Abstract Introduction The preservation of bone mass in elderly women is associated with better levels of practice of systematic physical exercises. Aerobic training combined with blood flow restriction seems to be a new alternative that determines this process, but knowledge gaps are still observed when referring to exercise associated with blood flow restriction (BFR) and adaptations on bone variables. Objective To analyze the chronic effects of aerobic training with and without BFR on bone mineral density and bone biomarker osteocalcin concentrations in older women. Methods Thirty women were randomized into the following groups: walking on a treadmill at low intensity with BFR; moderate treadmill walking with no BFR; only BFR (no exercise) for 20 minutes, twice a week, for 24 weeks. Bone mineral density was measured before and 24 weeks after intervention. Blood serum osteocalcin concentrations were measured before, 12 and 24 weeks after intervention. Results There were no differences between groups in bone mineral density (femoral neck, p = 0.31; total femur, p = 0.17; lumbar spin, p = 0.06) and osteocalcine (W(2) = 0.27; p = 0.87) ouctomes after 24 weeks of intervention. Conclusion There was no difference between walking training, blood flow restriction only, or walking+blood flow restriction on bone mineral density and osteocalcin concentrations after 24-weeks of intervention in older women with osteopenia/osteoporosis.

Resumo Introdução A preservação da massa óssea em mulheres idosas está associada a melhores níveis de prática de exercícios físicos sistemáticos. O treinamento aeróbico combinado com restrição de fluxo sanguíneo (RFS) parece ser uma nova alternativa que determina esse processo, mas ainda são observadas lacunas de conhecimento quando se refere ao exercício associado à RFS e adaptações nas variáveis ósseas. Objetivo Analisar os efeitos crônicos do treinamento aeróbico com e sem RFS na densidade mineral óssea e nas concentrações do biomarcador ósseo osteocalcina em mulheres idosas. Métodos Trinta mulheres foram randomizadas nos seguintes grupos: caminhada em esteira de baixa intensidade com RFS; caminhada moderada em esteira sem RFS; apenas RFS (sem exercícios) por 20 minutos, duas vezes por semana, durante 24 semanas. A densidade mineral óssea foi medida antes e 24 semanas após a intervenção. As concentrações séricas de osteocalcina no sangue foram medidas antes, 12 e 24 semanas após a intervenção. Resultados Não houve diferenças entre os grupos na densidade mineral óssea (colo do fêmur, p = 0,31; fêmur total, p = 0,17; giro lombar, p = 0,06) e osteocalcina (W(2) = 0,27; p = 0,87) após 24 semanas de intervenção. Conclusão Não houve diferença entre treinamento de caminhada, apenas restrição de fluxo sanguíneo ou caminhada + restrição de fluxo sanguíneo na densidade mineral óssea e nas concentrações de osteocalcina após 24 semanas de intervenção em mulheres idosas com osteopenia/osteoporose.

ABSTRACT The aim of the study was to systematically review the effect of resistance training with blood flow restriction on muscle strength and functional capacity of clinical populations. This research used SCOPUS, WEB OF SCIENCE and MEDLINE/PubMed databases from the first records until November 2021 and in English. The terms (“blood flow restriction” or “vascular occlusion” or “kaatsu training” and “low intensity”) and (“strength training” or “resistance training” or “strength”) and (“clinical populations” or “elderly” or “old” or “hypertension” or “diabetes” or “myositis” or “obesity” and “chronic diseases”) and (“functional capacity” or “functionality” or “muscle function”) were used. Clinical trials (randomised and non-randomised) were included when compared to high-intensity resistance training, low-intensity resistance training, low-intensity resistance training with blood flow restriction and a control group without physical exercise. The quality of the evidence was assessed using the Testex scale. During the research, 122 articles were pre-selected and analysed, and at the end of the selection, nine articles met all the inclusion criteria and established specifications. We conclude that resistance training associated with blood flow restriction has been an effective and tolerable alternative in improving muscle strength and functional capacity and, therefore, a potential tool for the clinical population.

