Resumo Objetivo: avaliar a adesão das instituições de longa permanência brasileiras às orientações de Prevenção e Controle de Infecções da Organização Mundial da Saúde, observando a associação entre seu porte e a adesão a essas recomendações. Método: estudo transversal realizado com gestores de estabelecimentos. Os autores desenvolveram um questionário de 20 itens, com base nessas orientações, e um escore global de adesão, com base na adesão a essas recomendações. A adesão foi classificada como (1) excelente para aquelas que atenderam ≥14 de 20 recomendações, (2) bom para 10 a 13 itens e (3) baixo para aquelas com menos de dez itens. O tamanho das instalações foi classificado como pequeno, médio e grande, de acordo com uma análise de cluster em duas etapas. Estatística descritiva e teste de qui-quadrado foram utilizados com nível de significância de 5%. Resultados: das 362 instituições incluídas, 308 (85,1%) aderiram a 14 ou mais recomendações. Em relação ao seu tamanho, a adesão à triagem de sintomas de COVID-19 dos visitantes (p=0,037) e ao isolamento de pacientes até que tenham dois exames laboratoriais negativos (p=0,032) foi menor em estabelecimentos maiores, em comparação com estabelecimentos de médio e pequeno porte. Conclusão: a adesão às medidas de mitigação da COVID-19 nas unidades brasileiras foi considerada excelente para a maioria das recomendações, independentemente do porte das unidades.

Abstract Objective: to evaluate the adherence of Brazilian long-term care facilities to the World Health Organization Infection Prevention and Control guidance, and assess the association of their size with the adherence to these recommendations. Method: cross-sectional study conducted with facilities’ managers. Authors developed a 20-item questionnaire based on this guidance, and a global score of adherence, based on the adoption of these recommendations. Adherence was classified as (1) excellent for those who attended ≥14 out of 20 recommendations; (2) good for 10 to 13 items; and (3) low for those with less than ten items. Facilities’ sizes were established as small, intermediate, and large according to a two-step cluster analysis. Descriptive statistics and chi-square tests were used at a 5% significance level. Results: among 362 included facilities, 308 (85.1%) adhered to 14 or more recommendations. Regarding its size, adherence to screening COVID-19 symptoms of visitors (p=0.037) and isolating patients until they have had two negative laboratory tests (p=0.032) were lower on larger ones compared to medium and small facilities. Conclusion: adherence to COVID-19 mitigation measures in Brazilian facilities was considered excellent for most of the recommendations, regardless of the size of the units.

Resumen Objetivo: evaluar la adhesión de instituciones brasileñas de larga estancia a las orientaciones de Prevención y Control de Infecciones de la Organización Mundial de la Salud y evaluar la asociación entre su tamaño y la adhesión a esas recomendaciones. Método: estudio transversal realizado con gerentes de establecimientos. Los autores desarrollaron un cuestionario de 20 ítems basado en estas directrices y un puntaje general de cumplimiento acorde a la observancia de estas recomendaciones. La adhesión se calificó (1) excelente para aquellas que cumplieron con ≥14 de 20 recomendaciones; (2) buena para 10 a 13 artículos; y (3) baja para aquellas con menos de diez elementos. El tamaño de las instalaciones se clasificó como pequeños, medianos y grandes de acuerdo con un análisis de clúster de dos pasos. Se utilizó estadística descriptiva y la prueba de chi-cuadrado con un nivel de significancia del 5%. Resultados: de las 362 instituciones incluidas, 308 (85,1%) se adhirieron a 14 o más recomendaciones. En cuanto a su tamaño, la adhesión al cribado de síntomas de COVID-19 de los visitantes (p=0,037) y al aislamiento de los pacientes hasta que tengan dos pruebas de laboratorio negativas (p=0,032) fue menor en los establecimientos más grandes en comparación con los establecimientos medianos y pequeños. Conclusión: la adhesión a las medidas de mitigación de la COVID-19 en las unidades brasileñas fue considerada excelente para la mayoría de las recomendaciones, independientemente del tamaño de las unidades.

