An experimental and theoretical approach on oxidative metabolism of paracetamol was applied for the pharmaceutical chemistry learning. Classical reactions, functional group identification, structural parameter, and chemical reactivity using frontier orbitals and Fukui index were used explaining the main products between N-acetyl-p-benzosemiquinone (NAPQI) and thiolic compounds. The chemoprotection mechanisms by N-acetyl-cysteine on high dosage of paracetamol are consistent with theoretical and experimental results. The methods also described the relationship between the chemical reactivity of quinone-imine system and the induced-toxicity of paracetamol by Michael reaction. These results can be applied in experimental pharmaceutical chemistry teaching.

Resumo O tratamento da doença de Parkinson (DP) constitui um desafio, especialmente por ser considerado muito individualizado. A Academia Brasileira de Neurologia (ABN) identificou a necessidade de disseminar o conhecimento sobre o manejo do tratamento da DP, adaptando as melhores evidências à realidade brasileira. Assim, foi realizada uma revisão sobre as principais orientações de tratamento publicadas, baseada nas recomendações elaboradas por um grupo de especialistas em transtornos do movimento do departamento científico da ABN.
ABSTRACT The treatment of Parkinson's disease (PD) is challenging, especially since it is considered highly individualized. The Brazilian Academy of Neurology has recognized the need to disseminate knowledge about the management of PD treatment, adapting the best evidence to the Brazilian reality. Thus, the main published treatment guidelines were reviewed based on the recommendations of group from the Movement Disorders Scientific Department of the Brazilian Academy of Neurology.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Time series evaluation of ascitic syndrome condemnation at poultry abattoirs under Federal Inspection Service of Brazil (2010-2019)

RESUMO: O objetivo deste trabalho foi avaliar a série histórica (2010-2019) de condenações parciais e totais de carcaças de aves por síndrome ascítica em abatedouros registrados no SIF localizados nos principais estados produtores de carne de frango. Através de dados oficiais, foram calculados o índice de ocorrência de condenações (IOC) e o índice de sazonalidade ajustado (ISA). A taxa de condenação foi de 1.140 carcaças condenadas total ou parcialmente por síndrome ascítica a cada 1.000.000 de aves abatidas. O menor e maior IOCs foram encontrados em São Paulo (fevereiro/2010) e em Goiás (janeiro/2017), respectivamente. A ocorrência de condenações por esta síndrome teve comportamento cíclico ao longo da série histórica, apresentando picos de condenação em todos os anos avaliados, sendo os maiores ISAs encontrados nos meses de julho, agosto e setembro, com índices variando entre 1,24 e 1,54 nestes meses. Considerando o período de alojamento pré-abate, os maiores valores de ISAs coincidiram com o período mais frio do ano (Maio a Agosto) para todos os estados analisados. Os resultados mostram que síndrome ascítica é um problema crescente no Brasil, com maior ocorrência durante os meses frios do ano, impactando negativamente a saúde dos animais e a lucratividade dos produtores e indústrias da cadeia produtiva de frango de corte.
ABSTRACT: Over the last decade, Brazil registered a 10.56% increase in the number of poultry slaughtered in establishments registered under the “Serviço de Inspeção Federal” (SIF - Federal Inspection Service), as a result of technological advances in management, health and genetics applied to national aviculture. At slaughter, during post-mortem inspection, carcasses can be totally or partially condemned for various reasons, including ascitic syndrome. This syndrome has economic implications for the industry, in addition to being a problem for the health and welfare of poultry. The objective of this work was to evaluate the historical series (2010-2019) of partial and total condemnations of poultry carcasses due to ascitic syndrome in slaughterhouses registered under the SIF and located in the main poultry-producing states. Through official data, the condemnation occurrence index (COI) and the adjusted seasonal index (ASI) were calculated. The condemnation rate was 1,140 carcasses condemned, totally or partially, for ascitic syndrome for every 1,000,000 poultry slaughtered. The smallest and largest COIs were found in São Paulo (February 2010) and in Goiás (January 2017), respectively. The occurrence of condemnations for this syndrome was cyclical throughout the historical series, showing peaks of condemnation in all years evaluated, with the highest ASIs in July, August and September, and with rates varying between 1.24 and 1.54 in these months. Considering the period of pre-slaughter housing, the highest ASIs coincide with the coldest period of the year (May to August) for all analyzed states. The results show that ascitic syndrome is a growing problem in Brazil, with greater occurrence during the coldest months of the year, having a negative impact on animal health and the profitability of producers and industries in the poultry production chain.