RESUMO O objetivo do estudo foi revisar sistematicamente o efeito do treinamento de força com restrição de fluxo sanguíneo no ganho de força muscular e capacidade funcional de populações clínicas. Esta pesquisa foi realizada nas bases de dados SCOPUS, WEB OF SCIENCE e MEDLINE/PubMed, desde os primeiros registros até novembro de 2021 e idioma em inglês. Os termos (“Blood flow restriction” OR “vascular occlusion” OR “Kaatsu training”AND “low intensity”) AND (“Strength Training” OR “resistance training” OR “Strength”) AND (“clinical populations” OR “elderly“OR “old” OR “hypertension” OR “diabetes” OR “myositis” OR “obesity”AND “chronic diseases”) AND (“functional capacity” OR “functionality” OR “muscle function”) foram usados. Ensaios clínicos (randomizados e não-randomizados) foram incluídos quando comparados com treinamento de força de alta intensidade, baixa intensidade, baixa intensidade com restrição de fluxo sanguíneo e grupo controle sem exercício físico. A qualidade das evidências foi avaliada pela escala Testex. Durante a pesquisa foram pré-selecionados e analisados 122 artigos e ao final da seleção, nove artigos preencheram todos os critérios de inclusão e especificações estabelecidas. Conclui-se que o treinamento de força associado à restrição de fluxo sanguíneo tem sido uma alternativa eficaz e tolerável na melhora da força muscular e capacidade funcional e, portanto, uma ferramenta potencial para população clínica.

RESUMO Introdução: O exercício de força com restrição de fluxo sanguíneo (RFS) consiste no uso de cargas baixas (20-30% de 1RM) associado à restrição de fluxo sanguíneo para melhorar o desempenho físico. A restrição pode ser realizada de forma contínua ou intermitente; contudo, não está claro como isso afeta a hemodinâmica de mulheres com hipertensão. Objetivo: Analisar o efeito agudo do exercício de força (EF) no membro superior e inferior com restrição de fluxo sanguíneo (RFS) contínua e intermitente sobre as variáveis hemodinâmicas em mulheres hipertensas. Métodos: Treze mulheres com hipertensão controlada (entre 40 e 65 anos de idade) foram submetidas a oito protocolos experimentais em um estudo randomizado, cruzado e contrabalançado, sendo quatro sessões de exercícios para o membro superior direito (flexão de cotovelo) e quatro sessões de exercícios para o membro inferior direito (extensão de joelho). As medidas das variáveis pressão arterial sistólica (PAS), pressão arterial diastólica (PAD) e frequência cardíaca (FC) foram realizadas antes, durante, imediatamente depois e nos minutos 15, 30, 45 e 60 no período pós-exercício. Resultados: Observou-se que não houve interações significativas entre protocolos × segmentos × tempo, protocolos × segmentos, protocolos × tempo, segmentos × tempo, protocolo, segmento e tempo nas variáveis PAS, PAD e FC e durante e depois do EF (p > 0,05). Embora todos esses protocolos tenham elevado significativamente a PAS, PAD e FC, os valores permaneceram dentro da normalidade. Os protocolos do estudo não promoveram efeito hipotensor. Conclusão: O EF de baixa carga combinado com RFS contínua e intermitente, no membro superior e inferior, parece promover alterações similares nas variáveis hemodinâmicas em mulheres hipertensas. ABSTRACT Introduction: Strength training with blood flow restriction (BFR) involves the use of low loads (20-30% of 1RM) with restriction of blood flow to promote gains in physical fitness. The restriction can be applied continuously or intermittently; however, it is unclear how it affects the hemodynamics of hypertensive women. Objective: To analyze the acute effect of resistance exercise (RE) on the upper and lower limbs with continuous and intermittent blood flow restriction (BFR) on the hemodynamic variables of women with hypertension. Methods: Thirteen women with controlled hypertension (40 to 65 years) underwent eight experimental protocols, with a randomized, counter balanced, crossover design; four exercise sessions for the right upper limb (elbow flexion) and four for the right lower limb (knee extension). The systolic arterial pressure (SAP), diastolic arterial pressure (DAP), and heart rate (HR) were measured before, during, immediately after and 15, 30, 45 and 60 minutes after the exercises. Results: There were no significant interactions between the protocols vs. segments vs. time, protocols vs. segments, protocols vs. time, segments