RESUMO Introdução: A doença de Alzheimer é a principal causa de demência em todo o mundo. Apesar das evidências alarmantes sobre a prevalência de demência, a condição ainda é subdiagnosticada por clínicos gerais na atenção primária. A detecção precoce da doença é benéfica para pacientes e familiares, que devem receber orientações abrangentes sobre como lidar com as complicações relacionadas às demências, abrangendo aspectos médicos, familiares e sociais, proporcionando assim uma oportunidade de planejar o futuro. Objetivo: O objetivo deste estudo foi avaliar os conhecimentos e as atitudes em relação à demência por parte de clínicos gerais de uma cidade do interior de São Paulo, Brasil. Métodos: Foi realizado um estudo de intervenção não randomizado, envolvendo seis palestras sobre demência. Antes e depois da intervenção, os médicos participantes completaram dois questionários sobre conhecimentos e atitudes em relação à demência. O estudo foi realizado nos serviços de atenção primária da cidade e um total de 34 clínicos gerais participaram do estudo. Resultados: A idade média da amostra foi de 33,9 (±10,2) anos e a maioria (76,5%) da amostra não havia realizado treinamento em residência médica. O número médio de respostas corretas no Questionário do Conhecimento sobre demência antes e após a intervenção de treinamento foi de 59,6 e 71,2% (p<0,001), respectivamente. A comparação das respostas médias no questionário de atitudes não revelou diferença estatisticamente significativa entre as duas aplicações do instrumento, antes e após a intervenção (p=0,059). Conclusões: Deve ser fornecido mais treinamento sobre demência para os clínicos gerais.

ABSTRACT Background: Alzheimer's disease (AD) is the leading cause of dementia worldwide. Despite alarming evidence on dementia prevalence, the condition is still underdiagnosed by general practitioners (GPs) in primary care. Early detection of the disease is beneficial for patients and relatives, who should be provided comprehensive guidance on dealing with dementia complications, covering medical, family and social aspects, thereby providing an opportunity to plan for the future. Objective: The objective of this study was to assess the knowledge of and attitudes toward dementia held by GPs from a city in the interior of São Paulo State, Brazil. Methods: A non-randomized intervention study was conducted involving six lectures about dementia. Before and after the intervention, the participating physicians completed two quizzes about knowledge of and attitudes towards dementia. The study was carried out in the primary care services of the town and a total of 34 GPs participated in the study. Results: The mean age of the sample was 33.9 (±10.2) years and the majority (76.5%) of the sample had not undertaken medical residency training. The mean number of correct answers on the Knowledge Quiz about dementia before and after the training intervention was 59.6 and 71.2% (p<0.001), respectively. The comparison of the mean responses on the Attitude Quiz revealed no statistically significant difference between the two applications of the instrument, before and after intervention (p=0.059). Conclusions: More training for GPs on dementia should be provided.

Abstract This research shows two new microalloyed steels with niobium (Nb) for railroad wheels (Nb1 and Nb2) applied on heavy haul transportation (load over 30 tons/axle). The steels have mixed microstructure composed of pearlite and bainite (finished wheel rim section), and another one composed of martensite with bainite (black wheel section removed on machining) obtained by continuous cooling quenching heat treatment followed by tempering. The bainitic and martensitic microstructure are sensitive to the temper embrittlement phenomenon; therefore, knowing the embrittlement temperature range of these steels is mandatory in order to avoid such an event in the tempering process. Charpy specimens were extracted from the rim of railroad wheels, austenitized (860 ºC), quenched in water for four minutes, and tempered at different temperatures to obtain the embrittlement curve. Three materials were compared: AAR (Association of American Railroads) Class C steel with pearlitic microstructure (not sensitive to embrittlement), Nb1, and Nb2 microalloyed steels with martensitic, bainitic, and pearlitic microstructures. The microalloyed steels have niobium (Nb), molybdenum (Mo), and silicon (Si) addition. The effect of Si content and the microstructure (analyzed by Scanning Electronic Microscope (SEM)), regarding the embrittlement phenomenon, was also studied. The microstructure of Nb2 steel was analyzed by with Transmission Electronic Microscope (TEM) for improving its characterization.

