Elements for assistance to patients with hematological malignancies to propose care lines: a scoping review

ABSTRACT Objectives: to identify the elements for assistance to patients with hematological malignancies to propose a care line. Methods: this is a scoping review, anchored in the JBI theoretical framework, with searches carried out in April 2021, in eight electronic databases and 10 repositories of theses and dissertations. Results: the final sample consisted of 93 studies, and the main forms of assistance provided that can support a care line for this public were imaging tests, immunophenotyping, chemotherapy regimens, radiotherapy, infection management, assessment of nutritional status, maintenance of oral function, symptom management and screening for second malignancies. Conclusions: the elaboration of a care line for onco-hematologic patients is necessary, considering the complexity surrounding the diagnosis and treatment of hematologic malignancies, in addition to the difficulties that are imposed in relation to access and continuity of care in the network.
RESUMEN Objetivos: identificar los elementos para la asistencia a pacientes con neoplasias hematológicas para proponer una línea de atención. Métodos: se trata de una revisión de alcance, anclada en el marco teórico del JBI, con búsquedas realizadas en abril de 2021 en ocho bases de datos electrónicas y 10 repositorios de tesis y disertaciones. Resultados: la muestra final estuvo compuesta por 93 estudios, y las principales formas de asistencia brindadas que pueden sustentar una línea de atención a este público fueron pruebas de imagen, inmunofenotipificación, regímenes de quimioterapia, radioterapia, manejo de infecciones, evaluación del estado nutricional, mantenimiento de la función oral, manejo de síntomas y detección de segundas neoplasias malignas. Conclusiones: es necesario el desarrollo de una línea de atención al paciente oncohematológico, dada la complejidad que rodea al diagnóstico y tratamiento de las neoplasias hematológicas, además de las dificultades que se imponen en relación al acceso y continuidad de la atención en una red.

Métodos: trata-se de uma scoping review, ancorada no referencial teórico do JBI, com buscas realizadas em abril de 2021 em oito bases de dados eletrônicas e 10 repositórios de teses e dissertações. Resultados: a amostra final foi composta por 93 estudos, e as principais formas de assistências prestadas que podem embasar uma linha de cuidado para esse público foram exames de imagem, imunofenotipagem, regimes quimioterápicos, radioterapia, gestão de infecções, avaliação do estado nutricional, manutenção da função oral, gerenciamento de sintomas e rastreio para segundas neoplasias. Conclusões: a elaboração de uma linha de cuidados para pacientes onco-hematológicos se faz necessária, tendo em vista a complexidade que cerca o diagnóstico e tratamento das neoplasias hematológicas, além das dificuldades que se impõem em relação ao acesso e continuidade do cuidado em rede.

Abstract Background Self-care in the management of systemic arterial hypertension (SAH) is crucial, and validated instruments can help researchers and health professionals to plan strategies to improve self-care in people with SAH. Objective The main objective of this study was to translate, cross-culturally adapt, and validated the Hypertension Self-Care Profile (HBP-SCP) for Brazilian Portuguese. Methods The translation and cross-cultural adaptation was performed in five phases, and the pre-final version was tested in 30 individuals, native speakers of Portuguese with a diagnosis of hypertension. The final version was administered in 100 individuals. The inclusion criteria were: diagnosis of SAH, characterized by systolic arterial blood pressure ≥ 140 mmHg and/or diastolic arterial blood pressure ≥ 90mmHg, regular use of antihypertensive medications and over 18 years of age. The present study also used two other questionnaires, previously validated for the Brazilian population, to verify the validity of the construct, the Healthy Habits Perception Questionnaire (HHPQ) and the Quality of Life in Hypertension Mini-Questionnaire (MINICHAL-BRASIL). Results During the translation and cross-cultural adaptation phase, there were no disagreements. Adequate reliability — intraclass correlation coefficient (ICC) ≥ 0.89, standard error of measurement (SEM) % ≤ 4.34, minimum detectable change (MDC) % ≤ 12.04 — and internal consistency (Cronbach's alpha ≥ 0.75) were observed. The behavior domain of HBP-SCP obtained significant correlations (p < 0.05) with the self-efficacy domain and HHPQ; the motivation domain with the self-efficacy domain; and the self-efficacy domain with the somatic manifestation domain of MINICHAL-BRASIL. No ceiling and floor effects were observed. Conclusions The Brazilian Portuguese version of the HBP-SCP has adequate psychometric properties, according to the best scientific recommendations.