Usefulness of early colonoscopy in the diagnosis and treatment of moderate or severe lower gastrointestinal bleeding

RESUMO No total, 38.686 colonoscopias foram realizadas entre janeiro de 1985 e dezembro de 2012 no Hospital Sírio-Libanês, em São Paulo, Brasil. 234 pacientes (0,6%) sofriam de sangramento gastrointestinal baixo agudo (SGIBA) de intensidade moderada ou grave. Em 151 casos (64,5% desses pacientes) foi possível estabelecer um diagnóstico definitivo.O estudo foi aprovado pelo Comitê de Revisão Institucional. Os prontuários clínicos foram revisados.Todos os exames foram realizados com o paciente sedado e pela mesma equipe clínica.As origens predominantes de sangramento foram divertículos colônicos (73 pacientes; 31%), colite isquêmica ou infecciosa (18 pacientes; 7,7%) e proctite por radiação (18 pacientes; 7,7%).Uma intervenção terapêutica específica foi realizada em 61 dos 151 pacientes com diagnóstico confirmado (40,4%), de acordo com a origem do sangramento. Em sua maioria, os pacientes com sangramento pós-polipectomia foram tratados com injeção de adrenalina (40%) e por clipping (40%). Os pacientes com angiodisplasia foram tratados predominantemente com coagulação com plasma de argônio (42%).O tratamento mais frequentemente administrado foi a injeção de adrenalina, independentemente da origem do sangramento (34,4%), seguida pela coagulação com plasma de argônio (31,1%).O controle da hemorragia ativa foi obtido por via endoscópica em 98,8% dos pacientes. Nossos dados revelam que o uso precoce da colonoscopia no tratamento de pacientes com suspeita de SGIBA é instrumento útil para o diagnóstico e tratamento.
ABSTRACT A total of 38,686 colonoscopies were performed between January 1985 and December 2012 at Hospital Sírio-Libanês, in São Paulo, Brazil. Two hundred thirty-four patients (0.6%) had acute lower gastrointestinal bleeding of moderate or severe intensity. A definitive diagnosis was possible in 151 cases, 64.5% of these patients.This study was approved by the Institutional Review Board. Medical charts were reviewed.All examinations were done under sedation by the same medical team.The predominant sources of bleeding were colonic diverticula (73 patients; 31%), ischemic or infectious colitis (18 patients; 7.7%) and radiation proctitis (18 patients; 7.7%).A specific therapeutic intervention was performed on 61 of the 151 patients who had the diagnosis confirmed (40.4%), according to the source of bleeding. Most patients with postpolypectomy bleeding were treated with injection of epinephrine (40%) and clipping (40%). Patients with angiodysplasia were treated predominantly with argon plasma coagulation (42%).Injection of epinephrine was the most frequent treatment, regardless of the source of bleeding (34.4%), followed by argon plasma coagulation (31.1%).Control of active hemorrhage was achieved endoscopically in 98.8% of the patients.Our data shows that early colonoscopy in the management of patients with suspected acute lower gastrointestinal bleeding is a useful tool for diagnosis and treatment.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Este artigo descreve a síntese e a atividade antileishmania de novos derivados pirazolil benzenossulfonamídicos. Estes foram elucidados por métodos espectrométricos. Alguns compostos mostraram uma atividade in vitro significativa contra Leishmania amazonensis, destacando-se o derivado 1e. Nenhum dos derivados pirazolil benzenossulfonamídicos mostraram qualquer toxicidade em macrófagos murinos.
This paper describes the synthesis and the antileishmanial activity of new pyrazolyl benzenesulfonamide derivatives. These were elucidated by spectrometric methods. Some compounds showed a significant in vitro activity against Leishmania amazonensis, highlighting the derivative 1e. These pyrazolyl benzenesulfonamide derivatives did not show any toxicity in murine macrophage.