ABSTRACT Mobile phones (MPs) have become an important work tool around the world including in hospitals. We evaluated whether SARS-CoV-2 can remain on the surface of MPs of first-line healthcare workers (HCW) and also the knowledge of HCWs about SARS-CoV-2 cross-transmission and conceptions on the virus survival on the MPs of HCWs. A cross-sectional study was conducted in the COVID-19 Intensive Care Unit of a teaching hospital. An educational campaign was carried out on cross-transmission of SARS-CoV-2, and its permanence in fomites, in addition to the proper use and disinfection of MPs. Herewith an electronic questionnaire was applied including queried conceptions about hand hygiene and care with MP before and after the pandemic. The MPs were swabbed with a nylon FLOQ Swab™, in an attempt to increase the recovery of SARS-CoV-2. All MP swab samples were subjected to SARS-CoV-2 RT-PCR; RT-PCR positive samples were subjected to viral culture in Vero cells (ATCC® CCL-81™). Fifty-one MPs were swabbed and a questionnaire on hand hygiene and the use and disinfection of MP was applied after an educational campaign. Most HCWs increased adherence to hand hygiene and MP disinfection during the pandemic. Fifty-one MP swabs were collected and two were positive by RT-PCR (4%), with Cycle threshold (Ct ) values of 34-36, however, the cultures of these samples were negative. Although most HCWs believed in the importance of cross-transmission and increased adherence to hand hygiene and disinfection of MP during the pandemic, SARS-CoV-2 RNA was detected in MPs. Our results suggest the need for a universal policy in infection control guidelines on how to care for electronic devices in hospital settings.

Resumo Introdução: A fisioterapia em pacientes com osteoartrite (OA) atua na diminuição da dor, aumento da funcionalidade articular e promove melhora na qualidade de vida. Objetivo: Analisar o efeito de um programa estruturado de fisioterapia aquática (FA) na capacidade funcional (CF) e mobilidade de idosos com OA. Métodos: Estudo de controle prospectivo, quantitativo e analítico, randomizado. A amostra foi composta por 29 pacientes com idade igual ou superior a 60 anos e com diagnóstico de OA alocados aleatoriamente no grupo intervenção, composto por 17 participantes submetidos à PA por dois meses, e no grupo controle de 12 participantes. O desfecho primário foi medido pelos instrumentos WOMAC e teste de caminhada de seis minutos. Como desfecho secundário, o Timed Up and Go Test mediu a mobilidade. Análise estatística descritiva foi utilizada para apresentar os dados, com valores de média e desvio padrão. A comparação entre os grupos para potenciais fatores de confusão e em relação à evolução foi realizada com o teste qui-quadrado não paramétrico, o teste exato de Fisher e o teste de Kruskal-Wallis, e o teste de Dunn para comparações múltiplas. As diferenças foram consideradas estatisticamente significantes quando p < 0,05. Resultados: Observou-se diferença significativa nos parâmetros físicos e funcionais relacionados à dor (p < 0,001), rigidez (p < 0,001) atividade física (p < 0,001), distância percorrida em seis minutos (p = 0,001) e mobilidade (p < 0,001) quando comparado o IG ao GC. Conclusão: O programa estruturado de FA contribuiu efetivamente para a melhoria da CF e mobilidade de idosos com OA de joelho.

Abstract Introduction: Physicaltherapy in patients with osteoarthritis (OA) acts to decrease pain, increase joint functionality and to promote improvement in quality of life. Objective: To analyze the effect of a structured program of aquatic physiotherapy (AP) on functional capacity (FC) and mobility of elderly people with OA. Methods: Prospective, quantitative and analytical, randomized control study. The sample consisted of 29 patients, aged 60 years or over, diagnosed with OA, randomly allocated to the intervention group (IG), composed of 17 participants who underwent BP for two months, and to the control group (CG) of 12 participants. The primary outcome was measured by the WOMAC instruments and six-minute walk test. As a secondary endpoint, the Timed Up and Go Test measured mobility. Descriptive statistical analysis was used to present the data, with mean and standard deviation values. The comparison between groups for potential confounding factors and in relation to evolution was performed using the non-parametric chi-square test, Fisher's exact test and the Kruskal-Wallis test, and Dunn's test for multiple comparisons. The differences were considered statistically significant when p < 0.05. Results: There was a significant difference in physical and functional parameters related to pain (p < 0.001), stiffness (p < 0.001) physical activity (p < 0.001), distance covered in six minutes (p = 0.001), and mobility (p < 0.001) when comparing IG to CG. Conclusion: The structured AP program effectively contributed to the improvement of FC and mobility of elderly people with knee OA.

