Obstetric Paralysis: Evaluation of the Sever-L'Episcopo Technique Modified by Hoffer

Resumo Avaliar os resultados de uma série de 28 casos de paralisia obstétrica alta tratadas com a técnica Sever-L'Episcopo modificada por Hoffer, entre 2003 e 2016. As crianças (idade média, quatro anos e sete meses) com contratura em adução e rotação interna do ombro sem deformidades ósseas secundárias (Mallet classe II) foram submetidas ao alongamento do músculo peitoral e tenotomia do músculo subescapular associada à transferência do latissimus dorsi e músculo redondo maior para o músculo infraespinhal, movendo-se para a função de rotadores externos e elevadores. O seguimento médio foi de 3 anos e 10 meses. Ao final do estudo, 24 pacientes obtiveram excelentes escores de avaliação funcional, principalmente de abdução e rotação externa, passando de Mallet classe II para classe IV. Quatro pacientes ainda demonstraram algum grau de limitação de movimento global, passando da classe II para a classe III. Independentemente do ganho funcional final, todos os pacientes foram capazes de realizar tarefas que antes eram difíceis. Os dados deste estudo sugerem que a cirurgia de Hoffer é um método eficaz no tratamento das sequelas de paralisia obstétrica alta sem deformidades ósseas secundárias.
Abstract Evaluate the results of a series of 28 cases of high obstetric paralysis treated with the Sever-L'Episcopo technique modified by Hoffer, between 2003 and 2016. Children (mean age, four years and seven months) with adduction contracture and internal rotation of the shoulder without secondary bone deformities (Mallet class II) underwent lengthening of the pectoralis major muscle and tenotomy of the subscapularis muscle associated with transfer of the latissimus dorsi and teres major muscle to the infraspinatus muscle, moving to the function of external rotators and elevators. The mean follow-up was three years and 10 months. At the end of the study, 24 patients achieved excellent functional assessment scores, mainly of the abduction and external rotation, passing from Mallet class II to class IV. Four patients still demonstrated some degree of global movement limitation, passing from class II to class III. Regardless of the final functional gain, all patients were able to perform tasks that were previously difficult. The data from this study suggest that Hoffer's surgery is an effective method in the treatment of the sequelae of high obstetric paralysis without secondary bone deformities.

RESUMO Objetivou-se avaliar as características físicas e o comportamento fisiológico de sementes de A. oleracea durante a embebição e germinação, sob diferentes temperaturas e fotoperíodos, bem como determinar a duração para a execução do teste de germinação. Para isso, realizaram-se a biometria das sementes (comprimento, largura e espessura) de 100 sementes e peso de mil sementes. Assim como, as descrições das estruturas externas das sementes e plântulas e o monitoramento da curva de embebição. Em seguida, realizou-se o teste de germinação sob temperaturas de 20, 25, 30, 35, 20-30, e 25-35 ºC, submetidas aos fotoperíodos de luz e escuro constantes e alternado (12 h), arranjados em fatorial 6 x 3, com quatro repetições de 50 sementes. As sementes foram semeadas em caixas de plástico transparente, tendo como substrato o papel mata-borrão e colocadas para germinar em Biochemical Oxygen Demand (B.O.D), sendo avaliadas durante seis dias. As variáveis analisadas foram: germinação, índice de velocidade de germinação, comprimento e massa seca de plântulas. O padrão de embebição, foi realizado com 4 repetições de 100 sementes, a 25 ºC, e avaliado até o surgimento de 50% de protrusão radicular. As sementes de A. oleracea apresentam características físicas uniformes e o modelo trifásico como padrão de absorção de água. As temperaturas de 25 e 30 °C e alternada de 20-30 °C, independente do fotoperíodo, proporcionaram maiores valores de germinação. As avaliações do teste de germinação de A. oleracea podem ser realizadas no quarto e sexto dia após a semeadura, como primeira e última contagem, respectivamente.
ABSTRACT The objective was to evaluate the physical characteristics and physiological behavior of A. oleracea seeds during imbibition and germination, under different temperatures and photoperiods, as well as to determine the duration of the germination test. For this, biometric parameters (length, width and thickness) were measured in 100 seeds and thousand-seed weight was determined. In addition, the external structures of seeds and seedlings were described and the imbibition curve was monitored. Subsequently, the germination test was conducted at temperatures of 20, 25, 30, 35, 20-30 and 25-35 ºC, under constant and alternating light and dark photoperiods (12 h), arranged in 6 x 3 factorial scheme, with four replicates of 50 seeds. Seeds were sown in transparent plastic boxes with blotting paper as substrate, germinated in Biochemical Oxygen Demand (B.O.D) incubator, and evaluated for six days. The following variables were analyzed: germination, germination speed index, seedling length and seedling dry mass. The imbibition pattern test was conducted at 25 ºC with 4 replicates of 100 seeds, evaluated until 50% root protrusion occurred. A. oleracea seeds exhibit uniform physical characteristics and the three-phase model as a water absorption pattern. Constant temperatures of 25 and 30 °C and alternating temperatures of 20-30 °C, regardless of the photoperiod, led to higher germination values. For A. oleracea, the germination test evaluations can be performed on the fourth and sixth day after sowing, as first and last counts, respectively.