vs. time, protocol, segment and time, on the variables SAP, DAP, and HR during and after the RE (p>0.05). Although all these protocols had significantly elevated SAP, DAP and HR, the values remained within the normal range. The protocols of this study did not cause hypotensive effect. Conclusion: Low-load RE combined with continuous and intermittent BFR, on the upper and lower limbs, appears to promote similar variations in the hemodynamic variables of women with hypertension. RESUMEN Introducción: El ejercicio de fuerza con restricción de flujo sanguíneo (RFS) consiste en el uso de cargas bajas (20 a 30% de 1RM) asociado a la restricción de flujo sanguíneo para mejorar el desempeño físico. La restricción puede ser realizada de forma continua o intermitente; sin embargo, no está claro cómo esto afecta a la hemodinámica de mujeres con hipertensión. Objetivo: Analizar el efecto agudo del ejercicio de fuerza (EF) en el miembro superior e inferior con restricción de flujo sanguíneo (RFS) continua e intermitente sobre las variables hemodinámicas en mujeres hipertensas. Métodos: Trece mujeres con hipertensión controlada (entre 40 y 65 años de edad), fueron sometidas a ocho protocolos experimentales en un estudio aleatorizado, cruzado y contrabalanceado, siendo cuatro sesiones de ejercicios para el miembro superior derecho (flexión de codo) y cuatro sesiones de ejercicios para el miembro inferior derecho (extensión de rodilla). Las medidas de las variables presión arterial sistólica (PAS), presión arterial diastólica (PAD) y frecuencia cardíaca (FC) se realizaron antes, durante, inmediatamente después y en los minutos 15, 30, 45 y 60 en el período post-ejercicio. Resultados: Se observó que no hubo interacciones significativas entre protocolos × segmentos × tiempo, protocolos × segmentos, protocolos × tiempo, segmentos × tiempo, protocolo, segmento y tiempo en las variables PAS, PAD y FC y durante y después del EF (p> 0,05). Aunque todos estos protocolos hayan elevado significativamente la PAS, PAD, y FC, los valores permanecieron dentro de la normalidad. Los protocolos del estudio no promovieron un efecto hipotensor. Conclusión: El EF de baja carga combinado con RFS continua e intermitente, en el miembro superior e inferior, parece promover alteraciones similares en las variables hemodinámicas en mujeres hipertensas. RESUMO Introdução: Melhorar os níveis de força é importante para as mulheres com osteoporose. Os exercícios de força e aeróbicos são eficazes para atingir esse objetivo; no entanto, o uso de exercícios de baixa carga com restrição do fluxo sanguíneo é uma alternativa aos métodos tradicionais de exercício para atingir os mesmos ganhos de força nessa população. Objetivo: Analisar o efeito crônico do treinamento aeróbico e de força combinado com a restrição de fluxo sanguíneo sobre a força dinâmica máxima (FDM) de mulheres com osteoporose. Métodos: Vinte mulheres (61,40 ± 4,63 anos; 61,82 ± 12,54 kg; 1,51 ± 0,05 m; 27,16 ± 5,55 kg/m²) foram randomizadas em quatro grupos: 1 - treinamento de força de alta intensidade (AI); 2 - treinamento de força de baixa intensidade com restrição de fluxo sanguíneo (BIRFS); 3 - treinamento aeróbico com restrição de fluxo sanguíneo (ARFS) e 4 - controle (GC). A avaliação da força dinâmica máxima (FDM) da extensão unilateral do joelho foi realizada pelo teste de 1RM antes e depois da 6ª e 12ª semanas de intervenção. Para a análise dos dados realizou-se o teste ANOVA de medidas repetidas com teste de Bonferroni post-hoc no software SPSS (versão 21.0), considerando um nível de significância de P < 0,05 em todas as análises. Resultados: As comparações antes da intervenção mostraram que os grupos AI e GC tinham menores níveis de força que os grupos ARFS e BIRFS (P < 0,05). O grupo ARFS apresentou aumento significativo na FDM entre a 1ª e a 6ª (9%; P = 0,001) e entre a 1ª e a 12ª semana (21,6%; P = 0,008). O grupo BIRFS teve aumento da FDM entre a 1ª e a 6ª (10,1%; P = 0,001), entre a 1ª e a 12ª semana (24,2%; P = 0,003) e entre a 6ª e 12ª semana (12,8%; P = 0,030). O grupo AI teve diferença significativa entre a 1ª e a 6ª semana (38,7%; P < 0,001), entre a 1ª e a 12ª semana (62%; P < 0,001) e entre a 6ª e a 12ª semana (17,4%; P = 0,020), enquanto o GC não apresentou diferença entre os pontos do tempo (P > 0,05). Conclusões: ARFS e BIRFS aumentaram efetivamente a FDM em mulheres com osteoporose.