Este estudo caracterizou a anatomia musculoesquelética normal do segmento cervical da coluna vertebral de cães por meio da ultrassonografia modo B. O objetivo do trabalho foi estabelecer o papel da ultrassonografia modo B na avaliação anatômica do segmento cervical de cães, comparando os achados ultrassonográficos com imagens por tomografia computadorizada e ressonância magnética. O exame ultrassonográfico, em cortes transversais e sagitais mediano, permitiu a identificação de parte da musculatura epaxial do pescoço, superfície óssea das vértebras cervicais e medula espinhal em áreas restritas com janelas acústicas naturais, como entre a articulação atlantoccipital, áxis e atlas, e áxis e terceira vértebra cervical. As imagens, em planos transversais e sagitais, por ressonância magnética de baixo campo foram superiores na identificação anatômica das estruturas, devido ao maior contraste entre os diferentes tecidos nessa modalidade. A tomografia computadorizada se mostrou superior no detalhamento ósseo da região quando comparada ao exame ultrassonográfico. Já a ressonância magnética, além dos músculos e vértebras cervicais, permitiu a identificação do líquido cefalorraquidiano, e diferenciar núcleo pulposo e anel fibroso dos discos intervertebrais. Embora não seja objetivo direto deste estudo, com o conhecimento da anatomia ultrassonográfica dessa região, acredita-se que algumas lesões possam ser identificadas, contudo de maneira restrita, quando comparadas as informações obtidas, principalmente na imagem por ressonância magnética. O exame ultrassonográfico apresentou valor morfológico inferior.
This study characterized the normal musculoskeletal anatomy of the cervical segment of the spine of dogs by means of B-mode ultrasonography. The objective was to establish the role of B-mode ultrasonography for the anatomical evaluation of the cervical spine segment in dogs, by comparing the ultrasonographic findings with images by computed tomography and magnetic resonance imaging. The ultrasound examination, in transverse and median sagittal sections, allowed to identify a part of the epaxial cervical musculature, the bone surface of the cervical vertebrae and parts of the spinal cord through restricted areas with natural acoustic windows, such as between the atlanto-occipital joint, axis and atlas, and axis and the third cervical vertebra. The images, on transverse and sagittal planes, by low-field magnetic resonance imaging, were superior for the anatomical identification of the structures, due to higher contrast between the different tissues in this modality. Computed tomography showed superiority for bone detailing when compared with ultrasonography. As for magnetic resonance imaging, in addition to the muscles and cervical vertebrae, it is possible to identify the cerebrospinal fluid and differentiate between the nucleus pulposus and annulus fibrosus of the intervertebral discs. Although not the scope of this study, with knowledge of the ultrasonographic anatomy of this region, it is believed that some lesions can be identified, yet in a limited manner, when compared with the information obtained mainly with magnetic resonance imaging. The ultrasound examination presented lower morphology diagnostic value compared with the other modalities.

Pterodon emarginatus Vogel, Fabaceae, is a tree species commonly known as "sucupira-branca". It is a popular medicinal plant in the Brazilian cerrado (Savanna). This study investigates the chemical variability of the essential oils from fruits of P. emarginatus. The fruits were collected from five sites in the Brazilian Cerrado and their essential oils were analyzed by GC/MS. The results obtained by Principal Component and Cluster Analysis identified two groups: cluster I containing β-caryophyllene and δ-elemene and cluster II containing α-copaene, β-cubebene, allo-aromadendrene, α-cubebene and γ-muurulene. The Canonical Discriminant Analysis was used to differentiate between clusters on the basis of essential oil composition. The results showed high variability in the composition of the essential oils from fruits of P. emarginatus, contributing to studies of domestication of this species.

