos. Conclusión: La TEG como herramienta diagnóstica y para guiar terapia transfusional está en aumento. La TEG aún continúa en estudio en diferentes áreas del conocimiento clínico y aún falta definir adecuadamente los alcances de esta técnica diagnóstica. Es evidente que se debe hacer un uso racional de la TEG, conocer a fondo sus fortalezas y debilidades y continuar explorando nuevas aplicaciones.

Introduction: Thrombelastography (TEG) is a method to assess clot formation and destruction. Various applications have been suggested in the literature. Objective: To provide an overview of the current knowledge about TEG applications. Methods: Adatabase search in PubMed was performed up to July 2012 using the term "Thrombelastography [MeSH Terms]". We analysed retrospective and prospective studies, reviews and guidelines with information about the applications of TEG written in English and Spanish. Results: The search resulted in 3139 papers since 1962. These were classified in 8 categories: 862 (27.6%) in non-surgical diseases, 294 (9.4%) in liver transplant, 711 (22.6%) in basic research, 174 (5.5%) in obstetrics, 228 (7.3%) in cardiovascular surgery, 177 (5.6%) in other types of surgery, 234 (7.4%) in anaesthetic techniques, and 459 (14.6%) in relation with medications. Conclusion: The application of TEG as a diagnostic tool and as a guide in transfusion therapy is increasing. Its use is still in development in different clinical fields and the advantages and limitations of this technique still have to be defined. It is evident that thrombelastography should be used with caution, and its strengths and weaknesses as well as new applications must continue to be explored.

Se obtuvieron recubrimientos de aluminio-silicio en los aceros inoxidables AISI 304 y AISI 317. La deposición se realizó a 540 ºC, con una proporción de gases activos (HCl/H2: 1/15.3), como gas de arrastre se utilizó argón. El lecho del proceso CVD-FBR estaba formado por 2,5 g de polvo de aluminio, 7,5 g polvo de silicio y 90 g de alúmina. Después de depositados los recubrimientos, se le dio un tratamiento térmico para mejorar sus propiedades mecánicas y su comportamiento frente a la oxidación, por entre la difusión de los elementos de aleación. La simulación termodinámica se realizó con el software Thermo-Calc para obtener información sobre la posible composición del material depositado. Las muestras recubiertas y sin recubrir, se expusieron a 750 ºC en una atmósfera donde el vapor se transporta a las muestras usando un flujo de N2 de 40 ml / min y 100% de vapor de agua (H2O). Aceros recubiertos ganaron algo de peso durante las mil horas de exposición y resisten muy bien el ataque corrosivo frente a los sustratos recubiertos. Además, los aceros inoxidables recubiertos muestran una velocidad de oxidación con tendencia logarítmica, mientras que la velocidad de oxidación de acero sin recubrimiento tiene tendencia lineal.

Aluminum-silicon coatings were deposited onto stainless steels AISI 304 and AISI 317. The deposition was performed at 540ºC with a ratio of active gases HCl/H2 of 1/15.3; argon was used as a carrier gas. The bed of the FBR-CVD process consisted of 2.5 g aluminum powder, 7.5 g silicon powder and 90 g alumina. After the coatings were deposited, each sample was given a heat treatment to improve its mechanical properties and oxidation behavior by diffusing the alloying elements. Thermodynamic simulation was performed with Thermo-Calc software to investigate the composition of the deposited material. The coated and uncoated specimens were exposed to temperatures of 750 ºC in an atmosphere where the vapor was transported to the samples using a flow of N2 of 40 ml/min and 100% water vapor (H2O). The coated specimens gained little weight during the thousand hours of exposure and will thus guard against a corrosive attack compared to the uncoated substrates. In addition, the coated stainless steels show an oxidation rate with a logarithmic trend while the uncoated steel oxidation rate showed a linear trend.

Introducción: Se ha relacionado la depresión con la ocurrencia de enfermedades cardiovasculares. Parece incrementar el riesgo de muerte en enfermedad coronaria. La presencia de depresión tras un síndrome coronario agudo (SCA) y su relación con factores socioeconómicos no se ha estudiado en Colombia. Objetivo: Describir la frecuencia de depresión y sus factores asociados en pacientes hospitalizados por SCA. Métodos: Entre abril y mayo de 2008, se llevó a cabo un estudio de corte transversal en tres unidades de cuidado coronario en Santa Marta, Colombia. La presencia de síntomas depresivos se evaluó con el Inventario de Depresión de Beck (BDI). Se consideró depresión si el BDI era ≥ 10. Se probó la relación entre la presencia de depresión y características demográficas, clínicas y paraclínicas. Resultados: Se encontró un BDI ≥ 10 (algún grado de depresión) en 21 (63,64%) pacientes; 9 de 10 (91,66%) pacientes sin ingresos fijos presentaron depresión, frente a 12 de 23 (57,14%) con ingresos fijos (p = 0,037). En 8 de 8 (100%) pacientes con inversión patológica de la onda T en el electrocardiograma, se evidenció depresión, frente a 13 de 25 (52%) sin cambios en la onda T (p = 0,014); 17 de 22 (77,27%) pacientes que tenían consumo previo de cigarrillos presentaron depresión, frente a 4 de 11 (36,36%) que nunca habían consumido cigarrillos (p = 0,021). Conclusiones: Los síntomas depresivos son altamente prevalentes entre los pacientes que sufren un SCA. El desempleo, la inversión patológica de la onda T y el consumo de cigarrillos parecen estar asociados con la presencia de síntomas depresivos tras un evento coronario agudo.