ABSTRACT Introduction: Improving strength levels is important to women with osteoporosis. Resistance and aerobic exercise are effective means of reaching this goal; however, the use of low-load exercises with blood flow restriction is an alternative to traditional methods of exercise to achieve the same strength gains in this population. Objective: To analyze the chronic effects of aerobic and resistance training combined with blood flow restriction on the maximal dynamic strength (MDS) of women with osteoporosis. Methods: Twenty women (61.40±4.63 years of age, 61.82±12.54 kg, 1.51±0.05 m, 27.16±5.55 kg/m²) were randomly assigned to four groups: 1 - high-intensity resistance training (HI); 2 - low-intensity resistance training with blood flow restriction (LI-BFR); 3 - aerobic training with blood flow restriction (ABFR); and 4 - control group (CG). Unilateral knee extension MDS was assessed using the one-repetition maximum (1RM) strength test before and after the 6th and 12th weeks of intervention. The data were analyzed using repeated measures analysis of variance (ANOVA) with a Bonferroni post-hoc test performed using SPSS (version 21.0), considering a significance level of P<0.05 for all tests. Results: Baseline comparisons showed that HI and CG had lower strength levels than LI-BFR and ABFR groups (P<0.05). The ABFR group exhibited a significant increase in MDS between the 1st and the 6th week (9%, P=0.001) and between the 1st and the 12th week (21.6%, P=0.008). The LI-BFR group exhibited increased MDS between the 1st and the 6th week (10.1%, P=0.001), between the 1st and the 12th week (24.2%, P=0.003) and between the 6th and 12th week (12.8%, P=0.030). The HI group exhibited a significant difference between the 1st and the 6th week (38.7%, P<0.001), between the 1st and the 12th week (62%, P<0.001) and between the 6th and 12th weeks (17.4%, P=0.020), whereas the CG had no significant differences between the timepoints (P>0.05). Conclusions: ABFR and LI-BFR effectively increased the MDS of women with osteoporosis.

RESUMEN Introducción: Mejorar los niveles de fuerza es importante para las mujeres con osteoporosis. Los ejercicios de fuerza y aeróbicos son eficaces para alcanzar ese objetivo; sin embargo, el uso de ejercicios de baja carga con restricción del flujo sanguíneo es una alternativa a los métodos tradicionales de ejercicio para alcanzar los mismos aumentos de fuerza en esa población. Objetivo: Analizar el efecto crónico del entrenamiento aeróbico y de fuerza combinado con la restricción de flujo sanguíneo sobre la fuerza dinámica máxima (FDM) de mujeres con osteoporosis. Métodos: Veinte mujeres (61,40 ± 4,63 años; 61,82 ± 12,54 kg, 1,51 ± 0,05 m; 27,16 ± 5,55 kg / m²) fueron aleatorizadas en cuatro grupos: 1 - entrenamiento de fuerza de alta intensidad (AI); 2 - entrenamiento de fuerza de baja intensidad con restricción del flujo sanguíneo (BIRFS); 3 - entrenamiento aeróbico con restricción del flujo sanguíneo (ARFS) y 4 - Control (GC). La evaluación de la fuerza dinámica máxima (FDM) de extensión unilateral de la rodilla se realizó mediante la prueba de 1RM antes y después de la 6ª y 12ª semana de intervención. Para el análisis de los datos se realizó la prueba ANOVA de medidas repetidas con prueba de Bonferroni post hoc en el software SPSS (versión 21.0), considerando un nivel de significación de P < 0,05 en todos los análisis. Resultados: Las comparaciones antes de la intervención mostraron que los grupos AI y GC tenían menores valores de fuerza que ARFS y BIRFS (P < 0,05). El grupo ARFS presentó un aumento significativo de la FDM entre las semanas 1 y 6 (9%; P = 0,001) y entre las semanas 1 y 12 (21,6%; p = 0,008). El BIRFS tuvo un aumento de la FDM entre las semanas 1 y 6 (10,1%; p = 0,001), entre las semanas 1 y 12 (24,2%; p = 0,003) y las semanas 6 y 12 (12,8%; P = 0,030). El grupo AI tuvo una diferencia significativa entre las semanas 1 y 6 (38,7%; P < 0,001), entre las semanas 1 y 12 (62%, P < 0,001) y entre las semanas 6 y 12 (17,4%; p = 0,020), mientras que el GC no presentó