A contaminação do solo e das águas subterrâneas por metais pesados é, extremamente perniciosa por serem basntante persistentes no ambiente. Objetivou-se, com este trabalho, a análise da sorção do Cu, Cr, Zn, Cd, Pb e Ni em Latossolo Vermelho Acriférrico (LVwf), Argissolo Vermelho Eutrófico (PVe), Nitossolo Vermelho Eutroférrico (NVef) e Neossolo Quartzarênico (RQ) pelos modelos potencial e linear da isoterma Freundlich. Para o estabelecimento de isotermas de sorção adicionaram-se a 5,0 mL de solo e 50,0 mL de solução contendo o metal pesado a ser avaliado com diferentes concentrações, respeitando-se os limites aceitáveis. As isotermas de sorção potencial e linear apresentaram bom ajuste para descrever o comportamento de adsorção dos metais pesados nas diferentes classes de solo estudadas. A sequência da retenção dos metais pesados em ordem decrescente, foi, para: o PVe: Cr+3 > Cr+6 > Ni+2 > Zn+2 > Cu+2 > Pb+2 > Cd+2; o LVwf: Cr+3 > Cr+6 > Ni+2 > Cu+2 > Cd+2 > Zn+2 > Pb+2; o RQ: Cr+6 > Cr+3 >Cu+2 > Pb+2 > Ni+2 > Zn+2 > Cd+2 e, para, o NVef: Cr+3 > Cr+6 > Pb+2 > Cu+2 > Zn+2 > Ni+2 >Cd+2. O RQ foi a classe de solo que apresentou menor retenção de metais em comparação com as demais classes de solo sendo, portanto, mais vulnerável à contaminação de águas subterrâneas.
The contamination of soil and groundwater by heavy metals is extremely damaging due to high persistence of heavy metals in the environment. This work targeted the analysis of the sorption in Oxisol Typic Acrustox(LVwf), Ultisol (PVe), Kandic Oxisol (NVef) and Quartzipsamment (RQ) of heavy metals Cu, Cr, Zn, Cd, Pb and Ni. The sorption of the heavy metals in the soil was evaluated by batch method and described by potential and linear models of the Freundlich isotherm. For the establishment the of sorption isotherm, 50.0 mL of solution containing the heavy metal were added in 5.0 mL of soil, assessing with different concentrations within the acceptable soil limits. The potential and linear sorption isotherms presented good fitting to describe the behavior of adsorption of heavy metals in different classes of studied soil. The detention of heavy metals in, descending order was for PVe: Cr+3 > Cr+6 > Ni+2 > Zn+2 > Cu+2 > Pb+2 > Cd+2; for LVwf: Cr+3 > Cr+6 > Ni+2 > Cu+2 > Cd+2 > Zn+2 > Pb+2; for RQ: Cr+6 > Cr+3 >Cu+2 > Pb+2 > Ni+2 > Zn+2 > Cd+2 and for NVef: Cr+3 > Cr+6 > Pb+2 > Cu+2 > Zn+2 > Ni+2 > Cd+2. The Quartzipsamment presented a less retention of metal compared to other classes of soil, and is therefore, more vulnerable to contamination of groundwater.