Resumo Coxiella burnetii é um agente zoonótico transmitido principalmente por pequenos ruminantes. No Brasil, a doença foi classificada como de notificação compulsória desde 2013, quando casos humanos foram relatados. O objetivo deste estudo foi identificar os fatores de risco associados à presença de anticorpos anti-Coxiella burnetii em caprinos e ovinos em uma região semiárida do Nordeste do Brasil. Este estudo envolveu um inquérito sorológico de 412 caprinos e 403 ovinos em fazendas do município de Petrolina, no estado de Pernambuco. Os soros foram examinados pela Reação de Imunofluorescência Indireta (RIFI) contra antígenos de C. burnetii . Informações sobre variáveis de manejo (variáveis independentes) que poderiam estar associadas à presença do microrganismo (variáveis dependentes) foram obtidas do proprietário de cada fazenda. Foi determinado que 2,2% (9/412) dos caprinos e 2,1% (9/403) dos ovinos tinham anticorpos reativos a C. burnetii. A presença de anticorpos anti-C. burnetii foram associados com a área seca do Sequeiro (região no norte do município de Petrolina) (P = 0,025), ovinos machos (P = 0,020) e criação intensiva de caprinos (P = 0,005). Este estudo, portanto, observou a presença de anticorpos anti-C. burnetii em pequenos ruminantes, confirmando pela primeira vez que este agente pode estar circulando em rebanhos caprinos no bioma Caatinga, semiárido do Brasil.
Abstract Coxiella burnetii is a zoonotic agent transmitted mainly by small ruminants. In Brazil the disease has been classified as a notifiable disease since 2013, when human cases were reported. This study aimed to identify risk factors associated with the presence of anti- Coxiella burnetii antibodies in goats and sheep in a semiarid region of Northeastern Brazil. Sera of 412 goats and 403 sheep from municipality of Petrolina, Pernambuco, were examined by the Indirect Fluorescent Antibody Test (IFAT) against antigens of C. burnetii. Information about management variables (independent variables) that could be associated with the presence of the microorganism (dependent variables) were obtained from the supervisor of each farm. It was determined that 2.2% (9/412) of the goats and 2.1% (9/403) of the sheep had antibodies reactive to C. burnetii. The presence of anti-C. burnetii antibodies was associated with the dry area of the Sequeiro (a region in the northern part of the municipality of Petrolina) (P = 0.025), male sheep (P = 0.020), and intensive goat breeding (P = 0.005). This study therefore showed the presence of anti-C. burnetii antibodies in goat and sheep, confirming for the first time that this agent is likely circulating among goat herds in the Caatinga Biome, semi-arid of Brazil.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Caries experience in young children with congenital heart disease in a developing country

Oral care is frequently suboptimal in children from developing countries, especially those suffering from severe systemic diseases. The aim of the present study was to analyze the oral epidemiological profile of 3-to-5-year-old children with congenital heart disease. Dental and medical records of children evaluated at the Dental Service of the National Institute of Cardiology, Rio de Janeiro, Brazil, were reviewed. Caries experience was reported using the dmft index. Negative behavior towards dental management was recorded. The sample consisted of 144 children aged 4.41 ± 0.95 years. The mean dmft value was 5.4 ± 4.9, and 80.5% had at least one caries lesion. Dmft index was greater in the presence of cyanotic cardiac disease and in children with negative behavior. An increase in the “missing” component of the dmft index was also found in children using medicine on a daily basis. A higher caries experience was associated with children whose fathers had only an elementary education. In conclusion, children with congenital heart disease had high levels of caries experience at a young age. Cyanosis, negative behavior, daily use of medicine, one-parent family and the educational level of fathers seem to influence caries experience in children with congenital cardiac disease.