Introduction: Mortality rates due to COVID-19 in older people living in long-term care homes reach 80% in some countries. In Brazil, little is known about the incidence and mortality in this population. Objective: To describe the occurrence of COVID-19 infection and mortality in care homes for the older people in Brazil. Methods: Cross-sectional and descriptive study using primary and secondary sources and non-probabilistic sampling. Between April and June 2020, through an electronic questionnaire, care homes administrators from all over Brazil were invited to report the occurrence of infection and mortality of residents by COVID-19. In addition, an active search for data was carried out with the services of the State Public Prosecutor Offices, State Health Departments and the Unified Social Assistance System. The data were analyzed using descriptive statistics. Results: Information from 1,802 care homes from 11 states was analyzed, where 42,216 older people lived. The incidence rate of COVID-19 was 6.14%, and 458 deaths were recorded in the period, with a case-fatality rate of 17.65%. Conclusion: The incidence and fatality observed in this study were lower than the rates observed in other countries. However, given the limitations regarding the collection of information and other barriers to the study, new structured data sources are essential for understanding the spread of the virus in care homes in the country.

Introdução: As taxas de mortalidade pela COVID-19 em idosos que vivem em Residenciais de Cuidado de Longa Duração (RCLD) chegam a 80% em alguns países. No Brasil, pouco se conhece sobre a incidência e mortalidade nesta população. Objetivo: Descrever a ocorrência da infecção e mortalidade por COVID-19 em RCLD para idosos no Brasil. Metodologia: Estudo transversal e descritivo com uso de fontes primárias e secundárias e amostragem não probabilística. Entre abril e junho de 2020, por meio de um questionário eletrônico, administradores de RCLD de todo o Brasil foram convidados a informar a ocorrência de infecção e mortalidade de residentes por COVID-19. Além disso, foi realizada busca ativa de dados junto aos serviços do Ministério Público Estadual, Secretarias de Saúde e Sistema Único de Assistência Social. Os dados foram analisados através de estatística descritiva Resultados: Foram analisadas as informações de 1,802 RCLD de 11 estados, onde residiam 42,216 idosos. A taxa de incidência de COVID-19 foi de 6,14%. Foram registrados no período 458 óbitos, com uma taxa de letalidade de 17,65%. Conclusão: A incidência e letalidade observados neste estudo foram menores do que as taxas observadas em outros países. Entretanto, dadas as limitações quanto a coleta de informações e outras barreiras para o estudo, novas fontes estruturadas de dados são fundamentais para o conhecimento da propagação do vírus nos RCLD no país.

RESUMO Objetivo: Avaliar se o uso do crescimento guiado com placas em oito é mais eficaz que os grampos de Blount na correção do geno varo idiopático ou geno valgo idiopático. Métodos: Foi realizada uma revisão sistemática (RS), de acordo com a metodologia apropriada para busca de ensaios clínicos randomizados (ECR). Pesquisamos em sete bancos de dados por meio de uma metodologia definida anteriormente e incluímos ECR, independentemente do idioma, período ou status da publicação. Resultados: Foram recuperados 6830 artigos. Destes, 14 estudos possivelmente elegíveis foram encontrados, mas apenas um ECR foi incluído para a RS. O ECR incluído compara placas em oito e grampos de Blount, não mostrando diferença estatisticamente significante para os resultados de tempo na correção da deformidade, dor pós-operatória após 24 horas e dor pós-operatória após 72 horas. O estudo é de nível baixo ou muito baixo de evidência para determinar a técnica mais eficaz. Não encontramos um ECR que comparasse a correção do geno varo. Conclusão: Ensaios clínicos randomizados de boa qualidade comparando grampos de Blount com oito placas devem ser realizados para determinar qual técnica é superior para correções do plano coronal. Nível de Evidência I, Revisão sistemática de ECRC.