Comumente, o cádmio é encontrado em resíduos industriais e urbanos, lodo de esgoto e fertilizantes que, quando dispostos no solo, podem representar riscos de poluição do solo e água. Propôs-se, neste trabalho, analisar a adsorção e o deslocamento do íon cádmio nos Latossolo Vermelho Acriférrico, Argissolo Vermelho Eutrófico, Nitossolo Vermelho Eutroférrico e Neossolo Quartzarênico, encontrados em áreas de cultivo do Estado de Goiás. Para a determinação da quantidade de cádmio adsorvido, 50 mL de solução de CaCl2.2H2O (0,01 mol L-1) com diferentes concentrações de Cd (0,10; 1,80; 19,00; 46,60 e 105,70 mg L-1) foram adicionados a 5,0 mL de terra fina secada em estufa e agitados 24 h; cuja sorção foi avaliada pelos modelos potencias e lineares da isoterma Freundlich. No ensaio do transporte utilizou-se uma deslocadora de Cd, em colunas de solo, na concentração de 105,70 mg L-1, em que os fatores de retardamento (R) e coeficientes de dispersão hidrodinâmica (D), foram ajustados pela minimização da soma dos quadrados dos desvios entre as curvas de eluição experimental e simulada. Tanto as isotermas lineares e potenciais como os parâmetros de transporte R e D apresentaram bom ajuste para descrever a adsorção e o deslocamento do Cd nas diferentes classes de solo estudadas. O Neossolo apresentou retenção menor do Cd em comparação com as demais classes de solo sendo, portanto, mais vulnerável à contaminação das águas subterrâneas, seguido do Latossolo, Argissolo e Nitossolo.
Cadmium is commonly found in industrial and urban residues, sewer sludge and fertilizers, and when disposed in the soil can represent risks of pollution of the soil and water. This study aimed to analyze the adsorption and movement of cadmium in Red Latosol, Red Argisol, Red Nitosol and Quartzarenic Neosol found in cultivated áreas in the State of Goiás. In order to quantify the adsorbed cadmium, 5 mL of oven-dried soil were stirred in beaker, for 24 h, with 50 mL of CaCl2.2H2O (0.01 mol L-1) solution containing different quantities of cadmium (0.10, 1.80, 19.00, 46.60 and 105.70 mg L-1). In the trial of the transport of Cd in soil columns a displacement solution of 105.70 mg L-1 was used. Sorption of Cd was evaluated by potential and linear models of the Freundlich isotherm and the retardation factor (R) and hydrodynamic dispersion coefficient (D) were adjusted by minimizing the sum of squares of deviations between the experimental and simulated breakthrough curve. Both isothermal and linear potential and the adjustment of the transport parameters D and R showed a good fit for describing the adsorption and the displacement of Cd in the different studied soils. The Quartzarenic Neosol showed lower retention of Cd in comparison to other classes of soil, and therefore more vulnerable to contamination of groundwater, followed by the Latosol, Argisol and Nitosol.

O Sysmex XS-1000i® é um novo e compacto analisador hematológico com tecnologia 5diff e citometria de fluxo fluorescente, tecnologias até então presentes em equipamentos de grande porte e disponíveis apenas para equivalentes rotinas laboratoriais. Neste estudo avaliamos seu desempenho com equipamento de análise de impedãncia (Coulter T-890®) e à contagem diferencial de 200 leucócitos em microscópio. Foram analizadas 100 amostras paralelamente nos dois equipamentos e submetidas à contagem diferencial de 200 células em distensão sanguínea. Os resultados foram analisados mediante análise de correlação de Spearman para dados não paramétricos, mostrando correlação superior a 95% para os parâmetros WBC, RBC, hemoglobina, hematócrito e neutrófilos; correlação entre 90% e 94,9% para VCM, HCM, linfócitos e eosinófilos ; correlação inferior a 89,9 % para CHCM, monócitos e basófilos. Os resultados obtidos sugerem que a tecnologia embarcada neste equipamento é uma versátil ferramenta para o analista na rotina de pequeno e médio porte. No entanto, em caso de emissão de "flags" não dispensa totalmente a confirmação ao microscópio para garantia da qualidade final do exame.
The Sysmex XS-1000i® is a new and compact blood analyzer with 5diff technology and fluorescent flow cytometry, technologies hitherto only present in large equipment and available only for laboratory equivalent routines. In this study its performance was compared with equipment for impedance analysis (Coulter T-890®) and the microscopic counting of 200 differential leukocytes. One hundred samples were evaluated in parallel by both apparatuses and subjected to differential counts of 200 cells. The results were analyzed using the Spearman correlation for parametric data showing no correlation greater than 95% for white blood cells, red blood cells, hemoglobin, hematocrit and neutrophils,; correlations between 90% and 94.9% for VCM, MCH, lymphocytes and eosinophils, and correlations lower than the 89.9% for MCHC, monocytes and basophils. The results suggest that this apparatus is a versatile tool for routine analyses in small and medium laboratories. However, in the case of flags, confirmation under a microscope is important to guarantee the quality of the final results.
