FUNDAMENTO: Considerando o alto número de pacientes infectados por HIV em uso de antirretrovirais, evoluindo com alteração da função endotelial e aterotrombose, levando ao alto custo médico social, é importante identificar mecanismos fisiopatológicos envolvidos com a função endotelial em portadores de HIV, para que, precocemente, possamos intervir e evitar a progressão da doença. OBJETIVO: Avaliar a função endotelial pela vasodilatação endotélio dependente e independente em pacientes HIV positivo e em grupo controle. MÉTODOS: O estudo avaliou 27 pacientes HIV positivo e 16 do grupo controle. A avaliação da função endotelial foi realizada por meio da vasodilatação da artéria braquial endotélio dependente (hiperemia reativa) e independente (nitroglicerina SL). RESULTADOS: Pacientes HIV positivo em uso de inibidor de protease (IP) apresentaram vasodilatação endotélio independente significativamente menor que os subgrupos HIV negativo (p = 0,020) e HIV positivo sem uso de IP (p = 0,034). A variação do diâmetro da artéria braquial durante hiperemia reativa não apresentou significância estatística em qualquer subgrupo. A análise de regressão linear múltipla mostrou que apenas o IP estava associado ao delta relativo da reatividade braquial pelo vasodilatador, nos pacientes HIV positivo, aos 60 e 90 segundos. CONCLUSÃO: Os pacientes HIV positivo em uso de IP apresentam disfunção endotélio independente quando comparados a pacientes HIV positivo que não fazem uso de IP e a um grupo controle.

BACKGROUND: A great number of HIV-infected patients using antiretroviral drugs develop endothelial dysfunction and atherothrombosis, which lead to a high medical and social burden. Thus, it is important to identify pathophysiological mechanisms involved with the endothelial function in these patients, so that early intervention can be made to avoid disease progression. OBJECTIVE: To evaluate endothelial function using endothelium-dependent and independent vasodilation in HIV-positive patients and in a control group. METHODS: A total of 27 HIV-positive patients and 16 controls were evaluated. Endothelium-dependent (reactive hyperemia) and independent (SL nitroglycerine) vasodilation of the brachial artery was used to evaluate the endothelial function. RESULTS: HIV-positive patients receiving protease inhibitors (PI) showed significantly lower endothelium-independent vasodilation than the HIV-negative (p=0.020) and HIV-positive without PI (p=0.034) subgroups. The change in brachial artery diameter during active hyperemia was not statistically significant in any subgroup. Multiple linear regression analysis showed that only PI was associated with the relative delta of brachial reactivity to vasodilator in HIV-positive patients at 60s and 90s. CONCLUSION: HIV-positive patients receiving PI presented endothelium-independent dysfunction when compared to HIV-positive patients not receiving PI and to the control group.

FUNDAMENTO: Considerando el alto número de pacientes infectados por VIH en uso de antirretrovirales, evolucionando con la alteración de la función endotelial y aterotrombosis, llevando al alto coste médico social, es importante identificar mecanismos fisiopatológicos implicados en la función endotelial en portadores de VIH, para que, precozmente, podamos intervenir y evitar la progresión de la enfermedad. OBJETIVO: Evaluar la función endotelial por la vasodilatación endotelio dependiente e independiente en pacientes VIH positivo y en grupo control. MÉTODOS: El estudio evaluó a 27 pacientes VIH positivo y a 16 del grupo control. La evaluación de la función endotelial se llevó a cabo mediante la vasodilatación de la arteria braquial endotelio dependiente (hiperemia reactiva) e independiente (nitroglicerina SL). RESULTADOS: Pacientes VIH positivo en uso de inhibidor de proteasa (IP) presentaron vasodilatación endotelio independiente significativamente menor que los subgrupos VIH negativo (p = 0,020) y VIH positivo sin uso de IP (p = 0,034). La variación del diámetro de la arteria braquial durante hiperemia reactiva no presentó significancia estadística en cualquier subgrupo. El análisis de regresión lineal múltiple evidenció que apenas el IP estaba asociado al delta relativo de la reactividad braquial por el vasodilatador, en los pacientes VIH positivo, a los 60 y 90 segundos. CONCLUSIÓN: Los pacientes VIH positivo en uso de IP presentan disfunción endotelio independiente cuando comparados a pacientes VIH positivo que no están en tratamiento con IP y a un grupo control.