Background: Depression has been related to the occurrence of cardiovascular diseases, and also appears to increase the risk of death from coronary artery disease. The presence of depression after the occurrence of an acute coronary syndrome (ACS) and its relationship with socioeconomic factors has not been studied in Colombia. Objective: To describe the frequency of depression and associated factors in patients hospitalized with an ACS. Methods: A cross-sectional study was carried out between April and May 2008 in three coronary care units in Santa Marta, Colombia. The presence of depression was evaluated with the Beck Depression Inventory (BDI). Depression was considered to be present if BDI was equal or greater than 10. The relationship between the presence of depression and demographic, clinic and laboratory characteristics was analyzed. Results: A BDI score ≥10 (any degree of depression) was seen in 21 (63.64%) of the patients. In 9 out of 10 (91.66%) patients without stable income had depression in contrast to 12 out of 23 (57.14%) with stable income (P=.037). Depression was found in in 8 of the 8 (100%) patients with electrocardiographic pathological T wave inversion, in contrast to 13 out of the 25 (52%) without changes in T wave (P=.014). Depression was detected in 17 of 22 (77.27%) patients who smoked cigarettes versus 4 of 11 (36.36%) of patients who have never smoked (P=.021). Conclusions: Depressive symptoms are significantly present in patients suffering an acute coronary syndrome. Unemployment, pathological T wave inversion, and smoking seem to be associated with the presence of depressive symptoms after an acute coronary event.

Terrace systems indicate that the Seno Otway and Skyring, and the Última Esperanza fjord formed the largest interconnected proglacial lake system of southern Patagonia (5700 km²) during the early deglaciation (<18 to 14 kyr BP). It drained through the eastern Seno Otway towards the Atlantic. The retreat of glaciers from the Jerónimo Channel at around 14.0 cal kyr caused a mega outflow event (320 km³) which lowered the Seno Otway lake level by 95 m and initiated a marine transgression as well as an intensive long term erosion of newly exposed shore lines around the Seno Otway. Between 11 to 10 kyr BP a more limited marine transgression occurred to the eastern sector of Seno Skyring, probably through the Gajardo Channel. This was caused by a further glacier retreat around the Gran Campo Nevado Ice Cap (GCN) during the early Holocene southern hemisphere thermal maximum (after 12 kyr BP). The salinities of the Seno Otway (28-30 psu) and Seno Skyring (17-21 psu) remained low throughout the Holocene. However, restricted salinity variations occurred due to further isostatic changes affecting the marine water inflow across sills as well as the interplay of precipitation/evaporation. The salinity changes affected clay mineral flocculation and sediment transport as well as aquatic ecosystems. In particular, Neoglacial glacier advances modified the degree of the marine transgression in this Andean sector due to closure of fjords and isostatic effects. Glaciers closed the Gajardo Channel and the outflow of Lago Muñoz Gamero at least several times in the periods from 3.0 to 2.2 and from 1.0 to 0.1 kyr BP, and left some glacial clay fingerprints in Seno Otway and Skyring sediments. Aquatic environment in fjord regions with stepper slopes in the western Seno Skyring and Otway area were also affected by regular mass flow events, partly triggered by earthquakes and events of extreme precipitation. Holocene tephra depositon caused additional impacts.

Los sistemas de terrazas evidencian que el Seno Otway y Skyring y el fiordo de Última Esperanza, formaron el mayor sistema lacustre proglacial interconectado de la Patagonia Austral (5.700 km²) durante la deglaciación temprana (< 18 a 14 ka BP). Este sistema drenaba por el este del Seno Otway hacia el Atlántico. El retroceso de los glaciares desde el Canal Jerónimo alrededor de 14,0 cal kyr causó un mega evento de desagüe (320 km³), que bajó 95 metros el nivel lacustre en el Seno Otway e inició una transgresión marina, así como una intensa erosión a largo plazo de las líneas de costa que quedaron expuestas alrededor del Seno Otway. Entre 11 a 10 ka BP se produjo una transgresión marina más limitada en el sector oriental del Seno Skyring, probablemente a través del Canal Gajardo. Esto fue causado por el retroceso de los glaciares alrededor del Gran Campo Nevado (GCN) durante el Máximo Termal del Holoceno Temprano en el Hemisferio Sur (después de 12 ka BP). Las salinidades del Seno Otway (28-30 psu) y Seno Skyring (17-21 psu) se mantuvieron bajas durante todo el Holoceno. Sin embargo, ocurrieron acotadas variaciones de salinidad debido a nuevos cambios isostáticos en la entrada de agua marina, así también por cambios en la interrelación entre precipitación/evaporación. Estos afectaron la floculación de los minerales de arcilla, el transporte de sedimentos y también a los ecosistemas acuáticos. En particular, el avance de los hielos neoglaciales modificaron el grado de la transgresión marina en este sector andino debido al cierre de los fiordos y a efectos isostáticos. Los glaciares cerraron el Canal Gajardo y la salida de Lago Muñoz Gamero al menos varias veces en los períodos de 3,0-2,2 y 1,0-0,1 ka BP, dejando algunas huellas características de arcilla glacial en los sedimentos de los Senos Otway y Skyring. Las regiones de fiordos con pendientes más pronunciadas al oeste de los Senos Skyring y Otway también se vieron afectados por los eventos de flujo de masa regulares, en parte provocados por terremotos y eventos de precipitaciones extremas.