RESUMO O lançamento de águas residuárias industriais em corpos aquáticos, sem o devido tratamento, pode causar danos severos ao meio ambiente e à saúde humana. O objetivo deste estudo foi realizar a secagem da casca de laranja cultivar Pêra (Citrus sinensis L. Osbeck) e avaliar a viabilidade do seu uso como biossorvente para remoção de um corante direto. A secagem foi realizada em estufa com circulação de ar nas temperaturas de 60 e 80 ºC. Os dados de umidade em função do tempo foram ajustados aos modelos matemáticos de Page, Henderson e Pabis, Logarítmico, Midilli e exponencial de dois termos. A caracterização do material foi realizada a partir da microscopia eletrônica de varredura, do ponto de carga zero, e da espectroscopia na região do infravermelho. No estudo da adsorção realizou-se um planejamento fatorial 24 completo, analisando-se a influência da massa, concentração inicial, pH da solução e tempo de contato, na quantidade adsorvida (qt) e no percentual de remoção do corante (R%). Na secagem, o modelo exponencial de dois termos apresentou melhor ajuste aos dados experimentais. A caracterização do material apontou que o adsorvente apresenta ponto de carga zero de 3,5 e estrutura porosa, e as análises de infravermelho indicaram a presença de grupos carboxílicos e hidroxilas. Na adsorção, a quantidade adsorvida do corante aumentou em condições de menor pH e massa de biossorvente, maior concentração inicial e tempo de contato. O percentual de remoção do corante aumenta com maior massa de biossorvente. O biossorvente utilizado é um resíduo promissor para a adsorção do corante bordô-16.
ABSTRACT The disposal of industrial wastewater into aquatic bodies without proper treatment can cause severe damage to the environment and human health. The objective of this study was to perform the drying of the sweet orange (Citrus sinensis L. Osbeck) peel cultivar Pêra and evaluate the viability of its use as biosorbent in the removal of a direct dye. Drying was carried out in an oven with air circulation at temperatures of 60 and 80 ºC. The mathematical models of Page, Henderson and Pabis, Logarithmic, Midilli and Two-term exponential were fitted to the moisture data as a function of time. The material was characterized by scanning electron microscopy, point of zero charge, and infrared spectroscopy. In the adsorption study, a complete 24 factorial design was used to analyze the influence of mass, initial concentration, solution pH and contact time on adsorbed quantity (qt) and removal percentage of the dye (R%). In the drying, the two-term exponential model fitted best to the experimental data. The characterization of the material indicated that the adsorbent has zero charge point of 3.5 and porous structure, and the infrared analysis indicated the presence of carboxylic and hydroxyl groups. In the adsorption, the adsorbed quantity of the dye increased under conditions of lower pH and biosorbent mass and higher initial concentration and contact time. The removal percentage of dye increases with higher biosorbent mass. The biosorbent used is a promising waste for the adsorption of the burgundy-16 dye.

Zika virus (ZIKV) is a mosquito-borne pathogen which is a current global public health concern. There are currently no approved vaccines or antivirals against ZIKV infection. Taking into account that naphthoquinones have shown promising antiviral activity, the aim of this study was to describe the screening of two bis-naphthoquinones series against ZIKV. Twenty seven compounds were evaluated against ZIKV using Vero cells. The findings showed that among the compounds analyzed four were promising. Compound 3,3’-((2-nitrophenyl)methylene)bis(2-hydroxynaphthalene-1,4-dione) containing the nitro group at the ortho position showed the best selectivity index, followed by compound 3,3’-(4-chlorophenylmethylene)bis(naphthalene-1,2,4-triyl triacetate) with the chlorophenylmethylene radical. These results demonstrate that these bis-naphthoquinones are largely effective in inhibiting the replication of ZIKV.