ABSTRACT Objective: Our objective is to evaluate whether the use of guided growth with eight-plates is more effective than the use of Blount staples for the correction of the idiopathic genu varum or idiopathic genu valgum. Methods: A systematic review (SR) was carried out according to the appropriate methodology for randomized clinical trials (RCTs). We searched seven databases through a previously defined methodology, and we included RCTs, regardless of language, period of publication and status of publication. Results: Resulted in 6830 articles retrieved. Of theses, we identified 14 potential eligible studies. but just one RCT was included for the SR. The included RCT compares the eight-plate and the Blount staple and showed no statistically significant difference for the outcomes of time to correct the deformity, postoperative pain after 24 hours and postoperative pain after 72 hours. The study is of low or very low level of evidence to determine the most effective technique. We didn’t find a RCT that compared the correction of the genu varum. Conclusion: Good quality randomized clinical trials comparing Blount staples versus eight-plaque must be performed to determine which technique is superior for coronal plane corrections. Introduction: Few information is available on coping and mitigating COVID-19 in long-term care services for older people (ILPIs) in Latin America (LA). Objectives: To describe how ILPI managers in LA planned and adapted their coping routines to the COVID-19 pandemic, and whether they were able to comply with the recommendations of the World Health Organization (WHO). Methodology: Cross-sectional study, based on the application of an online survey aimed at LTCF managers in LA. A 46-question questionnaire (adopting WHO principles) was sent to participants. Descriptive statistics was used to summarize the data. Results: 23 managers answered the survey (excluding Brazilian respondents), totaling 874 older person (5 -270); a questionnaire was excluded due to lack of answers. Fourteen ILPIs (63.60%) were private for profit. The rate of adherence to WHO recommendations was over 70% for most issues. A little more than half of the institutions developed a strategic coping plan, or identified strategies to deal with deaths from suspected cases. Difficulty in acquiring personal protective equipment was reported by 59.10% of the investigated LTCIs. The testing capacity for SARS-Cov-2 has been reduced (36.36% had no test). Conclusions: The rate of adherence to the recommendations proposed by WHO for coping with COVID-19 was over 70% for most investigated LTCIs. Strategic coping plans were developed in just over half of the institutions. The availability of PPE and the testing capacity for SARS-Cov-2 proved to be quite unsatisfactory.

Introdução: Pouco se sabe sobre o enfrentamento e mitigação à COVID-19 em serviços de longa permanência para idosos (ILPIs) na América Latina (AL). Objetivos: Descrever como os gestores de ILPIs na AL planejaram e adequaram suas rotinas de enfrentamento à pandemia de COVID-19, e se foram capazes de cumprir com as recomendações da Organização Mundial da Saúde(OMS). Metodologia: Estudo transversal, baseado na aplicação de uma pesquisa online dirigida aos gestores de ILPIs na AL. Um questionário de 46 questões (adotando os princípios da OMS) foi enviado aos participantes. Estatística descritiva foi usada para resumir os dados. Resultados: 23 gestores responderam a pesquisa (excluídos os respondentes brasileiros), totalizando 874 idosos (5 –270); um questionário foi excluído por falta de respostas. Quatorze ILPIs (63,60%) eram privadas com fins lucrativos. A taxa de adesão às recomendações da OMS foi superior a 70% para a maioria das questões. Pouco mais da metade das instituições elaborou um plano estratégico de enfrentamento, ou identificou estratégias para lidar com óbitos de casos suspeitos. Dificuldade para a aquisição de equipamentos de proteção individual foram relatados por 59,10% das ILPIs investigadas. A capacidade de testagem para o SARS-Cov-2 foi reduzida (36,36% não dispunham de nenhum teste). Conclusões: A taxa de adesão às recomendações propostas pela OMS para o enfrentamento da COVID-19 foi superior a 70% para a maioria das ILPIs investigadas. Planos estratégicos de enfrentamento foram elaborados em pouco mais da metade das instituições. A disponibilidade de EPIs e a capacidade de testagem para o SARS-Cov-2 mostrou-se bastante insatisfatória.

Obtevese prevalência da dermatite e associações pelo cálculo da razão. A dimensão do efeito foi estimada pela razão de chances com intervalo de confiança de 95% e significância estatística p < 0,05. Resultados: Incluíram-se 138 idosos, média de idade 77,2 anos (± 9,3); 69 (50%) com incontinência fecal e urinária combinadas. A prevalência de dermatite foi 36,2% (50); 28% (14) apresentavam lesão por pressão; 14% (7), candidose. Foram fatores de risco: maior tempo de internação (Odds Ratio = 5,8 [2,6-12,9]), obesidade (Odds Ratio = 3,6 [1,2-10,4]), alto nível de dependência (Odds Ratio = 2,4 [1,1-5,0]) e alto risco para lesão por pressão (Odds Ratio = 6,1 [1,4-26,9]). Conclusão: Houve alta prevalência de dermatite associada à incontinência. O reconhecimento precoce dos fatores de risco favorece ações preventivas efetivas.