Periodontal disease is the major cause of tooth loss in adults. Porphyromonas gingivalis and Aggregatibacter actinomycetemcomitans are considered key pathogens in periodontitis. The treatment consists of oral hygiene education, instrumentation for removal of calculus (scaling), chemotherapy and periodontal surgery. Several agents are commercially available; these chemicals can alter oral microbiota and have undesirable sideeffects such as vomiting, diarrhea and tooth staining. Hence, the search for alternative products continues and natural phytochemicals isolated from plants used as traditional medicine and the use of biomaterials are considered good alternatives. Chitosan and pullulan are polymers that have been proposed due to their favorable properties such as biocompatibility, biodegradability, and adhesion ability. They can be used as local delivery systems of active principles of plant extracts. Thymus vulgaris, Matricaria chamomilla, Croton lechleri, Calendula officinalis L. and Juliana adstringens Schl. are known to have medicinal activity, and they are used in Mexican traditional medicine. Their extracts were tested in vitro for antimicrobial activity against P. gingivalis and A. actinomycetemcomitans, using agar diffusion and microdilution methods. The antimicrobial activity of films from biopolymers with plant extracts was evaluated by measuring the zones of inhibition against the tested organisms. The aim of this study was to develop bioadhesive films from chitosan and pullulan with added plant extracts and determine the antimicrobial activity of films against periodontal pathogens.

La enfermedad periodontal es la principal causa de perdida de dientes en los adultos. Los agentes causales comunmente identificados con la enfermedad son Porphyromonas gingivalis y Aggregatibacter actinomycetemcomitans. El tratamiento de la enfermedad consiste en educacion sobre higiene oral, remocion de calculos por medio de instrumentacion (raspado y alisado de la raiz), la administracion de medicamentos y cirugia. Hay multiples agentes quimicos disponibles comercialmente; estos pueden alterar la microflora oral y tener efectos secundarios indeseables como vomito, diarrea y pigmentacion dental. Por lo tanto, los productos naturales como los fitoquimicos aislados de plantas que son usadas como medicinas tradicionales y los biomateriales, son considerados buenas alternativas. El quitosan y el pululan son polimeros que han sido propuestos debido a sus propiedades de biocompatibilidad, biodegradabilidad, habilidad de adhesion y que pueden ser usados como sistemas de liberacion de los principios activos de extractos de plantas. Los extractos de Thymus vulgaris, Matricaria chamomilla, Croton lechleri, Calendula officinalis L. y Juliana adstringens Schl. son conocidos por tener actividad medicinal y se usan en la medicina tradicional Mexicana. La actividad antimicrobiana de sus extractos fue probada in vitro contra P. gingivalis y A. actinomycetemcomitans usando los metodos de difusion en agar y de microdilucion. La actividad antimicrobiana de peliculas a base de biopolimeros con extractos de plantas fue evaluada midiendo las zonas de inhibicion de crecimiento de los organismos probados. El proposito de este estudio fue desarrollar peliculas bioadhesivas de quitosan y pululan adicionadas con extractos de plantas y evaluar su actividad antimicrobiana contra periodontopatogenos.

Most molecular trees of trypanosomatids are based on point mutations within DNA sequences. In contrast, there are very few evolutionary studies considering DNA (re) arrangement as genetic characters. Waiting for the completion of the various parasite genome projects, first information may already be obtained from chromosome size-polymorphism, using the appropriate algorithms for data processing. Three illustrative models are presented here. First, the case of Leishmania (Viannia) braziliensis/L. (V.) peruviana is described. Thanks to a fast evolution rate (due essentially to amplification/deletion of tandemly repeated genes), molecular karyotyping seems particularly appropriate for studying recent evolutionary divergence, including eco-geographical diversification. Secondly, karyotype evolution is considered at the level of whole genus Leishmania. Despite the fast chromosome evolution rate, there is qualitative congruence with MLEE- and RAPD-based evolutionary hypotheses. Significant differences may be observed between major lineages, likely corresponding to major and less frequent rearrangements (fusion/fission, translocation). Thirdly, comparison is made with Trypanosoma cruzi. Again congruence is observed with other hypotheses and major lineages are delineated by significant chromosome rearrangements. The level of karyotype polymorphism within that "species" is similar to the one observed in "genus" Leishmania. The relativity of the species concept among these two groups of parasites is discussed.