Introdução: O impacto extra-articular do quadril envolve um grupo de lesões reconhecidas apenas recentemente, comumente subdiagnosticadas e pouco compreendidas por muitos especialistas da área ortopédica. Porém, são lesões capazes de levar a dor e declínio funcional importante nos pacientes acometidos. O correto diagnóstico dessas condições é fundamental. Envolve a necessidade de um conhecimento adequado de sua patogênese e avaliação clínica através de manobras do exame físico específicas, assim como correta interpretação dos exames de imagem. Objetivo: Atualizar profissionais médicos, fisioterapeutas, educadores físicos ou quaisquer outros que trabalhem em contato direto com os pacientes acometidos. Material e métodos: Foi realizado uma revisão da literatura nas bases de dados PubMed e Birene/Lilacs a respeito da anatomia, patogênese, achados clínicos, exames de imagem e tratamento das principais causas descritas de impacto extra-articular do quadril. Resultados: As síndromes de impacto extra-articular mais citadas na literatura são os impactos isquiofemoral, subespinhal, pélvico trocantérico e o do iliopsoas. Essas síndromes são condições dinâmicas e o diagnóstico deve ser feito observando a presença de alterações anatômicas, exame clínico positivo e confirmação por exame de imagem ou achado intra-operatório. Por serem lesões com baixa prevalência relatada, existem poucas evidências a respeito dessas condições e mais estudos são necessários. Conclusão: O conhecimento das síndromes de impacto extra-articular do quadril é essencial na avaliação de pacientes com dor no quadril e deve fazer parte do diagnóstico diferencial.
Introduction: The extra-articular hip impingement involves a group of recently recognized lesions, commonly underdiagnosed and poorly understood by many orthopedic specialists. However, these lesions are capable of leading to pain and important functional decline in the affected patients. The correct diagnosis of these conditions is primal. It involves the need for an adequate knowledge of its pathogenesis and clinical evaluation through specific physical examination maneuvers, as well as correct interpretation of the imaging exams. Objective: To update medical professionals, physiotherapists, physical educators or any others who work in direct contact with the affected patients. Material and methods: A review of the literature was conducted on the PubMed and Birene /Lilacs databases on regard of anatomy, pathogenesis, clinical findings, imaging and treatment of the main causes of extra-articular hip impingement. Results: The most cited extra-articular impingement syndromes in the literature are the ischiofemoral, subspinal, trochanteric and iliopsoas impingements. These syndromes are dynamic conditions and the diagnosis must be done observing the presence of anatomical abnormalities, positive clinical examination and confirmation by imaging exams or intraoperative finding. Because they are lesions with a low prevalence reported, there is little evidence regarding these conditions and more studies are needed. Conclusion: Knowledge of extra-articular hip impingement syndromes is essential in the evaluation of patients with hip pain and should be part of the differential diagnosis.

Cesarean sections in Brazil: will they ever stop increasing?

OBJECTIVE: To describe trends, geographic distribution, and risk factors for cesarean deliveries in Brazil in 2000-2011, and to determine if efforts to curtail rates have had a measurable impact. METHODS: This was an observational study using nationwide information from the Department of Informatics of the Unified Health System (DATASUS). Individual level analyses were based on data regarding maternal education, age, parity, and skin color. Ecological analyses at the level of 431 health districts investigated the relationships with health facility density and poverty level. RESULTS: Cesarean rates increased markedly, from 37.9% in 2000 to 53.9% in 2011. Preliminary results from 2012 showed a rate of 55.8%, with the richest geographic areas showing the highest rates. Rates at the municipal level varied from 9%-96%. Cesareans were more common in women with higher education, white skin color, older age, and in primi- paras. In the ecological analyses, the number of health facilities per 1 000 population was strongly and positively correlated with cesarean rates, with an increase of 16.1 percentage points (95% Confidence Interval [95%CI] = 4.3-17.8) for each facility. An increase of 1 percentage point in the poverty rate was associated with a decline of 0.5 percentage point in cesarean rates (95%CI = 0.5-0.6). CONCLUSIONS: The strong associations with maternal education and health facility density suggest that the vast majority of cesareans are not medically indicated. A number of policies and programs have been launched to counteract this trend, but have had virtually no impact.
OBJETIVO: Describir las tendencias, la distribución geográfica, y los factores de riesgo de parto por cesárea en el Brasil durante el período del 2000 al 2011, y determinar si las iniciativas dirigidas a reducir las tasas de cesáreas han tenido una repercusión cuantificable. MÉTODOS: Se trata de un estudio de observación que utilizó información a escala nacional del Departamento de Informática del Sistema Unificado de Salud (DATASUS). Los análisis a nivel individual se basaron en datos sobre el nivel de formación materna, la edad, la paridad y el color de la piel. Se investigaron las relaciones con la densidad de establecimientos de salud y el nivel de pobreza mediante análisis ecológicos a nivel de 431 distritos de salud. RESULTADOS: Las tasas de cesáreas aumentaron notablemente, de 37,9% en el 2000 a 53,9% en el 2011. Los resultados preliminares del 2012 mostraron una tasa de 55,8%, con tasas más elevadas en las zonas geográficas más ricas. Las tasas a escala municipal variaron de 9 a 96%. Los partos por cesárea fueron más frecuentes en las mujeres blancas, en las que tenían un mayor nivel de formación, en las de mayor edad y en las primíparas. En los análisis ecológicos, el número de establecimientos de salud por 1 000 habitantes se correlacionó intensa y positivamente con la tasa de cesáreas, con un incremento de 16,1 puntos porcentuales (intervalo de confianza (IC) de 95% = 4,3-17,8) para cada establecimiento. Un aumento de un punto porcentual en la tasa de pobreza se asociaba con una disminución de medio punto porcentual en la tasa de cesáreas (IC de 95% = 0,5-0,6). CONCLUSIONES: Las intensas asociaciones con el nivel de formación materna y la densidad de establecimientos de salud indican que la mayor parte de las cesáreas no están indicadas médicamente. Se han puesto en marcha diversos programas y políticas dirigidos a contrarrestar esta tendencia, pero prácticamente no han tenido ninguna repercusión.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