The effect dimension was estimated by the odds ratio with a 95% confidence interval and statistical significance p <0.05. Results: 138 elderly people were included, with an average age of 77.2 years old (± 9.3); 69 (50%) had combined fecal and urinary incontinence. The dermatitis prevalence was 36.2% (50); 28% (14) had pressure injuries; 14% (7), candidiasis. Risk factors were: longer hospital stay (Odds Ratio = 5.8 [2.6-12.9]), obesity (Odds Ratio = 3.6 [1.2-10.4]), high level of dependence (Odds Ratio = 2.4 [1,1-5,0]) and high risk for pressure injury (Odds Ratio = 6.1 [1,4-26,9]). Conclusion: The study found a high prevalence of dermatitis associated with incontinence. The early recognition of risk factors favors effective preventive actions.

Método: Estudio transversal exploratorio con ancianos incontinentes en hospitales públicos. Se obtuvo la prevalencia de la dermatitis y relaciones por el cálculo de la razón, y la dimensión del efecto ha sido estimada por la razón de oportunidades con intervalo de confianza de 95% y significación estadística p < 0,05. Resultados: Se incluyeron 138 ancianos, la edad promedio de 77,2 años (± 9,3); 69 (50%) de ellos, con incontinencia fecal y urinaria combinadas. La prevalencia de dermatitis ha sido 36,2% (50); 28% (14) presentaban lesión por presión; y 14% (7), candidosis. Han sido factores de riesgo para la dermatitis: mayor tiempo de internación (OR = 5,8 [2,6-12,9]), obesidad (OR = 3,6 [1,2-10,4]), alto nivel de dependencia (OR = 2,4 [1,1-5,0]) y alto riesgo para lesión por presión (OR = 6,1 [1,4-26,9]). Conclusiones: Hubo alta prevalencia de dermatitis relacionada a la incontinencia, y el reconocimiento precoz de los factores de riesgo pode favorecer acciones preventivas efectivas.

Método: Estudio transversal exploratorio con ancianos incontinentes en hospitales públicos. Se obtuvo la prevalencia de la dermatitis y relaciones por el cálculo de la razón, y la dimensión del efecto ha sido estimada por la razón de oportunidades con intervalo de confianza de 95% y significación estadística p < 0,05. Resultados: Se incluyeron 138 ancianos, la edad promedio de 77,2 años (± 9,3); 69 (50%) de ellos, con incontinencia fecal y urinaria combinadas. La prevalencia de dermatitis ha sido 36,2% (50); 28% (14) presentaban lesión por presión; y 14% (7), candidosis. Han sido factores de riesgo para la dermatitis: mayor tiempo de internación (OR = 5,8 [2,6-12,9]), obesidad (OR = 3,6 [1,2-10,4]), alto nivel de dependencia (OR = 2,4 [1,1-5,0]) y alto riesgo para lesión por presión (OR = 6,1 [1,4-26,9]). Conclusiones: Hubo alta prevalencia de dermatitis relacionada a la incontinencia, y el reconocimiento precoz de los factores de riesgo pode favorecer acciones preventivas efectivas.

RESUMO Objetivo: Determinar prevalência e fatores de risco para dermatite associada à incontinência em idosos. Método: Estudo transversal exploratório realizado em hospitais públicos. Obtevese prevalência da dermatite e associações pelo cálculo da razão. A dimensão do efeito foi estimada pela razão de chances com intervalo de confiança de 95% e significância estatística p < 0,05. Resultados: Incluíram-se 138 idosos, média de idade 77,2 anos (± 9,3); 69 (50%) com incontinência fecal e urinária combinadas. A prevalência de dermatite foi 36,2% (50); 28% (14) apresentavam lesão por pressão; 14% (7), candidose. Foram fatores de risco: maior tempo de internação (Odds Ratio = 5,8 [2,6-12,9]), obesidade (Odds Ratio = 3,6 [1,2-10,4]), alto nível de dependência (Odds Ratio = 2,4 [1,1-5,0]) e alto risco para lesão por pressão (Odds Ratio = 6,1 [1,4-26,9]). Conclusão: Houve alta prevalência de dermatite associada à incontinência. O reconhecimento precoce dos fatores de risco favorece ações preventivas efetivas.

