Resumo Sling de uretra média é o tratamento padrão para a incontinência urinária de esforço. A abordagem transobturatória (TOT) surgiu como alternativa para minimizar os riscos da inserção às cegas das agulhas com taxa de complicações perfurativas menores quando comparadas à abordagem retropúbica. Apresentamos um caso de lesão em ramo da artéria obturatória esquerda após sling TOT que evoluiu com sangramento intenso e instabilidade hemodinâmica, sendo tratado com embolização.

Abstract The placement of a suburethral sling is standard treatment for stress urinary incontinence. The transobturator technique (TOT) emerged as an alternative to minimize the risks of the blind insertion of needles, leading to a lower rate of perforation complications compared with the retropubic approach. We present a case of injury to a branch of the left obturator artery following the placement of a urethral sling using TOT, followed by intense bleeding and hemodynamic instability, which was treated with embolization.

RESUMO A ejaculação precoce é a disfunção sexual masculina mais frequente, com uma estimativa de acometimento de 20 a 30% dos homens em algum momento da vida. Foi realizada uma busca no Pubmed, do ano 2000 até os dias atuais, com a finalidade de revisar publicações relacionadas ao manejo e ao tratamento da ejaculação precoce. Terapias comportamentais foram a base do manejo da ejaculação precoce por muitos anos, embora as evidências de sua eficácia a curto prazo sejam limitadas. Terapias de uso tópico agem por meio de dessensibilização do pênis, mas não alteram a sensação da ejaculação. Os inibidores seletivos da recaptação da serotonina são utilizados para depressão e, em geral, também para tratar ejaculação precoce, com base na observação de que o retardo na ejaculação é um efeito colateral frequente dessa classe de drogas. A dapoxetina é um inibidor seletivo da recaptação da serotonina de curta ação, que foi formulado para tratar a ejaculação precoce, e seus resultados parecem muito promissores.

ABSTRACT Premature ejaculation is the most frequent male sexual dysfunction, estimated to affect 20 to 30% of men at some time in their life. A Pubmed search from the year 2000 to the present was performed to retrieve publications related to management or treatment of premature ejaculation. Behavioral techniques have been the mainstay of premature ejaculation management for many years, although evidence of their short-term efficacy is limited. Topical therapies for premature ejaculation act by desensitizing the penis and do not alter the sensation of ejaculation. Selective serotonin reuptake inhibitors (SSRIs), commonly used in the treatment of depression, are often used to treat premature ejaculation, based on the observation that delayed ejaculation is a frequent side effect of this drug class. Dapoxetine is a short-acting SSRI formulated to treat premature ejaculation, and results seem very promising.

The cyclamate, a sweetner substance derived from N-cyclo-hexyl-sulfamic acid, is largely utilized as a non-caloric artificial edulcorant in foods and beverages as well as in the pharmaceutical industry. The objective of this study was to evaluate fetal hepatic karyometric and stereological alterations in the rat fetal liver resulting from the intraperitoneal administration of sodium cyclamate. The livers of ten rats were evaluated, five treated and five controls chosen at random, in which five rats that received from the 10th to 14th days of pregnancy an intraperitoneal daily injection of sodium cyclamate at 60 mg/Kg of body weight during 5 days. At the 20th day of gestation, the animals were removed and weighed, as were their placentas, on a precision balance; the length of the umbilical cords also were measured. After the laboratory processing, semi-seriated 6mm cuts stained with haematoxyline and eosine were performed. In seven karyometric parameters (major, minor, and medium diameters, volume, area, perimeter, and volume-area ratio), the increase was statistically significant in the treated group when compared with control group. Stereological parameters showed in the treated group a significant increase in the cytoplasmatic and cellular volume, and a significant reduction in the nucleus-cytoplasm ratio as well as in the numerical cellular density. These results showed that the sodium cyclamate in pregnant rats led to retardation of fetal development and hepatic-cellular hypertrophy in the offspring, suggesting toxicity in liver of rat fetuses

El ciclamato, es una substancia derivada del ácido N-ciclo-hexil-sulfámico, bastante usada como edulcorante no calórica en alimentos y bebidas, así como en la industria farmacéutica. El objetivo del trabajo fue evaluar los efectos del ciclamato de sodio en hígados de fetos de ratas, considerándose las alteraciones cariométricas y estereológicas. Fueron utilizadas 10 ratas adultas (Rattus norvegicus) variedad Wistar, con peso medio de 240 g, siendo 5 el grupo control y 5 tratadas con ciclamato de sodio. Entre el 10 y 14 día de la preñez, 5 ratas recibieron una inyección diaria intraperitoneal de 60mg/Kg/día de ciclamato de sodio durante 5 días. En el 20 día, los animales fueron sacrificados y los fetos fijados en solución de Alfac, incluidos en parafina, cortados a 6 µm y teñidos com H-E. Hubo aumento estadísticamente significativo en siete parámetros cariométricos (diámetros mayor, menor y medio, volumen, área, perímetro y relación área/volumen) en el grupo tratado con ciclamato de sodio comparado con el grupo control. Parámetros estereológicos mostraron aumento significativo en los volúmenes citoplasmático y celular y disminución significativa en la relación núcleo/citoplasma y densidad numérica celular. Los resultados mostraron que el uso del ciclamato de sodio en las ratas preñadas causó retardo en el desarrollo fetal e hipertrofia celular hepática en los fetos, sugerente de toxicidad en el hígado fetal de las ratas