O parvovírus B19 replica-se em precursores eritróides. Não há geralmente manifestações hematológicas aparentes. Entretanto, nas altas taxas de reposição de eritrócitos, como as que ocorrem na anemia falciforme ou no feto, a infecção pode resultar, respectivamente, em aplasia transitória e hidropisia fetal. Em pacientes com AIDS, a infecção persistente pode resultar em anemia crônica. Já num paciente HIV-positivo sem AIDS, a infecção evolui como uma doença exantemática leve. Duas descrições exemplificam estas diferentes modalidades de apresentação clínica. Na primeira, um paciente com AIDS apresentou aplasia medular que respondeu a imunoglobulina. Na segunda, um paciente HIV-positivo sem AIDS apresentou um exantema morbiliforme, que cedeu sem tratamento. Diagnosticar a anemia crônica por B19 diminui as preocupações infundadas sobre anemia por drogas, evita que o paciente seja submetido a manobras diagnósticas invasivas, e impede que o paciente dissemine a infecção. Em pacientes com AIDS acometidos de aplasia eritrocítica pura, está indicada a pesquisa de DNA do parvovírus B19 no soro ou na medula óssea.

Human parvovirus B19 replicates in erythrocyte precursors. Usually, there are no apparent hematological manifestations. However, in individuals with high erythrocyte turnover, as in patients with sickle-cell disease and in the fetus, the infection may lead to severe transient aplasia and hydrops fetalis, respectively. In AIDS patients, persistent infection may result in chronic anemia. By contrast, in HIV-positive patients without AIDS the infection evolves as a mild exanthematous disease. Two clinical descriptions exemplify these forms of presentation. In the first, an AIDS patient presented with bone marrow failure that responded to immunoglobulin. In the second, an HIV-positive patient without AIDS had a morbilliform rash, and needed no treatment. Knowing that an AIDS patient has chronic B19 anemia lessens concern about drug anemia; protects the patient from invasive diagnostic maneuvers; and prevents the patient from disseminating the infection. In AIDS patients with pure red cell aplasia, a search for parvovirus B19 DNA in the serum or in the bone marrow is warranted.

O parvovírus B19 infecta predominantemente células eritróides, causando inibição transitória da eritropoiese. Pacientes imunocomprometidos podem ser incapazes de produzir anticorpos neutralizantes, evoluindo com grave anemia crônica. Estudos epidemiológicos da população de Niterói mostraram que a infecção ocorre periodicamente no final da primavera e no verão. Descrevem-se 55 pacientes infectados pelo HIV atendidos num ambulatório de doenças infecciosas nesta cidade num período de cinco meses, no qual a circulação do parvovírus B19 foi documentada. Todos os pacientes estavam sob terapia anti-retroviral. Não se encontrou IgM anti-B19, mas notou-se uma prevalência alta de IgG anti-B19 (91%). Em seis pacientes verificou-se a presença de DNA do B19 por hibridização em dot-blot, o que não se confirmou por PCR. Nenhum destes seis pacientes tinha anemia, e apenas um tinha células CD4 abaixo de 200 x 10(7)/L. Conclui-se que infecção persistente causando anemia é um achado infreqüente em nossos pacientes HIV positivos sob terapia medicamentosa.

Parvovirus B19 infects predominantly erythroid cells, leading to transient inhibition of erythropoiesis. Immunocompromised patients may be unable to produce neutralizing antibodies and may develop severe chronic anemia. Epidemiological studies done on Niterói population showed that B19 infection occurs periodically in late spring and summer. We report a study from 55 HIV infected patients attending an infectious diseases outpatient clinic in this city during a 5-month period in which B19 circulation was well documented. All patients were under anti-retroviral therapy. No anti-B19 IgM was found, but a high prevalence of IgG anti-B19 (91%) was observed. In six patients, B19 DNA was found by dot-blot hybridization techniques, but this was not confirmed by PCR. None of these 6 patients manifested anemia and only one had CD4 cell count below 200 x 10(7)/L. We conclude that persistent infection causing anemia is an infrequent finding in our HIV positive patients under drug therapy.