Abstract INTRODUCTION Human retroviruses and the hepatitis B and C viruses (HBV and HCV, respectively) share routes of transmission; thus, coinfections occur and could alter subsequent disease outcomes. A preliminary study on human T-lymphotropic virus types 1 and 2 (HTLV-1/2) in serum samples from HBV- and HCV-infected individuals in São Paulo revealed 1.3% and 5.3% rates of coinfection, respectively. These percentages were of concern since they were detected in HTLV-endemic regions and in high-risk individuals in Brazil. The present study was conducted to extend and confirm these data. METHODS HTLV-1/2 and human immunodeficiency virus (HIV) infection status were identified in 1,984 sera for HBV and HCV viral load quantification - 1,290 samples from HBV-infected individuals (53.3% men, mean age: 47.1 years) and 694 samples from HCV-infected individuals (56.3% men, mean age: 50.1 years). HTLV-1/2 antibodies were detected by enzyme immunoassay, followed by western blotting and line immunoassay; HIV infection was detected by enzyme immunoassay. RESULTS HTLV-1/-2 infection was detected in 1.9% HBV-infected individuals (0.7% HTLV-1 and 1.2% HTLV-2) and in 4.0% (2.4% HTLV-1 and 1.6% HTLV-2) HCV-infected individuals; HIV infection was detected in 9.2% and 14.5%, respectively. Strong associations with HTLV and HIV, male sex, and older age were found in HBV/HTLV and HCV/HTLV-coinfected individuals (p<0.05). CONCLUSIONS HTLV-1 and HTLV-2 were confirmed to be prevalent in individuals with HBV and HCV in São Paulo; coinfected individuals deserve further clinical and laboratory investigation.

The objective of this article is to identify the Brazilian marketing attributes presented in the fashion trends of the 2017 collection presented at the two brazilian events, São Paulo Fashion Week (SPFW) and Rio Moda Rio (RMR). This work can be considered an exploratory and descriptive article, especially when it comes to detect trends and changes in behavior. It is possible to conclude that, in part, the attributes of brasility are present in the beach fashion segment. The beach fashion presented in the collection does not contain all the elements of Brazilianness due to market trends. The cultural metaphors are absorbed by the fashion industry, which adapts its products to contemporary women, adapting its portfolio according to the profile of women who are consumers of the moment. It is possible to conclude that the Brazilian attributes are part of the beach fashion segment, however, for the trends of 2017, conclude that the 4 requirements, cited by Sutter (2014), lifestyle, application of craft techniques, shape and volumen, and print attribute, are included at actual trends. Among the attributes of Brazilian studies only the tissue was not found in any of the events studied, the use of ecological and sustainable tissues, apparently, were not used in the collections. This study becomes relevant and original in the way it is conducted, since it recognizes that the trends presented in the fashion collections represent the marketing work and show that several characteristics that are also Brazilian attributes.

El objetivo de este artículo es identificar los atributos brasileños de marketing presentes en las tendencias de moda de la colección 2017 presentada en los dos eventos brasileños, São Paulo Fashion Week (SPFW) y Rio Moda Rio (RMR). Este trabajo puede considerarse un artículo exploratorio y descriptivo, especialmente cuando se trata de detectar tendencias y cambios en el comportamiento. Es posible concluir que, en parte, los atributos de la brasilidad están presentes en el segmento de la moda de playa. La moda de playa presentada en la colección no contiene todos los elementos de brasileño debido a las tendencias del mercado. Las metáforas culturales son absorbidas por la industria de la moda, que adapta sus productos a las mujeres contemporáneas, adaptando su cartera de acuerdo con el perfil de las mujeres que son consumidoras del momento. Es posible concluir 4 os atributos brasileños, citados por Sutter (2014), como tendencias del segmento de moda de playa, sin embargo, para 2017. Solo se incluyen estilos de vida, aplicación de técnicas artesanales, requisitos de impresión, y formas y volúmenes. Entre los atributos de los estudios brasileños solo el tejido no se encontró en ninguno de los eventos estudiados, aparentemente el uso de tejidos ecológicos y sostenibles no se utilizó en las colecciones. Este estudio se vuelve relevante y original en la forma en que se lleva a cabo, ya que reconoce las tendencias que se presentan en las colecciones de moda, el trabajo de mercadeo y la muestra de varias características que también son los atributos brasileños.