The cyclamate is used as artificial non-caloric sweetner in a wide variety of foods and beverages, being 30 times as sweet as sugar without the bitter after-taste of saccharin. It appears in the composition of the products as sodium and calcium cyclamates and cyclamic acid. The purpose of this research was to assess the effect of sodium cyclamate in kidneys of rats fetuses considering morphometric changes of glomerulus, proximal and distal convoluted tubules, and collecting duct. Ten pregnant rat (Rattus norvegicus) weighting an average of 238 g were divided into two groups: 5 in the control group and 5 in treated group with sodium cyclamate. Between the 10th and the 14th pregnancy day, 5 rats received an intraperitoneal daily injection of 60 mg/kg of body weight of sodium cyclamate during 5 days. At the 20th, the animals were sacrificed and the fetuses were fixed in Alfac solution, included in paraffin, sectioned and stained with haematoxyline and eosine. Morphometry by the karyometric technique was the method used. The principal measurements were glomerular volume and cell nuclei size of proximal and distal convoluted tubules and collecting duct. There were a significant decrease in fetal and placentae weight in treated group with sodium cyclamate (p = 0.004) when compared with control group. In the glomerular volume and cell nuclei size of proximal and distal convoluted tubules and collecting duct of kidneys of rats fetuses treated with sodium cyclamate, the increase was statistically significant. The results showed that the use of sodium cyclamate was effective in the reduction of fetal and placentae weight, and lenght of umbilical cords. There was a significant increase in glomerular volume and in cell nuclei size of proximal and distal convoluted tubules and collecting duct, suggesting nephrotoxicity

El ciclamato es usado como edulcorante no calórico en muchos alimentos y bebidas y es 30 veces más dulce que la sacarosa, sin el gusto amargo de la sacarina. Aparecen en su composición, los productos como ciclamato de sodio, ciclamato de calcio y ácido ciclámico. El objetivo del trabajo fue evaluar los efectos del ciclamato de sodio en riñones de fetos de ratas, considerándose las alteraciones morfométricas en glomérulo, túbulos contornados proximal y distal y conducto coletor. Fueron utilizadas 10 ratas adultas (Rattus norvegicus) variedad Wistar, con peso medio de 238 g, 5 ratas para el grupo control y 5 ratas tratadas con ciclamato de sodio. Entre el 10 y 14 día de la preñez, 5 ratas recibieron una inyección diaria intraperitoneal de 60mg/Kg/día de ciclamato de sodio durante 5 días. En el 20 día, los animales fueron sacrificados y los fetos fijados en solución de Alfac, incluidos en parafina, cortados y teñidos com H-E. El método utilizado fue la morfometría por la técnica cariométrica. Hubo disminución significativa en los pesos de los fetos y de la placenta en el grupo tratado con ciclamato de sodio (p= 0,004) comparado con el grupo control. En el volumen glomerular y tamaño nuclear de las células de los túbulos contornados proximal, distal y conducto colector del riñón fetal de las ratas tratadas con ciclamato de sodio, hubo aumento estadísticamente significativo. Los resultados mostraron que el uso del ciclamato de sodio produjo la reducción del peso de los fetos, placenta y longitud del cordón umbilical. Hubo aumento significativo en el volumen glomerular y en el tamaño nuclear de las células de los túbulos contornados proximal, distal y conducto colector, sugerente de nefrotoxicidad

O ciclamato é usado como adoçante artificial não calórico em diversos alimentos e bebidas, sendo 30 vezes mais doce que a sacarose sem o sabor amargo da sacarina. Aparece na composição dos produtos como ciclamato de sódio, ciclamato de cálcio e ácido ciclâmico. O ciclamato e a ciclohexilamina, seu principal metabólito, atravessam a barreira placentária em humanos e desse modo podem ser expostos ao feto. O rim de ratos pode ser afetado por elevadas doses de ciclamato de sódio. Estudos sobre efeitos do ciclamato de sódio na espécie humana são necessários, pois, além de poder substituir a sacarose - prejudicial em casos de diabetes ou quando o controle e a redução do peso corporal são essenciais para a saúde dos pacientes - não propicia desenvolvimento de cárie dentária.

Cyclamate and cyclohexylamine, its principal metabolite, can cross the human placenta exposing the fetus. It has been demonstrated in rats that the kidney may be adversely affected by high doses of cyclamate. Studies on the effects of sodium cyclamate in the human species are necessary, because in addition to replacing saccharose - harmful in individuals with diabetes or patients in whom weight reduction and control are essential for health - it does not cause dental caries.