Salvaguarda do acervo documental da Reserva Florestal Ducke - Manaus-AM: registros de uma experiência

A Reserva Florestal Ducke é uma das mais importantes bases de pesquisa do Instituto Nacional de Pesquisas da Amazônia (Inpa). Embora a reserva se destaque no meio científico, seus documentos produzidos e acumulados ao longo de sua existência encontravam-se desorganizados. Com a realização de intensa coleta documental, os arquivos foram resgatados e salvaguardados. No presente artigo, relatam-se os resultados desta experiência. A salvaguarda do acervo foi realizada em três etapas: primeira - recolha dos documentos; segunda - digitalização do material recolhido e terceira - compilação de instrumentos de pesquisa. Com o material coletado e organizado, criou-se uma edição de fontes documentais contendo documentos manuscritos e impressos e dois catálogos, sendo um documental e outro bibliográfico. O acervo da Reserva Ducke oferece múltiplas possibilidades de estudos temáticos com base no conteúdo dos documentos. Neste sentido, foram realizados alguns apontamentos temáticos das peças documentais, entre os quais se destacam: Adolfo Ducke, criação da reserva, infraestrutura, as pesquisas, ocupações ilegais, degradação versus preservação e jardim botânico.
The Ducke Forest Reserve (Reserva Florestal Ducke) is one of the most important research bases of the National Research Institute of Amazonia (Instituto Nacional e Pesuisas da Amazônia - INPA). Although this reserve stands out in the scientific milieu, the documents produced and accumulated during its existence were disorganized. By undertaking an intense collection of documents, these archives were rescued and safeguarded. In the present article, the results of this experience are related. The safeguarding of the document patrimony of the Ducke Reserve was undertaken in three stages: first - collecting documents; second - putting the material collected into digital notation; and third - the compilation of research instruments. Having collected and organized the material, a source of edition was carried out as well as two documentary and bibliographic catalogues The documentary patrimony of the Ducke Reserve presented multiple possibilities of thematic studies based on the contents of the documents. In this sense, some thematic annotations were made about the documents, emphasizing the following: Adolfo Ducke, creation of the reserve, infrastructure, research, illegal occupation, degradation versus preservation of the botanical garden. Finally, the results of this research are expected to disseminate and facilitate public access to the documents on Ducke Reserve and consequently reveal hidden and forgotten facts along its existence.

Foi investigada a influência da via de inoculação sobre o estabelecimento e a evolução da leptospirose em hamsters (Mesocricetus auratus) experimentalmente infectados com Leptospira interrogans sorovar pomona. As vias de inoculação ensaiadas foram: intraperitoneal, subcutânea, oral, conjuntival e escarificação cutânea. O inóculo infeccioso foi constituído por uma cultura em meio de Fletcher, com 20 a 30 leptospiras por campo microscópio no aumento de 200 vezes. Os animais controle foram inoculados apenas com meio de Fletcher. Foram colhidas amostras de soro sanguíneo e fragmentos de rins na fase agônica da doença ou no 21º dia pós-infecção, quando todos os animais foram sacrificados. Para a pesquisa de leptospiras, foi feito o exame direto com microscopia óptica em campo escuro e cultivo em meio de Fletcher, pela técnica das diluições seriadas. A detecção de aglutininas anti-leptospiras foi realizada pela técnica de soroaglutinação microscópica. A instalação e evolução da leptospirose foram afetadas pela via de inoculação. A via oral foi a menos efetiva em estabelecer a infecção. Não foi observada associação estatística entre a freqüência de portadores e a via de inoculação.
The influence of the route of inoculation on the development of the leptospiral infection was investigated in 60 hamsters (Mesocricetus auratos) experimentally infected with Leptospira interrogans serovar pomona. The routes of inoculation were intraperitoneal, subcutaneous, oral, conjuntival and scrapped skin. All animals received the leptospiral inoculum cultivated in Fletcher's medium, with 20 to 30 leptospires per field under 200x magnification. The controls were inoculated only with Fletcher's medium. Samples of blood sera and the kidney fragments were collected in the agonic stage of the disease or at 21st experimental day, when all survivors were killed. The antibodies detection was done by the microscopic agglutination test and the bacteria detection by the dark field microscopic examination and culture in Fletcher's medium, by the tenfold dilutions technique. The presence and development of disease was affected by the route of infection. The oral route was the less effective for promoting the disease. There was no association between the frequency of carrier state and the route of infection.