diferencia entre los puntos del tiempo (P > 0,05). INTRODUCTION: Physiotherapy is one of the most important components of therapy for osteoarthritis of the knee. The objective of this prospective case series was to assess the efficiency of a guidance manual for patients with osteoarthritis of the knee in relation to pain, range of movement , muscle strength and function, active goniometry, manual strength test and function. METHODS: Thirty-eight adults with osteoarthritis of the knee (> 45 years old) who were referred to the physiotherapy service at the university hospital (Santa Casa de Misericórdia de São Paulo) were studied. Patients received guidance for the practice of specific physical exercises and a manual with instructions on how to perform the exercises at home. They were evaluated for pain, range of movement, muscle strength and function. These evaluations were performed before they received the manual and three months later. Patients were seen monthly regarding improvements in their exercising abilities. RESULTS: The program was effective for improving muscle strength, controlling pain, maintaining range of movement of the knee joint, and reducing functional incapacity. DISCUSSION: A review of the literature showed that there are numerous clinical benefits to the regular practice of physical therapy exercises by patients with osteoarthritis of the knee(s) in a program with appropriate guidance. This study shows that this guidance can be attained at home with the use of a proper manual. CONCLUSIONS: Even when performed at home without constant supervision, the use of the printed manual for orientation makes the exercises for osteoarthritis of the knee beneficial.

OBJETIVO: O Harris Hip Score é instrumento de avaliação específica, desenvolvido originalmente para avaliar os resultados da artroplastia total de quadril. O objetivo deste estudo foi traduzir e adaptar culturalmente este instrumento para a língua portuguesa. MÉTODO: O método de tradução e adaptação cultural do Harris Hip Score envolveu quatro etapas: 1 - tradução inicial; 2 - retrotradução; 3 - apreciação das versões com elaboração da versão de consenso; 4 - pré-teste comentado com elaboração da versão final. RESULTADOS: A versão de consenso foi aplicada em 30 pacientes com afecção do quadril, sendo verificadas dificuldades no entendimento de algumas expressões, as quais foram substituídas por termos de mais fácil entendimento. Na reaplicação do questionário com a nova versão houve entendimento por 100% dos pacientes no que diz respeito ao sentido semântico, idiomático e contextual. CONCLUSÃO: A versão brasileira do Harris Hip Score permitiu a disponibilização de mais este instrumento para avaliação da qualidade de vida de pacientes com afecções do quadril. Há necessidade de um estudo de avaliação da confiabilidade e validade da versão adaptada culturalmente, a qual já está em desenvolvimento.

OBJECTIVE: The Harris Hip Score is a specific evaluation tool, originally developed to assess the results of hip arthroplasty. The objective of this study was to translate and cross-culturally adapt the Harris Hip Score for the Portuguese language. METHOD: The method of translating and culturally adapting the Harris Hip Score involved four steps: 1 - initial translation, 2 - back-translation, 3 - evaluation of the pre-final versions with the development of a consensus version, 4 - commented pre-test with development of the final version. RESULTS: The consensus version was applied to thirty patients with hip disorders. Some difficulties were identified in understanding some expressions, which were replaced by more commonly-used expressions. When the questionnaire was re-applied, it was understood by 100% of the patients, in relation to the semantic, idiomatic and conceptual meanings. CONCLUSION: The Brazilian version of the Harris Hip Score provides another important tool for assessing quality of life of patients with hip disorders. A further study is currently underway to evaluate the reliability and validity of the culturally adapted version.