A aplicação de K está muitas vezes relacionada ao processo de rustificação, que tem por objetivo adaptar a muda às condições de plantio no campo. Neste sentido, este experimento foi conduzido visando avaliar o efeito do K na qualidade de mudas de três clones híbridos de Eucalyptus (E. grandis x E. urophylla). O experimento foi instalado no viveiro de mudas da empresa CENIBRA. As doses de potássio utilizadas foram: 0, 249, 747, 1494 e 2988 mg L-1. Aos 87 e 88 dias, foram avaliadas a altura da parte aérea, incremento em diâmetro de colo, relação altura da parte aérea/diâmetro de colo, concentração de K nas folhas e a relação N/K nas folhas. A aplicação de K na fase de rustificação aumentou, até determinada dose, o diâmetro de colo para os três clones avaliados, mas não se verificou influência do K na altura. A ausência de K resultou nos maiores valores da relação altura: diâmetro de colo para os três clones avaliados, ao passo que em mudas onde o K foi aplicado, esta relação apresentou valores menores e semelhantes. A aplicação de doses crescentes de K aumentou as concentrações foliares do nutriente. Todos os tratamentos apresentaram valores da relação N/K dentro do considerado adequado, exceto na maior dose de K aplicada. Conclui-se que, durante a fase de rustificação, a adubação potássica é recomendável, sendo indicada a dose de 249 mg L-1 de K por proporcionar os maiores valores em incremento de diâmetro de colo.
The K is often related to the hardening process, in order to adapt the seedling to the field planting conditions. Thus, the objective of this experiment was evaluate the effect of K on the quality of seedlings of three hybrids clones of Eucalyptus (E. grandis x E. urophylla). The experiment was carried out in the seedling nursery of the CENIBRA enterprise. The doses of K was: 0, 249, 747, 1494 and 2988 mg L-1. With 87 and 88 days, were avaluated: the shoot height, collar diameter increment, ratio shoot height / collar diameter, K concentration in leaves and N/K ratio in leaves. The K application in the hardening phase increased, up to a given dose, the collar diameter for the three clones evaluated, but the height was not influenced. The absence of K resulted in the greatest values of the ration height / collar diameter for the three clones evaluated, while on the seedlings that received K, the rate showed smaller and similar values. The application with increasing doses of K resulted in greater leaf concentrations of this nutrient. All the treatment showed values of N/K ratio within what is considered appropriate, except for the greatest level of K. It can be concluded that, during the hardening phase, the potassic fertilization is recommended, and being indicated the dose of 249 mg L-1 of K because provides the best values of collar diameter increment.

Kalliapseudes schubartii (Crustacea: Tanaidacea) é um invertebrado tubícola que habita fundos moles de regiões estuarinas e que distribui-se ao longo da costa sudeste e sul do Brasil e na costa uruguaia. Este tanaidaceo é comedor de depósitos e filtrador, e é presa de vários consumidores secundários. Taxas de mortalidade sazonal foram examinadas através de amostras feitas durante um ano num local da região estuarina da Lagoa dos Patos. Os resultados não demonstraram diferenças significativas entre as taxas de mortalidade nas diferentes estações do ano, o que sugere que populações de K. schubartii são fortemente predadas ao longo de todo o ano e que a premissa de maior mortalidade no verão/outono pode não ser correta em fundos de águas rasas não vegetados.
Studies were carried iut on seasonal mortality by taking samples of Kalliapseudes schubartii (Crustacea: Tanaidacea) for a year in the estuarine region of the Lagoa dos Patos (southern Brazil). Results demonstrated no significant difference in seasonal mortality rates, which suggest that populations of K. schubartii was heavily preyed on year round and that an assumption of a significantly higher mortality in summer/autumn than in spring/winter might not be held in unvegetated soft bottoms of the estuarine regions of the Lagoa dos Patos.