Heavy haul transportation (load over 30 tons/axle), as well as the axle load, has been more and more used in Brazil and worldwide. The stress generated in the wheel-rail contact with loads up to 30 tons/axle is around 760 MPa, which causes premature wear and cracks of conventional wheels (AAR (Association of American Railroads) class C). Microalloyed wheels are fundamental on heavy haul transport, whose main function is to combine high hardness, ductility, and yield strength of the material in order to prevent shelling. The main purpose of this research is to develop a new microalloyed wheel steel with niobium addition that meets all the requirements of the AAR class D material with mixed microstructure composed of pearlite and bainite. The 0.71C/0.020Nb steel developed in this study (Nb material) for railroad achieved the standards required for AAR class D in all mechanical properties, with fracture toughness higher than the usual vanadium microalloyed steel used in comparison. The Continuous Cooling Transformation (CCT) diagram showed the presence of bainite even at very low cooling rate, in the range between 0.3 - 2 °C/s. These cooling rates to form bainite are much lower than those observed in other steels with similar composition.

Resumo Objetivo: Verificar a trajetória da funcionalidade em idosos hospitalizados por condições clínicas em hospital universitário. Método: Estudo descritivo, prospectivo, de coorte, realizado entre 2015 e 2016. Foram avaliados idosos internados no Hospital das Clínicas da Faculdade de Medicina de Botucatu - Unesp, quanto à funcionalidade por avaliação das atividades básicas de vida diária (ABVD) pela Escala de Katz, estado nutricional (índice de massa corporal-IMC) e presença de Síndrome da Fragilidade (SF) (critério de Fried). Realizada descrição da trajetória da funcionalidade em quatro momentos: 15 dias antes da internação (M0), na internação (M1), na alta hospitalar (M2) e 30 dias após a alta (M3). Resultados: Foram avaliados 99 idosos com média de idade de 74 (+7,35) anos, 59,6% do sexo masculino. Da casuística 81,8% apresentavam independência funcional em M0; 45,5% no M1; 57,6% no M2 e 72,8% no M3. De acordo com as trajetórias da funcionalidade verificou-se que 28,2% dos idosos perderam função entre M0 e M3. Houve associação entre piora da funcionalidade entre M0 e M3 e SF (RR 4,56; IC 95% 1,70-12,26; p=0,003). Conclusão: Idosos apresentam pior funcionalidade na alta hospitalar quando comparada com o momento antes da internação. Cerca de 28,0% dos idosos apresentaram pior função 30 dias após a alta em relação a 15 dias antes da internação. Os idosos com Síndrome da Fragilidade apresentam maior risco para resultados funcionais piores após 30 dias da alta.

Abstract Objective: To verify the trajectory of the functional capacity of elderly persons hospitalized due to clinical conditions in a university hospital. Method: A descriptive, prospective cohort study was conducted between 2015 and 2016. Elderly patients admitted to the Hospital das Clínicas of Botucatu Medical School (Unesp), Brazil, were evaluated for the functional assessment of basic activities of daily living (BADL) using the Katz scale, nutritional status (body mass index (BMI)) and presence of the Frailty Syndrome (FS) (Fried criteria). A description of the trajectory of functional capacity was carried out at four times: 15 days before admission (T0), at admission (T1), at hospital discharge (T2) and 30 days after discharge (T3). Results: 99 elderly people with a mean age of 74 (+7.35) years, 59.6% of whom were male, were evaluated. Of these, 81.8% presented functional independence at T0, 45.5% at T1, 57.6% at T2 and 72.8% at T3. According to their functional trajectories, 28.2% of the elderly lost functional capacity between T0 and T3. There was an association between worsening of functional capacity between T0 and T3 and the FS (RR 4.56; 95% CI 1.70-12.26, p=0.003). Conclusion: Elderly patients have worse functional capacity at hospital discharge than before hospitalization. About 28.0% of the elderly had worse functional capacity 30 days after discharge than 15 days before admission. The elderly with Frailty Syndrome have a greater risk for worse functional capacity results 30 days after discharge.
